E. coli responsible for urinary tract infections (UTI) became resistant to obtainable therapies. So, nanotechnology appears currently as promising innovation among various strategies under development. In the present study we evaluate the antimicrobial effects of CuO and ZnO nanoparticles against E. coli isolated from UTI. Biofilm production and expression of quorum sensing genes luxS and motA were performed on E. coli treated with the nanoparticles. luxS and motA genes were detected in the tested E. coli. All isolates produced strong, moderate and weak biofilm. All isolates had a weak biofilm formation and decreased gene expression for luxS and motA genes after treatment with CuO, ZnO and their mixture. Besides, CuO and ZnO had non-toxic effects on Human Dermal Fibroblasts and neonatal normal cells. In conclusion, the mixture of CuO and ZnO had a role in decreasing the gene expression of luxS and motA genes more than each alone.

BACKGROUND: Orthotopic urinary diversion is the preferred diversion after cystectomy. Neobladder reconstruction with a longer ileum segment (60 cm) is advantageous for obtaining a large capacity and continence at the beginning; however, the long-term risk of residual urine, chronic infection, and the need for intermittent catheterization is more pronounced with the neobladder constructed with a longer ileal segment compared to the neobladder tailored from the shorter ileal segment. OBJECTIVE: To establish the differences in the functional outcome of a shorter (< 45 cm) and longer (⩾ 45 cm) ileal segment usage in the reconstruction of the Hautmann ileal neobladder following the radical cystectomy. METHODS: Between July 2013 and September 2015, 121 patients with muscle-invasive bladder cancer underwent radical cystectomy and Hautmann ileal neobladder reconstruction. Patients were divided into two groups, depending on the length of the ileum used for the diversion creation: < 45 cm of the ileum was used in the first group and ⩾ 45 cm in the second group. Five-year follow-up was performed at 1, 3, 6, and 9 months and 1 and 5 years. The main measured outcomes were functional outcomes and the quality of life. The evaluation included clinical, laboratory, and QLQ-C30 questionnaires. Statistical analysis was performed using descriptive statistics, Mann-Whitney U test, Kolmogorov-Smirnov test, ANOVA, and chi-squared tests. RESULTS: Patients with a smaller neobladder had a better quality of life, and higher global health status scale score. Thirteen early and 21 late complications developed in 10 and 17 patients, respectively. There were significant differences in the need for clean intermittent self-catheterization (CIC) between the two groups: smaller-volume pouch patients had statistically decreased need for CIC in 5 year follow-up compared to larger-volume pouch patients (χ 2 test = 8.245; df = 1; P = 0.004). Eighteen percent of patients with smaller neobladders had urinary tract infections in 5 years, compared to 35% with larger neobladders (χ 2 test = 4.447; df = 1; P = 0.034). CONCLUSION: Minimizing the length of the ileal segment needed for Hautmann neobladder reconstruction is feasible and provides better long-term results than larger-volume neobladders. BackgroundType 2 diabetes mellitus is considered a risk factor for developing recurrent urinary tract infections. This review examined current knowledge on the incidence rates, bacterial strains, risk factors, treatments, and outcomes of recurrent urinary tract infections in type 2 diabetes, predominantly in women.MethodsA systematic review was conducted for all English language articles from inception to June 2022 utilizing the Cochrane and Preferred Reporting Items for Systematic Reviews and Meta-Analyses standards in the databases PubMed, OVID Embase, and Cochrane Library. References were cross-examined for further articles. Data collected described the prevalence, characteristics, and management of recurrent urinary tract infections. Risk of bias assessments were performed for all studies.ResultsFrom 3342 identified articles, 597 met initial study criteria. Fifteen studies from 10 countries were included after full-text reviews. Four studies found higher recurrent urinary tract infection rates in diabetics versus non-diabetics meanwhile others reported recurrence rates from 23.4% to 37%. Four of five studies found diabetes to be a risk factor for recurrent urinary tract infection. E. coli was the most frequent causative pathogen. Antibiotic prescription results varied; however, multiple studies determined that longer treatment (≥ 5 days) did not correlate with lower recurrence rates. Risk of bias assessments found the most frequent study weakness to be identification of confounding variables.ConclusionThis review covered multiple subtopics, with few comprehensive or generalizable results, suggesting a need for more research on how recurrent urinary tract infections can be better evaluated and managed in women with type 2 diabetes.

