Abstract: The author summarizes briefly the history of hand allograft transplantation, the basics of the operative technics, and the medicinal treatment of the immunosuppression. He establishes that this operation requires complicated team-work: many specialists must be united in the interest of the successful final outcome. The biggest problem is not the technical challenge of the complicated operation, but the ineffectiveness of immunosuppression, its complications; even though significant development has happened in this field and experimental results are also encouraging. The author discusses these questions in this publication in the mirror of literature data. Abstract: For a long time in the clinical practice the autologous nerve transplants are used in reconstruction of the segmental defects of injured hand nerves. The published outcomes, especially in case of the motor and mixed (sensory and motor) nerves, however, were not satisfactory. Hence, it remained a problem and in the focus of research. The author aimed to study this topic and drawing conclusions in his literary review publication. According to the recent results, the replacement of the autologous nerve transplants with other materials (e.g. nerve conduits, various allografts, etc.) has shown good outcomes in mixed nerves too, if the distance of the resected nerve ends do not exceed 6 mm and the diameter of the nerves were not more than 3–7 mm. The applications of these methods have shown poor results, if the defect and/or the nerve diameter were larger. So, in these cases the autologous nerve transplantation remains the optimal management in the future. Orv Hetil. 2017; 158(30): 1163–1167.

In the reconstruction of hand flexor tendon injuries it is fundamental to select the best suture technique, which makes possible early, active postoperative mobilization and achievement of the best results. The author reviews the development of suture techniques of the flexor tendon injuries during the last fifteen years, and discusses experimental tendon reconstruction results as well as clinical outcomes. The author describes the importance of different tendon suture materials, the significance of the pulley system of the fingers, the stretching between the sutured tendon ends by tendon sutures and, finally, the importance of the moving course in the reconstructed tendon. Ez a felgyorsult növekedés a kézsebészetben is tapasztalható; e területen is felhasználásra kerültek a különböző vázszerkezetek segítségével végrehajtott szövetpótlások. A szerző irodalmi áttekintést nyújtó munkájában röviden tárgyalja a szövetépítés jelenlegi lehetőségeit az inak, a szalagok, az idegek, a csont- és porcszövetek, végül az erek sérüléseinek, illetve károsodásainak eseteiben. Véleménye szerint – tekintetbe véve az eddigi, igen intenzív, eredményes, úttörő jellegű (főként experimentális) tudományos kutatásokat – az újabb szövetépítési eljárások már a közeljövőben jelentős szerepet játszhatnak a helyreállító kézsebészetben. The tendons of flexor digitorum profundus are most often interconnected and making them less able to move independently than the superficial tendons. The reason for this is the cross connective tissue-ligamentous connection between the tendons of the profundus. The recognition of this so called quadriga phenomenon is important in the clinical practice, when the presence of this syndrome hinders the strength and the movement of the tendons after operative reconstruction. In his work the author describes the anatomical foundation, the biomechanical correlations and the clinical importance of the quadriga syndrome in the mirror of literature data.A flexor digitorum profundus inai egymással összeköttetésben állnak és kevésbé képesek különálló mozgásra, mint a flexor digitorum superficialis inak. Ezt a különbözőséget gyakran a mély hajlítók közös izomhasának tulajdonítják, azonban sokkal jelentősebbek azok az alkari és csuklótáji, haránt irányú, kötőszövetes-szalagos összeköttetések, amelyek ezen inak között ép körülmények között találhatók. A quadrigajelenségnek nevezett ezen elváltozásnak a felismerése fontos lehet egyes klinikai esetekben: így, amikor a betegeknél az inak erőkifejtését és mozgását a szindróma jelenléte gátolja, az ínsérülés helyreállítása után. A szerző irodalmi értékelő munkájában összefoglalta a kézsebészetben fellelhető gyakori diagnosztikus tévedéseket és a kezelés során kialakult szövődményeket. Munkáját három fő részre osztotta: jelen, első részben a kórismézési hibákat és a kezelések komplikációit tárgyalja. A tanulmány második és harmadik részében a kézsérülések okozta szövődményekkel kíván foglalkozni. A jelenlegi munkában értékeli a megkésett diagnózis elkerülésének módjait: a gondos anamnézis felvételt, a kéz gyors vizsgálatának módszereit, beleértve a nem kooperáló betegek kezének vizsgálatát és az elnézett esetek felismerésének lehetőségeit. A kezelések során kialakult leggyakoribb komplikációknál megemlíti a sebzárás, a lágyrész sérülések, a sebkötözések és a kézen végzett egyéb eljárások okozta károsodások felismerését és kezelését. A szerző a kézsérülések szövődményeivel foglalkozó, irodalmi összefoglaló dolgozatának második részében a késői, a különleges, a komplex és a súlyos sérülések ellátása során kialakult szövődmények felismerését és kezelési lehetőségeit tekinti át. A késői szövődmények közül tárgyalja a kozmetikai deformitások és a komplex regionális fájdalom tünetcsoport mellett a compartment szindróma kezelését. A különleges sérülések fejezetben a harapásos, a kémiai égési és az injekció okozta sérülések szövődményeinek leírását és terápiáját adja meg. A komplex sérüléseknél pedig a traumás csonkolások, az idegen testek, az égési és fagyási sérülések utáni komplikációkat veszi sorra. Végül a súlyos kézsérülések tárgyalása során foglalkozik a replantációs műtétek, illetve az érsérülések műtéteit követő szövődményekkel. Dolgozatának végéhez az érdeklődő szakemberek számára bőséges publikációs listát mellékelt.

