Benzophenone-2 (2,2', 4,4'- Tetrahydroxybenzophenone; BP-2) is widely used as a sunscreen in Personal and Care Products (PCPs) for protection against ultraviolet (UV) radiation. The effects of BP-2 on random-sex adult zebrafish (Danio rerio) cytochrome P450 (CYP450) were studied. The main goal was to investigate the detoxification mechanisms underlying the adverse consequences of exposure to xenobiotic chemicals such as BP-2. Total protein content, CYP450 content, and erythromycin N-demethylase (ERND) activity were evaluated as indicators of protein CYP3A expression. Five sets of pooled random-sex adult zebrafish were exposed to 0.0, 0.1, 5.0, and 10.0 mg/L of BP-2 to evaluate their acute and chronic toxicity (4 and 15 days, respectively). ERND activity was significantly increased in the chronic toxicity group compared to that in the control group, whereas CYP450 remained unchanged. The results suggest a sufficiently fast catalytic process that does not alter the total CYP450 content. It implies a mediation of CYP450 3A induction by BP-2 and the pregnane X receptor ligand-binding domain (PXR LBD) interaction. Ligand-protein interactions were confirmed via in silico docking with AutoDock Vina. Further computational studies indicate BP-2 potential binding affinity for the Estrogen receptor alpha ligand binding domain (ERα LBD). These results suggest that CYPs effects may result in significant toxicity in the zebrafish. The benzophenone (BP) family, including oxybenzone (BP-3), a prevalent sunscreen ingredient and environmental contaminant, has raised concerns since the year 2005. This study investigated oxybenzone toxicity in zebrafish ( Danio rerio ) eleutheroembryos and brine shrimp ( Artemia salina ) nauplii, focusing on the LC 50 and developmental impacts. Zebrafish embryos (0.100-1.50 mg/L BP-3, 96 h) and A. salina (0.100-5.00 mg/L BP-3, 48 h) were tested with ultrasound-assisted emulsified liquid-phase microextraction (UA-ELPME) used for zebrafish tissue analysis. HPLC-DAD determined BP-3 concentrations (highest: 0.74 ± 0.13 mg/L). Although no significant zebrafish embryo mortality or hatching changes occurred, developmental effects were evident. Lethal concentrations were determined ( A. salina LC 50 at 24 h = 3.19 ± 2.02 mg/L; D. rerio embryos LC 50 at 24 h = 4.19 ± 3.60 mg/L), with malformations indicating potential teratogenic effects. A. salina displayed intestinal tract alterations and D. rerio embryos exhibited pericardial edema and spinal deformities. These findings highlight oxybenzone's environmental risks, posing threats to species and ecosystem health.

Among nanoparticles (NPs), titanium dioxide is one of the most highly manufactured worldwide and widely used in multiple products for both industrial use and personal care products. This increases the probability of release into aquatic environments, potentially affecting these ecosystems. The present study aimed to evaluate TiO 2 P25 NP toxicity in zebrafish embryos and eleutheroembryos by evaluating LC 50 , hatching rate, embryo development, and chemical analysis of the TiO 2 concentration accumulated in eleutheroembryo tissues. Zebrafish embryos ~2 h post-fertilization (hpf) were exposed to 75, 100, 150, 200, and 250 mg/L TiO 2 P25 NPs for 48 and 96 h. A total of 40-60 embryos were placed in each Petri dish for the respective treatments. Three replicates were used for each treatment group. Ti 4+ concentrations were determined by inductively coupled plasma optical emission spectrometry (ICP-OES), and a conversion factor was used to calculate the TiO 2 concentrations in the tissues. The highest calculated concentrations of TiO 2 in zebrafish larvae were 1.0199 mg/L after 48 h and 1.2679 mg/L after 96 h of exposure. The toxicological results indicated that these NPs did not have a significant effect on the mortality and hatching of zebrafish embryos but did have an effect on their development. LC 20 and LC 30 were determined experimentally, and LC 50 and LC 80 were estimated using four different methods. Up to 11% of embryos also presented physical malformations. These effects can be detrimental to a species and affect ecosystems. Physical malformations were observed in all treatments, indicating teratogenic effects.

The widespread use of zinc oxide nanoparticles (ZnO NPs) in multiple applications has increased the importance of safety considerations. ZnO NPs were synthesized, characterized, and evaluated for toxicity in Artemia salina and zebrafish ( Danio rerio ). NPs were characterized by X-ray diffraction (XRD), Fourier-transform infrared spectroscopy (FT-IR), and ultraviolet-visible (UV-Vis) spectroscopy. The hydrodynamic size and stability of the ZnO NP surface were examined using a Zetasizer. Characterization techniques confirmed the ZnO wurtzite structure with a particle size of 32.2 ± 5.2 nm. Synthesized ZnO NPs were evaluated for acute toxicity in Artemia salina using the Probit and Reed and Muench methods to assess for lethal concentration at 50% (LC50). The LC50 was 86.95 ± 0.21 μg/mL in Artemia salina . Physical malformations were observed after 96 h at 50 μg/mL of exposure. The total protein and cytochrome P450 contents were determined. Further analysis was performed to assess the bioaccumulation capacity of zebrafish ( Danio rerio ) using ICP-OES. ZnO NP content in adult zebrafish was greater in the gastrointestinal tract than in the other tissues under study. The present analysis of ZnO NPs supports the use of Artemia salina and adult zebrafish as relevant models for assessing toxicity and bioaccumulation while considering absorption quantities.

Rotavirus (RV) infection causes acute rotavirus gastroenteritis (RVGE) in infants. Safe and effective RV vaccines are available, of which Mexico has included one in its national immunization program (NIP) since 2007. Health outcome gains, expressed in quality-adjusted life years (QALYs), and cost improvements are important additional factors for the selection of a NIP vaccine. These two factors were analyzed here for Mexico over one year implementing three RV vaccines: 2-dose Rotarix (HRV), versus 3-dose RotaTeq (HBRV), and 3-dose Rotasiil (BRV-PV), presented in a 1-dose or 2-dose vial). HRV would annually result in discounted QALY gains of 263 extra years compared with the other vaccines by averting an extra 24,022 homecare cases, 10,779 medical visits, 392 hospitalizations, and 12 deaths. From a payer's perspective and compared with HRV, BRV-PV 2-dose vial and BRV-PV 1-dose vial would annually result in $13,548,179 and $4,633,957 net savings, respectively, while HBRV would result in $3,403,309 extra costs. The societal perspective may also show savings compared with HRV for BRV-PV 2-dose vial of $4,875,860, while BRV-PV 1-dose vial and HBRV may show extra costs of $4,038,363 and $12,075,629 respectively. HRV and HBRV were both approved in Mexico, with HRV requiring less investment than HBRV with higher QALY gains and cost savings. The HRV vaccine produced those higher health gains due to its earlier protection and greater coverage achieved after its schedule completion with two doses only, providing full protection at four months of age instead of longer periods for the other vaccines.

