1. Measurements of ${}^{nat}Pb(p,xn){}^{201-207}Bi$, ${}^{204}Pb(p,1-4n){}^{201-204}Bi$ and ${}^{206}Pb(p,3n){}^{204}Bi$ cross-sections at astrophysical energies ($E_p \leq 30 \, MeV$)

Abstract

Cross-sections for ${}^{nat}Pb(p,xn){}^{201-207}Bi$, ${}^{204}Pb(p,1-4n){}^{201-204}Bi$ and ${}^{206}Pb(p,3n){}^{204}Bi$ reactions have been determined in the astrophysical energy range $20-30 \, MeV$. The analysis were performed by $\gamma$-spectroscopy associated with half-lives measurements. The results were compared with previously experimental data, when available, and with theoretical calculations performed using TALYS code. We report a possible new $\gamma$ transition from ${}^{204m}{Pb}$ and other theoretical discrepancies, probably due overestimation of the Coulomb barrier and neutron binding energy. Also reported are new determinations of the energies and intensities of the $\gamma$'s emitted in the decays of ${}^{205,206}Bi$. We discuss possible implications for the r/rp-processes in neutrino-driven wind in supernovae and neutron stars mergers.

Abstract

The 6.1, 6.9 and 7.1 MeV photon yields in the reaction with protons in the 1.32 to 1.42 MeV energy range were determined from the comparison between experimental and simulated spectrum. The gamma-ray spectra were measured with a HPGe detector which was 72.5 mm in diameter and 60.5 mm in length. The reaction kinematics and the detector response function were simulated by different programs using the Monte Carlo method. The relative photon yields were obtained from the least-squares fit of the experimental gamma-ray spectra to the convolution of the detector response function with the photon energy distribution arising from the reaction kinematics., Comment: Proceedings of 2007 International Conference on Nuclear Data for Science and Technology, 4 pages and 4 figures

Currently, the standard treatment with transarterial chemoembolisation (TACE) is unsatisfactory with high recurrence rate and limited effect on survival. Laser Ablation (LA) has emerged as a relatively new technique characterized by high efficacy and good safety. This study is aimed to evaluate the efficacy of LA in comparison to TACE in patients with large HCC. Methods: Eighty-two patients with a single HCC nodule ≥40 mm (BCLC stage A or B) were enrolled in this case-control study. Forty-one patients were treated with LA and 41 patients were treated with TACE. Response to therapy was evaluated according to the mRECIST criteria. Survival was calculated with Kaplan-Meier from the time of cancer diagnosis to death with values censored at the date of the last follow-up. Results: Twenty-six (63.4%) and 8 (19.5%) patients had a complete response after LA and TACE, respectively ( p 60 mm. LA resulted superior to TACE especially in nodules ranging between 51 and 60 mm in diameter, with a complete response rate post-LA and post-TACE of 75% and 14.3%, respectively ( p = 0.0133). The 36 months cumulative survival rate in patients treated with LA and TACE was 55.4% and 48.8%, respectively. The disease recurrence rates after LA and TACE were 19.5% and 75.0%, respectively. Conclusions: LA is a more effective therapeutic option than TACE in patients with solitary large HCC.

Abstract

Background & Aims Hepatitis C virus (HCV) is the leading aetiological factor of HCC in the western world where, overall, its incidence is increasing, despite data suggesting an initial drop in some areas. The aim of this study was to evaluate epidemiology, clinical features and survival of HCV-related HCC (HCV-HCC) in a wide time range in Italy. Methods Multicentre retrospective study including 3695 patients prospectively recruited by the ITA.LI.CA group. Patients were classified into three subgroups according to aetiology (Group A[GA], pure HCV; Group B[GB], HCV + cofactors; and Group C[GC], non-HCV) and in 5 time cohorts (5 years each), according to the year of diagnosis. Abstract

