Objective The available literature regarding outcome after pancreatic resection in locally advanced non-functional pNEN (LA-pNEN) is sparse. Therefore, this study evaluates the current survival outcomes and prognostic factors in after resection of LA-pNEN. Materials and methods This population-based analysis was derived from 17 German cancer registries from 2000 to 2019. Patients with upfront resected non-functional non-metastatic LA-pNEN were included. Results Out of 2776 patients with pNEN, 277 met the inclusion criteria. 137 (45%) of the patients were female. The median age was 63 ± 18 years. Lymph node metastasis was present in 45%. G1, G2 and G3 pNEN were found in 39%, 47% and 14% of the patients, respectively. Resection of LA-pNEN resulted in favorable 3-, 5- and 10-year overall survival of 79%, 74%, and 47%. Positive resection margin was the only potentially modifiable independent prognostic factor for overall survival (HR 1.93, 95% CI 1.71–3.69, p value = 0.046), whereas tumor grade G3 (HR 5.26, 95% CI 2.09–13.25, p value p value = 0.012) were the only independent prognostic factors for disease-free survival. Conclusion Resection of LA-pNEN is feasible and associated with favorable overall survival. G1 LA-pNEN with negative resection margins and absence of lymph node metastasis and lymphangiosis might be considered as cured, while those not fulfilling these criteria might be considered as a high-risk group for disease progression. Herein, negative resection margins represent the only potentially modifiable prognostic factor in LA-pNEN but seem to be influenced by tumor grade.

Background Gastrointestinal stromal tumors (GIST) are rare mesenchymal tumors. They are most frequently located in the stomach but are also found in the esophagus and the gastroesophageal junction (GEJ). Information regarding the prognostic factors associated with upper gastrointestinal GIST is still scarse. Methods In this study, datasets provided by the German Clinical Cancer Registry Group, including a total of 93,069 patients with malignant tumors in the upper GI tract (C15, C16) between 2000 and 2016 were analyzed to investigate clinical outcomes of GIST in the entire upper GI tract. Results We identified 1361 patients with GIST of the upper GI tract. Tumors were located in the esophagus in 37(2.7%) patients, at the GEJ in 70 (5.1%) patients, and in the stomach in 1254 (91.2%) patients. The incidence of GIST increased over time, reaching 5% of all UGI tumors in 2015. The median age was 69 years. The incidence of GIST was similar between males and females (53% vs 47%, respectively). However, the proportion of GIST in female patients increased continuously with advancing age, ranging from 34.7% (41–50 years) to 71.4% (91–100 years). Male patients were twice as likely to develop tumors in the esophagus and GEJ compared to females (3.4% vs. 1.9% and 6.7% vs. 3.4%, respectively). The median overall survival of upper gastrointestinal GIST was 129 months. The 1-year, 5-year, and 10-year OS was 93%, 79%, and 52% respectively. Nevertheless, tumors located in the esophagus and GEJ were associated with shorter OS compared to gastric GIST (130 vs. 111 months, p = 0.001). The incidence of documented distant metastasis increased with more proximal location of GIST (gastric vs. GEJ vs. esophagus: 13% vs. 16% vs. 27%) at presentation. Conclusion GIST of the esophagus and GEJ are rare soft tissue sarcomas with increasing incidence in Germany. They are characterized by worse survival outcomes and increased risk of metastasis compared to gastric GIST.

ZusammenfassungLaut aktuellen revidierten Fukuoka-Leitlinien besteht die Indikation zur Resektion von BD-IPMN des Pankreas mit Worrisome Features, da hier ein Risiko der malignen Entartung von bis zu 30% besteht. Die Resektion kann bei fehlenden klinischen, bildmorphologischen und laborchemischen Malignitätszeichen als eine nicht anatomische, lokale Exzision durchgeführt werden.Eine robotische Enukleation bei benignen Raumforderungen der Bauchspeicheldrüse stellt eine sehr gute Alternative zu den resezierenden Verfahren, insbesondere zu denjenigen in offener Technik, dar. Diese operative Behandlungsoption wird bei einem Mindestabstand bis zum pankreatischen Hauptgang von mindestens 2 mm im „International consensus statement on robotic pancreatic surgery“ nahegelegt.Neben den bekannten Vorteilen der minimalinvasiven Chirurgie führt diese parenchymsparende Methode zum Erhalt von endo- und exokriner Funktion (ca. 90%) und zu einem progressfreien 10-Jahres-Überleben von ca. 75% bei etwas erhöhter Morbidität (ca. 60%) verglichen mit den resezierenden Verfahren.Der folgende Videobeitrag präsentiert das Operationsvideo einer robotischen Zystenenukleation (bei Verdacht auf eine BD-IPMN mit Worrisome Features) im Pankreaskopf und Processus uncinatus bei einer 62-jährigen Patientin mit besonderer Betonung der wichtigsten vaskulären Landmarken, Besonderheiten der Herangehensweise und Vorteile der robotischen Technik.

