Compared to the other groups, LTOT patients had more severe airway obstruction and lung hyperinflation, greater number and severity of comorbidities, shorter 6MWD, as well as lower mean SpO2 during 6MWD and worse quality of life. LTOT patients had a lower daily energy expenditure, shorter time spent > 3.0 METs and longer sedentary time compared to the COPD group, and less daily steps compared to the other groups. No significant difference in any parameter of PA was found between COPD and HYPOX. In LTOT patients, daily steps showed a strong correlation with 6MWD, and a moderate correlation with airway obstruction, level of oxygenation, comorbidities and quality of life but not with peripheral and respiratory muscle strength. In COPD and HYPOX patients, daily steps were strongly correlated with 6MWD and level of oxygenation as assessed by PaO2/FiO2. There was no significant correlation between mean SpO2 and 6MWD in any group.Conclusion: COPD patients on LTOT perform less physical activity than patients not needing LTOT, both with and without exercise-induced desaturations. The estrogen blockade plays a leading role in the therapeutic management of hormone receptor-positive breast cancer together with surgery, radiotherapy, and selective antiestrogen treatments. For several years, selective estrogen receptor modulators, such as tamoxifen, have represented the mainstay of therapy. The role of amenorrhea has been extensively elucidated in the past year: the benefit observed with chemotherapy-induced amenorrhea has strengthened its therapeutic role. Luteinizing hormone-releasing hormone (LHRH) has been introduced in oncology practice to induce amenorrhea in order to increase the advantage obtained from endocrine treatment. Triptorelin is one of the most widely used LHRH analogs currently available in clinical practice. It was recently investigated in two major clinical trials that studied the role of complete estrogen blockade in the premenopausal setting. Both showed the clinical benefit due to ovarian suppression treatment, primarily in high-risk patients. Furthermore, triptorelin and other LHRH analogs have recently been investigated in the attempt to preserve the ovarian function in young patients. The medical treatment of early breast cancer is always evolving in the effort to search for safe and efficacious treatments. The role of LHRH analogs is actually well recognized as contributing to the improvement of the medical treatment of premenopausal women with early breast cancer. 10. The corticomuscular response to experimental pain via blood flow occlusion when applied to the ipsilateral and contralateral leg during an isometric force task

Abstract

Blood flow occlusion (BFO) has been previously used to investigate physiological responses to muscle ischemia, showing increased perceptual effort (RPE) and pain along with impaired neuromuscular performance. However, at present, it is unclear how BFO alters corticomuscular activities when either applied to the exercising or nonexercising musculature. The present study therefore set out to assess the corticomuscular response to these distinct BFO paradigms during an isometric contraction precision task. In a repeated measures design, fifteen participants (age = 27.00 ± 5.77) completed 15 isometric contractions across three experimental conditions; no occlusion (CNTRL), occlusion of the contralateral (i.e., nonexercising) limb (CON-OCC), and occlusion of the ipsilateral (i.e., exercising) limb (IPS-OCC). Measures of force, electroencephalographic (EEG), and electromyographic (EMG) were recorded during contractions. We observed that IPS-OCC broadly impaired force steadiness, elevated EMG of the vastus lateralis, and heightened RPE and pain. IPSI-OCC also significantly decreased corticomuscular coherence during the early phase of contraction and decreased EEG alpha activity across the sensorimotor and temporoparietal regions during the middle and late phases of contraction compared with CNTRL. By contrast, CON-OCC increased perceived levels of pain (but not RPE) and decreased EEG alpha activity across the prefrontal cortex during the middle and late phases of contraction, with no changes observed for EMG and force steadiness. Together, these findings highlight distinctive psychophysiological responses to experimental pain via BFO showing altered cortical activities (CON-OCC) and altered cortical, corticomuscular, and neuromuscular activities (IPS-OCC) when applied to the lower limbs during an isometric force precision task.

