Introducción: La lactancia materna (LM) hace parte fundamental del cuidado integral que ofrecen los profesionales de enfermería en la Unidad de Cuidados Neonatales (UCN). Debido a la carga laboral y la delegación de diferentes acciones de cuidado, las dinámicas en estas unidades se centran en la innovación por parte del uso tecnológico, y postergan la humanización del servicio perjudicando el apoyo a la LM. Objetivo: Diseñar un programa de apoyo a la LM en la UCN. Materiales y métodos: Investigación cualitativa acción participación. Se realizaron 5 entrevistas semiestructuradas a profesionales de la salud expertos en LM, unos que trabajan en la unidad de la institución en que se realizó el estudio y otros externos, indagando, desde su experiencia y conocimiento, los elementos clave para el diseño de un programa de apoyo a la LM en la UCN. Resultados: Se exponen en las categorías: a. Cultura en lactancia, comprende: política institucional en lactancia y estandarización del proceso; b. Calidad, comprende: registro y control de actividades y; c. Seguimiento de indicadores y auditoría constante. Discusión: Se diseñó un programa de LM el cual será aplicado por la institución con el fin de que la LM exclusiva no se vea frustrada, por medio del acompañamiento, la capacitación, la estandarización y la promoción. Conclusión: El personal de enfermería está interesado en que se estandarice un programa que permita que la información brindada a los pacientes sea veraz y concisa, plantean la educación como pilar fundamental para el desarrollo de un programa exitoso en LM.

Objective: to identify the challenges faced by nursing mothers, newborns and family members and the strategies for breastfeeding newborns admitted to Brazilian neonatal intensive care units. Method: integrative review of the literature carried out in Scopus, Latin American and Caribbean Health Sciences Literature, Medical Literature Analysis and Retrieval System Online and Base de Dados de Enfermagem, in July 2023. The search strategy used was "Breast Feeding" AND "Intensive Care Units" AND "Neonatal" OR "Newborn". The organization and pre-analysis were developed in Rayyan®, and the studies were subjected to Thematic Content Analysis. Results: it was found that the challenges are multifactorial and include: damage to the bond between the mother and the newborn, newborn's physiological immaturity, prematurity, low weight, clinical complications, surgical procedures, difficulties in breast milk expression, insufficient knowledge and psychological factors. The strategies point to ongoing health education, skin-to-skin contact, a multidisciplinary approach, the inclusion of family members in support groups, speech therapy assessment prior to breastfeeding and institutional spaces for health professionals to share experiences. Conclusion: care regarding breastfeeding, in the critical context evaluated, is longitudinal, multifactorial and encompasses care, teaching and management actions. [ABSTRACT FROM AUTHOR]

Objetivo: conocer la experiencia del equipo de enfermería al actuar en situaciones de aflicción parental en Cuidado Intensivo Neonatal. Método: estudio cualitativo desarrollado a través de entrevistas semiestructuradas en el período de marzo a abril de 2021 con profesionales de enfermería de una Unidad de Cuidado Intensivo Neonatal en Rio Grande do Sul, Brasil. Los resultados se sometieron a un análisis temático de contenido. Resultados: el acto de vivir lo luto parental por parte del equipo de enfermería atraviesa la tristeza, la impotencia y el sufrimiento. Las estrategias a las que nos enfrentamos en este momento se basan en la empatía y la humanización. Para los profesionales, el conocimiento para afrontar la situación viene dado por las propias experiencias de pérdida en la vida personal y profesional, de la fuerza y la religiosidad. Conclusión: se espera que este estudio contribuya a la práctica de los profesionales de la salud en la asistencia a los padres que experimentan el proceso de luto parental en el contexto de los cuidados intensivos neonatales.

Objetivo: presentar un algoritmo para la indicación de accesos venosos para recién nacidos en la Unidad de Cuidados Intensivos Neonatales. Desarrollo: estudio descriptivo realizado en una Unidad de Cuidados Intensivos Neonatales en el sur de Brasil, en el período de 2017 a 2020 en dos etapas de idealización: diseño del algoritmo; y la adecuación basada en la aplicación de acuerdo con las observaciones realizadas por las enfermeras involucradas. El algoritmo desarrollado contemplaba la predicción de acceso venoso para pacientes con indicación de cirugías abdominales y de otro tipo, prematuros estratificados en edad gestacional superior e inferior a 30 semanas, cardiopatías con dependencia o no de canal arterial e hipoglucemia neonatal persistente. En función de la red venosa, el número de terapias prescritas, el ayuno, la antibioticoterapia y los fármacos vasoactivos, existe una indicación inicial para un tipo de catéter que puede ir seguida de otra según las necesidades. Conclusión: la participación del enfermero en la indicación de acceso venoso integra las prácticas interprofesionales e incrementa la asistencia neonatal.

