Introduction: Temporal bone dissection is an acceptable method of developing the necessary skills required to successfully carry out surgical procedures on the temporal bones. It is the gold standard for the acquisition of otological/neuro-otological skills among otorhinolaryngological residents training globally. Very few graduate schools in Nigeria have the facilities and expertise to train graduate doctors in temporal bone dissection. The West African College of Surgeons Faculty of Otorhinolaryngology collaborated with Jos University Teaching Hospital in conjunction with Professor Jack Van Doren Hough Temporal Bone Dissection Laboratory to provide this temporal bone dissection course. Aim: The aim of this study was to develop the basic skills and dexterity necessary for otological/neuro-otological surgeries through temporal bone dissection. Participants and Methods: Twenty-three course participants who are trainees with the West African College of Surgeons Otorhinolaryngology Specialties registered for this course, out of which all of the 23 participants presented for the course from various teaching hospitals across the country. Results: Fourteen participants (60.9%) had no prior experience with temporal bone dissection, while 9 (39.1) had previous dissection experience. Twelve participants (52.2%) successfully mounted their temporal bones on the House–Urban temporal bone holder with two attempts, 5 (21.7%) mounted it on the 1
st and 3rd attempts, respectively, while 1 (4.3%) mounted it on the 4th attempt. Conclusion: Temporal bone dissection with standard equipment in a setting akin to the operating room helps otorhinolaryngology residents to learn the anatomy of the temporal bone. [ABSTRACT FROM AUTHOR]- Published

This work describes the fabrication and characterization of a Micro-Electro-Mechanical System (MEMS) sensor for gas sensing applications. The sensor is based on standard PolyMUMPs (Polysilicon Multi-Users MEMS Process) technology to control the temperature over the sensing layer. Due to its compact size and low power consumption, micro-structures enable a well-designed gas-sensing-layer interaction, resulting in higher sensitivity compared to the ordinary materials. The aim of conducting the characterization is to compare the measured and calculated resistance values of the micro-heater and the temperature sensor. The temperature coefficient of resistance (TCR) of the temperature sensor has been estimated by raising and dropping the temperature throughout a 25–110 °C range. The sensitivity of these sensors is dependent on the TCR value. The temperature sensor resistance was observed to rise alongside the rising environmental temperatures or increasing voltages given to the micro-heater, with a correlation value of 0.99. When compared to the TCR reported in the literature for the gold material 0.0034 °C−1, the average TCR was determined to be 0.00325 °C−1 and 0.0035 °C−1, respectively, indicating inaccuracies of 4.6% and 2.9%, respectively. The variation between observed and reported values is assumed to be caused by the fabrication tolerances of the design dimensions or material characteristics.

- Abstract
Background: Globally, every minute, at least one woman dies from complications related to pregnancy or childbirth. Knowing the obstetric danger signs helps in avoiding the first delay. This is because early recognition of complications is a critical determinant of timely decision-making. Objectives: This study was conducted to assess the knowledge of obstetric danger signs (ODS) and its determinants among women of reproductive age in a rural community in north-western Nigeria. Methods: A cross-sectional, community based descriptive study was conducted in Kwaba as part of a Community Diagnosis field practical experience for final year medical students of Ahmadu Bello University, Zaria, from 19th September to 14th October 2016. An interviewer- administered questionnaire was used to collect data from 395 women of reproductive age in the community. Data was analyzed using SPSS version 20. Results: Among the 395 respondents, 373 (94.4%) were married and prolonged labor was the most known ODS (316/395, 80%). A majority of the respondents, 286 (72.4%), had good knowledge of ODS. Determinants of having a good knowledge of ODS were: petty trading profession (OR= 1.65, 95% C.I: 1.04 – 2.64, P=0.03); formal education (OR=1.97, 95% CI: 1.24-3.01, P=0.01) and ever been pregnant (OR=4.23, 95% CI: 1.90 - 9.42, P < 0.01). Conclusions: Formal education, ever been pregnant and petty trading profession were determinants of having a good knowledge of ODS among respondents. Increased enrolment of children into formal education; increased antenatal care attendance and health education of both men and women on ODS in the study area is recommended. This may help in avoiding delay in seeking for emergency obstetric care services and may reduce maternal mortality in the community.

