Enquadramento: A cirurgia abdominal alta está associada a alta incidência de complicações pulmonares. A técnica breath stacking (BS) perspetiva-se como recurso com potencial terapêutico para a enfermagem de reabilitação. Objetivo: Avaliar a eficácia da técnica BS na melhoria da função respiratória. Metodologia: Estudo quasi-experimental com 36 mulheres submetidas a cirurgia bariátrica, distribuídas pelos grupos de controlo (GC) e intervenção (GI). Foram avaliadas no pré e pós-operatório as medidas capacidade vital forçada (CVF), volume expiratório forçado no primeiro segundo (VEF1), pressão inspiratória máxima (PIm), pressão expiratória máxima (PEm), saturação periférica de oxigénio (SpO2) e frequência respiratória (FR). Foi aplicada a técnica BS no GI no pré e pós-operatório. Recorreu-se à análise estatística descritiva e inferencial. Resultados: Observaram-se diferenças estatisticamente significativas pós-operatórias, entre o GC e o GI, na CVF ( -20,29 vs. -13,60), VEF1 (-23,05 vs. -13,38), PIm (-22,96 vs. -14,93), PEm (-14,10 vs -10,32) e FR (12,29 vs 6,45). Conclusão: A técnica de BS permitiu melhorar a função respiratória do GI e reverter as alterações previsíveis no pós-operatório de cirurgia bariátrica.

Objective: To assess the rib cage expansion and respiratory rate in newborns using an abdominal stabilization band. Methods: The study included 32 newborns of both genders, with gestational age between 35 and 41 weeks. The abdominal stabilization band was used for 15 minutes between the xiphoid process and the anterosuperior iliac crest, with an abdominal contention 0.5cm smaller than the abdominal circumference. The rib cage expansion was evaluated by a breathing transducer (Pneumotrace II™) three minutes before using the band, during the use (15 minutes), and ten minutes after removing the band. The Shapiro-Wilk test verified data normality, and the Wilcoxon test compared the variables considering rib cage expansion and respiratory rate. Significance was set to p<0.05. Results: There was an increase in respiratory rate when comparing before and ten minutes after removing (p=0.008) the abdominal stabilization band, as well as when comparing during its use and ten minutes after its removal (p=0.001). There was also an increase in rib cage expansion when comparing before and during the use of the abdominal stabilization band (p=0.005). Conclusions: The use of the abdominal stabilization band promoted an increase in the rib cage expansion and respiratory rate in the assessed newborns and may be a viable option to improve the respiratory kinematics of this population. Introduction: In nursing practice, this concept has been identified in studies that address the nursing diagnosis of impaired spontaneous ventilation. Nursing performance facing this unwanted human response is considered essential for the maintenance and control of vital signs, cardiovascular monitoring, gas exchange and respiratory pattern, as well as constant surveillance aimed at signs of hypoventilation and inadequate ventilation. Objective: To analyze the concept of impaired spontaneous ventilation in critically ill patients in an intensive care unit. Methodology: This study is a concept analysis, according to Walker’s and Avant’s method, conducted using an integrative review. All the referential steps were followed: concept selection, determining the objectives and proposals for concept analysis, identifying the possible uses of the concept, determining the defining attributes, identifying a model case, identifying a contrary case, and identifying the precedents and consequences of the concept. Results: A sample of 38 studies was selected. The following were identified as attributes: distress and fatigue, respiratory distress, tachycardia, hemodynamic instability, altered mental status, abnormal arterial blood gas results, dyspnea, anxiety, agitation, sweating, hypoxemia and hypercapnia. Antecedents: sex, age, oxygen saturation lower than 90%, diseases of the respiratory, cardiovascular, neurological, gastrointestinal, neuromuscular, and metabolic systems, respiratory infections, trauma, poisons, toxins, and sedatives. Consequences: increased heart rate, decreased partial oxygen saturation, increased use of respiratory muscles, dyspnea, increased metabolic rate and restlessness. The model case and the contrary case were used to illustrate the attributes, antecedents, and consequences. Conclusion: The study directed the strengthening of evidence about the phenomenon and demonstrated a strong occurrence in patients who need assistance in intensive care units, thus demanding critical care. Resumo Fundamento O tratamento com sacubitril-valsartana teve seu benefício prognóstico confirmado no ensaio PARADIGM-HF. No entanto, dados sobre alterações no teste de esforço cardiopulmonar (TECP) com o uso de sacubitril-valsartana são escassos. Objetivo O objetivo deste estudo foi comparar os parâmetros do TECP antes e depois do tratamento com sacubitril-valsartana. Métodos Avaliação prospectiva de pacientes com insuficiência cardíaca (IC) crônica e fração de ejeção do ventrículo esquerdo ≤40%, mesmo sob terapia padrão otimizada, que iniciaram tratamento com sacubitril-valsartana, sem expectativa de tratamentos adicionais para a IC. RESUMO O objetivo deste artigo é avaliar o índice de função sexual de mulheres submetidas à histerectomia total com ooforectomia bilateral (HT-OB), a prevalência de incontinência urinária por esforço (IUE) e sua associação com a realização desse procedimento cirúrgico em um hospital de referência em Belém (PA). Foram incluídas 162 mulheres, com vida sexual ativa, alocadas em dois grupos: aquelas que realizaram HT-OB em período superior a 12 meses (n=68), e aquelas que não realizaram (n=94). Utilizou-se o questionário female sexual function index (FSFI) para avaliação da função sexual, e um questionário desenvolvido pelos pesquisadores para coletar dados sociais, econômicos e clínicos, incluindo informações quanto à presença de IUE. RESUMO O objetivo deste estudo foi descrever características de sucesso e insucesso do uso da ventilação não invasiva (VNI) na unidade de terapia intensiva (UTI) de um hospital universitário. Trata-se de um estudo observacional prospectivo no qual foram incluídos 75 pacientes, com idade média de 58,3±18,8 anos. Desses, doze necessitaram do uso da VNI por mais de uma vez, totalizando 92 utilizações. Evidenciou-se que, delas, a taxa de sucesso foi de 60,9% (56). O grupo insucesso apresentou mais indivíduos do sexo masculino (p=0,006) e número maior de pacientes com diagnóstico de infecção extrapulmonar (p=0,012). Não foram encontradas diferenças entre os grupos de sucesso e insucesso nos quesitos de modo, modelo, máscara, tempo total de permanência e razões para a instalação da VNI. No grupo insucesso, a pressão positiva inspiratória nas vias aéreas (Ipap) e o volume corrente (VC) foram superiores (p=0,029 e p=0,011, respectivamente). A saturação periférica de oxigênio (p=0,047), o pH (p=0,004), base excess (p=0,006) e o bicarbonato (p=0,013) apresentaram valores inferiores. Concluiu-se que os indivíduos do sexo masculino com diagnóstico de infecção extrapulmonar e que evoluíram com acidose metabólica evoluíram com mais insucesso na utilização da VNI. Esses, necessitaram de parâmetros elevados de Ipap e VC.