Introduction: Diarrhoeal illness is the second cause of morbidity/mortality among children from less-developed regions worldwide. Nonetheless, there is scarce information regarding their gut microbiome., Aim: Microbiome characterization, with an emphasis on the virome, of children's stools with diarrhoea, by a commercial microbiome array., Methodology: Nucleic acids extraction, optimised for viral identification, of stool samples from 20 Mexican children with diarrhoea (10 children < 2 and 10 ≥ 2-years-old), collected 16 years ago and kept at -70 °C, were analysed for the presence of viruses, bacteria, archaea, protozoa, and fungi species sequences., Results: Only viral and bacterial species sequences were identified among children's stools. Most stool samples harboured species belonging to the bacteriophages (95%), anellovirus (60%), diarrhoeagenic viruses (40%), and non-human pathogens viruses (45% avian virus and 40% plant viruses) groups. Among the children's stools, virome inter-individual species composition was observed, even in presence of illness. The < 2-years-old children group has significantly higher viral richness (p = 0.01), conferred mainly by bacteriophages and diarrheagenic-viruses (p = 0.01) species, in comparison with the ≥ 2-years-old group., Conclusions: The virome of stools of children with diarrhoea revealed inter-individual viral species composition. Similarly, to the few virome studies in healthy young children, the bacteriophages group was the most abundant. A significantly higher viral richness, conferred by bacteriophages and diarrheagenic-viral species, was observed among < 2-years-old children in comparison with older children. This paper examines the application of electroencephalogram-based methods to assess the effects of audio-tactile substitution training in young, profoundly deaf (PD) participants, with the aim of analyzing the neural mechanisms associated with vibrotactile complex sound discrimination. Electrical brain activity reflects dynamic neural changes, and the temporal precision of event-related potentials (ERPs) has proven to be key in studying time-locked processes while performing behavioral tasks that involve attention and working memory. The current protocol was designed to study electrophysiological activity in PD subjects while they performed a continuous performance task (CPT) using complex-sound stimuli, consisting of five different animal sounds delivered through a portable stimulator system worn on the right index finger. As a repeated-measures design, electroencephalogram (EEG) recordings in standard conditions were performed before and after a brief training program (five 1 h sessions over 15 days), followed by offline artifact correction and epoch averaging, to obtain individual and grand-mean waveforms. Behavioral results show a significant improvement in discrimination and a more robust P3-like centroparietal positive waveform for the target stimuli after training. In this protocol, ERPs contribute to the further understanding of learning-related neural changes in PD subjects associated with audio-tactile discrimination of complex sounds.

Challenges in early oral language acquisition in profoundly deaf individuals have an impact on cognitive neurodevelopment. This has led to the exploration of alternative sound perception methods involving training of vibrotactile discrimination of sounds within the language spectrum. In particular, stimulus duration plays an important role in linguistic categorical perception. We comparatively evaluated vibrotactile temporal discrimination of sound and how specific training can modify the underlying electrical brain activity. Fifteen profoundly deaf (PD) and 15 normal-hearing (NH) subjects performed a vibrotactile oddball task with simultaneous EEG recording, before and after a short training period (5 one-hour sessions; in 2.5-3 weeks). The stimuli consisted of 700 Hz pure-tones with different duration (target: long 500 ms; non-target: short 250 ms). The sound-wave stimuli were delivered by a small device worn on the right index finger. A similar behavioral training effect was observed in both groups showing significant improvement in sound-duration discrimination. However, quantitative EEG measurements reveal distinct neurophysiological patterns characterized by higher and more diffuse delta band magnitudes in the PD group, together with a generalized decrement in absolute power in both groups that might reflect a facilitating process associated to learning. Furthermore, training-related changes were found in the beta-band in NH. Objective: Type-1 diabetes (T1D) is a disruptive metabolic disease that has an impact on neurodevelopment through its effects on the structure and function of the brain. One of the cognitive domains affected by T1D is sustained attention. The aim of this study was to analyze this process in children with T1D and compare their results to those of healthy controls., Methods: Seventeen T1D children attending regular primary school and a similar group of healthy children matched by gender, age, handedness, and educational level were evaluated while identifying happy faces in a Go-NoGo task presented visually with simultaneous electrophysiological recording., Results: Behavioral performance in the two groups was similar but, the T1D children showed greater prefrontal and frontoparietal spectral power in the theta and alpha bands, compared to controls. Recent studies aiming to facilitate mathematical skill development in primary school children have explored the electrophysiological characteristics associated with different levels of arithmetic achievement. The present work introduces an alternative EEG signal characterization using graph metrics and, based on such features, a classification analysis using a decision tree model. This proposal aims to identify group differences in brain connectivity networks with respect to mathematical skills in elementary school children. The methods of analysis utilized were signal-processing (EEG artifact removal, Laplacian filtering, and magnitude square coherence measurement) and the characterization (Graph metrics) and classification (Decision Tree) of EEG signals recorded during performance of a numerical comparison task. Our results suggest that the analysis of quantitative EEG frequency-band parameters can be used successfully to discriminate several levels of arithmetic achievement. Specifically, the most significant results showed an accuracy of 80.00% (α band), 78.33% (δ band), and 76.67% (θ band) in differentiating high-skilled participants from low-skilled ones, averaged-skilled subjects from all others, and averaged-skilled participants from low-skilled ones, respectively. The aim of this study was to compare a reconfigurable mobile electroencephalography (EEG) system (M-EMOTIV) based on the Emotiv Epoc® (which has the ability to record up to 14 electrode sites in the 10/20 International System) and a commercial, clinical-grade EEG system (Neuronic MEDICID-05®), and then validate the rationale and accuracy of recordings obtained with the prototype proposed. In this approach, an Emotiv Epoc® was modified to enable it to record in the parieto-central area. All subjects (15 healthy individuals) performed a visual oddball task while connected to both devices to obtain electrophysiological data and behavioral responses for comparative analysis. A Pearson's correlation analysis revealed a good between-devices correlation with respect to electrophysiological measures. The present study not only corroborates previous reports on the ability of the Emotiv Epoc® to suitably record EEG data but presents an alternative device that allows the study of a wide range of psychophysiological experiments with simultaneous behavioral and mobile EEG recordings.

