Abstract: Extragalactic globular clusters (EGCs) are an abundant and powerful tracer of galaxy dynamics and formation, and their own formation and evolution is also a matter of extensive debate. The compact nature of globular clusters means that they are hard to spatially resolve and thus study outside the Local Group. In this work we have examined how well EGCs will be detectable in images from the Euclid telescope, using both simulated pre-launch images and the first early-release observations of the Fornax galaxy cluster. The Euclid Wide Survey will provide high-spatial resolution VIS imaging in the broad IE band as well as near-infrared photometry (YE, JE, and HE). We estimate that the galaxies within 100 Mpc in the footprint of the Euclid survey host around 830 000 EGCs of which about 350 000 are within the survey's detection limits. For about half of these EGCs, three infrared colours will be available as well. For any galaxy within 50Mpc the brighter half of its GC luminosity function will be detectable by the Euclid Wide Survey. The detectability of EGCs is mainly driven by the residual surface brightness of their host galaxy. We find that an automated machine-learning EGC-classification method based on real Euclid data of the Fornax galaxy cluster provides an efficient method to generate high purity and high completeness GC candidate catalogues. We confirm that EGCs are spatially resolved compared to pure point sources in VIS images of Fornax. Our analysis of both simulated and first on-sky data show that Euclid will increase the number of GCs accessible with high-resolution imaging substantially compared to previous surveys, and will permit the study of GCs in the outskirts of their hosts. Euclid is unique in enabling systematic studies of EGCs in a spatially unbiased and homogeneous manner and is primed to improve our understanding of many understudied aspects of GC astrophysics., Comment: Submitted to A&A

Abstract: We study the intracluster light (ICL) and intracluster globular clusters (ICGCs) in the nearby Perseus galaxy cluster using Euclid's EROs. By modelling the isophotal and iso-density contours, we mapped the distributions and properties of the ICL and ICGCs out to radii of 200-600 kpc (up to ~1/3 of the virial radius) from the brightest cluster galaxy (BCG). We find that the central 500 kpc hosts 70000$\pm$2800 GCs and $1.7\times10^{12}$ L$_\odot$ of diffuse light from the BCG+ICL in the near-infrared H$_E$. This accounts for 38$\pm$6% of the cluster's total stellar luminosity within this radius. The ICL and ICGCs share a coherent spatial distribution, suggesting a common origin or that a common potential governs their distribution. Their contours on the largest scales (>200 kpc) are offset from the BCG's core westwards by 60 kpc towards several luminous cluster galaxies. This offset is opposite to the displacement observed in the gaseous intracluster medium. The radial surface brightness profile of the BCG+ICL is best described by a double S\'ersic model, with 68$\pm$4% of the H$_E$ light in the extended, outer component. The transition between these components occurs at ~60 kpc, beyond which the isophotes become increasingly elliptical and off-centred. The radial ICGC number density profile closely follows the BCG+ICL profile only beyond this 60 kpc radius, where we find an average of 60-80 GCs per $10^9$ M$_\odot$ of diffuse stellar mass. The BCG+ICL colour becomes increasingly blue with radius, consistent with the stellar populations in the ICL having subsolar metallicities [Fe/H] ~ -0.6 to -1.0. The colour of the ICL, and the specific frequency and luminosity function of the ICGCs suggest that the ICL+ICGCs were tidally stripped from the outskirts of massive satellites with masses of a few $\times10^{10}$ M$_\odot$, with an increasing contribution from dwarf galaxies at large radii., Comment: Paper submitted as part of the A&A special issue `Euclid on Sky', which contains Euclid key reference papers and first results from the Euclid Early Release Observations. 27 pages, 21 figures, 5 tables. Abstract: We make use of the unprecedented depth, spatial resolution, and field of view of the Euclid Early Release Observations of the Perseus galaxy cluster to detect and characterise the dwarf galaxy population in this massive system. The Euclid high resolution VIS and combined VIS+NIR colour images were visually inspected and dwarf galaxy candidates were identified. Their morphologies, the presence of nuclei, and their globular cluster (GC) richness were visually assessed, complementing an automatic detection of the GC candidates. Structural and photometric parameters, including Euclid filter colours, were extracted from 2-dimensional fitting. Based on this analysis, a total of 1100 dwarf candidates were found across the image, with 638 appearing to be new identifications. The majority (96%) are classified as dwarf ellipticals, 53% are nucleated, 26% are GC-rich, and 6% show disturbed morphologies. A relatively high