In this study, we explored the incidence of double J (JJ) contamination of patients who underwent an endourological procedure for urinary stones and ureteral stenosis. We developed a prospective study between January 2019 and December 2021. Ninety-seven patients, 54 male and 43 female, were enrolled. Urine culture was taken during four steps: before stent insertion, a sample from selective renal pelvis catheterization, a sample two days after the JJ insertion and finally, after the stent removal procedure. At the time of the stent removal, 1 cm of proximal and distal ends were cut off and placed in the culture for bacterial evaluation. Cohen's kappa coefficient value (k) and concordance rates of microbiological culture results were evaluated. The study group comprised 56% of male patients. Proximal and distal stent cultures were positive in 81 and 78 patients. The concordance rate of microbiological cultures between proximal and distal double J stent is 88% (k 0.6). The most common pathogens isolated from urine and stent cultures were Enterococcus spp. in 52 cases and Klebsiella spp. in 27 cases. [ABSTRACT FROM AUTHOR]

Introduction: Since COVID-19 pandemic spread, strict preventive measures were adopted to reduce the risk of transmission. Antiseptic dispensers for hand hygiene were diffusely available for patients and hospital staff. To investigate the prophylactic role played by the strict antiseptic rules adopted during pandemic, the rates of nosocomial urinary infections in 2019 and 2020 were compared. Materials and methods: Patients' clinical pre-operative characteristics, symptoms, fever, and laboratory data were recorded pre- and post-operatively. Urological surgery was classified in five categories: 1. major surgery 2. upper urinary tract endoscopy, 3. lower urinary tract endoscopy, 4. minor surgery, and 5. Nephrostomy and ureteral stenting. Clavien-Dindo complication score was used. Statistical analysis was performed with R 3.4.2 software. Results: Out of 495 patients, 383 (57.1%) underwent surgical intervention in pre-pandemic March–May 2019 period and 212 (42.9%) in the same pandemic 2020 interval. Preoperatively, 40 (14.1%) and 11 (5.2%) and 77 (27.3%) and 37 (17.5%) patients had fever (p < 0.003) and leukocytosis (p < 0.02), in 2019 and 2020 respectively. Urine culture was positive in 29 (10.2%) and 13 (6.2%) patients respectively (p = 0.22). Post-operatively, 54 (19.1%) and 22 (10.4%) patients and 17 (6.1%) and 2 (0.6%) patients showed fever (p < 0.003) and positive urineculture (p < 0.03), in 2019 and 2020 respectively. Discussion and conclusion: Preoperative and post-operative clinical and laboratory signs of nosocomial urinary infection showed a statistically significant lower incidence during the pandemic period in 2020. Background and Objective: Urinary reflux is the most common urological abnormality in children. About half of the children with urinary infection have urinary reflux at the same time. Considering the importance of urinary reflux in children and contradictory results reported regarding the relationship between different grades of vesicoureteral reflux and recurrent urinary tract infection, this study was conducted with the aim of comparing recurrent urinary tract infection in children with different grades of vesicoureteral reflux. Methods: This cross-sectional study was conducted on 150 children with pyelonephritis and vesicoureteral reflux who refered to Amirkola Children's Hospital in 2011-2019. Reflux grades were determined by voiding cystourethrogram (VCUG) or direct radionuclide cystography (DRNC). Children were followed up for 2 years and in case of clinical symptoms with positive urine culture, they were considered as recurrent urinary tract infection. Findings: Of the 150 children studied, 126 (84%) were girls and 24 (16%) were boys with a mean age of 27.17±11.53 months. 95 people (63.3%) had once, 37 people (24.7%) twice, 8 people (5.3%) three times, and 10 people (6.7%) four times experience of recurrent urinary tract infection. The odds ratio of recurrent urinary tract infection was more than two times higher in children with grade 3 and 4 reflux involvement than in children with grade 1 and 2 reflux (OR=3.20, 95% CI=1.05-9.75, p=0.041). However, there was no significant difference in recurrent urinary tract infection in children based on age at diagnosis, gender, and whether the reflux was unilateral or bilateral. Conclusion: The results of the study showed that children with moderate and severe involvement of vesicoureteral reflux experience higher recurrent urinary tract infection compared to mild involvement.

Background & aim: There are several reports regarding the increase and growing prevalence of ESBLs producing organisms from different regions in the country. Considering that Klebsiella pneumoniae includes a significant percentage of hospital infections, therefore, knowing the pattern of resistance and sensitivity of this bacterium to beta-lactam antibiotics plays a significant role in the treatment and control of infections caused by this bacterium. As a result, the aim of the present study was to determine and evaluate the frequency of beta-lactamases The broad spectrum of Klebsiella pneumoniae urinary isolates was drug-resistant by genotypic and phenotypic methods. Methods: In the present cross-sectional descriptive study conducted in 2018, 100 strains of Klebsiella pneumoniae were isolated from urine samples of patients with urinary tract infection from Al-Zahra Hospital, Isfahan. Klebsiella pneumoniae strains were identified and confirmed using standard biochemical tests. Antibiotic sensitivity pattern was investigated by disk diffusion method based on CLSI criteria. To perform the phenotypic verification test of broad-spectrum β-lactamase producing strains, the combined disc method was used based on CLSI criteria. PCR test was done with specific primers to identify blaSHV and bla TEM. The collected data were analyzed using one-way and independent one-way analysis of variance statistical tests. Results: Among 100 isolates related to urinary tract infection, 33 isolates indicated resistance to at least 4 antibiotics, the highest resistance to cefotaxime antibiotic (38%) and the least antibiotic resistance to cotrimaxazole (22%). The results of combined disk indicated that 10 isolates (86.8%) were broad-spectrum β-lactamase producers. Among them, 7 isolates (70%) had the SHV gene and the other 3 isolates did not have any of the TEM and SHV genes. Conclusion: The results indicated a higher frequency of ESBLs and correspondingly a higher prevalence of antibiotic resistance in Klebsiella pneumoniae bacteria isolated from urinary tract infections.

Purpose: Urinary tract infections are among the most common infections in childhood. In this study, it was aimed to evaluate the factors of urinary tract infection in our hospital and to determine antimicrobial resistance patterns and empirical treatment options. Materials and Methods: The results of urine cultures that were sent from the Department of Pediatrics between 01.01.2015 and 31.12.2020 to the Microbiology Laboratory were evaluated retrospectively. The identification of the bacterial growth was tested in Vitek MS (Biomeriux, France) and the antimicrobial susceptibility of the isolates was tested in Vitek2 Compakt (Biomeriux, France). Results: A total of 4257 urine culture samples were evaluated. The most frequently isolated bacteria were E. coli 62.93% followed by Klebsiella spp. 19.6%, Pseudomonas spp. 5.37%, Proteus spp. 4.48%. The highest resistance rates were determined against ampicillin in E. coli (78.96%) and for cefuroxime (63.11%) in Klebsiella spp isolates. The lowest resistance rate was determined against imipenem and meropenem in E.coli (0.12%, 0.15%) isolates and, amikacin and nitrofurantoin in Klebsiella spp. isolates (6.10%, 2.46%). Conclusion: In order to provide effective treatment for urinary tract infections and to minimize morbidity and long-term complications, the frequency of the pathogens causing UTIs, local or regional antibiotic resistance should be known and empirical treatment should be arranged accordingly.