Several executive functions decline with the development of type-1 diabetes (T1D), particularly working memory (WM). In adults, WM ensures efficient cognitive processing by focusing on task-relevant information while suppressing distractors. It has been well documented that WM can be influenced by emotional stimuli, which may facilitate the retention of information, interfere with uptake, or even affect its capacity. We evaluated the effect of T1D on visual WM processing using emotional faces as stimuli, in young patients with satisfactory clinical evolution, and matched controls without T1D. All subjects performed a 2-back task detecting facial identity using neutral, happy or fearful faces in a block design for fMRI. Behavioral performance was similar with the exception that patients responded significantly slower. Most importantly, between-group differences were found in patterns of brain activation. In comparison, more widespread brain activation -predominantly prefrontal- was found in the participants with T1D when processing neutral faces, while a decrease was observed when processing happy and fearful ones. Statistical contrasts demonstrated significantly-different activation patterns between groups when processing emotional faces, as controls exhibited greater activation in the cuneus, posterior cortex and parahippocampal gyrus, while the patients showed greater activation in the prefrontal structures. Results may reflect compensatory efforts made to minimize the deleterious effects of disease development on attention allocation processes and the operational efficiency of WM. This article presents the data related to the research paper entitled "The analysis of EEG coherence reflects middle childhood differences in mathematical achievement" (González-Garrido et al., 2018). The dataset is derived from the electroencephalographic (EEG) records registered from a total of 60 8-9-years-old children with different math skill levels (High: HA, Average: AA, and Low Achievement: LA) while performing a symbolic magnitude comparison task. The average brain patterns are shown through Time-Frequency Representations (TFR) for each group, and also grand-mean amplitudes within specific EEG epochs in a 19-electrode array are provided. Making this information publicly available for further analyses could significantly contribute to a better understanding on how math achievement in children associates with cognitive processing strategies.

The aim of the study was to evaluate the neurofunctional effect of gender in Type-1 Diabetes Mellitus (T1DM) patients during a Visual Spatial Working Memory (VSWM) task. The study included 28 participants with ages ranging from 17-28 years. Fourteen well-controlled T1DM patients (7 female) and 14 controls matched by age, sex, and education level were scanned performing a block-design VSWM paradigm. Behavioral descriptive analyses and mean comparisons were done, and between-group and condition functional activation patterns were also compared. Whole-brain cumulative BOLD signal (CumBS), voxel-wise BOLD level frequency, Euclidean distance, and divergence indices were also calculated. There were no significant differences between or within-group sex differences for correct responses and reaction times. Functional activation analyses showed that females had activation in more brain regions, and with larger clusters of cortical activations than males. Furthermore, BOLD activation was higher in males. Despite the preliminary nature of the present study given the relatively small sample size, current results acknowledge for the first time that sex might contribute to differences in functional activation in T1DM patients. Symbolic numerical magnitude processing is crucial to arithmetic development, and it is thought to be supported by the functional activation of several brain-interconnected structures. In this context, EEG beta oscillations have been recently associated with attention and working memory processing that underlie math achievement. Due to that EEG coherence represents a useful measure of brain functional connectivity, we aimed to contrast the EEG coherence in forty 8-to-9-year-old children with different math skill levels (High: HA, and Low achievement: LA) according to their arithmetic scores in the Fourth Edition of the Wide Range Achievement Test (WRAT-4) while performing a symbolic magnitude comparison task (i.e. determining which of two numbers is numerically larger). The analysis showed significantly greater coherence over the right hemisphere in the two groups, but with a distinctive connectivity pattern. Whereas functional connectivity in the HA group was predominant in parietal areas, especially involving beta frequencies, the LA group showed more extensive frontoparietal relationships, with higher participation of delta, theta and alpha band frequencies, along with a distinct time-frequency domain expression. The results seem to reflect that lower math achievements in children mainly associate with cognitive processing steps beyond stimulus encoding, along with the need of further attentional resources and cognitive control than their peers, suggesting a lower degree of numerical processing automation., (Copyright © 2018 Elsevier Inc. The study of orthographic errors in a transparent language such as Spanish is an important topic in relation to writing acquisition because in Spanish it is common to write pseudohomophones as valid words. The main objective of the present study was to explore the possible differences in activation patterns in brain areas while processing pseudohomophone orthographic errors between participants with high (High Spelling Skills (HSS)) and low (Low Spelling Skills (LSS)) spelling orthographic abilities. We hypothesize that (a) the detection of orthographic errors will activate bilateral inferior frontal gyri, and that (b) this effect will be greater in the HSS group. Two groups of 12 Mexican participants, each matched by age, were formed based on their results in a group of spelling-related ad hoc tests: HSS and LSS groups. During the fMRI session, two experimental tasks were applied involving correct and pseudohomophone substitution of Spanish words. First, a spelling recognition task and second a letter searching task. The LSS group showed, as expected, a lower number of correct responses (F(1, 21) = 52.72, p <.001, η2 = .715) and higher reaction times compared to the HSS group for the spelling task (F(1, 21) = 60.03, p <.001, η2 = .741). However, this pattern was reversed when the participants were asked to decide on the presence of a vowel in the words, regardless of spelling. The fMRI data showed an engagement of the right inferior frontal gyrus in HSS group during the spelling task. However, temporal, frontal, and subcortical brain regions of the LSS group were activated during the same task.

Orthographic competence allows automatic word recognition and reading fluency. To elucidate how the orthographic competence in Spanish-speaking adults might affect the neurofunctional mechanisms of visual word recognition, 32 young adults equally divided in two groups (HSS: High Spelling Skills, and LSS: Low Spelling Skills) were evaluated using fMRI methods, while they performed an orthographic recognition task involving pseudohomophones. HSS achieved significantly more correct responses and lower reaction times than LSS. Interestingly, LSS showed greater activation in the left angular and supramarginal regions with increased bilateral activation pattern in the inferior frontal gyrus, and the anterior temporal and posterior parietal regions. In contrast, HSS showed a more left-lateralized pattern over these regions along with higher activation of the anterior cingulated gyrus for misspelled words. Results suggest that the differences found in cortical activation patterns might be explained by the higher degree of specialization for word recognition in HSS, a group of participants that due to their greater orthographic skills require less engagement of processing resources to succeed in the task., (Copyright © 2017 Elsevier Inc. Children with mathematical difficulties usually have an impaired ability to process symbolic representations. Functional MRI methods have suggested that early frontoparietal connectivity can predict mathematic achievements; however, the study of brain connectivity during numerical processing remains unexplored. With the aim of evaluating this in children with different math proficiencies, we selected a sample of 40 children divided into two groups [high achievement (HA) and low achievement (LA)] according to their arithmetic scores in the Wide Range Achievement Test, 4th ed.. Participants performed a symbolic magnitude comparison task (i.e. determining which of two numbers is numerically larger), with simultaneous electrophysiological recording. Partial directed coherence and graph theory methods were used to estimate and depict frontoparietal connectivity in both groups. The behavioral measures showed that children with LA performed significantly slower and less accurately than their peers in the HA group. Significantly higher frontocentral connectivity was found in LA compared with HA; however, when the connectivity analysis was restricted to parietal locations, no relevant group differences were observed. These findings seem to support the notion that LA children require greater memory and attentional efforts to meet task demands, probably affecting early stages of symbolic comparison.