fraction of galaxies, 8%, are categorised as ultra-diffuse galaxies. The majority of the dwarfs follow the expected scaling relations. Globally, the GC specific frequency, S_N, of the Perseus dwarfs is intermediate between those measured in the Virgo and Coma clusters. While the dwarfs with the largest GC counts are found throughout the Euclid field of view, those located around the east-west strip, where most of the brightest cluster members are found, exhibit larger S_N values, on average. The spatial distribution of the dwarfs, GCs, and intracluster light show a main iso-density/isophotal centre displaced to the west of the bright galaxy light distribution. The ERO imaging of the Perseus cluster demonstrates the unique capability of Euclid to concurrently detect and characterise large samples of dwarfs, their nuclei, and their GC systems, allowing us to construct a detailed picture of the formation and evolution of galaxies over a wide range of mass scales and environments., Comment: 44 pages, 24 figures, 5 tables, paper submitted to A&A as part of the A&A special issue `Euclid on Sky', which contains Euclid key reference papers and first results from the Euclid Early Release Observations

Abstract: The Euclid ERO programme targeted the Perseus cluster of galaxies, gathering deep data in the central region of the cluster over 0.7 square degree, corresponding to approximately 0.25 r_200. The data set reaches a point-source depth of IE=28.0 (YE, JE, HE = 25.3) AB magnitudes at 5 sigma with a 0.16" and 0.48" FWHM, and a surface brightness limit of 30.1 (29.2) mag per square arcsec. The exceptional depth and spatial resolution of this wide-field multi-band data enable the simultaneous detection and characterisation of both bright and low surface brightness galaxies, along with their globular cluster systems, from the optical to the NIR. This study advances beyond previous analyses of the cluster and enables a range of scientific investigations summarised here. We derive the luminosity and stellar mass functions (LF and SMF) of the Perseus cluster in the Euclid IE band, thanks to supplementary u,g,r,i,z and Halpha data from the CFHT. We adopt a catalogue of 1100 dwarf galaxies, detailed in the corresponding ERO paper. We identify all other sources in the Euclid images and obtain accurate photometric measurements using AutoProf or AstroPhot for 138 bright cluster galaxies, and SourceExtractor for half a million compact sources. Cluster membership for the bright sample is determined by calculating photometric redshifts with Phosphoros. Our LF and SMF are the deepest recorded for the Perseus cluster, highlighting the groundbreaking capabilities of the Euclid telescope. Both the LF and SMF fit a Schechter plus Gaussian model. The LF features a dip at M(IE)=-19 and a faint-end slope of alpha_S = -1.2 to -1.3. The SMF displays a low-mass-end slope of alpha_S = -1.2 to -1.35. Abstract: We present an analysis of Euclid observations of a 0.5 deg$^2$ field in the central region of the Fornax galaxy cluster that were acquired during the performance verification phase. With these data, we investigate the potential of Euclid for identifying GCs at 20 Mpc, and validate the search methods using artificial GCs and known GCs within the field from the literature. Our analysis of artificial GCs injected into the data shows that Euclid's data in $I_{\rm E}$ band is 80% complete at about $I_{\rm E} \sim 26.0$ mag ($M_{V\rm } \sim -5.0$ mag), and resolves GCs as small as $r_{\rm h} = 2.5$ pc. In the $I_{\rm E}$ band, we detect more than 95% of the known GCs from previous spectroscopic surveys and GC candidates of the ACS Fornax Cluster Survey, of which more than 80% are resolved. We identify more than 5000 new GC candidates within the field of view down to $I_{\rm E}$ mag, about 1.5 mag fainter than the typical GC luminosity function turn-over magnitude, and investigate their spatial distribution within the intracluster field. We then focus on the GC candidates around dwarf galaxies and investigate their numbers, stacked luminosity distribution and stacked radial distribution. While the overall GC properties are consistent with those in the literature, an interesting over-representation of relatively bright candidates is found within a small number of relatively GC-rich dwarf galaxies. Our work confirms the capabilities of Euclid data in detecting GCs and separating them from foreground and background contaminants at a distance of 20 Mpc, particularly for low-GC count systems such as dwarf galaxies., Comment: Paper submitted as part of the A&A special issue `Euclid on Sky', which contains Euclid key reference papers and first results from the Euclid Early Release Observations

