Introduction Determining the length of the root canal is one of the key factors that can affect the outcome of endodontic therapy. Incorrectly determined working length results in underfilled or overfilled canal. The aim of this work was to verify the effectiveness of cone-beam computerized tomography (CBCT) and apex locators in determining working length of the root canal. Material and method Endodontic procedure was performed according to established endodontic protocol on 98 teeth. Teeth were divided into two groups: single-rooted and multi-rooted teeth. In both groups, working length was measured with an apex locator (Woodpex III) and CBCT (Planmeca Viso). In multi-rooted teeth, the canals were classified according to their morphological position into the groups Lingvomesial (LM), Buccomesial (BM), Bucodistal (BD), Palatal (P) and Distal (D) canals. Root canal length was determined by a dentist using apex locator at the University of Banja Luka while CBCT analysis was performed by a radiology engineer at the 3DENT radiology center in Banja Luka. Results CBCT odontometry had small deviations from apex locator odontometry. The mean value of the absolute difference between CBCT measurement and apex locator measurement was the smallest in the group of linguomesial canals (0.284 mm), and the largest in the group of distal canals of the lower molars (1.939 mm). Based on the results of the Mann-Whitney U test, it was concluded that there was no statistically significant difference (p > 0.05) in measured length of the root canal between the two methods in all groups of roots of multi-rooted teeth as well as in the group of single-rooted teeth. Conclusion CBCT odontometry is as reliable as apex locator odontometry.

Introduction Exposure to lead (Pb) can have harmful effects on the general state of the body, including oral health. The aim of this study was to examine the prevalence of changes in gingiva and periodontal tissue in adult rats intoxicated with lead acetate by histological analysis. Material and methods The study was conducted on 42 Wistar rats. Intoxication of rats with lead acetate was carried out through drinking water ad libitum. The first group (A1) consisted of 16 rats that received lead-acetate in water at a concentration (1500 ppm) for 14 days, and the second group (A2) consisted of 16 rats that received lead-acetate in water at the same concentration for 30 days. Results Gingivitis and periodontitis in the first group of rats (A1) were observed in 41.7% of cases, and in the second group (A2) in 84% of cases. A statistically significant difference was found between the investigated groups (ch2 = 6.955; p < 0.018). Conclusion Since changes in the gingiva and periodontium have been observed in rats intoxicated with lead for a long period of time, it is necessary to propose a preventive program for patients exposed to lead, which includes dental procedures such as oral hygiene training and regular visits to the dentist.

Introduction Diabetes mellitus (DM) is a state of chronic hyperglycemia that is a predisposing factor to caries, gingivitis, inflammation of periodontium, oral candidiasis, xerostomia and many other diseases of the oral cavity. Inflammation of the supporting tissue of the tooth is a chronic disease that destroys the supporting structure of the tooth, i.e. periodontal ligament and alveolar bone. The aim of this study was to examine using histological analysis the presence of periodontitis and gingivitis in rats with experimentally induced DM that were sacrificed after 14 and 30 days. Material and methods The research was conducted on 42 Wistar rats. DM in experimental animals was induced by the use of Alloxan intraperitoneally. The first group (Exp_14) consisted of 16 rats in which DM was induced and sacrificed after 14 days, the second group (Exp_30) consisted of 16 rats in which DM was induced and they were sacrificed after 30 days, while the control consisted of 10 healthy rats. Results Periodontitis and gingivitis in the first group of rats (Exp_14) were determined in 54.5% of cases, while in the second group (Exp_30) in 88% of cases. In the control group no case of periodontitis and gingivitis was recorded. A highly statistically significant difference was found between the examined groups (Chi-square = 14.685; p < 0.001). Conclusion In the group of rats with experimentally induced DM that were sacrificed after 30 days, a significantly higher incidence of periodontitis and gingivitis was found compared to the group of rats that were sacrificed after 14 days.