- Abstract
Schiff base derived from phenylalanine and vanillin and its corresponding binuclear Cu(II) and Zn(II) complexes have been synthesized by mechanochemical synthesis. The composition and structure of the complexes were characterized by elemental analysis, Infrared spectroscopy, ESI- mass spectrometry, thermogravimetric analysis, conductivity, and magnetic susceptibility measurements. The presence of a new band at 1660 cm-1 in the IR spectrum of the ligand due to ʋ(C=N) co nfirms the formation of the Schiff base and the shifts of this band (1660 cm-1) to higher frequencies in the spectra of the metal(II) complexes indicate complexation. The experimental results showed that the Schiff base binds to the metal ion in a tridentate (O, N, O) manner, resulting in the formation of binuclear metal(II) complexes. The copper and zinc ions are all six coordinated and coordinates through the azomethine nitrogen, carboxylic and phenolic oxygen in addition to oxygen from three coordinated water molecules respectively. The copper(II) and zinc(II) complexes have the general formula[M2L3(H2O)6] xH2O (where x = 3 or 0 and M = Cu or Zn). ESI- MS analysis showed m/z =376.03, 1166.77 and 1171.44 correspond to singly charged ions which confirms the molecular weight of the Schiff base and the binuclear Cu(II) and Zn(II) complexes respectively. The molar conductance of the complexes measured is low in the range 20.01-23.68 Ω-1 cm2 mol-1, indicating their non-electrolytic nature. TGA analysis showed the gradual loss of water of hydration, coordinated water molecules, followed by the stepby- step decomposition of the Schiff base into its metal oxide residue. The magnetic susceptibility measurements indicate a diamagnetic binuclear Zn(II) complex, while Cu(II) complex was found to be paramagnetic. The percentage yield of the synthesized compounds was found to be greater than 90%. The anti-fungal and antibacterial activities of the Schiff base and their corresponding metal complexes were investigated in vitro using the disc diffusion method against Staphylococcus aureus, Streptococcus pneumoniae, Salmonella typhi, Escherichia coli, Tinea capitis, and Trichophyton rubrum. Both Schiff base and metal complexes were found to be active against all selected bacterial and fungal species but a much enhanced activity was observed for the complexes.

A mixed methods study was undertaken in the Kachia Grazing Reserve of northern Nigeria. Surveys in March, June and October 2011 included focus group discussions, key informant and in-depth household interviews, concerning livelihood practices, animal health, ownership, and productivity. In May 2011, 249 Fulani families fleeing post-election violence entered the reserve with their livestock, increasing the number of households by one third.Despite being settled within a grazing reserve, over half of households sent all their cattle away on seasonal transhumance and another third sent some away. Cattle accounted for 96% of total tropical livestock units (TLU), of which 26% were cattle kept permanently outside the reserve. While all households cited livestock as their main source of income, 90% grew crops and 55% derived income from off-farm activities. A multiple correspondence analysis showed that for each extra member of a household its TLU value increased by 2.0 [95% CI, 1.4-2.7], while for each additional marriage its TLU increased by 15.7 [95% CI, 7.1-24.3]. A strong association was also observed between small herds, small households with only one wife, alongside marked geographical wealth differences within the reserve. New immigrant families had larger household sizes (33) and livestock holdings (122 TLU) than old settlers (22 people and 67 TLU). Prior to the mass immigration, the distribution of TLU per person was unimodal: 41% of households were classified as 'poor' and 27% as 'medium', whereas post-immigration it was bi-modal, with 26% classified as 'very poor' and 28% as 'medium'.While cattle remain the principal source of Fulani income and wealth, the inhabitants of Kachia Grazing Reserve have diversified their livelihood strategies to respond to changing circumstances and stress, especially the limited availability of grazing within the reserve and political insecurity outside, resulting in continued transhumance, the maintenance of smaller livestock holdings and pushing households into poverty.

