2. Refraction beats attenuation in breast CT

Abstract

For a century, clinical X-ray imaging has visualised only the attenuation properties of tissue, which fundamentally limits the contrast, particularly in soft tissues like the breast. Imaging based on refraction can overcome this limitation, but so far has been constrained to high-dose ex-vivo applications or required highly coherent X-ray sources, like synchrotrons. It has been predicted that grating interferometry (GI) could eventually allow computed tomography (CT) to be more dose-efficient. However, the benefit of refraction in clinical CT has not been demonstrated so far. Here we show that GI-CT is more dose-efficient in imaging of breast tissue than conventional CT. Our system, based on a 70kVp X-ray tube source and commercially available gratings, demonstrated superior quality, in terms of adipose-to-glandular tissue contrast-to-noise ratio (CNR), of refraction-contrast compared to the attenuation images. The fusion of the two modes of contrast outperformed conventional CT for spatial resolutions better than 263{\mu}m and an average dose to the breast of 16mGy, which is in the clinical breast CT range. Our results show that grating interferometry can significantly reduce the dose, while maintaining the image quality, in diagnostic breast CT. Unlike conventional absorption-based CT, the sensitivity of refraction-based imaging is far from being fully exploited, and further progress will lead to significant improvements of clinical X-ray CT.

Abstract

PURPOSE: The second International Consensus Conference on B3 lesions was held in Zurich, Switzerland, in March 2018, organized by the International Breast Ultrasound School to re-evaluate the consensus recommendations. METHODS: This study (1) evaluated how management recommendations of the first Zurich Consensus Conference of 2016 on B3 lesions had influenced daily practice and (2) reviewed current literature towards recommendations to biopsy. RESULTS: In 2018, the consensus recommendations for management of B3 lesions remained almost unchanged: For flat epithelial atypia (FEA), classical lobular neoplasia (LN), papillary lesions (PL) and radial scars (RS) diagnosed on core-needle biopsy (CNB) or vacuum-assisted biopsy (VAB), excision by VAB in preference to open surgery, and for atypical ductal hyperplasia (ADH) and phyllodes tumors (PT) diagnosed at VAB or CNB, first-line open surgical excision (OE) with follow-up surveillance imaging for 5 years. Analyzing the Database of the Swiss Minimally Invasive Breast Biopsies (MIBB) with more than 30,000 procedures recorded, there was a significant increase in recommending more frequent surveillance of LN [65% in 2018 vs. 51% in 2016 (p = 0.004)], FEA (72% in 2018 vs. 62% in 2016 (p = 0.005)), and PL [(76% in 2018 vs. 70% in 2016 (p = 0.04)] diagnosed on VAB. A trend to more frequent surveillance was also noted also for RS [77% in 2018 vs. 67% in 2016 (p = 0.07)]. CONCLUSIONS: Minimally invasive management of B3 lesions (except ADH and PT) with VAB continues to be appropriate as an alternative to first-line OE in most cases, but with more frequent surveillance, especially for LN.

Abstract

Családi Kör (1860–1880) was the leading Hungarian domestic magazine of the mid-19th period, which, under the editorship of the first Hungarian woman of letters Emília Kánya, played a major role in introducing the domestic readership to contemporary European literature and in discussing the struggle of women’s employment opportunities before a wider public. Critical studies have also suggested that it was edited and published under the influence of the German Gartenlaube (1853–1944), the journal credited with embedding the journal of domestic magazine in the broader regime of 19th century print culture. Based on a close reading of the two magazines’ coverage in its European cultural historical context, this chapter offers an account of the possible connections and affinities between the two periodicals, and argues that the Hungarian magazine was significantly more daring in its politics and more systematic in its pursuit of introducing the local audience to European literary trends and works.

Abstract

Aim: Predicting neurodevelopmental outcomes in hypoxic‐ischaemic encephalopathy (HIE) remains imprecise, despite advanced imaging and neurophysiological tests. We explored the predictive value of socio‐economic status (SES). Methods: The cohort comprised 93 infants (59% male) with HIE, who had received therapeutic hypothermia. Patients underwent magnetic resonance imaging, and brain injuries were quantified using the Barkovich scoring system. Family SES was self‐reported using a questionnaire. Adverse outcomes were defined as mild to severely delayed development with a score of ≤85 in any domain at 2 years of age, based on the Bayley Scales of Infant Development, Second Edition. Data are presented as odds ratios (OR) with 95% confidence intervals (95% CI). Results: Multiple regression modelling revealed that higher parental education was strongly associated with good cognitive development, when adjusted for gestational age, serum lactate and brain injuries (OR 2.20, 95% CI 1.16–4.36). The effect size of parental education (β = 0.786) was higher than one score for any brain injury using the Barkovich scoring system (β = −0.356). The literacy environment had a significant effect on cognitive development in the 21 infants who had brain injuries (OR 40, 95% CI 3.70–1352). Conclusion: Parental education and the literacy environment influenced cognitive outcomes in patients with HIE. [ABSTRACT FROM AUTHOR]