Early auditory deprivation has serious neurodevelopmental and cognitive repercussions largely derived from impoverished and delayed language acquisition. These conditions may be associated with early changes in brain connectivity. Vibrotactile stimulation is a sensory substitution method that allows perception and discrimination of sound, and even speech. To clarify the efficacy of this approach, a vibrotactile oddball task with 700 and 900 Hz pure-tones as stimuli [counterbalanced as target (T: 20% of the total) and non-target (NT: 80%)] with simultaneous EEG recording was performed by 14 profoundly deaf and 14 normal-hearing (NH) subjects, before and after a short training period (five 1-h sessions; in 2.5-3 weeks). A small device worn on the right index finger delivered sound-wave stimuli. The training included discrimination of pure tone frequency and duration, and more complex natural sounds. A significant P300 amplitude increase and behavioral improvement was observed in both deaf and normal subjects, with no between group differences. However, a P3 with larger scalp distribution over parietal cortical areas and lateralized to the right was observed in the profoundly deaf. A graph theory analysis showed that brief training significantly increased fronto-central brain connectivity in deaf subjects, but not in NH subjects. Together, ERP tools and graph methods depicted the different functional brain dynamic in deaf and NH individuals, underlying the temporary engagement of the cognitive resources demanded by the task. Our findings showed that the index-fingertip somatosensory mechanoreceptors can discriminate sounds. Further studies are necessary to clarify brain connectivity dynamics associated with the performance of vibrotactile language-related discrimination tasks and the effect of lengthier training programs.

The ability to map between non-symbolic and symbolic magnitude representations is crucial in the development of mathematics and this map is disturbed in children with math difficulties. In addition, positive parietal ERPs have been found to be sensitive to the number distance effect and skills solving arithmetic problems. Therefore we aimed to contrast the behavioral and ERP responses in children with different levels of mathematical achievement: low (LA), average (AA) and high (HA), while comparing symbolic and non-symbolic magnitudes. The results showed that LA children repeatedly failed when comparing magnitudes, particularly the symbolic ones. In addition, a positive correlation between correct responses while analyzing symbolic quantities and WRAT-4 scores emerged. The amplitude of N200 was significantly larger during non-symbolic comparisons. In addition, P2P amplitude was consistently smaller in LA children while comparing both symbolic and non-symbolic quantities, and correlated positively with the WRAT-4 scores. The latency of P3 seemed to be sensitive to the type of numerical comparison. The results suggest that math difficulties might be related to a more general magnitude representation problem, and that ERP are useful to study its timecourse in children with different mathematical skills., (Copyright © 2015 Elsevier B.V. Background: Postlicensure surveillance studies suggest a small temporal increase in the risk for intussusception with both currently available rotavirus vaccines (RV1; Rotarix, GSK and RV5; RotaTeq, Merck & Co., Inc.). This meta-analysis was undertaken to provide a single overall estimate of the relative risk of intussusception during the 7-day period after administration of RV1 and RV5., Methods: Meta-analysis based on estimates of relative risk and corresponding 95% confidence intervals from 5 postlicensure studies providing an estimate of risk of intussusception during the 7-day period after administration of dose 1 and/or dose 2 of RV1 and/or RV5, based on active and/or passive surveillance, for confirmed intussusception cases (Brighton or other method of case confirmation). For each vaccine, the relative risk of intussusception was estimated postdose 1 and postdose 2. Results were pooled using the inverse variance method using both fixed-effect and random-effect models., Results: The overall estimate of relative risk of intussusception during the 7 days postdose 1 was 5.4 (95% confidence interval: 3.9-7.4, 3 studies) for RV1 and 5.5 (3.3-9.3, 3 studies) for RV5. The overall estimate of relative risk of intussusception during the 7 days postdose 2 was 1.8 (1.3-2.5, 4 studies) for RV1 and 1.7 (1.1-2.6, 3 studies) for RV5., Conclusions: This meta-analysis showed a similar increased risk of intussusception, during the first 7 days after administration of dose 1 and, to a lesser extent, dose 2, for both currently available rotavirus vaccines. This suggests that intussusception may be a class effect of currently available oral rotavirus vaccines.

Processing and storage in visuospatial working memory (VSWM) seem to depend on attention-based mechanisms. In order to explore the effect of attention-attractive stimuli, such as emotional faces on VSWM performance, ERPs were obtained from 20 young adults while reproducing spatial sequences of six facial (happy and neutral) and non-facial control stimuli in inverse order. Behavioral performances revealed that trials with happy facial expressions resulted in a significantly higher amount of correct responses. For positive emotional facial stimuli, N170 amplitude was higher over right temporo-parietal regions, while P2 amplitude was higher over frontal and lower over parietal regions. In addition, LPP amplitude was also significantly higher for this type of stimuli. Both behavioral and electrophysiological results support the notion of the domain-general attention-based mechanism of VSWM maintenance, in which spatial to-be-remembered locations might be influenced by the emotional content of the stimuli., (Copyright © 2014 Elsevier Ireland Ltd. Reading speed and efficiency are achieved through the automatic recognition of written words. Difficulties in learning and recognizing the orthography of words can arise despite reiterative exposure to texts. This study aimed to investigate, in native Spanish-speaking late adolescents, how different levels of orthographic knowledge might result in behavioral and event-related brain potential differences during the recognition of orthographic errors. Forty-five healthy high school students were selected and divided into 3 equal groups (High, Medium, Low) according to their performance on a 5-test battery of orthographic knowledge. All participants performed an orthographic recognition task consisting of the sequential presentation of a picture (object, fruit, or animal) followed by a correctly, or incorrectly, written word (orthographic mismatch) that named the picture just shown. Electroencephalogram (EEG) recording took place simultaneously. Behavioral results showed that the Low group had a significantly lower number of correct responses and increased reaction times while processing orthographical errors. Tests showed significant positive correlations between higher performance on the experimental task and faster and more accurate reading. The P150 and P450 components showed higher voltages in the High group when processing orthographic errors, whereas N170 seemed less lateralized to the left hemisphere in the lower orthographic performers. Also, trials with orthographic errors elicited a frontal P450 component that was only evident in the High group. The present results show that higher levels of orthographic knowledge correlate with high reading performance, likely because of faster and more accurate perceptual processing, better visual orthographic representations, and top-down supervision, as the event-related brain potential findings seem to suggest.

