1. Organizational Governance of Emerging Technologies: AI Adoption in Healthcare

Abstract

Private and public sector structures and norms refine how emerging technology is used in practice. In healthcare, despite a proliferation of AI adoption, the organizational governance surrounding its use and integration is often poorly understood. What the Health AI Partnership (HAIP) aims to do in this research is to better define the requirements for adequate organizational governance of AI systems in healthcare settings and support health system leaders to make more informed decisions around AI adoption. To work towards this understanding, we first identify how the standards for the AI adoption in healthcare may be designed to be used easily and efficiently. Then, we map out the precise decision points involved in the practical institutional adoption of AI technology within specific health systems. Practically, we achieve this through a multi-organizational collaboration with leaders from major health systems across the United States and key informants from related fields. Working with the consultancy IDEO [dot] org, we were able to conduct usability-testing sessions with healthcare and AI ethics professionals. Usability analysis revealed a prototype structured around mock key decision points that align with how organizational leaders approach technology adoption. Concurrently, we conducted semi-structured interviews with 89 professionals in healthcare and other relevant fields. Using a modified grounded theory approach, we were able to identify 8 key decision points and comprehensive procedures throughout the AI adoption lifecycle. This is one of the most detailed qualitative analyses to date of the current governance structures and processes involved in AI adoption by health systems in the United States. We hope these findings can inform future efforts to build capabilities to promote the safe, effective, and responsible adoption of emerging technologies in healthcare.

Abstract

Artificial intelligence (AI) and machine learning (ML) have occupied the center stage in healthcare as research groups and institutions investigate their capabilities and risks. Dermatology is often cited as one of the medical specialties most ripe for disruption with AI technology due to the heavy incorporation of visual information into clinical decisions and treatments. Although the literature on AI in dermatology is rapidly growing, there has been a noticeable absence of mature AI solutions utilized by dermatology departments or patients. This commentary provides insight into the regulatory challenges facing AI solutions for the specialty of dermatology and the unique considerations that should be factored into AI development and deployment.

7. Comparison of multiphase SPH and LBM approaches for the simulation of intermittent flows

Abstract

Smoothed Particle Hydrodynamics (SPH) and Lattice Boltzmann Method (LBM) are increasingly popular and attractive methods that propose efficient multiphase formulations, each one with its own strengths and weaknesses. In this context, when it comes to study a given multi-fluid problem, it is helpful to rely on a quantitative comparison to decide which approach should be used and in which context. In particular, the simulation of intermittent two-phase flows in pipes such as slug flows is a complex problem involving moving and intersecting interfaces for which both SPH and LBM could be considered. It is a problem of interest in petroleum applications since the formation of slug flows that can occur in submarine pipelines connecting the wells to the production facility can cause undesired behaviors with hazardous consequences. In this work, we compare SPH and LBM multiphase formulations where surface tension effects are modeled respectively using the continuum surface force and the color gradient approaches on a collection of standard test cases, and on the simulation of intermittent flows in 2D. This paper aims to highlight the contributions and limitations of SPH and LBM when applied to these problems. First, we compare our implementations on static bubble problems with different density and viscosity ratios. Then, we focus on gravity driven simulations of slug flows in pipes for several Reynolds numbers. Finally, we conclude with simulations of slug flows with inlet/outlet boundary conditions. According to the results presented in this study, we confirm that the SPH approach is more robust and versatile whereas the LBM formulation is more accurate and faster.

