The Southern Ocean modulates global biogeochemical (BGC) cycles substantially, affecting biological production and the global air-sea balance of carbon dioxide and interior dissolved oxygen content. Concurrently, the Southern Ocean is rich in highly dynamic mesoscale eddies. These eddies have the potential to alter local carbon, nutrient, and oxygen distributions through eddy pumping, stirring, and trapping. Additionally, the strong westerly winds could result in significant eddy-induced Ekman pumping counteracting the eddy pumping effects. However, the impact of mesoscale eddies on upper-ocean Southern Ocean biogeochemistry has not been quantified observationally at a regional scale.We now have nearly a decade of BGC observations from Argo floats deployed as part of the Southern Ocean Carbon and Climate Observations and Modeling project (SOCCOM). In addition, the Mesoscale Eddy Trajectory Atlas, version 3.2, delayed time (Meta3.2DT) database provides us with a robust assessment of eddies as detected by satellite altimeter measurements. Together, the two datasets allow us to investigate the three-dimensional structure of the biogeochemistry in Southern Ocean eddies. Here, we co-locate Southern Ocean eddies with BGC Argo floats to characterize composite vertical and horizontal structures of dissolved inorganic carbon (DIC), oxygen, and nitrate inside anticyclonic and cyclonic eddies compared to the mean climatological fields. We conduct this analysis in several subregions with different dominant processes. We find positive DIC and nitrate anomalies in cyclonic eddies, which we attribute to upward eddy pumping. We also find positive oxygen anomalies near the surface, which we attribute to upwelled nutrients that enhance biological production, leading to enhanced photosynthesis. At depth, we find negative oxygen anomalies in cyclonic eddies, which may be driven both by enhanced respiration due to increased biological production as well as the heaving of isopycnals via eddy pumping. The opposite is true for anticyclonic eddies due to downward eddy pumping (negative DIC and nitrate anomalies; negative oxygen anomalies near the surface and positive oxygen anomalies at depth). The magnitudes of the eddy imprints on biogeochemistry vary by region, indicating that stratification and other background signals influence the magnitude of the effect of eddies in a region. Our findings can help us to interpret the influence of mesoscale eddies on the Southern Ocean carbon fluxes and biogeochemistry, including assessing the relative dominance of eddy pumping and eddy-induced Ekman pumping in different subregions of the Southern Ocean.

Abstract: The Southern Ocean is a region of intense air–sea exchange that plays a critical role for ocean circulation, global carbon cycling, and climate. Subsurface chlorophyll‐a maxima, annually recurrent features throughout the Southern Ocean, may increase the energy flux to higher trophic levels and facilitate downward carbon export. It is important that model parameterizations appropriately represent the chlorophyll vertical structure in the Southern Ocean. Using BGC‐Argo chlorophyll profiles and the Biogeochemical Southern Ocean State Estimate (B‐SOSE), we investigate the sensitivity of chlorophyll vertical structure to model parameters. Based on the sensitivity analysis results, we estimate optimized parameters, which efficiently improve the model consistency with observations. We characterize chlorophyll vertical structure in terms of Empirical Orthogonal Functions and define metrics to compare model results and observations in a series of parameter perturbation experiments. We show that chlorophyll magnitudes are likely to respond quasi‐symmetrically to perturbations in the analyzed parameters, while depth and thickness of the subsurface chlorophyll maximum show an asymmetric response. Perturbing the phytoplankton growth tends to generate more symmetric responses than perturbations in the grazing rate. We identify parameters that affect chlorophyll magnitude, subsurface chlorophyll or both and discuss insights into the processes that determine chlorophyll vertical structure in B‐SOSE. We highlight turbulence, differences in phytoplankton traits, and grazing parameterizations as key areas for improvement in models of the Southern Ocean.

Here, we co-locate eddies, identified in the Meta3.2DT satellite altimeter-based product, with biogeochemical Argo floats to determine the effects of eddies on the dissolved inorganic carbon (DIC), nitrate, and dissolved oxygen concentrations in the upper 1,500 m of the ice-free Southern Ocean, as well as the eddy effects on the carbon fluxes in this region. DIC and nitrate concentrations are lower in anticyclonic eddies (AEs) and increased in cyclonic eddies (CEs), while dissolved oxygen anomalies switch signs above (CEs: positive, AEs: negative) and below the mixed layer (CEs: negative, AEs: positive). We attribute these anomalies primarily to eddy pumping (isopycnal heave), as well as eddy trapping for oxygen. Maximum anomalies in all tracers occur at greater depths in the subduction zone north of the Antarctic Circumpolar Current (ACC) compared to the upwelling region in the ACC, reflecting differences in background vertical structures. Eddy effects on air-sea CO2 exchange have significant seasonal variability, with additional outgassing in CEs in fall (physical process) and additional oceanic uptake in AEs and CEs in spring (biological and physical process). Integrated over the Southern Ocean, AEs contribute ∼0.03± 0.01 Pg C yr-1 (7 ±2% ) to the Southern Ocean carbon uptake, and CEs offset this by ∼0.01± 0.01 Pg C yr-1 (2 ±2% ). These findings underscore the importance of considering eddy impacts in observing networks and climate models.