Zusammenfassung Hintergrund In den letzten Jahren haben sich Veränderungen in der Behandlung des duktalen Pankreaskarzinoms bezüglich der multimodalen Therapie und auch der chirurgischen Therapie ergeben. Diese Veränderungen sind bisher noch nicht in großen landesweiten Studien in Deutschland untersucht worden. Die vorliegende Arbeit gibt einen ersten Überblick aus chirurgischer Perspektive über die Entwicklungen in Diagnose, Therapie und Überleben des Pankreaskarzinoms innerhalb der letzten 19 Jahre in Deutschland. Methoden In dieser Kohorte aus 18 klinischen Krebsregistern Deutschlands wurden Patienten mit Diagnose eines duktalen Pankreaskarzinoms von 2000–2018 eingeschlossen. Die Patienten wurden nach Diagnosejahren (2000–2009 vs. 2010–2018) und nach Behandlungsmodalitäten aufgeteilt und verglichen. Ergebnisse In der Kohorte von ca. 48000 Patienten mit duktalem Pankreaskarzinom zeigte sich eine Erhöhung der Zahl der Neuerkrankungen von ca. 18000 auf 30000 Patienten in den beiden 10-Jahres-Abschnitten. Das mediane Gesamtüberleben stieg marginal, aber statistisch signifikant von 7,1 auf 7,9 Monate (p Zusammenfassung Die Krebsregisterdaten scheinen den bekannten Anstieg der Inzidenz des Pankreaskarzinoms in der westlichen Welt zu bestätigen. Resektionsraten sowie die Raten an Behandlung in neoadjuvanter sowie palliativer Intention erhöhten sich ebenfalls. Das Gesamtüberleben aller Patienten mit einem duktalen Pankreaskarzinom verlängerte sich allerdings nur marginal. In Subgruppen von Patienten, die in kurativer Intention behandelt wurden, zeigten sich jedoch mitunter deutlich längere Überlebenszeiten.

Intratumoral heterogeneity (ITH) is an intrinsic feature of malignant tumors that eventually allows a subfraction of resistant cancer cells to clonally evolve and cause therapy failure or relapse. ITH, cellular plasticity and tumor progression are driven by epithelial–mesenchymal transition (EMT) and the reverse process, MET. During these developmental programs, epithelial (E) cells are successively converted to invasive mesenchymal (M) cells, or back to E cells, by passing through a series of intermediate E/M states, a phenomenon termed E–M plasticity (EMP). The induction of MET has clinical potential as it can block the initial EMT stages that favor tumor cell dissemination, while its inhibition can curb metastatic outgrowth at distant sites. In pancreatic ductal adenocarcinoma (PDAC), cellular models with which to study EMP or MET induction are scarce. Here, we have generated single cell-derived clonal cultures of the quasimesenchymal PDAC-derived cell line, PANC-1, and found that these differ strongly with respect to cell morphology and EMT marker expression, allowing for their tentative classification as E, E/M or M. Interestingly, the different EMT phenotypes were found to segregate with differences in tumorigenic potential in vitro, as measured by colony forming and invasive activities, and in circadian clock function. Moreover, the individual clones the phenotypes of which remained stable upon prolonged culture also responded differently to treatment with transforming growth factor (TGF)β1 in regard to regulation of growth and individual TGFβ target genes, and to culture conditions that favour ductal-to-endocrine transdifferentiation as a more direct measure for cellular plasticity. Of note, stimulation with TGFβ1 induced a shift in parental PANC-1 cultures towards a more extreme M and invasive phenotype, while exposing the cells to a combination of the proinflammatory cytokines IFNγ, IL1β and TNFα (IIT) elicited a shift towards a more E and less invasive phenotype resembling a MET-like process. Finally, we show that the actions of TGFβ1 and IIT both converge on regulating the ratio of the small GTPase RAC1 and its splice isoform, RAC1b. Our data provide strong evidence for dynamic EMT–MET transitions and qualify this cell line as a useful model with which to study EMP.

According to current revised Fukuoka guidelines, there is an indication for resection of BD-IPMN of the pancreas with "worrisome features", as there is a risk of malignant degeneration of up to 30%. This can be performed as a non-anatomical local excision in the absence of clinical, radiological and laboratory signs of malignancy.Robotic enucleation for benign tumours of the pancreas is a very good alternative to resecting procedures, especially those using open techniques. Introduction: 2–8% of all gastric cancer occurs at a younger age, also known as early-onset gastric cancer (EOGC). The aim of the present work was to use clinical registry data to classify and characterize the young cohort of patients with gastric cancer more precisely. Methods: German Cancer Registry Group of the Society of German Tumor Centers—Network for Care, Quality and Research in Oncology (ADT)was queried for patients with gastric cancer from 2000–2016. An approach that stratified relative distributions of histological subtypes of gastric adenocarcinoma according to age percentiles was used to define and characterize EOGC. Demographics, tumor characteristics, treatment and survival were analyzed. Results: A total of 46,110 patients were included. Comparison of different groups of age with incidences of histological subtypes showed that incidence of signet ring cell carcinoma (SRCC) increased with decreasing age and exceeded pooled incidences of diffuse and intestinal type tumors in the youngest 20% of patients. We selected this group with median age of 53 as EOGC. The proportion of female patients was lower in EOGC than that of elderly patients (43% versus 45%; p < 0.001). EOGC presented more advanced and undifferentiated tumors with G3/4 stages in 77% versus 62%, T3/4 stages in 51% versus 48%, nodal positive tumors in 57% versus 53% and metastasis in 35% versus 30% (p < 0.001) and received less curative treatment (42% versus 52%; p < 0.001). Survival of EOGC was significantly better (five-years survival: 44% versus 31% (p < 0.0001), with age as independent predictor of better survival (HR 0.61; p < 0.0001). Conclusion: With this population-based registry study we were able to objectively define a cohort of patients referred to as EOGC. Despite more aggressive/advanced tumors and less curative treatment, survival was significantly better compared to elderly patients, and age was identified as an independent predictor for better survival.

