Abstract

In September 2020, we tested 13,398 persons in Portugal for antibodies against severe acute respiratory syndrome coronavirus 2 by using a quota sample stratified by age and population density. We found a seroprevalence of 2.2%, 3–4 times larger than the official number of cases at the end of the first wave of the pandemic.

Abstract

Abstract Background Rubella infection can affect several organs and cause birth defects that are responsible for congenital rubella syndrome (CRS). Congenital hearing loss is the most common symptom of this syndrome, occurring in approximately 60% of CRS cases. Worldwide, over 100 000 babies are born with CRS every year. There is no specific treatment for rubella, but the disease is preventable by vaccination. Since 1969, the rubella vaccine has been implemented in many countries, but in Africa, only a few countries routinely immunize against rubella. The aim of this study was to estimate the rate of infection from the wild-type rubella virus in São Tomé and Príncipe by determining rubella seroprevalence with a DBS method. The goal of this study was to reinforce the need for implementation of the rubella vaccine in this country. As secondary objectives, the validation of a DBS method was first attempted and an association between seroprevalence and hearing loss was assessed. Methods We collected samples from individuals observed during humanitarian missions in São Tomé and Príncipe. All individuals underwent an audiometric evaluation, and a drop of blood was collected for the dried blood spot (DBS). We define two groups: the case group (individuals with unilateral or bilateral hearing loss (HL)) and the control group (individuals with two normal ears). Patients were excluded if they suffered from conductive HL, if they showed evidence of possible causes of HL, if they had developmental delay or if they refused to participate in the study. Results Among the 315 subjects, we found 64.1% individuals with IgG for the rubella virus, 32.1% without immunity for the rubella virus and 3.8% who were borderline. In the control group, 62.6% were positive for the rubella IgG, whereas in the case group, 72% were positive. Analyzing both groups, with ages ranging from 2 to 14 years of age and from 15 to 35 years of age, we found a seroprevalence of 50.3% to rubella in the younger group and 82.1% in the older group, with a significant difference between cases and control group noted within the younger patients (p = 0.025). Conclusions Rubella is a disease that can be prevented. Rubella infections are still very common in São Tomé and Príncipe, and women of child-bearing age are still at risk for rubella infection during pregnancy, justifying the urgency of vaccination against rubella. A statistically significant association between the group of children under 14 years of age with HL and immunity for rubella was observed in this country, although this study did not allow us to establish a cause-effect relationship between rubella infection and SNHL.

Abstract

To evaluate the bond strength of different universal adhesives on deciduous tooth dentineand their relationship with the composition and potential of hydrogen (pH). Methods: An in vitromicro push-out test on 150 samples (n = 50) per group per adhesive, namely, Adhese Universal(ADH; Ivoclar Vivadent), Futurabond U (FUT; Voco GmbH) (Test), and Scotchbond Universal withpre-conditioning (SCO; 3M) (Control), to record bond strength (BS) and type of adhesive failure.Results: The results of the different adhesives (megapascals (MPa)) varied, showing no statisticalsignificance. The corresponding averages are in MPa: ADH, 13.66 2.81; FUT, 14.48 2.88; SCO,14.98 3.96. Additionally, the frequency of type of failure was as follows: mixed (60.7%), adhesive(27.3%), and cohesive (12%). Conclusions: SCO, with a pH of 2.7, showed greater resistance tofracture, while FUT, with a pH of 2.3 and no pre-conditioning, approached the same values, being aone-step adhesive. Abstract

