Objective: To investigate microanatomic organizations of the extratemporal facial nerve and its branches, hypoglossal nerve, sural nerve, and great auricular nerve. Methods: Nerve samples were dissected in 12 postmortem autopsies, and histomorphometric analyses were conducted. Results: There was no significant difference between the right and left sides of the nerve samples for the nerve area, fascicle area, number of fascicles and average number of axons. The lowest mean fascicle number was found in the hypoglossal nerve (4.9 ± 1.4) while the highest was in great auricular nerve (11.4 ± 6.8). The highest nerve area (3,182,788 ± 838,430 μm2), fascicle area (1,573,181 ± 457,331 μm2) and axon number (14,772 ± 4402) were in hypoglossal nerve (p < 0.05). The number of axons per unit nerve area was higher in the facial nerve, truncus temporofacialis, truncus cervicofacialis and hypoglossal nerve, which are motor nerves, compared to the sural nerve and great auricular nerve, which are sensory nerves (p < 0.05). The number of axons per unit fascicle area was also higher in motor nerves than in sensory nerves (p < 0.05). Conclusion: In the present study, it was observed that each nerve contained a different number of fascicles and these fascicles were different both in size and in the number of axons they contained. All these variables could be the reason why the desired outcomes cannot always be achieved in nerve reconstruction.

We studied conduit-related risk factors for mortality, conduit reintervention, conduit replacement, and pulmonary artery (PA) reinterventions after truncus repair. Patients who underwent truncus repair at our institution between 1995 and 2019 were studied. Cox proportional hazards modeling evaluated variables for association with mortality, time to conduit reintervention, time to conduit replacement, and time to PA reintervention. Truncus was repaired in 107 patients at median age of 17 days (IQR 9-45). Median follow-up time was 7 years. Aortic homografts were implanted in 57 (53%) patients, pulmonary homograft in 40 (37%), and bovine jugular conduit in 10 (9%). Median conduit size was 11 mm (IQR 10-12) and median conduit Z-score was 1.71 (IQR 1.08-2.34). At 5 years, there was 87% survival, 21% freedom from conduit reinterventions, 37% freedom from conduit replacements, and 55% freedom from PA reinterventions. Conduit size (HR 0.7, 95%CI 0.4-1.4, p=.41) and type (aortic homograft reference; bovine jugular vein graft HR 0.6, 95% CI 0.08-5.2, p=.69; pulmonary homograft HR 0.7, 95% CI 0.2-2.3, p=.58) were not associated with mortality. On multivariate analysis, the hazard for conduit reintervention, conduit replacement, and PA reintervention decreased with increasing conduit Z-score values of 1 to 2.5 (non-linear relationship, p.01), with little additional reduction in hazard beyond this range. Implantation of a larger conduit within Z-score values of 1 and 2.5 is associated with a decreased hazard for conduit reintervention, conduit replacement, and PA reintervention after truncus repair. The type and size of the conduits did not impact mortality.

Surgical repair of Truncus arteriosus aims to improve long term survival. A clear understanding of the natural history and achievable surgical outcomes informs surgical decision making with respect to the timing of surgery. We sought to determine if it is ever too late to repair T. arteriosus. Pubmed and Google scholar were searched between January 1st 1966 and July 31st 2021. The proximal similarly model was used to determine the most externally valid natural history and surgical outcome data. The most externally valid data were used to develop a composite survival graph to help clinicians optimize surgical decision making. Four hundred and twenty eight results were retrieved. Five studies representing 938 patients who had repair of T. arteriosus were included for external validity assessment. The resultant composite graph suggests that repair after 5 month of age, may not improve 15 year survival when compared to non-operative care. T. arteriosus repair after 5 months of age may not improve survival. Key words: Pediatric, truncus artriosus, operative, outcomes.

