Reconstruction of soft-tissue defects in the head and face region requires attention to both functional and aesthetic outcomes. In general, large postburn scars are still a daunting challenge for plastic surgeons. Various types of free flaps were previously performed for head and face reconstruction, including the anterolateral thigh (ALT) flap. However, to fully cover large and complex defects, the skin pedicle needs a sufficient width. Thus, we have combined dual ALT flaps that were harvested from both the lateral thighs. In this article, we described the case of a 49-year-old female who was presented with a severe scar on the right side of the head and face and zygomatic along with the exposure of temporal bones after extensive burns. Two ALT flaps were supplied by perforators of the descending branches of lateral circumflex femoral arteries. These two source arteries were anastomosed end-to-end to merge into a chimeric flap. The 6-month follow-up revealed an acceptable aesthetic outcome. The effectiveness of the ALT chimeric flap for reconstructing head and face postburn contracture is discussed. [ABSTRACT FROM AUTHOR]

Breast deformities caused by silicone injections affect aesthetic results and cause irreversible complications in patients. In the treatment, it is necessary to entirely remove silicone particles and infiltrated and fibrous breast tissues. The maximal preservation of healthy breast tissues is also critical. This report presents a case of severe breast deformities as complications 20 years after silicone injections at an unreputable aesthetic center. During the surgery, the authors separately removed fluid (silicone) masses and reconstructed mammary glandular tissues. Breast reconstruction was performed by the anchor breast lift along with the functional preservation of the nipple-areola complex and the superomedial pedicle. The surgery entirely addressed complications after injecting a large amount of silicone. 6 months postoperatively, the surgical outcomes were satisfactory. The surgical excision should be done to remove silicone-infiltrated tissues as much as possible before the reconstructive surgery. The combination of radical surgical excision and reconstructive surgery using the anchor breast lift as a single-stage procedure brought good aesthetic results. [ABSTRACT FROM AUTHOR]

Introduction: Radiation-induced ulceration is a late-stage skin reaction after RT for cancer treatment., Objective: The present study examined the use of a single-stage reconstructive procedure to manage radiation-related wounds., Materials and Methods: Nine patients with radiation-induced chronic ulcer with accompanying severe complications were admitted to the Plastic, Reconstructive, and Regenerative Center of Viet Nam National Burn Hospital between October 2015 and September 2019. The patients ranged in age from 49 to 77 years. Complications included exposed cheekbone (n = 1), exposed trachea (n = 1), exposed carotid artery (n = 2), exposed axillary artery (n = 2), exposed pleura (n = 1), and exposed pericardium (n = 2). Flap type used to achieve defect coverage after debridement was individualized to each patient and included ALT, LD, SCA, and DIEP flaps. Additionally, a high-density polyethylene was used to reconstruct the trachea to recover breathing function in the patient with exposed trachea., Results: Complete survival of all flaps was achieved. Most vital organs (the trachea, axillary artery, and carotid artery) were covered. Hospital length of stay ranged from 15 to 120 days., Conclusions: The successful management of patients with severe complications suggests that immediate single-stage reconstruction may be a valuable option for managing radiation-induced ulcers.