Objective: The aim of our study to present the results of the clinical profile, diagnosis and treatment of children with urolithiasis in the Pediatric Urology Department of the Scientific center of Pediatria and Pediatric surgery in Almaty, Kazakhstan. Materials and methods: A retrospective data from pediatric patients with urolithiasis admitted from January 2015 to December 2021. The data was obtained from medical records and included those with diagnosis confirmed by imaging and laboratory test, as 24-hour urine exams corrected for creatinine, stone composition. Results: We reviewed 204 children with urolithiasis. The male-to-female ratio was 1.34:1. Congenital anomalies of the urinary tract were found in 40 patients (19.6%). Urolithiasis was more common in adolescent girls, whereas in boys the frequency of observation is higher between 11 and 17 years old (p = 0.006). Majority of the patients were from southern regions (51%) in our country. Renal colic, urinary tract infection and macroscopic hematuria were the most common clinical manifestations. Urinary tract infection was diagnosed in 185 patients (90.6%). The most frequent metabolic disorders were hypercalciuria, hyperoxaluria. In 35% of cases there was a metabolic disorder and appropriate therapy was prescribed. In the remaining cases, surgical treatment was used. Conclusion: In this study we presented the prevalence of urolithiasis in the pediatric population in different regions of our country. Introduction: Ceftazidime–avibactam has proven activity against multidrug-resistant (MDR) bacteria in clinical trials and real-world studies. This study was conducted to describe the patterns of use of ceftazidime–avibactam (including indications and associated antibiotics), and the effectiveness and safety of ceftazidime–avibactam in real-world clinical practice. Methods: This non-interventional medical chart review study was conducted in 11 countries across the European and Latin American (LATAM) regions. Consecutive patients treated in clinical practice with at least one dose of ceftazidime–avibactam for an approved indication per country label since 01 January 2018 (or launch date in the country if posterior) were enrolled. Effectiveness analyses were conducted in patients treated with ceftazidime–avibactam for at least 72 h. Results: Of the 569 eligible patients enrolled, 516 (90.7%) were treated for at least 72 h (354 patients from Europe and 162 patients from LATAM); 390 patients (75.7%) had switched from another antibiotic line for Gram-negative coverage. Infection sources were intra-abdominal, urinary, respiratory, bloodstream infections, and other infections (approximately 20% each). K. pneumoniae was the most common microorganism identified in the latest microbiological evaluation before starting ceftazidime–avibactam (59.3%). Two-thirds of microorganisms tested for susceptibility were MDR, of which 89.3% were carbapenem-resistant. The common MDR mechanisms for K. pneumoniae were carbapenemase (33.9%), oxacillinase 48 (25.2%), extended-spectrum beta-lactamase (21.5%), or metallo-beta-lactamase (14.2%) production. Without prior patient exposure, 17 isolates (mostly K. pneumoniae) were resistant to ceftazidime–avibactam. Treatment success was achieved in 77.3% of patients overall (88.3% among patients with urinary infection), regardless of first or second treatment line. In-hospital mortality rate was 23.1%. Adverse events were reported for six of the 569 patients enrolled. Conclusion: This study provides important real-world evidence on treatment patterns, effectiveness, and safety of ceftazidime–avibactam in clinical practice through its recruitment in the European and LATAM regions. Objectives: To generate evidence-based recommendations through formal consensus regarding the treatment of upper urinary tract infections during gestation. Materials and methods: Experts in microbiology, public health, internal medicine, infectious diseases, obstetrics, maternal fetal medicine and obstetric and gynecological infections participated in the consensus development group. The group also included professionals with training in clinical epidemiology, systematic data search, and representatives from the Health Secretariat and the Bogota Obstetrics and Gynecology Association. The participants disclosed their conflicts of interest. Starting with a clinical question, outcomes were graded and a systematic search was conducted in the Medline via PubMed, Embase, Lilacs, and Bireme databases. The search was expanded to include institutional repositories and antimicrobial resistance surveillance systems, with no language or date restrictions. The search was updated on October 1, 2022. The GRADE (Grading of Recommendations Assessment, Development and Evaluation) methodology was used to assess the quality of the evidence and determine the strength of the recommendations. Finally, the RAND/UCLA (Research and Development/University of California Los Angeles) methodology was applied for the formal consensus. This document was reviewed by academic peers before publication. Results: The following are the consensus recommendations. Recommendation 1. The initial management of pregnant women with upper urinary tract infections (UTIs) should be approached in a hospital setting. Recommendation 2. The use of second generation cephalosporins is the suggested first option for empirical antimicrobial management in pregnant women with upper UTI in order to improve the rates of clinical and microbiological cure. Recommendation 3. Because of the risk-benefit balance, the use of aminoglycosides is suggested as a second option for empirical antimicrobial treatment in pregnant women presenting with upper UTIs in the second and third trimester. Recommendation 4. The use of third-generation cephalosporins is suggested as the third option for empirical antimicrobial treatment in pregnant women with upper UTIs given that the risk of inducing microbial resistance is high with this group of antibiotics. Recommendation 5. The use of carbapenems is suggested as a first option in pregnant women with upper UTIs and a history of infections caused by microorganisms with resistance to third or fourth-generation cephalosporins. Recommendation 6. The use of aminoglycosides or