Összefoglaló. Az intrauterin echokardiográfiás és MR-diagnosztika fejlődése napjainkban jelentős szerepet tulajdonít már az első trimeszter idejében kimutatható aortaív-fejlődési rendellenességeknek. Célunk volt részletezni a vascularis gyűrűk megjelenési formáit, ezek különböző életkorokban jelentkező tüneteit, sebészi kezelését, hogy hozzájárulhassunk a helyes felvilágosításhoz, mely alapvető lehet a szülés helyének megválasztásában, így meghatározhassák a magzat sorsát, perinatalis ellátását. A situs inversustól eltekintve a jobb oldali aortaív jelenléte mindig felhívja a figyelmet vascularis gyűrű, társuló szívhibák, genetikai betegségek, kromoszómaanomáliák jelenlétére, tehát a magzat további vizsgálata szükséges. Kettős aortaív esetén jelentkezhetnek olyan súlyos tünetek, melyek a megszületés után, kora csecsemőkorban sebészi beavatkozást igényelhetnek (szükséges lehet a szülés helyének megválasztása!). Az aberráns jobb arteria subclavia önállóan nem alkot érgyűrűt, a ritkán társuló truncus caroticusszal csak későbbi életkorban okozhat sebészi beavatkozást igényló enyhe tüneteket. Nowadays, the development of intrauterine echocardiography and MR diagnostics plays a significant role in aortic arch malformations detected during the first trimester. Our aim was to detail the manifestations of vascular rings, their symptoms at different ages and their surgical treatment options in order to determine the fate of the fetus and its perinatal care. Apart from situs inversus, the presence of the right aortic arch always draws the attention to the possible presence of vascular rings, associated heart defects, genetic diseases, or chromosomal abnormalities, therefore further examinations of the fetus are necessary. In the case of a double aortic arch, severe symptoms may occur, which may require surgery after birth and in early infancy (it may be necessary to choose the place of birth!). The right aberrant subclavian artery does not form a ring and may cause mild symptoms requiring surgery at a later age with rarely associated truncus carotid. Orv Hetil. 2021; 162(48): 1920–1923.

Background: Aneurysm formation is a multifactorial process involving genetic, anatomical and environmental risk factors. A research focusing on the relationship between the presence of aneurysm and the morphology of the arteries will help in the pathogenesis and prediction of intracranial aneurysms. In this study, the relationship between the presence of aneurysm and various morphological parameters of aneurysm-related arteries was evaluated in patients with saccular middle cerebral artery (MCA) bifurcation aneurysm. Materials and methods: The archival images of 74 patients (62.2% women) were evaluated retrospectively. In this study, the angle between the ipsilateral MCA M1 segment and the dominant truncus (Φ1), the angle between the M1 segment and the recessive truncus (Φ2), and the bifurcation angle (Φ1 + Φ2) were compared. Bilateral internal carotid artery (ICA), MCA M1 segment, dominant and recessive truncus diameters and these diameters ratios were compared with the aneurysmal side and the contralateral side without aneurysm. Results: When the dominant truncus, recessive truncus angles and bifurcation angle were compared, a significant difference was found on the aneurysmal side (p < 0.0001). In the receiver operating characteristic analysis, when the bifurcation angle of 147.5° was accepted as the limit value, 78.4% sensitivity, 79.7% specificity, 79.5% positive predictive value and 78.7% negative predictive value were determined (area under the curve: 0.85). Conclusions: Our study of the morphological features of arteries associated with MCA bifurcation aneurysms showed that the presence of MCA aneurysms was significantly associated with large bifurcation angles.

Endobronchial resection using a bronchoscope is often selected as treatment for carcinoid tumors located in the central airways. However, massive bleeding is one of the most serious complications during bronchoscopic surgery. Here, we report the case of a 77‐year‐old female with a typical carcinoid tumor located in the right truncus intermedius who underwent bronchial artery embolization (BAE) one day before endobronchial intervention using a flexible bronchoscope. The tumor was successfully resected without bleeding. BAE prior to endobronchial resection of carcinoid tumors may be useful for reducing the risk of bleeding., We report a female case with a typical carcinoid tumor. She underwent bronchial artery embolization (BAE) before endobronchial intervention using a flexible bronchoscope. Background/aim In this study, we aimed to investigate what should be regarded as potential determinants of treatment strategies when evaluating 3D digital subtraction angiography (DSA) images. Material and methods Our inclusion criteria were as follows: (1) presence of at least one intracranial aneurysm demonstrated by conventional angiography, (2) having both 2D and 3D images, and (3) being over 18 years old. First, two-dimensional (2D) and then 3D angiography images of 226 aneurysms of 150 patients were scanned. Morphological characteristics such as size, configurations, relationship with parent artery, baby counts, and other incidental findings were determined. Results Of the 226 aneurysms, 11 (4.9%) were only detected on 3D images. Four of these 11 additional aneurysms were believed to be babies of other aneurysms seen in 2D images. Middle cerebral artery (MCA) M1 segment was the most common localization in terms of missed aneurysms. Of the 28 aneurysms located in the communicating segment of the internal carotid artery, the absolute locations of 7 (25%) could not be detected in 2D images or detected in the wrong location. Of the 24 aneurysms located in the ophthalmic segment, the origin of 8 (33%) could not be clearly identified in 2D images. Truncus relationships of MCAs bifurcation/trifurcation aneurysms were seen in 41 of 63 aneurysms (65%) on 2D images, whereas all were confirmed on 3D images. Fenestrations not seen in 2D images were identified in 3D images of 4 patients (3%). Conclusion The superiority of 3D images compared to 2D images in determining the morphologic characteristics of intracranial aneurysms has been known for a long time. The contribution of 3D images to the treatment can be summarized as evaluating the parent artery relationship, revealing the number and shapes of aneurysm babies more clearly, detecting fenestrations, and shortening procedure time by finding the correct working angle.