The author summarizes the new therapeutic tendencies in hand surgery at the past one and a half decade. He discusses the development of hand surgery, as an independent field, in a form of a short historical summary, then he reviews in detail new therapeutic methods considered important such as rehabilitation procedures after tendon injuries, present position of complete hand transplantation, new operations of regeneration of the injured skin and repair of nerve damages, as well as the conservative therapeutic options of Dupuytren's disease. Finally he outlines the modified, new operative procedures in bone and joint injuries of the hand. He concludes that constant development of hand surgical knowledge will likely result in further novel therapeutic methods.A szerző munkájában összefoglalja az elmúlt másfél évtized fontosabb, új kézsebészeti kezelési irányzatait. A kézsebészetnek mint önálló szakterületnek a kialakulását rövid történeti összefoglalás formájában tárgyalja, majd részletesen ismerteti az általa fontosnak tartott új kezelési módszereket: az ínsérülések utáni új rehabilitációs eljárásokat, a teljeskéz-átültetés jelenlegi helyzetét, a sérült bőr regenerációját elősegítő és az idegsérülések helyreállítását célzó újabb műtéteket, valamint a Dupuytren-betegség konzervatív kezelési lehetőségét. Végül ismerteti a kéz csont- és ízületi sérüléseiben alkalmazott módosított, új műtéti eljárásokat. 17. The role of mesenchymal stem cells in the reconstruction of nerve injuries in the hand

At present the end results of reconstruction of peripheral nerve injuries cannot be considered unequivocally advantageous. It seems that the level of reconstructive possibilities of these injuries has already peaked. Hence, ongoing research focuses on experimental studies to further improve results. One of the methods is the joint usage of pluripotent mesenchymal stem cells with tissue inductive polypeptides (growth factors) and frame structures to enhance the attachment of these cells with the aim creating new neural tissue (tissue engineering). The conditions to create new tissue can be further improved by gene technology. Based on recent literature data, the author summarizes the basic characteristics of the method related to nerve injuries, and the possibilities and modalities of clinical applications. In conclusion, future direction is a the wider use of stem cells, however, the currently established surgical and rehabilitation methods have to be performed at high levels since stem cell research data are not established in the clinical setting yet. Reconstruction of the flexor tendon injuries is one of the most difficult problems in hand surgery, because the postoperative end-results are often unfavorable. The author discusses the history of the flexor tendon surgery, and then he describes the development of the knowledge regarding the anatomy, the blood supply and the tendon healing of the flexor tendons from the beginning until now. After that he describes the development in suture materials, suture techniques, primary and secondary tendon reconstruction operations, postoperative treatment and rehabilitation programmes. The author describes what kind of progress to be expected in this field in the near future, and then he expresses his viewpoint about the development in the international and national hand surgical societies. 20. [Use of tissue engineering in the reconstruction of flexor tendon injuries of the hand]

In his literary analysis, the author describes a novel method applied in the reconstruction of flexor tendon injuries of the hand. This procedure is named tissue engineering, and it is examined mainly under experimental circumstances. After definition of the method and descriptions of literary preliminaries the author discusses the healing process of the normal tendon tissue, then development of the scaffold, an important step of tissue engineering is described. After these topics the introduction of the pluripotent mesenchymal stem cells into the scaffold, and proliferation of these cells and development of the sliding systems are presented. The mechanical resisting ability of the formed tendon tissue is also discussed. Finally, the author concludes that as long as results of experimental research cannot be successfully applied into clinical practice, well-tried tendon reconstruction operations and high quality postoperative rehabilitation are needed.A szerző irodalmi elemző tanulmányában a kéz hajlítóín-sérüléseinek helyreállításában alkalmazott újabb módszert, a szövetépítésnek (tissue engineering) nevezett, ma még csak jobbára kísérletes körülmények között vizsgált eljárást írja le. A módszer meghatározása és irodalmi előzményeinek leírása után tárgyalja az ép ínszövet gyógyulási folyamatát, a szövetépítés fontos lépését, a vázszerkezet kialakítását. Ezt követően áttekinti a pluripotens mesenchymalis őssejteknek a vázszerkezetbe történő bevitelét, e sejtek proliferációját és a csúszófelszín kialakítását. Befejezésül a kialakított ínszövet mechanikai ellenálló képességét ismerteti. Megállapítja, hogy mindaddig, amíg a kísérletes