Background. Shift from laparotomy to laparoscopy qualitatively changed surgery aggressiveness, allowed to reduce the number and severity of postoperative complications. New methods of minimally invasive interventions introduction have generated a number of legitimate questions relating to the desirability, safety, possible range of applications, the effectiveness of interventions. However, no studies in the literature devoted to the development of criteria for predicting the likely complications of surgical data and adverse postoperative course.Aim of the study - to identify the most significant predictors of adverse postoperative course of the laparoscopic cholecystectomy and laparotomy cholecystectomy from the minimum access.Material and methods. Retrospective study included 102 patients with cholelithiasis who routinely were performed cholecystectomy. Cholecystectomy from minimum laparotomy access was performed in 48 (47,1%) patients. 54 (52,9%) patients underwent laparoscopic cholecystectomy. The groups were comparable by age, sex, height, weight, body mass index.Statistical processing of the material was carried out with software package Statistica 6.0. and MedCalc10.2.0.0. Parametric (t-test for dependent and independent variables, ANOVA ANOVA, paired Pearson correlation) and nonparametric (Wald-Wolfowitz runs test, Kolmogorov-Smirnov two-sample test, Mann-Whitney U test, correlation Spearman) statistic methods were used. С целью определения наиболее значимых предикторов неблагоприятного течения послеоперационного периода после холецистэктомии провели ретроспективное исследование. Обследовали 102 пациентов с желчнокаменной болезнью, которым в плановом порядке проведена холецистэктомия из минимального лапаротомного доступа и лапароскопическим способом. По результатам ROC-анализа, негативной предикторной способностью обладают такие исходные показатели: уровень гемоглобина >131 г/л, лейкоцитоз >11×109/л, размеры конкрементов желчного пузыря >2 мм, фибриноген >4,44 г/л, повышение прямого билирубина >12,7 мкМоль/л; позитивные предикторы: СОЭ, Перехід від лапаротомії до лапароскопії якісно змінив агресивність хірургічних втручань, дав змогу знизити кількість і важкість післяопераційних ускладнень. З метою визначення найбільш значущих предикторів несприятливого перебігу післяопераційного періоду після холецистектомії здійснили ретроспективне дослідження. INTRODUCTION: The aim of our study was to compare cholecystectomy from the minimum access laparotomy (CMAL) and laparoscopic cholecystectomy (LC) with respect to complications, operating time, postoperative length of hospital stay (PLOS).METHODS: One hundred fifty nine patients with biliary colic and documented gallstones were randomized to CMAL (n = 86) or LC (n = 73) groups. Data measures included operating time, postoperative complication (hyperthermia), and postoperative length of hospital stay.RESULTS: Patients were similar except for body mass index (BMI), age, gender, ASA. The CMAL group had longer operative time (65 vs. 39 min, p = 0.004) compared with the LC group. The postoperative length of hospital stay was significantly shorter in the LC groups than the CMAL groups (5,3 vs. 3,2 days, p=0,002). Postoperative time of hyperthermia was longer for CMAL (2,71 vs. 0,91 days, p=0.008). There was no major complication in either group.CONCLUSION: CMAL is associated with longer operating time, period of hyperthermia after operation and length of stay, but equals LC with respect to safety., Приведены данные сравнительного анализа особенностей послеоперационного течения у 159 больных желчнокаменной болезнью после выполнения холецистэктомии из минимального лапаротомного доступа (73 пациентов) и лапароскопическим способом (86 больных). Течение послеоперационного периода зависело от вида хирургического вмешательства (холецистэктомия из минимального лапаротомного доступа или лапароскопическая холецистэктомия). Более благоприятное течение послеоперационного периода у больных после лапароскопической холецистэктомии определяется меньшей длительностью оперативного вмешательства, снижением частоты и продолжительности гипертермии и сокращением сроков пребывания в стационаре., Наведено результати порівняльного аналізу особливостей післяопераційного перебігу у 159 хворих на жовчнокам’яну хворобу після виконання холецистектомії з мінімального лапаротомного доступу (73 пацієнти) і лапароскопічним способом (86 осіб). Перебіг післяопераційного періоду залежав від виду хірургічного втручання (холецистектомія з мінімального лапаротомного доступу чи лапароскопічна холецистектомія). Сприятливіший перебіг післяопераційного періоду у хворих після лапароскопічної холецистектомії визначається меншою тривалістю оперативного втручання, зниженням частоти і тривалості гіпертермії та скороченням термінів перебування у стаціонарі.

The discussion about the necessary shift from teaching to learning has been considerably boosted by the Bologna Process and related issues such as qualification frameworks, the European Credit Accumulation and Transfer System (ECTS) and the Diploma Supplement.Teaching and learning in higher education is a shared process, with responsibilities on both student and teacher to contribute to their success. Within this shared process, higher education must engage students in questioning their preconceived ideas and their models of how the world works, so that they can reach a higher level of understanding. Nobody would contest that we need high quality teaching and learning throughout our education and training systems.The aim of this work was to examine the effect of student scientific circle of Surgery on the formation of professional motivation of students.An important form of scientific-research work of students in Zaporizhia state medical university (ZSMU) is functioning of student’s scientific circle as branche of Scientific Student Association.In task group are also practical skills, which is primarily important for, the formation of a future doctor character qualities such as tenacity, ambition, determination.The feature of the circle is that students study discipline independently, using help of the department teachers, collaborate with students of other scientific circle and societies of ZSMU and other Ukraine medical high schools. The best teaching helps students to motivates them to learn.The results of monitoring in 2012-2013 showed that about 85% of students improved learning outcomes from such disciplines as operative surgery and topographic anatomy, propaedeutic Internal Medicine, General Surgery, Faculty surgery, Hospital surgery.Presented the possibilities of using various forms information technology to enhance learning, acquire new knowledge and information sharing. Every year students of surgery circle participate in student conferences with international participation, receive prizes.Teaching and learning are fundamental core missions of university, that involve students in governance, curricular design, development and review, quality assurance and review of the procedures. Academic staffs are employed not just to teach, but to teach on a high professional level.Thus, student scientific circle is an important means to improve the efficiency of education contributes to future surgeon., Описан опыт организации работы научного студенческого кружка по факультетской хирургии.