Approximately 30-40% of pancreatoduodenectomies for adenocarcinomas result in nonpancreatic periampullary adenocarcinoma as the final diagnosis. Depending on the origin, a distinction is made between four different carcinomas with histomorphological subtypes.Carcinoma location and subtype are of prognostic and therapeutic relevance; however, the preoperative differentiation is often incorrect despite modern diagnostics.Overview of the current literature on the classification and preoperative diagnostics of periampullary adenocarcinomas.A precise knowledge of the papillary anatomy is necessary for the correct classification of diagnostic findings. Current studies demonstrate diagnostically valuable information from the anamnesis, imaging and endoscopy.In ca. 70-80% of cases a correct diagnosis of the type of periampullary adenocarcinoma is possible on the basis of interdisciplinary diagnostics. Pancreatic carcinoma in the body and on the left side of the mesentericoportal axis is often only detected in late stages owing to unspecific or even missing clinical symptoms. In approximately 20% of the cases, there is already infiltration of the tumour into the surrounding arteries or veins. Despite locally advanced growth, 30% of patients do not have distant metastases and would potentially qualify for local resection. Arterial resections and vascular reconstruction are associated with an almost 9-fold increase in postoperative mortality compared with resections without vascular reconstruction. The Appleby procedure is a complex surgical technique originally developed for advanced gastric cancer. The technique has been further developed for patients with advanced pancreatic body and tail tumours with infiltration of the coeliac trunk (modified Appleby procedure). The advantage of the procedure is that technically, no reconstruction of the resected arteries is required. This is because a natural internal anastomosis in the pancreatic head between the A. mesenterica superior and the A. hepatica via branches of the A. gastroduodenalis is used to maintain liver perfusion and gastric blood flow. However, the surgical procedure is also associated with high morbidity and mortality, with comparably poor oncological results (R0 rates of approximately 60%). Therefore, the procedure was not recommended until a few years ago, and patients were considered inoperable. With developments in neoadjuvant therapy for pancreatic carcinoma, the Appleby procedure is being performed more frequently, with the goal of improving oncological outcomes in the context of multimodal treatment. With developments in robotics in visceral surgery, the previous limitations of minimally invasive pancreatic surgery can be overcome, and significantly more patients may benefit from the advantages of this minimally invasive surgery, such as faster convalescence. The use of robotic surgical techniques allows the extension of minimally invasive techniques into the field of complex vessel resection and reconstruction. In this video contribution, we describe a robot-assisted modified Appleby procedure using the Da Vinci Xi Surgical System in a patient with advanced pancreatic carcinoma of the pancreatic body, after neoadjuvant therapy.Das Pankreaskarzinom im Bereich des Korpus und linksseitig der mesenterikoportalen Achse wird wegen der unspezifischen oder gar fehlenden klinischen Symptome häufig erst in späten Stadien erkannt, wobei in etwa 20% der Fälle bereits eine Infiltration der umliegenden arteriellen oder venösen Gefäße vorliegt. Trotz eines lokal fortgeschrittenen Wachstums (T4, UICC-Stadium III) weisen 30% der Patienten keine Fernmetastasen auf und würden sich potenziell für eine lokale Resektion qualifizieren. Arterielle Resektionen mit Anastomosierung und Gefäßrekonstruktion sind einschränkend mit einer fast 9-fach erhöhten Mortalität des Eingriffs im Vergleich zu Resektionen ohne Gefäßrekonstruktion verbunden. Die Appleby-Operation stellt eine komplexe Operationstechnik dar, die ursprünglich für das fortgeschrittene Magenkarzinom entwickelt wurde. Die Adaptation der Technik wurde für Patienten mit fortgeschrittenen Pankreaskorpus- und Pankreasschwanztumoren mit einer Infiltration des Truncus coeliacus weiterentwickelt. Der Vorteil der Operation besteht darin, dass technisch keine Rekonstruktion der resezierten Arterien erfolgen muss, da man sich eine natürliche interne Anastomose im Pankreaskopf zwischen der A. mesenterica superior und der A. hepatica über Äste der A. gastroduodenalis zum Erhalt der Leberperfusion und der Magendurchblutung zu Nutze macht. Das Operationsverfahren ist jedoch ebenso mit einer hohen Morbidität und Mortalität verbunden mit vergleichbar schlechten onkologischen Ergebnissen (R0-Raten ca. 60%), sodass das Verfahren bis vor einigen Jahren nicht empfohlen wurde und die Patienten als inoperabel galten. Im Zuge der Entwicklung neoadjuvanter Therapieverfahren beim Pankreaskarzinom wird die Appleby-Operation wieder häufiger durchgeführt, da der Bedarf an lokal radikal resezierenden Operationsverfahren und Techniken im Kontext der multimodalen Behandlung gestiegen ist. Im Zuge der derzeitigen Etablierung der Robotik in der Viszeralchirurgie könnten bisherige Limitationen der minimalinvasiven Pankreaschirurgie überwunden werden und deutlich mehr Patienten die Vorteile der minimalinvasiven Chirurgie – wie eine schnellere Rekonvaleszenz – ermöglicht werden. Der Einsatz robotischer Operationsverfahren ermöglicht die Erweiterung minimalinvasiver Techniken in den Bereich der komplexen Gefäßresektion und Rekonstruktion. Im vorliegenden Videobeitrag beschreiben wir eine robotisch assistierte modifizierte Appleby-Operation mit dem Einsatz des Da Vinci Xi Surgical System bei einer Patientin mit einem fortgeschrittenen Pankreaskorpuskarzinom nach neoadjuvanter Therapie.