This research explores how green ideologies are embedded in the policy agendas of ruling parties, including the three main political parties that won the 2020 local head elections in East Java Province, Indonesia. The research approach used is mixed methods, with quantitative and qualitative analysis to understand the awareness, discourse, and strategy of environmental issues in the policy agenda of the winning political parties. The results show that within political parties, sustainability considerations are largely reactive, oriented towards short-term goals, and mainly used for instrumental purposes. Despite the existing literature, parties that are more likely to survive electorally are in areas where voters are highly concerned about environmental issues and have different party strategies in terms of general policy. However, in East Java province, the increasing public concern for environmental issues has not been used as a strategic tool by political parties. These findings suggest that moving beyond superficial environmental initiatives towards strategic environmental integration can improve electoral competitiveness and promote sustainable development. The results suggest that by aligning their strategies with broader sustainability goals and adopting proactive measures for environmental management, regional political parties in East Java can not only respond to short-term electoral pressures; they can even begin to resolve some of the key sustainability challenges that underpin future electoral success and the quality of governance in their regions.

Recently, a national French study reported a very low rate of PR uptake (8.6%); however, important clinical data were missing. Here, we aimed to identify the main factors associated with insufficient PR uptake after hospitalisation for COPD exacerbation.Patients and Methods: This multicentre retrospective study included patients hospitalised with COPD exacerbation between 1 January 2017 and 31 December 2018, as identified by both coding and a detailed review of medical records. PR was defined as inpatient care in a specialised centre or unit within 90 days of discharge. Multivariate logistic regression was used to identify associations between PR uptake and patient characteristics, such as comorbidities, non-invasive ventilation (NIV), inhaled treatment, and forced expiratory volume in 1 second (FEV1).Results: Among the 325 patients admitted for severe COPD exacerbation, 92 (28.3%) underwent PR within 90 days of discharge. In univariate analysis, relative to those who underwent PR, patients without PR had significantly more comorbidities, were less often treated with triple bronchodilator therapy or NIV, and had a higher FEV1. In multivariate analysis, variables independently associated with the lack of PR uptake were the presence of comorbidities (adjusted odds ratio (aOR) = 1.28 [1.10– 1.53], p = 0.003) and a higher FEV1 (aOR = 1.04 [1.02– 1.06], p < 0.001). There was no significant correlation between PR uptake and departmental PR centre capacity (notably, some departments had no PR facilities).Conclusion: These data highlight the lack of PR in the early stages of COPD. Collaboration among all healthcare providers involved in patient management is crucial for improved PR uptake.Plain Language Summary: Pulmonary rehabilitation (PR) is multidisciplinary care strongly recommended after severe exacerbation of chronic obstructive pulmonary disease (COPD); however, referral remains very low in France. Stress is prone to occur in children with autism. According to the study, around 85% of children who have autism suffer from anxiety disorders that can exacerbate their condition, leading to self-harm and harm to those in their vicinity. Heart rate, skin conductance, and finger temperature changes occur during stress. In this paper, we design a system to monitor heart rate, body temperature, and skin conductance to detect signs of stress. Subsequently, the measurement data is processed using the fuzzy logic (FL) method as a decision-maker algorithm. In particular, we use 64 fuzzy rules with membership functions for each parameter. Parameter measurement results will be displayed using a widget called Gauge, while stress conditions will be displayed using a label widget. The results will be displayed on the Blynk application with an IoT system and viewed remotely via Android devices. The test was conducted on five children aged 5-9 years with varying body conditions. From the test results, the mean accuracy of the heart rate sensor was 95.01%, the mean temperature sensor accuracy was 97.7%, and the mean conductance sensor accuracy was 93.75%. The stress levels range from a minimum of 25% to a maximum of 75%. These findings indicate that the developed tool has performed effectively, and it is feasible to monitor its operation remotely.

