Abstract

Optimum nutrition plays a major role in the achievement and maintenance of good health. The Nutrition Society of the UK and Ireland and the Sabri Ülker Foundation, a charity based in Türkiye and focused on improving public health, combined forces to highlight this important subject. A hybrid conference was held in Istanbul, with over 4,000 delegates from 62 countries joining the proceedings live online in addition to those attending in person. The primary purpose was to inspire healthcare professionals and nutrition policy makers to better consider the role of nutrition in their interactions with patients and the public at large to reduce the prevalence of non-communicable diseases such as obesity and type 2 diabetes. The event provided an opportunity to share and learn from different approaches in the UK, Türkiye and Finland, highlighting initiatives to strengthen research in the nutritional sciences and translation of that research into nutrition policy. The presenters provided evidence of the links between nutrition and disease risk and emphasised the importance of minimising risk and implementing early treatment of diet related disease. Suggestions were made including improving health literacy and strengthening policies to improve the quality of food production and dietary behaviour. A multidisciplinary approach is needed whereby Governments, the food industry, non-governmental groups and consumer groups collaborate to develop evidence-based recommendations and appropriate joined up policies that do not widen inequalities. This summary of the proceedings will serve as a gateway for those seeking to access additional information on nutrition and health across the globe.

9. The Role of Family Factors and School Achievement in the Progression of Adolescents to Regular Smoking

Abstract

This study examines whether parental smoking and single parenting were related to adolescents' school achievement and anti-smoking parental practices as well as how these factors predicted later smoking. The sample comprised 1163 Finnish students in Grades 7 through 9. Results show that at the beginning of the seventh grade, parental smoking and single parenting were related to adolescents' lower levels of school achievement. Moreover, parental smoking had moderate association with lack of house smoking rules. At the beginning of the ninth grade, these associations were strengthened and lack of house smoking rules as well as loosened perceived parental punishment for smoking was related to both parental smoking and single parenting. The likelihood of ninth grade regular smoking was greater among adolescents whose parents smoked, who had no smoking rules in their homes and had substandard school achievement. These results suggest that smoking parents and single parents had similar anti-smoking regulations for their children at the baseline but once children became older smoking parents were not able to maintain these rules as successfully as non-smoking parents and families with two parents. Motivating parents to uphold these anti-smoking regulations offers a prospective intervention opportunity.

Abstract

Objective: Previous studies on the association between metabolic biomarkers and hypertension have been limited by small sample sizes, low number of studied biomarkers, and cross-sectional study design. In the largest study to date, we assess the cross-sectional and longitudinal associations between high-abundance serum biomarkers and blood pressure (BP). Methods: We studied cross-sectional (N = 36 985; age 50.5 ± 14.2; 53.1% women) and longitudinal (N = 4197; age 49.4 ± 11.8, 55.3% women) population samples of Finnish individuals. We included 53 serum biomarkers and other detailed lipoprotein subclass measures in our analyses. We studied the associations between serum biomarkers and BP using both conventional statistical methods and a machine learning algorithm (gradient boosting) while adjusting for clinical risk factors. Results: Fifty-one of 53 serum biomarkers were cross-sectionally related to BP (adjusted P < 0.05 for all). Conventional linear regression modeling demonstrated that LDL cholesterol, remnant cholesterol, apolipoprotein B, and acetate were positively, and HDL particle size was negatively, associated with SBP change over time (adjusted P < 0.05 for all). Adding serum biomarkers (cross-sectional root-mean-square error: 16.27 mmHg; longitudinal: 17.61 mmHg) in the model with clinical measures (cross-sectional: 16.70 mmHg; longitudinal 18.52 mmHg) improved the machine learning model fit. Glucose, albumin, triglycerides in LDL, glycerol, VLDL particle size, and acetoacetate had the highest importance scores in models related to current or future BP. Conclusions: Our results suggest that serum lipids, and particularly LDL-derived and VLDL-derived cholesterol measures, and glucose metabolism abnormalities are associated with hypertension onset. Use of serum metabolite determination could improve identification of individuals at high risk of developing hypertension.

