Abstract Background To evaluate the hemodynamic characteristics of normal upper extremity arteries from the brachial artery to the fingertip arterioles. Methods We analyzed the characteristics and changes in the regularities of ultrasonic parameters in the upper extremity arteries of 104 healthy volunteers using color Doppler flow imaging and Angio Planewave UltraSensitive™ imaging. The measured ultrasonic parameters included the vessel diameter, blood-flow spectrum waveform, peak systolic velocity, end-diastolic velocity, resistance index, pulsatility index, ratio of PSV to EDV, blood-flow volume, along with systolic acceleration of each BA, radial artery, superficial palmar arch artery, palmar proper digital artery, and third-grade artery arch of the fingernail bed. Results: From BA to FN3AA, the diameter, PSV, RI, S/D, VFlow, and slope of the artery significantly decreased (P < 0.001), and size of the parameters significantly correlated with the anatomic position of the arteries. The blood-flow spectrum gradually changed from various waveforms to fixed monophasic waves. There was a linear relationship between the distribution of monophasic waveforms and arterial groups (R = -0.453; P < 0.001). When the blood-flow spectrum of BA and RA showed a monophasic waveform, their branch arteries showed the same. The waveforms of PPDA and FN3AA were also monophasic. Conclusion: Regular changes in the blood-flow spectrum waveform and hemodynamic parameters are related to the anatomic position of the upper limb arteries. CDFI combined with Angio PLUS accurately and systematically evaluates the hemodynamic characteristics of the upper extremity arteries. [ABSTRACT FROM AUTHOR]

Anatomical variations are deviations from the standard structures and morphologies of bodily arrangements, a phenomenon commonly seen in arteries and nerves. A typical axillary artery extends from the outer border of the first rib to the lower edge of the teres major before the pectoralis minor divides the axillary artery into three parts. Within existing research, several variations have been seen in the branching of the axillary artery, particularly among the second and third divisions. This case report examines and describes the anatomical differences of the second and third division of the axillary artery in a 64-year-old Caucasian female; variations were found in the second division of the axillary artery, two thoracoacromial arteries, as well as the absence of the stemming lateral thoracic artery. Within the third division of the axillary artery, the subscapular artery supplying the latissimus dorsi (two branches), subscapularis (circumflex scapular artery), and the serratus anterior (two branches) muscles were identified and distally, the axillary artery bisected into the brachial and accessory brachial arteries. A genetic mutation, environmental factors, and embryological malformations are among the hypotheses associated with these anatomical variations. From a clinical standpoint, these variations may not produce impairment though the knowledge of an individual’s distinctive arrangements would aid in preventing unnecessary complications that could arise during particular interventions.

Background: The prolonged bed-rest required achieving hemostasis after splanchnic angiography and interventional procedures can be avoided if the upper limb arteries are used. In such procedures, the use of long sheaths capable of reaching the descending aorta may be advantageous. Purpose: To analyze the results of procedures that utilizes an upper-limb-artery approach and long sheaths. Material and Methods: Two hundred forty-two patients with a mean age of 64 years underwent splanchnic angiography and interventional procedures via an upper limb artery using a long sheath (85 cm, 4-French). Repeat examinations were performed on 48 patients and the total number of examinations was 296. The records of these 296 examinations were reviewed and the success rate and complications were evaluated. Results: Overall, 295 of 296 (99.7%) examinations were successful, and one (0.3%) failed. Complications and side effects occurred in six cases (2.0%), a painful sheath manipulation occurred in two examinations (0.7%), and arterial occlusion (including temporary occlusion), hematoma of the puncture site, and pseudoaneurysm occurred in two (0.7%), one (0.3%), and one (0.3%) patient, respectively. Conclusion: The use of a long sheath capable of reaching the descending aorta enables the performance of splanchnic angiography and interventional procedures via the upper limb arteries.