Van Praagh (VP) A3 variant of truncus arteriosus (or common arterial trunk) is defined by only one pulmonary artery (usually the right) originating from the common trunk, while the other lung is supplied either by collaterals or a pulmonary artery arising from the aortic arch. This report describes a staged approach to manage a VP-A3 variant truncus arteriosus with ductal origin of the left pulmonary artery (LPA), a hypoplastic right pulmonary artery, and cyanosis. Initially, the ductal portion of the proximal LPA was stented with a Resolute Onyx drug-eluting stent. The pulmonary arteries grew and at four months of age had an acceptable McGoon ratio and Nakata index. The patient then underwent repair which included unifocalization of the branch pulmonary arteries, closure of the ventricular septal defect, and placement of a right ventricle-to-pulmonary artery homograft conduit.

Background: Currently, there is no scientifically based diagnostic complex for diffuse axonal brain injury (DAI) which allows to reveal its morphological substrate and determine the time elapsed after it, especially in the early post-traumatic period. Aims: The aim of the study was to identify of morphological changes in the corpus callosum in various post-traumatic periods after DAI. The material of the study is the corpus callosum of victims who died from traumatic brain injury with DAI within the first hour (group I, n=25), in the period from 1 to 12 hours (group II, n=30) and in the period from 12 to 48 hours (group III, n=17) before the death. The control group of the study comprises the corpus callosum (n=25) of those who died from various causes of non-violent and violent death (without any head trauma). Results: The main diagnostic criterion for DAI is represented by hemorrhages in the substance of the corpus callosum truncus. The proposed complex of morphological changes in the neuro-gliovascular module will allow to determine the time elapsed after DAI. Conclusions: DAI as a special type of traumatic brain injury is manifested by a pathognomonic complex of pathomorphological changes in the deep structures of the brain, including the corpus callosum. Objectification of the time elapsed after DAI should be based on a comparative assessment of all morphological features of the identified neuro-gliovascular module, while its individual features can not be considered as diagnostic.

The study was conducted for the determination of the main nerves of the lumbosacral plexus in the helmeted guineafowl. Five helmeted guineafowls were used. Fowls were anaesthetised and the a. carotis communis was cut for blood drainage. Body cavities were revealed and were fixated with 10% formaldehyde. Nerves forming the lumbosacral plexus were dissected and photographed. Results were named according to the Nomina Anatomica Avium. It was determined that the lumbosacral plexus forms by 8 synsacral ventral rami from the ventrolateral side of synsacrum which include (2-9) synsacral spinal nerves. It was seen that the lumbar plexus was formed by the ventral rami of the 2nd, 3rd and 4th spinal nerves, and the sacral plexus was formed by the ventral rami of the 5th, 6th, 7th, 8th and 9th synsacral spinal nerves. It was observed that following nerves of n. pubicus (ilioinguinalis), r. cutaneous femoris lateralis, r. cutaneous femoris medialis (n. saphenus), n. femoralis and n. obturatorius originate from the lumbar plexus, and following nerves of n. ischiadicus, the common branch of n. fibularis and n. tibialis originate from the sacral plexus. It was determined that the n. ischiadicus was formed by the truncus cranialis, medianus and caudalis. In conclusion, it was determined that there are macro anatomical differences between different avian species in the quantity, thickness and distribution of the spinal nerves that form the lumbosacral plexus, and in formations of the plexus, and in separations of nerve branches.

