Objective: This study aimed to investigate the impact of drugs used in the treatment of diabetic neuropathy on the frequency of hospitalization, need for debridement and amputation. Methods: One hundred forty-four patients who received inpatient treatment for diabetic foot infection between 2017 and 2020 were analyzed retrospectively through patient files. Patients without diabetic neuropathy were defined as Group 0, those with diabetic neuropathy who used drugs for diabetic neuropathy as Group 1, and those who did not use drugs for diabetic neuropathy were defined as Group 2. The groups were compared in terms of age, height, weight, gender, comorbidities, time elapsed after diabetes mellitus diagnosis and diabetic neuropathy, quantity of diabetic foot attacks, hospitalizations due to diabetic foot infection, need and quantity of debridement and amputation. Results: The time since diagnosis of diabetes mellitus, the rate of insulin use, and hospitalizations due to diabetic foot infection were higher in patients with diabetic neuropathy compared with patients without neuropathy (p=0.01, p=0.02, p=0.024). The difference between Groups 1 and 2 in terms of diabetic foot infection attacks, hospitalizations and debridement needs was statistically significant (p=0.039, p=0.02, p=0.06). When comparing whether or not debridement or amputation was required due to diabetic foot infection and the number of amputations, there was no statistically significant difference between the groups. Conclusions: The results of our study suggest that although the use of medications effective for neuropathic pain does not significantly affect amputation due to diabetic foot, it has a protective effect on the number of diabetic foot attacks, the number of hospitalizations due to diabetic foot, and the number of debridement needs. Objectives: Achilles tendinopathy can be noticed in both acromegaly and ankylosing spondylitis (AS). Acromegaly patients presenting with tendinopathy findings may be confused with AS findings. In this study, sonoelastrographic findings of Achilles tendon are explored in patients with AS and acromegaly. Methods: 25 patients with AS, 30 patients with acromegaly, and 18 healthy controls were enrolled in the study. Achilles tendon was evaluated by sonoelastography in all the study participants. Results: The thickness of Achilles tendon in neutral positions was higher in acromegaly patients than those in AS patients. The sonoelastography measurement of Achilles tendon was increased in acrome)galy patients when compared to the control group and AS patients. Conclusion: The thickness of Achilles tendon can increase in patients with acromegaly and AS. However, the sonoelastographic features of Achilles tendon can be similar in patients with AS and acromegaly [ABSTRACT FROM AUTHOR]

Objectives We compared the laboratory and clinical symptoms of coronavirus disease 2019 (COVID-19), other viral (beside COVID-19), and bacterial conjunctivitis patients with the values of controls. Methods Twenty COVID-19, 15 other viral, 15 bacterial patients, and 15 control group were included in the study. COVID-19 was diagnosed with the real time reverse transcription–polymerase chain reaction while the indirect immunofluorescent antibody test was used in the detection of other viral agents. Bacterial agents were determined with the detection of the agent. The ophthalmologic examination of all cases was carried out by direct penlight, and the anterior segment evaluation was performed. The laboratory findings of all cases included in the study were obtained from the hospital records. Results It was determined that 5% conjunctivitis and 5% diplopia developed in patients diagnosed with COVID-19, 20% conjunctivitis developed in patients due to other viral agents, and 66.6% conjunctivitis developed in patients due to bacterial agents. Conclusion The incidence of conjunctivitis in COVID-19 patients was lower than in other viral and bacterial groups. Neutrophil/lymphocyte ratio and C-reactive protein come to the forefront as precious parameters with high specificity and sensitivity that might be useful to distinguish these diseases. Amaç Koronavirüs hastalığı 2019 (COVID-19), diğer virüs (COVID-19 dışında) ve bakteriyel konjunktivit hastalarının laboratuvar ve klinik semptomlarını kontrol değerleri ile karşılaştırdık. Gereç ve Yöntemler Çalışmaya 20 COVID-19, 15 diğer viral, 15 bakteriyel hasta ve 15 kontrol grubu dahil edildi. COVID-19, gerçek zamanlı ters transkripsiyon-polimeraz zincir reaksiyonu ile teşhis edilirken, diğer viral ajanların saptanmasında indirekt immünofloresan antikor testi kullanıldı. Etken tespiti ile bakteriyel etkenler belirlendi. Tüm olguların oftalmolojik muayenesi direk ışıkla yapıldı ve ön segment değerlendirmesi yapıldı. Çalışmaya dahil edilen tüm olguların laboratuvar bulguları hastane kayıtlarından elde edildi. Bulgular COVID-19 tanısı alan hastalarda% 5 konjunktivit ve% 5 diplopi geliştiği, diğer viral ajanlara bağlı hastalarda% 20 konjunktivit geliştiği ve hastalarda bakteriyel ajanlara bağlı% 66.6 konjunktivit geliştiği belirlendi. Sonuç COVID-19 hastalarında konjunktivit insidansı diğer viral ve bakteriyel gruplara göre daha düşüktü. Nötrofil / lenfosit oranı ve C-reaktif protein, bu hastalıkları ayırt etmede faydalı olabilecek yüksek özgüllük ve duyarlılığa sahip değerli parametreler olarak öne çıkmaktadır.