Abstract: The Euclid ERO showcase Euclid's capabilities in advance of its main mission, targeting 17 astronomical objects, from galaxy clusters, nearby galaxies, globular clusters, to star-forming regions. A total of 24 hours observing time was allocated in the early months of operation, engaging the scientific community through an early public data release. We describe the development of the ERO pipeline to create visually compelling images while simultaneously meeting the scientific demands within months of launch, leveraging a pragmatic, data-driven development strategy. The pipeline's key requirements are to preserve the image quality and to provide flux calibration and photometry for compact and extended sources. The pipeline's five pillars are: removal of instrumental signatures; astrometric calibration; photometric calibration; image stacking; and the production of science-ready catalogues for both the VIS and NISP instruments. We report a PSF with a full width at half maximum of 0.16" in the optical and 0.49" in the three NIR bands. Our VIS mean absolute flux calibration is accurate to about 1%, and 10% for NISP due to a limited calibration set; both instruments have considerable colour terms. The median depth is 25.3 and 23.2 AB mag with a SNR of 10 for galaxies, and 27.1 and 24.5 AB mag at an SNR of 5 for point sources for VIS and NISP, respectively. Euclid's ability to observe diffuse emission is exceptional due to its extended PSF nearly matching a pure diffraction halo, the best ever achieved by a wide-field, high-resolution imaging telescope. Euclid offers unparalleled capabilities for exploring the LSB Universe across all scales, also opening a new observational window in the NIR. Median surface-brightness levels of 29.9 and 28.3 AB mag per square arcsec are achieved for VIS and NISP, respectively, for detecting a 10 arcsec x 10 arcsec extended feature at the 1 sigma level., Comment: Submitted to A&A, 44 pages, 36 figures - Part of the A&A special issue `Euclid on Sky', which contains Euclid key reference papers and first results from the Euclid Early Release Observations

Abstract: The current standard model of cosmology successfully describes a variety of measurements, but the nature of its main ingredients, dark matter and dark energy, remains unknown. Euclid is a medium-class mission in the Cosmic Vision 2015-2025 programme of the European Space Agency (ESA) that will provide high-resolution optical imaging, as well as near-infrared imaging and spectroscopy, over about 14,000 deg^2 of extragalactic sky. In addition to accurate weak lensing and clustering measurements that probe structure formation over half of the age of the Universe, its primary probes for cosmology, these exquisite data will enable a wide range of science. This paper provides a high-level overview of the mission, summarising the survey characteristics, the various data-processing steps, and data products. Abstract: Sprouted products are a food alternative that contribute to improving human nutrition. Quinoa has a large amount of natural antioxidants. The dry and hard grains when they undergo a germination process change to tender and slightly acidic sprouts that contain amino acids, minerals, vitamins, enzymes that help digestion and antioxidants. Furthermore, sprouted quinoa has a high protein content and a low concentration of saponins. Previous studies have shown that sprouted quinoa has shown an increase in protein, calcium and ascorbic acid content. Quinoa sprouts improve the organoleptic characteristics of quinoa grains; few studies have used sprouted quinoa in the preparation of beverages. The objective of this study was to prepare a beverage based on sprouted quinoa using a simplex lattice mix design to determine the optimal formula. Three varieties of sprouted quinoa were used: white (Salcedo INIA), red (INIA-415 Pasankalla), and black (INIA 420-Negra Collana). The quinoa was incubated at 30[degrees]C in the dark for 48 hours at a relative humidity of 98%. Ten treatments with different proportions of the three types of sprouted quinoa were used. The antioxidant capacity and consumer satisfaction of the ten treatments were evaluated. The ABTS method was used to measure the antioxidant capacity of each treatment. The optimal drink mix was composed of 81.67% sprouted black quinoa, 18.33% sprouted white quinoa, and 0% sprouted red quinoa. Furthermore, the consumer acceptability results were similar across all treatments (p>0.05). Abstract: The consumption of insects, also known as entomophagy, is not a new eating habit; on the contrary, the insects are eaten in many countries worldwide, where insects are served as the main dish in many of them. In Peru, insect consumption is limited to regions of the central jungle where locals consume ants of the 'Atta sexdens' variety in the form of toasted snacks with salt. Analysis of insects has shown a wide variation in macronutrient content relative to most wild vertebrate meats, encompassing a wide range of protein, fat and carbohydrate concentrations. In that sense, in terms of their proportional macronutrient composition, insects could serve as equivalents not only to wild meat, but also to other foods, such as seafood, nuts, legumes, vegetables or fruits. The aim of this study was to assess