Background: Intussusception (IS) is a form of acute intestinal obstruction that occurs mainly in infants and is usually of unknown cause. An association between IS and the first licensed rotavirus vaccine, a reassortant-tetravalent, rhesus-based rotavirus vaccine (RRV-TV), led to the withdrawal of the vaccine. New rotavirus vaccines have now been developed and extensively studied for their potential association with IS. This study aimed to describe the epidemiology and to estimate the incidence of IS in Latin American infants prior to new vaccine introduction., Methods: Children under 2 years of age representing potential IS cases were enrolled in 16 centers in 11 Latin American countries from January 2003 to May 2005. IS cases were classified as definite, probable, possible or suspected as stated on the Brighton Collaboration Working Group guidelines., Results: From 517 potential cases identified, 476 (92%) cases were classified as definite, 21 probable, 10 possible and 10 suspected for intussusception. Among the 476 definite IS cases, the median age at presentation was 6.4 months with 89% of cases aged <1 year. The male to female ratio was 1.5:1. The incidence of definite IS per 100,000 subject-years ranged from 1.9 in Brazil to 62.4 in Argentina for children <2 years of age, and from 3.8 in Brazil to 105.3 in Argentina for children aged <1 year. Median hospital stay was 4 days with a high prevalence of surgery as the primary treatment (65%). Most cases (88%) made a complete recovery, but 13 (3%) died. No clear seasonal pattern of IS cases emerged., Conclusions: This study describes the epidemiology and estimates the incidence of IS in Latin American infants prior to the introduction of new rotavirus vaccines. The incidence of IS was found to vary between different countries, as observed in previous studies., Trial Registration: Clinical study identifier 999910/204 (SERO-EPI-IS-204).

Background: Despite vaccine availability, the infection rate and disease burden associated with rotavirus infection are still high. Thus, accurate diagnosis of rotavirus infection continues to be necessary for proper patient clinical management and disease control., Objective: To evaluate the performance of a novel, second generation, commercial latex tests (Pastorex™ Rotavirus latex agglutination test, BIORAD, Marnes-La-Coquette, France), for the detection of rotavirus in human feces., Study Design: Using 166 fecal samples collected from children with acute diarrhea, and previously tested for rotavirus, calicivirus and astrovirus, the second generation Pastorex™ Rotavirus latex agglutination test was evaluated., Results and Conclusion: The test showed a sensitivity of 85.9% and a specificity of 97.7%. Positive and negative predicted values for the test were 97% and 88%, respectively. Background: Published economic assessments of rotavirus vaccination typically use modelling, mainly static Markov cohort models with birth cohorts followed up to the age of 5 years. Rotavirus vaccination has now been available for several years in some countries, and data have been collected to evaluate the real-world impact of vaccination on rotavirus hospitalisations. This study compared the economic impact of vaccination between model estimates and observed data on disease-specific hospitalisation reductions in a country for which both modelled and observed datasets exist (Belgium)., Methods: A previously published Markov cohort model estimated the impact of rotavirus vaccination on the number of rotavirus hospitalisations in children aged <5 years in Belgium using vaccine efficacy data from clinical development trials. Data on the number of rotavirus-positive gastroenteritis hospitalisations in children aged <5 years between 1 June 2004 and 31 May 2006 (pre-vaccination study period) or 1 June 2007 to 31 May 2010 (post-vaccination study period) were analysed from nine hospitals in Belgium and compared with the modelled estimates., Results: The model predicted a smaller decrease in hospitalisations over time, mainly explained by two factors. First, the observed data indicated indirect vaccine protection in children too old or too young for vaccination. This herd effect is difficult to capture in static Markov cohort models and therefore was not included in the model. Second, the model included a 'waning' effect, i.e. reduced vaccine effectiveness over time. The observed data suggested this waning effect did not occur during that period, and so the model systematically underestimated vaccine effectiveness during the first 4 years after vaccine implementation., Conclusions: Model predictions underestimated the direct medical economic value of rotavirus vaccination during the first 4 years of vaccination by approximately 10% when assessing hospitalisation rates as compared with observed data in Belgium.

Background: Mexico initiated mass vaccination with the attenuated human rotavirus vaccine (Rotarix) in 2006. This postlicensure study aimed to assess any potential temporal association between vaccination and intussusception in Mexican infants., Methods: Prospective, active surveillance for intussusception among infants aged less than 1 year was conducted in 221 hospitals across Mexico from the Mexican Institute of Social Security between January 2008 and October 2010. The temporal association between vaccination and intussusception was assessed by self-controlled case-series analysis., Results: Of the 753 episodes of intussusception reported in 750 infants, 701 were in vaccinated infants (34.5% post-dose 1, 65.5% post-dose 2). The relative incidence of intussusception within 31 days of vaccination was 1.75 (95.5% confidence interval [CI]: 1.24-2.48; P=0.001) post-dose 1 and 1.06 (95.5% CI: 0.75-1.48; P=0.75) post-dose 2. The relative incidence of intussusception within 7 days of vaccination was 6.49 post-dose 1 (95.5% CI: 4.17-10.09; P<0.001) and 1.29 post-dose 2 (95.5% CI: 0.80-2.11; P=0.29). Clustering of intussusception within 7 days of vaccination was observed post-dose 1. An attributable risk of 3 to 4 additional cases of intussusception per 100,000 vaccinated infants was estimated., Conclusion: This is the largest surveillance study for intussusception after rotavirus vaccination to date. A temporal increase in the risk for intussusception was seen within 7 days of administration of the first vaccine dose. It is still uncertain whether rotavirus vaccination has any impact on the overall incidence of intussusception. This finding has to be put in perspective with the well-documented substantial benefits of rotavirus vaccination.

Objectives: To estimate the health impact and the costs of treatment associated with rotavirus diarrhea in six yearly cohorts (2001-2006) of Mexican infants., Methods: The perspective of study is from the health care system. We estimated the effect of rotavirus diarrhea on disability adjusted life years (DALYS) and diarrhea treatment costs in hypothetical cohorts of infants who are followed from birth up to five years of age beginning in years from 2001 to 2006. We used information from administrative databases on mortality and health care from the National System of Information on Health and from the Mexican Institute for Social Security to feed a decision analysis to project the burden of disease and costs of treatment., Results: Estimates of DALYS were 19,426 in 2001 and decreased by 28.9% for 2006 meanwhile costs of treatment were relatively constant, estimated at US$ 38.7 million and increased only by 5%., Conclusion: Rotavirus diarrhea in Mexican children is a major disease burden, presenting significant treatment costs. Rotavirus diarrhea mortality is decreasing; however this has not led to a steady decrease in treatment costs in the 6 years period of analysis. A sensitivity analysis showed that incidences of rotavirus diarrhea as well as the parameters associated with health-care access were the main factors, which had a significant effect on the projected burden of disease and costs., (Copyright © 2011 Elsevier Ltd. Rapid Automatized Naming (RAN) deficits have been associated with less developed orthographic abilities that may affect lexical decisions. The effects of Spanish-speaking children's RAN performance on lexical decisions were evaluated by analyzing ERP and behavioral measures. Based upon their naming speed in four RAN tasks, 28 normal IQ, right-handed, 7-year-old children were selected and divided uniformly into two groups: average-naming (AN), and slow-naming (SN). ERPs were obtained during a lexical decision task consisting of 100 strings of four sequentially-presented letters that completed words (50 trials) or pseudowords (also 50 trials). The SN group showed major reading difficulties when compared to the AN group, as well as a significantly lower number of correct responses and slower reaction times in the lexical task. Two main ERP components were observed: parietal N320, interpreted as analogous to N170/N200; and a subsequent P3-like component (P500) with a higher amplitude for pseudowords, which probably reflects higher cognitive demands. Better reading comprehension and fewer misread pseudowords correlated with minor N320 latencies, while lower N320 amplitudes for words correlated with faster reading speeds, lower naming times and fewer errors while reading a text. The present results suggest that naming speed and ERP seem to be valuable in distinguishing early orthographic stored code retrieval abilities through a lexical decision task. Moreover, RAN and ERP emerge as accurate tools for evaluating reading processes in the early stages of reading acquisition.