Objective: to report cases of complications and retention of peripherally inserted central catheters in newborns. Method: Case report on two cases of coiling and retention of a peripherally inserted central catheter in a Neonatal Intensive Care Unit of a hospital in Southern Brazil, between May and October 2022. Results: In the first case there was difficulty in removing the catheter soon after its insertion, requiring surgical removal. In the second, the folding in the antecubital fossa of the neonatal upper limb was identified by radiography, without migration of the tip to the central region, despite showing blood reflux, being removed by manual traction. Conclusion: the formation of tangles or knots in peripherally inserted and retained central catheters are rare complications. Being aware of this event makes it possible to build evidence and strategies for prevention and appropriate management in professional practice. [ABSTRACT FROM AUTHOR]

Introduction: Birth, hospitalization, transition from hospital to home and care for preterm infants are critical events for parents. Materials and Methods: A descriptive qualitative study was conducted with four fathers and 12 mothers who had experienced preterm infant care. Semi-structured interviews were conducted between March and October 2020 and grounded theory tools were used for analysis. Results: The following four categories were identified: coping with preterm birth, experiencing day-to-day prematurity in neonatal units, empowering parents to care for preterm infants and caring at home. The category for empowering parents to care for preterm infants was identified as the central category in the experience. Discussion: Experiencing preterm birth is complex and dynamic as it involves a high emotional burden for parents. Caring for preterm infants requires developing specific skills before discharge in which empowerment plays a central role. Parental empowerment facilitates this transition supported by nursing accompaniment, information provided, involvement in the care of their child in the neonatal unit, family support and individual conditions of parents. Conclusions: Experiencing parental empowerment in the care of their preterm infants is a key element that facilitates the transition from hospital to home and encourages the development of home care skills. Introducción. El 50 % de los episodios de sepsis neonatal se originan en la comunidad, con un gran porcentaje de mortalidad y complicaciones. Objetivo. Estimar los costos directos de la hospitalización por infección neonatal adquirida en la comunidad en neonatos a término con bajo riesgo al nacer. Materiales y métodos. Se utilizó la perspectiva del tercer pagador y la técnica de microcosteo; el horizonte de tiempo fue la duración de la hospitalización. La determinación de las situaciones generadoras de costos se obtuvo por medio de un consenso de expertos y se cuantificaron con base en la factura detallada de la atención de 337 neonatos hospitalizados. Los costos de los medicamentos se calcularon con base en el Sistema de Información de Precios de Medicamentos (SISMED) y, el de los procedimientos, según los manuales tarifarios ISS 2001 con porcentaje de ajuste y el seguro obligatorio de accidentes de tráfico (SOAT). Para incorporar la variabilidad de la información en la estimación, se obtuvo una distribución de los costos usando el método de bootstrapping. Resultados. Se incluyeron las facturas por la atención de 337 recién nacidos. El promedio de costos directos de la atención por paciente fue de COL$ 2’773.965 (desviación estándar, DE=$ 198.813,5; IC95%: $ 2’384.298 - $ 3’163.632). Las principales categorías generadoras de costos fueron la internación en la unidad de cuidados intensivos y las tecnologías en salud. Los costos siguieron una una distribución logarítmica normal (log-normal). Conclusiones. Las categorías con mayor impacto en los costos fueron la internación en la unidad neonatal y las tecnologías en salud. Los costos se ajustaron a una distribución logarítmica normal.