fourth-generation cephalosporins is suggested as a second option in pregnant women with upper UTIs and a history of infection caused by microorganisms with resistance to third-generation cephalosporins, taking risk-benefit into account. Recommendation 7. The use of piperacillin/tazobactam is suggested as a third option in pregnant women with upper UTIs and a history of infection caused by microorganisms with resistance to third or fourthgeneration cephalosporins. Recommendation 8. Getting a urine culture is recommended in pregnant women with upper UTIs before initiating empirical antimicrobial treatment. Recommendation 9. In pregnant women with upper UTIs, it is suggested to modify therapy in accordance with the results of the sensitivity test when the culture report shows resistance to the antimicrobial agent initiated empirically. Recommendation 10. In pregnant women hospitalized due to upper UTIs, it is suggested to switch to oral antimicrobial therapy after at least 48 hours of modulation of the systemic inflammatory response and the clinical signs of infection, and when tolerance to oral intake is adequate. Recommendation 11. In pregnant women with upper UTIs with no complications secondary to the primary infection, it is recommended to administer antibiotic therapy for a period of 7 to 10 days. Conclusions: It is expected that with this Colombian upper UTI consensus variability in clinical practice will be reduced. Antibiotic-resistant bacteria are currently an important public health concern posing a serious threat due to their resistance to the current arsenal of antibiotics. Uropathogens Escherichia coli (UPEC), Proteus mirabilis, Klebsiella pneumoniae and Enterococcus faecalis , antibiotic-resistant gram-negative bacteria, cause serious cases of prolonged UTIs, increasing healthcare costs and potentially even leading to the death of an affected patient. This review discusses current knowledge about the increasing resistance to currently recommended antibiotics for UTI therapy, as well as novel therapeutic options. Traditional antibiotics are still a part of the therapy guidelines for UTIs, although they are often not effective and have serious side effects. Hence, novel drugs are being developed, such as combinations of β-lactam antibiotics with cephalosporins and carbapenems. Siderophoric cephalosporins, such as cefiderocol, have shown potential in the treatment of individuals with significant gram-negative bacterial infections, as well as aminoglycosides, fluoroquinolones and tetracyclines that are also undergoing clinical trials. The use of cranberry and probiotics is another potential curative and preventive method that has shown antimicrobial and anti-inflammatory effects. However, further studies are needed to assess the efficacy and safety of probiotics containing cranberry extract for UTI prevention and treatment. An emerging novel approach for UTI treatment is the use of immuno-prophylactic vaccines, as well as different nanotechnology solutions such as nanoparticles (NP). NP have the potential to be used as delivery systems for drugs to specific targets. Furthermore, nanotechnology could enable the development of nano antibiotics with improved features by the application of different NPs in their structure, such as gold and copper NPs. Prostatic malakoplakia (MP) is rare, with only case reports and small series (< five patients) available in the literature. In this study we analysed an international multi‐institutional series of 49 patients with prostatic MP to more clearly define its clinicopathological features. The median age was 67 years and the median serum prostate‐specific antigen (PSA) was 7.5 ng/ml. MP was clinically manifest in most cases (28 of 45 patients with data available, 62%). Of 43 patients with detailed clinical history available, 21 (49%) had concurrent or metachronous malignancies (including prostate cancer). Diabetes or insulin resistance was present in 11 patients (26%). Additionally, three patients had a history of solid organ transplantation and one had HIV. Of note, six of 34 patients (18%) without concurrent prostate cancer had an abnormal digital rectal examination and/or lesions on magnetic resonance imaging (MRI) with prostate imaging reporting and data system (PIRADS) scores 4–5. The initial diagnosis was made on core biopsies (25 of 49, 51%), transurethal resection specimens (12 of 49, 24%), radical prostatectomies (10 of 49, 20%), Holmium‐laser enucleation (one of 49, 2%) and cystoprostatectomy (one of 49, 2%). Tissue involvement was more commonly diffuse or multifocal (40 of 49, 82%). Von Kossa and periodic acid‐Schiff stains were positive in 35 of 38 (92%) and 26 of 27 lesions (96%), respectively. Of note, two cases were received in consultation by the authors with a preliminary diagnosis of mesenchymal tumour/tumour of the specialised prostatic stroma. The present study suggests that prostatic MP is often associated with clinical findings that may mimic those of prostate cancer in a subset of patients. Introduction and hypothesis: Increased post-voiding residual volume (PVRV), known as covert postpartum urinary retention (PUR), is an asymptomatic condition with unknown long-term adverse effects. The objectives were to determine the frequency of this phenomenon 3 days after delivery and to examine the associated risk factors and consequences of the increased residuum on women´s health 6 weeks postpartum. Methods: We carried out a prospective observational study including a total of 926 primiparous women, giving birth to singletons. All participants underwent ultrasound determination of PVRV on day 3 postpartum. Then, risk factors were determined using logistic regression analysis. After 6 weeks, participants were invited to return for PVRV determination and to complete urogynecological and general health questionnaires. Using these data, the consequences of increased PVRV were determined. Results: A total of n=90 women were diagnosed with abnormal PVRV. Mean age in the studied population was 30.4 years, BMI prior to delivery 27.8, weight of the newborn 3,420 g, and percentage of cesarean sections 15.9%. Gestational week (p=0.043), vaginal tear (p=0.032), and induction of labor (p=0.003) were risk factors for covert PUR. Puerperal