Abstract

Background: The multimorbid burden and use of systemic immunosuppressants in people with psoriasis may confer greater risk of adverse outcomes of coronavirus disease 2019 (COVID-19), but the data are limited. Objective: Our aim was to characterize the course of COVID-19 in patients with psoriasis and identify factors associated with hospitalization. Methods: Clinicians reported patients with psoriasis with confirmed/suspected COVID-19 via an international registry, Psoriasis Patient Registry for Outcomes, Therapy and Epidemiology of COVID-19 Infection. Multiple logistic regression was used to assess the association between clinical and/or demographic characteristics and hospitalization. A separate patient-facing registry characterized risk-mitigating behaviors. Results: Of 374 clinician-reported patients from 25 countries, 71% were receiving a biologic, 18% were receiving a nonbiologic, and 10% were not receiving any systemic treatment for psoriasis. In all, 348 patients (93%) were fully recovered from COVID-19, 77 (21%) were hospitalized, and 9 (2%) died. Increased hospitalization risk was associated with older age (multivariable-adjusted odds ratio [OR] = 1.59 per 10 years; 95% CI = 1.19-2.13), male sex (OR = 2.51; 95% CI = 1.23-5.12), nonwhite ethnicity (OR = 3.15; 95% CI = 1.24-8.03), and comorbid chronic lung disease (OR = 3.87; 95% CI = 1.52-9.83). Hospitalization was more frequent in patients using nonbiologic systemic therapy than in those using biologics (OR = 2.84; 95% CI = 1.31-6.18). No significant differences were found between classes of biologics. Independent patient-reported data (n = 1626 across 48 countries) suggested lower levels of social isolation in individuals receiving nonbiologic systemic therapy than in those receiving biologics (OR = 0.68; 95% CI = 0.50-0.94). 20. Análisis descriptivo del índice de discapacidad vocal pediátrico en niños con patología vocal benigna

Abstract

Resumen Introducción: La disfonía infantil puede afectar negativamente la autoestima del niño y su calidad de vida relacionada con la voz. Objetivo: Describir los resultados del cuestionario Pediatric Voice Handicap Index (pVHI) en niños con patología vocal benigna. Material y Método: Se diseñó un estudio descriptivo en pacientes con patología vocal benigna entre 3 y 15 años en la Unidad de Voz del Hospital de Niños Dr. Luis Calvo Mackenna entre octubre de 2016 y febrero de 2020. La evaluación se realizó mediante un examen laringoscópico y el cuestionario pVHI para evaluar el impacto en la calidad de vida. Resultados: Se incluyeron 49 pacientes, 35 varones (71,4%) y 14 mujeres (28,6%). La edad media fue de 9,27 años. En el examen laringoscópico, el hallazgo más frecuente fueron los nódulos vocales. La puntuación media total en el pVHI fue de 38,77: 11,67 en la subescala o categoría funcional, 18,59 en la física y 8,42 en la emocional. En la evaluación de pVHI por género no existieron diferencias significativas. Conclusión: La patología vocal benigna en la edad pediátrica ocasiona un impacto negativo en la calidad de vida relacionada con la voz. El cuestionario pVHI es un valioso instrumento para evaluar dicha repercusión.

Item does not contain fulltext, IMPORTANCE: A clear dosing regimen for methotrexate in psoriasis is lacking, and this might lead to a suboptimal treatment. Because methotrexate is affordable and globally available, a uniform dosing regimen could potentially optimize the treatment of patients with psoriasis worldwide. OBJECTIVE: To reach international consensus among psoriasis experts on a uniform dosing regimen for treatment with methotrexate in adult and pediatric patients with psoriasis and identify potential future research topics. DESIGN, SETTING, AND PARTICIPANTS: Between September 2020 and March 2021, a survey study with a modified eDelphi procedure that was developed and distributed by the Amsterdam University Medical Center and completed by 180 participants worldwide (55 [30.6%] resided in non-Western countries) was conducted in 3 rounds. The proposals on which no consensus was reached were discussed in a conference meeting (June 2021). Participants voted on 21 proposals with a 9-point scale (1-3 disagree, 4-6 neither agree nor disagree, 7-9 agree) and were recruited through the Skin Inflammation and Psoriasis International Network and European Academy of Dermatology and Venereology in June 2020. Apart from being a dermatologist/dermatology resident, there were no specific criteria for participation in the survey. The participants worked mainly at a university hospital (97 [53.9%]) and were experienced in treating patients with psoriasis with methotrexate (163 [91.6%] had more than 10 years of experience). MAIN OUTCOMES AND MEASURES: In a survey with eDelphi procedure, we tried to reach consensus on 21 proposals. Consensus was defined as less than 15% voting disagree (1-3). For the consensus meeting, consensus was defined as less than 30% voting disagree. RESULTS: Of 251 participants, 180 (71.7%) completed all 3 survey rounds, and 58 participants (23.1%) joined the conference meeting. Abstract

