17. Mirror Movements and the Role of Handedness: Isometric Grip Forces Changes.

Abstract

Co-activation of both hemispheres seems to play a critical role in the generation of mirror movements, involuntary muscle contractions opposite to voluntary movements of one hand. The relationship between handedness and mirror movements is unclear. We measured repetitive grip force changes performed at slow and fast frequencies with the active hand and recorded mirror activity in the opposite inactive hand in 17 healthy left-handed (LH) and 17 right-handed (RH) participants. Data suggest significant differences between RH and LH with respect to the temporo-spatial characteristics of mirror activity, but not in general mirror movement strength. Mirror activity correlation was more pronounced when the non-dominant hand was active in RH, while LH did not show such an asymmetry. We suggest that the dependence of mirror activity on the active hand in right-handers but not in left-handers reflects the more general finding of stronger manual asymmetry in brain activation as well as in functional performance in right-handers. Abstract

Primary progressive aphasias (PPA), including the semanticvariant (svPPA), non-fluent variant (nfvPPA) and logopenic variant(lvPPA), are neurodegenerative diseases characterized by a pro-gressive decline in speach. Neuropathologically, svPPA and nfvPPAare assigned to frontotemporal dementia whereas lvPPA is mainlyassociated with Alzheimer0s disease (AD). Differential diagnosis ofPPAs is based on clinical symptoms and objective neurochemicalbiomarkers are highly appreciated to improve diagnostic accuracy.Here, we investigated a panel of biomarker candidates in cere-brospinal fluid (CSF) of PPA patients (n = 66) in a multicentercohort from the German FTLD consortium.There were no significant differences in CSF concentrations ofubiquitin (Kruskal-Wallis test, p = 0.50), progranulin (p = 0.12),Tau (p = 0.12) and pTau181 (p = 0.07) between PPAs. Ab42 wassignificantly reduced in lvPPA versus nfvPPA (p < 0.05) but notsvPPA and there was a great overlap between groups. The ratio ofthe AD biomarkers Ab42/pTau181 significantly differentiatedlvPPA from nfvPPA (p < 0.01) and svPPA (p < 0.05) and showeda sensitivity and specificity of 91.7% and 64.3% (lvPPA vs.nfv+svPPA). Neurofilament light chain (NfL) was significantlylower in lvPPA compared with nfvPPA (p < 0.01) and svPPA(p < 0.001) and sensitivity and specificity was 70.8% and 92.9%.Combination of NfL and AD biomarkers in the term Ab42*NfL/pTau181 improved diagnostic accuracy for the differentiation oflvPPA from nfvPPA and svPPA patients (sensitivity 87.5% andspecificity 85.7%). The phosphorylated neurofilament heavy chain(pNFH) was significantly increased in nfvPPA vs. svPPA (p < 0.05)and lvPPA (p < 0.01) and showed a sensitivity and specificity of80.0% and 64.3% to discriminate nfvPPA and svPPA.In conclusion, the combination of the AD biomarkers Ab42 andpTau181 with NfL is promising in the discrimination of lvPPApatients from nfvPPA and svPPA. CSF pNfH concentrations may assist in the differential diagnosis of nfvPPA and svPPA althoughadditional biomarkers are appreciated to increase sensitivity andspecificity of pNfH.

Abstract

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is an arteriopathy related to a genetic defect of the notch 3 gene on chromosome 19. The purpose of this study was to evaluate lesion distribution and volume using MR imaging and to correlate the lesion volume with the neurologic and neuropsychological findings.Twenty members of two families (14 with CADASIL as determined by linkage analysis, six healthy) were studied with MR imaging. Two observers evaluated the MR findings semiquantitatively and quantitatively. MR results were then correlated with neurologic and neuropsychological findings.A typical pattern of lesion distribution in patients with CADASIL was found: the frontal lobe was the site with the highest lesion load, followed by the temporal lobe and the insula. The total lesion volume on T1-weighted MR images correlated significantly with the degree of disability and the degree of impairment in neuropsychological functions (including attention, memory, and conceptual and visuospatial functions).In CADASIL patients, a common pattern of cerebral lesion distribution is found. The total T1 lesion volume is an important parameter to correlate with disability, as it may prove to be helpful in predicting the natural history of the disease.