Abstract Atherosclerotic cardiovascular diseases (ASCVDs) cause more deaths than any other disease group worldwide. However, it is possible to prevent the progression of atherosclerosis and the risk of ASCVD events by reducing the circulating concentrations of low-density-lipoprotein cholesterol (LDL-C). Large meta-analyses have demonstrated that decreasing the dietary intake of saturated fat and cholesterol can reduce the risk of ASCVD events. Here, we discuss the recent information regarding the prevention of subclinical atherosclerosis, ASCVD, and the risk of ASCVD events with a focus on dietary means. Plant stanols as fatty-acid esters were developed as a dietary adjunct to reduce LDL-C levels as part of a heart-healthy diet. They reduce cholesterol absorption so that less cholesterol is transported to the liver, the expression of LDL receptors is upregulated, and LDL-C concentrations are reduced on average by 0.40 mmol/L (12%) by consuming 3 g of plant stanols as esters/day. Using the equations of Keys, Anderson, and Grande, and the FINRISKI-counter equation, we also demonstrate that a heart-healthy diet combined with plant stanol ester consumption plausibly reduces the risk of coronary artery disease events by 23%. Thus, plant stanol esters as part of a heart-healthy diet seem to offer an effective means to reduce the risk of ASCVD events both at a population level and for subjects with different risk levels of ASCVD, when needed, in combination with statins.

38th European Photovoltaic Solar Energy Conference and Exhibition; 1601-1607, Green hydrogen will be an essential part of the future 100% sustainable energy and industry system. Up to one third of the required solar and wind electricity would eventually be used for water electrolysis to produce hydrogen, increasing the cumulative electrolyser capacity to about 17 TWel by 2050. Key method applied in this research are a learning curve approach for the key technologies, i.e. solar photovoltaic (PV) and water electrolysers, and levelised cost of hydrogen (LCOH). Sensitivities for the hydrogen demand and various input parameters are considered. Electrolyser capital expenditure (CAPEX) for a large utility-scale system is expected to decrease from current 400 €/kWel to 240 €/kWel by 2030 and to 80 €/kWel by 2050. With the continuing solar PV cost decrease, this will lead to an LCOH decrease in Europe from current 47-81 €/MWhH2,LHV (1.6-2.7 €/kgH2) to 31-54 €/MWhH2,LHV (1.0-1.8 €/kgH2) by 2030 and 16-27 €/MWhH2,LHV (0.5-0.9 €/kgH2) by 2050, depending on the location. The share of PV electricity cost in the LCOH will increase from the current 63% to 74% by 2050.

PV system prices are still expected to decrease by about 60% from 2020 to 2050. PV system and module price reduction is usually projected by applying historical learning rates (LR) on the main cost components. In this study, a detailed bottom-up analysis showed that the future PV module LR is likely to be close to the long-term historical average of 23.5% whereas it has been about 40% for the past decade. Module and cell efficiency is by far the most important cost component and it will cover about half of the future price reduction. Efficiency improvement will also decrease Balance of System (BoS) cost, Operational Expenditure (OPEX) and Levelised Cost of Electricity (LCOE).

Abstract

Background: To help adapt cardiovascular disease risk prediction approaches to low-income and middle-income countries, WHO has convened an effort to develop, evaluate, and illustrate revised risk models. Here, we report the derivation, validation, and illustration of the revised WHO cardiovascular disease risk prediction charts that have been adapted to the circumstances of 21 global regions. Methods: In this model revision initiative, we derived 10-year risk prediction models for fatal and non-fatal cardiovascular disease (ie, myocardial infarction and stroke) using individual participant data from the Emerging Risk Factors Collaboration. Models included information on age, smoking status, systolic blood pressure, history of diabetes, and total cholesterol. For derivation, we included participants aged 40–80 years without a known baseline history of cardiovascular disease, who were followed up until the first myocardial infarction, fatal coronary heart disease, or stroke event. We recalibrated models using age-specific and sex-specific incidences and risk factor values available from 21 global regions. For external validation, we analysed individual participant data from studies distinct from those used in model derivation. We illustrated models by analysing data on a further 123 743 individuals from surveys in 79 countries collected with the WHO STEPwise Approach to Surveillance. Findings: Our risk model derivation involved 376 177 individuals from 85 cohorts, and 19 333 incident cardiovascular events recorded during 10 years of follow-up. The derived risk prediction models discriminated well in external validation cohorts (19 cohorts, 1 096 061 individuals, 25 950 cardiovascular disease events), with Harrell's C indices ranging from 0·685 (95% CI 0·629–0·741) to 0·833 (0·783–0·882). For a given risk factor profile, we found substantial variation across global regions in the estimated 10-year predicted risk. For example, estimated cardiovascular disease risk for a 60-year-old male smoker without diabetes and with systolic blood pressure of 140 mm Hg and total cholesterol of 5 mmol/L ranged from 11% in Andean Latin America to 30% in central Asia. When applied to data from 79 countries (mostly low-income and middle-income countries), the proportion of individuals aged 40–64 years estimated to be at greater than 20% risk ranged from less than 1% in Uganda to more than 16% in Egypt. Interpretation: We have derived, calibrated, and validated new WHO risk prediction models to estimate cardiovascular disease risk in 21 Global Burden of Disease regions. The widespread use of these models could enhance the accuracy, practicability, and sustainability of efforts to reduce the burden of cardiovascular disease worldwide. Abstract