Blood flow occlusion is a common experimental model to induce pain and fatigue during exercise. However, the corticomuscular mechanisms underlying this model remain poorly understood. In this study, we show that blood flow occlusion of non‐exercising musculature elicits a suppression of electromyographic (EEG) alpha activity across the prefrontal cortex. By contrast, occlusion of exercising musculature elicits a suppression of EEG alpha activity across central and posterior cortical regions and impairs both brain‐muscle communication and neuromuscular activation. Abstract

Background and Aims: A few case reports of autoimmune hepatitis–like liver injury have been reported after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination. We evaluated clinical features, treatment response and outcomes of liver injury following SARS-CoV-2 vaccination in a large case series. Approach and Results: We collected data from cases in 18 countries. The type of liver injury was assessed with the R-value. The study population was categorized according to features of immune-mediated hepatitis (positive autoantibodies and elevated immunoglobulin G levels) and corticosteroid therapy for the liver injury. We identified 87 patients (63%, female), median age 48 (range: 18–79) years at presentation. Liver injury was diagnosed a median 15 (range: 3–65) days after vaccination. Fifty-one cases (59%) were attributed to the Pfizer-BioNTech (BNT162b2) vaccine, 20 (23%) cases to the Oxford-AstraZeneca (ChAdOX1 nCoV-19) vaccine and 16 (18%) cases to the Moderna (mRNA-1273) vaccine. The liver injury was predominantly hepatocellular (84%) and 57% of patients showed features of immune-mediated hepatitis. Corticosteroids were given to 46 (53%) patients, more often for grade 3–4 liver injury than for grade 1–2 liver injury (88.9% vs. 43.5%, p = 0.001) and more often for patients with than without immune-mediated hepatitis (71.1% vs. 38.2%, p = 0.003). All patients showed resolution of liver injury except for one man (1.1%) who developed liver failure and underwent liver transplantation. Steroid therapy was withdrawn during the observation period in 12 (26%) patients after complete biochemical resolution. None had a relapse during follow-up. Conclusions: SARS-CoV-2 vaccination can be associated with liver injury. Corticosteroid therapy may be beneficial in those with immune-mediated features or severe hepatitis. Outcome was generally favorable, but vaccine-associated liver injury led to fulminant liver failure in one patient.

17. Fasting-Mimicking-Diet does not reduce skeletal muscle function in healthy young adults: a randomized control trial

Abstract

Purpose: The aim of this study was to evaluate the short- and long-term effects of the Fasting-Mimicking-Diet (FMD) intervention on neuromuscular parameters of force production in healthy young men. Methods: Twenty-four physically active men completed the study. Participants were randomly assigned to Fasting-Mimicking (FMD) or Normal Diet (ND) and asked to follow three cycles of dietary intervention. Neuromuscular parameters of force production during maximal voluntary isometric contractions (MVCs) with the leg extensors muscles and anthropometrics were measured at baseline (T0), at the end of the first cycle (T1), and 7–10 days after the 3rd cycle of the nutritional intervention (T2). The study was registered on Clinicaltrials.gov (No. NCT04476615). Results: There was a significant decrease in body mass at T1 for FMD (− 2.6 kg, ∆ from baseline, on average; p < 0.05) but not in ND (− 0.1 kg;). Neuromuscular parameters of force production, muscle volume, and MVC torque did not change or differ between groups across visits. Results were similar even when parameters were normalized by muscle volume. Conclusion: The consumption of FMD in a group of young healthy male subjects showed to be feasible, and it did not affect neuromuscular parameters of force production. The results suggest that FMD could be safely adopted by strength athletes without detrimental effects on force and muscle volume. Further research in clinical population at risk of muscle mass loss, such as elderly and obese subjects with sarcopenia, is warranted.