Chronic pancreatitis is a recurrent disease with repeating exacerbations of inflammation of the pancreatic gland - associated with belt-like back pain. Without treatment, recurrent chronic pancreatitis leads to development of opioid-dependent pain. The chronic pancreatitis leads to recurrent hospital stays for the affected patient and socioeconomic challenges. In progress it can lead to local complications of chronic pancreatitis, such as formation of pseudocysts, biliary duct obstruction, duodenal obstruction or portal hypertension. The aim of this article is a detailed description of the indication for surgical therapy in chronic pancreatitis. The underlying analysis was a systematic literature research and evaluation, the formulation of key questions according to the PICO system and the evaluation of indications and key statements and questions, as implemented in a three level Delphi process among the members of the pancreas research group and the indications for the surgery group of the German Society of General and Visceral Surgery (DGAV). Surgical resection of the inflammatory pancreatic head pseudotumour, after initial conservative therapy, is a highly efficient therapy for the control of pain and the avoidance of complications in chronic pancreatitis. For this purpose, well evaluated surgical strategies are available. Delay in surgical therapy can lead to chronic pain, kachexia and malnutrition and increase complications of surgical therapy.Die chronische Pankreatitis stellt eine über die Jahre rezidivierende Erkrankung mit wiederkehrenden Entzündungsschüben der Bauchspeicheldrüse und gürtelförmigen Oberbauchschmerzen dar. Unbehandelt führt die chronische Pankreatitis zu rezidivierenden Schmerzen, die letztendlich häufig eine Therapie mit Opiaten erfordern. Die chronische Pankreatitis führt zu rezidivierenden Krankenhausaufenthalten der Betroffenen sowie zu sozioökonomischen Nachteilen für die Patienten. Im Verlauf kann es zu lokalen Folgen der chronischen Pankreatitis kommen wie Pseudozystenbildung, Gallengangstenose, Duodenalobstruktion oder portaler Hypertension. Ziel dieses Artikels ist die Darstellung der Indikationen zur chirurgischen Therapie für die chronische Pankreatitis. Zugrunde liegend ist eine systematische Literaturrecherche und Bewertung, die Erstellung von Schlüsselfragen nach dem PICO-Prinzip und die Evaluation der Indikationen in einem 3-stufigen Delphi-Verfahren unter den Mitgliedern der Arbeitsgruppe Pankreas und der Arbeitsgruppe Indikationsqualität der Deutschen Gesellschaft für Allgemein- und Viszeralchirurgie. Die chirurgische Resektion des entzündlichen Pankreaskopftumors nach einem primär konservativen Therapieversuch stellt eine hocheffiziente Therapie zur Kontrolle der Schmerzen und zur Vermeidung von Komplikationen bei der chronischen Pankreatitis dar. Hierfür stehen gut evaluierte duodenumerhaltende (Operationen nach Frey, Bern, Beger) und duodenumresezierende Verfahren (pyloruserhaltende Pankreaskopfresektion, Whipple-Operation) sowie Drainageoperationen zur Verfügung. Protrahierung der chirurgischen Therapie bei Versagen eines initialen konservativen Therapieversuchs kann zur Chronifizierung von Schmerzen, Kachexie und Malnutrition des Patienten und zur Erhöhung der Komplikationen bei der chirurgischen Therapie führen.

Surgery for pancreatic cancer in Germany is increasing due to the climbing incidence of this cancer in the population. This review presents a summary of modern evidence-based indications for surgery in patients with pancreatic ductal adenocarcinoma (PDAC).The German Society for General and Visceral Surgery (DGAV) authorised a task force to define evidence based indications for surgery in patients with PDAC. A systematic literature search in Medline and Cochrane Library databases (1989 - 2019) was performed. Recommendations were summarised on the basis of the most relevant and recent guidelines and clinical studies and then voted by members of the Working Group on Hepato-Biliary and Pancreatic Diseases (CALGP) in a Delphi procedure.Indications for surgery in patients with PDAC should be set by experienced pancreatic surgeons within a tumour board. Decisions should consider the guidelines as well as the individual patient characteristics. Large vessel infiltration, metastatic disease and severe comorbidities are the most common contraindications for surgery. Borderline-resectable, primary resectable oligometastatic and secondary resectable PDAC should be preferably managed at high-volume centres as a part of clinical studies. Centralisation of pancreatic surgery reduces mortality and improves survival. Palliative bypass surgery as well as staging laparoscopy are still indicated in a large proportion of patients with PDAC.Irrespective of the recent development of multimodal therapeutic concepts, surgical resection remains the sole chance of long-term cure for patients with PDAC. Due to the significant proportion of patients in advanced stages of the disease, palliative surgery still plays an important role in the complex management of this cancer.Die Zahl der Operationen für Pankreaskarzinome in Deutschland nimmt kontinuierlich zu. Das entspricht der steigenden Inzidenz dieser Erkrankung in der Bevölkerung. Diese Übersichtsarbeit fasst die evidenzbasierten aktuellen Operationsindikationen bei Patienten mit Pankreaskarzinom zusammen.Als DGAV-Initiative wurde eine Arbeitsgruppe von Pankreasexperten eingerichtet. Diese formulierte konkrete Schlüsselfragen nach dem PICO-Schema, führte eine systematische Literatursuche in Medline und Cochrane Library (1989 – 2019) zu Studien und Leitlinien mit Aussagen zu Operationsindikationen bei Pankreaskarzinom durch und erstellte daraus resultierende evidenzbasierte Empfehlungen. Diese wurden innerhalb der CALGP im Rahmen eines Delphi-Verfahrens abgestimmt.Die Operationsindikation bei Pankreaskarzinom soll im Tumorboard von erfahrenen Pankreaschirurgen leitliniengerecht und unter Berücksichtigung der individuellen Besonderheiten der Patienten gestellt werden. Fortgeschrittene Infiltration der großen Viszeralgefäße, multiple Fernmetastasen und schwere Komorbiditäten, die einen Eingriff in Vollnarkose verbieten, stellen die häufigsten Kontraindikationen zur Operation dar. Die Therapie von Borderline-resektablen und primär resektablen oligometastatischen Patienten sowie solchen mit sekundärer Resektabilität nach neoadjuvanter Behandlung soll bevorzugt an Zentren und im Rahmen von Studien erfolgen. Die Behandlung an Pankreaszentren reduziert die Mortalität und verbessert das Überleben. Die palliative Bypasschirurgie kann bei endoskopisch nicht therapierbaren Gallenwegs- und Duodenalobstruktionen indiziert sein. Bei diagnostischen Schwierigkeiten kann die Staging-Laparoskopie mit histologischer Sicherung eingesetzt werden.Unabhängig von der Entwicklung erfolgversprechender multimodaler Behandlungskonzepte bleibt die chirurgische Resektion weiterhin der einzig kurative Therapieeinsatz. Wegen des hohen Anteils von primär fortgeschrittenen und metastasierten Pankreaskarzinomen spielt auch die palliative Chirurgie weiterhin eine wichtige Rolle in der komplexen Versorgung dieses Patientenkollektivs.