Few studies exist regarding the frequency of human caliciviruses as single etiologic agents in sporadic cases, or in outbreaks occurring in children hospitalized for acute gastroenteritis. In this study, a total of 1,129 children of <5 years of age and hospitalized due to acute diarrhea were enrolled from three main hospitals in Mexico City during a period of 3 years (March 1998 to December 2000). After analyzing all fecal samples for several enteropathogens, 396 stools that remained negative were further screened for human caliciviruses by RT-PCR using a primer set specific to norovirus and sapovirus. Human caliciviruses were detected in 5.6% (22/396) of the children. The minimum incidence rate for 1999 were 5.3% (7/132) for 1999 and 7.8% (13/167) for 2000, since only fecal specimens that tested negative to other enteric pathogens were examined. Positive samples were further characterized using specific GI and GII primers and sequencing. Norovirus GII was detected in 19/22 samples, most of them were GII/4, while sapovirus GI/2 was detected in one sample. Associations between the presence of human calicivirus and clinical and epidemiological data revealed that diarrhea occurred with a seasonal pattern, and that children hospitalized due to human calicivirus disease scored an average of 13 +/- 3.2 (SD) points on the Vesikari scale, which corresponded to severe episodes. These results highlight that human caliciviruses, by themselves, are enteropathogens of acute severe diarrhea among young Mexican children requiring hospitalization and that their detection is important in order to reduce the diagnosis gap., (2010 Wiley-Liss, Inc.)

Introduction: Attention deficit/hyperactivity disorder (ADHD) is a common behavioral disorder found mainly in males, thus current knowledge on its clinical expression in female adults is extremely limited. AIM. To evaluate the behavioral and neural substrates associated with the performance of a short-term memory task in female ADHD adults, with and without methylphenidate exposure, with respect to a control group., Subjects and Methods: Two groups of eight young right-handed, female, university students with ADHD and healthy controls matched by age, gender, handedness and academic level, voluntarily participated. All subjects performed twice an easy auditory short-term memory task (ADHD group without, and 90 minutes post-intake of methylphenidate 0.4 mg/kg in a counterbalanced order). The BOLD-fMRI response was used as a measure of neural activity during task performance., Results: ADHD subjects showed a tendency to improve their performances under medication, showing an increased widespread functional activation, especially relevant over left frontal and cerebellar areas, in comparison with control subjects., Conclusions: Methylphenidate slightly improves short-term memory task performance in adult female ADHD subjects by modifying underlying neural functioning patterns.

Rotavirus is a ubiquitous infection that is the leading cause of severe diarrhea worldwide. Severe infections are most commonly observed in the first 2 years of life. Rotavirus-induced diarrhea is associated with substantial morbidity and mortality rates and socioeconomic costs with adverse outcomes particularly prevalent in developing countries. The natural history of rotavirus infection can provide guidance for the development and optimization of an effective vaccine. Epidemiologic studies have demonstrated that children who acquire natural rotavirus infections develop immunity to subsequent infections, with the protective effect increasing with each natural infection. Natural infections also decrease the severity of any subsequent rotavirus infections. Notably, asymptomatic infections provide protection similar to that induced by symptomatic infections. Data also suggest that the antibody response to natural infection is heterotypic, and therefore may provide protection against multiple serotypes. These data suggest that the development of a vaccine that produces asymptomatic infection at an optimal time point may provide effective immunity. An effective vaccine should mimic protection provided by natural infection and provide protection against the most common rotavirus serotypes (ie, G1, G2, G3, G4, G9) and be able to decrease disease severity, reduce hospitalizations, and decrease disease-related costs.

Facial emotional processing can be bypassed when faces are task-irrelevant and attention is diverted, although this effect has not been examined when cognitive task occurs within a facial background. Event-related potential (ERP) measures were obtained to evaluate the influence of different irrelevant facial emotional contexts on a simultaneous "ear-size" detection task performance in five processing contexts: (1) neutral face, (2) happy face, (3) fearful face, (4) facial contour, and (5) non-facial context. Reaction times were longer when visual processing occurred in a facial context, regardless of its emotional content. The context of neutral faces also demonstrated a lower number of correct responses, with fewer incorrect responses found during the presentation of fearful faces compared to the neutral facial context. ERP morphology was similar across all conditions, but ERP amplitude from components for the non-facial context was larger than that of the alternative conditions from 100 to 300 ms, with a similar N170-like potential also observed. The findings suggest that simultaneous irrelevant emotional facial stimuli may affect cognitive processing by altering two temporarily overlapped neural mechanisms: one responsible for earlier face detection, and the other involved in emotional recognition. The first might delay simultaneous cognitive actions by diverting attention, whereas while the latter may enhance the availability of processing resources through the participation of a subcortical pathway.

It has been reported that limitations in different components of working memory could underlie reading disabilities. In addition, reading-disabled (RD) children seem to perform worse when digit name processing is required. With the purpose to explore further these assumptions one inverse serial digit detection task was evaluated using event-related brain potentials in fifteen 8-year-old RD children and a control group (CG). CG obtained significantly more correct responses than RD, but had similar reaction times. The experimental task performance significantly correlated with the performance on reading tests. Difference event-related potentials showed a voltage component peaking at 160 ms over frontocentral leads (P160d) that reached significantly higher amplitude in RD group, and was interpreted as an index of the amount of neural resources involved in visual working memory load. The amplitude of P160d significantly correlated with reading speed, the backward digit span and with the experimental task performance. Present results point out that highly demanding working memory tasks reveal behavioral and electrophysiological differences in RD children with respect to healthy controls.

Sixteen ADHD children and a control group were asked to reproduce the varying time duration of successively presented visual stimuli. Time estimation was poorer in ADHD children, who showed more impulsive errors. ERPs exhibited similar grand-mean waveforms for both groups during the estimating period, but they were significantly different during the reproducing stage, when an early positive wave over frontal regions characterized the control group, interpreted as memory-guided motor output, followed by a slow negativity probably reflecting an inhibitory motor closure process, both probably involving central executive networks that seem to be improperly activated in ADHD children.

