Background The inguinal region is an area of complex anatomy that could contain diverse uncommon contents in routine clinical practice. Although inguinal hernia repair is one of the commonest surgeries done routinely, thorough preoperative imaging has a significant impact on the outcome of the surgery, by revealing the presence of unusual contents in the inguinal region. Aim The aim of this article is to review the differential diagnosis of the uncommon inguinal pathologies, which can simulate an inguinal hernia, to determine, and to simplify the treatment approach. Conclusions A profound understanding of the imaging characteristics of uncommon inguinal pathologies is crucial for both the radiologists (to prevent misdiagnosis) and the treating physicians (to avoid surgical complications) and ensure optimal management.

Abstract Background Various abdominal vessels can compress the adjacent structures or in turn can get compressed by them. Most of these compression syndromes present with non-specific symptoms. Unlike the common causes of acute abdomen, the various vascular compression syndromes have bizarre clinical presentations and subtle imaging findings, which can easily be missed by the physicians as well as the radiologists. Main body of the abstract This is a retrospective study which was done for a period of 3 years from April 2015 to April 2018 using a 64-slice CT scanner. Among 2412 cases that came for evaluation, 114 patients were diagnosed to have one of the various vascular compression syndromes. These 114 cases were further managed either conservatively or surgically depending on the pathology and the severity of the compression. The syndromes discussed in this article include median arcuate ligament syndrome (29 cases), superior mesenteric artery syndrome (23 cases), portal biliopathy (3 cases), nutcracker syndrome (6 cases), pelvi-ureteric junction obstruction due to crossing of vessels (8 cases), and retrocaval ureter and May-Thurner syndrome (45 cases). Conclusions The primary goal of this article is to reinforce the knowledge of the radiologists of the various vascular compression syndromes and to make them possess a high degree of vigilance to detect them. This article elaborates the imaging findings of these syndromes and the role of multidetector CT angiography in diagnosing them.

Fascioliasis is a re-emerging food-borne parasitic zoonosis which presents with nonspecific clinical symptoms. Definitive diagnosis requires demonstration of parasitic ova in stool which may often be elusive. Imaging plays a crucial role in raising the possibility of this diagnosis early in the disease course. Contrast-Enhanced Computed Tomography (CECT) of the abdomen was performed in a 44-year-old male with fever, right upper quadrant pain and dyspepsia. The patient later underwent an ultrasound-guided biopsy of liver lesions. The stool examination for ova was negative. Biopsy of the liver lesions showed ill-defined granulomas. Imaging plays a crucial role in establishing the early diagnosis of hepatic fascioliasis especially in nonendemic regions where the clinical suspicion for this diagnosis is low. This article places emphasis on the description of typical imaging features of this condition and the usage of additional investigative/testing techniques recommended by the Center for Disease Control and Prevention (CDC).

Melioidosis is a multi-system disorder which is often mistaken for tuberculosis in the Indian sub-continent. It is caused by the gram-negative bacterium Burkholderia pseudomallei. The imaging findings of melioidosis often mimic many other bacterial infections and the differentiation is sometimes difficult. The clinical manifestations of melioidosis range from asymptomatic and subclinical to acute localized forms, acute septicemia and chronic forms. One of the key strategies for decarbonization and green transportation is using electric vehicles (EVs). However, challenges like limited charging infrastructure, EV battery characteristics, and grid integration complexities persist. This study proposes a mixed-integer linear programming (MILP) approach to optimize a grid-connected solar PV-based commercial EV charging station (SPEVCS) with a battery energy storage system (BESS) for profit maximization. The MILP model efficiently manages SPEVCS operations, considering solar power fluctuations, EV charging patterns, and BESS usage. By coordinating charging schedules, grid stability is reinforced, and excess solar power can be lucratively managed. Comparing grid-connected and off-grid SPEVCS scenarios highlights grid integration benefits. Solar power mismatches with optimal charging periods pose a challenge, addressed here by BESS utilization and import/export of deficit/surplus power from/to the grid. The proposed framework incorporates solar power forecasts and probabilistic EV arrival predictions, enhancing decision accuracy. This approach fosters viable commercial EV charging, promotes green transportation, and reinforces grid resilience. [ABSTRACT FROM AUTHOR]
