Background Etiological diagnosis of febrile illnesses in returning travelers is a great challenge, particularly when presenting with no focal symptoms [acute undifferentiated febrile illnesses (AUFI)], but is crucial to guide clinical decisions and public health policies. In this study, we describe the frequencies and predictors of the main causes of fever in travelers. Methods Prospective European multicenter cohort study of febrile international travelers (November 2017-November 2019). A predefined diagnostic algorithm was used ensuring a systematic evaluation of all participants. After ruling out malaria, PCRs and serologies for dengue, chikungunya and Zika viruses were performed in all patients presenting with AUFI

BACKGROUND: Few previous retrospective studies suggest that Plasmodium ovale wallikeri seems to have a longer latency period and produces deeper thrombocytopaenia than Plasmodium ovale curtisi. Prospective studies were warranted to better assess interspecies differences. METHODS: Patients with imported P. ovale spp. infection diagnosed by thick or thin film, rapid diagnostic test (RDT) or polymerase chain reaction (PCR) were recruited between March 2014 and May 2017. All were confirmed by DNA isolation and classified as P. o. curtisi or P. o. wallikeri using partial sequencing of the ssrRNA gene. Epidemiological, analytical and clinical differences were analysed by statistical methods. RESULTS: A total of 79 samples (35 P. o. curtisi and 44 P. o. wallikeri) were correctly genotyped. Males predominate in wallikeri group (72.7%), whereas were 48.6% in curtisi group. Conversely, 74.3% of curtisi group were from patients of African ethnicity, whilst 52.3% of Caucasians were infected by P. o. wallikeri. After performing a multivariate analysis, more thrombocytopaenic patients (p = 0.022), a lower number of platelets (p = 0.015), a higher INR value (p = 0.041), and shorter latency in Caucasians (p = 0.034) were significantly seen in P. o. wallikeri. RDT sensitivity was 26.1% in P. o. curtisi and 42.4% in P. o. wallikeri. Nearly 20% of both species were diagnosed only by PCR. Total bilirubin over 3 mg/dL was found in three wallikeri cases. Two patients with curtisi infection had haemoglobin under 7 g/dL, one of them also with icterus. A wallikeri patient suffered from haemophagocytosis. Chemoprophylaxis failed in 14.8% and 35% of curtisi and wallikeri patients, respectively. All treated patients with various anti-malarials which included artesunate recovered. Diabetes mellitus was described in 5 patients (6.32%), 4 patients of wallikeri group and 1 curtisi. CONCLUSIONS: Imported P. o. wallikeri infection may be more frequent in males and Caucasians. Malaria caused by P. o. wallikeri produces more thrombocytopaenia, a higher INR and shorter latency in Caucasians and suggests a more pathogenic species. Severe cases can be seen in both species. Chemoprophylaxis seems less effective in P. ovale spp. infection than in P. falciparum, but any anti-malarial drug is effective as initial treatment. Diabetes mellitus could be a risk factor for P. ovale spp. infection. Most of the studies reporting symptoms during the travel are done retrospectively (1),(2), increasing the odds of recall bias. Moreover, there is no information about the percentage of travelers that suffer mild or no symptoms during their trips because they never attend to a clinic afterwards. A IM The main aim of the study is to detect incidence of symptoms real-time amongst travelers visiting tropical and subtropical countries. Also, comparing symptoms between travelers taking or not malaria chemoprophylaxis and other demographic variables.

INTRODUCTION: Cutaneous anthrax is a zoonotic disease caused by the spore-forming bacterium Bacillus anthracis, which typically presents with ulcers after contact with animals or animal products, and is rarely seen in high-income countries but is common in those with low- and middle-incomes. Objective. The aim of this study is to show the main clinical characteristics of cutaneous anthrax in endemic areas. MATERIAL AND METHODS: The study describes the main clinical characteristics of cutaneous anthrax in eight patients (six female and two male, age range 1 - 56 years) admitted to the rural General Hospital of Gambo, West Arsi Province of Ethiopia from 2010-2013. RESULTS: In all cases, lesions began as an erythematous papule located on exposed sites (n=7 head; n=1 thigh) and subsequently became a necrotic black eschar surrounded by an edematous halo. Two patients presented with painful ipsilateral adenopathy near the black eschar. Four patients developed a malignant pustule on the suborbital region of the face. Patients responded positively to treatment, and the lesions resolved, leaving eschars. However, one patient suffered the loss of an eyeball, and another died 12 hours after starting treatment. CONCLUSIONS: Physicians working in rural areas of resource-poor settings should be trained in the clinical identification of cutaneous anthrax. Early antibiotic treatment is essential for decreasing morbidity and mortality.

The chemical composition of virgin olive oils (95 samples) from Aragón (Spain) from two successive crop seasons (1997/98 and 1998/99) and its relationship with quality and oxidative stability is examined. The main characteristics were: free acidity 0.5 (% oleic acid), peroxide value 11.8 meq/kg, K232 1.92, K270 0.13, oxidative stability 51.4 h (Rancimat method), and antioxidant phenolic substances 168.5 mg/kg (as caffeic acid). From the distribution of fatty acids, a high percentage of linoleic acid (9.46 %) and a low level of stearic (2.10 %) and arachidic (0.36 %) acids, is worth noting. The effect of sample origin (7 productive regions from Aragón), olive variety (Empeltre and Arbequina), and the different steps through processing are also investigated. However, the major factor affecting results was the plague of olive flies during 1997/98 crop season.Se ha estudiado la composición química de 95 muestras de aceite de oliva virgen de Aragón procedente de dos campañas sucesivas (1997/98 y 1998/99) y su relación con la calidad y la estabilidad oxidativa. Las principales características fueron una acidez de 0,5 (% ácido oleico), índice de peróxidos 11,8 meq/kg, K232 1,92, K270 0,13, estabilidad a la oxidación 51,4 h (método Rancimat) y contenido en polifenoles antioxidantes 168,5 mg/kg (como ácido caféico). Del perfil de ácidos grasos, destaca el elevado contenido en linoleico (9,46 %) y las bajas tasas de esteárico (2,10 %) y aráquico (0,36 %). Se analiza el efecto de la procedencia de las muestras (7 comarcas productoras de Aragón), de la variedad de la aceituna (Empeltre o Arbequina) y de las distintas fases del procesado del aceite. Se destaca la gran influencia sobre la calidad del aceite de la plaga de mosca sufrida durante la campaña de 1997/98.

At a time when the Health Effects Institute, Centers for Disease Control, and Environmental Protection Agency are creating an Environmental Public Health Tracking Program on Air Pollution Effects in the USA, it seemed useful to share the experience acquired since 1999 by the Apheis project (Air Pollution and Health-A European Information System), which has tracked the effects of air pollution on health in 26 European cities and continues to do so as the new Aphekom project. In particular, this paper first describes the continuing impact of air pollution on health in Europe, how the Apheis project came to be and evolved, what its main objectives and achievements have been, and how the project benefited its participants. The paper then summarizes the main learnings of the Apheis project. © The Author(s) 2009.