When we talk about premature ovarian insufficiency (POI) we accept the existence of decreased stem cell and their regenerative abilities. the present study was to explain how restorating damaged ovarian cells following injury to maintain sequential reproductive events in evidence-based medicine indexed in PubMed and Web of Science. the exact mechanism is unclear stem cells transfer may improve compromised ovarian function and fertility outcome in women with POI. Soluble factors secreted by stem cell may rescue impaired mitochondrial function in oogonial stem cells, enhance metabolic capacity of resident stem cells, induce local neovascularization in the ovary, and activate gene shifting between transferred stem cells and germ cell precursors. This review may provide insight into how stem cells show some of their beneficial effects on compromised ovarian microenvironment and germ cell niche and paves the way for clinical trials for improving ovarian function of women with POI. We also had the opportunity to share our hypothesis about the design and development of induced oogonial stem cell (iOSC) and its use in POI.

WOS: 000457530200011, PubMed: 30402933, Aim Prevalence rates of gestational diabetes mellitus (GDM) show considerable variation among different countries and regions of the world. The primary aim of this study was to determine the nationwide prevalence and predictors of GDM in Turkey. Methods We conducted prospective nationwide screening among pregnant women. Between August 2016 and November 2017, a total of 2643 pregnant women from 51 centres in 12 different regions were enrolled. A two-step screening method and Carpenter and Coustan criteria were used in the diagnosis of GDM. Clinical and biochemical data were obtained using electronic database software. Results The national prevalence of GDM was found to be 16.2% [95% confidence intervals (CI) 15.0% to 17.4%] without a significant difference between urban and rural regions. Women with GDM were older (mean age: 32 +/- 5 vs. 28 +/- 5 years, P < 0.001) and heavier (mean BMI: 27.2 +/- 5.1 vs. 24.7 +/- 4.7 kg/m(2), P < 0.001) than their counterparts without GDM. The prevalence of GDM tended to increase with age (< 25 years, 6.9%; 26-35 years, 15.6%; and 36-45 years, 32.7%; P < 0.001). Maternal age, maternal BMI, history of previous GDM and family history of diabetes mellitus were independent predictors of developing GDM (P < 0.05 for all). Low-risk women (age < 25 years, BMI < 25 kg/m(2), no family history of diabetes) comprised 10.7% of the total population and the prevalence of GDM in these women was 4.5% (95% CI 2.4% to 7.8%). Conclusion The results of this nationwide study indicate that GDM is very common, affecting one in seven pregnancies in Turkey. Implementation of international guidelines on screening and management of this public health problem is required.