Background Studies suggest that up to 70% of chronic obstructive pulmonary disease (COPD) cases globally are undiagnosed worldwide. Some of these undiagnosed patients may present with severe exacerbation and factors associated with underdiagnosis in this population are unknown. We investigated the key factors associated with underdiagnosis in two cohorts of patients hospitalised for severe COPD exacerbation at different time points.Methods This retrospective, multicentre study analysed data from patients hospitalised for COPD exacerbation at two independent centres during two distinct time periods: between 1 January 2017 and 31 December 2018 in the Aquitaine region and between 1 January and 31 December 2022 in Paris. Undiagnosed COPD was defined as the absence of pulmonary function testing before the index exacerbation. Multivariate logistic regression was used to evaluate associations between underdiagnosis and patient characteristics.Results Among the 424 patients, 93 (21.9%) were undiagnosed before hospitalisation, with a stable rate over time (22% in 2017–2018 and 21% in 2022). Multivariate analysis revealed that underdiagnosis was related to higher forced expiratory volume in one second (FEV1; adjusted OR (aOR)=1.02, p=0.043) and female sex (aOR=1.91, p=0.015). Patients with undiagnosed COPD had significantly lower rehospitalisation and mortality rates. After the initial severe exacerbation, higher mortality was associated with a higher Charlson Comorbidity Index (HR=1.24, p=0.007) and older age (HR=1.05, p=0.008).Conclusion This retrospective, multicentre study demonstrated that about 20% of patients admitted with severe exacerbation were undiagnosed for COPD. Higher FEV1 and female sex were associated with underdiagnosis, emphasising the need for special attention to this population. These findings highlight the need to improve training and access to spirometry and develop new diagnostic tools that facilitate earlier detection and management of COPD.

PurposeIn young adults (18 to 49 years old), investigation of the acute respiratory distress syndrome (ARDS) after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has been limited. We evaluated the risk factors and outcomes of ARDS following infection with SARS-CoV-2 in a young adult population.MethodsA retrospective cohort study was conducted between January 1st, 2020 and February 28th, 2021 using patient-level electronic health records (EHR), across 241 United States hospitals and 43 European hospitals participating in the Consortium for Clinical Characterization of COVID-19 by EHR (4CE). To identify the risk factors associated with ARDS, we compared young patients with and without ARDS through a federated analysis. We further compared the outcomes between young and old patients with ARDS.ResultsAmong the 75,377 hospitalized patients with positive SARS-CoV-2 PCR, 1001 young adults presented with ARDS (7.8% of young hospitalized adults). Their mortality rate at 90 days was 16.2% and they presented with a similar complication rate for infection than older adults with ARDS. Peptic ulcer disease, paralysis, obesity, congestive heart failure, valvular disease, diabetes, chronic pulmonary disease and liver disease were associated with a higher risk of ARDS. We described a high prevalence of obesity (53%), hypertension (38%- although not significantly associated with ARDS), and diabetes (32%).ConclusionTrough an innovative method, a large international cohort study of young adults developing ARDS after SARS-CoV-2 infection has been gather. It demonstrated the poor outcomes of this population and associated risk factor.

The inventory and variability of oceanic dissolved inorganic carbon (DIC) is driven by the interplay of physical, chemical, and biological processes. Quantifying the spatiotemporal variability of these drivers is crucial for a mechanistic understanding of the ocean carbon sink and its future trajectory. Here, we use the Estimating the Circulation and Climate of the Ocean-Darwin ocean biogeochemistry state estimate to generate a global-ocean, data-constrained DIC budget and investigate how spatial and seasonal-to-interannual variability in three-dimensional circulation, air-sea CO2 flux, and biological processes have modulated the ocean sink for 1995-2018. Our results demonstrate substantial compensation between budget terms, resulting in distinct upper-ocean carbon regimes. For example, boundary current regions have strong contributions from vertical diffusion while equatorial regions exhibit compensation between upwelling and biological processes. When integrated across the full ocean depth, the 24-year DIC mass increase of 64 Pg C (2.7 Pg C year-1) primarily tracks the anthropogenic CO2 growth rate, with biological processes providing a small contribution of 2% (1.4 Pg C). In the upper 100 m, which stores roughly 13% (8.1 Pg C) of the global increase, we find that circulation provides the largest DIC gain (6.3 Pg C year-1) and biological processes are the largest loss (8.6 Pg C year-1). Interannual variability is dominated by vertical advection in equatorial regions, with the 1997-1998 El Niño-Southern Oscillation causing the largest year-to-year change in upper-ocean DIC (2.1 Pg C). Our results provide a novel, data-constrained framework for an improved mechanistic understanding of natural and anthropogenic perturbations to the ocean sink.