incidence of urinary tract infection was 1.1% (6 out of 526) and of urinary incontinence 29.2% (155 out of 530), with no differences between the groups. In the second examination, covert PUR was no longer present, and the values of residual urine decreased for all patients in the case group. No statistically significant differences were observed in questionnaire scores in general health and wellbeing perceptions between the groups. Conclusions: We have found a few significant obstetrical–pediatric risk factors for abnormal PVRVs. Data from the follow-up suggest that covert PUR has no impact on morbidity and quality of life 6 weeks postpartum. Therefore, abnormal PVRV is a self-limited phenomenon with a tendency toward self-correction. Our findings support those of previous studies that advocate against screening for asymptomatic retention in the postpartum period, despite some similar previous recommendations. Urinary culture sensitivity after antibiotics administration is unknown. This study aimed to describe the diagnostic sensitivity of urine cultures from patients' first, second, and third micturition samples after a single dose of empirical antibiotics given for upper and/or febrile urinary tract infections, as well as searched for factors influencing diagnostic sensitivity over time. We collected consecutive urine samples from adult patients with an upper or febrile urinary tract infection diagnosed at four secondary hospital emergency rooms. One sample was collected before a first dose of empirical antibiotic treatment and up to three samples were collected from consecutive postadministration micturition. The main outcome was the number of positive cultures growing uropathogens with ≥103 colony forming units (CFUs) for men and ≥104 for women. Identical analyses were performed for any identified CFU and ≥105 CFU cut-off points. Time between antibiotic administration and first negative urinary culture was noted, which could have been at the time of any of the three postantibiotic urine samples. We used a Cox regression analysis for age- and sex-adjusted analyses. A total of 86 of 87 patients' preantibiotic cultures (99%) were positive compared with 26 of 75 (35%; p < 0.001), 15 of 50 (30%; p < 0.001), and 1 of 15 (7%; p < 0.001) of the first, second, and third postantibiotic samples, respectively, and missing 14 of 21 (67%), 13 of 17 (76%), and 7 of 7 (100%) of uropathogens with antibiotic resistance, respectively. The times needed for 25%, 50%, and 75% of cultures to be negative were 1.5, 2.9, and 9 hours, respectively, after antibiotic administration. Older age, male sex, non- Escherichia coli pathogens, urinary tract disease, comorbidity burdens, and urinary catheters prolonged time to negative culture, but were not significantly associated after adjustment. Uropathogens were found at ≥105 CFU in 15 of 75 (20%), 7 of 50 (14%), and 0 of 15 (0%) of the three postantibiotic micturition samples, respectively, and in any identified CFU in 48 of 75 (64%), 23 of 50 (46%), and 1 of 15 (7%), respectively. Acute pyelonephritis (APN) is a bacterial infection resulting in kidney inflammation, typically arising as a complication of an ascending urinary tract infection that ascends from the bladder to the kidneys. Clinical diagnosis is generally based on clinical and laboratory findings. Recent guidelines recommend not performing diagnostic imaging unless a complicated APN is suspected or the infection affects high-risk patients such as the elderly, immunocompromised individuals, or diabetics. Contrast-enhanced ultrasound (CEUS) is a valuable tool in both the diagnosis and follow-up of APN. It aids in distinguishing small simple nephritic involvement from abscess complications and monitoring their evolution over time during antibiotic therapy. Given its lack of ionizing radiation and nephrotoxicity, CEUS is a valid diagnostic modality for approaching and monitoring pyelonephritis, improving early identification and characterization of inflammatory lesions. Introduction and hypothesis: Intravesical antimicrobials (IVA) provide a localized modality of treatment for recurrent urinary tract infections (rUTIs). Owing to the sporadic use of these treatments, we conducted a systematic review on the efficacy of IVA in the management of uncomplicated rUTIs. Methods: A systematic review was conducted for all English language articles from inception to April 2021 utilizing the Cochrane and Preferred Reporting Items for Systematic Reviews and Meta-Analyses standards with the following databases: PubMed, OVID Embase, Biomed Central, and Scopus. References were cross-examined for further articles. Risk of bias was assessed in the articles included using the Cochrane and Joanna Briggs Institute tools. Results: The initial search resulting in 476 titles led to 15 full-text articles. Of the 13 in the final review (2 RCTs), 3 used gentamicin and 10 used hyaluronic acid IVA. These included 764 participants, mostly women, with a mean age range of 27–80 (median: 53.1). There was a reduction in UTI frequency in 12 out of 13 studies, with 10 studies showing a statistically significant decrease. Dosages of 80 mg of gentamicin per instillation and both 40 mg and 800 mg of hyaluronic acid per instillation were found to be effective in reducing the frequency of UTIs in most studies. Eleven participants reported gentamicin-resistant infections after IVA treatment. Despite high levels of bias in selected categories, the 13 studies were designated to be of high quality for inclusion. Conclusions: The IVAs gentamicin and hyaluronic acid with chondroitin sulphate demonstrated efficacy in the management of uncomplicated rUTIs, mostly in women. [ABSTRACT FROM AUTHOR]