Supplemental vitamin C has a beneficial effect on plasma concentrations of ascorbic acid in men in the Republic of Karelia. An intervention study on men in North Karelia, Finland, and Pitkaranta, Republic of Karelia, was done to determine the likely effect of plasma ascorbic acid concentrations on mortality from cardiovascular diseases. Prior to the intervention, plasma levels of 93% of Pitkaranta men and 2% of North Karelian men revealed acute vitamin C deficiency. The intervention resulted in normal concentration levels for plasma ascorbic acid in 46% of men in the treatment group.

Market prices of PV modules and systems have developed so fast that it is difficult to find reliable up to date public data on real PV capital (CAPEX) and operational expenditures (OPEX) on which to base the levelised cost of electricity (LCOE) calculations. This paper projects the future utility-scale PV LCOE until 2050 in several European countries. It uses the most recent and best available public input data for the PV LCOE calculations and future projections. Utility-scale PV LCOE in 2019 in Europe with 7% nominal Weighted Average Cost of Capital (WACC) ranges from 24 €/MWh in Malaga to 42 €/MWh in Helsinki. This is remarkable since the average electricity day-ahead market price in Finland was 47 €/MWh and in Spain 57 €/MWh in 2018. This means that PV is already cheaper than average spot market electricity all over Europe. By 2030, PV LCOE will range from 14 €/MWh in Malaga to 24 €/MWh in Helsinki with 7% nominal WACC. This range will be 9-15 €/MWh by 2050, making PV clearly the cheapest form of electricity generation everywhere. Sensitivity analysis shows that apart from location, WACC is the most important input parameter in the calculation of PV LCOE. Increasing nominal WACC from 2% to 10% will double the LCOE. Changes in PV CAPEX and OPEX, learning rates or market volume growth scenarios have a relatively smaller impact on future PV LCOE.

Abstract

Background\ud Several studies have shown that diabetes confers a higher relative risk of vascular mortality among women than among men, but whether this increased relative risk in women exists across age groups and within defined levels of other risk factors is uncertain. We aimed to determine whether differences in established risk factors, such as blood pressure, BMI, smoking, and cholesterol, explain the higher relative risks of vascular mortality among women than among men.\ud \ud Methods\ud In our meta-analysis, we obtained individual participant-level data from studies included in the Prospective Studies Collaboration and the Asia Pacific Cohort Studies Collaboration that had obtained baseline information on age, sex, diabetes, total cholesterol, blood pressure, tobacco use, height, and weight. Data on causes of death were obtained from medical death certificates. We used Cox regression models to assess the relevance of diabetes (any type) to occlusive vascular mortality (ischaemic heart disease, ischaemic stroke, or other atherosclerotic deaths) by age, sex, and other major vascular risk factors, and to assess whether the associations of blood pressure, total cholesterol, and body-mass index (BMI) to occlusive vascular mortality are modified by diabetes.\ud \ud Results\ud Individual participant-level data were analysed from 980 793 adults. During 9·8 million person-years of follow-up, among participants aged between 35 and 89 years, 19 686 (25·6%) of 76 965 deaths were attributed to occlusive vascular disease.