Pneumonia, tuberculosis, and Covid-19 are different lung diseases but have similar characteristics. One of the reasons for the worsening of disease in lung sufferers is a diagnosis that takes a long time. Another factor, the results of the X-ray photos look blurry and lack contracture, causing different diagnostic results of X-ray photos. This research classifies lung images into four categories: normal lungs, tuberculosis, pneumonia, and Covid-19 using the Convolutional Neural Network method and VGG-16 architecture. The results of the research with models and scenarios without pre-trained use data with a ratio of 9:1 at epoch 50, an accuracy of 94%, while the lowest results are in scenarios using data with a ratio of 8:2 at epoch 50, non-pre-trained models, accuracy by 87%.

ABSTRACT Suicide is a deliberate act to end one’s life. Suicidal incidents are often found among medical students. The suicide rate among medical students is very high when compared with students of other majors. A study in the United States stated that prevalence of suicidal ideation in medical students was 11%, twice of the general population, while the prevalence of attempted suicide was 6.9%. Several factors related to the academic situation that contribute to mental disorders among medical students include pressure in medical education, demand for good skills and long education time. These factors make them prone to depression, burnout, and various emotional and mental disorders that can trigger suicide. Therefore, efforts are needed to prevent and detect the possibility of suicidal behavior. This effort must be carried out in a multidisciplinary manner. Optimizing the academic atmosphere, family involvement, the social environment, and the ability of adequate health facilities are needed. Keywords: Suicide, Medical Students, Prevention

High-strength, high-conductivity copper/silver-alloyed materials were prepared by cold-spray (CS) manufacturing. For DC high-field application at room temperature, bulk Cu/Ag (5% vol. Ag) alloys with high mechanical properties and high electrical conductivity can be obtained by CS and post-heat treatments. For pulsed-field application at liquid nitrogen temperature, bulk Cu/Ag (5% vol. Ag) alloys serve as precursors for room-temperature wire drawing. The Cu/Ag-alloyed bulk CS deposit presents a high yield strength of about 510 MPa with a corresponding electrical resistivity of 1.92 µΩ·cm (at 293 K). The Cu/Ag-alloyed wires show a very high ultimate tensile strength (1660 MPa at 77 K or 1370 MPa at 293 K) and low electrical resistivity (1.05 µΩ·cm at 77 K or 2.56 µΩ·cm at 293 K). Microstructural studies via STEM allow us to understand this very high level of mechanical strength. The results evidence that materials developed by CS exhibit very high mechanical properties compared to materials prepared by other routes, due to the high velocity of the deposited particles, which leads to high initial deformation rates and specific microstructural features.

Abstract The risk profiles of post-acute sequelae of COVID-19 (PASC) have not been well characterized in multi-national settings with appropriate controls. We leveraged electronic health record (EHR) data from 277 international hospitals representing 414,602 patients with COVID-19, 2.3 million control patients without COVID-19 in the inpatient and outpatient settings, and over 221 million diagnosis codes to systematically identify new-onset conditions enriched among patients with COVID-19 during the post-acute period. Compared to inpatient controls, inpatient COVID-19 cases were at significant risk for angina pectoris (RR 1.30, 95% CI 1.09–1.55), heart failure (RR 1.22, 95% CI 1.10–1.35), cognitive dysfunctions (RR 1.18, 95% CI 1.07–1.31), and fatigue (RR 1.18, 95% CI 1.07–1.30). Relative to outpatient controls, outpatient COVID-19 cases were at risk for pulmonary embolism (RR 2.10, 95% CI 1.58–2.76), venous embolism (RR 1.34, 95% CI 1.17–1.54), atrial fibrillation (RR 1.30, 95% CI 1.13–1.50), type 2 diabetes (RR 1.26, 95% CI 1.16–1.36) and vitamin D deficiency (RR 1.19, 95% CI 1.09–1.30). Outpatient COVID-19 cases were also at risk for loss of smell and taste (RR 2.42, 95% CI 1.90–3.06), inflammatory neuropathy (RR 1.66, 95% CI 1.21–2.27), and cognitive dysfunction (RR 1.18, 95% CI 1.04–1.33). The incidence of post-acute cardiovascular and pulmonary conditions decreased across time among inpatient cases while the incidence of cardiovascular, digestive, and metabolic conditions increased among outpatient cases. Our study, based on a federated international network, systematically identified robust conditions associated with PASC compared to control groups, underscoring the multifaceted cardiovascular and neurological phenotype profiles of PASC.

