1. Ultraviolet H$_2$ luminescence in molecular clouds induced by cosmic rays

Abstract

Galactic cosmic rays (CRs) play a crucial role in ionisation, dissociation, and excitation processes within dense cloud regions where UV radiation is absorbed by dust grains and gas species. CRs regulate the abundance of ions and radicals, leading to the formation of more and more complex molecular species, and determine the charge distribution on dust grains. A quantitative analysis of these effects is essential for understanding the dynamical and chemical evolution of star-forming regions. The CR-induced photon flux has a significant impact on the evolution of the dense molecular medium in its gas and dust components. This study is intended to evaluate the flux of UV photons generated by CRs to calculate the photon-induced dissociation and ionisation rates of a vast number of atomic and molecular species, as well as the integrated UV photon flux. Our study takes advantage of recent developments in the determination of the spectra of secondary electrons, in the calculation of state-resolved excitation cross sections of H$_2$ by electron impact, and of photodissociation and photoionisation cross sections. We calculate the H$_2$ level population of each rovibrational level of the $X$, $B$, $C$, $B'$, $D$, $B''$, $D'$ and $a$ states. We then compute the UV photon spectrum of H$_2$ in its line and continuum components between 72 and 700 nm, with unprecedented accuracy as a function of the CR spectrum incident on a molecular cloud, the H$_2$ column density, the isomeric H$_2$ composition, and the dust properties. The resulting photodissociation and photoionisation rates are, on average, smaller than previous determinations by a factor of about 2, with deviations up to a factor of 5. A special focus is given to the photoionisation rates of H$_2$, HF, and H$_2$, as well as to the photodissociation of H$_2$, which we find to be orders of magnitude higher than previous estimates., Comment: 22 pages, 14 figures, accepted by Astronomy & Astrophysics

Abstract

Cosmic rays (CRs) at sub-TeV energies play a fundamental role in the chemical and dynamical evolution of molecular clouds, as they control the ionisation, dissociation, and excitation of H$_{2}$. Their characterisation is important both for the interpretation of observations and for the development of theoretical models. The methods used so far for estimating the CR ionisation rate ($\zeta$) in molecular clouds have several limitations due to uncertainties in the adopted chemical networks. We refine and extend the method proposed by Bialy (2020) to estimate $\zeta$ by observing rovibrational transitions of H$_{2}$ at near-infrared wavelengths, which are mainly excited by secondary CR electrons. Combining models of interstellar CR propagation and attenuation with the calculation of the expected secondary electron spectrum and updated H$_{2}$ excitation cross sections by electron collisions, we derive the intensity of the four H$_{2}$ rovibrational transitions observable in dense, cold gas: (1-0)O(2), (1-0)Q(2), (1-0)S(0), and (1-0)O(4). The proposed method allows the estimation of $\zeta$ for a given observed line intensity and H$_{2}$ column density. We are also able to deduce the shape of the low-energy CR proton spectrum impinging upon the molecular cloud. We present a look-up plot and a web-based application that can be used to constrain the low-energy spectral slope of the interstellar CR proton spectrum. We comment on the capability of the James Webb Space Telescope to detect these near-infrared H$_{2}$ lines, making it possible to derive for the first time spatial variation of $\zeta$ in dense gas. Abstract

This monograph contains the proceedings from the Advanced Study Institute on'Vascular Endothelium: Physiological Basis of Clinical Problems'which took place in Corfu, Greece in June 1990. The meeting consisted of twenty-eight lectures, most of them adapted as full length papers in this volume, as well as numerous short oral and poster communications which are abstracted and also included in alphabetical order (pages 239-302). There were ninety-six participants from ten NATO and four other European countries. The meeting was the second in as many years dealing with a specific subject in Endothelial Cell biology. Following the 1988 discussion on'Receptors and Transduction Mechanisms', the present ASI recognized and tried to deal with the increasing overlap in interest between basic scientists studying endothelial cell functions and clinicians facing problems of known or suspected endothelial pathological involvement. As with any similar effort, we opted to be selective, rather than fail by trying to be inclusive, in the subjects covered. We chose to discuss diseases, such as atherosclerosis, sepsis, ARDS and stroke, based on their relevance to endothelial cell function and urgent need for new insights into their pathogenesis and treatment. Similarly, we examined endothelial cell functions by considering their relevance to disease and their potential for elucidating important pathologies. Obviously, some areas were covered superficially or not at all; this should not distract from their importance, but rather reflect on the constraints of time and -not at all negligibly -the bias of the organizing committee.