The current study was conducted to analyze the protective effect of rosemary leaves extract on renal defect experimentally induced by infecting with E. coli O157. Sixty male mice were divided into 6 groups as follow: C-(control negative), C+ (infected with E.coli O157 without treatment), T1(Immunomodiolater group), T2 (infected with E. coli 0157 then treated with Rosemary Alcoholic extract at 0.5ml/20mg B.W. orally, T3 (infected with E coli 0157 then treated with meropenem 90 mg/kg.bw only and T4 (Infected with E.coli 0157 then treated with meropenem and Rosemary Alcoholic extract). The result showed significant decrease (P £ 0.05) in serum creatinine in groups treated with plant extract alone when administered as protective and as immunomodulatory. Furthermore, there were maximum depletion of serum urea levels which were significant (P £ 0.05) in the group treated with plant as treatment and also in the group treated with combination of plant extract with meropenem drug. Groups treated with meropenem and those treated with Rosemary extract alone (before infection), showed significant depletion in urea level in comparison to control +ve. Histopathological slides revealed vascular degeneration of most cortical renal tubules in kidney of animals (C+ group). While T1 group showed the presence of nephrosis associated with dilation of collecting tubules. In addition there were mild cloudy swelling of cortical renal tubules in animals of T2 group. Finally normal appearance of renal cortex was noticed in kidneys of T3 group. In conclusion, the present results provide a clear experimental evidence for the promising role of Rosmarinus officinalis leaves extract in the protection against E.coli O157. [ABSTRACT FROM AUTHOR]

Background: Acute obstructive pyelonephritis due to urolithiasis represents a medico-surgical emergency that can lead to life-threatening complications. There are still no established factors that reliably predict progression toward acute pyelonephritis in patients presenting with a simple renal colic. Objective: To investigate clinical and paraclinical factors that are associated with the onset of acute obstructive pyelonephritis. Methods: Patients presenting to the emergency department for renal colic with obstructive urolithiasis on imaging were enrolled in the study. Demographic data, vital signs, medical comorbidities, blood test results, urinalysis, and radiological findings were recorded. Obstructive pyelonephritis was defined by the presence of two or more of the following criteria: fever, flank pain or costovertebral angle tenderness, and a positive urine culture. Results: Seventeen patients out of 120 presenting with renal colic, were diagnosed with acute obstructive pyelonephritis (14%). Parameters that were associated with the onset of obstructive pyelonephritis were: diabetes (p = 0.03), elevated CRP (p = 0.01), stone size (>5 mm) (p = 0.03), dilatation of renal pelvis (p = 0.01), peri-renal fat stranding (p = 0.02), and positive nitrites on urinalysis (p < 0.01). Hyperleukocytosis, acute kidney injury, multiple stones, pyuria (>10/mm3), hypertension, and were not associated with the onset of obstructive pyelonephritis. Conclusion: This study showed that known diabetic status, elevated CRP, positive urine nitrites, stone size (>5 mm), pyelic dilatation, and peri-renal fat stranding were associated with the onset of pyelonephritis in patients presenting to the emergency department with obstructive urolithiasis. Background: Antimicrobial resistance (AMR) is a major global health threat. In small animals such as dogs and cats, antimicrobials are most commonly prescribed for skin and genitourinary diseases; therefore, the AMR of bacteria involved in these infections should be monitored. In addition, the results of antimicrobial susceptibility testing (AST) may be interpreted as a local epidemiological history of AMR. The Preventive Veterinary Medicine Laboratory (PVML) received clinical samples from dogs and cats for bacterial isolation and AST. Thus, this study aimed to assess the AMR of bacteria isolated from the samples of dogs and cats received at the Preventive Veterinary Medicine Laboratory (PVML). Materials, Methods & Results: Data from bacteriological examinations performed at the PVML of the Universidade Federal do Rio Grande do Sul (UFRGS) during 5 years were analyzed. Skin and ear canal samples were inoculated in 5% sheep blood agar, and urine samples were streaked on CHROMagar™ orientation. After incubation at 36±1°C for up to 72 h, identification and AST were performed according to routine protocols. Of 1,534 samples submitted to the PVML, 1,086 (70.8%) were collected from dogs and 29.2% from feline patients. Otological swabs (n = 533, 49.1%) were the most frequent samples from dogs, while cat urine samples (n = 384, 84.8%) predominated by far. Considering the canine samples, no bacterial growth (NBG) was observed in 443 (40.8%) samples, while only one colony type was noted in 516 (47.5%) samples. Gram-positive bacteria (n = 298) were more frequent than gram-negative bacteria (n = 77) in the skin. In urine samples, gram-negative bacteria (n = 94) were isolated more frequently than gram-positive bacteria (n = 47). In feline samples, a high number of NBG (n = 308, 68%) was observed. Gram-positive (n = 22) was predominant in comparison to gram-negative bacteria (n = 9) in cultures from the ear and skin swabs. Enterococcus spp. and Escherichia coli were the most frequently identified bacteria in urine samples. Among the Staphylococcus sp. strains of any origin, AMR frequency varied from 4.22% (amikacin) to 50.70% (sulfa/trimethoprim). Enterococcus spp. showed AMR frequencies from 12.5% (amoxicillin/clavulanic acid) to 62.06% (enrofloxacin). Among the gram-negative genera, E. coli presented AMR frequencies from 10.20% (gentamicin) to 60.0% (neomycin). The frequency of AMR was stable over time, and a profile of much higher resistance to fluoroquinolones in comparison to beta-lactams was observed. Discussion: The recurrence of skin and urinary infections implies the need for frequent treatment with antibiotics, which exerts selection pressure for resistance and multidrug resistance. In this study, the frequency of multidrug resistance was low, and the resistance to the tested antimicrobials showed high variation. However, a trend of high resistance to the fluoroquinolone group was observed in contrast to the low resistance to beta-lactams. This trend was consistent among the isolated bacteria, regardless of the type of sample or origin. The overprescription of fluoroquinolones in small animal practices has been widely documented in several countries. However, this class of antimicrobials, is highly prioritized for the treatment of infections in humans. Therefore, the selection of resistant strains has gained