Introduction In addition to good biological properties, biomaterials should also possess appropriate physical properties in order to provide stability and longevity at the place of application. The aim of this work was to evaluate physical properties of an experimental nanostructured material based on calcium aluminate (CAL) and calcium silicate (CS). Material and method The research used nanostructured calcium aluminate synthesized by the hydrothermal solgel method from the individual components of calcium aluminate (CaO·Al2 O3 ), calcite (CaCO3 ) and barium sulfate (BaSO4 ) as an X-ray contrast agent and calcium silicate (CS). The prepared material was placed in plastic molds with a diameter of 5 ± 0.1 mm and a height of 2 ± 0.1 mm. After setting time, the materials were left in an incubator at 37°C for 24 hours, and then they were removed from the mold and absorption and solubility of the materials was calculated. MTA (Angelus Londrina, Brazil) was used as a control material. Results The lowest material solubility was recorded with MTA (0.255 mg/mm3 ), followed by calcium silicate (0.267 mg/mm3 ), and the highest with calcium aluminate (0.725 mg/mm3 ). The difference was statistically significant between calcium aluminate and MTA (p = 0.001901) and between calcium aluminate and calcium silicate (p = 0.002550). After 28 days in deionized water, the lowest water sorption was recorded with MTA (0.347 mg/mm3 ), followed by calcium silicate (0.357 mg/mm3 ), and the highest water sorption was measured with calcium aluminate (0.474 mg/mm3 ). Statistically significant differences were observed between calcium aluminate and MTA (p = 0.000283) and between calcium aluminate and calcium silicate (p = 0.001576). Conclusion Material solubility and water absorption of calcium aluminate-based nanostructured material was significantly higher compared to calcium silicate (CS) and MTA.

Introduction Due to its simplicity, sliding mechanism is very often used in clinical practice for post-extraction space closure, however, the efficiency of this method may be reduced due to friction and changes in the properties of the materials used in this method. The most commonly used methods of sliding mechanics are nickel titanium (NiTi) closed coil spring and elastic chain. The aim of this study was to analyze force degradation in the application of nickel titanium closed coil springs and elastic chains during post-extraction space closure within treatment with fixed orthodontic appliances. Material and Methods The total sample in this study consisted of 78 post-extraction spaces in patients who were indicated for extraction of first premolars and treatment with fixed orthodontic appliances. Nickel titanium closed coil springs and elastic chains were used to close the post-extraction spaces. Post-extraction spaces were monitored for 6 months with follow up examinations every 4 weeks. Measurements of initial force, at the beginning of mechanism activation and residual force in the observation period were performed during control examinations. Results Results of this research showed that with both methods of sliding mechanism there was a significant decrease in the observation period. When using NiTi closed coil springs, the value of average initial force at control examination was between 189.00-210.25 g, while residual was in the range of 117.56-133.50 g that NiTi closed coil springs kept an average 61.57% of initial force. The average initial force on the control examinations for elastic chains was between 184.5-205.38 g, while residual force was in range of 100.39-113.00 g, that elastic chains retained an average 53.41% of initial force. Conclusion There was a significant force degradation between inspections, when sliding mechanics were applied. The loss of force between activation phases of NiTi closed coil springs was smaller compared to the forces produced by elastic chains.

Introduction: When planning orthodontic treatment, it is sometimes necessary to apply the extraction of individual teeth, in order to achieve proper occlusion and an acceptable aesthetic appearance. Clinicians give the greatest advantage to elastic chains as a method for post-extraction space closure during treatment with fixed orthodontic appliances. When closing post-extraction spaces, it is necessary to measure them in order to adjust the treatment plan. One of the most acceptable methods is photogrammetry. The aim of this study was to analyze, using a photogrammetric method, the efficacy of post-extraction space closure with elastic chains during orthodontic treatment with fixed orthodontic appliances. Material and Methods: The total sample in study consisted of 38 post-extraction spaces in 19 patients who were indicated for first premolars extraction and use of fixed orthodontic appliances in order to achieve treatment goal. Elastic chains were used to close post-extraction spaces. Post-extraction sites were monitored for 6 months with follow-up examinations every 4 weeks. Mechanisms were activated at controlled examinations, and measurements of the width of post-extraction spaces were performed by photogrammetry and digital caliper. Results: The results showed that average reduction of post-extraction space width was 1.00 mm per month by photogrammetric measurement, while the average values of measurements with a digital caliper were 1.02 mm. The average values were measured in six time intervals and a statistically significant change in average intervals of post-extraction spaces during 6 months was determined. Conclusion The elastic chain has proven to be a very effective mechanism for closing post-extraction space, and photogrammetry as a simple and precise method for monitoring results of treatment. Since the difference in relation to measurements with a digital caliper is minimal, photogrammetry can be routinely applied in everyday practice.