Background: [corrected] The Attention Deficit Disorder with Hyperactivity (ADDH) Barkley model predicts concomitant deficits in working memory and in the development of a sense of time as a consequence of poor behavioral inhibition., Objective: To evaluate electrophysiologically Barkley's theory (1997)., Subjects and Methods: We studied the electroencephalogram (EEG) of fifteen right-handed, normal limit IQ ADDH children (inattentive subtype), aged 7-11 years, and a control group. Subjects were studied at rest, with the eyes open (RO) while performing a time estimation task (ET). Forty-two data matrices were analyzed in three conditions: RO, ET and ET-RO, to assess broad-band spectral parameters; the anteriority, temporal-referenced and laterality quotients (Aq, Aqt and Lq, respectively); and the theta/alpha and theta/beta ratios., Results: Analyses of variance revealed significant differences for ES-RA, in absolute (Aq &Aqt) and relative powers (Aq). Discriminant function analysis was able to predict group membership with an accuracy over 80%, but it decreased slightly when the leave-one-out method was applied Linear discrimination based on right hemisphere Aqt values showed the highest discrimination accuracy., Conclusions: These results support Barkley's theory, suggesting further evaluation of the temporal-referenced anteriority quotient as a promising electrophysiological tool for the diagnosis and follow up of ADDH children.

Aims: To determine whether natural rotavirus infection or infection by another enteropathogen is associated to intussusception (IS); and to describe the seasonality of IS compared with severe diarrhea (SD) and rotavirus SD in Mexican children., Methods: A prospective, observational, multicenter and case-control study was conducted in Mexico City from December 1999 to February 2001. Cases were children younger than 1 year old hospitalized for IS; diagnosis was made by clinical features, radiologic and/or surgery findings. Controls were children younger than 1 year old hospitalized for another disease than a gastrointestinal illness (NGI). Cases and controls were paired by age and date of admission (+/-3 months; for both), in a 1:2 ratio. A surveillance of IS cases, SD and rotavirus SD episodes was conducted during the study period. Stool samples collected soon after IS resolution or at admission were tested for rotavirus, adenovirus, astrovirus, bacteria and parasites., Results: Thirty cases of IS and 60 controls with NGI were studied. Rotavirus was not detected in any case of IS. Adenovirus (17%) was the only enteropathogen detected in IS. Rotavirus (8%), adenovirus (2%), astrovirus (2%) and bacteria (2%) were detected in NGI. Rotavirus infection was not associated with IS (odds ratio, 0; 95% confidence interval, 0-2.9), whereas adenovirus infection was strongly associated as risk factor for IS (odds ratio undefined; P = 0.003), compared with NGI. Seasonal variation in admissions for IS was small, whereas admissions for SD and rotavirus SD showed a marked seasonal increase during fall-winter., Conclusions: In Mexican children, rotavirus infection was not associated to IS; whereas a significant association was observed between adenovirus and IS. Also there was no increase in IS cases during the sharply defined fall-winter rotavirus outbreak. Observations from this controlled study suggest that natural rotavirus infection is not a risk factor for IS. This information may have implications for development of a safer and effective rotavirus vaccine.

Aim: To analyze changes in prevalence and seasonality of diarrhea morbidity and mortality and to evaluate the impact of rotavirus disease among Mexican children younger than 5 years old., Methods: Diarrhea surveillance was performed from 1990 to 2002. Rotavirus testing was performed on stool specimens from 1996 to 2002. Data were obtained from different surveillance systems considering a nationwide representation in Mexico. Diarrhea morbidity and mortality rates were analyzed against time to determine trends or seasonal patterns., Results: Improvement of surveillance for all diarrhea episodes denoted an initial morbidity increase from 1995 to 1999, followed by a decrease by 2002, without any seasonal pattern. However, from 1990 to 1995, morbidity for severe diarrhea decreased 63%. From 1996 to 2002, 62-68% of severe diarrhea episodes occurring during the fall-winter season (FWS) were rotavirus-positive compared with 6-12% in the spring-summer season (SSS). From 1990 to 2002, diarrhea mortality decreased 84%. Higher mortality rates for children younger than 1 year old coincided precisely during the FWS, annually. Both severe diarrhea episodes and diarrhea deaths denoted a changing seasonal pattern. In 1990-1991, 2 waves of increased diarrhea activity occurred. The increase in SSS was much more pronounced than that in FWS. From 1992 to 1995 for severe diarrhea and from 1993 to 2002 for diarrhea deaths, the SSS frequencies subsequently reduced, whereas the FWS peaks remained annually., Conclusions: A significant reduction in morbidity and mortality of severe diarrhea has occurred from 1990 and 2002 in Mexican children younger than 5 years old. This is a consequence of preventive programs initiated for cholera control since 1991, which had greater impact on SSS diarrhea and limited response for FWS diarrhea, when rotavirus is mainly present. Currently rotavirus diarrhea requires new prevention strategies and specific control measures, such as a specific national vaccine program.

Background: The existence of outstanding cognitive talent in mentally retarded subjects persists as a challenge to present knowledge. We report the case of a 16-year-old male patient with exceptional mental calculation abilities and moderate mental retardation., Methods: The patient was clinically evaluated. Data from standard magnetic resonance imaging (MRI) and two 99mTc-ethyl cysteine dimer (ECD)-single photon emission computer tomography (SPECT) (in resting condition and performing a mental calculation task) studies were analyzed., Results: Main neurologic findings were brachycephalia, right-side neurologic soft signs, obsessive personality profile, low color-word interference effect in Stroop test, and diffuse increased cerebral blood flow during calculation task in 99mTc-ECD SPECT. MRI showed anatomical temporal plane inverse asymmetry., Conclusions: Evidence appears to support the hypothesis that savant skill is related to excessive and erroneous use of cognitive processing resources instigated by probable failure in central executive control mechanisms.

Three inverse serial digit detection tasks were evaluated with event-related brain potentials (ERPs) in 15 11-year-old children to determine how the increase of perceptual or memory demands could modify detection processing. Reaction times were significantly longer for the task that used visual blurring, compared to that with a greater memory demand. Difference-ERPs (target minus non-target conditions) showed three significant parietal components; one earlier positive peak at 162 ms interpreted as an index of working memory load; a same polarity 295 ms peak which probably represents a P3 analogous and a subsequent negative polarity component (520 ms) possibly involved with motor preparation. A fourth difference-component was a frontal positive peak at 680 ms, interpreted as related to task difficulty.