Background: In surgical aortic valve replacement (SAVR), coronary arteries are routinely assessed by transesophageal echocardiography (TEE) to prevent undesirable complications. This study evaluated the capabilities and pitfalls of TEE assessment. Methods and Results: Of 147 consecutive SAVR patients undergoing aortic stenosis, the TEE records for 130 patients, in which the procedures were conducted by a single examiner, were analyzed retrospectively regarding data acquisition and the accuracy of detecting an anomalous origin, high or low takeoff, ostial diameter, and short left main truncus (LMT). The left and right coronary arteries could be visualized in every patient. A left coronary ostium >5 mm was found in 33 patients (25.4%). TEE revealed an anomalous origin in 2 patients (1.5%) that had not been diagnosed, but missed it in another patient. High takeoff was noted in 11 patients (8.3%), often associated with aortic disease necessitating aortic repair. In one such patient, occlusion of the right coronary artery was detected, necessitating coronary revascularization. Short LMT was found in 15 patients (11.8%) but misdiagnosed due to artifact in 1. During selective cardioplegia, malperfusion of the left anterior descending artery due to deep cannula placement was detected. Conclusions: TEE provides fairly accurate assessment in SAVR, including detection of undiagnosed pathologies or pitfalls related to coronary arteries, although misdiagnosis due to artifacts should be kept in mind.

Aim To evaluate the frequency and pattern of variations in the origins of right inferior phrenic artery (RIPA) and left inferior phrenic artery (LIPA) on 256 slice multidetector computed tomography (MDCT). Materials and Methods MDCT abdominal images of 600 patients (male: 344, female: 256; mean age 56.45 ± 12.96 years) who underwent technically successful multiphase computed tomography were assessed for IPA variations with emphasis on their origins and results analyzed. Results Both IPA origins were documented in all patients. Both RIPA and LIPA originated from the common trunk in 128 (21.3%) patients. IPAs with common trunk most commonly originated from the aorta (68, 11.3%). Without common trunk RIPA most commonly originated from the aorta (225, 37.5%) and LIPA from the celiac artery (278, 46.3%). The least frequently detected IPA variations were RIPA originating from the common hepatic artery (1, 0.2%), superior mesenteric artery (1, 0.2%), and common truncus originating from left renal artery (1, 0.2%). Conclusion MDCT demonstrates the IPA origins very well, enabling planning of interventional procedures related to IPA. Without common trunk RIPA most commonly originates from aorta and LIPA from the celiac artery. IPAs with common trunk most commonly originate from aorta.

Objectives Studies on the short-, medium and long-term effects of flow guiding stents are still limited. In this case report, we present three-year follow-up of the multilayer flow modulator stent in a 55-year-old patient with Crawford Type 2 thoracoabdominal aortic aneurysm. Methods A 55-year-old male patient with Crawford Type 2 thoracoabdominal aortic aneurysm had applied to our medical center. The aneurysm involved coeliac truncus and superior mesenteric artery and extended to the renal artery ostia. Multilayer flow modulator stent was successfully placed, and follow-up CT (Computed tomography) angiographic examination images recorded intermittently (36 months). Results After three years, it was observed that the left renal artery was thrombosed and the left kidney went to atrophy. Other major vascular branches were observed to be open. During this time, the aneurysm was completely closed with thrombus, but the diameter of the aneurysm continued to increase. Conclusions Multilayer flow modulator stents are safe in complex aortic aneurysms. The device increases the thrombus load in the aortic aneurysm and maintains the flow of the main vascular branches. But re-interventions, dilatation of the aneurysm sac and visceral branch obstructions are still challenging for multilayer flow modulator stents.

Case Report: Barium swallow study showed posterior indentation on esophagus. On MDCT, an abnormal couse of RSA was seen behind the esophagus. Bilateral carotids were seen to be arising from a common midline trunk- often called truncus bicaroticus double SVC was seen with persistent left SVC draining into the coronary sinus. A prominent retroaortic azygous vein was seen draining into the left SVc with absence of right azygous.

We report a case of an undiagnosed descending aortic aneurysm, combined with coarctation and persistent left brachiocephalic truncus in a 59‐year‐old male. It highlights the necessity for aortic imaging, when facing a poorly controlled hypertension.