nutritional quality of 'Atta sexdens' ant flour and determine its physical and chemical characteristics. The ants were obtained from the department of San Martin, province and district of Rioja and underwent a rigorous cleaning process to eliminate impurities such as wings, leaves and soil which are attached to the ants. The ants were dried using a Biobase oven, model BOV-T3[degrees]C, and then ground in an electric grinder and finally passed through a sieve. The final product was measured for physicochemical properties such as pH, % acidity in aqueous extract and % acidity in ethanolic extract, resulting in 6.57, 0.21, and 0.29, respectively. The nutritional composition assessment was based on the AOAC standard method where the fat content of the meal was 35.40%; protein, 35.51%; carbohydrate, 16.12%; ash, 5.39%; and moisture, 7.58%. For the assessment of pepsin digestibility, the AOAC 971.09 test method was used, resulting in 99.77% activity. In regard to techno-functional properties, it was found that swelling capacity (SC) and water absorption capacity (WAC) resulted in 2.91 WAC g water/g m and 1.86 SC ml/g, respectively. On the other hand, the microbiological results were within the permissible limits for health; therefore, we can consider this ant meal a safe and innocuous product as well as an excellent food source with exceptional nutritional characteristics. The purpose of this study was to assess the quality of nutrition information published by health weeklies in the surroundings of four hospitals in Lima, Peru. A cross- sectional study was conducted using a mixed methodological design involving both quantitative and qualitative analyses. Thirty-seven hard copies of 4 health weeklies were purchased over the 10- week study period. Bivariate Logistic Regression was performed to examine all available journal characteristics associated with the quality of provided information. Logistic Regression Models were estimated for the independent variables that showed statistical significance in the bivariate analysis. Weekly publications with the highest percentage of pages dedicated to nutrition were 'My Health' (41.67%), 'Natural Health' (48.6%) and those with the lowest percentage were 'Sun, medicine and beauty' (19.37%) and 'Health, Money and Love' (18.34%). Collected publications included 185 articles that were classified into two groups regarding the validity of the nutrition information presented: supported 50.3% (93/185) and unsupported 49.7% (92/185). Statistical analysis for quality estimated that the 'name of weekly' had a p value=0.000, the 'topic' had a p value=0.035, and the 'objective, source description, quarter of publications' had a p value >0.05. Multivariate logistic regression reports that only the variable 'name of the weekly' had statistical significance with p values less than 0.05. The percentage of nutrition information suggests that editors may not be applying journalistic principles and shows the importance of interdisciplinary work, between nutritionists and health journalists, to improve health status of the general population. In the case of weekly health publications, editors responsible for this type of written press could benefit from including nutritionists in charge of providing nutritional information. Abstract: Aims Recently, the cardiovascular outcomes for people using anticoagulation strategies (COMPASS) trial demonstrated that dual therapy reduced cardiovascular outcomes compared with aspirin alone in patients with stable atherosclerotic disease. Methods and We sought to assess the proportion of patients eligible for the COMPASS trial and to compare the epidemiology results and outcome of these patients with those without COMPASS inclusion or with any exclusion criteria in a contemporary, nationwide cohort of patients with stable coronary artery disease. Among the 4068 patients with detailed information allowing evaluation of eligibility, 1416 (34.8%) did not fulfil the inclusion criteria (COMPASS-Not-Included), 841 (20.7%) had exclusion criteria (COMPASS-Excluded), and the remaining 1811 (44.5%) were classified as COMPASS-Like. At 1 year, the incidence of major adverse cardiovascular event (MACE), a composite of cardiovascular death, myocardial infarction, and stroke, was 0.9% in the COMPASS-Not-Included and 2.0% in the COMPASS-Like (P = 0.01), and 5.0% in the COMPASS-Excluded group (P < 0.0001 for all comparisons). Among the COMPASS-Like population, patients with multiple COMPASS enrichment criteria presented a significant increase in the risk of MACE (from 1.0% to 3.3% in those with 1 and >_3 criteria, respectively; P = 0.012), and a modest absolute increase in major bleeding risk (from 0.2% to 0.4%, respectively; P = 0.46). Conclusion In a contemporary real-world cohort registry of stable coronary artery disease, most patients resulted as eligible for the COMPASS. These patients presented a considerable annual risk of MACE that consistently increases in the presence of multiple risk factors.