Background: Respiratory syncytial virus (RSV) is the major viral cause of severe respiratory infections in children younger than 2 years of age. Nevertheless there are not enough epidemiologic data about the role of RSV as a cause of infantile mortality from pneumonia, mainly in young children from developing countries Aim. To determine the frequency of RSV infection in lung tissue samples from Mexican children deceased with pneumonia, by reverse transcription (RT) and PCR., Methods: Postmortem lung tissue samples from 98 children younger than 2 years of age who died of pneumonia during the period of 1989 to 1997 were studied. Paraffin was removed with xylene from 10-microm lung sections, the total RNA was extracted and complementary DNA was obtained by RT reaction. A nested PCR with the use of oligonucleotides specific for the F glycoprotein gene was developed. Samples negatives for RSV were tested for the absence of polymerase inhibitors and for complementary DNA integrity., Results: Twenty-nine of the 98 (30%) children deceased with pneumonia were positive for RSV by RT-PCR; 8 were detected from 13 (62%) children with histopathologic diagnosis of viral pneumonia and 21 from 85 (25%) children with histopathologic diagnosis of bacterial pneumonia (P = 0.018). There was no significant difference in RSV infection according to age groups or seasonal pattern., Conclusions: RSV infection is frequent in Mexican children younger than 2 years of age who died of pneumonia. Although RSV was more common in viral pneumonia, mixed infections with RSV and bacterial pneumonia were also common.

Background: Continuous Performance Test (CPT) is a commonly used paradigm to assess attention disorders that could involve working memory processes., Methods: Event-related potentials (ERPs) during a CPT (X-AX) test were obtained in 16 healthy male students, with ages ranging from 9 to 11 years (X = 10.3). In grouped averaged ERPs, an endogenous slow positive potential was recorded in the first task (infrequent letter detection); maximum was at 460 msec with a slight lateralization tendency toward left parietal area. In the second task (target detection with an A as warning signal), an early (maximum at 330 msec) and more acute peak was detected without evidence of any lateralization., Results: Reaction times were significantly shorter for the second task. Electrophysiologic differences between both target conditions showed an early, remarkable, and statistically significant component located at the parietal area at 340 msec., Conclusions: These ERPs findings could be interpreted as correlates with working memory processes in children.

To determine whether naturally acquired serum IgA and IgG antibodies were associated with protection against rotavirus infection and illness, a cohort of 200 Mexican infants was monitored weekly for rotavirus excretion and diarrhea from birth to age 2 years. Serum samples collected during the first week after birth and every 4 months were tested for anti-rotavirus IgA and IgG. Children with an IgA titer >1:800 had a lower risk of rotavirus infection (adjusted relative risk [aRR], 0.21; P<.001) and diarrhea (aRR, 0. 16; P=.01) and were protected completely against moderate-to-severe diarrhea. However, children with an IgG titer >1:6400 were protected against rotavirus infection (aRR, 0.51; P<.001) but not against rotavirus diarrhea. Protective antibody titers were achieved after 2 consecutive symptomatic or asymptomatic rotavirus infections. These findings indicate that serum anti-rotavirus antibody, especially IgA, was a marker of protection against rotavirus infection and moderate-to-severe diarrhea.

EEGs and behavioral responses were studied in two sex matched groups of 58 epileptic and 20 healthy children between 8 and 12 years of age, during the execution of a go-no go CPT (X; A-X) task to determine transitory cognitive impairment (TCI) incidence. Paroxysmal discharges were found on 87.9% and 5% of the EEGs in the epileptic and control groups respectively, with no differences related to sex. The predominant EEG findings with respect to paroxysmal discharges were the association of two or more types of paroxysms with frequency higher than 5/minute, an average duration less than 0.5 second and topographical distribution over temporal-parietal-occipital areas without significant interhemispheric differences. TCI was detected in 36.2% of epileptic children. The epileptic group showed significantly higher numbers of behavioral errors and longer reaction times (RTs) in relation to the control group. Analyzing RTs on the two blocks of the task, linear discriminant analysis showed an acceptable classification of TCI incidence between groups.

Aim: To describe the epidemiologic and clinical characteristics of astrovirus-associated diarrhea in a cohort of young children from a periurban community in Mexico City., Methods: From November, 1988, through December, 1991, a total of 214 children were enrolled in a longitudinal study of diarrhea and monitored from birth to 18 months of age. A stool specimen was collected during each episode of diarrhea. Specimens from a total of 510 diarrhea episodes were tested for astrovirus by enzyme immunoassay and examined for other enteric pathogens. The antigenic types of astrovirus were determined by a typing enzyme immunoassay., Results: Astrovirus was detected in 26 (5%) of 510 diarrhea episodes, with an incidence rate of 0.1 episode/child year; the highest rate was in children 13 to 18 months of age. Astrovirus-associated diarrhea was characterized by a median of 4 stools (range, 2 to 10) during the first 24 h, a median duration of 3 days (range, 1 to 21), vomiting (20%), and fever (7%). No cases of dehydration or repeat symptomatic infections were observed. Coinfection with another pathogen was detected in 11 of the 26 episodes (42%). Serotype 2 (35%) was most common, followed by serotypes 4 (15%), 3 (11%), and 1 and 5 (4% each); 31% were nontypable. Astrovirus-associated diarrhea was less severe, as measured by the number of stools (4.3 +/- 1.9), than diarrhea caused by rotavirus (7.1 +/- 2.8) or when coinfections occurred (5.5 +/- 1.6; P = 0.008)., Conclusions: Astrovirus was associated with 5% of the episodes of diarrhea in this cohort of young Mexican children and presented as a mild secretory diarrhea. Five predominant antigenic types were detected with type 2 being the most common.

Background: Rotavirus is the leading cause of severe diarrhea in infants. To provide a base line for assessing the efficacy of rotavirus vaccines, we evaluated the protection that is conferred by natural rotavirus infection., Methods: We monitored 200 Mexican infants from birth to two years of age by weekly home visits and stool collections. A physician assessed the severity of any episodes of diarrhea and collected additional stool specimens for testing by enzyme immunoassay and typing of strains. Serum collected during the first week of life and every four months thereafter was tested for antirotavirus IgA and IgG., Results: A total of 316 rotavirus infections were detected on the basis of the fecal excretion of virus (56 percent) or a serologic response (77 percent), of which 52 percent were first and 48 percent repeated infections. Children with one, two, or three previous infections had progressively lower risks of both subsequent rotavirus infection (adjusted relative risk, 0.62, 0.40, and 0.34, respectively) and diarrhea (adjusted relative risk, 0.23, 0.17, and 0.08) than children who had no previous infections. No child had moderate-to-severe diarrhea after two infections, whether symptomatic or asymptomatic. Subsequent infections were significantly less severe than first infections (P=0.024), and second infections were more likely to be caused by another G type (P=0.054)., Conclusion: In infants, natural rotavirus infection confers protection against subsequent infection. This protection increases with each new infection and reduces the severity of the diarrhea.