kutatási eredményeket a klinikai gyakorlatban széles körben sikeresen alkalmazni nem lehetséges, a jelenlegi, jól bevált ínhelyreállító műtétek és a posztoperatív rehabilitációs tevékenység magas színvonalú kivitelezése szükséges. The author gives an overview of the literature and presents a new surgical procedure developed for the reconstruction of severe injuries to the PIP joint: an osteochondral autograft harvested from the dorsal part of the hamatum is implanted in order to reconstruct the damaged joint surfaces. The introduction describes the importance of that injury, and also the treatment options, which are predominantly determined by the extent of the injury to the joint surface. According to the literature, transplantation of a hemi-hamatum autograft provides good results in case of comminuted fractures involving more than 50% of the joint surface and also the long term functional outcomes are promising. Furthermore, it describes the anatomy of the hamatum and delineates the most important steps of the surgical technique, methods of the follow-up and postoperative complications. Finally concludes that this method is an effective way of treatment in severe PIP joint dislocations combined with fractures. The author did not find any reference about this procedure in the Hungarian literature on surgery of the hand. The damaged pulley system requires repair in all cases after injuries to the flexor tendons, because - in addition to unimpaired tendon action - proper function of the pulleys is essential for the full range of motion and grasping power of the fingers. In his analytical literature overview, author specifies the anatomical principles of the fibro-osseal pulley system and disjoints the possible ways of surgical pulley repair, including also the most recent surgical techniques. He ascertains that careful restoration of the anatomical structures is of high importance during the course of flexor tendon, tendon sheath and pulley reconstruction in order to achieve the expected optimal result. Author gives an analytic overview of the literature on the diagnosis and treatment of isolated, acute and inveterate injuries to the flexor digitorum profundus tendon. After summarising the importance of the injury, he briefly cites the international and Hungarian literature on this topic and confirms that there are numerous foreign publications even nowadays, but, in turn, Hungarian authors published their articles mostly in the second half of the last century. After delineation of the principles of the diagnostics and treatment, he reviews the technical details of the surgical treatment. He confirms that in case of acute, closed or open injuries reinsertion of the tendon or fusion of the DIP joint may be performed, but, in turn, in case of inveterate injuries - in addition to the above methods - free tendon transplantation can be performed, strictly observing the appropriate indications. He recommends this overview to young and senior resident doctors preparing themselves for their specialist's examination of surgery of the hand, orthopaedic/trauma surgery or plastic surgery; he hopes that also young specialists being active in the above specialties can make use of this paper during the course of their everyday work. Author deals in his publication with the sensory nerve terminals of the skin of the hand, which topic is infrequently discussed in the literature. He lists the major sensory receptors in the skin; he delineates their morphologic characteristics with schematic figures and histological micro photos, and reviews their role played in the sensation, pointing out, which is responsible for a given quality of sensation. He emphasises the very important role of the central nervous system in the summation of the multitudinous nerve-impulses of different origin mediated by the receptors, where through stimuli coming from numerous nerve terminals make up a complex nerve-impulse pattern in the brain. In conclusion, he confirms that despite of the fact that lesions to the fine sensory nerve terminals of the skin of the hand cannot be restored according to our present knowledge, but recognition of these structures may be important for all specialists dealing with treatment of hand disorders. Sterling Bunnell, father of the surgery of the hand (1882-1957). Commemoration on the occasion of the 130th anniversary of his birth and 55th anniversary of his death. Sterling Bunnell is famous for developing a number of still valid principles of surgical techniques applied in the surgery of the hand, furthermore description of more, at that time revolutionary new ways of surgical treatments performed on the palmar surface of the fingers and tendon sheaths at the level of the "no man's land', which naming is anachronistic nowadays. He published numerous literary works, among them his monumental monograph, entitled Surgery of the hand, first published in 1944, which was the bible of hand surgeons for several decades. He was one of the establishers and the first president of the American Society for the Surgery of the Hand. Thus he can be deemed as one of the