15 to 20% of patients with acute pancreatitis develop necrosis of the pancreatic parenchyma or extrapancreatic tissue. The disease is associated with a mortality rate of up to 20%. The mainstays of treatment consist of intensive medical care and surgical and interventional therapy.A systematic literature search focused on indications for surgical and interventional therapy of necrotising pancreatitis. 85 articles were analysed for this review. By using the Delphi method, the results were presented to the quality committee for pancreas diseases of the German Society for General and Visceral Surgery and to expert pancreatologists in an interactive conference using plenary voting during the visceral medicine congress 2019 in Wiesbaden. For the finalised recommendations, an agreement of 84% of participants was achieved.Documented or clinical suspicion of infected, necrotising pancreatitis are indications for surgical and interventional therapy (recommendation grade: strong; evidence grade; low). Sterile necrosis is a less common indication for intervention due to late complications or persistent severe pancreatitis. Invasive interventions should be delayed when possible until four weeks after onset of pancreatitis. Optimal treatment strategy consists of a "step-up approach" (evidence grade: high; recommendation grade: strong). The first step is catheter drainage, followed, if necessary, by minimally invasive surgical or interventional necrosectomy. If minimally invasive techniques do not result in clinical improvement, open necrosectomy is necessary. 35 to 50% of patients are successfully treated with drainage alone. Indications for emergency intervention are bowel perforation, bowel ischemia and bleeding. Surgical decompression of abdominal compartment syndrome is indicated if the patient is refractory to medical treatment and percutaneous drainage. Abscesses and symptomatic pseudocysts are indications for interventional drainage. Early cholecystectomy during index admission is recommended for patients with mild biliary pancreatitis. Cholecystectomy should be delayed after severe, biliary pancreatitis.The recommendations for surgical an interventional therapy of necrotising pancreatitis address the basis of current indications in literature. They should serve in daily practice as a reference standard for decision making in multidisciplinary teams.Bei 15 – 20% der Patienten mit akuter Pankreatitis entwickelt sich eine nekrotisierende Form. Die Mortalität beträgt bis zu 20%. Entscheidende Säulen der Behandlung sind die intensivmedizinische Therapie sowie chirurgische und interventionelle Maßnahmen.Diese Übersichtsarbeit zur Indikationsstellung von chirurgischen und interventionellen Maßnahmen bei der nekrotisierenden Pankreatitis wurde auf der Basis einer systematischen Literaturrecherche verfasst. In der Analyse wurden 85 Artikel ausgewertet. Die Ergebnisse wurden im Delphi-Befragungsverfahren der Qualitätskommission der Deutschen Gesellschaft für Allgemein- und Viszeralchirurgie (DGAV) und Experten auf einer Sitzung des Viszeralmedizin-Kongresses in Wiesbaden 2019 vorgestellt. Bei der abschließenden Formulierung der Empfehlungen wurde ein starker Konsensus von 84% bei den Befragten erreicht.Bei Nachweis oder Verdacht auf eine Infektion der Pankreasnekrosen besteht eine Indikation zur chirurgischen und interventionellen Behandlung (Empfehlungsgrad: stark; Evidenzstärke: schwach). In Ausnahmefällen stellen auch sterile Nekrosen eine Indikation zur Intervention dar. Wenn es der klinische Zustand erlaubt, sollte die Intervention bis in die 4. Krankheitswoche geschoben werden. Als Therapiestrategie wird mit hohem Evidenz- und Empfehlungsgrad der „step-up approach“ empfohlen. Diese Strategie der Therapieeskalation beinhaltet als 1. Maßnahme eine Drainagebehandlung und bei Erfolglosigkeit die minimalinvasive chirurgische oder endoskopische Nekrosektomie. Sind die minimalinvasiven Techniken nicht effektiv, ist die offene Nekrosektomie indiziert. Nach primärer Drainagebehandlung kann bei 35 – 50% der Patienten die 2. Therapiestufe der Nekrosektomie vermieden werden. Indikationen zur Notfalloperation sind Organperforation, Darmischämie, interventionell nicht stillbare Blutungen und das abdominelle Kompartmentsyndrom bei erfolgloser konservativer Therapie. Spätabszesse und symptomatische Pseudozysten sind Indikationen für eine interventionelle Drainagebehandlung. Bei der biliären Pankreatitis ist die Cholezystektomie indiziert, bei der milden Form frühelektiv und bei schwerem Verlauf frühestens nach 6 – 8 Wochen.Die vorliegenden Empfehlungen enthalten Kriterien für die Indikationsstellung zur chirurgischen und interventionellen Behandlung der akuten Pankreatitis. Sie sollen als Referenzstandards für die Entscheidungsfindung in einem multidisziplinären Team dienen.