special emphasis, especially when considering the possibility of the transmission of resistant bacteria between pets and humans. In summary, the results of bacteriological tests conducted at the PVML-Universidade Federal do Rio Grande do Sul confirmed that ubiquitous bacteria predominate in clinical samples of dogs and cats. The high frequency of resistance to the fluoroquinolone group, while a predominance of susceptible strains in the first-choice drugs such as amoxicillin/clavulanic acid, may indicate excessive and empirical use of the second-choice drugs in clinical practice. Objective: To determine the emerging antibiotic resistance pattern of adult patients of MDR UTI cases caused by E.Coli. Study Design: Retrospective Data. Setting: Departments of Medicine and Obstetrics & Gynecology, Madinah Teaching Hospital / University Medical and Dental College, Faisalabad. Period: August 2016 to Sep 2018. Material & Methods: This retrospective study included 187 patients meeting the operational definition of multi-drug resistance cases of urinary tract infection caused by E. coli. All cases either male or female of age above 15 years with positive reports of urine culture and sensitivity for E.coli were included in the study. Along with the demographic variables urine WBC's, results of urine culture and sensitivity were noted. Antibiotic sensitivity pattern of all the E.coli resistant UTI cases was analyzed. After identification of the microbe, antimicrobial sensitivity was accessed using Kirby-Bauer disc diffusion method on the Mueller-Hinton agar. The antimicrobial susceptibility test was performed against E. coli strains by using the following antibiotics: Piperacilin, Amoxicilin- Calvulanic, Pipmedic Acid, Cefipime, Ceftrixone, Cefuroxime, Cefoperazone-sulbactam, Ceftazidime, Cephradine, Ciprofloxacin, Ofloxacin, Levofloxacin, ofloxacin, Nitrofurantoin, Amikacin, Tombramycin, Gentamicin, Linezolid, Doxycycline, Sulphamethoxazole, Imipenem, Meropenem, Aztreonam in order to obtain an antibiogram. Results: In our data, 66.8% (n=125) were MDR UTI, and more than half of these cases were 63.2% (n=79) were females. Among the total 187 cases, 97.3% patients were resistant to most of the beta-lactam antibiotics, 95.7% were resistant to most of the quinolones and 68.4% were resistant to most of the aminoglycosides. Conclusion: This increasing antibiotic resistance is very alarming and steps should be taken to reduce the misuse of antibiotics. Studies should be done to control the risk factors leading to E. coli UTI. By following proper pharmacological guidelines, principles to use antibiotics and manage outpatient cases of UTIs, the incidence of MDR UTI can be controlled. [ABSTRACT FROM AUTHOR]

Those using intermittent catheterization (IC) have many catheter types and styles to select from; including what manufacturers refer to as discreet catheters. Discreet catheters are often manufactured to be smaller in length, coiled in design, or foldable, thus allowing for easier concealment or discretion when carried in public and offering a degree of personal privacy. This study explores discreet catheter use and its alignment with the lifestyle experience of those who use such catheters. This study assesses the user and user experience with discreet catheters. User experience is defined as catheter choice, associated activities, and urinary tract health. A cross‐sectional survey investigated life and health status of individuals in the Netherlands requiring IC. The study enrolled 1892 individuals of which 784 use a discreet catheter as their primary catheter. Catheter choice: 82% of study respondents state they have the ability to choose the type of catheter they use. Of those indicating not having the ability to choose their catheter, they state their health care professional did not inform them of different catheters, brands, or options. Catheter associated activities: Those employed full or part time are satisfied with their discreet catheter (95.1%) claiming it is discreet, and gives them confidence to live their life the way they choose; that the catheter did not interference with social activities (72.6%), and had little to no interference with work outside of the home (76.1%). Urinary tract infections (UTI): 39.4% report a clinician diagnosed UTI within 6 months prior to the survey. The data indicates that 18.3% of the sample are deficient in UTI knowledge stating they do not know what a UTI is, or what associated symptoms are; 41% followed a clinician suggested regimen for catheterizing; 25.5% altered catheterization schedules to fit daily activities; 33.3% stated they did not follow a regimen, but catheterized when convenient The data supports positive experiences for individuals who use discreet catheters as their primary method of bladder voiding, particularly socially active individuals requiring an element of personal privacy in their life. Organ abscesses caused by Streptococcus anginosus are relatively rare. We report the case of an elderly woman with splenic abscess caused by S. anginosus bacteremia after urinary tract infection. An 82-year-old woman had a history of frequency of urination, urgency, and fever with chills for over 10 days prior to admission. An abdominal computed tomography (CT) scan performed in the emergency room revealed a low-density lesion in the spleen, kidney cysts, some exudation around the kidney, and cystitis should be valued. She was treated with ceftriaxone and imipenem/cilastatin. After admission, the blood culture yielded positive results for S. anginosus. A contrast-enhanced abdominal CT scan showed that the low-density lesion previously found in the spleen was smaller than before. After percutaneous drainage of the splenic abscess and treatment with piperacillin/tazobactam based on the antibiotic sensitivity pattern, repeated abdominal CT scan revealed a significant reduction in the low-density lesion. The patient was discharged without recurrence or complications. A systematic review of organ abscess caused by S. anginosus bacteremia was performed. To our knowledge, there has been no report of splenic abscess caused by S. anginosus bacteremia secondary to urinary system tract infection, although urinary tract infections are also an important source.