This article presents an integrative review on the effects of Slow and Prolonged Expiration (ELPr) associated with respiratory physiotherapy techniques in infants with acute viral bronchiolitis (BVA). BVA is a bronchiole injury caused by viral infection, which obstructs the small airways in infants. The integrative review used the databases PUBmed, LILACS, Scielo, and Cochrane, selecting clinical trials and reviews published between 2012 and 2024. Three clinical trials and one integrative review were included, which showed that ELPr associated with other techniques reduces the severity of BVA in infants, but the effectiveness in oxygen therapy and respiratory discomfort is uncertain. ABSTRACT Dysfunctional breathing (DB) is a respiratory condition characterized by irregular breathing patterns that occur either in the absence of concurrent diseases or secondary to cardiopulmonary diseases. Although the primary symptom is often dyspnea or “air hunger”, DB is also associated with nonrespiratory symptoms such as dizziness and palpitations. DB has been identified across all ages. Its prevalence among adults in primary care in the United Kingdom is approximately 9.5%. In addition, among individuals with asthma, a positive diagnosis of DB is found in a third of women and a fifth of men. Although DB has been investigated for decades, it remains poorly understood because of a paucity of high-quality clinical trials and validated outcome measures specific to this population. Accordingly, DB is often underdiagnosed or misdiagnosed, given the similarity of its associated symptoms (dyspnea, tachycardia, and dizziness) to those of other common cardiopulmonary diseases such as COPD and asthma. The high rates of misdiagnosis of DB suggest that health care professionals do not fully understand this condition and may therefore fail to provide patients with an appropriate treatment. Given the multifarious, psychophysiological nature of DB, a holistic, multidimensional assessment would seem the most appropriate way to enhance understanding and diagnostic accuracy. The present narrative review was developed as a means of summarizing the available evidence about DB, as well as improving understanding of the condition by researchers and practitioners.

RESUMO Objetivo: Determinar a eficácia da manobra de hiperinsuflação pulmonar com o ventilador mecânico, em comparação à aspiração traqueal isolada, para remover secreções, normalizar a hemodinâmica e melhorar a mecânica pulmonar em pacientes em ventilação mecânica. Métodos: Ensaio clínico randomizado cruzado incluindo pacientes em ventilação mecânica por mais de 48 horas internados na unidade de terapia intensiva. Os pacientes foram randomizados para receber a aspiração traqueal isolada (Grupo Controle) e hiperinsuflação pulmonar por meio do ventilador mecânico (Grupo HVM). Mensuraram-se parâmetros hemodinâmicos e de mecânica respiratória, assim como a quantidade de secreção aspirada. Resultados: Foram incluídos 50 pacientes. A média de idade dos pacientes foi de 44,7 ± 21,6 anos, sendo 31 do sexo masculino. O Grupo HVM apresentou maior quantidade de secreção aspirada (3,9g versus 6,4g; p = 0,0001), variação na média da complacência dinâmica (-1,3 ± 2,3 versus -2,9 ± 2,3; p = 0,008), volume corrente expirado (-0,7 ± 0,0 versus -54,1 ± 38,8; p = 0,0001) e diminuição significativa da pressão inspiratória de pico (0,2 ± 0,1 versus 2,5 ± 0,1; p = 0,001), em comparação com o Grupo Controle. Conclusão: Na amostra estudada, a técnica de HVM apresentou maior quantidade de secreção aspirada, aumento significativo da complacência dinâmica e volume corrente expirado, além de diminuição significativa da pressão de pico inspiratória.

RESUMO A Síndrome do Respirador Oral é caracterizada por um conjunto de sinais e sintomas que podem estar presentes em indivíduos que substituem o modo adequado e eficiente da respiração nasal pelo modo respiratório oral ou misto, por um período igual ou superior a seis meses. O modo respiratório oral ou misto pode estar associado a mudanças na função e mecânica ventilatória. Esta revisão tem como objetivo investigar mais profundamente as consequências da respiração oral na função pulmonar e músculos respiratórios, ressaltando o desenvolvimento de tais alterações desde a infância até a idade adulta. Foram selecionados 18 artigos por meio das bases de dados Pubmed e Web of Science e foram agrupados no texto, compreendendo os seguintes tópicos: 1) Implicações da respiração oral sobre a função pulmonar e 2) Implicações da respiração oral sobre os músculos respiratórios. A partir das informações oriundas dos resultados dos artigos analisados, percebe-se que poucos estudos refutam ou não encontram alguma relação entre as alterações pulmonares e a respiração oral. Sugere-se que, o desequilíbrio muscular produzido por estas alterações pode contribuir para a desvantagem mecânica do músculo diafragma e aumento do trabalho dos músculos acessórios da inspiração. Entretanto, são necessários estudos com métodos mais criteriosos, incluindo avaliações objetivas e reprodutíveis dos músculos respiratórios.

Objetivo: Conhecer a percepção das mães sobre as principais dificuldades na assistência a uma criança com necessidades especiais, dependente de ventilação mecânica. Métodos: Pesquisa qualitativa, desenvolvida de agosto a novembro de 2013 no Hospital Dr. Waldemar Alcântara, em Fortaleza-CE. Os informantes foram sete mães de crianças sob ventilação mecânica. A coleta de dados deu-se através de questionários sociodemográficos e questões norteadoras, das quais emergiram categorias temáticas, analisadas pela análise de conteúdo, sendo os dados discutidos pela literatura pertinente. Resultados: A média das idades maternas variou de 18 a 36 anos. Seis eram casadas ou viviam união estável. O nível de escolaridade variou de analfabeta ao segundo grau completo. A renda familiar foi de no máximo um salário mínimo. As mães mencionaram angústia e medo diante do impacto inicial do diagnóstico, com pouca compreensão da linguagem biomédica e muitos questionamentos durante todo o período após o diagnóstico e durante a internação hospitalar. Conclusão: As dificuldades envolvem aspectos relacionados ao abandono familiar, ao distanciamento dos demais filhos em detrimento do cuidado ao filho deficiente, ao obstáculo em se relacionar socialmente e acerca do prognóstico e cuidados com seu filho. O ambiente hospitalar gera repercussões psicológicas diante da expectativa e da falta de esperança em relação à cura do filho, sabendo que este poderá ir a óbito. Os profissionais de saúde podem favorecer transformações incríveis, gerando um novo “cuidar”, mais amplo e humanizado, facilitando a recuperação/reestruturação familiar diante desse novo universo.

Bronchial stenosis can impair regional lung ventilation by causing abnormal, asymmetric airflow limitation. Electrical impedance tomography (EIT) is an imaging technique that allows the assessment of regional lung ventilation and therefore complements the functional assessment of the lungs. We report the case of a patient with left unilateral bronchial stenosis and a history of tuberculosis, in whom regional lung ventilation was assessed by EIT. The EIT results were compared with those obtained by ventilation/perfusion radionuclide imaging. The patient was using nasal continuous positive airway pressure (CPAP) for the treatment of obstructive sleep apnea syndrome. Therefore, we studied the effects of postural changes and of the use of nasal CPAP. The EIT revealed heterogeneous distribution of regional lung ventilation, the ventilation being higher in the right lung, and this distribution was influenced by postural changes and CPAP use. The EIT assessment of regional lung ventilation produced results similar to those obtained with the radionuclide imaging technique and had the advantage of providing a dynamic evaluation without radiation exposure.

