The formation of dentinal microscopic on the external or internal surface of the tooth can become new bacterial colonization and MD, which can lead to successful endodontic treatment. In addition, these possible dental teeth, when present, can evolve into a vertical root fracture (VRF), which compromises the tooth's permanence in the oral environment (DE-DEUS et al., 2016). However, to achieve an effective cleaning of the SCR, additional care is necessary, especially if necrotic and bacterial debris are present in their irregularities, as well as if there is the presence of residual biofilm after instrumentation. These factors were of paramount importance in the recommendation of intensional foraminal enlargement (AFI), which, in turn, consists of going beyond the modeling instrument beyond the apical foramen during endodontic treatment (SUSILA; MINU, 2019). Over-instrumentation is commonly performed from 1 to 3 mm beyond the foramen and has become widely applied in cases of teeth with periapical lesions, since in these cases the mechanical chemical preparation must be intensified due to the high bacterial load (DE GOD et al., 2016). Although some endodontists may have concerns regarding postoperative pain within this approach, previous studies have shown that foraminal enlargement is not responsible for significantly increasing postoperative pain rates (CRUZ et al., 2016; YAYLALI et al., 2017). In recent years, nickel-titanium (NiTi) instruments have become indispensable items for endodontic treatment. These instruments have presented better performance and safety elements to make root canal preparation more efficient and, thus, obtain higher success rates in endodontic treatment (CAPAR et al., 2016). However, it is still unclear in the literature the real effect of instrumentation beyond the apex on the morphology of the foramen, as well as on the formation of dentin microcracks, since even the microtomography methodologies present divergent results regarding the evaluation of microcrack formation. dentinal changes and foraminal alterations (DEDeus et al., 2014, DEDeus et al., 2015, DEDeus et al., 2016, CEYHANLI et al., 2016, JAMLEH et al., 2016). Considering these points and the fact that the number of studies that address this topic at the moment is undoubtedly limited, this study aims at a scoping review on the morphological changes of the apical foramen and formation of dentinal microcracks after intentional foraminal enlargement with instruments of heat-treated mechanized NiTi and in their different kinematics.

This clinical case reports bone repair after paraendodontic surgery with the use of mineral trioxide aggregate (MTA) and guided tissue regeneration (GTR). The patient sought the Orthodontics Clinic at Faculdade COESP, showing disocclusion on element 11, grade II mobility. On radiographic examination, an apical radiolucency was observed, from the root of tooth 13 to 11, needing to be referred for endodontics. With a doubtful prognosis, it followed with endodontic retreatment and after 3 months the surgical intervention was performed. Then, the graft was accommodated in the bone store, followed by adaptation of the membrane and suture. After 6 months, there was no sensitivity to vertical percussion and palpation. After 3 years, new bone formation was observed in the periapical region, thus, it was concluded that the chosen method of disinfecting the environment and using a bone graft in addition to guided tissue regeneration promoted a satisfactory bone regeneration with the consequent maintenance of the dental element in the oral cavity. Este caso clínico reporta la reparación ósea después de una cirugía paraendodóntica con el uso de agregado de trióxido mineral (MTA) y regeneración tisular guiada (RTG). El paciente acudió a la Clínica de Ortodoncia de la Faculdade COESP, evidenciando la entrada del elemento 11, movilidad grado II. En el examen radiográfico se observó una radiolucidez apical, desde la raíz del diente 13 al 11, requiriendo ser referido para endodoncia. Con pronóstico dudoso, siguió con retratamiento endodóntico y a los 3 meses se realizó la intervención quirúrgica. Poco después, el injerto se acomodó en el depósito óseo, seguido de la adaptación de la membrana y la sutura. Después de 6 meses, no hubo sensibilidad a la percusión vertical y palpación en el fondo del vestíbulo. A los 3 años se observó neoformación ósea en la región periapical, por lo que se concluyó que el método elegido de desinfección del ambiente y el uso de injerto óseo además de la regeneración tisular guiada promovió una regeneración ósea satisfactoria con el consiguiente mantenimiento del elemento dental en la cavidad bucal. Este caso clínico relata o reparo ósseo após cirurgia paraendodôntica com o uso do agregado trióxido mineral (MTA) e regeneração tecidual guiada (RTG). Paciente procurou a Clínica de Ortodontia da Faculdade COESP apresentando vestibularização do elemento 11, mobilidade grau II. Ao exame radiográfico, observou-se uma radiolucência apical, da raiz do dente 13 ao 11 necessitando ser encaminhada para endodontia. Com prognóstico duvidoso, seguiu-se com o retratamento endodôntico e após 3 meses a intervenção cirúrgica foi realizada. Logo após, o enxerto foi acomodado na loja óssea, seguido da adaptação da membrana e sutura. Após 6 meses, não havia sensibilidade à percussão vertical e à palpação em região de fundo de vestíbulo. Após 3 anos observou-se neoformação óssea na região periapical, dessa forma, conclui-se que a conduta escolhida de desinfecção do meio e utilização de enxerto ósseo somado à regeneração tecidual guiada promoveu uma regeneração óssea satisfatória com a consequente manutenção do elemento dentário na cavidade oral.