20. Continuous versus routine EEG in patients after cardiac arrest-Analysis of a randomized controlled trial (CERTA) - RESUS-D-22-00369

Abstract: BACKGROUND Electroencephalography (EEG) is essential to assess prognosis in patients after cardiac arrest (CA). Use of continuous EEG (cEEG) is increasing in critically-ill patients, but it is more resource-consuming than routine EEG (rEEG). Observational studies did not show a major impact of cEEG versus rEEG on outcome, but randomized studies are lacking. METHODS We analyzed data of the CERTA trial (NCT03129438), including comatose adults after CA undergoing cEEG (30-48 hours) or two rEEG (20-30 minutes each). We explored correlations between recording EEG type and mortality (primary outcome), or Cerebral Performance Categories (CPC, secondary outcome), assessed blindly at 6 months, using uni- and multivariable analyses (adjusting for other prognostic variables showing some imbalance across groups). RESULTS We analyzed 112 adults (52 underwent rEEG, 60 cEEG,); 31 (27.7%) were women; 68 (60.7%) patients died. In univariate analysis, mortality (rEEG 59%, cEEG 65%, p=0.318) and good outcome (CPC 1-2; rEEG 33%, cEEG 27%, p=0.247) were comparable across EEG groups. This did not change after multiple logistic regressions, adjusting for shockable rhythm, time to return of spontaneous circulation, serum neuron-specific enolase, EEG background reactivity, regarding mortality (rEEG vs cEEG: OR 1.60, 95% CI 0.43 - 5.83, p=0.477), and good outcome (OR 0.51, 95% CI 0.14 - 1.90, p=0.318). CONCLUSION This analysis suggests that cEEG or repeated rEEG are related to comparable outcomes of comatose patients after CA. Pending a prospective, large randomized trial, this finding does not support the routine use of cEEG for prognostication in this setting. We evaluated a potential association between FOXO1 gene locus and the response to AChEI treatment in patients with sporadic AD.Methods: In this prospective study, 109 Caucasian AD patients were treated with standard doses of donepezil, galantamine, or rivastigmine for 6 months. Functional and cognitive status were evaluated at baseline and after treatment. Response to therapy was defined according to the National Institute for Health and Clinical Excellence criteria. Genotype analyses, including the APOE polymorphism, were made in blinded fashion.Results: A significantly higher frequency of FOXO1 rs7981045 G/G genotype was observed in nonresponders compared with responders (17.14% versus 2.70%, P=0.010). Age, sex, and APOE-adjusted logistic regression analysis confirmed that patients with the G/G genotype had a significantly higher risk of poor response to AChEI treatment (odds ratio =10.310; 95% confidence interval, 1.510–70.362). Haplotype analysis revealed significant differences in haplotype frequency distribution between these groups.Conclusion: FOXO1 may influence the clinical response to AChEIs in AD patients. Abstract: Aims: To evaluate the impact of the COVID-19 pandemic on patient admissions to Italian cardiac care units (CCUs). Methods and Results: We conducted a multicentre, observational, nationwide survey to collect data on admissions for acute myocardial infarction (AMI) at Italian CCUs throughout a 1 week period during the COVID-19 outbreak, compared with the equivalent week in 2019. We observed a 48.4% reduction in admissions for AMI compared with the equivalent week in 2019 (P < 0.001). The reduction was significant for both ST-segment elevation myocardial infarction [STEMI; 26.5%, 95% confidence interval (CI) 21.7-32.3; P = 0.009] and non-STEMI (NSTEMI; 65.1%, 95% CI 60.3-70.3; P < 0.001). Among STEMIs, the reduction was higher for women (41.2%; P = 0.011) than men (17.8%; P = 0.191). A similar reduction in AMI admissions was registered in North Italy (52.1%), Central Italy (59.3%), and South Italy (52.1%). The STEMI case fatality rate during the pandemic was substantially increased compared with 2019 [risk ratio (RR) = 3.3, 95% CI 1.7-6.6; P < 0.001]. A parallel increase in complications was also registered (RR = 1.8, 95% CI 1.1-2.8; P = 0.009). Conclusion: Admissions for AMI were significantly reduced during the COVID-19 pandemic across Italy, with a parallel increase in fatality and complication rates. This constitutes a serious social issue, demanding attention by the scientific and healthcare communities and public regulatory agencies.