Introduction: Visceral artery aneurysms are rare vascular pathologies. They are usually incidental findings during the examination for other reasons. The most common clinical symptoms are nonspecific abdominal pain and bleeding caused by their rupture, with a severe clinical presentation. Aneurysms of pancreaticoduodenal arcade are more common in patients with well-developed collateral circulation due to the coeliac trunk stenosis or occlusion. Case report: In this case report the authors present a rare case of a patient with incidental finding of pancreaticoduodenal arcade aneurysm in the setting of severe stenosis of coeliac trunk origin caused by medial arcuate ligament compression. The diameter of the aneurysm was 40 mm and endovascular treatment was not possible because of unfavorable anatomical setting. The patient was successfully treated with resection of the aneurysm and the division of medial arcuate ligament during one surgery. Conclusion: There is no correlation between the diameter and the risk of rupture of the pancreaticoduodenal arcade aneurysm. Because of high morbidity and mortality of their rupture, most authors recommend active treatment of these aneurysms. The necessity to treat truncus coeliacus stenosis or occlusion remains a controversial issue.

Congenital heart defects (CHDs) are the most common types of birth defects, and global incidence of CHDs is on the rise. Despite the prevalence of CHDs, the genetic determinants of the defects are still in the process of being identified. Herein, we report a consanguineous Saudi family with three CHD affected daughters. We used whole exome sequencing (WES) to investigate the genetic cause of CHDs in the affected daughters. We found that all affected individuals were homozygous for a novel splice-altering variant (NM_001330069.1: c.265-1G>T) of PRKD1, which encodes a calcium/calmodulin-dependent protein kinase in the heart. The homozygous variant was found in the affected patients with Pulmonary Stenosis (PS), Truncus Arteriosis (TA), and Atrial Septal Defect (ASD). Based on the family’s pedigree, the variant acts in an autosomal recessive manner, which makes it the second autosomal recessive variant of PRKD1 to be identified with a link to CHDs, while all other previously described variants act dominantly. Interestingly, the father of the affected daughters was also homozygous for the variant, though he was asymptomatic of CHDs himself. Since both of his sisters had CHDs as well, this raises the possibility that the novel PRKD1 variant may undergo autosomal recessive inheritance mode with gender limitation. This finding confirms that CHD can be associated with both dominant and recessive mutations of the PRKD1 gene, and it provides a new insight to genotype–phenotype association between PRKD1 and CHDs. To our knowledge, this is the first report of this specific PRKD1 mutation associated with CHDs.

Persistent truncus arteriosus is a rare congenital heart disease with four variants, and the last being the rarest. The prognosis without surgical intervention is poor. In such cases, an echocardiography is not sufficient hence computed tomography (CT) imaging is required. We report a 26-year-old female with difficulty in breathing since childhood with cyanosis. Her echocardiography showed a ventricular septal defect (VSD) and the CT showed a single arterial trunk overriding the interventricular septum with a VSD, and the descending aorta giving rise to the pulmonary arteries suggestive of pseudo truncus, known as truncus arteriosus type IV.

While most patients with arteria lusoria and common carotid trunk conditions are asymptomatic, discovery of such anomalies periprocedurally may affect the cardiac catheterization access site, catheter selection, and additional imaging.

Cardiac haemangiomas are rare cases and can be seen at any age of life. A 49-year-old woman was admitted to our hospital with chest pain. A well-circumscribed soft tissue mass extending from the pulmonary artery truncus to the left ventricular inferior neighborhood. Following surgical resection, the patient was discharged 4 days after surgery.

Videothoracoscopic resections are among the mostly preferred minimally invasive thoracic surgical techniques to treat lung cancers especially in the last two decades. In thoracoscopic surgery video camera technology, high-tech equipment and surgical instruments including staplers are required. We have developed a technique for dissection and cutting of truncus anterior and right upper lobe vein in one step with stapler by this way we aimed to provide less operation time and more cost- effectiveness for right upper lobectomies.

Compared to a left trisegmentectomy, there is a functional benefit in performing an S1 + 2 resection (Tane et al. 2020). This segmentectomy is usually easier than a left upper lobe trisegmentectomy as there is no need to control the truncus anterior (Handa et al. 2020). In addition, it is a safer procedure without the risk of lingular ischemia, as frequently faced after trisegmentectomy (Brunswicker et al. 2016; Eguchi et al. 2008; Onuki et al. 2019). Depending on the target and on the broncho-vascular branching, an S2 + S1a can be advisable (Nomori and Okada 2012).