Studies on dental anatomy and internal morphology of the root canal system are important to optimize endodontic therapy. Knowledge of variations in the number of channels, number of roots, horizontal dimensions and channel width increase the understanding and mastery of the anatomy for the clinician. The present study aims to describe the morphometric analysis of lower incisors through computerized microtomography. A total of 47 (n = 47) permanent lower incisors had their internal anatomy evaluated through three-dimensional computerized microtomography with parameters of standardized acquisition, reconstruction and analysis. When evaluating the presence of two root canals at the level of the middle and apical thirds, a total of 3 elements (6.4%) were found. The most prevalent channel format was oval long channels (47.7%), followed by flat channels (34.1%). No irregular channels found. By the present study it was possible to conclude that the most prevalent canal format in permanent inferior incisors is the long oval, and no irregular channels were found. Los estudios sobre la anatomía dental y la morfología interna del sistema de conductos radiculares son importantes para optimizar la terapia endodóntica. El conocimiento de las variaciones en el número de canales, el número de raíces, las dimensiones horizontales y el ancho del canal aumenta la comprensión y el dominio de la anatomía para el médico. El presente estudio tiene como objetivo describir el análisis morfométrico de los incisivos inferiores mediante microtomografía computarizada. Se evaluó la anatomía interna de un total de 47 (n = 47) incisivos inferiores permanentes mediante microtomografía computarizada tridimensional con parámetros de adquisición, reconstrucción y análisis estandarizados. Al evaluar la presencia de dos conductos radiculares a nivel de los tercios medio y apical, se encontraron un total de 3 elementos (6,4%). El formato de canal más prevalente fueron los canales largos ovalados (47,7%), seguido de los canales planos (34,1%). No se encontraron canales irregulares. Mediante el presente estudio se pudo concluir que el formato de canal más prevalente en los incisivos inferiores permanentes es el óvalo largo y no se encontraron canales irregulares. Estudos sobre anatomia dental e morfologia interna do sistema de canais radiculares são importantes para otimizar a terapia endodôntica. O conhecimento das variações no número de canais, número de raízes, dimensões horizontais e largura de canais aumentam a compreensão e domínio da anatomia para o clínico. O presente estudo tem por objetivo descrever a análise morfométrica de incisivos inferiores através de microtomografia computadorizada. Um total de 47 (n=47) incisivos inferiores permanentes tiveram sua anatomia interna avaliada através de microtomografia computadorizada tridimensional com parâmetros de aquisição, reconstrução e análise padronizados. Quando avaliado a presença de dois canais radiculares ao nível do terço médio e apical, foram encontrado um total de 3 elementos (6,4%). O formato de canal mais prevalente foi de canais ovais longos (47,7%), seguido de canais achatados (34,1%). Não foram encontrados canais irregulares. Pelo presente estudo foi possível concluir que o formato de canal mais prevalente em incisivos inferiores permanentes é o oval longo, não sendo encontrado canais irregulares.