Introduction: The aim of this study was to develop a surgical risk prediction model in patients undergoing anatomic lung resections from the registry of the Spanish Video-Assisted Thoracic Surgery Group (GEVATS). Methods: Data were collected from 3533 patients undergoing anatomic lung resection for any diagnosis between December 20, 2016 and March 20, 2018. We defined a combined outcome variable: death or Clavien-Dindo grade IV complication at 90 days after surgery. Univariate and multivariate analyses were performed by logistic regression. Internal validation of the model was performed using resampling techniques. Results: The incidence of the outcome variable was 4.29% (95% CI 3.6-4.9). The variables remaining in the final logistic model were: age, sex, previous lung cancer resection, dyspnea (mMRC), right pneumonectomy, and ppo DLCO. The performance parameters of the model adjusted by resampling were: C-statistic 0.712 (95% CI 0.648-0.750), Brier score 0.042 and bootstrap shrinkage 0.854. Conclusions: The risk prediction model obtained from the GEVATS database is a simple, valid, and reliable model that is a useful tool for establishing the risk of a patient undergoing anatomic lung resection. The aim of this work was to evaluate the accumulated effect of crop rotations with or without manure amendment on the physical properties of the soil and its relationship with the growth of grape (Vitis vinifera). In a Xerollic Haplargid belongs to Cavancha soil series, plots of 8 plants of grapevine distributed in two rows were established, defining 4 treatments: T1, control, bare soil; T2, crop rotation (rapeseed-broad bean-mustard-oats); T3 manure (12.5 Mg ha-1 year-1); T4, crop rotation (rapeseed-broad bean-barley-mustard) established with manure amendment (same dose of T3), with manure application on the grape row and crop rotations in the entire plots. The soil physical properties were evaluated at three depths (0-10; 10-30; 30-50 cm), while in the vine the pruning weight, the grape yield and the root growth were measured. The applications of manure (T3 and T4) promoted an increase of the soil organic matter, which resulted in an improvement of physical properties; nevertheless, the treatment that combine the use of manure and cover crops (T4) achieves the best results because of a synergic effect. T3 and T4 showed a decrease in bulk density and penetration resistance, with higher aggregate stability and water and air fluxes; in addition, T4 favored a significant higher yield in one season and a sustained biomass growth over time., En el presente trabajo se evaluó el efecto de una rotación de cultivos y el sinergismo de ésta con el estiércol sobre las propiedades físicas del suelo y el crecimiento de plantas de vid (Vitis vinifera). La investigación se realizó en Vallenar, Región de Atacama, entre los años 2006 y 2010. En un suelo de la Serie Cavancha (Xerollic Haplargid), se establecieron 4 tratamientos en parcelas que incluyeron 8 plantas de vid distribuidas en dos hileras: T1, testigo, suelo desnudo; T2, rotación de cultivos (raps-haba-mostaza-avena); T3, aplicación de estiércol de cabra (12,5 Mg ha-1 año-1); T4, rotación de cultivos (raps-haba-cebada-mostaza) establecidos sobre estiércol (misma dosis de T3), con aplicaciones del estiércol en la sobrehilera y siembra de cultivos en toda la superficie. Las propiedades físicas del suelo se evaluaron a tres profundidades (0-10; 10-30 y 30-50 cm) en tanto en la vid se evaluó masa de poda, rendimiento y biomasa de raíces. Las aplicaciones de estiércol (T3 y T4) generaron un incremento de la materia orgánica del suelo, lo que resultó en una mejora de las propiedades físicas; sin embargo, el tratamiento que conjugó aplicación de estiércol y cultivo de cobertura (T4) fue el que logró los mejores resultados por un efecto sinérgico entre ambas estrategias. Así, T3 y T4 presentaron disminuciones en la densidad aparente y la resistencia a la penetración, con una mayor estabilidad de agregados y capacidad de flujo de agua y aire; además, T4 aumentó el rendimiento en una de las temporadas y mostró un crecimiento sostenido en el tiempo de la biomasa de la vid.

Abstract

In the present work have studied two samples with the same pigment, Egyptian Blue, but from different sources. The difference between them is caused by the functionality. In the case of Nefertari´s tomb is decorating interior rooms carved into rock in which they would not suffer any damage since it does not accessed to that place anymore. In the case of Balneum´s baths is decorating very warm and humid rooms, and should resist deterioration resulting from human activity during the use of these baths.En el trabajo presentado se han estudiado dos muestras de pinturas con el mismo pigmento, azul egipcio, de distinta procedencia. Las diferencias entre ellas vienen causadas por la funcionalidad. En el caso de la tumba de Nefertari se trata de decorar estancias interiores excavadas en roca en las que no sufrirían deterioro alguno, dado que no se accedería a ese lugar nunca más. En el caso del Balneum se trata de pinturas que decoran ambientes muy húmedos y cálidos y que, además, deben resistir el deterioro derivado de la actividad humana en dichas termas.

