Abstract

Background and objectivesAntimicrobial stewardship programmes (ASPs) have resulted in antimicrobial consumption (AMC) reduction and quality of prescription (QOP) improvement. However, evidence of ASP impact in paediatrics is still limited. This study aims to assess a paediatric ASP long-term outcomes.MethodsA quality improvement study assessed by a interrupted time series analysis was conducted in a paediatric tertiary hospital. QOP expressed as proportion of adequate prescriptions, AMC measured by defined daily dose incidence per 1000 occupied bed days, incidence density of bloodstream infections (BSIs) and its related all-cause crude death rate (CDR) were compared between pre (from January 2013 to December 2015) and post (from January 2016 to December 2019) ASP activities intensification, which included a dedicated paediatric infectious diseases physician to actively perform educational interviews with prescribers.ResultsInappropriate prescribing showed a significant downward shift associated to the intervention with a −51.4% (−61.2% to −41.8%) reduction with respect to the expected values. Overall AMC showed no trend change after the intervention. For neonatology a28.8% (−36.8% to −20.9%) reduction was observed. Overall anti-pseudomonal cephalosporin use showed a −51.2% (−57.0% to −45.4%) reduction. Decreasing trends were observed for carbapenem use, with a quarterly per cent change (QPC) of −2.4% (−4.3% to −0.4%) and BSI-related CDR (QPC=−3.6%; −5.4% to −1.7%) through the study period. Healthcare-associated multi-drug-resistant BSI remained stable (QPC=2.1; −0.6 to 4.9).ConclusionsIntensification of counselling educational activities within an ASP suggests to improve QOP and to partially reduce AMC in paediatric patients. The decreasing trends in mortality remained unchanged.

Abstract

In the past few years, antimicrobial resistance has increased, becoming a serious public health problem. The irrational use of antimicrobials is one of the main contributors to antimicrobial resistance. The paediatric population is not free from this problem, as antimicro-bials are widely prescribed in this age group, often inappropriately. The introduction of antimicrobial stewardship programmes (ASPs) has proven crucial in cur-bing the emergence of antimicrobial resistance. At the international level, the need to develop specific paediatric ASPs has been recognised on account of the differences between adult and paediatric patients as concerns infection and approaches to diagnosis and treatment. For this reason, paediatric ASPs should be multidisciplinary programmes led by paediatric infectious disease specialists and use specific paediatric indicators (such as days of treatment, antimicro-bial susceptibility patterns in the paediatric population, or clinical indicators) to help identify areas of improvement and develop effective targeted interventions. On the other hand, the support and leadership of the pertinent scientific societies are also essential. The purpose of this document is to present the position of the Sociedad Espanola de Infectologia Pediatrica (SEIP, Spanish Society of Paediatric Infectious Diseases) concerning the implementation of paediatric ASPs in hospitals in Spain and to provide tools to facilitate their application in hospitals throughout the regional health care systems in the country. 29. Export Controls and the Tensions between Academic Freedom and National Security

Abstract

In the U.S.A., advocates of academic freedom--the ability to pursue research unencumbered by government controls--have long found sparring partners in government officials who regulate technology trade. From concern over classified research in the 1950s, to the expansion of export controls to cover trade in information in the 1970s, to current debates over emerging technologies and global innovation, the academic community and the government have each sought opportunities to demarcate the sphere of their respective authority and autonomy and assert themselves in that sphere. In this paper, we explore these opportunities, showing how the Social Contract for Science set the terms for the debate, and how the controversy turned to the proper interpretation of this compact. In particular, we analyze how the 1985 presidential directive excluding fundamental research from export controls created a boundary object that successfully demarcated science and the state, but only for a Cold War world that would soon come to an end. Significant changes have occurred since then in the governance structures of science and in the technical and political environment within which both universities and the state sit. Even though there have been significant and persistent calls for reassessing the Cold War demarcation, a new institutionalization of how to balance the concerns of national security and academic freedom is still only in its nascent stages. We explore the value of moving from a boundary object to a boundary organization, as represented in a proposed new governance body, the Science and Security Commission.