The use of robots in minimally invasive surgery has become increasingly common in recent years. Robot-assisted pancreatoduodenectomy is more frequent than the laparoscopic procedure especially due to the greater flexibility of instruments and therefore better handling and better angulation. Furthermore, there are benefits from enhanced 3D visibility, software-based tremor control and reduction in the physical exertion of the surgeon.This review delivers a point-by-point approach to the setup of a robotic pancreatic programme and a detailed approach to robot-assisted pancreatoduodenectomy.In our standardised SOP approach, we use 5 trocars, 4 robotic trocars and one assist trocar. We prefer the position of the robot ports to be in a straight horizontal line with a distance of 20 cm away from the operational field. The operation is dissected in 11 standardised procedural steps, namely 1. Access to the pancreas and visualisation, 2. extended Kocher manoeuvre, 3. lower rim and mesenterico-portal axis, 4. upper rim and hepato-duodenal ligament, 5. dissection of the pancreatic neck, 6. mesenteric root and pars IV duodeni, 7. mesopancreas, 8. pancreatic anastomosis reconstruction, 9. bilio-enteric anastomosis, 10. dudenojejunal anastomosis, 11. drainage and closure. The set up of the pancreas program and the structured approach to complex pancreatic resections are elucidated.In summary, this review describes the approach to robotic pancreatic surgery in a high-volume pancreas centre at a structural and procedural level, in order to support establishment of such programs at other locations.EINFüHRUNG: Der Einsatz von Robotern in der minimalinvasiven Chirurgie hat in den letzten Jahren immer weiter zugenommen. Die roboterunterstützte Pankreaschirurgie hat gegenüber der laparoskopischen Pankreaschirurgie den Vorteil einer besseren Handhabung und verbesserten Angulation durch die größere Flexibilität der Instrumente. Darüber hinaus besteht durch die 3-D-Technik eine verbesserte Sicht und durch eine softwarebasierte Tremorkontrolle eine exakte Instrumentenführung. Die körperliche Belastung des Chirurgen durch die sitzende Position ist bei langen Eingriffen geringer.Dieser Artikel liefert einen Ansatz für die Einrichtung eines Roboter-Pankreas-Programms und eine detaillierte Anleitung im Sinne von SOPs für die robotergestützte Pankreaslinksresektion und robotergestützte Pankreatoduodenektomie.In unserem standardisierten SOP-Ansatz verwenden wir sowohl bei der Pankreaslinksresektion als auch bei der Pankreatduodenektomie jeweils 5 Trokare – 4 Da-Vinci-Trokare und einen laparoskopischen Hilfstrokar. Wir bevorzugen die Position der Robotertrokare auf einer geraden horizontalen Linie mit einem Abstand von etwa 20 cm vom Operationsfeld. Die Operation wird in standardisierten Arbeitsschritten durchgeführt. Bei der Pankreaslinksresektion sind dies: Zugang und Visualisierung des Pankreas, Mobilisierung der linken Dickdarmflexur und des Pankreasunterrandes, Präparation der V. lienalis und der mesenterikoportalen Achse, Präparation des Pankreasoberrandes mit Lymphadenektomie und Darstellung der A. lienalis, Durchtrennung des Pankreas, retrograde Präparation des Pankreas und der Milz (bei onkologischen Eingriffen). Bergung, Einlage von Drainagen und Wundverschluss. Bei der Pankreatoduodenektomie sind es folgende Schritte: Zugang und Visualisierung des Pankreas, erweitertes Kocher-Manöver, Darstellung der Unterkante und der mesenterikoportalen Achse, Darstellung des Pankreasoberrandes und des Lig. hepatoduodenale, Präparation des Pankreashalses, Darstellung der Mesenterialwurzel und Pars IV duodeni, Präparation des Mesopankreas, Durchführung der Pankreasanastomose und der biliodigestiven Anastomose, Naht der dudenojejunalen Anastomose, Einlage von Drainagen und schichtweiser Wundverschluss. Der graduelle Aufbau unseres Roboter-Pankreas-Programms und der strukturierte Ansatz für komplexe Pankreasresektionen werden erläutert.Zusammenfassend beschreiben wir den Ansatz der robotischen Bauchspeicheldrüsenchirurgie in einem minimalinvasiven Exzellenzzentrum und Bauchspeicheldrüsenzentrum auf struktureller und verfahrenstechnischer Ebene, um die Einrichtung derartiger Programme an anderen Standorten zu unterstützen.

Robot-assisted surgery is a promising technique for overcoming the limitations of laparoscopic surgery, especially for complex and advanced surgical procedures. We now describe the implementation of our robotic upper GI and HPB surgery program in our centre of excellence for minimally invasive surgery and the results of our first 100 surgical procedures.Robot-assisted surgery was performed using the Da VinciThe first step of establishing our robotic surgical program included 26 procedures. Here, conversion rate, morbidity and mortality were 0%. In the second step of implementation, 23 procedures were performed. Conversion rate, morbidity and mortality were 28, 8 and 0% respectively. The last step included 51 advanced and highly complex procedures. These procedures had a morbidity of 41%, a mortality of 4% and a conversion rate of 43%.Our stepwise approach enables safe implementation of a robotic surgical program for upper GI and HPB surgery with comparable morbidity and mortality even for highly complex procedures. However, highly complex procedures in the learning curve required a high conversion rate.Die robotisch assistierte Chirurgie ist eine vielversprechende Technik zur Überwindung der Grenzen der laparoskopischen Chirurgie – insbesondere bei komplexen chirurgischen Eingriffen in der Oberbauchchirurgie. Im Folgenden beschreiben wir die Etablierung komplexer robotisch assistierter Oberbauchchirurgie in unserem Zentrum für minimalinvasive Chirurgie und präsentieren die Ergebnisse aus den ersten 100 Eingriffen.Die robotisch assistierte Chirurgie wurde mit dem Da VinciDer 1. Schritt der Etablierung unseres Roboterprogramms umfasste 26 Eingriffe. Hier betrugen die Konversionsrate, die Morbidität und die Mortalität jeweils 0%. Im 2. Schritt wurden 23 Eingriffe durchgeführt. Die Konversionsrate, Morbidität und Mortalität lagen bei 28, 8 und 0%. Der letzte Schritt umfasste 51 hochkomplexe operative Verfahren. Diese Verfahren hatten eine Konversionsrate 43%, eine Morbidität von 41% und eine Mortalität von 4%.Unser Stepwise Approach ermöglicht eine sichere Etablierung robotisch assistierter Oberbauchchirurgie mit vergleichbaren Morbiditäts- und Mortalitätsraten – auch bei hochkomplexen operativen Eingriffen. Hochkomplexe Eingriffe erforderten jedoch vergleichsweise häufig eine Konversion, was durch die geringen Erfahrungswerte und durch die Lernkurve beeinflusst sein könnte.