Anxiety-Depression Disorders and Bone Mineral Density in Diabetic Foot Patients Objective: In this study it is aimed to evaluate the depression and anxiety scores as well as biochemical measures in diabetic foot (DF) and none diabetic foot (Non-DF) patients, also to compare the bone mineral densitometry (BMD) measures in order to point out any possible correlation. Material and Method: Diabetic patients who admitted to Firat University Hospital among which 37 had a diagnosis of DF and 40 Non-DF were included in the study according to the Hospital Anxiety and Depression Scale. HbA1c, magnesium (Mg), albumin, urea, creatinine, calcium (Ca), phosphorus (P), parathormone, vitamin D levels and body mass index. (BMI) were recorded for all patients and Wagner classification scores are noted for the cases with DF. In addition, BMD was measured via Dual-energy X-ray absorptiometry from lumbar region and femoral head. Results: The depression and anxiety scores were both significantly higher in female patients DF or Non-DF. It is also reported that the BMD scores were significantly lower in female patients. Results of bone mineral densitometry were not statistically significant in the DF group. There was a positive correlation between BMI and BMD. Also, it has been found that Ca, P, Mg in DF were remarkably lower than in Non-DF. Conclusion: Anxiety, depression and BMI increase while BMD decreases in diabetic foot patients. Diabetic foot complications are not only considered as endocrine and metabolic disorders, so that they should be treated with a multidisciplinary approach including psychiatry. [ABSTRACT FROM AUTHOR]

Diabetes Mellitus insülin direnci veya yetersizliğinin neden olduğu kronik bir metabolik hastalıktır. Hipertansiyon, semptomları ile rahatsızlıklara, komplikasyonları ile morbidite ve mortaliteye sebep olan önemli bir sağlık problemidir. Salusinler; en önemli görevlerini kardiyovasküler sistem üzerinde gösteren vazodilatör, hipotansif peptidlerdir. Apelin, insülin duyarlılaştırıcı, vazokonstrüktif ve pozitif inotropik etkili bir peptiddir. Bu çalışmada, diyabetik ve hipertansif hastalarda, serum salusin-? ve salusin-ß ve apelin düzeylerinin belirlenmesi, diabetes mellitus ve hipertansiyon ile ilişkilerinin incelenmesi amaçlanmıştır.Çalışmaya 36 diyabet olup hipertansiyon olmayan, 41 diyabet + hipertansiyon olan, 32 diyabet olmayıp hipertansiyon olan ve 30 diyabet ve hipertansiyon olmayan sağlıklı kontrol grubu alındı. Objective: Diabetes has become epidemic not only in our country but also all over the world. Foot ulcers in diabetic patients should be considered not only as an ulcer or a complication of diabetes but also as changes in the metabolic composition of the body. Therefore, it is aimed, to compare the demographic characteristics, anthropometric measurements and some biochemical parameters of healthy controls, diabetic foot ulcer patients (DFUP) and non-ulcer diabetic patients (NUDP) in this study. Material and Method: A total of 90 individuals, 26 DFUP, 34 NUDP and 30 healthy controls were included. The DFUP group was evaluated according to the Wagner classification. Body mass index (BMI), neck, waist and hip circumference, fasting blood sugar level, HbA1c, lipid profile, aspartat aminotransferase (AST), alanin aminotransferase (ALT), albumin, urea and creatinine levels were obtained from patient database and body composition analysis was detected by using TANITA device. Results: It is reported that proportion of fat was 27.4% in DFUP group, 33.9% in the NUDP group and 22.3% in the control group. Lipid profile measures in the DFUP group were significantly lower than the other two groups. It is observed that in the DFUP group, the Wagner score increases as the BMI (p =0,452), waist circumference (p =0,848) and hip circumference (p =0,614) decreases. Also, in DFUP patients, an increase in the amount of urea and decrease in the albumin were reported. Conclusion: Decrease in fat, albumin and lipid profile measures and increase in urea levels in DFUP patients indicate that these parameters play a role in the etiopathology of this disease. Therefore, we conclude that keeping the body fat composition close to ideal values in the diabetic patient group may have a protective role against DFUP formation. [ABSTRACT FROM AUTHOR]