9. Análisis de micro-TC de diente fracturado en el segundo molar mandibular con anatomía C después de trauma oclusal: Reporte de caso

The aim of the present study was to analyze in a micro-CT tooth extracted by cracked. A 38-year-old white woman, complaining of occasional discomfort when biting popcorn kernels, sought emergency care. After intraoral and radiographic clinical examination, the right second mandibular molar (tooth 47) was diagnosed with live pulp and fracture of the lingual cusps, and after removal of the amalgam restoration, an incomplete fracture initiated from the crown was found in the distal proximal crest. The treatment plan started with prosthetic rehabilitation using a full crown. Three months later symptomatic irreversible pulpitis was diagnosed. The pulp cavity was accessed through the prosthetic crown and the tooth was treated endodontically. Two years later, the patient presented a fistula close to the gingival sulcus and a 12mm periodontal probe in the center of the distal surface. Cone-beam tomographic examination reveals extensive bone loss reaching the mandibular canal. The treatment plan was extraction of the dental elemento and subsequent implant planning. The extracted tooth was then subjected to scanning at the micro-CT. In the analysis of the micro-CT images of the extracted tooth, it was found cracked tooth, extending from the crown to the proximal root (distal face). There was also a bad adaptation of the gutta-percha cone in the apical third of the endodontic treatment performed. Through this report, we can infer that the diagnosis of cracked tooth is a challenge in clinical practice, and the corono-radicular spread of crack is associated with microbial infiltration and consequent risk of loss of the dental element. El objetivo del presente estudio fue analizar en micro-TC diente extraído por fracturado. Una mujer blanca de 38 años, que se quejaba de molestias ocasionales al morder granos de palomitas de maíz, buscó atención de emergencia. Después del examen clínico intraoral y radiográfico, el segundo molar mandibular derecho (diente 47) fue diagnosticado con pulpa viva y fractura de las cúspides linguales, y después de retirar la restauración de amalgama, se encontró una fractura incompleta iniciada desde la corona en la cresta proximal distal. El plan de tratamiento comenzó con la rehabilitación protésica con una corona completa. Tres meses después pulpitis irreversible sintomática fue diagnosticado. Se accedió a la cavidad pulpar a través de la corona protésica y el diente se trató endodónticamente. Dos años después, el paciente desarrolló una fístula próxima al surco gingival y una sonda periodontal de 12 mm en el centro de la superficie distal. El examen tomográfico de haz cónico revela una pérdida ósea extensa que llega al canal mandibular. El plan de tratamiento fue la extracción del elemento dental y posterior planificacíon de implantes. Luego, el diente extraído se sometió a un escaneo en el micro-CT. En el análisis de las imágenes de micro-TC del diente extraído, se encontró diente fracturado, extendiéndose desde la corona hasta la raíz proximal (cara distal). También hubo una mala adaptación del cono de gutapercha en el tercio apical del tratamiento endodóntico realizado. A través de este informe, podemos inferir que el diagnóstico de diente fracturado es un desafío en la práctica clínica, y la diseminación corono-radicular de la fractura está asociada con la infiltración microbiana y la consecuente riesgo de pérdida del elemento dentario. O objetivo do presente estudo foi analisar em micro-CT dente extraído por fissura. Uma mulher de 38 anos, branca, com queixa de desconforto ocasional após morder caroço de milho de pipoca, procurou a urgência. Após exame clínico intra-oral e radiográfico, o segundo molar mandibular direito (dente 47) foi diagnosticado com polpa viva e fratura das cúspides linguais, e após remoção de restauração de amálgama constatou-se fratura incompleta iniciada na crista proximal distal da coroa. O plano de tratamento iniciou-se com reabilitação protética através de coroa total. Três meses após pulpite irreversível sintomática foi diagnosticada. A cavidade pulpar foi acessada através da coroa protética e o dente foi tratado endodonticamente. Dois anos depois a paciente apresentou fístula próxima a sulco gengival e sondagem periodontal de 12mm no centro da superfície distal. Ao exame tomográfico de feixe cônico observou-se extensa perda óssea atingindo o canal mandibular. O plano de tratamento foi a exodontia do elemento dentário e posterior planejamento de implante. O dente extraído foi então submetido ao escaneamento no micro-CT. Na análise das imagens do micro-CT do dente extraído, constatou-se dente rachado, com trinca estendendo-se da coroa até a raiz proximal (face distal). Foi verificado também má adaptação do cone de guta-percha no terço apical do tratamento endodôntico realizado. Através deste relato, podemos inferir que, o diagnóstico de dente rachado é um desafio na prática clínica, e a propagação corono-radicular da rachadura está associada a infiltração microbiana e consequente risco de perda do elemento dental.