OBJECTIVE: To compare pressure-support ventilation with spontaneous breathing through a T-tube for interrupting invasive mechanical ventilation in patients undergoing cardiac surgery with cardiopulmonary bypass. METHODS: Adults of both genders were randomly allocated to 30 minutes of either pressure-support ventilation or spontaneous ventilation with "T-tube" before extubation. Manovacuometry, ventilometry and clinical evaluation were performed before the operation, immediately before and after extubation, 1h and 12h after extubation. RESULTS: Twenty-eight patients were studied. There were no deaths or pulmonary complications. The mean aortic clamping time in the pressure support ventilation group was 62 ± 35 minutes and 68 ± 36 minutes in the T-tube group (P=0.651). The mean cardiopulmonary bypass duration in the pressure-support ventilation group was 89 ± 44 minutes and 82 ± 42 minutes in the T-tube group (P=0.75). The mean Tobin index in the pressure support ventilation group was 51 ± 25 and 64.5 ± 23 in the T-tube group (P=0.153). The duration of intensive care unit stay for the pressure support ventilation group was 2.1 ± 0.36 days and 2.3 ± 0.61 days in the T-tube group (P=0.581). The atelectasis score in the T-tube group was 0.6 ± 0.8 and 0.5 ± 0.6 (P=0.979) in the pressure support ventilation group. The study groups did not differ significantly in manovacuometric and ventilometric parameters and hospital evolution. CONCLUSION: The two trial methods evaluated for interruption of mechanical ventilation did not affect the postoperative course of patients who underwent cardiac operations with cardiopulmonary bypass.

Introdução: Um dos objetivos da ventilação mecânica (VM) é a aplicação de terapêuticas específicas na higiene brônquica, como a hiperinsuflação mecânica com o ventilador. Terapêutica está visa promover a expansão das unidades atelectasiadas através dos canais colaterais, favorecendo o deslocamento da secreção pulmonar das vias aérea periféricas para as centrais. Objetivos: Comparar a segurança e eficácia da técnica de hiperinsuflação com ventilador mecânico na quantidade de secreção pulmonar aspirada com a técnica de aspiração isolada, através da avaliação dos parâmetros ventilatórios e hemodinâmicos e da quantidade de secreção pulmonar aspirada antes e após as manobras e entre elas. Métodos: Ensaio clínico randomizado cruzado, desenvolvido no Centro de Terapia Intensiva (CTI) do Hospital de Clínicas de Porto Alegre. A amostra constituiu-se de 23 pacientes em VM. Resultados: Não foram encontradas diferenças significativas entre as manobras quanto à quantidade de secreção aspirada e valores hemodinâmicos e ventilatórios. A pressão arterial média (PAM) teve um aumento significativo após a manobra de aspiração, mas sem relevância clínica. Conclusão: A hiperinsuflação com o ventilador mecânico é uma técnica segura e tão eficaz quanto à aspiração isolada como técnica de higiene brônquica. Este estudo objetivou atualizar os conhecimentos em relação à utilização da ventilação mecânica não-invasiva (VMNI) no pós-operatório de cirurgia cardíaca e identificar se há indícios da superioridade de uma forma de modalidade de VMNI em relação à outra. Foi realizada revisão da literatura entre 2006 a 2011, a partir das bases de dados PubMed, SciELO e Lilacs, utilizando os descritores respiração artificial, pressão positiva contínua nas vias aéreas, ventilação com pressão positiva intermitente e cirurgia cardíaca, e seus correspondentes na língua inglesa, os quais foram pesquisados em cruzamentos. A partir dos critérios adotados, foram selecionados nove artigos, dos quais seis demonstraram aplicações de VMNI, por meio de modalidades como pressão positiva contínua nas vias aéreas, pressão positiva com dois níveis pressóricos e respiração com pressão positiva intermitente, no pós-operatório de cirurgia cardíaca, e, três realizaram comparações entre as diferentes modalidades. As modalidades de VMNI descritas na literatura foram utilizadas com resultados satisfatórios. Estudos que comparam diferentes modalidades são escassos, contudo alguns demonstraram superioridade de uma modalidade de VMNI, como é o caso da respiração com pressão positiva intermitente na reversão da hipoxemia e da pressão positiva com dois níveis pressóricos na melhora da oxigenação, da frequência respiratória e frequência cardíaca desses pacientes, em comparação a outras modalidades.This study aimed to update knowledge regarding to noninvasive ventilation (NVI) on postoperative of cardiac surgery in addition at investigating if exists superiority of any modalities NVI in relation to the others. The literature review was performed on the period between 2006 and 2011, on PubMed, SciELO and Lilacs databases crossing the keywords: artificial respiration, continuous positive airway pressure, intermittent positive-pressure ventilation, cardiac surgery and their corresponding in English. Based on the criteria adopted, nine articles were selected being six of them use NVI, through the modalities such as continuous positive airway pressure, positive pressure with bilevel pressure and intermittent positive-pressure ventilation in postoperative of cardiac surgery; only three of them performed comparisons between different modalities. The NVI modalities that were described on the literature had showed satisfactory results. A few studies compare different NVI modalities; however some of them showed superiority in relation to the others, such as the intermittent positive-pressure ventilation to threat hypoxemia and to positive pressure with bilevel pressure to increase oxygenation, respiratory rate and heart rate in these patients, when compared with other modalities.