A steady improvement in modern imaging as well as increasing age in society have led to an increasing number of cystic pancreatic tumours being detected. Pancreatic cysts are a clinically challenging entity because they span a broad biological spectrum and their differentiation is often difficult, especially in small tumours. Therefore, they require a differentiated indication for indication of surgery. To determine recommendations for the surgical indication in cystic tumours of the pancreas, a quality committee for pancreatic diseases of the German Society for General and Visceral Surgery performed a systematic literature search and created this review. Based on the current evidence, signs of malignancy and high-risk criteria (icterus due to cystic pancreatic duct obstruction in the bile duct, enhancing mural nodules ≥ 5 mm or solid components in the cyst or pancreatic duct ≥ 10 mm), as well as symptoms, are a surgical indication, independently of the cyst entity (except pseudocysts). If the entity of the pancreatic cyst is detectable by diagnostic imaging, all main duct IPMN and IPMN of the mixed type, all MCN 4 cm and all SPN should be resected. SCN and branch-duct IPMN without worrisome features do not constitute an indication for surgery. The indication of operation in branch-duct IPMN with relative risk criteria and MCN 4 cm is the subject of current discussions and should be individualised. By defining indication recommendations, the present work aims to improve the indication quality in cystic pancreatic tumours. However, the surgical indication should always be individualised, taking into account age, comorbidities and the patient's wishes.Eine stetige Verbesserung der modernen Bildgebung sowie ein steigendes Alter in der Gesellschaft haben dazu geführt, dass zunehmend häufiger zystische Pankreastumoren detektiert werden. Zystische Pankreasraumforderungen stellen eine klinisch herausfordernde Entität dar, da sie ein breites biologisches Spektrum umfassen und ihre Differenzierung untereinander vor allem bei kleinen Tumoren häufig schwierig ist. Sie bedingen daher eine differenzierte Operationsindikationsstellung. Zur Bestimmung von Empfehlungen zur Operationsindikation bei zystischen Tumoren des Pankreas wurde von einer Qualitätskommission für Pankreaserkrankungen der Deutschen Gesellschaft für Allgemein- und Viszeralchirurgie eine systematische Literaturrecherche durchgeführt und daraus resultierend folgende Übersichtsarbeit erstellt. Auf Grundlage der aktuellen Evidenz besteht bei Vorliegen von Malignitätszeichen oder Hochrisikokriterien (Ikterus bei Obstruktion des Ductus choledochus durch die zystische Pankreasläsion, kontrastmittelaufnehmende intramurale Knoten ≥ 5 mm oder solide Komponenten in der Zyste oder Pankreashauptgang ≥ 10 mm) sowie bei Symptomen unabhängig von der Zystenentität (ausgenommen Pankreaspseudozysten) eine Operationsindikation. Ist die Entität der Pankreaszyste mittels bildgebender Diagnostik eruierbar, so sollten alle intraduktalen papillär-muzinösen Neoplasien (IPMN) vom Hauptgang und vom Mischtyp, alle muzinös-zystischen Neoplasien (MCN) 4 cm sowie alle soliden pseudopapillären Neoplasien (SPN) reseziert werden. Serös-zystische Neoplasien (SCN) und Seitengang-IPMN ohne Risikokriterien stellen primär keine Operationsindikation dar. Die Operationsindikationsstellung bei Seitengang-IPMN mit relativen Risikokriterien und MCN 4 cm ist Gegenstand aktueller Diskussionen und sollte individuell erfolgen. Die vorliegende Arbeit soll durch ein Herausarbeiten von Indikationsempfehlungen helfen, die Indikationsqualität bei zystischen Pankreastumoren zu verbessern. Die letztendliche Operationsindikationsstellung sollte jedoch immer individualisiert unter Berücksichtigung von Alter, Komorbiditäten und Patientenwunsch erfolgen.

Simple Summary Less than 1% of all pancreatic malignancies are acinar cell carcinomas. Based on data from the German Cancer Registry Group, we performed a comparative analysis of characteristics and prognostic factors of pancreatic acinar cell carcinoma and the most common type of pancreatic cancer—pancreatic ductal adenocarcinoma. Compared to pancreatic ductal adenocarcinoma, patients with pancreatic acinar cell carcinoma were younger at the time of diagnosis and the percentage of males was higher. The prognosis of patients with pancreatic acinar carcinoma was better than that of patients with pancreatic ductal adenocarcinoma. Surgical resection was the strongest positive prognostic factor for pancreatic acinar cell carcinoma. The study shows that pancreatic acinar cell carcinoma has features distinct from pancreatic ductal adenocarcinoma. Radical resection should be advocated, whenever feasible. Abstract Background: Pancreatic acinar cell carcinoma (PACC) is a distinct type of pancreatic cancer with low prevalence. We aimed to analyze prognostic factors and survival outcome for PACC in comparison to pancreatic ductal adenocarcinoma (PDAC), based on data from the German Cancer Registry Group. Methods: Patients with PACC and PDAC were extracted from pooled data of the German clinical cancer registries (years 2000 to 2019). The distribution of demographic parameters, tumor stage and therapy modes were compared between PACC and PDAC. The Kaplan–Meier method and Cox regression analysis were used to delineate prognostic factors for PACC. Propensity score matching was used to compare survival between PACC and PDAC. Results: There were 233 (0.44%) patients with PACC out of 52,518 patients with pancreatic malignancy. Compared to PDAC, patients with PACC were younger (median age 66 versus 70, respectively, p < 0.001) and the percentage of males was higher (66.1% versus 53.3%, respectively, p < 0.001). More patients were resected with PACC than with PDAC (56.2% versus 38.9%, respectively, p < 0.001). The estimated overall median survival in PACC was 22 months (95% confidence interval 15 to 27), compared to 12 months (95% confidence interval 10 to 13) in the matched PDAC cohort (p < 0.001). Surgical resection was the strongest positive prognostic factor for PACC after adjusting for sex, age, and distant metastases (hazard ratio 0.34, 95% confidence interval 0.22 to 0.51, p < 0.001). There was no survival benefit for adjuvant therapy in PACC. Conclusions: PACC has overall better prognosis than PDAC. Surgical resection is the best therapeutic strategy for PACC and should be advocated even in advanced tumor stages.