Patients with end-stage renal failure require renal replacement therapy, one of which is hemodialysis. one of the challenges in treating CKD patients is the lack of interventions that match the beliefs, expectations, and emotional reactions associated with the disease. The purpose of this study was to determine the patient's perceptions of undergoing hemodialysis and how the patient deals with the changes that occur in him. This research is a type of qualitative research with a phenomenological method. the research was conducted in the hemodialysis room hospital in Surabaya. The research was conducted in June-July 2019. The research subjects were determined to be snowball in nature. The research subjects were 6 people, who were chronic kidney disease sufferers who performed routine hemodialysis 2-3 times a week. Data were obtained through in-depth interviews using interview guidelines. The data analysis process includes data reduction, data categorization, synthesis and ends with compiling a working hypothesis. The results of the analysis obtained several themes, namely expectations, experiences, motives, and social/family conditions. in the early stages of the disease they experience some negative disease perceptions which can ultimately affect their coping actions. The role of disease perception is to help patients develop positive disease management strategies that in turn slow the rate of disease progression and improve psychosocial outcomes.

Abstract: The spatial correlation scales of oceanic dissolved inorganic carbon, heat content, and carbon and heat exchanges with the atmosphere are estimated from a realistic numerical simulation of the Southern Ocean. Biases in the model are assessed by comparing the simulated sea surface height and temperature scales to those derived from optimally interpolated satellite measurements. While these products do not resolve all ocean scales, they are representative of the climate scale variability we aim to estimate. Results show that constraining the carbon and heat inventory between 35°S and 70°S on time‐scales longer than 90 days requires approximately 100 optimally spaced measurement platforms: approximately one platform every 20° longitude by 6° latitude. Carbon flux has slightly longer zonal scales, and requires a coverage of approximately 30° by 6°. Heat flux has much longer scales, and thus a platform distribution of approximately 90° by 10° would be sufficient. Fluxes, however, have significant subseasonal variability. For all fields, and especially fluxes, sustained measurements in time are required to prevent aliasing of the eddy signals into the longer climate scale signals. Our results imply a minimum of 100 biogeochemical‐Argo floats are required to monitor the Southern Ocean carbon and heat content and air‐sea exchanges on time‐scales longer than 90 days. However, an estimate of formal mapping error using the current Argo array implies that in practice even an array of 600 floats (a nominal float density of about 1 every 7° longitude by 3° latitude) will result in nonnegligible uncertainty in estimating climate signals.

Abstrak Bumi Serpong Damai (BSD) merupakan salah satu kota mandiri di kawasan Jabodetabek. Sinarmas Land sebagai developer kawasan BSD telah memulai untuk membangun Angkutan Massal Cepat yang dioperasikan khusus di kawasan tersebut. Konsep Angkutan Massal yang dipilih berupa Bus Rapid Transit (BRT). Jumlah halte bus BSD Link untuk satu koridor dengan dua arah tidak persis sama, karena rute untuk keberangkatan dan kembalinya berbeda. Hal itu disebabkan karena perbedaan karakteristik dari pola tata guna lahan dari rute BSD Link tersebut. Oleh sebab itu, interaksi antara pola tata guna lahan dengan fluktuasi volume penumpang BSD Link setiap halte perlu diketahui. Upaya ini membutuhkan pengetahuan terkait tata guna lahan sepanjang rute BSD Link dan fluktuasi volume penumpang BSD Link di setiap halte. Survei terkait hal tersebut telah dilakukan pada Koridor Sektor 1.3-Greenwich Park pada jam puncak pagi. Hasil survei menunjukkan bahwa bangkitan perjalanan terbesar berasal dari halte East Bussiness District (50%), tarikan perjalanan terbesar berasal dari halte The Breeze (45%) untuk arah Sektor 1.3 – Greenwich Park. Bangkitan perjalanan terbesar berasal dari halte Naava Park (55%) dan tarikan perjalanan terbesar berasal dari halte Sektor 1.3 (35%) untuk arah Greenwich Park – Sektor 1.3. Sinarmas as developer of BSD started to develop Mass Rapid Transit which special operated on that. Concept of MRT has been operated in the form of Bus Rapid Transit. Amount of bus shelter for one corridor with two lines not exactly equal, because route for departure and return are different. That was caused by differences characteristic of land use. Thus, interaction between land use with fluctuation of passengers volume need to known. This attempt needs knowledge on land use along the route and fluctuation of passengers volume in each bus shelter. Survey on that knowledge were executed on Corridor Sector 1.3 - Greenwich Park during morning peak hour. The result indicates that peak of trip production generated by East Bussines Distcrict (50%) shelter, peak of trip attraction generated by The Breeze (45%) shelter for Sector 1.3 - Greenwich Park line. Peak of trip production generated by Naava Park (55%) shelter and peak of trip attraction generated by Sektor 1.3 (35%) shelter for Greenwich Park - Sector 1.3 line.