INTRODUÇÃO: A aplicação de ventilação por dois níveis de pressão positiva (BiPAP®) associada à fisioterapia respiratória convencional (FRC) no pós-operatório (PO) imediato de cirurgia cardíaca pode contribuir para a diminuição das complicações pulmonares. OBJETIVO: Avaliar a segurança e a adesão da aplicação preventiva do BiPAP® associado a FRC no PO imediato de revascularização do miocárdio. MÉTODOS: Vinte e seis pacientes submetidos a revascularização do miocárdio foram aleatoriamente alocados. O Grupo Controle (GC) foi tratado com FRC, o Grupo BiPAP (GB) foi submetido a 30 minutos de BiPAP®, duas vezes ao dia, associado à FRC. A FRC foi realizada em ambos os grupos, duas vezes ao dia. Todos os pacientes foram avaliados quanto: capacidade vital, permeabilidade das vias aéreas, pressões respiratórias máximas, saturação de oxigênio, frequência cardíaca, frequência respiratória, volume minuto, volume corrente, pressões arteriais sistólica e diastólica. As avaliações foram realizadas durante a internação no pré-operatório, imediatamente após a extubação, e na 24ª e 48ª horas após extubação. RESULTADOS: No GC, 61,5% dos pacientes tiveram algum grau de atelectasias, no GB, 54% (P=0,691). A capacidade vital foi estatisticamente maior no GB no PO (PINTRODUCTION: The application of two levels of ventilation by positive pressure (BiPAP®) associated with conventional respiratory therapy (CRT) in postoperative periord of cardiac surgery may contribute to reduction of pulmonary complications. OBJECTIVES: To evaluate the safety and compliance of preventive application of BiPAP® CRT associated with immediate postoperative myocardial revascularization. METHODS: 26 patients undergoing coronary artery bypass grafting were randomly allocated in one of the groups. Patients of the Control Group (CG) were treated only with conventional respiratory therapy, compared to BiPAP group (BG) (in addition to conventional respiratory therapy the patients were subjected to 30 minutes of ventilation by two levels twice a day). The conventional respiratory therapy was held in both groups, twice a day. All patients were evaluated for vital capacity, airway permeability, maximal respiratory pressures, oxygen saturation, heart rate, respiratory frequency, Volume Minute, tidal volume, systolic and diastolic blood pressure. Evaluations were performed during hospitalization preoperatively, immediately after extubation, 24h and 48h after extubation. FUNDAMENTO: Os valores de referência de teste cardiopulmonar (TCP) disponíveis no Brasil foram derivados de cicloergômetro, em população sedentária e relativamente pequena. OBJETIVO: Fornecer valores de referência para o TCP em brasileiros de ambos os sexos, sedentários e ativos. MÉTODOS: ENtre 2006 e 2008, 3.992 TCP de indivíduos saudáveis foram selecionados de nosso laboratório. Atletas, fumantes, portadores de qualquer patologia conhecida, usuários de medicação contínua e obesos foram excluídos. VO2 pico foi considerado VO2 máx. Analisamos também VO2 de limiar anaeróbico, ventilação máxima e pulso de oxigênio de acordo com sexo, faixa etária, sedentários e ativos. As faixas etárias foram assim divididas: G1 (15-24 anos), G2 (25-34), G3 (35-44), G4 (45-54), G5 (55-64) e G6 (65-74). RESULTADOS: De acordo com as faixas etárias, os valores médios de VO2 em ml/kg/min com os respectivos desvios-padrão foram: Homem ativo: G1-50,6 ± 7,3; G2-47,4 ± 7,4; G3-45,4 ± 6,8; G4-40,5 ± 6,5; G5-35,3 ± 6,2; G6-30,0 ± 6,1. Mulher ativa: G1-38,9 ± 5,7; G2-38,1 ± 6,6; G3-34,9 ± 5,9; G4-31,1 ± 5,4; G5-28,6 ± 6,1; G6-25,1 ± 4,4. OBJECTIVE: Provide reference values for CPET in Brazilians of both sexes, either sedentary or active. METHODS: From 2006 to 2008, 3,992 CEPT of healthy individuals were selected from our laboratory. Athletes, smokers, patients with any known pathology, users of continuous medication and obese patients were excluded. Peak VO2 was considered max VO2. We also analyzed the anaerobic threshold VO2, maximum ventilation and oxygen pulse according to sex, age, sedentary and active patients. Age groups were divided as follows: G1 (15-24), G2 (25-34), G3 (35-44), G4 (45-54), G5 (55-64) and G6 (65-74). RESULTS: According to age groups, the mean values of VO2 in ml/kg/min with their standard deviations were: Active man: G1-50.6 ± 7.3, G2-47, 4 ± 7.4, G3-45, 4 ± 6.8, G4-40.5 ± 6.5; G5-35.3 ± 6.2; G6-30.0 ± 6.1. Active woman: G1-38.9 ± 5.7; G2-38.1 ± 6.6; G3-34.9 ± 5.9; G4-31.1 ± 5.4; G5-28.6 ± 6.1; G6-25.1 ± 4.4. Sedentary man: G1-47.4 ± 7.9; G2-41.9 ± 7.2; G3-39.0 ± 6.8; G4-35.6 ± 7.7; G5-30.0 ± 6.3; G6-23.1 ± 6.3. Sedentary woman: G1-35.6 ± 5.7; G2-34.0 ± 4.8; G3-30.0 ± 5.4; G4-27.2 ± 5.0; G5-23.9 ± 4.2; G6-21.2 ± 3.4. CONCLUSION: This article provides reference values of max VO2, among other parameters, in the Cardiopulmonary Exercise Testing performed on the treadmill in individuals of both sexes, either active and sedentary.

PURPOSE: To develop an animal model of diaphragmatic electrical stimulation able to generate an appropriate ventilatory support through the direct implantation of electrodes in the diaphragm (electroventilation). METHODS: Six New Zealand female rabbits (2-3 kg) were placed on mechanical ventilation. Then, a laparotomy was performed in order to identify the motor points in each hemidiaphragm, followed by the implantation of the electrodes for diaphragmatic stimulation. We tested two types of electrodes according to the conduction of electrical stimulation: unipolar and bipolar. The electrodes were placed on different occasions in the same animals and tested with current intensities of 20, 26 and 32 mA. Each current intensity was repeated three times for 10 respiratory cycles with 1 minute interval between each cycle, and 5 minutes for new current intensity. We recorded the relationship between current intensity and inspiratory volume. RESULTS: The electrodes adequately stimulate the diaphragm and obtain inspired volumes using different intensity currents. The bipolar electrode generated inspiratory volumes as high as 4.5 times of baseline while the unipolar electrode reached up to 3.5 times of baseline. CONCLUSION: This model has proved to be effective for studying the performance of the diaphragm under different electrical stimulations using different set of electrodes.OBJETIVO: Desenvolver um modelo animal de estimulação elétrica diafragmática capaz de gerar um suporte ventilatório adequado através da implantação direta de eletrodos no diafragma (eletroventilação). MÉTODOS: Seis coelhas da raça Nova Zelândia (2-3 kg) foram colocadas em ventilação mecânica. Através de uma laparotomia foram identificados os pontos motores em ambas as cúpulas diafragmáticas, seguido pela implantação dos eletrodos para estimulação diafragmática. Foram testados dois tipos de eletrodos de acordo com a condução do estímulo elétrico: unipolar e bipolar. Os eletrodos foram posicionados em diferentes ocasiões nos mesmos animais e testados com intensidades de corrente de 20, 26 e 32 mA, sendo cada uma delas repetidas três vezes durante 10 ciclos respiratórios com intervalo de 1 minuto entre cada ciclo e repouso de 5 minutos para nova intensidade de corrente. Foram registradas a relação entre a intensidade de corrente e o volume de ar inspirado. RESULTADOS: Os eletrodos estimularam adequadamente o diafragma com um volume de ar inspirado até 4,5 vezes maior no modo bipolar e de 3,5 vezes no modo unipolar em relação à linha de base. CONCLUSÃO: Este modelo demonstrou ser efetivo para o estudo do desempenho do diafragma sob diferentes estimulações elétricas e diferentes tipos de eletrodos.