Introduction/Objective. After an orthodontic brackets debonding procedure it is necessary to remove any residual adhesive from the tooth surface, as this is a common cause of enamel damage. The aim of this study is to evaluate the enamel surface after the application of six different methods of adhesive removal following brackets debonding, as well as to compare the duration of these procedures. Methods. For the purpose of this study, 245 human premolars were extracted as part of the orthodontic treatment. Metal brackets were bonded to 210 human premolars with the Aspire adhesive system. After the debonding of brackets, the samples were divided into six groups according to the adhesive removal method applied: tapered fissure tungsten carbide bur, round tungsten carbide bur, composite bur, abrasive disc, adhesive removing pliers, and ultrasonic scaler. Out of 245 premolars, 35 served as a control group. The duration of adhesive removal was recorded. Enamel damages were estimated according to the enamel surface index on the scanning electron microscopy images. Results. Maximum preservation of the enamel surface was accomplished by using a composite bur (1.08). The application of abrasive disc was significantly less time-consuming in comparison to the application of a composite bur (p < 0.01) and an ultrasonic scaler (p < 0.01). Conclusion. The most harmful for the enamel surface was the use of an ultrasonic scaler as well as a round tungsten carbide bur. Adhesive removal done by an abrasive disc thus proved one of the least damaging and the least time-consuming methods.

Introduction/Objective. Rapid developments in information technologies lead to the wider use of digital representations of dental study models in orthodontics. Most popular way of digitizing the models is to use a 3D scanner and then perform measurements on 3D models, which requires additional and expensive hardware and software resources. In this paper we present an alternative approach based on the use of photogrammetry in the newly developed OrthoPhoto4D software that calculates and corrects perspective distortion errors. Methods. We measured individual tooth width for 24 teeth, 12 two-teeth segments as well as inter-molar and inter-canine distances on 50 models. Measurements are performed in OrthoPhoto4D software that uses four photographs of each model for measurements, uses QR codes for automation, calculates the camera position and corrects perspective distortion-caused errors in measurements. Obtained measurements are compared to ones obtained from models generated by structured light 3D scanner. Results. Statistical analysis strongly indicates that there is no significant difference between the two methods. The recorded differences also have no clinical impact as they have mean values of 0.2 mm for individual tooth widths, approximately 0.2 mm for two teeth segments, and under 0.3 mm for both intercanine and inter-molar distances. All recorded differences fall within the expected measurement error. Conclusion. We concluded that the described photogrammetry measurements performed in OrthoPhoto4D can be used in diagnosis and therapy planning.

Introduction In everyday clinical practice, we often encounter a lack of space for placing all the teeth present into dental arch; therefore it is often recommended to extract teeth within orthodontic treatment. In clinical practice, the most commonly used methods of closing space after are elastic chains and NiTi closed spiral springs. The aim of this paper was to compare postextraction space closure speed using two different sliding mechanisms, NiTi closed coil springs and elastic chains within treatment with fixed orthodontic appliances. Material and Method the total sample in this study consisted of 46 postexstraction spaces in 23 patients indicating the extraction of first premolars using treatment with fixed orthodontic appliances. Two sliding mechanisms, NiTi closed coil spring and elastic chains were applied to postextraction space closure. Postextraction spaces were monitored for 4 months with appointments every 4 weeks. During appointments mechanisms were activated and digital caliper was used to measure the width of the postextraction space. Results the results showed that NiTi closed coil springs method achieved greater reduction in postexstraction space (3.94 mm) while with elastic chain method the closure of 3.10 mm was achieved. The total difference between these two methods in the observed period was 0.84mm and no statistically significant difference was found (p> 0.05). The lowest value for NiTi closed springs was 2.19 mm, while for the elastic chain it was 1.29 mm. The best postextraction space closure was 5.70 mm and it was completed using NiTi closed springs while for elastic chain the best value was 4.80 mm. Conclusion: NiTi closed coil springs lead to faster closure of postextraction spaces in relation to elastic chain. Since this difference is minimal, in practice, both methods can be used equally.