The aim of this study was to undertake a literature review on the use of XP-endo Shaper (FKG Dentaire SA, La Chaux-de-Fonds, Switzerland) for root canal preparation evaluated using microcomputed tomography (micro-CT) technology, with emphasis on the analysis of untouched canal walls. We searched the PubMed, Science Direct, SciELO, and MEDLINE databases for articles published from 2017 to 2019 using the following keywords: micro-CT, untouched walls, XP shaper, and XP endo. Articles without an abstract were excluded. Five papers were selected. Few studies on this topic are available. The studies published so far show that the XP-endo® Shaper system performs well in terms of canal preparation but leaves untouched walls.

Regenerative endodontic procedures associated with apical surgery may represent an alternative for the treatment of patients whose teeth have extensive or multiple apical lesions. Leukocyte platelet-rich fibrin (L-PRF) has advantages and may promote optimal bone healing. The present case report describes a regenerative endodontic therapy using PRF in three teeth bearing chronic apical periodontitis. The treatment performed was apicectomy, retropreparation and retrofilling with MTA Repair HP, and an approach using LPRF. The follow-up was performed after the periapical radiography, concomitant computed tomography and clinical examination that showed no symptoms, as well as radiographic evaluations that have been corrected. Finally, images of the computed tomography of the periapical lesions and reestablishment of the buccal cortical bone. The successful results in this clinical case indicate a therapy which can be used as a complement, providing an alternative treatment strategy for complex clinical cases. [ABSTRACT FROM AUTHOR]

20. Morphological changes of the apical foramen after enlargement: A scoping review of in vitro studies.

- Abstract
Background: To outline the current evidence on root morphological changes after enlarging the apical foramen with NiTi instruments., Material and Methods: A search was performed in the Virtual Health Library, PubMed, EMBASE, Scopus, Web of Science, Science Direct and SciELO databases, in addition to a manual search in Google Scholar, between January 2017 and October 2023. Articles published in English that describe in vitro studies investigating root morphological changes after instrumentation 1 mm beyond the major apical foramen were included. The quality of evidence in the included studies was also analyzed., Results: The search retrieved 367 articles. Of these, four studies were eligible for data synthesis and analysis, all of them in vitro studies. Synthesis of the results of these in vitro studies showed a larger number of root morphological changes such as experimental dentinal microcraks in samples submitted to instrumentation beyond the apical foramen when compared to micro-CT images obtained before preparation., Conclusions: The in vitro studies analyzed in this scoping review indicate that instrumentation beyond the major foramen of the root canal, promotes morphological changes in this area and that the adoption of standardized methodologies would not only increase the accurate detection and characterization of these changes but also facilitate the application of these findings in clinical trials and patient care. The aim of this study was to evaluate the root canal morphology of permanent maxillary molars by cone-beam computed tomography (CBCT) using the classifications of Weine et al. and Vertucci and to correlate the findings with sex, age, position in the dental arch, and prevalence of a second canal in the mesiobuccal root (MB2). A total of 414 scans were evaluated, corresponding to 1,000 teeth. The assessment consisted of coronal, axial, and sagittal reconstructions using i-CAT Workstation®. Type 0 was assigned when neither classification could be applied. The data were entered into an Excel spreadsheet and analyzed using SPSS. The chi-squared test or Fisher's exact test was used to compare the distribution of root canal morphology classified using the two systems. Analysis of the distribution of Weine types showed a predominance of type III in mesiobuccal roots, while type 0 predominated in distobuccal and palatal roots. Vertucci type IV predominated in mesiobuccal and distobuccal roots and type VII in palatal roots. There was no difference in the distribution of morphological canal types in permanent maxillary molars evaluated by CBCT according to sex, age group, or position in the dental arch of the patients. Successful endodontic treatment depends on cleaning and disinfecting the root canals, in order to provide conditions for three-dimensional filling, which should prevent root canal reinfection. However, anatomical complexities pose a challenge during endodontic treatment. The present study was a literature review carried out in the following databases: PubMed, SciELO, and MEDLINE, which were searched for articles published from 2017 to 2021. Micro-CT studies published in English, which analysed the capacity for preparation of oval and long-oval root canals, were included. The following keywords were used: "oval-shaped canals," "long-oval-shaped canals," "endodontics," and "micro-CT."