OBJETIVO: Comparar a influência de duas estratégias ventilatórias - ventilação controlada a volume (VCV) e ventilação controlada a pressão (VCP) - no desempenho funcional de enxertos pulmonares em um modelo canino de transplante pulmonar unilateral esquerdo, utilizando-se doadores cujos pulmões foram captados após três horas de parada cardiocirculatória em temperatura ambiente e sob ventilação mecânica. MÉTODOS: O estudo incluiu 40 cães mestiços randomizados nos grupos VCV e VCP. Dos 20 receptores, 5 não sobreviveram ao transplante, e 5 não sobreviveram ao período de avaliação pós-transplante. Os 10 receptores sobreviventes (5 em cada grupo) foram avaliados durante 360 min após o término do transplante pulmonar. O desempenho funcional dos enxertos foi estudado através da avaliação da mecânica respiratória, trocas gasosas e histologia do enxerto. RESULTADOS: Não houve diferenças significativas entre os grupos quanto às variáveis de mecânica respiratória (pressão de pico inspiratória, pressão de platô, pressão média de vias aéreas, complacência dinâmica e complacência estática) e de trocas gasosas (PaO2, pressão venosa mista de oxigênio, PaCO2, pressão venosa mista de CO2 e diferença arteriovenosa de oxigênio). As alterações histopatológicas foram compatíveis com o padrão de lesão pulmonar aguda não específica e não diferiram entre os grupos. CONCLUSÕES: Este modelo de transplante pulmonar mostrou que o desempenho funcional do enxerto não foi influenciado pela estratégia ventilatória utilizada até seis horas após a reperfusão.OBJECTIVE: To compare the influence of two different ventilation strategies-volume-controlled ventilation (VCV) and pressure-controlled ventilation (PCV)-on the functional performance of lung grafts in a canine model of unilateral left lung transplantation using donor lungs harvested after three hours of normothermic cardiocirculatory arrest under mechanical ventilation. METHODS: The study comprised 40 mongrel dogs, randomized into two groups: VCV and PCV. Of the 20 recipients, 5 did not survive the transplant, and 5 died before the end of the post-transplant assessment period. The remaining 10 survivors (5 in each group) were evaluated for 360 min after lung transplantation. The functional performance of the grafts was evaluated regarding respiratory mechanics, gas exchange, and lung graft histology. RESULTS: There were no significant differences between the groups regarding respiratory mechanics (peak inspiratory pressure, plateau pressure, mean airway pressure, dynamic compliance, and static compliance) or gas exchange variables (PaO2, venous oxygen tension, PaCO2, venous carbon dioxide tension, and the arterial-venous oxygen content difference). The histopathological findings were consistent with nonspecific acute lung injury and did not differ between the groups. CONCLUSIONS: This model of lung transplantation showed that the functional performance of lung grafts was not influenced by the ventilation strategy employed during the first six hours after reperfusion.

OBJETIVO: Comparar os efeitos da traqueostomia precoce e da traqueostomia tardia em pacientes com lesão cerebral aguda grave. MÉTODOS: Estudo retrospectivo com 28 pacientes admitidos na UTI do Hospital Universitário da Universidade Federal de Juiz de Fora com diagnóstico de lesão cerebral aguda grave e apresentando escore na escala de coma de Glasgow (ECG) < 8 nas primeiras 48 h de internação. Os pacientes foram divididos em dois grupos: traqueostomia precoce (TP), realizada em até 8 dias; e traqueostomia tardia (TT), realizada após 8 dias. Dados demográficos e os escores Acute Physiology and Chronic Health Evaluation (APACHE) II, ECG e Sequential Organ Failure Assessment (SOFA) do dia da admissão foram coletados. RESULTADOS: Não houve diferenças significativas em relação aos dados demográficos ou aos escores coletados nos grupos TP e TT: APACHE II (26 ± 6 vs. 28 ± 8; p = 0,37), SOFA (6,3 ± 2,7 vs. 7,2 ± 3,0; p = 0,43) e ECG (5,4 ± 1,7 vs. 5,5 ± 1,7; p = 0,87). A mortalidade em 28 dias foi menor no grupo TP (9% vs. 47%; p = 0,04). Pneumonia nosocomial precoce (até 7 dias) foi menos frequente no grupo TP, mas essa diferença não foi significativa (0% vs. 23%, p = 0,13). Não houve diferenças em relação à ocorrência de pneumonia tardia ou ao tempo de ventilação mecânica entre os grupos. CONCLUSÕES: Baseado nesses achados, a traqueostomia precoce deve ser considerada em pacientes com lesão cerebral aguda grave.OBJECTIVE: To compare the effects of early tracheostomy and of late tracheostomy in patients with acute severe brain injury. METHODS: A retrospective study involving 28 patients admitted to the ICU of the Federal University of Juiz de Fora University Hospital in Juiz de Fora, Brazil, diagnosed with acute severe brain injury and presenting with a Glasgow coma scale (GCS) score < 8 within the first 48 h of hospitalization. The patients were divided into two groups: early tracheostomy (ET), performed within the first 8 days after admission; and late tracheostomy (LT), performed after postadmission day 8. At admission, we collected demographic data and determined the following scores: Acute Physiology and Chronic Health Evaluation (APACHE) II, GCS and Sequential Organ Failure Assessment (SOFA). RESULTS: There were no significant differences between the groups (ET vs. LT) regarding the demographic data or the scores: APACHE II (26 ± 6 vs. 28 ± 8; p = 0.37), SOFA (6.3 ± 2.7 vs. 7.2 ± 3.0; p = 0.43) and GCS (5.4 ± 1.7 vs. 5.5 ± 1.7; p = 0.87). The 28-day mortality rate was lower in the ET group (9% vs. 47%; p = 0.04). Nosocomial pneumonia occurring within the first 7 days was less common in the ET group, although the difference was not significant (0% vs. 23%; p = 0.13). There were no differences regarding the occurrence of late pneumonia or in the duration of mechanical ventilation between the groups. CONCLUSIONS: On the basis of these findings, early tracheostomy should be considered in patients with acute severe brain injury.