This book is a compilation of the lectures and oral and poster communications presented at the Advanced Study. Institute on'Vascular Endothelium: Physiological Basis of Clinical Problems II,'which took place between June 20 and 30, 1992 in Rhodes, Greece. This third in a series of ASIs on vascular endothelium continued on the theme of the first (1988) ASI on'Receptors and Transduction Mechanisms'and particularly expanded that of the 1990 conference on''Physiological Basis of Clinical Problems.'We continued the successful practice of bringing together clinicians and scientists: this was reflected equally well in the composition of the organizing committee as in the background of the particiJ?ants. Endothelial cell functions and dysfunctions present as many challenges to the mvestigator as they do to the curious clinical practitioner. As these problems are necessarily different, this unique ten-day co-habitation of these individuals continued to offer fresh outlooks to each, stimulated potential collaborative efforts and, most importantly, advanced --ever so slightly--our knowledge of vascular biology. This year's conference was further enriched by the presence of several of our colleagues from Eastern Europe whom we are delighted to welcome as officially sponsored participants to this and future NATO-supported meetings. It is never superfluous to remind readers and participants that those signing at the bottom of this page, while responsible for many of the ASI's and the book's deficiencies, are but three of the many contributors to the successes.

Introducción: Los pacientes oncológicos pueden presentar dolor de tipo neuropático, el cual representa un reto difícil, con buena respuesta a la medicación antiepiléptica. Existen diversos estudios que confirman los buenos resultados asociados al uso de topiramato en pacientes con cáncer y sintomatología neuropática acompañante. Las diferentes drogas antiepilépticas difieren de forma sustancial en sus efectos sobre el peso corporal. El topiramato conlleva cuadros de anorexia y/o pérdida del peso hasta en un 15% de los casos. Casos clínicos: Tres pacientes oncológicos en los que la pérdida de peso asociada al empleo de topiramato fue vivida como un efecto negativo físico y psíquico, por temor a una posible recidiva tumoral. Conclusiones: Topiramato es un fármaco eficaz en el tratamiento del dolor de tipo neuropático. La pérdida de peso que resulta de su utilización puede ser beneficiosa en pacientes con sobrepeso importante. Este efecto podría ser una contraindicación relativa a su uso en pacientes con clínica de anorexia, desnutrición severa y en pacientes oncológicos. En estos últimos, la aparición de este síndrome constitucional puede conllevar un importante aumento de episodios de ansiedad en la familia y el propio enfermo, así como un exceso de pruebas médicas para descartar una progresión tumoral. Introduction: Neuropatic pain in cancer patients is a difficult problem which represents a difficult challenge, with good answer to the antiepileptic drugs. Diverse studies exist that confirm the good results associated to the use of topiramate in cancer patients with neuropatic pain. The data demonstrate that antiepileptic drugs can differ from substantially in their effects on weight. Approximately a fifth of patients treated with topiramate experience anorexia and/or weight loss. Clinical cases: Three oncologic patients in the reduction weight associated to the topiramate use was lived like a physical and psychic negative effect. Conclusions: Topiramate is an effective drug in the treatment of the pain neuropatic. The reduction in weight that is from its use can be beneficial in patients with important overweight. This effect could be a contraindication relative to its use in patients with anorexic clinic, severe desnutrition and cancer patients. In these last ones, the appearance of this constitutional syndrome can entail an important increase of episodes of anxiety in the family and the own patient, as well as an excess of medical tests to discard a tumorlike progression.

Presentamos el caso de una paciente que sufrió accidente de tráfico con avulsión del plexo braquial izquierdo, y que presentaba dolor muy intenso (escala visual analógica 8) de características neuropáticas en la fase aguda postraumática. Dosis altas de remifentanilo fueron inefectivas para control del cuadro álgico, el cual se trato con éxito con una infusión de ropivacaína a través de catéter epidural cervical (C5-6). El dolor es controlado en fase crónica (escala visual analógica 2) con agentes específicos contra dolor neuropático (gabapentina, amitriptilina, clonacepam y tramadol). We presented you a patient who suffered a left brachial plexus avulsión with hard neuropatic pain in the posttraumatic acute phase (visual analogue scale 8). High-dose remifentanil infusión was uneffective in controlling pain, which was further ameliorated by ropivacaine infused through a cervical (C5-6) epidural catheter. At discharge pain remained controlled (visual analogue scale 2) with specific treatment against neuropathic pain (gabapentin, amytriptiline, clonacepam, and tramadol).

40. A Paper-Based Multiplexed Transaminase Test for Low-Cost, Point-of-Care Liver Function Testing

Abstract

In developed nations, monitoring for drug-induced liver injury through serial measurements of serum transaminases [aspartate aminotransferase (AST) and alanine aminotransferase (ALT)] in at-risk individuals is the standard of care. Despite the need, monitoring for drug-related hepatotoxicity in resource-limited settings is often limited by expense and logistics, even for patients at highest risk. This article describes the development and clinical testing of a paper-based, multiplexed microfluidic assay designed for rapid, semiquantitative measurement of AST and ALT in a fingerstick specimen. Using 223 clinical specimens obtained by venipuncture and 10 fingerstick specimens from healthy volunteers, we have shown that our assay can, in 15 min, provide visual measurements of AST and ALT in whole blood or serum, which allow the user to place those values into one of three readout “bins” [<3× upper limit of normal (ULN), 3 to 5× ULN, and >5× ULN, corresponding to tuberculosis/HIV treatment guidelines] with >90% accuracy.