El registro clínico es la documentación de las intervenciones realizadas por el personal de enfermería, es instrumento ético, legal que visibiliza la calidad del cuidado asistencial del profesional que brinda a sus pacientes. Objetivo: Identificar la calidad de los registros clínicos de enfermería con base en el indicador nacional y estatal en la unidad de cuidados intensivos neonatales (UCIN) del Hospital Regional de Alta Especialidad del Niño. Material y Método: Investigación cuantitativa, observacional, descriptiva, transversal. Población 68 expedientes clínicos de neonatos de la UCIN, la muestra 192 registros clínicos de enfermería. Se utilizaron dos instrumentos: indicador de registros clínicos nacional de 10 ítems y la evaluación estatal de 6 dimensiones y 46 ítems. Se utilizó el paquete estadístico SPSS versión 26 para el analisis de datos. Resultados: El indice de calidad de los registros clínicos de 79.58 (nacional) y de 71.61 (estatal) ambos representan calidad media. Los criterios con mayor cumplimiento son: la evaluación de la evolución del neonato al plan de cuidados e intervenciones de enfermería (99%), registra el diagnóstico de Enfermería (95.8%) y los objetivos o resultados esperados en el recién nacido (91.7%). En contraste con 100% de incumplimiento fueron la identificación y valoración del neonato, el plan de alta al egreso, aspectos prioritarios para la intervención adaptándolos a la normatividad vigente. Conclusión: Los registros clínicos de enfermería presentaron un nivel de calidad medio, es evidente la necesidad de rediseñarlos congruente con el conocimiento disciplinar y capacitar a los profesionales de la UCIN.

FUNDAMENTOS // La administración de oxigenoterapia en las unidades de cuidados intensivos neonatales (UCIN) supone un aumento del riesgo de desarrollar lesiones por presión (LPP). Los objetivos de este trabajo fueron describir la incidencia de LPP asociadas a dispositivos de oxigenoterapia, así como identificar, analizar y relacionar los factores de riesgo y sus medidas preventivas. MÉTODOS // Se realizó un estudio retrospectivo, observacional y analítico de neonatos hospitalizados que desarrollaran una LPP secundaria a dispositivos de O2 en la UCIN del Hospital Universitario Miguel Servet de Zaragoza. Las variables registradas fueron las sociodemográficas, las clínicas, el tipo de cuna, la humedad, la temperatura, el tipo de oxigenoterapia, el tipo de ventilación, el dispositivo utilizado, la presencia de úlcera (número, grado, localización) y las medidas preventivas aplicadas. El estudio fue aprobado por el Comité de Ética de la Investigación de la Comunidad Autónoma de Aragón. El análisis estadístico se realizó mediante Jamovi 2.3.13®. RESULTADOS // Se incluyeron 191 neonatos, de los cuales 158 (82,7%) recibieron oxigenoterapia. El 64,10% de ellos fueron niños, la media de edad fue de 5,20±8,46 días y la de peso de 1.460,0±777,57 gramos. El 24,68% presentaron LPP asociada a dispositivo, con una media de días de ingreso en el momento de la aparición de 3,98±5,03 días. El 94,74% de las lesiones fueron de grado I y el 84,62% se localizaron en tabique nasal. CONCLUSIONES // La incidencia de LPP asociada a los diferentes dispositivos de oxigenoterapia aumenta a medida que disminuye la edad gestacional. El riesgo aumenta con la estancia hospitalaria, siendo la presencia de dispositivos médicos, en particular la ventilación mecánica no invasiva, la principal causa.

To identify the dietary profile of premature newborns admitted to a Neonatal Unit of a federal public maternity hospital in the city of Rio de Janeiro. This is an excerpt from a descriptive, retrospective study with a quantitative approach, based on the review of medical records analyzed using simple descriptive statistics. The most prevalent diet was the enteral diet (65.63%), followed by the oral diet (33.33%). The average in days of each one was 26 days in enteral diet, eight days in oral diet. Mostly used techniques were gavage (57.29%), breastfeeding (27.08%), cup (8.33%), gastroclysis (4.17%) and bottle feeding (3.13%). The current research has made it possible to know the dietary profile of preterm infants in terms of their growth and also through the practices carried out in the unit, in addition to detailing how this transition process occurs during hospitalization, as well as the main complications encountered during this period. Introducción: Los neonatos tienen una reserva nutricional limitada que los hace extremadamente vulnerables, por lo que es necesario un adecuado soporte nutricional. Los pacientes neonatales con procedimientos quirúrgicos son frecuentemente admitidos en la unidad de cuidados intensivos neonatales (UCIN). El objetivo del estudio fue analizar la frecuencia y características de las complicaciones metabólicas en los recién nacidos posoperatorios que recibieron soporte nutricional parenteral en la unidad de cuidados intensivos en el Hospital Pediátrico Dr. Hugo Mendoza en República Dominicana. Material y métodos: Estudio descriptivo y retrospectivo. Resultados: Las complicaciones más frecuentes fueron hiperglucemia para un 33,3%, seguido de trombocitopenia (21%) e hiponatremia (18%). Conclusiones: No se evidencia relación entre los días de administración de NP y la incidencia de complicaciones, ni se muestra relación entre la cantidad de días de nutrición parenteral y días de internamiento.