O tratamento da contusão pulmonar quando instituído de forma correta é bastante simples na maioria das vezes. As alterações fisiopatológicas acontecem como decorrência dos efeitos produzidos pela perda da integridade da parede torácica, acúmulo de líquidos na cavidade pleural, obstrução da via aérea e disfunção pulmonar. A manobra de recrutamento alveolar consiste na reabertura de áreas pulmonares colapsadas através do aumento da pressão inspiratória na via aérea. O objetivo deste relato foi apresentar um caso de contusão pulmonar, avaliando a efetividade da manobra de recrutamento alveolar e revisão da literatura. Paciente do sexo masculino, 33 anos, com quadro clínico de trauma de tórax bilateral e trauma crânio-encefálico, evoluiu com rebaixamento do nível de consciência, insuficiência respiratória aguda, choque hipovolêmico, hemoptise. Foi submetido a toracocentese, drenagem torácica bilateral e submetido a ventilação mecânica invasiva. Após 48 horas de ventilação mecânica invasiva, segundo os preceitos da estratégia protetora, iniciou-se manobras de recrutamento alveolar modo, Pressão controlada 10 cmH2O, freqüência respiratória 10rpm, tempo inspiratório 3.0, pressão positiva no final da expiração 30 cmH2O, FIO2 100%, durante dois minutos. Após a aplicação da manobra de recrutamento alveolar O paciente apresentou melhora pulmonar significativa da oxigenação, caracterizada por aumento da relação PaO2/FiO2, porém houve variação da mesma entre 185 a 322. Obteve alta da unidade na terapia intensiva após 22 dias e hospitalar após 32 dias da admissão. A manobra de recrutamento alveolar neste paciente apresentou resultados significativos no tratamento da contusão pulmonar, melhorando a oxigenação arterial, prevenindo o colapso alveolar e revertendo quadros de atelectasias.Treatment of pulmonary contusion when adequately established is very simple in most cases. Pathophysiological changes occur as a result of the effects produced by loss of chest wall integrity, accumulation of fluid in the pleural cavity, obstruction of the airways and lung dysfunction. The alveolar recruitment maneuver is the reopening of collapsed lung areas by increasing inspiratory pressure in the airway. The primary objective of this case report was to evaluate the effectiveness of the alveolar recruitment maneuver in a patient with pulmonary contusion. A 33 year old male patient, with a clinical condition of bilateral chest trauma and traumatic brain injury, evolved with reduction of the level of consciousness, acute respiratory failure, hypovolemic shock and hemoptysis. The patient underwent thoracentesis, bilateral thoracic drainage and was also submitted to invasive mechanical ventilation. After 48 hours of invasive mechanical ventilation, in accordance with protective strategy an alveolar recruitment maneuver mode, pressure-controlled ventilation, pressure controlled 10 cmH2O, respiratory rate 10 rpm, inspiratory time 3.0, positive end-expiratory pressure 30 cmH2O and FI0(2) 100%, for two minutes. After the alveolar recruitment maneuver, the patient presented clinical pulmonary improvement, but there was a variation of 185 to 322 of Pa0(2)/FiO2 (arterial partial pressure of oxygen/ fraction of inspired oxygen). He was discharged from the intensive care unit 22 days after admission. The alveolar recruitment maneuver in this patient showed significant results in the treatment of pulmonary contusion, improving blood oxygenation, preventing alveolar collapse and reversing atelectasis.

OBJETIVO: Determinar possíveis alterações do volume corrente e da pressão de pico inspiratório fornecidos por sete modelos de reanimador manual com balão auto-inflável em função do fluxo de entrada de oxigênio utilizado. MÉTODOS: Os reanimadores testados foram: Oxigel, modelos A e B; CE Reanimadores; ProtecSolutions; Missouri; Axmed; e Narcosul. Para as aferições, acoplaram-se um fluxômetro de oxigênio de parede, um fluxômetro/respirômetro, um reanimador, um sensor (aparelho Tracer 5) e um pulmão-teste. Além disso, acoplou-se o aparelho Tracer 5 a um notebook. Utilizaram-se fluxos de entrada de oxigênio de 1, 5, 10 e 15 L/min. RESULTADOS: O volume corrente fornecido pelo reanimador Oxigel modelo A ao receber 15 L/min de oxigênio foi aproximadamente 99% maior que o fornecido ao receber 1 L/min de oxigênio. Da mesma forma, a pressão de pico inspiratório foi 155% maior. Nas mesmas condições, o volume corrente fornecido pelo reanimador Narcosul foi 48% maior, e a pressão de pico inspiratório foi 105% maior. Os demais reanimadores testados não apresentaram alterações significativas do volume corrente e da pressão de pico inspiratório fornecidos. CONCLUSÕES: Nas condições de resistência e complacência utilizadas, os reanimadores em que o fluxo de entrada de oxigênio é direcionado diretamente ao interior do balão tiveram a válvula do paciente presa em posição de inspiração ao receberem um fluxo > 5 L/min, aumentando significativamente o volume corrente e a pressão de pico inspiratório fornecidos. Isso não ocorreu nos reanimadores em que o fluxo de entrada de oxigênio é direcionado ao exterior do balão.OBJECTIVE: To assess possible alterations in the tidal volume and peak inspiratory pressure delivered by seven models of manually operated self-inflating resuscitation bags as a function of the oxygen supply rate used. METHODS: The resuscitation bags tested were the following: Oxigel, models A and B; CE Reanimadores; ProtecSolutions; Missouri; Axmed; and Narcosul. For the measurements, a wall oxygen flow meter, a flow meter/respirometer, a resuscitation bag, a sensor (Tracer 5 unit), and a test lung were connected. In addition, the Tracer 5 unit was connected to a notebook computer. Oxygen supply rates of 1, 5, 10, and 15 L/min were used. RESULTS: The tidal volume delivered by the Oxigel model A resuscitation bag when receiving oxygen at a rate of 15 L/min was approximately 99% greater than that delivered when receiving oxygen at a rate of 1 L/min. Similarly, peak inspiratory pressure was approximately 155% greater. Under the same conditions, the tidal volume delivered by the Narcosul resuscitation bag was 48% greater, and peak inspiratory pressure was 105% greater. The remaining resuscitation bags tested showed no significant alterations in the tidal volume or peak inspiratory pressure delivered. CONCLUSIONS: Under the resistance and compliance conditions used, the resuscitation bags in which the oxygen inflow is directly to the interior of the bag had the patient valve stuck at the inspiratory position when receiving oxygen at a rate > 5 L/min, significantly increasing the tidal volume and peak inspiratory pressure delivered. This did not occur with the resuscitation bags in which the oxygen inflow is directed to the exterior of the bag.

Introdução: O Rugby em Cadeira de Rodas (RCR) foi desenvolvido para que pessoas com comprometimento nos quatro membros (tetraplegia ou tetraequivalência) pudessem praticar um esporte coletivo em cadeira de rodas. Devido às características cardiopulmonares associadas às lesões, é fundamental que o treinador tenha conhecimento das capacidades e limitações dos atletas, para otimizar o treinamento e o rendimento. Objetivo: Verificar a recuperação da capacidade cardiopulmonar após esforço de atletas de RCR. Métodos: Um estudo seccional com 22 atletas realizando um teste de esforço cardiopulmonar até a intensidade máxima suportada, analisando as variáveis consumo oxigênio (VO2) e Ventilação pulmonar (VE) no pico do esforço e a cada minuto de recuperação (1º, 2º, e 3º). O teste seguiu um protocolo de rampa em um cicloergômetro com carga inicial de 20 W e implementos de 2 W ou 5 W de acordo com a Classificação Funcional (CF) do atleta, permanecendo no mesmo após a interrupção realizando uma recuperação ativa controlada. Comparou-se os valores obtidos entre as duas equipes com o teste T de Student e feita uma análise de correlação de Pearson com a recuperação cardiopulmonar e a CF dos atletas. Resultados: O Grupo de Alto Rendimento possui uma maior recuperação do VO2 (ml/kg/min) nos três minutos subsequentes ao esforço com média de 38,59 (+13,62, p=0,095); 47,68 (+14,18, p=0,255) e 57,10 (+13,07, p=,080) respectivamente, bem como uma maior recuperação da VE (L/min) no segundo 34,21 (+13,42, p=0,040) e terceiro 50,20 (+15,12, p=0,026) minutos quando comparado ao Grupo de Atletas Profissionais - VO2: 20,23 (+36,82); 38,83 (+19,87) e 45,21 (+14,54) – VE: 20,03 (+13,54) e 34,18 (+9,14) para os mesmos tempos subsequentes. Tempo de lesão e CF foram similares entre os grupos. Conclusão: Maior volume de prática do RCR tende a propiciar melhor capacidade de recuperação cardiopulmonar para essa população.