Sistemik lupus eritematoz (SLE) ve romatoid artrit (RA), kronik inflamatuar hastalıklardır. Aterosklerozun da kronik inflamatuar bir süreç olduğu kabul edilmektedir. SLE ve RA'te, erken ve hızlanmış ateroskleroz sık görülmekte ve sonuçta ortaya çıkan kardiyovasküler hastalıklar morbidite ve mortalitede artışa neden olmaktadır. Adiponektin insülin duyarlandırıcı, anti-aterojenik ve anti-inflamatuar etkileri olan, visfatin ise insülin duyarlandırıcı özelliğine ek olarak inflamatuar süreçte rol aldığı düşünülen adipositokinlerdir. Type 2 diabetes mellitus is a chronic disease characterized by insulin resistance or relative insulin deficiency. Alarin and galanin are neuropeptides that play an important role in energy metabolism. In this study, we have studied alarin and galanin saliva and blood which we think may play a role in the etiopathogenesis of type 2 diabetes mellitus. In addition, two salivary glands (submandibular and parotid glands) were stained with alarin and galanin antibodies to determine whether human salivary glands synthesize alarin and galanin.Thirty newly diagnosed type 2 diabetes patients and 30 healthy controls were included in the study. The patients were divided into 3 groups of new type 2 diabetes (pre-treatment), metformin therapy (post-treatment) for 12 weeks (control) and healthy subjects (control). ÖZET Hem hipertansiyon (HT) hem de diabetes mellitus (DM) gelişmekte olan ve gelişmiş ülkelerde önemli bir halk sağlığı sorunudur. DM ve HT birlikteliği mikro ve makrovasküler komplikasyonları hızlandırarak mortalite ve morbidite riskini arttırır. Hipertansiyon, diyabetli hastaların yarısında görülmektedir. Yeni tanı alan diyabetlilerin yaklaşık %40'ında HT mevcuttur. Normal sağlıklı popülasyonla karşılaştırıldığında hipertansif hastalarda DM sıklığı 2,5 kat artmıştır. Hipertansiyon ile DM' nin birlikteliği her iki hastalığın patogenezinde suçlanan ortak birkaç mekanizmaya bağlı ortaya çıkar. Katekolaminler ile HT ve HT+DM arasında ilişkiyi araştıran sınırlı sayıda çalışma mevcut olup katekolaminlerin sentezinde görev alan cerebellin adlı molekül henüz çalışılmamıştır. Bu yüzden bu çalışmada HT ile HT+DM tanısı alan hastaların tedavi öncesi ve tedavi sonrası hem idrar hem de kanlarında katekolamin ile cerebellin düzeylerinin kaderinin ne olduğunu ortaya çıkarmayı amaçlandı. Bu çalışmaya yeni tanı almış 30 tane hipertansiyon hastası, daha önce diabetes mellitus tanısı olup yeni hipertansiyon gelişen 30 hasta ve bilinen bir hastalığı olmayan 30 tane sağlıklı gönüllü bireyden oluşan kontrol grubu dahil edildi. Tedavi öncesi ve tedavi sonrası eş zamanlı olarak 5 cc idrar ve kan alındı. Alınan biyolojik numunelerden cerebellin, adrenalin, noradrenalin, metanefrin ile normetanefrin düzeyleri ELISA yöntemiyle çalışıldı. Diğer biyokimyasal parametreler (AKŞ, HbA1c, LDL, TG) ise otoanalizörle çalışıldı. Ayrıca cinsiyet, yaş, kan basıncı ve VKİ kayıt edildi. Gruplar arası karşılaştırma yapıldı. Kontrol ve HT, HT+DM grupları arasında VKİ açısından anlamlı farklılık saptanmadı(p>0,05). Tip 2 diyabetes mellitus (DM) hastalığı günümüzde hızla artan bir toplum sağlığı sorunudur. Hastalık seyri sırasında gelişen mikro ve makrovasküler komplikasyonlar hastaların mortalite ve morbiditesini etkilemektedir. Bu çalışmada tip 2 DM lu hastalarda cerebellin ile betatrophin adlı moleküllerin tip 2DM lu hastalarda patofizyolojideki etkilerini ve birbirleriyle olan ilişkilerini aydınlatmaya çalışıldı.Fırat Üniversitesi Tıp Fakültesi İç Hastalıkları Anabilim Dalı, Endokrinoloji ve Metabolizma Hastalıkları ve Genel ahiliye Bilim Dalı polikliniklerinde yapılan bu çalışmaya yeni tanı almış toplam 30 tane tip 2 diyabet hastası ile 30 tane sağlıklı bireyden oluşan kontrol grubu dahil edildi. Hastalar yeni tip 2 DM tanısı almış (tedavi öncesi), 12 hafta boyunca 2000 mg metformin tedavisi almış (tedavi sonrası) ve sağlıklı bireylerden oluşan (kontrol) 3 gruba ayrıldı.Hastalardan tedavi öncesi ve tedavi sonrası kan ve tükrük numuneleri alındı. Sağlıklı bireylerden de kan ve tükrük numuneleri alındı. Mevcut numunelerden cerebellin ve betatrophin, AKŞ, TKŞ, HOMA-IR, HgbA1c, HDL, LDL, TRG değerleri, c-peptit, insülin düzeyleri çalışıldı. Grublar arası karşılaştırma yapıldı.Kontrol ve tedavi öncesiguruplar arasında AKŞ, TKŞ, HOMA-IR ve HgbA1c düzeyleri istatiksel olarak anlamlı farklılık saptandı.
