BackgroundLesbian, gay, bisexual, transgender, and queer (LGBTQ+) people are at higher risk of mental health problems due to widespread hetero- and cisnormativity, including negative public attitudes toward the LGBTQ+ community. In addition to combating social exclusion at the societal level, strengthening the coping abilities of young LGBTQ+ people is an important goal. ObjectiveIn this transdiagnostic feasibility study, we tested a 6-week internet intervention program designed to increase the ability of nonclinical LGBTQ+ participants to cope with adverse events in their daily lives. The program was based on acceptance and commitment therapy principles. MethodsThe program consists of 6 web-based modules and low-intensity assistance for homework provided by a single care provider asynchronously. The design was a single-group assignment of 15 self-identified LGB community members who agreed to participate in an open trial with a single group (pre- and postintervention design). ResultsBefore starting the program, participants found the intervention credible and expressed high satisfaction at the end of the intervention. Treatment adherence, operationalized by the percentage of completed homework assignments (32/36, 88%) was also high. When we compared participants’ pre- and postintervention scores, we found a significant decrease in clinical symptoms of depression (Cohen d=0.44, 90% CI 0.09-0.80), social phobia (d=0.39, 90% CI 0.07-0.72), and posttraumatic stress disorder (d=0.30, 90% CI 0.04-0.55). There was also a significant improvement in the level of self-acceptance and behavioral effectiveness (d=0.64, 90% CI 0.28-0.99) and a significant decrease in the tendency to avoid negative internal experiences (d=0.38, 90% CI 0.09-0.66). The level of general anxiety disorder (P=.11; d=0.29, 90% CI –0.10 to 0.68) and alcohol consumption (P=.35; d=–0.06, 90% CI –0.31 to 0.19) were the only 2 outcomes for which the results were not statistically significant. ConclusionsThe proposed web-based acceptance and commitment therapy program, designed to help LGBTQ+ participants better manage emotional difficulties and become more resilient, represents a promising therapeutic tool. This study aimed to assess whether gender and age influenced differences in some strength indices in female and male primary school students. A total of 105 primary school students were sampled, 46 girls and 59 boys. The muscle regions on which the tests were applied were: the lumbar region, lower limbs, upper limbs, and abdominal region. The tests performed on the subjects were in accordance with age-specific motor characteristics and the current school curriculum, they were as follows: torso raises from dorsal decubitus and torso raises from facial decubitus, squats, and push-ups. A significant difference was identified in the Push Up and Squats samples.

Although the therapeutic efficacy and commercial success of monoclonal antibodies (mAbs) are tremendous, the design and discovery of new candidates remain a time and cost-intensive endeavor. In this regard, progress in the generation of data describing antigen binding and developability, computational methodology, and artificial intelligence may pave the way for a new era of in silico on-demand immunotherapeutics design and discovery. Here, we argue that the main necessary machine learning (ML) components for an in silico mAb sequence generator are: understanding of the rules of mAb-antigen binding, capacity to modularly combine mAb design parameters, and algorithms for unconstrained parameter-driven in silico mAb sequence synthesis. We review the current progress toward the realization of these necessary components and discuss the challenges that must be overcome to allow the on-demand ML-based discovery and design of fit-for-purpose mAb therapeutic candidates.

The informative role sub-systems of the integrated unitary economic management process are highlighted. Depending on the precedence and subsequence to material activities, their systematization is carried out. At the conceptual level, according to the logic of the solution succession, imposed by the managerial functions realization, within each sub-system are profiled the complexes of issues, and in each complex – the issues totalities. According to such an approach, the place, role and functional value are elucidated, determined the composition and content of the algorithmic interface of these managerial units (sub-system – issues complex – issue). Material incorporates the composition of groups of general character algorithms of the information sub-systems, precedent the economic activities of human materials (manufacturing, distribution (commercialization), consumption) – those of rate-setting, settlement, current foreseeing, as well as operative foreseeing and conducting of these activities. Starting from the predestination of economic management functions, the overall functional content of the fundamental algorithms of the nominated sub -systems is revealed.

Functional value, role and place of the informational subsystem of economic analysis, contributing factors to its accomplishment, evolution, current situation and prospects of this achievement are highlighted and characterized. It is approached the unitary concept of achievement a such examined subsystem within the integrated management system of the organizational unit. The particularities and principles of the establishment of the informatics subsystem of economic analysis are formulated.