Introduction Lundstrom segmental analysis is often used analysis in orthodontic diagnosis. It includes measurements of available and needed space in the arch in order to determine whether there is a lack or excess of space for proper teeth alignment. Measurements are traditionally performed on plaster study models, but with recent developments of computer-based systems, there is an increase in use of digital models in measuring process. The aim of this study was to present a photogrammetry based measurement approach that requires no specialized and expensive hardware and compare results with ones obtained on 3D scanned models. Material and method On 50 plaster study models measurements of 24 teeth, widths of 12 segments and Lundstrom segmental analysis were performed. 3D scanned study models were analyzed in the photogrammetry software OrthoPhoto4D on the set of four photographs of the study model in custom made measurement apparatus. The software corrects for finite distance of the camera and corrects errors due to perspective distortion. Results Statistical analysis performed on obtained measurements provided Bland-Altman plots that strongly suggested high degree of correspondence between the two measurements methods. Discrepancies for maxilla for individual segments were under 0.25 mm with standard deviation of up to 0.16 mm, and less than 1 mm and deviation of up to 0.4 mm for complete arch. For mandible the differences were up to 0.27 mm for segments with 0.15 mm deviation and 0.6 mm for complete arch with up to 0.24 mm deviation. Correlation coefficient was over 0.985 in all cases. Conclusions Both analyzed methods can be equally used in clinical practice.

Introduction After the completion of treatment with fixed orthodontic appliances, it is necessary to remove the brackets and bands from teeth using an appropriate method. The aim of this study was to determine the most common way of bond failure between teeth and metal brackets, as well as to compare bond failure between the brackets and upper and lower premolars. Material and Method Metal brackets were bonded with Aspire composite material on 154 human premolars, extracted for orthodontic purposes. After debonding, the surface of remaining adhesive on the teeth and brackets was measured. Adhesive Remnant Index (ARI) was used to estimate bond failure between teeth and metal brackets. Results The average size of remaining adhesive surface after removing brackets from the upper premolars was 12.06 mm2 , while it was 9.32 mm2 on the lower premolars. The average size of the remaining adhesive surface area on the brackets removed from the upper premolars was 0.37 mm2 , while it was 2.08 mm2 on the brackets removed from lower premolars. A statistically significant difference was found between these values. The most common score of ARIteeth was 3 (85.71%) and the most frequent score of ARIbrackets was 0 (85.71%). Conclusion The most common way of bond failure between teeth and metal brackets was between the bracket base and adhesive surface. A statistically significant difference was found between the values of the size of residual adhesive surface on the upper and lower premolars as well as on the brackets debonded from them.

Introduction Need for orthodontic treatment is determined by the severity of specific malocclusion. Orthodontic indices are used to objectively present treatment needs. The most commonly used is the Index of Orthodontic Treatment Need (IOTN). The aim of this study was to determine the need for orthodontic therapy in 11 to 13 years old schoolchildren in Republika Srpska. Material and Methods 1377 patients, 11 to 13 years old, were included in this study. The examinations were performed in elementary schools, using daylight, flat mirror and two-dimensional orthodontic caliper. Patients were ranked into 5 grades according to the Dental Health Component (DHC) and graded on a scale 1 to 10 for the Aesthetic Component (AC) of the IOTN index. Results According to the Dental Health Component (DHC), 79.23% of patients needed orthodontic treatment. Little and borderline orthodontic need was present in 27.74% and 24.83% children, respectively, while severe need was present in 19.98% and 6.68% respectively. 20.77% of subjects did not need orthodontic treatment. In regards to the Aesthetic Component (AC) (face appearance related to orthodontic need) little or no treatment was needed in 91.42% of subjects while 6.42% were borderline and 2.16% of subjects had severe relation between orthodontic need and facial appearance. Conclusion Due to the wide presence of orthodontic irregularities in schoolchildren 11-13 years old in Republika Srpska, it is important to introduce orthodontic indices in clinical practice in order to determine orthodontic treatment needs.