- Abstract
The study was carried out to assess the adoption of family planning methods among rural women in Kaduna State. The study revealed that over 85% of the respondents were in the productive age of between 15 - 44 years with about 60.28% in a polygamous home. The main sources of information used by the respondents were health officials (21.08%), radio (20.48%) and friends (12.65%). The results further disclosed low use of modern family planning methods (38.28%); mainly preferred use of traditional methods of family planning (61.72%) and preferred use of modern methods of family planning (38.28%). Prolonged lactation (28.70%), abstinence (21.29%) and withdrawal method (18.06%) ranked first, second and third, respectively, among traditional family planning method mainly used by the respondents. Of those using modern methods, 35.07% used oral contraceptives while 26.12% of the respondents used the condom. Majority (22.00%) of the respondents reported the perceived religious prohibition against family planning, lack of approval by husband (19.14%) and fear of side effects associated as the main constraints faced in the adoption of family planning in the study area. The study recommended raising the level of both spouses in the understanding of positive aspects of family planning methods by the public health workers through use of trusted scholars via governmental and non-governmental agencies.

20. Validity of exploring a head-injured patient with a unilateral fixed dilated pupil: A case report
- Author

- Abstract
Background: Globally, head injury is a leading cause of morbidity and mortality among the young productive age group. Expanding Extradural haematoma is potentially a life-threatening condition, and could leave a surviving patient with a life-long disability. Usually, the clinical diagnoses supported by skull X-ray showing fracture are confirmed with Computed Tomography Scan (CT Scan) when available. When CT Scan is not available but all evidence is pointing towards an Extra Dural Haematoma, can one still go ahead with the Exploration? Case Summary: We present a 27-year-old man with a 21-hour history of Motor Vehicle Accident, loss of consciousness, Lucid interval and right hemiparesis. No seizure or vomiting. Other systems were essentially normal. His Glasgow Coma Scale score was 10/15 (EO=2, BVR=3, BMR=5). He had Left-Sided Fixed Dilated Pupil (FDP). Skull X-ray revealed a left-sided comminuted fracture involving the temporal, parietal, and occipital bones. CT Scan was not available (non-functional at the time). Based on the clinical diagnoses supported by the fracture on the skull X-ray, he had an urgent Exploratory burr hole where Extra Dura Haematoma was found and evacuated. The postoperative period was uneventful. Conclusions: The availability and functionality of CT Scans are still a problem in Low and Medium Income countries. Therefore, an Exploratory burr hole to diagnose an intracranial haematoma and its evacuation may be valid.

- Abstract
The aim of this study was to assess the effect of Mercuric chloride on the growth and haematological parameters in the freshwater catfish, Clarias gariepinus. A total of 30 fishes were used for each concentration as well as in the control. Clarias gariepinus was exposed to 0.02, 0.04, 0.06, 0.08 and 0.10mg/l of HgCl2 for 56 days. The treatment with mercuric chloride was found to inflict a drastic reduction in the total count of RBC’s. The reduction was time dependent; as concentration of mercuric chloride increased, the RBC levels declined. Exposed fishes showed a significant decrease in WBC count when compared to the control. The morphological indices MCV, MCH and MCHC fluctuate as the test concentration increased. The chronic exposure to sublethal concentration of mercuric chloride to the studied fish showed a significant decrease in final body weight in comparison to control group. Also, Growth parameters such as specific growth rate (SGR), food conversion efficiency (FCE), protein efficiency ratio (PER), food conversion rate (FCR) decreased with increased concentration of mercuric chloride. The mercuric chloride caused a significant decrease in the survival rate (P < 0.05). The study recommends timely fish fed since time was a strong dependent variable in all the treatment, so as increase concentration of mercuric chloride increased.