30. Equity Considerations in the Assessment of the Bayh-Dole Act

Abstract

Extant evaluation studies of the Bayh-Dole Act of 1980 have focused primarily on its effects on the pace of innovation and on the norms and practices of academic research but neglected other public values. Seeking to redress this shortcoming, I begin by examining Bayh-Dole with respect to other relevant public values following the "Public Value Failure" approach. From that analysis, equity emerges as a pressing issue. I define equity issues, in a loosely "Rawlsian" sense, as situations of unfair distribution of political power and economic resources. My analysis identifies a business model of offices of technology transfer--that I call "nurturing start-ups"--that is likely to become a standard of practice. This model can foster either firm competition or concentration in emerging industries and will therefore have an impact on the distribution of economic benefits from innovation. In addition, political influence to reform Bayh-Dole is allocated disproportionately in favor of those who stand to gain from this policy. For instance, elite universities hold a larger share of the resources and voice of the university system. Consequently, adjusting the nurturing start-ups model to foster competition and increasing cooperation among universities should lead to a more equitable distribution of economic benefits and political voice in technology transfer. Conventional policy evaluation is also responsible for the neglect of equity considerations in Bayh-Dole studies. Currently, "what is the policy impact?" can be answered far more systematically than "why the impact matters?" or "is this policy designed and implemented legitimately?" The problem lies with the consequentialist theory of value that undergirds evaluation. Hence, I propose a deontological theory of evaluation to reaffirm the discipline's commitment to democratic policy making. Abstract

La presente investigación titulada “Gestión de las Contrataciones Estatales en la Sub Región Pacifico - Áncash, año 2020”, tuvo como objetivo determinar los principales problemas que presentan las contrataciones estatales del sistema de abastecimiento público en la Sub Región Pacífico - Áncash, año 2020. En cuanto a la metodología, tuvo enfoque cualitativo. Se utilizó la entrevista como técnica para la investigación, teniendo como instrumento a la guía de entrevista. Para los resultados, se utilizó la triangulación de datos, recopilando todos los datos cualitativos obtenidos; con ello se obtuvo que no hay un correcto llenado de los requerimientos, debido a que las áreas usuarias desconocen su elaboración, además, se observó que el compromiso por parte de las áreas usuarias es bajo, evidenciándose que no se involucran en la formulación y ejecución de los requerimientos. Abstract

[EN]: A progressive increase in the incidence of infections caused by multidrug-resistant microorganisms is being reported. Among these resistant microorganisms, the main threats are extended-spectrum β-lactamase-, AmpC-, and carbapenemase-producing Gram-negative bacilli, methicillin-resistant Staphylococcus aureus and vancomycin-resistant Enterococcus faecium. To address this important problem, it is essential to establish pediatric Antimicrobial Stewardship programs, perform active epidemiological surveillance and develop an adequate infection control policy. The therapeutic approach of these infections is often complex, frequently requiring antibiotics with less experience in children. In this position document made by the Spanish Association of Pediatrics and the Spanish Society of Pediatric Infectious Diseases, the epidemiology and treatment of these infections are reviewed according to the best available evidence., [ES]: En los últimos años se ha evidenciado un incremento en la incidencia de infecciones por bacterias multirresistentes. Las principales amenazas son los bacilos gramnegativos productores de β-lactamasas de espectro extendido, AmpC o carbapenemasas, Staphylococcus aureus resistente a meticilina y Enterococcus faecium resistente a vancomicina. Para hacer frente a este problema, es fundamental establecer programas de optimización en el uso de antimicrobianos específicos para pediatría, realizar una vigilancia epidemiológica activa y desarrollar una adecuada política de control de infecciones. Su abordaje terapéutico es, a menudo, complejo y multidisciplinar, y precisa frecuentemente del uso de antibióticos menos empleados.