18. Effects of exercise induced muscle damage on cardiovascular responses to isometric muscle contractions and post-exercise circulatory occlusion

Abstract

Purpose: The aim of the present study was to investigate whether exercise-induced muscle damage (EIMD) influences cardiovascular responses to isometric exercise and post-exercise circulatory occlusion (PECO). We hypothesized that EIMD would increase muscle afferent sensitivity and, accordingly, increase blood pressure responses to exercise and PECO. Methods: Eleven male and nine female participants performed unilateral isometric knee extension at 30% of maximal voluntary contraction (MVC) for 3-min. A thigh cuff was rapidly inflated to 250 mmHg for two min PECO, followed by 3 min recovery. Heart rate and blood pressure were monitored beat-by-beat, with stroke volume and cardiac output estimated from the Modelflow algorithm. Measurements were taken before and 48 h after completing eccentric knee-extension contractions to induce muscle damage (EIMD). Results: EIMD caused 21% decrease in MVC (baseline: 634.6 ± 229.3 N, 48 h: 504.0 ± 160 N), and a 17-fold increase in perceived soreness using a visual-analogue scale (0–100 mm; VASSQ) (both p < 0.001). CV responses to exercise and PECO were not different between pre and post EIMD. However, mean arterial pressure (MAP) was higher during the recovery phase after EIMD (p < 0.05). Significant associations were found between increases in MAP during exercise and VASSQ, Rate of Perceived Exertion (RPE) and Pain after EIMD only (all p < 0.05). Conclusion: The MAP correlations with muscle soreness, RPE and Pain during contractions of damaged muscles suggests that higher afferent activity was associated with higher MAP responses to exercise.

Abstract

BACKGROUND: Recent population-based female breast cancer and prostate cancer polygenic risk scores (PRS) have been developed. We assessed the associations of these PRS with breast and prostate cancer risks for male BRCA1 and BRCA2 pathogenic variant carriers. METHODS: 483 BRCA1 and 1318 BRCA2 European ancestry male carriers were available from the Consortium of Investigators of Modifiers of BRCA1/2 (CIMBA). A 147-single nucleotide polymorphism (SNP) prostate cancer PRS (PRSPC) and a 313-SNP breast cancer PRS were evaluated. There were 3 versions of the breast cancer PRS, optimized to predict overall (PRSBC), estrogen receptor (ER)-negative (PRSER-), or ER-positive (PRSER+) breast cancer risk. RESULTS: PRSER+ yielded the strongest association with breast cancer risk. The odds ratios (ORs) per PRSER+ standard deviation estimates were 1.40 (95% confidence interval [CI] =1.07 to 1.83) for BRCA1 and 1.33 (95% CI = 1.16 to 1.52) for BRCA2 carriers. PRSPC was associated with prostate cancer risk for BRCA1 (OR = 1.73, 95% CI = 1.28 to 2.33) and BRCA2 (OR = 1.60, 95% CI = 1.34 to 1.91) carriers. The estimated breast cancer odds ratios were larger after adjusting for female relative breast cancer family history. By age 85 years, for BRCA2 carriers, the breast cancer risk varied from 7.7% to 18.4% and prostate cancer risk from 34.1% to 87.6% between the 5th and 95th percentiles of the PRS distributions. CONCLUSIONS: Population-based prostate and female breast cancer PRS are associated with a wide range of absolute breast and prostate cancer risks for male BRCA1 and BRCA2 carriers. These findings warrant further investigation aimed at providing personalized cancer risks for male carriers and informing clinical management.

Abstract

Knowledge is growing on the safety of assisted reproductive techniques (ART) in cancer survivors. No data exist, however, for the specific population of breast cancer patients harboring germline BRCA1/2 pathogenic variants. This is a multicenter retrospective cohort study across 30 centers worldwide including women diagnosed at ≤40 years with stage I-III breast cancer, between January 2000 and December 2012, harboring known germline BRCA1/2 pathogenic variants. Patients included in this analysis had a post-treatment pregnancy either achieved through use of ART (ART group) or naturally (non-ART group). ART procedures included ovulation induction, ovarian stimulation for in vitro fertilization or intracytoplasmic sperm injection, and embryo transfer under hormonal replacement therapy. Among the 1424 patients registered in the study, 168 were eligible for inclusion in the present analysis, of whom 22 were in the ART group and 146 in the non-ART group. Survivors in the ART group conceived at an older age compared with those in the non-ART group (median age: 39.7 versus 35.4 years, respectively). Women in the ART group experienced more delivery complications compared with those in the non-ART group (22.1% versus 4.1%, respectively). No other apparent differences in obstetrical outcomes were observed between cohorts. The median follow-up from pregnancy was 3.4 years (range: 0.8-8.6 years) in the ART group and 5.0 years (range: 0.8-17.6 years) in the non-ART group. Two patients (9.1%) in the ART group experienced a disease-free survival event (specifically, a locoregional recurrence) compared with 40 patients (27.4%) in the non-ART group. In the ART group, no patients deceased compared with 10 patients (6.9%) in the non-ART group. Abstract