It is customary, at the current time, to consider many, if not most, of the lesions involving the ventricular outflow tract in terms of conotruncal malformations. This reflects the introduction, in the early 1940s, of the terms conus and truncus to describe the components of the developing outflow tract. The definitive outflow tracts in the postnatal heart, however, possess three, rather than two, components. These are the intrapericardial arterial trunks, the arterial roots, and the subvalvar ventricular outflow tracts. Congenital lesions afflicting the arterial roots, however, are not currently considered to be conotruncal malformations. This suggests a lack of logic in the description of cardiac development and its use as a means of categorizing congenital malformations. It is our belief that the developing outflow tract, like the postnatal outflow tracts, can readily be described in tripartite fashion, with its distal, intermediate, and proximal components forming the primordiums of the postnatal parts. In this review, we present evidence obtained from developing mice and human hearts to substantiate this notion. We show that the outflow tract, initially with a common lumen, is divided into its aortic and pulmonary components by a combination of an aortopulmonary septum derived from the dorsal wall of the aortic sac and outflow tract cushions that spiral through its intermediate and proximal components. These embryonic septal structures, however, subsequently lose their septal functions as the outflow tracts develop their own discrete walls. We then compare the developmental findings with the anatomic arrangements seen postnatally in the normal human heart. We show how correlations with the embryologic findings permit logical analysis of the congenital lesions involving the outflow tracts.

Upper division segmentectomy is quite similar to a left upper lobectomy and is gaining acceptance, as it seems that its oncological results are equivalent to those of a left upper lobectomy (Soukiasian et al. 2012). This segmentectomy can be tricky because the bronchial trunk is in close contact with the truncus anterior, especially if adherent lymph nodes are present.

The right upper lobectomy is most often done via an anterior approach with a primary control of the truncus anterior (TA) and the superior root of the superior pulmonary vein (SPV) as usually done during conventional open surgery (Ke et al. 2017). However, an anterior dissection can be hazardous, especially in some overweight patients in whom identification of vessels is not easy. Confusing the main PA and the TA, or stapling the PA trunk has been reported (Decaluwe et al. 2015).

Left upper lobectomies are reputed hazardous, because of the need to control the mediastinal branches of the pulmonary artery, which can be short and fragile. Any tear would cause massive hemorrhage and would require immediate conversion to thoracotomy. However, thanks to the close-up vision and magnification provided by the thoracoscope, dissection can be accurate and safe. Most of the dissection is performed from behind, including control of the truncus anterior (Zhang et al. 2016; Xu and Zhang 2019). But the technique should be adapted to the anatomical conditions and an anterior dissection can sometimes be required, for instance, when the truncus anterior is short.

Background Congenital heart defect affects around 10 in a 1000 live born babies. Truncus arteriosus is a rare conotruncal defect in which a common arterial trunk supplies systemic, pulmonary and coronary circulation. The incidence of truncus arteriosus is quoted between 1–2% of all congenital heart malformations. Aim To evaluate the initial neonatal management and outcomes of newborns with truncus arteriosus. Method We conducted a retrospective review of all newborns with truncus arteriosus admitted to our neonatal unit between 01/04/2009 and 31/03/2019. Data were collected from Badger and Cardiology databases to evaluate the initial neonatal management and long-term outcomes of this cohort of patients. Results Sixteen newborns with truncus arteriosus were admitted to our neonatal unit in the last 10 years. Their mean gestational age was 36 weeks with a mean birth weight of 2407 grams. Cardiac defects were detected prenatally in 13 patients (82%). Three babies were diagnosed postnatally. One following failed pulse oximetry screening requiring non-invasive respiratory support and 2 diagnosed following on-going respiratory support on echocardiography. Respiratory support was initiated within the first 24 hours of life in 8 patients; these included all patients with a postnatal diagnosis of truncus arteriosus. Four patients required mechanical ventilation. Whilst in the neonatal intensive care unit, 3 babies developed heart failure and were treated with diuretics and 2 developed necrotising enterocolitis which was managed conservatively. Median length of stay before transfer to our local surgical cardiology centre was 3 days. Fourteen patients (87%) underwent common arterial trunk repair. 11 (78%) babies survived to date. Two patients died in the initial post-operative period and one patient at 14 months of age. The remaining two patients did not receive any surgical intervention and subsequently died (4 and 11 days old). Conclusion Truncus arterisus is a rare conotruncal defect with the majority being detected antenatally in our cohort. They are likely to require respiratory support in the neonatal period. Despite the complex nature of their congenital heart disease, their surgical outcomes are good in our cohort of babies. The following information can be used when counselling parents antenatally and managing their expectations.