AIMS Our aim was to report on a survey initiated by the European Association of Percutaneous Cardiovascular Interventions (EAPCI) concerning opinion on the evidence relating to dual antiplatelet therapy (DAPT) duration after coronary stenting. METHODS AND RESULTS Results from three randomised clinical trials were scheduled to be presented at the American Heart Association Scientific Sessions 2014 (AHA 2014). A web-based survey was distributed to all individuals registered in the EuroIntervention mailing list (n=15,200) both before and after AHA 2014. A total of 1,134 physicians responded to the first (i.e., before AHA 2014) and 542 to the second (i.e., after AHA 2014) survey. The majority of respondents interpreted trial results consistent with a substantial equipoise regarding the benefits and risks of an extended versus a standard DAPT strategy. Two respondents out of ten believed extended DAPT should be implemented in selected patients. After AHA 2014, 46.1% of participants expressed uncertainty about the available evidence on DAPT duration, and 40.0% the need for clinical guidance. CONCLUSIONS This EAPCI survey highlights considerable uncertainty within the medical community with regard to the optimal duration of DAPT after coronary stenting in the light of recent reported trial results. Updated recommendations for practising physicians to guide treatment decisions in routine clinical practice should be provided by international societies.

The incidence of diabetes has increased globally in recent years and figures of diabetic patients were estimated to rise up to 642 million by 2040. The disorder is accompanied with various complications if not managed at the early stages, and interlinked high mortality rate and morbidity with time. Different classes of drugs are available for the management of type 2 diabetes but were having certain limitations of their safety. Alphaglucosidase is a family of enzyme originated from the pancreas which plays a role in the anabolism of 80-90% of carbohydrate consumed into glucose. This glucose is absorbed into the blood and results in frank postprandial hyperglycemia and worsens the conditions of diabetic patients which precipitate complications. Inhibition of these enzymes helps to prevent postprandial hyperglycemia and the formation of glycated end products. Alphaglucosidase inhibitors are reported to be more important in adequate control of type 2, but marketed drugs have various side effects, such as poor patient compliance and also expensive. This proves the needs for other class of drugs with better efficacy, safety, patient compliance and economic. In this review, we have emphasized the recent advances in the field of new alpha-glucosidase inhibitors with improved safety and pharmacological profile.

Box-Behnken response surface methodology was employed to optimize the process variables in alkalinecatalysed transesterification of Lophira lanceolata seed oil into biodiesel following initial acid-catalysed esterification. The model equation obtained from regression analysis adequately expressed the relationship between the biodiesel yield and the process variables with R2 of 96.05%.The optimization result suggested the best combination of the process variables for optimum biodiesel yield of 85.0% are: reaction temperature (500C); reaction time (120min); amount of methanol (6 cm3/g oil) and catalyst concentration (0.30 mol/dm-3). Validation yield of 83% compares favorably with the predicted value, showing model reliability in describing the transesterification process. Results of GC-MS analysis of the biodiesel revealed it mainly consists of methyl esters of octadecenoic acid (69.5%), palmitic acid (9.4%) and hexacosanoic acid (5.6%). The biodiesel was very fluid 2.70±0.12 mm/s2 with most of it fuel properties in conformity with ASTM standards making it suitable as fuel for diesel engines. Keywords: Optimization, Lophira lanceolata, transesterification, Biodiesel, Behnken design.

- Abstract
This study examines the effect of foreign bank penetration on banking competition in Indonesia. Penetration of foreign banks is measured by using the ratio between total assets, total loans and total deposits of foreign banks to total assets, total loans and total deposits of the banking industry in Indonesia, such as those used by Levy and Micco (2007). Bank concentration is measured by using Concentration Ratio (CR3 and CR5) and the Herfindahl index. Bank competition is measured by using the Conjectural Variation approach as used by Cetorelli Angelini (2003). By using panel regression method with SUR (Seemingly Unrelated Regression) during 1998 to 2009, we found that the increase of foreign bank penetration will increase competition of banking in Indonesia through spillover effects on the domestic banking system. These results support the research of Jeon et al (2011).