Objective: To acknowledge that minimally invasive pelvic exenteration is a feasible alternative to open surgery and potentially identify prediction factors for patient outcome. Methods: The study was designed as a retrospective single team analysis of 12 consecutive cases, set between January 2008 and January 2022. Results: Six anterior and 6 total pelvic exenterations were performed. A 75% of cases were treated using a robotic approach. In 4 cases, an ileal conduit was used for urinary reconstruction. Mean operative time was 360±30.7 minutes. for anterior pelvic exenterations and 440±40.7 minutes. for total pelvic exenterations and mean blood loss was 350±35 mL. An R0 resection was performed in 9 cases (75%) and peri-operative morbidity was 16.6%, with no deaths recorded. Median disease-free survival was 12 months (10-14) and overall survival (OS) was 20 months (1-127). In terms of OS, 50% of patients were still alive 24 months after surgery. Taking into consideration the follow up period, 16.6% of females under 50 or above 70 years old did not reach the cut off and 4 out of 6 patients that failed to reach it were diagnosed with distant metastases or local recurrence (p=0.169). Conclusion: Our experience is very much consistent with literature in regard to primary site of cancer, post-operative complications, R0 resection and survival rates. On the other hand, minimally invasive approach and urinary reconstruction type were in contrast with cited publications. Minimally invasive pelvic exenteration is indeed a safe and feasible procedure, providing patients selection is appropriately performed. [ABSTRACT FROM AUTHOR]

Objectives: Minimally invasive surgery (MIS) has become the preferred option for many gynecologic pathologies since complication rate and postoperative recovery time have decreased considerably. Postoperative complications remain an important aspect when using the MIS approach, if they are not timely or accurately diagnosed and treated. The main aim of the study is to first assess their incidence, followed by identifying possible risk factors. Furthermore, the secondary aim is to identify if the type of MIS approach used, robotic or laparoscopic, may render some additional benefits. Materials and Methods: The database of the General Surgery Department was queried between 2008 and 2019 for patients with gynecologic pathology: 2907 cases were identified. An additional selection was performed using the following filters: MIS and neoplasia. All emergency surgeries were excluded. One hundred and ninety-eight cases were obtained. Results: The majority of complications were urological (11.6%) with only 7.07% requiring a specific urological procedure. The second most common was lymphorrhea 4.5%. Dindo–Clavien classification correlates positively with the postoperative hospital stay (PHS) (P = 0.000), the type of surgery (P = 0.046), the primary tumor location (P = 0.011), conversion rate (P = 0.049), the expertise of the lead surgeon (P = 0.012), and the operative time (P = 0.002). The urological complications correlate positively with the type of surgery (P = 0.002), the tumor location (P = 0.001), early reintervention (P = 0.000), operative time (P = 0.006), postoperative hemorrhage (P = 0.000), pelvic abscess (P = 0.000), venous thrombosis (P = 0.011), and postoperative cardiac complications (P = 0.002). Laparoscopic and robotic approaches were comparatively assessed. The PHS (P = 0.025), the type of surgery performed (P = 0.000), and primary tumor location (P = 0.011) were statistically significantly different. Conclusion: Postoperative complications reported after MIS for gynecological malignancies show similar incidence as in the current literature, also taking into consideration those for the open approach. The robotic approach seems to be able to perform more complex surgeries with no difference in the postoperative complication rates. The expertise of the lead surgeon in gynecology correlates with lower postoperative complications. Further prospective studies are needed to confirm these results. [ABSTRACT FROM AUTHOR]

The necessity of return and revision of elaborative stage of constitution of the economic informatics systems (E.Ic.S.) it’s substantiated. Also, the idea of opposite order of the concept of invention, elaboration, production and application of the informatics resources, as well as, and the concept of them estimation and selection are formulated.

Concepts of economic information are analyzed, transforming, processing, carried out the general classification and elucidated their conceptual content. Also, it's established the place, the role, interconnections and interactions of economic informative subsystems. Starting of the composition and structure of the unitary economic informational process, are highlighted the categories of the stages and operations of this process, the succession of the achievement, current situation ways and prospects of its integral informatics achievement. The necessity of the conscientiousness and consideration of situational information (informative, descriptive) as a decisive factor of the interspersing and interconnection of the material compartment and decisional informational under compartment of the unitary economic administrational process is motivated. The essential factors what motivated the integration of functional and informatics processing of situational information of the unitary economic managerial process are revealed in evidence, enumerated and examined.