Introdução A osteoartrite (OA) é uma das dez doenças mais incapacitantes nos países desenvolvidos e uma das principais causas de dor e incapacitação no mundo. O diagnóstico precoce aumenta a probabilidade de prevenção da progressão da doença. Objetivos Estimar a prevalência de osteoartrite auto-referida e a qualidade de vida em adultos portugueses com 45 ou mais anos de idade. Métodos Estudo observacional, transversal, implementado em domicílios por entrevista interpessoal. Resultados Foram incluídos no estudo 1039 indivíduos com idade média de 62 anos, sendo 54,2% do gênero feminino. A prevalência de osteoartrite auto-referida foi de 9,9%. Os joelhos e as mãos foram o local mais freqüente da doença. A prevalência de OA foi maior em mulheres e em participantes sem atividade profissional. A presença de OA foi maior em participantes com comorbidades. A maioria dos indivíduos já tinham passado por algum tratamento em alguma ocasião de suas vidas para esta doença: 94,5% tiveram tratamento farmacológico, 49,5% fisioterapia, e 19,8% atividade física. A dor estava associada com a estatura, com alguns locais da doença, especificamente pescoço, coluna lombar e ombros, pontuação do SF12 para qualidade de vida, e medidas de impacto no cotidiano dos participantes, gravidade da doença e incapacitação. O impacto da OA no dia-a-dia foi maior em indivíduos que tinham gozado licença por doença ou que pararam de trabalhar por causa da OA, apresentavam-se com pior saúde física e mental, e exibiam maior gravidade da doença. Conclusão Este estudo confirmou que a osteoartrite é uma doença muito relevante, com impacto potencial elevado na qualidade de vida, no funcionamento e na capacidade para o trabalho e, por causa de sua prevalência, exerce um impacto social muito elevado e crescente.

22. Prediction and failure analysis of composite resin restorations in the posterior sector applied in teaching dental students

Abstract

The failure of composite resin restorations in the posterior region is an ongoing concern in current clinical practice This study assesses possible factors and causes of the failure of restoration 1 year after their placement by fourth year dental students (on a 5-year degree course). While the systematic assessment of dental students does not appear to have received much attention in the field, this study asserts the need and benefit of an assessment methodology that can (1) predict the success or failure of restorations placed by dental students and (2) assist the clinical instructor in identifying the performance profile of each student. Eighty-one patients aged 26–77 years were treated by 81 undergraduates in a prospective cohort study from November 2013 to December 2015. One year after treatment, restorations were assessed by the same staff member who acted as the supervisor during the restoration placement. A CBR system was applied to make predictions about restorations. The CBR includes different machine learning techniques and statistical tests in the CBR cycle. The system calculates the relevant variables, which are used to predict failures. The accuracy of the system is measured with the AUC and the accuracy. The AUC obtained is 0.935 while the Kappa index and the accuracy are 91.36 and 0.75, respectively. In conclusion, factors related to the patient and to the treatment are associated to the failure of the restorative treatment. Of particular interest, the CBR was useful for the performance of a predictive model to estimate the probability of failure of resin restorations placed by students.

Abstract

Objectives: After consultations, the physician’s perceptions differ from the patient’s perceptions concerning illness level, cause, and nature of the problem and content of the consultation. Agreement on problems requiring follow-up was associated with a better outcome. The primary aim of this study was to build and validate an instrument that could assess physician-patient agreement in the rheumatology consultation. The secondary objective was to assess agreement association with patient’s clinical and sociodemographic data. Materials and Methods: A ten-item questionnaire - “Consultation Assessment Instrument” (CAI) - was developed for this study to assess the physician-patient agreement. Ten physicians and 102 patients diagnosed with an inflammatory joint disease under biological therapy were included. The items were evaluated and the index of proportional agreement for the dichotomized answers “agree” (Ppos) and “disagree” (Pneg) was calculated. Results: Consultation satisfaction was the item with the highest agreement. On the opposite end, the item about the explanation of treatment importance was the item with the lowest agreement between patient and physician. Except for one item, the high level of agreement between patient and physician was due to a higher Ppos. Index of proportional agreement was high for 9 of the 10 items (0.816≤ Iv ≤0.990). Patients with lower disease activity scores had a more positive experience. A good internal consistency was obtained for both patient’s and physician’s questionnaire (α = 0.88 and α = 0.80, respectively). Conclusions: Both patient and physician showed a positive experience towards Rheumatology consultation. Physician-patient agreement was high in the majority of the consultation aspects (mean Iv = 0,93). A good internal consistency was obtained for both patient’s and physician’s questionnaire. CAI could be useful as a mental checklist in daily practice or as an educational tool for training consultation skills.

The system was applied to genetic sequencing data to extract relevant information about insertions, deletions or polymorphisms.