establishers of the independent surgery of the hand in the USA. Thanks to his authority and career, surgery of the hand could become an independent specialty in numerous countries later on. His followers deservedly called him „Father of the surgery of the hand“ yet in his lifetime. Despite of all these facts, there are only a few references about his life and career in the Hungarian literature on the surgery of the hand. The author commemorates the establisher of the surgery of the hand on the occasion of the 130th anniversary of his birth and 55th anniversary of his death; furthermore, based on the data available in the literature, reviews Bunnel's personal walk of life, professional and scientific career and activities in the public life. Hand surgeon specialists consider the hand as a sensory organ - apart from its other functions -- and an extension of the brain towards the environment. They are aware of its specific sensory functions and of the strong connection, which links the hand with the intellectual activity of the brain, the psyche. The face and the hand at the same time are such parts of our body that are always exposed to the environment and both express our thoughts and feelings. The author in his work discusses -- accordingly -- the role of the hand as a sensory organ, then describes some expressive formats, moreover its role in connection and sensitivity. Later on he deals separately with the status of the hand in fine arts and in music. Finally, he discusses the plasticity of the function of the brain -- using data from the professional literature. He concludes that following restoration of the nerve injury, during the regeneration processes the functional reorganization of the brain cortex means the creation of a new "language" for the brain. In these cases the demand of reprogramming arises to regain the best possible sensitivity of the damaged areas of the hand. According to our actual knowledge the professional, special operative management of nerve injuries of the hand is the most important factor of the good outcome; however it is very likely that swift and possible complete brain cortex reorganization, managed by professionals and described in the study in detail, will take a considerable role in the not even distant future. This paper is recommended for young hand surgeons, orthopedist-traumatologist doctors for their everyday work, especially since domestic publication has not been available in this subject. The author reviews function-restoring procedures following failed peripheral nerve reconstruction on the hand with analysis of data from the literature. He realizes that the number of these operations decreased in the recent decades, the majority of them comprise the replacement procedures following nerve injuries. In his paper he discusses the possibilities and basic concepts of these secondary replacement procedures. He discusses in detail the symptoms of the hand following median, radial and ulnar nerve palsy as well as the outcomes of the interventions. Finally, the therapeutic possibilities of the definitive, combined hand neural palsy are outlined. He establishes that the end results of these motor function restoring operations can be optimal only with the coordinated activity of the expert hand surgeons and physiotherapists. The first written recollections of the surgical treatment of the flexor tendon injuries and other pathological changes of the hand originate from the 2nd century. Descriptions of the therapy at the time -- looking back from today -- were primitive and obscure. The real advancement of the tendon surgery can be considered in the second half of the 17th century but the real breakthrough began at the end of the 19th and in the beginning of the 20th century and the development is ongoing in our days. The stations of this process and the work of the authors playing significant role in the development of the tendon surgery are discussed. The fundamental changes, driving into positive direction in the fields of theoretical approach of the tendon healing, the suturing material and techniques, the postoperative care and assessing the postoperative results are discussed, furthermore the names and activities of the surgeons playing important role in the tendon surgery. It is concluded that in our days hand surgery has become a recognized, important specialty all over the world, mainly due to the activity of the world (IFSSH) and European (FESSH) hand surgery societies, and the national organizations connected to them. summary A two-stage procedure is described for severely damaged flexor tendons in “no man's land”. In the first stage the damaged tendons and scarred tendon sheath are excised and a segment of the saphenous vein, cannulated by a silicone rubber rod is transplanted into the tendon bed. In the second stage the silicone-rod is replaced by a free transplanted tendon. The endothelial lining of the vein is a guarantee of a good gliding surface and an obstacle to adhesion formation, and so it ensures good function. A review of the method and a survey of thirty-nine cases are presented.