Resumen El cáncer gástrico es una patología de alta incidencia en Chile. Afortunadamente es baja la incidencia de márgenes R1 cuando se realiza una cirugía con intención curativa. Al tener un tumor subcardial o cardial es perentorio realizar una biopsia rápida (intraoperatoria), en caso que ésta resulte positiva, es difícil tomar una decisión sobre qué realizar a continuación, ya que el realizar una esofagoyeyunoanastomosis intratorácica aumenta el riesgo quirúrgico. Para esto, es de suma importancia tener la información de las características histopatológicas del tumor, el riesgo de filtración, la recurrencia, la sobrevida, las terapias adyuvantes y la realidad país. Se realizó una revisión de la literatura con el fin de poder guiar la toma de decisiones. Gastric cancer is a pathology with a high incidence in Chile, fortunately, the incidence of R1 margins is low when performing surgery with curative intent. When having a sub cardial or cardial tumor, it is essential to perform a rapid (intraoperative) biopsy, if this is positive when cutting the esophagus just under the pillars, it is difficult to make the decision of what to do next, since performing an inthrathoracic esophagojejunostomy increases the surgical risk. For this, it is very important to have information about the histopathological characteristics of the tumor, the leakage risk, there currence, the survival, the adjuvant therapies and the reality of the country. A review of the literature was done in order to guide the decisions.

Abstract

Myofibroblastoma of the breast is a rare benign stromal tumor that occurs in both sexes with a higher prevalence in male breast of older populations. Furthermore, myofibroblastoma can arise in extra mammary sites, along the milk-line. A variety of morphological variants in addition to the classic type have been identified. The differential diagnosis includes both benign and malignant entities that, through the use of clinical and radiological imaging, is difficult to characterize. Histopathological examination and immunohistochemistry are fundamental in the establishment of appropriate management of the disease and avoidance of overtreatment. The present study focuses on two cases of male mammary myofibroblastoma, with a short literature review.

41. Hernias de pared abdominal complicadas operadas de urgencia mediante una hernioplastía con malla. Factores de riesgo de morbilidad postoperatoria

Abstract

Introduction:Currently, there are no known risk factors for postoperative complications and mortality in patients who are undergoing surgery for a complicated abdominal wall hernia operated emergency through a hernioplasty with mesh. The main objective is to determine the risk factors for presenting a complication during the postoperative period in patients undergoing emergency with a diagnosis of complicated abdominal wall hernia by a hernioplasty with mesh in the Base HospitalOsorno (HBO) between the years 2002 and 2008.Material and methods:retrospective cohort study. We selected patients older than 14 years operated by hernial pathology of the registration of surgeries of the Emergency Service at the Hospital Base Osorno (HBO) between the years 2002 and 2008. We excluded those operated without mesh. It was felt the risk, using odds ratio (OR) and its confidence interval of 95%. Software used: Stata 10.0.Results:The cohort included 52 patients, median age 64 years (26-85). The 50% (26 patients) were male. The most frequent location hernia was the groin with 23 cases (44.23%). 49 (94.23%) hernias were catalogd as stuck and 3 (5.77%) as being strangled. Operative morbidity 17.31% (9 patients). Median follow-up, 32 (5-90%) months. 42. Factores pronósticos en el cáncer vesicular subseroso