Abstract: Aims Recently, the cardiovascular outcomes for people using anticoagulation strategies (COMPASS) trial demonstrated that dual therapy reduced cardiovascular outcomes compared with aspirin alone in patients with stable atherosclerotic disease. Methods and results We sought to assess the proportion of patients eligible for the COMPASS trial and to compare the epidemiology and outcome of these patients with those without COMPASS inclusion or with any exclusion criteria in a contemporary, nationwide cohort of patients with stable coronary artery disease. Among the 4068 patients with detailed information allowing evaluation of eligibility, 1416 (34.8%) did not fulfil the inclusion criteria (COMPASS-Not-Included), 841 (20.7%) had exclusion criteria (COMPASS-Excluded), and the remaining 1811 (44.5%) were classified as COMPASS-Like. At 1 year, the incidence of major adverse cardiovascular event (MACE), a composite of cardiovascular death, myocardial infarction, and stroke, was 0.9% in the COMPASS-Not-Included and 2.0% in the COMPASS-Like (P = 0.01), and 5.0% in the COMPASS-Excluded group (P Conclusion In a contemporary real-world cohort registry of stable coronary artery disease, most patients resulted as eligible for the COMPASS. These patients presented a considerable annual risk of MACE that consistently increases in the presence of multiple risk factors.

Abstract: The consumption of insects, also known as entomophagy, is not a new eating habit; on the contrary, the insects are eaten in many countries worldwide, where insects are served as the main dish in many of them. In Peru, insect consumption is limited to regions of the central jungle where locals consume ants of the "Atta sexdens" variety in the form of toasted snacks with salt. Analysis of insects has shown a wide variation in macronutrient content relative to most wild vertebrate meats, encompassing a wide range of protein, fat and carbohydrate concentrations. In that sense, in terms of their proportional macronutrient composition, insects could serve as equivalents not only to wild meat, but also to other foods, such as seafood, nuts, legumes, vegetables or fruits. The aim of this study was to assess nutritional quality of "Atta sexdens" ant flour and determine its physical and chemical characteristics. The ants were obtained from the department of San Martín, province and district of Rioja and underwent a rigorous cleaning process to eliminate impurities such as wings, leaves and soil which are attached to the ants. The ants were dried using a Biobase oven, model BOV-T30C, and then ground in an electric grinder and finally passed through a sieve. The final product was measured for physicochemical properties such as pH, % acidity in aqueous extract and % acidity in ethanolic extract, resulting in 6.57, 0.21, and 0.29, respectively. The nutritional composition assessment was based on the AOAC standard method where the fat content of the meal was 35.40%; protein, 35.51%; carbohydrate, 16.12%; ash, 5.39%; and moisture, 7.58%. For the assessment of pepsin digestibility, the AOAC 971.09 test method was used, resulting in 99.77% activity. In regard to techno-functional properties, it was found that swelling capacity (SC) and water absorption capacity (WAC) resulted in 2.91 WAC g water/g m and 1.86 SC ml/g, respectively. On the other hand, the microbiological results were within the permissible limits for health; therefore, we can consider this ant meal a safe and innocuous product as well as an excellent food source with exceptional nutritional characteristics. Abstract: Background Septic shock is characterized by breakdown of the endothelial glycocalyx and endothelial damage, contributing to fluid extravasation, organ failure and death. Albumin has shown benefit in septic shock patients. Our aims were: (1) to identify the relations between circulating levels of syndecan-1 (SYN-1), sphingosine-1-phosphate (S1P) (endothelial glycocalyx), and VE-cadherin (endothelial cell junctions), severity of the disease, and survival; (2) to evaluate the effects of albumin supplementation on endothelial dysfunction in patients with septic shock. Methods This was a retrospective analysis of a multicenter randomized clinical trial on albumin replacement in severe sepsis or septic shock (the Albumin Italian Outcome Sepsis Trial, ALBIOS). Concentrations of SYN-1, S1P, soluble VE-cadherin and other biomarkers were measured on days 1, 2 and 7 in 375 patients with septic shock surviving up to 7 days after randomization. Results Plasma concentrations of SYN-1 and VE-cadherin rose significantly over 7 days. SYN-1 and VE-cadherin were elevated in patients with organ failure, and S1P levels were lower. SYN-1 and VE-cadherin were independently associated with renal replacement therapy requirement during ICU stay, but only SYN-1 predicted its new occurrence. Both SYN-1 and S1P, but not VE-cadherin, predicted incident coagulation failure. Only SYN-1 independently predicted 90-day mortality. Albumin significantly reduced VE-cadherin, by 9.5% (p = 0.003) at all three time points. Conclusion Circulating components of the endothelial glycocalyx and of the endothelial cell junctions provide insights into severity and progression of septic shock, with special focus on incident coagulation and renal failure. Albumin supplementation lowered circulating VE-cadherin consistently over time. Clinical Trial Registration: ALBIOS ClinicalTrials.gov number NCT00707122.