We offer in this paper an example of the possibilities of Cryptography, as a branch of Applied Mathematics, to cover pleasantly and fruitfully many essential questions of Education. And when we say Education we want to highlight its broader and moreextensive semantics, so necessary and so longed for many times today, without losing the necessary precision of especialization, grouping in this study issues related to areas such as Mathematics, Communications Security, Literature, Linguistics, or Classical Languages. For this purpose, we have relied on a famous literary work by the great French writer JulesG. Verne, “Journey to the Center of the Earth”. Based solely on the enigmatic message that appears at the beginning of the novel we offer the solution in detail and by cryptanalytic means, showing step by step the various approaches to generally resolve an unknown text. Finally, this example helps us to detail some fundamental elements of Algebra andCommunication Theory that are typical of Cryptology., Ofrecemos en este artículo un ejemplo de las posibilidades de la Criptografía, como rama de la Matemática Aplicada, para abarcar de manera grata y fructífera numerosas cuestiones imprescindibles de la Educación. Y cuando decimos Educación queremos resaltar su semántica más amplia y extensa, tan necesaria y tan añorada muchas veces hoy en día, sin perder la necesaria precisión de la especialización, agrupando en este estudio cuestiones relativas a áreas como la Matemática, la Seguridad de las Comunicaciones, la Literatura, la Lingüística, o las Lenguas Clásicas. Con el reciente crecimiento del sector de las tecnologías Blockchain y la monumental inversión de empresas como Facebook y Microsoft en la creación del metaverso, lo que hace unos años parecía ciencia ficción está comenzando a materializarse. El objetivo de este trabajo es esclarecer el concepto de metaverso, estudiar las tecnologías necesarias para su implementación y entender por qué la tecnología Blockchain es una pieza fundamental. Se analizará la terminología relacionada con los mundos virtuales y las tecnologías de realidad extendida para aclarar así la difusa línea entre lo material y lo virtual. Se presentarán los proyectos de metaverso más prometedores y sus características diferenciadoras. Finalmente, se hará una demostración del código necesario para desplegar un “smart contract” que facilite la venta de un token ERC20.---ABSTRACT---With the recent growth of all Blockchain-related technologies and the huge investments that companies such as Facebook and Microsoft are making to become part of the metaverse, what seemed like science fiction a few years ago is starting to materialize. The aim of this paper is to clarify the concept of the metaverse, explore the technologies required for its implementation and understand why Blockchain is a fundamental part of creating the metaverse. First, we will analyze the terminology related to virtual worlds and extended reality technologies to clarify the blurred line between the material and the virtual. Then, we will present the most promising metaverse projects and their distinctive features. Finally, we will demonstrate the code necessary to deploy a smart contract that facilitates the sale of an ERC20 token.

Abstract

Esta investigación buscó Determinar si el programa liderazgo pedagógico mejora el desempeño docente del centro escolar “Alonso de Illescas” Guayaquil, Ecuador, 2020. Se desarrolló dentro de la metodología cuantitativa y siguió el tipo de diseño preexperimental. Se aplicó un cuestionario valido y confiable tanto en el pretest como en el postest, en una muestra de 30 docentes. Los resultados obtenidos revelaron que predominó el nivel alto en el pretest con el 83.33%, mientras que en el postest hubo un incremento al 96.67%. Además, los resultados inferenciales mostraron una diferencia significativa de 11.833 (Sig.= 0.000 < 0.05) entre las medias del pretest y postest del grupo experimental, lo que permitió aceptar la hipótesis de investigación Hi y se rechazó la hipótesis nula H0. Concluyendo que la aplicación del programa liderazgo pedagógico mejora el desempeño docente del centro escolar “Alonso de Illescas” Guayaquil, Ecuador, 2020. Abstract