none 31 no Nonalcoholic fatty liver disease (NAFLD) represents the hepatic manifestation of metabolic syndrome and may evolve into hepatocellular carcinoma (HCC). Only scanty clinical information is available on HCC in NAFLD. The aim of this multicenter observational prospective study was to assess the clinical features of patients with NAFLD-related HCC (NAFLD-HCC) and to compare them to those of hepatitis C virus (HCV)-related HCC. A total of 756 patients with either NAFLD (145) or HCV-related chronic liver disease (611) were enrolled in secondary care Italian centers. Survival was modeled according to clinical parameters, lead-time bias, and propensity analysis. Compared to HCV, HCC in NAFLD patients had a larger volume, showed more often an infiltrative pattern, and was detected outside specific surveillance. Cirrhosis was present in only about 50% of NAFLD-HCC patients, in contrast to the near totality of HCV-HCC. Regardless of tumor stage, survival was significantly shorter (P = 0.017) in patients with NAFLD-HCC, 25.5 months (95% confidence interval 21.9-29.1), than in those with HCV-HCC, 33.7 months (95% confidence interval 31.9-35.4). To eliminate possible confounders, a propensity score analysis was performed, which showed no more significant difference between the two groups. Additionally, analysis of patients within Milan criteria submitted to curative treatments did not show any difference in survival between NAFLD-HCC and HCV-HCC (respectively, 38.6 versus 41.0 months, P = nonsignificant) CONCLUSIONS: NAFLD-HCC is more often detected at a later tumor stage and could arise also in the absence of cirrhosis, but after patient matching, it has a similar survival rate compared to HCV infection; a future challenge will be to identify patients with NAFLD who require more stringent surveillance in order to offer the most timely and effective treatment. Abstract

The method of group capacity regulation of centrifugal mechanisms is proposed to solve the problem of increasing the energy efficiency of thermal power plant auxiliaries. The method is based on using a group frequency converter for basic capacity regulation of centrifugal mechanisms. Additional regulation is commonly carried out by throttling, bypassing or changing the guide vane opening angle. Analysis of auxiliary power consumption showed that due to ineffective regulation methods, the share of power consumption can reach 50 %. A mathematical model of the centrifugal mechanism that allows for studying various regulation methods is developed. The task of finding optimal control parameters for a group of centrifugal mechanisms that ensure power boiler operation is stated and solved. By the results obtained, it was determined that group capacity regulation of boiler draft mechanisms is most rational. For the feed pump, an individual variable-frequency drive shall be used. In contrast to the individual variable-frequency drive, the introduction of the group regulation method requires less investment. This method can significantly reduce the auxiliary power consumption of thermal power plants. The projected decrease in auxiliary power consumption is 10.7 %. The results show the high efficiency of the group capacity regulation of centrifugal mechanisms, so this method can become a promising direction in increasing the energy efficiency of thermal power plants. Abstract

Background & Aims Significant proportion of Hepatocellular Carcinoma (HCC) cases are diagnosed in stage B of Barcelona Clinic Liver Cancer (BCLC) algorithm, in which the standard of care is Transcatheter Arterial ChemoEmbolization (TACE). We aimed to ascertain adherence to current guidelines, survival and prognostic factors in BCLC stage B patients. Methods From 3027 HCC cases recruited from 1986 to 2008 by the Italian Liver Cancer group (2430 with data allowing a correct allocation in the BCLC system), a retrospective analysis was conducted on those diagnosed in BCLC stage B (405 patients, 17%). Statistics were performed with Kaplan–Meier (log rank) method and Cox multivariate analysis. Results Median overall survival in BCLC stage B patients was 25 months (Confidence Interval - C.I. - 22–28 months) with a 5-year survival of 18%. Child–Pugh class, oesophageal varices and Alpha-foetoprotein (AFP) were the independent predictors of survival. Abstract

A geometric model of a new method of delivering fire-extinguishing substances to a fire zone located at a considerable distance was offered. The idea of delivery is based on the mechanical action of throwing. To this end, a substance (e.g. extinguishing powder) is loaded in a hard shell made as a special container. After delivery by means of a launcher to a fire zone, the container has to release the substance which will promote fire extinguishing. The known method of remote delivery of extinguishing substances uses a pneumatic gun with a cylindrical container. During delivery, the cylinder must rotate around its axis to ensure flight stability. The cylinder is rotated by a special turbine when passing through the gun barrel. There are difficulties in regulating the distribution of compressed air flows during the turbine operation. In addition, the tightness of the pneumatic part of the gun should be monitored. The new delivery method uses a container in a form of two spaced loads similar to a sports dumbbell. The dumbbell motion is initiated by simultaneous action of explosion-generated pulses directed at each of its loads in a pre-calculated manner. This results in the rotational motion of the container. To describe the dynamics of the dumbbell motion, a Lagrangian was defined and a system of Lagrange differential equations of the second kind was set up and solved. Examples of modeling trajectories of the centers of masses of the dumbbell loads taking into account air resistance were given. The proposed method is planned to be a basis of a new fire extinguishing technology. This is evidenced by the new scheme of launching the dumbbell by means of explosion-generated pulses of charges of two pyro cartridges. Abstract