Abstract

Introduction:It is important to identify the prognostic factors of the gallbladder cancer dye to be able to raise different therapies within this group tumor.Objective:To evaluate prognostic factors that affect the survival of patients with subserous gallbladder cancer.Material and methods:retrospective cohort study. We analyze the patients with histopathological diagnosis of subserous gallbladder cancer (T2) from the biopsy of the surgical specimen, operated in the Regional Hospital Valdivia (HRV) between the years 2001-2005. To characterize the population descriptive statistical analysis was carried out. Overall survival was described by Kaplan Meier curves. To measure the prognostic factors ( reoperation, adjuvant therapy (chemo), degree of differentiation and commitment linfonodal), bivariate analysis was performed using the Long rank test and multivariate analysis bymeans of a Cox's regression.Results:The cohort included 18 patients. Average age 65 (39-86) years. A predominance of females, 12 (66.66%). The preoperative diagnosis "vesicular likely cancer", was found in 5 patients (27.77%), acute cholecystitis in 8 (44.44%), symptomatic cholelithiasis in 3 (16.66%). Eleven patients (61.11%) were classified as N1 and 7 (38.88%) as N0. Five tumors (27.77%) were classified as poorly differentiated, 9 (50.00%) as moderately and 3 (16.66%) as well differentiated. Four patients surgery (22.22%). The survival at 5 years was 81.71% and 18.18% for the N0 and N1 respectively. Both the bivariate analysis (p=0.0049) as the multivariate (HR:18.34; 95% CI 1.18 -283, p=0,037) showed that the linfonodal involvement is a poor prognostic factor.Conclusion:The linfonodal involvement would be a prognostic factor in cancer subserous vesicular., Introducción:Es importante identificar los factores pronósticos del cáncer vesicular subseroso para poder plantear distintas terapias dentro de este grupo tumoral.Objetivo:Evaluar qué factores pronósticos afectan la sobrevida en pacientes con cáncer vesicular subseroso.Material y métodos:Cohorte retrospectiva. Se analizan los pacientes con diagnóstico histopatológico de cáncer vesicular subseroso (T2), a partir de la biopsia de la pieza quirúrgica, operados en el Hospital Regional Valdivia (HRV) entre los años 2001-2005. Para caracterizar a la población se realizó estadística descriptiva. La sobrevida global se describió mediante curvas de Kaplan Meier. Para medir los factores pronósticos (reoperación, terapia adyuvante (quimioradioterapia), grado de diferenciación y el compromiso linfonodal), se realizó análisis bivariado mediante la prueba de Long Rank y análisis multivariado por medio de una regresión de Cox.Resultados:La cohorte incluyó 18 pacientes. Edad promedio 65 (39-86) años. Predominó el sexo femenino, 12 (66,66%). El diagnóstico preoperatorio "probable cáncer vesicular", se encontró en 5 pacientes (27,77%), colecistitis aguda en 8 (44,44%), colelitiasis sintomática en 3 (16,66%). Once pacientes (61,11%) fueron catalogados como N1 y 7 (38,88%) como N0. Cinco tumores (27,77%) se clasificaron como mal diferenciados, 9 (50,00%) como moderadamente y 3 (16,66%) como bien diferenciados. Cuatro pacientes se reoperaron (22,22%). La sobrevida a 5 años fue 81,71% y 18,18% para los N0 y N1 respectivamente. Tanto el análisis bivariado (p=0.0049) como el multivariado (HR:18,34; IC 95%; 1,18-283, p=0.037) arrojó que el compromiso linfonodal es un factor de mal pronóstico.Conclusión:El compromiso linfonodal sería un factor pronóstico en el cáncer vesicular subseroso.

El Pectus Excavatum o torax de zapatero es la malformacion mas frecuente de la cara anterior del torax .Desde que el Profesor de Cirugia Sauerbruch en 1920 intervino por primera vez un paciente adulto-joven, la tecnica quirurgica abierta con traccion postoperatoria se continuo aplicando; hasta que; Ravitch en 1947 publica su experiencia eliminando la traccion toraxica post operatoria.