This article evaluates the value perceived by the user of the Center of Primary Health in study versus the value delivered by this organization with the purpose to determine the gap in the quality of the service and the factors that affect. There has used the model SERVQUAL already validated for the sector health. The results indicate that the value of the gap is negative since the polled users were waiting for a better service of what actually they received or perceived. The most important dimensions in the study to evaluate the quality of the service were Security and Tangibility.

Abstract

Resumen Introducción: La prevalencia de patología vocal en niños fluctúa entre el 6% y el 23%. El cuestionario Pediatric Voice Handicap Index (pVHI) se ha transformado en el instrumento más utilizado a nivel internacional para determinar el impacto de la disfonía en la calidad de vida de los niños. Objetivo: Realizar la traducción, adaptación cultural y validación del Pediatric Voice Handicap Index al español chileno. Material y Método: Estudio transversal que incluyó a 151 niños y niñas entre 3 y 15 años. Se siguieron los 5 pasos recomendados por Beaton. La versión original del pVHI en inglés fue traducida al español por dos traductoras. Posteriormente, la versión traducida y unificada fue revisada por una lingüista chilena quien realizó la adaptación cultural al idioma español chileno. El cuestionario se aplicó a dos grupos de estudio, un grupo de niños con disfonía (n = 51) y un grupo control de niños sin alteraciones de la voz (n = 100) para obtener la versión final. Resultados: Se encontraron diferencias significativas entre el grupo de niños con disfonía y el grupo control en la puntuación global del pVHI y las diferentes subescalas (p < 0,001). Se encontró una consistencia interna óptima con un excelente alfa de Cronbach (α = 0,93), con una alta fiabilidad test-retest (puntuación de correlación de Pearson = 0,95). Conclusión: La versión chilena del cuestionario pVHI presenta un alto grado de validez y confiabilidad. Recomendamos su uso e implementación como protocolo estándar en la evaluación y seguimiento de la voz pediátrica.

Le rilzabrutinib (SAR444671) est un inhibiteur de la tyrosine kinase de Bruton (iBTK) covalent, réversible et oral. La BTK est exprimée à la fois dans les mastocytes et les lymphocytes B et joue un rôle essentiel dans de multiples processus pathologiques à médiation immunitaire, y compris ceux impliqués dans l’urticaire chronique spontanée (UCS). Nous rapportons ici l’analyse de la sécurité d’emploi de l’étude de phase 2 RILECSU chez des adultes atteints d’UCS modérée à sévère randomisés pour recevoir le rilzabrutinib ou le placebo pendant la période en double aveugle de 12 semaines.

Se ha construido un protocolo multiprofesional con participación de pediatra, psicóloga, matronas, obstetra y servicio social que sigue el caso con reuniones grupales y de especialidad desde antes del parto hasta después del desenlace, cualquiera sea éste. Abstract

Introducción: en urgencias el dolor torácico es la segunda causa de consulta después del trauma. Los sín­tomas en orden de frecuencia son: osteomusculares, cardiovasculares, respiratorios, gastrointestinales y otros. Presentación del caso: hombre de 29 años con dolor precordial opresivo de siete horas de evolución; el ECG reportó una lesión subepicárdica de cara inferior. Se pensó en infarto agudo del miocardio de cara inferior vs pericarditis. Se inició terapia anti-isquémica. El ecocardiograma evidenció discreto aumento de la refringencia del pericardio adyacente a la pared inferior del ventrículo izquierdo y función biventricular conservada. La evolución fue satisfactoria y se dió de alta a las 72 horas. Conclusiones: ante dolor toráci­co en urgencias hay que considerar la pericarditis y la miopericarditis, cuyos diagnósticos se basan en la historia clínica, el ECG, los biomarcadores y el ecocardiograma transtorácico. El diagnóstico temprano y manejo oportuno reduce las complicaciones. Abreviaturas: 1AM, infarto del miocardio; ECG, electrocardiograma.

Resumen El bruxismo es una condición que se observa frecuentemente en los pacientes y estos a menudo consultan por sus consecuencias físicas, como pueden ser el desgaste o destrucción dentaria, el dolor orofacial, así como también por el deterioro de la calidad de vida tanto de ellos mismos como de las personas cercanas. En la última década han aumentado en forma importante las investigaciones en torno a esta condición, así como los consensos en cuanto a su definición, clasificación y manejo clínico. Los dentistas son quienes actualmente reconocen esta actividad parafuncional y manejan estos problemas, pero es importante que los profesionales de otras áreas de la salud, como médicos y en especial otorrinolaringólogos, puedan identificar los signos, síntomas y consecuencias del bruxismo, ayudando en la detección de esta condición. Así, el objetivo de esta revisión es establecer un estado del arte sobre bruxismo e incentivar la formación de equipos multidisciplinares que ayuden en el diagnóstico y la terapéutica de esta condición.