Urinary tract infection is one of the most common indications for prescribing antibiotics among otherwise healthy patients. The lifetime risk of having urinary tract infection among the female population reaches up to 50%. Empirical antibiotic therapy is the first line of treatment in uncomplicated cases. Bacterial resistant to antibiotics is constantly increasing and it is resulting in the evolvement of multidrug-resistant pathogens but also causes a limitation in empirical treatment options. Due to growing antimicrobial resistance, local antimicrobial susceptibility patterns should always be considered especially when empirical treatment is introduced. The article shows the ongoing need for updates in local resistance patterns. Recommendations for the empirical treatment should be constantly verified with reference to the local antimicrobial resistance status. Non-antibiotic preparations may be useful only in the prevention of urinary tract infections.

Renal emphysema, as described by Kelly and MacCallum in 1898, refers to the spontaneous generation of gas within the renal parenchyma and surrounding tissues. Since its initial description, it has become apparent that the spectrum of radiologically visible renal and perirenal gas includes three distinct clinical entities: (1) emphysematous pyelonephritis, a necrotizing infection associated with gas formation in the renal parenchyma, (2) emphysematous pyelitis, in which gas is confined to the renal pelvis and calyces, and (3) gas-forming perinephric abscess. In this article, we will review gas-forming infections of the urinary system in terms of radiological features, clinical manifestations, predisposing factors, and appropriate management guidelines.

Introduction: Self-catheterisation (CIsC) is the gold standard treatment for bladder emptying disorders. A frequent complaint of patients undergoing CIsC is urinary tract infection (UTI). However, the epidemiology of UTIs remains poorly documented, particularly in the urological population. The aim of our study was to establish the epidemiology of infectious complications of CIsC., Method: A prospective, descriptive cohort study was carried out on a population educated in CIsC in a urology outpatient department of a university hospital., Results: From January 1, 2019 to November 15, 2020, 411 patients completed a CIsC education session. Sixty patients could be included and integrated for analysis. The mean age was 58.6±16.3years. Among the patients, 68% had a neurological pathology. The most common bacteria found was Escherichia coli. The incidence of total UTIs within the first 6weeks was 18%. After a mean follow-up of 15±6.5months, the median number of UTIs was 0 [0; 4]. The mean interval between two infectious episodes was 9±6.7months. Only one patient met the criteria for recurrent UTI. Introduction. Colorenal fistula is rare in the pediatric population. It may occur at any segment involved by ischemia, chronic inflammation, or necrosis. It is typically associated with a preliminary renal lesion that may arise as a result of interventional procedures, inflammatory conditions, colon tumor, and xanthogranulomatous pyelonephritis, among others. Clinical case. 15-year-old female patient diagnosed with acute lymphoblastic leukemia admitted at our institution for baseline condition management. During her stay, she experienced gastrointestinal and urinary infectious events. In the assessment and management of those, a left colorenal fistula was found. Surgical treatment was decided upon. Discussion. Colorenal fistula typically occurs secondary to renal inflammation or infection. There are growing concerns regarding the rise of antimicrobial-resistant bacteria in companion animals. This study aimed to bring new insights into the current scenario of Portugal’s antimicrobial resistance bacteria isolated from companion animals with urinary tract infections and is the first to be performed during a long period on a large scale. Of a total of 17472 urine samples analyzed, 12,166 (69.6%) (CI 12,200–12,200) were negative for bacterial growth, and 5306 (30.4%) (95% CI 5310–5310) had bacterial growth. Of the culture-positive samples, 5224 (96.6%) (95% CI 5220–5220) were pure cultures and 82 (3.2%) (95% CI 81.9–82.1) had mixed growth. Escherichia coli was the most frequently isolated bacteria (n = 2360, 44.5%) (95% CI 2360–2360), followed by Proteus mirabilis (n = 585, 11%) (95% CI 583–583), Enterococcus faecium (n = 277, 5.2%) (95% CI 277–277) and Staphylococcus pseudintermedius (n = 226, 4.3%) (95% CI 226–226). The overall susceptibility rates were low for erythromycin (45.3%) and clindamycin (51.3%), and high for aminoglycosides (96.3%), carbapenems (92.4%), trimethoprim-sulfamethoxazole (81.2%), and quinolones (79.9%). E. coli also showed considerable resistance to amoxicillin-clavulanic acid. The rates of multidrug-resistant bacteria are still high compared to the northern countries of Europe. This study’s findings show the emergence of antibiotic resistance in the antibiotic agents commonly used in the treatment of UTIs in dogs and cats in Portugal.

Abstract The purple urine bag syndrome is an underrecognized but quite common condition, resulting in marked discoloration of the urine bag system due to bacterial metabolism. This syndrome is associated with advanced age and bedridden persons.

Bioactive compounds and phenolic compounds are viable alternatives to antibiotics in recurrent urinary tract infections. This study aimed to use a natural functional product, based on the bioactive compounds' composition, to inhibit the uropathogenic strains of Escherichia coli. E.coli ATCC 25922 was used to characterize the IVCM (new in vitro catheterization model). As support for reducing bacterial proliferation, the cytotoxicity against a strain of Candida albicans was also determined (over 75% at 1 mg/mL). The results were correlated with the analysis of the distribution of biologically active compounds (trans-ferulic acid-268.44 ± 0.001 mg/100 g extract and an equal quantity of Trans-p-coumaric acid and rosmarinic acid). A pronounced inhibitory effect against the uropathogenic strain E. coli 317 (4 log copy no./mL after 72 h) was determined. The results showed a targeted response to the product for tested bacterial strains. The importance of research resulted from the easy and fast characterization of the functional product with antimicrobial effect against uropathogenic strains of E. coli. This study demonstrated that the proposed in vitro model was a valuable tool for assessing urinary tract infections with E. coli. [ABSTRACT FROM AUTHOR]