We present a 67‐year‐old man with allergic bronchopulmonary aspergillosis (ABPA), whose chest computed tomography (CT) scans showed localized circumferential bronchial wall thickening in the right main bronchus to middle truncus. Chest CT scans showed high‐attenuation mucus in the right B8 bronchus. Bronchoscopy showed narrowing of the right main bronchus and inspissated mucus plugging in the right B8 bronchus. Histological examination of the right main bronchus revealed bronchial inflammation with numerous neutrophils and plasma cells. Treatment with corticosteroid and antifungal agents resulted in resolution of the symptoms and right bronchial lesions. The clinical course may indicate that the bronchial lesions were associated with ABPA., The typical bronchial manifestations of allergic bronchopulmonary aspergillosis (ABPA) include intraluminal retention of inspissated mucus plugging and chronic inflammation of airway walls with eosinophils, lymphocytes, and neutrophils. Bronchiectasis and bronchial wall thickening can be seen on chest computed tomography (CT) scans. However, localized circumferential narrowing of the bronchus has not been reported. Herein, we report such a patient with ABPA, whose bronchial lesions disappeared after treatment with prednisolone and antifungal agents.

Truncus arteriosus (TA) is a relatively uncommon cyanotic congenital cardiac anomaly accounting for 1.3% of all congenital cardiac malformations. TA associated with an ascending aortic aneurysm is an extremely rare congenital cardiovascular abnormality. A 15-year-old male presented with shortness of breath and cough for 2 weeks. Radiological examinations showed that the ascending aorta and main pulmonary arteries originate from a single truncus with ascending aortic aneurysm. TA has a poor prognosis if remain unrepaired and surgical intervention is necessary to avoid severe pulmonary vascular occlusive disease.

Although the variability of the upper limb arteries is a clinically important problem, the prevalence varies across the existing studies and the classification is rather complicated, uncoined, and sometimes even unclear for its simple and direct understanding and usage. Multiple case reports appearing in the last years apply incorrect, inappropriate, and sometimes misleading terminology. Here we performed an anatomical cadaveric study of the variability of the arteries of the upper limb, namely the axilla, arm and forearm, in 423 upper limbs (Central European population) embalmed with the classical formaldehyde method. We proposed the application of the Equality system based on common trunks for denomination of the axillary artery branches principal variations: truncus subscapulocircumflexus (22.9%), truncus profundocircumflexus (13.75%), and truncus bicircumflexus (13.95%). Further, we proposed the terminology system developed by Rodríguez-Niedenführ et al. for the free upper limb principal arterial trunk variations based on the origin, location (in the arm only or in the arm and forearm), and course (related to the forearm flexor muscles) of the involved artery: arteria brachialis superficialis (9.5%), arteria brachioradialis superficialis (6.4%), arteria brachioulnaris superficialis (1.9%), arteria brachiomediana superficialis (0.5%), and arteria comitans nervi mediani manus (3.3%). Extensive development of the catheterisation methods via the arteria radialis et ulnaris as well as surgical procedures using flaps based on perforating branches of these arteries (including arteria brachioradialis superficialis et brachioulnaris superficialis) necessitate thorough data on the prevalence of the variant vessels for safe performance of these procedures and to prevent any unexpected situations or to react adequately when they occur.

Background One of the rare complex congenital anomalies is truncus arteriosus—modified Van Praagh’s type 3A. Survival of this type of truncus arteriosus child beyond infancy without surgical treatment is unreported. Anesthesiologists do anesthetize children with complex congenital heart disease during the cardiac catheterization study. The final diagnosis of such children is often made after the anesthesia and cardiac catheterization study. We report a 12-year-old with truncus arteriosus with absent right pulmonary artery and main pulmonary artery with multiple Major Aorto-Pulmonary Collateral Arteries. (MAPCAs) for the right lung, who is surviving without surgical treatment. Case presentation A 12-year-old girl was brought by her parents to Meenakshi Hospital at Thanjavur (India) with complaints of shortness of breath during respiratory infection. The patient was diagnosed to have congenital heart disease at 6 years of age and not on any treatment. There was no history of cyanotic spell. Her echocardiography revealed tetralogy of Fallot, situs solitus, levocardia, large mal-aligned ventricular septal defect with bidirectional shunt, VSD size 12 mm, pulmonary atresia, moderate tricuspid regurgitation (TR pressure gradient, 103 mmHg), thickened aortic valve, grade II aortic regurgitation, right ventricular hypertrophy, intact interatrial septum, dilated right atrium/right ventricle, dilated coronary sinus, and persistent left superior vena cava, good biventricular function 65%, multiple MAPCAs, no coarctation of aorta, normal veno atrial, atrio-ventricular connections, normal pulmonary venous drainage, and no pericardial effusion. She underwent cardiac catheterization study for further evaluation under anesthesia. Her final diagnosis was truncus arteriosus with absent right pulmonary artery and main pulmonary artery with multiple MAPCAs for right lung, (truncus arteriosus—modified Van Praagh’s type 3A). Conclusion An anesthesiologist may be encountering such patients during cardiac catheterization study or emergency non-cardiac surgery, where an understanding of the complex anatomy (the aorta, left pulmonary artery, coronary artery, all arising from the common arterial trunk, the truncus arteriosus) and the physiology of their circulation would help in safe anesthesia. From our report, we conclude intra venous ketamine along with regional analgesia would be safe for sedating such patients coming for cardiac catheterization study.