To verify whether it is possible, in people with diabetes mellitus (DM) considered at very high cardiovascular (CV) risk, stratify this risk better and identify significant modifiable risk factor (including lifestyle habits) to help patients and clinicians improve CV prevention. People with DM and microvascular diseases or one or more CV risk factors (hypertension, hyperlipidemia, smoking, poor dietary habits, overweight, physical inactivity) included in the Risk and Prevention study were selected. We considered the combined endpoint of non-fatal acute myocardial infarction and stroke and CV death. A multivariate Cox proportional analysis was carried out to identify relevant predictors. We also used the RECPAM method to identify subgroups of patients at higher risk. In our study, the rate of major CV events was lower than expected (5 % in 5 years). Predictors of CV events were age, male, sex, heart failure, previous atherosclerotic disease, atrial fibrillation, insulin treatment, high HbA1c, heart rate and other CV diseases while being physically active was protective. RECPAM analysis indicated that history of atherosclerotic diseases and a low BMI defined worse prognosis (HR 4.51 95 % CI 3.04–6.69). Among subjects with no previous atherosclerotic disease, men with HbA1c more than 8 % were at higher CV risk (HR 2.77; 95 % CI 1.86–4.14) with respect to women. In this population, the rate of major CV events was lower than expected. This prediction model could help clinicians identify people with DM at higher CV risk and support them in achieving goals of physical activity and HbA1c.

Abstract

Lead-time is the time by which diagnosis is anticipated by screening/surveillance with respect to the symptomatic detection of a disease. Any screening program, including surveillance for hepatocellular carcinoma (HCC), is subject to lead-time bias. Data regarding lead-time for HCC are lacking. Aims of the present study were to calculate lead-time and to assess its impact on the benefit obtainable from the surveillance of cirrhotic patients. Background & Aims: Lead-time is the time by which diagnosis is anticipated by screening/surveillance with respect to the symptomatic detection of a disease. Any screening program, including surveillance for hepatocellular carcinoma (HCC), is subject to lead-time bias. Data regarding lead-time for HCC are lacking. Aims of the present study were to calculate lead-time and to assess its impact on the benefit obtainable from the surveillance of cirrhotic patients. Methods: One-thousand three-hundred and eighty Child–Pugh class A/B patients from the ITA.LI.CA database, in whom HCC was detected during semiannual surveillance (n = 850), annual surveillance (n = 234) or when patients came when symptomatic (n = 296), were selected. Lead-time was estimated by means of appropriate formulas and Monte Carlo simulation, including 1000 patients for each arm. Abstract

Головною метою даної публікації є розробка теоретичних основ для побудови та використання комп'ютерних динамічних геометричних моделей різних технічних об'єктів. Актуальність наведеної задачі обумовлена особливою важливістю процесів формоутворення під час проектування, виготовлення й експлуатації промислової продукції. Сучасні комп'ютерні технології, відповідне методичне, математичне, інформаційне та програмне забезпечення відкривають великі перспективи в цьому напрямі. Запропонована методика комп'ютерного динамічного геометричного моделювання проілюстрована на прикладі сучасного літака як одного з типових представників складних технічних об'єктів. The modern stage of the scientific-technical progress is based on the wide use of information technologies for creation of different technical objects. Therefore, the elaboration of new effective methods for computer-aided design is relevant now. The purpose of this article is the development of the theoretical basis for the computer dynamic geometric models of different technical objects. The urgency of this problem is caused by the special importance of the geometric processes in the design, manufacture and operation of industrial products. Computer technologies and the corresponding methodical, mathematical, information and software support offer great perspectives in this direction. The geometric models perform an integrating role for many computational models of technical objects. This applies to the design models, the strength models, the technological models etc. The proposed methodology of the dynamic geometric modeling is illustrated by the example of the modern airplane as one of the typical representatives of the complex technical objects. The basic techniques of this methodology were shown. The main emphasis was placed on their universal role for computer modeling of various technical objects and processes. The general classification of used geometric objects was made. The dynamic transformations of various geometric objects were arranged. Some perspectives of further improvement of proposed techniques were analyzed. The outlined information is the theoretical base for the development of the appropriate software for computer-aided design of different technical objects and processes for their manufacture and use. The proposed methodology is a further improvement of structural-parametric approach for geometric modeling of technical objects which was developed by the scientific school of applied geometry at National Technical University of Ukraine «Kyiv Polytechnic Institute». The considered methodology of computer dynamic geometric modeling can be the basis for the complex optimization of various technical objects. This requires the further intensive theoretical researches and introduction of the received results into practice.