Abstract

Objectives Surface the urgent dilemma that healthcare delivery organizations (HDOs) face navigating the US Food and Drug Administration (FDA) final guidance on the use of clinical decision support (CDS) software. Materials and Methods We use sepsis as a case study to highlight the patient safety and regulatory compliance tradeoffs that 6129 hospitals in the United States must navigate. Results Sepsis CDS remains in broad, routine use. There is no commercially available sepsis CDS system that is FDA cleared as a medical device. There is no public disclosure of an HDO turning off sepsis CDS due to regulatory compliance concerns. And there is no public disclosure of FDA enforcement action against an HDO for using sepsis CDS that is not cleared as a medical device. Discussion and Conclusion We present multiple policy interventions that would relieve the current tension to enable HDOs to utilize artificial intelligence to improve patient care while also addressing FDA concerns about product safety, efficacy, and equity. Background: High Frequency Ultrasonography (HFUS) is a rapid, reliable, and effective diagnostic technique that has become progressively employed within the past years among dermatologists. Objective: To describe the use of HFUS among dermatology departments in Spain. Methods: An observational, cross-sectional descriptive study was conducted in 23 dermatology departments at Spanish public hospitals. Study data was collected during 10 working days. Results: Fifty dermatologists from 11 Spanish regions participated in the study; 659 HFUS procedures were undertaken in 633 patients. HFUS indications were benign tumors (41%), malignant tumors (28%), inflammatory diseases (18%), skin appendage conditions (10%), and cosmetic conditions (1%). HFUS was performed for assessing the clinical differential diagnosis (58% of cases), surgical planning (17%), follow-up (17%), and other reasons (7%). Among the diagnostic group, the scan confirmed clinical diagnosis in 82% of cases and helped to change the clinical diagnosis in 17% cases (10% of total scans).  HFUS procedure duration was less than 5 minutes (45%), between 5 and 10 minutes (32%), and more than 10 minutes (21%). Mean patient satisfaction score (range 0 to 5) was 4.8.Conclusions: The use of HFUS in usual practice is increasing, especially among younger dermatologists. This procedure is effective both for confirming and changing diagnosis, as well as for therapy decision making and follow-up. It is also worth remarking that a very high degree of patient satisfaction was recorded. The descriptive results reported in this study support the use of ultrasonography techniques and further reinforce their use in Dermatology.

Abstract

Taking Itamar Even-Zohar’s ‘polisystem theory’ and Pierre Bourdieu’s ‘theory of the field of cultural production’ as a starting point, the objective of this research is to analyse the development of Galician theatre and the re-configuration of the literary system during the period of institutionalization of the Galician culture after Franco’s death and the coming of democracy and political autonomy. By studying the 105 Cadernos published by the Escola Dramática Galega between 1978 and 1994, which cover a wide range of literary activities, the conflicts and tensions between different models and repertoires as well as the position and the contradictions of the Escola Dramática Galega during that process are brought to light. Gonzalez Millán’s theoretical concepts of ‘literary nationalism’ and ‘national literature’ will be useful in order to analyse the role that drama had on the nation-building process in Galicia and the consequences that that fact had on the development of the system. Conclusions are also drawn concerning the determinations imposed on the development of contemporary drama by the historical subsidiary position of the Galician literary system.

Abstract

This article proposes a user-adaptable and personalized authentication paradigm for healthcare organizations, which anticipates to seamlessly reflect patients' episodic and autobiographical memories to graphical and textual passwords aiming to improve the security strength of user-selected passwords and provide a positive user experience. We report on a longitudinal study that spanned over 3 years in which three public European healthcare organizations participated to design and evaluate the aforementioned paradigm. Three studies were conducted (n = 169) with different stakeholders: (1) a verification study aiming to identify existing authentication practices of the three healthcare organizations with diverse stakeholders (n = 9), (2) a patient-centric feasibility study during which users interacted with the proposed authentication system (n = 68), and (3) a human guessing attack study focusing on vulnerabilities among people sharing common experiences within location-aware images used for graphical passwords (n = 92). Results revealed that the suggested paradigm scored high with regard to users' likeability, perceived security, usability, and trust, but more importantly it assists the creation of more secure passwords. On the downside, the suggested paradigm introduces password guessing vulnerabilities by individuals sharing common experiences with the end users. Findings are expected to scaffold the design of more patient-centric knowledge-based authentication mechanisms within today's dynamic computation realms.

Abstract

The purpose of this work is to numerically model the Rotating Electrode Process (REP) for liquid nuclear fuel atomization. Smoothed Particle Hydrodynamics (SPH) method is chosen. Based on an existing SPH code, inflow boundary conditions were added, a surface-tension model was implemented and a new density model was developed. The SPH results obtained were compared to semi-empirical models. It was found that the predicted droplets diameter agrees with the models with a proportionality constant., Ziel dieser Arbeit ist die Simulation des Rotating Electrode Process zur Flüssigkeitszerstäubung mit der Methode der Smoothed Particle Hydrodynamics. Zufluss-Randbedingungen wurden zu einem bestehenden SPH-Code hinzugefügt, ein Oberflächenspannungsmodell implementiert und ein neues Dichtemodell entwickelt. Die erhaltenen SPH-Ergebnisse wurden mit semi-empirischen Modellen verglichen. Der prognostizierte Tropfendurchmesser stimmt mit den Modellen mit einer Proportionalitätskonstante überein.