Pancreatic ductal adenocarcinoma (PDAC) is characterized by aggressive biology and poor prognosis even after resection. Long-term survival is very rare and cannot be reliably predicted. Experimental data suggest an important role of epithelial-mesenchymal transition (EMT) in invasion and metastasis of PDAC. Tumor budding is regarded as the morphological correlate of local invasion and cancer cell dissemination. The aim of this study was to evaluate the biological and prognostic implications of EMT and tumor budding in PDAC of the pancreatic head.Patients were identified from a prospectively maintained database, and baseline, operative, histopathological, and follow-up data were extracted. Serial tissue slices stained for Pan-Cytokeratin served for analysis of tumor budding, and E-Cadherin, Beta-Catenin, and Vimentin staining for analysis of EMT. Baseline, operative, standard pathology, and immunohistochemical parameters were evaluated for prediction of long-term survival (≥ 30 months) in uni- and multivariate analysis.Intra- and intertumoral patterns of EMT marker expression and tumor budding provide evidence of partial EMT induction at the tumor-host interface. Lymph node ratio and E-Cadherin expression in tumor buds were independent predictors of long-term survival in multivariate analysis.Detailed immunohistochemical assessment confirms a relationship between EMT and tumor budding at the tumor-host interface. A small group of patients with favorable prognosis can be identified by combined assessment of lymph node ratio and EMT in tumor buds.

Pancreatic ductal adenocarcinoma (PDAC) is the fourth most common cause of death from cancer. Its 5-year survival rate is less than 5%. This poor prognosis is mostly due to the cancer’s early invasion and metastasis formation, leading to an initial diagnosis at an advanced incurable stage in the majority of patients. The only potentially curative treatment is radical surgical resection. The effect of current chemotherapeutics or radiotherapy is limited. Novel therapeutic strategies are therefore much needed. One of the hallmarks of PDAC is its abundant desmoplastic (stromal) reaction. The Hedgehog (Hh) signaling pathway is critical for embryologic development of the pancreas. Aberrant Hh signaling promotes pancreatic carcinogenesis, the maintenance of the tumor microenvironment and stromal growth. The canonical Hh-pathway in the tumor stroma has been targeted widely but has not yet lead to hopeful clinical results. Targeting both the tumor and its surrounding stroma through Hh pathway inhibition by also targeting non-canonical pathways as apparent in the tumor cell may therefore be a novel treatment strategy for PDAC. Background Pancreatoduodenectomy is one of the most complex abdominal operations, usually performed for tumors of the periampullary region and chronic pancreatitis. Leakage of pancreatic juice from the pancreatoenteric anastomosis, called postoperative pancreatic fistula, is the most prominent postoperative complication. Retrospective studies show a significant reduction of fistula rates with pancreatogastrostomy as compared to pancreatojejunostomy, the most frequently employed method of pancreatoenterostomy. Most single-center prospective trials, however, have not validated this finding. A large multicenter trial is needed for clarification. Methods/design RECOPANC is a prospective, randomized, controlled multicenter trial with two treatment arms, pancreatogastrostomy versus pancreatojejunostomy. The trial hypothesis is that postoperative pancreatic fistula rate is lower after pancreatogastrostomy when compared to pancreatojejunostomy. Fourteen academic centers for pancreatic surgery will participate to allocate 360 patients to the trial. The duration of the entire trial is four years including prearrangement and analyses. Discussion Postoperative pancreatic fistula is the main reason for clinically important postoperative morbidity after pancreatoduodenectomy. The primary goal of the chosen reconstruction technique for pancreatoenteric anastomosis is to minimize postoperative fistula rate. A randomized trial performed at multiple high-volume centers for pancreatic surgery is the best opportunity to investigate one of the most crucial issues in pancreatic surgery. Background Ampullary cancer is a relatively rare form of cancer and usually treated by pancreatoduodenectomy, followed by adjuvant therapy. The intestinal subtype is associated with markedly improved prognosis after resection. At present, only few cell lines are available for in vitro studies of ampullary cancer and they have not been collectively characterized. Methods We characterize five ampullary cancer cell lines by subtype maker expression, epithelial-mesenchymal transition (EMT) features, growth and invasion, drug sensitivity and response to cancer-associated fibroblast conditioned medium (CAF-CM). Results On the basis of EMT features, subtype marker expression, growth, invasion and drug sensitivity three types of cell lines could be distinguished: mesenchymal-like, pancreatobiliary-like and intestinal-like. Heterogeneous effects from the cell lines in response to CAF-CM, such as different growth rates, induction of EMT markers as well as suppression of intestinal differentiation markers were observed. In addition, proteomic analysis showed a clear difference in intestinal-like cell line from other cell lines. Conclusion Most of the available AMPAC cell lines seem to reflect a poorly differentiated pancreatobiliary or mesenchymal-like phenotype, which is consistent to their origin. We suggest that the most appropriate cell line model for intestinal-like AMPAC is the SNU869, while others seem to reflect aggressive AMPAC subtypes.