To evaluate the bond strength of different universal adhesives on deciduous tooth dentineand their relationship with the composition and potential of hydrogen (pH). Methods: An in vitromicro push-out test on 150 samples (n = 50) per group per adhesive, namely, Adhese Universal(ADH; Ivoclar Vivadent), Futurabond U (FUT; Voco GmbH) (Test), and Scotchbond Universal withpre-conditioning (SCO; 3M) (Control), to record bond strength (BS) and type of adhesive failure.Results: The results of the different adhesives (megapascals (MPa)) varied, showing no statisticalsignificance. The corresponding averages are in MPa: ADH, 13.66 2.81; FUT, 14.48 2.88; SCO,14.98 3.96. Additionally, the frequency of type of failure was as follows: mixed (60.7%), adhesive(27.3%), and cohesive (12%). Conclusions: SCO, with a pH of 2.7, showed greater resistance tofracture, while FUT, with a pH of 2.3 and no pre-conditioning, approached the same values, being aone-step adhesive. 38. Some Notes on a Formal Algebraic Structure of Cryptology

Abstract

Cryptology, since its advent as an art, art of secret writing, has slowly evolved and changed, above all since the middle of the last century. It has gone on to obtain a more solid rank as an applied mathematical science. We want to propose some annotations in this regard in this paper. To do this, and after reviewing the broad spectrum of methods and systems throughout history, and from the traditional classification, we offer a reordering in a more compact and complete way by placing the cryptographic diversity from the algebraic binary relations. This foundation of cryptological operations from the principles of algebra is enriched by adding what we call pre-cryptological operations which we show as a necessary complement to the entire structure of cryptology. From this framework, we believe that it is improved the diversity of questions related to the meaning, the fundamentals, the statute itself, and the possibilities of cryptological science.

41. Prevención de úlceras por presión en pacientes geriátricos.

Abstract

Pressure ulcers (UPP), are part of the biggest geriatric syndromes, they're a problem of everyday medical practice, this affect to all population, although they occur more frequently, in elderly with different disabilities, or terminal stage. The aim is to determine the causes of pressure ulcers in their different forms of appearance in geriatric patients, in order to improve their quality of life. The methodology used is qualitative, descriptive, and it allowed to know how this problem manifests in terms of frequency, characteristics, conditions, incidence and prevalence. Theoretical and empirical methods were applied such as documentary analysis, surveys and interviews to verify the insufficiencies in the management and care of the lesions due to this disease, it has a predominance in patients ranging from 61 to 70 years old, with a percentage of 41.3% in female users, in a percentage of 63.6%, these being the cases of users who have suffered trauma, fractures, are bedridden in 27.5% or saddled in a 26.5% %. The way to prevent pressure ulcers is of vital importance since it will be possible to demonstrate the causes, different stages, evolution of the disease; provide education to keepers, develop a continuity of social and health services that are possible, accessible, of high quality and respectful with age, and the women and men consider their need according as the years go by. Abstract

Curs acadèmic: 2018/2019 Metaphors in antiquity were considered an element of decoration in poetry and rhetorical discourse, but in recent decades metaphors have become an element more closer to thought than language. Metaphor of thought or conceptual metaphor is a figurative comparison in which an idea (or conceptual domain) is understood in terms of another conceptual domain (Pascual & Silvestre-López, 2015). The main objective of the present study is to show how conceptual metaphors function, how they are classified, and how our previous social and cultural experiences are an important factor in their understanding. To achieve our goal, we focus our attention to the publication of the book Metaphors We Live By (Lakoff & Johnson, 1980), which is considered the starting point of the investigation of conceptual metaphors. In addition, following the research base of verbal metaphors, we will also focus our attention on visual metaphors, taking the study of visual metaphors in the billboards as a reference (Forceville, 1996), which have had a growing interest since the 1990s . Therefore, we also deal with the classification of visual metaphors according to Forceville (2007), and how social and cultural factors help us to interpret their meaning (Bulmer & Buchanan-Oliver, 2004) . In my opinion, it seems quite obvious that both conceptual metaphors and visual metaphors can not be understood without being a part of thought, because in the emergence of these metaphors there is a close relationship with social and cultural factors, beliefs and previous experiences that help us to understand the meaning of these metaphors. However, although the world today is quite globalized, there are small differences that can lead to a misunderstanding. On the other hand, I believe that visual metaphors are replacing verbal forms in ads because we live in a stressful way with no time to stop and read ads, and using visual metaphors allows us to capture the message of the visual metaphor at a simple glance.