Abstract: Background Pro-protein convertase subtilisin/kexin 9 (PCSK9) is a proenzyme primarily known to regulate low-density lipoprotein receptor re-uptake on hepatocytes. Whether PCSK9 can concurrently trigger inflammation or not remains unclear. Here, we investigated the potential association between circulating levels of PCSK9 and mortality in patients with severe sepsis or septic shock. Methods Plasma PCSK9 levels at days 1, 2 and 7 were measured in 958 patients with severe sepsis or septic shock previously enrolled in the Albumin Italian Outcome Sepsis (ALBIOS) trial. Correlations between levels of PCSK9 and pentraxin 3 (PTX3), a biomarker of disease severity, were evaluated with ranked Spearman's coefficients. Cox proportional hazards models were used to assess the association of PCSK9 levels at day 1 with 28- and 90-day mortality. Results Median plasma PCSK9 levels were 278 [182-452] ng mL-1 on day 1. PCSK9 correlated positively with PTX3 at the three time-points, and patients with septic shock within the first quartile of PCSK9 showed higher levels of PTX3. Similar mortality rates were observed in patients with severe sepsis across PCSK9 quartiles. Patients with septic shock with lower PCSK9 levels on day 1 (within the first quartile) showed the highest 28- and 90-day mortality rate as compared to other quartiles. Conclusion In our sub-analysis of the ALBIOS trial, we found that patients with septic shock presenting with lower plasma PCSK9 levels experienced higher mortality rate. Further studies are warranted to better evaluate the pathophysiological role of PCSK9 in sepsis.

Abstract: Aims We evaluated the 1-year clinical events, pharmacological management, and quality of life in a contemporary cohort of stable coronary artery disease (CAD) patients managed by cardiologists. Methods and results START (STable Coronary Artery Diseases RegisTry) was a prospective, observational, nationwide study that enrolled 5070 stable CAD patients over 3 months in 183 cardiology centres in Italy. At 1 year, 4790 (94.5%) patients had data on vital status. Death occurred in 107 (2.2%) patients and the cause of death was cardiovascular in 41 (38.3%) of cases. Among the 4775 patients with follow-up data on clinical events available, a hospitalization due to cardiovascular and non-cardiovascular causes occurred in 523 (11.0%) and in 231 (4.8%) of cases, respectively. Over 60% of patients reported as ‘no problems’ in all domains (61.4–84.5%) of the EuroQoL quality of life 5D-5L questionnaire. Among the 3239 patients with clinical visit/telephone interview at follow-up, in whom optimal medical therapy (OMT; aspirin or thienopyridine, β-blocker, and statin) was prescribed at enrolment, 2971 (91.7%) were still receiving OMT at follow-up. At multivariable analysis, only increasing age (odds ratio 0.98; 95% confidence interval 0.97–0.99; P = 0.04) resulted as independent negative predictor of OMT persistence at 1 year from enrolment. Conclusion In this large, contemporary registry, stable CAD patients managed by cardiologists presented a high rate of clinical events at 1 year. Nevertheless, the persistence to OMT and quality of life appeared reasonable.