Drug abuse, prevalence, determinants, youth, rural, Nigeria - Abstract
Drug abuse is a global public health problem that impacts negatively on health, family, society, educational and professional life. Majority of youths ignorantly depend on one form of substance or the other for various daily activities such as social, educational, political and moral. The objective of this study was to assess the prevalence and determinants of drug abuse among youths in a rural agrarian community in North Western Nigeria. A cross-sectional, community baseddescriptive study conducted among 359 rural youths aged 15-35 years. The study was conducted from 19th September 2016 to 14th October 2016.An interviewer-administered questionnaire was used to collect data which was analyzed using SPSS (version 20). The prevalence of drug abuse was 10% and the most abused drug was Tramadol (52.8%). There was a statistically significant association between drug abuse and the following variables: Type of family (P=0.025); occupation of respondents (P=0.001) and monthly income (P= 0.005). Farming occupation was a determinant of drug abuse. Farmers were 6.52 times more likely to abuse drugs than non-farmers (OR= 6.52, 95% C.I: 2.25 – 18.91,P=0.001). The result indicated that youth in the study area abuse drugs such Tramadol and Marijuana and farming occupation was a determinant of drug abuse. For effective control of drug abuse in the study area, there is need for health education campaigns on harmful effects of drug abuse.Key words: Drug abuse, prevalence, determinants, youth, rural, Nigeria

A pregnant primer cow reported to be in labor 12 hours prior to presentation was brought into the large animal clinic of the Veterinary Teaching Hospital of the Ahmadu Bello University, Zaria and was referred to the emergency unit of our facility. Examination revealed intermittent straining, serosanguinous discharge from the vulva, one fore left limb protruding from the vulva. Rectal palpation revealed an extra-large head (which felt conjoined) presented posteriorly with two separate oral cavities exhibiting suckling reflexes. Ultrasound confirmed a double rib cage with heartbeat emanating from two different locations. A tentative diagnosis of dystocia caused by fetal monster was made. Caesarian section was therefore carried out according to standard surgical procedure. The fetus died few hours later. The dam recovered post-surgery and calved a healthy calf two and a half years later. [ABSTRACT FROM AUTHOR]

Background A mixed methods study was undertaken in the Kachia Grazing Reserve of northern Nigeria. Surveys in March, June and October 2011 included focus group discussions, key informant and in-depth household interviews, concerning livelihood practices, animal health, ownership, and productivity. In May 2011, 249 Fulani families fleeing post-election violence entered the reserve with their livestock, increasing the number of households by one third. Results Despite being settled within a grazing reserve, over half of households sent all their cattle away on seasonal transhumance and another third sent some away. Cattle accounted for 96% of total tropical livestock units (TLU), of which 26% were cattle kept permanently outside the reserve. While all households cited livestock as their main source of income, 90% grew crops and 55% derived income from off-farm activities. A multiple correspondence analysis showed that for each extra member of a household its TLU value increased by 2.0 [95% CI, 1.4–2.7], while for each additional marriage its TLU increased by 15.7 [95% CI, 7.1–24.3]. A strong association was also observed between small herds, small households with only one wife, alongside marked geographical wealth differences within the reserve. New immigrant families had larger household sizes (33) and livestock holdings (122 TLU) than old settlers (22 people and 67 TLU). Prior to the mass immigration, the distribution of TLU per person was unimodal: 41% of households were classified as ‘poor’ and 27% as ‘medium’, whereas post-immigration it was bi-modal, with 26% classified as ‘very poor’ and 28% as ‘medium’. Conclusions While cattle remain the principal source of Fulani income and wealth, the inhabitants of Kachia Grazing Reserve have diversified their livelihood strategies to respond to changing circumstances and stress, especially the limited availability of grazing within the reserve and political insecurity outside, resulting in continued transhumance, the maintenance of smaller livestock holdings and pushing households into poverty.

NECROSIS ,NEWBORN infants ,HISTOLOGY - Abstract
Subcutaneous fat necrosis (SCFN) is a rare dermatologic condition which until recently was thought to occur only in term neonates. Its presentations are protean and early in the disease; it mimics other more common neonatal dermatologic conditions. Although benign, it runs a prolonged course and may be associated with some complications and therefore needs to be differentiated from other more life-threatening neonatal dermatologic conditions. We present a case of a term neonate with histology report highlighting some characteristic features of SCFN and the need to differentiate it from sclerema neonatorum (SN), a condition associated with fatal outcome.