Absztrakt: Esetismertetés: Esetünkben neoadjuváns onkológiai kezelést követően egy olyan pancreas baloldali tumor sebészi eltávolítását ismertettük, ami infiltrálta a truncus coeliacus oszlást és a gyomort is. A beavatkozás során a truncust resecáltuk, distalis pancreatectomiát, splenectomiát, total gastrectomiát, bal mellékvese-resectiót és cholecystectomiát végeztünk. Betegünk zavartalanul gyógyult, csupán átmeneti májfunkciós laboreltérést tapasztaltunk. A histologiai feldolgozás R1 resectiót mutatott, ezért adjuváns onkológiai kezelésre kerül sor. Megbeszélés: A pancreas distalis tumorai gyakran inoperábilisak. Ilyennek számított sokáig a truncus coeliacus oszlását involváló elváltozás is, azonban van lehetőség radikális műtétre a truncus resectiójával, ami után a máj artériás vérellátását a pancreaticoduodenalis árkád biztosítja. Egy esetünk kapcsán ismertettük a műtéti technikát és az ezzel kapcsolatosan felmerülő kérdéseket. Artériás infiltrációt mutató pancreastumor esetén feltétlenül ajánlott a preoperatív kemoterápia, mivel megfelelő válasz esetén van esély a daganat radikális eltávolítására.

Persistent truncus arteriosus is a rare congenital cardiac malformation. It is characterized by a single great artery arising from the heart which supplies the aorta, the origin of coronary arteries and pulmonary arteries. Without surgery, prognosis is poor and 90% of these patients die before one year of age. We report a rare case of an asymptomatic 35-year-old woman with uncorrected persistent truncus arteriosus and hypoplastic right and left pulmonary arteries. Hypoplastic branch pulmonary arteries prevented the development of severe pulmonary arterial hypertension.

Varanus salvator bivittatus has a pair of hemipenes, which is macroscopic anatomically like the copulatory organ in other amniotes, cylindrical-shaped (truncus) with quite flexible, and it located on the base of the caudal tailof the cloaca. However, information about the microscopic anatomy of hemipenes of this animal is scientifically unpublicized. Therefore, the aim of this research is to study the microscopic anatomy of hemipenes of male Varanussalvator bivittatus. The animals were sacrificed by exsanguination under deep anesthetized and fixed in 4 % paraformaldehyde through perfusion then observed visceral site and morphometric. Histomorphological evaluationwas obtained by paraffin preparation with section thickness of 3-4 μm then stained in Hematoxylin-Eosin (HE) and Masson’s Trichrome (MT). The results showed that truncus of hemipenes was lined by stratified squamousepithelium and supported with thick of dense connective tissue and contain cavernous body and blood vessels are found, the muscles not found. The presence of connective tissue that supported in the down part sometimesmake hemipenes are rigid while prurient condition. In the caudal of truncus hemipenes there is retractor muscle of hemipenes which arranged by striated muscles. Hemipenes is flexible because contain with much of blood vesselthat found in truncus hemipenes.

Although the use of robot-assisted thoracoscopic surgery is increasing rapidly, it allows only a limited visual field on the head side because the system's camera port is usually placed in the eighth or ninth intercostal space. Because the visual field on the intrathoracic head side is critical during lung cancer surgery, such as when peeling off the first branches of the pulmonary artery (right truncus superior artery or left upper ventral lobe branch), a poor visual field could be fatal. We therefore devised a new port arrangement, the "Hamamatsu method," which ensures a good visual field.
