Patients with dialysis-related amyloidosis suffer from bone and tissue damage that may be due to the reaction of sugar aldehydes with amino acids. These advanced glycation products form deposits in the destroyed bone tissue which are associated with amyloid fibrils formed by beta-microglobulin. Abstract

There are several reports on the use of the over-the-scope clip (OTSC) for gastrointestinal bleeding/fistula and endoscopic iatrogenic perforation. However, there are almost no reports on OTSC use for anastomotic leakage (AL) after colorectal cancer surgery. The purpose of this study was to evaluate the outcome of AL closure using the OTSC. Five patients who had undergone AL after laparoscopic surgery for colorectal cancer from April 2017 to April 2019 were evaluated. The average distance from the anal verge of the anastomosis site was 12 (5–18) cm. The average diameter of the dehiscent part was 10.9 (9.3–14.4) mm. The average number of OTSC days after the occurrence of AL was 11 (5–22). On the contrast examination immediately after OTSC, all cases were completely closed, but in the later contrast examination, only one case remained completely closed. The average incompletely closed diameter was 3.6 (2.9–5.1) mm, and the diameter of the dehiscent part was reduced in all cases. Only one patient ultimately underwent colostomy; the rest were cured with OTSC alone. AL site closure using the OTSC after colorectal cancer surgery is a useful minimally invasive treatment when combined with appropriate drain management. [ABSTRACT FROM AUTHOR]

Abstract

Fast atom bombardment mass spectrometry, amino acid sequence analysis and reverse-phase high performance liquid chromatography of purified beta2 microglobulin (beta2M) from hemodialysis patients for identifying its glycated sites reveal that beta2M glycated sites are present in vivo and are homologous to the sites of in vitro glycated beta2M. An alpha-amino group of the isoleucine amino terminal is the primary glycated site while Lys-91, -58, -48, -94, -41 and -19 epsilon-amino groups are minor glycated sites. Computer graphic analysis of the three-dimensional beta2M conformation indicates that high level solvent accessibility and an acid-base catalyst account for the high Ile-1 specificity of the glycated site.

Abstract

Background. Reactive carbonyl compounds (RCOs) present in heat-sterilized peritoneal dialysis (PD) fluid have been incriminated in the progressive deterioration of the peritoneal membrane observed in long-term PD patients. The present study utilized the glyoxalase I (GLO I) system as a new approach to lower in vitro the peritoneal fluid content of RCOs such as methylglyoxal (MGO), glyoxal (GO) and 3-deoxyglucosone (3-DG). Methods. GO, MGO, and 3-DG solutions or conventional glucose PD fluids were incubated in vitro with various RCO lowering compounds. The evolution of GO, MGO, and 3-DG levels was monitored by high-performance liquid chromatography. The tested compounds included aminoguanidine and glutathione (GSH), alone or together with GLO I. The human GLO I gene was overexpressed in Chinese hamster ovary (CHO) cells, or ubiquitously in transgenic mice. Cell supernatant of the CHO transfectant and protein extracts of various organs of the transgenic mice were also tested. Results. Aminoguanidine incubated with MGO/GO/3-DG mixtures, promptly reduced RCO levels. GSH alone had a similar but milder and slower effect. Together with GLO I, it promptly decreased GO and MGO levels but was less efficient toward 3-DG. After incubation with glucose PD fluid, GSH together with GLO I had the same effect on MGO, GO, and 3-DG levels. Addition of transfected cell supernatant or tissue extracts overexpressing GLO I, together with GSH to either GO, MGO, or 3-DG solutions, promptly and markedly reduced GO and MGO but not 3-DG levels. Conclusions. GLO I together with GSH efficiently lowers glucose-derived RCOs, especially GO and MGO, both in conventional glucose PD fluids and in RCO solutions. The fact that genetically manipulated cells overexpressing GLO I activity have a similar effect suggests that maneuvers raising GLO I activity in peritoneal cells or in the peritoneal cavity might help prevent the deleterious effects of the peritoneal carbonyl stress in PD patients. Abstract

We recently demonstrated that pentosidine, an advanced glycation end product,accumulates markedly as albumin-linked form (P-alb) and in free-form (P-free) in the plasma of patients with end-stage renal failure. The present study was undertaken to examine the clearance of P-alb and P-free by different modalities of renal replacement therapy, that is, hemodialysis (HD), continuous ambulatory peritoneal dialysis (CAPD), and renal transplantation. HD cleared P-free (9.4 +/- 4.3 nmol/kg/HD) but not P-alb, by diffusion but not by membrane adsorption, whereas CAPD cleared both P-alb (4.03 +/- 2.01 nmol/kg/day) and P-free (2.43 +/- 1.24 nmol/kg/day). Plasma total pentosidine levels were significantly (P < 0.05) lower in CAPD (0.97 +/- 0.41 nmol/ml) than in HD (1.19 +/- 0.41 nmol/ml), as the result of a lower serum albumin level in the former patients. Indeed, P-alb expressed per mg albumin was virtually identical in HD and CAPD. By contrast, P-free was significantly lower in CAPD than in HD. P-alb levels were significantly correlated with plasma P-free levels in both HD and CAPD patients, but not in the CAPD dialysate. Pentosidine transport across the peritoneum occurs mainly by diffusion, both as P-alb and P-free. Interestingly, peritoneal P-alb clearance (0.17 +/- 0.07 ml/min) significantly (P < 0.00001) exceeded albumin clearance (0.11 +/- 0.05 ml/min). P-alb levels being significantly higher (P < 0.0005) in the peritoneal fluid (36.28 +/- 18.55 pmol/mg) than in the serum (27.12 +/- 11.71 pmol/mg), thus raises the possibility of a facilitated diffusion of P-alb or an active transport mechanism for protein-linked pentosidine into the peritoneal cavity. After renal transplantation, plasma P-free fell rapidly, remained barely detectable after one month, and returned to normal at six months. By contrast, P-alb fell more slowly and remained significantly above normal at six months, but returned eventually to normal levels. These findings demonstrate that: (1) both HD and CAPD