OBJETIVO: Avaliar o pico de pressão inspiratória e a freqüência ventilatória obtidos por médicos quando da utilização de balão auto-inflável neonatal em modelo de pulmão. MÉTODOS: Quinze médicos ventilaram simuladores de pulmão de recém-nascidos a termo e pré-termo, sendo os desfechos captados através de um monitor de ventilação. RESULTADOS: As medianas dos picos de pressão foram 23 (intervalo interquartil, 15-47) e 26 (intervalo interquartil, 14-51) cmH2O, sendo menor que 20 em 41,2 e 35,8% das vezes; maior que 40 em 29,7 e 33,6% das vezes e entre 27 e 33 cmH2O em 8,2 e 6,5% das vezes, para o pulmão de termo e de pré-termo, respectivamente. As medianas das freqüências ventilatórias foram de 45 (intervalo interquartil, 36-57) e 48 (intervalo interquartil, 39-55,5) ciclos por minuto, sendo menor que 30 em 9,3 e 6,7% das vezes e maior que 60 em 12 e 13,3% das vezes, respectivamente, para o pulmão de termo e de pré-termo. As diferenças nas medianas não foram estatisticamente significantes. CONCLUSÃO: A ventilação utilizando o balão auto-inflável promoveu adequação para a freqüência ventilatória em aproximadamente 80% das vezes, e os médicos não conseguiram promover ventilação com mínima variabilidade na pressão, alcançando níveis diferentes daqueles definidos no curso de reanimação neonatal em 70% das vezes. Essa situação independeu da ventilação do pulmão-teste análogo ao sistema respiratório de recém-nascidos a termo ou pré-termo.OBJECTIVE: To evaluate the peak inspiratory pressure and ventilation rate achieved by physicians when using a neonatal self-inflating bag on a lung model. METHODS: Fifteen physicians ventilated full term and preterm infant lung simulators while the outcomes were captured by a ventilation monitor. RESULTS: Median peak pressures in cmH2O for full term and preterm lungs were 23 (interquartile range: 15-47) and 26 (interquartile range: 14-51), being less than 20 in 41.2 and 35.8% of the pressure curves analyzed, more than 40 in 29.7 and 33.6%, and between 27 and 33 cmH2O in 8.2 and 6.5% of the curves, respectively. Median ventilation rates were 45 (interquartile range: 36-57) and 48 (interquartile range: 39-55.5) cycles per minute, being more than 30 in 9.3 and 6.7% of pressure curves and more than 60 in 12 and 13.3% of pressure curves, for the full term and preterm lungs, respectively. The differences between these medians were not statistically significant. CONCLUSIONS: Ventilation rates achieved with the self-inflating bag were adequate in approximately 80% of pressure curves analyzed, but the physicians were unable to provide ventilation with minimal pressure variation, producing pressures that diverged from those defined by the neonatal resuscitation training course in 70% of the curves. This was irrespective of whether they were ventilating the lung model analogous to preterm or full term infant lungs.

OBJETIVO: Determinar a freqüência relativa de hipomagnesemia em pacientes com limitação crônica do fluxo aéreo atendidos num ambulatório de referência do norte do Paraná, nos anos de 2000 a 2001, e verificar se há relação entre esse distúrbio e hipoxemia, outros distúrbios eletrolíticos e com a gravidade da doença. MÉTODOS: Estudo descritivo sobre a freqüência relativa de hipomagnesemia em 72 pacientes com limitação crônica do fluxo aéreo. Os pacientes realizaram dosagens séricas de magnésio e outros eletrólitos, além de realizarem o estadiamento de sua doença de base. RESULTADOS: A prevalência encontrada de hipomagnesemia foi de 27,8%. A idade média foi de 65 ± 9,9 anos, com predominância de homens. O volume expiratório forçado no primeiro segundo médio foi de 1,13 ± 0,52 L. A maioria dos pacientes encontrava-se em estádios avançados da doença (68,1%). Não houve associação do magnésio sérico baixo com outros distúrbios eletrolíticos, hipoxemia ou estádios de gravidade. CONCLUSÃO: A alta freqüência de pacientes em estádios avançados deve-se, provavelmente, ao fato de o ambulatório ser um centro de referência da região. Novos estudos devem ser realizados para determinar prováveis causas dessa alta prevalência de hipomagnesemia.OBJECTIVE: To determine the relative frequency of hypomagnesemia among patients with chronic airflow limitation treated as outpatients at a referral center in the northern part of the state of Paraná between 2000 and 2001, as well as to determine whether hypomagnesemia correlates with hypoxia, with other electrolyte disturbances and with the severity of airflow limitation. METHODS: This was a descriptive study of the relative frequency of hypomagnesemia in 72 patients with chronic airflow limitation. All of the patients were submitted to blood tests to determine serum levels of magnesium and other electrolytes, as well as to staging of the underlying disease. RESULTS: The prevalence of hypomagnesemia was 27.8%. The mean age was 65 ± 9.9 years, and there was a predominance of males. The mean forced expiratory volume in one second was 1.31 ± 0.52 L. Most of the patients (68.1%) were found to be in the advanced stages of the disease. Hypomagnesemia was not found to correlate with other electrolyte disturbances, hypoxemia or disease stage. CONCLUSION: The high frequency of patients in the advanced stages is likely attributable to the fact that the outpatient facility is a referral center for the region. Further studies should be conducted in order to determine the probable causes of this high prevalence of hypomagnesemia.

Introduction: One of the objectives of mechanical ventilation (MV) is the application of specific therapies in bronchial hygiene, such as mechanical ventilator hyperinflation, which aims to promote the expansion of atelectatic units through the collateral channels, favoring the displacement of the pulmonary secretion from the peripheral airways to the central ones. Objectives: To compare the safety and efficacy of the mechanical ventilator hyperinflation technique in the amount of secretion aspirated with the technique of isolated aspiration. This was done by. evaluating the ventilatory and hemodynamic parameters and the amount of pulmonary secretion aspirated before and after the maneuvers and by comparing the results found in both of them.Methods: A randomized crossover clinical trial developed at the intensive care unit (ICU) of the Hospital de Clínicas de Porto Alegre. The sample consisted of 23 patients on MV. Results: There were no significant differences between the maneuvers regarding the amount of aspirated secretion and hemodynamic and ventilatory values. Mean arterial pressure (MAP) had a significant increase after the aspiration maneuver, but without clinical relevance. Conclusion: Hyperinflation with the mechanical ventilator is a safe technique and as effective as isolated aspiration as a bronchial hygiene technique. [ABSTRACT FROM AUTHOR]