Introduction : Periodontal status and malocclusions are closely related. Retention places that enable for detention of food caused by various malocclusions, make selfcleaning and proper hygiene difficult and as such endanger the integrity of the soft tissues. Correctly applied orthodontic treatment has positive effects on periodontal tissues as it reduces the risk of damage and inflammations. One of the key factors for successful orthodontic therapy is adequate maintaining of the oral hygiene, which includes removal of dental plaque, the main cause of deterioration of the soft tissues of the oral cavity. Aim of the study: Establish the connection between changes in the soft tissues of the oral cavity and the type of used orthodontic appliance. Establish the time interval for appearance of changes on the soft tissues of the oral cavity. Patients and Methods: This research included 120 patients who were divided into three groups depending on the type of the orthodontic appliance used (mobile orthodontic appliance - active, mobile orthodontic appliances - functional and fixed orthodontic appliances). Examinations were performed three times: at the beginning of the treatment, after three months of therapy and after six months of therapy. In order to analyze the status of the soft tissues of the oral cavity and level of hygiene we used: Silness-Loe plaque index and Loe-Silness gingival index. Results: In patients divided by the type of orthodontic therapy, a statistically significant difference in the value of indexes after three and six months of wearing orthodontic appliances was found in patients who wore fixed orthodontic appliances. There was a difference in increased index values among this group when compared to subjects treated with mobile orthodontic appliances. Conclusion: During the orthodontic therapy, especially with fixed orthodontic appliances, patients must pay attention to adequate oral hygiene maintenance in order to avoid any unwanted changes on soft tissues of the oral cavity.

Introduction Clinical extraoral examination prior to orthodontic treatment includes face analysis (front and profile). Development of computer technology has increased efficacy and simplified this process through automating several steps of the analysis. The aim of this paper was to examine the possibility of automatic determining of linear measurements based on the facial image of a patient. Material and Methods Based on the set of 20 patients in NHP (Natural Head Position) position, three sets of measurements were conducted. Trained orthodontist performed positioning of predefined points on the image of the patient two times with one week apart, after which the points were automatically determined using customized computer software. Based on the position of the points, measurements for bizygomatic distance, upper and lower facial height and full facial height were computed. Three sets of measurements were compared and statistically analyzed. Results showed that computer software produced measurements comparable to measurements obtained by a trained orthodontist. Statistical analysis included calculating mean values and standard deviations, as well as paired two-tailed T-test. Differences between measurements ranged from 0.03% to 0.6% suggesting that automatic method can be successfully used. Conclusions The results of this research suggest that it is possible to ease, accelerate and automate work of the orthodontist on the image analysis using suitable software without significant differences in measured values.

Introduction In order to establish an accurate diagnosis and proper therapy planning in orthodontics, it is necessary to perform the analysis of transverse and sagittal dental arch development in relation to the facial type of the patient. The aim of this study was to determine sagittal and transverse parameters of dental arches in the population of Republika Srpska (Bosnia and Herzegovina) based on Schwarz analysis. Material and Methods Facial type was determined in 300 patients of both genders, aged 18 to 25, with class I occlusion. After taking impressions of the upper and lower jaw, cast study models were poured and used to determine basic sagittal and transverse parameters of dental arches. The obtained values were compared with the values defined by Schwarz in order to determine the existence of certain variations. Results Out of 300 subjects, 50.33% were dolichofacial, 30.76% were mesofacial and 19.00% were brachyfacial. The average values of anterior width, posterior width and height of dental arch in patients with all three types of face showed lower range of values compared to the values defined by Schwarz, who determined his values by examining the same parameters in the population of Vienna. These parameters did not show linear proportional increase with the increase of the sum of upper incisors, what is the case with the values defined by Schwarz. Conclusion The results of this study showed significant differences compared to Schwarz's values. Modification of the size and shape of dental arch leads to changes in the appearance of patient's face. Therefore, an individual approach during orthodontic diagnosis and treatment planning must be present despite the great importance of biometric standards.