Abstract: Background Current guidelines suggest to consider dual antiplatelet therapy (DAPT) continuation for longer than 12 months in selected patients with myocardial infarction (MI). Hypothesis We sought to assess the criteria used by cardiologists in daily practice to select patients with a history of MI eligible for DAPT continuation beyond 1 year. Methods We analyzed data from the EYESHOT Post-MI, a prospective, observational, nationwide study aimed to evaluate the management of patients presenting to cardiologists 1 to 3 years from the last MI event. Results Out of the 1633 post-MI patients enrolled in the study between March and December 2017, 557 (34.1%) were on DAPT at the time of enrolment, and 450 (27.6%) were prescribed DAPT after cardiologist assessment. At multivariate analyses, a percutaneous coronary intervention (PCI) with multiple stents and the presence of peripheral artery disease (PAD) resulted as independent predictors of DAPT continuation, while atrial fibrillation was the only independent predictor of DAPT interruption for patients both at the second and the third year from MI at enrolment and the time of discharge/end of the visit. Conclusions Risk scores recommended by current guidelines for guiding decisions on DAPT duration are underused and misused in clinical practice. A PCI with multiple stents and a history of PAD resulted as the clinical variables more frequently associated with DAPT continuation beyond 1 year from the index MI.

Abstract: Aims: To evaluate the impact of the COVID-19 pandemic on patient admissions to Italian cardiac care units (CCUs). Methods and Results: We conducted a multicentre, observational, nationwide survey to collect data on admissions for acute myocardial infarction (AMI) at Italian CCUs throughout a 1 week period during the COVID-19 outbreak, compared with the equivalent week in 2019. We observed a 48.4% reduction in admissions for AMI compared with the equivalent week in 2019 (P < 0.001). The reduction was significant for both ST-segment elevation myocardial infarction [STEMI; 26.5%, 95% confidence interval (CI) 21.7-32.3; P = 0.009] and non-STEMI (NSTEMI; 65.1%, 95% CI 60.3-70.3; P < 0.001). Among STEMIs, the reduction was higher for women (41.2%; P = 0.011) than men (17.8%; P = 0.191). A similar reduction in AMI admissions was registered in North Italy (52.1%), Central Italy (59.3%), and South Italy (52.1%). The STEMI case fatality rate during the pandemic was substantially increased compared with 2019 [risk ratio (RR) = 3.3, 95% CI 1.7-6.6; P < 0.001]. A parallel increase in complications was also registered (RR = 1.8, 95% CI 1.1-2.8; P = 0.009). Conclusion: Admissions for AMI were significantly reduced during the COVID-19 pandemic across Italy, with a parallel increase in fatality and complication rates. This constitutes a serious social issue, demanding attention by the scientific and healthcare communities and public regulatory agencies.

Abstract: Background: Pro-protein convertase subtilisin/kexin 9 (PCSK9) is a proenzyme primarily known to regulate low-density lipoprotein receptor re-uptake on hepatocytes. Whether PCSK9 can concurrently trigger inflammation or not remains unclear. Here, we investigated the potential association between circulating levels of PCSK9 and mortality in patients with severe sepsis or septic shock. Methods: Plasma PCSK9 levels at days 1, 2 and 7 were measured in 958 patients with severe sepsis or septic shock previously enrolled in the Albumin Italian Outcome Sepsis (ALBIOS) trial. Correlations between levels of PCSK9 and pentraxin 3 (PTX3), a biomarker of disease severity, were evaluated with ranked Spearman’s coefficients. Cox proportional hazards models were used to assess the association of PCSK9 levels at day 1 with 28- and 90-day mortality. Results: Median plasma PCSK9 levels were 278 [182–452] ng mL−1 on day 1. PCSK9 correlated positively with PTX3 at the three time-points, and patients with septic shock within the first quartile of PCSK9 showed higher levels of PTX3. Similar mortality rates were observed in patients with severe sepsis across PCSK9 quartiles. Patients with septic shock with lower PCSK9 levels on day 1 (within the first quartile) showed the highest 28- and 90-day mortality rate as compared to other quartiles. Conclusion: In our sub-analysis of the ALBIOS trial, we found that patients with septic shock presenting with lower plasma PCSK9 levels experienced higher mortality rate. Further studies are warranted to better evaluate the pathophysiological role of PCSK9 in sepsis.