JUSTIFICATIVA E OBJETIVOS: O suporte ventilatório mecânico é uma das principais modalidades de apoio usadas em terapia intensiva. Na modalidade de pressão pré-determinada a pressão máxima é regulada, mas o volume corrente (V T) é uma função complexa da pressão aplicada e da sua velocidade em alcançar a pressão-alvo, do tempo inspiratório disponível e da resistência à respiração. O objetivo deste trabalho foi avaliar a influência do incremento do tempo inspiratório na ventilação pulmonar. MÉTODO: O estudo foi realizado na UTI Adulto do Hospital Regional de Mato Grosso do Sul em Campo Grande/MS. Foram incluídos pacientes adultos, de ambos os sexos, com idade entre 16 e 84 anos, submetidos à ventilação mecânica no modo pressão controlada ou assistido-controlada. Foi ajustado o tempo inspiratório em 1 seg, incrementando em 0,2 seg até o valor limite de 1,6 seg. Foram avaliados o volume-corrente (V T) e o volume-minuto (V E) de 13 pacientes nos tempos inspiratórios de 1,0s; 1,2s; 1,4s e 1,6s. RESULTADOS: Nas médias dos V T e V E observou-se um aumento crescente após o incremento do tempo inspiratório. Não foram encontrados na literatura consultada, dados que correlacionassem o tempo inspiratório com alterações no volume-corrente. CONCLUSÕES: O incremento do tempo inspiratório na ventilação por controle de pressão pode ter influência na determinação do volume-corrente ofertado ao paciente.BACKGROUND AND OBJECTIVES: The mechanical ventilator support is one of the main used modalities of support in intensive therapy. In the modality of predetermined pressure, the maximum pressure is regulated, but the current volume (V T) is a complex function of the applied pressure and its speed to reach the pressure-target, of the available breathing time and the resistance to the breath. This paper has as objective to evaluate the influences of the increment of the breathing time in the pulmonary ventilation. METHODS: The study was carried through in the Adult ICU of the Regional Hospital of Mato Grosso do Sul, located in Campo Grande. They had been enclosed individuals adult, both genders, between 16 and 84 years, submitted to the mechanical ventilation in controlled pressure mode or in controlled-watched mode. The breathing time was adjusted in 1 sec, developing in 0.2 sec until the boundary-value of 1.6 sec. The tidal volume (V T) and the volume minute (V E) had been evaluated of 13 patients in the breathing times of 1s; 1.2s; 1.4s and 1.6s. RESULTS: In the mean of V T and V E an increasing increase was observed after-increment of the breathing time. We did not find in consulted literature, data which correlated the breathing time with alterations in the current volume. CONCLUSIONS: The increment of the breathing time in the ventilation for pressure control can have influence in the determination of the current volume offered to the patient.

INTRODUÇÃO: A espirometria é muito utilizada no diagnóstico e na quantificação dos distúrbios ventilatórios como a asma. O micro espirômetro é um equipamento portátil e de fácil utilização que pode substituir o espirômetro convencional, embora existam poucas evidências de sua acurácia. OBJETIVO: Comparar o micro espirômetro com o espirômetro convencional em crianças e adolescentes asmáticos. MÉTODO: Os aparelhos utilizados foram o Micro Spirometer, da marca Micro Medical, e o espirômetro convencional Cosmed-Pony graphic 3.5, ambos com turbinômetros (sensores de fluxo). Foram incluídos na amostra 62 crianças e adolescentes, de ambos os sexos, com idades compreendidas entre cinco e dezesseis anos, com diagnóstico clínico de asma, em acompanhamento em serviço de Pneumologia. Os exames espirométricos foram realizados dentro das normas da American Thoracic Society e da Associação Brasileira de Normas Técnicas. Primeiro realizou-se a micro espirometria e em seguida, com o mesmo paciente, realizou-se a espirometria convencional. Os parâmetros analisados em ambos os aparelhos foram: capacidade vital forçada, volume expiratório forçado no primeiro segundo e pico de fluxo expiratório. Os dados foram analisados através do teste t de Student (pareado) e do teste de correlação de Pearson. RESULTADOS: Obteve-se forte correlação entre os parâmetros analisados nos dois aparelhos: para volume expiratório forçado no primeiro segundo r = 0,97, para capacidade vital forçada r = 0,97, e para pico de fluxo expiratório r = 0,91. CONCLUSÃO: Os resultados demonstraram que o micro espirômetro é um equipamento diagnóstico útil, que pode ser utilizado quando o espirômetro convencional não estiver disponível.BACKGROUND: Spirometry is widely used in the diagnosis and quantification of respiratory disorders such as asthma. A microspirometer is a portable and easily used apparatus that can substitute for a regular spirometer, although there is little evidence of its accuracy. OBJECTIVE: Tocompare the microspirometer to a regular spirometer for use in asthmatic children and adolescents. METHODS: The instruments used were a Micro Spirometer, manufactured by Micro Medical, and a conventional Cosmed Pony Graphic 3.5 spirometer, both with turbinometers (flow sensors). The study sample consisted of 62 children and adolescents, of both genders, clinically diagnosed with asthma and under treatment at a pulmonology clinic. Ages ranged from 5 and 16 years. All spirometric tests were carried out according to the guidelines established by the American Thoracic Society and by the Associacão Brasileira de Normas Técnicas (Brazilian Technical Standards Association). For each patient, microspirometry was performed first, followed by conventional spirometry. The parameters analyzed in both devices were forced vital capacity, forced expiratory volume in one second and peak expiratory flow. Data were analyzed using the Student's t-test and Pearson's correlation test. RESULTS: Strong correlations were found between the two devices in the parameters analyzed: forced expiratory volume in one second: r = 0.97; forced vital capacity: r = 0.97; and peak expiratory flow: r = 0.91. CONCLUSION: These results demonstrate that the microspirometer is a useful diagnostic tool that can be used when a conventional spirometer is unavailable.

Introdução: Pacientes submetidas a tratamento cirúrgico para câncer de mama poderão apresentar prejuízos físicos, entre eles: alterações respiratórias, que podem acarretar em futuras complicações, e diminuição da qualidade de vida. Objetivo: Avaliar a função pulmonar e a força muscular respiratória pré e pós-operatórias em pacientes submetidas à cirurgia oncologia de mama. Método: Participaram do estudo 20 mulheres, com diagnóstico de câncer de mama, submetidas à cirurgia conservadora (quadrantectomia) ou a mastectomia, que foram avaliadas pela manovacuometria e espirometria nos pré e pós-operatórios. Resultados: Constatou-se que, no pré-operatório, a força muscular respiratória e a função pulmonar apresentaram-se diminuídas em relação aos valores previstos para cada paciente, estando a força muscular respiratória: pressão inspiratória máxima (43,14%) e pressão expiratória máxima (40,09%); e a função pulmonar: pico de fluxo expiratório (49,86%) e volume expiratório forçado no primeiro segundo (99,14%), apenas a capacidade vital forçada (125%) estava acima do previsto.