The aim of this study was to find the correlation coefficient between mesiodistal width of permanent lower incisors and permanent canine and premolars for each quadrant, to establish equations for prediction of the sum of canine and premolars based on the sum of the lower incisors for the population of the Republic of Srpska as well as to compare the results with the values provided by Moyers's and Tanaka-Johnston's analysis and detremine sexual dimorphizm in tooth size. We analyzed 100 (50 male and 50 female) subjects with permanent dentition and class I molar relationship. The mesiodistal width of the lower permanent incisors, upper and lower canines and premolars were measured using a vernier caliper with the accuracy of 0.1 mm. The aim of this research was to determine the prevalence of three different face types and average values for dental arch size among population of Republic of Srpska. With this research we wanted to determine whether there are any differences in the sagittal and transversal development of dental arches between males and females. The application of anthropometry in dentistry gives us better understanding and determination of morphological characteristics of the craniofacial system. Data obtained by measuring these parameters is very important in order to make an accurate orthodontic diagnosis. The sample consisted of 300 study subjects with I class dentoalveolar relationship. After determing the face type of each subject, arch parameters were measured on subjects' dental casts. Using facial indices 50.33% of 300 subjects were classified as leptoprosopic, 30.67% as mesoprosopic and 19% as euryprosopic. The average value for interpremolar width was 36.75 mm for maxillary dental arch and for mandibular dental arch was 35.93 mm. The average intermolar width of the maxillary dental arch was 46.53 mm and for mandibular dental arch was 46.52 mm. The average value for maxillary dental arch was 24,22 mm while the same parameter for mandibular dental arch was 19,32 mm. The comparison of average values for dental arch height and width in females and males showed significant differences between these parameters.

Introduction Periodontal disease is a chronic inflammatory disease caused by pathologic microorganisms/periopathogens from oral biofilm. Standard periodontal therapy consists of scaling and root planing (SRP). Probiotics can be used as an adjunctive to standard periodontal therapy, since it is known that probiotics can modify pathogenic potential of biofilm by suppressing the colonization of periopathogens. The aim of this study was to assess the clinical effect of Bifidobacterium and Lactobacillus probiotic lozenges, probiotic mouthwash, as an adjuvant to SRP in the treatment of initial to moderate chronic periodontitis. Material and methods Thirty patients with initial to moderate chronic periodontitis were recruited and monitored clinically at baseline (before SRP) and 60 days following SRP. All patients were randomly assigned to experimental group: SRP + probiotic (n = 15) and control group: SRP only (n = 15). The probiotic mouthwash was used twice a day for 60 days. Clinical parameters: the probing pocket depth (PPD), clinical attachment level (CAL) and bleeding on probing (BOP) were measured at baseline and 60th day following SRP. Data were statistically analyzed using the one-way Anova test and SPSS 19 software (IMB Company, New York, U.S.). The Friedman and Mann Whitney tests were used as a post hoc test for intergroup analysis. Statistical significance was set at p < 0.05. Results After 60 days of treatment, the clinical parameters PPD, CAL and BOP were significantly lower in both groups compared to the baseline. In the experimental group, the clinical parameters PPD, CAL and BOP were significantly reduced after 60 days of treatment compared to the initial measurements (p < 0.05). In the control group, statistically significant decrease after 60 days of treatment was recorded only for BOP parameter, while there was no statistically significant decrease of PPD and CAL values (p > 0.05). Conclusion The results of the present study demonstrated clinical benefits of adjunctive use of probiotics to SRP in terms of pocket depth reduction in initial to moderate periodontal disease.
