Abstract: Maintaining erect human posture depends on graviceptive information. This can come from at least of three origins: vestibular, visual and somaesthetic. We hypothesize here that subject''s use proprioception rather than visual or vestibular cues for their control of upright body posture and this even when subjects stand on a tilting body support surface. In order to find experimental evidence for this hypothesis, we exclude in our experiments visual cues (eyes close) and by keeping frequency and amplitude of the tilt stimulus so low that it would be below the detection threshold for vestibular semi-circular canal stimuli. The orientations of body segments were analysed during various phases of the perturbation cycle. Segmental stabilisations were defined in terms of both the global anchoring index calculated during the whole perturbation cycle and an appropriate sequential anchoring index calculated during various phases in the perturbation cycle. We show that subjects tend to align their bodies with the space vertical and do so better for their heads than for their upper bodies and lower bodies. A further finding is that stabilisation is related to the tilt stimulus in the form that it is minimal at the turning points of the tilt, where peak tilt velocity is minimal with the sinusoidal stimulus used. These finding suggest first that proprioceptive cues are predominant in the control of body orientation in quasi-static condition and second that the head and trunk stabilisation strategies used as the basis of postural control depend on the properties of the moving support. Abstract: The aim of the present study was to investigate how the orientation and stabilization components of postural control may be affected as the result of the impaired proprioceptive integration possibly occurring in Parkinson’s disease. To determine the proprioceptive contribution to postural control, parkinsonian patients and control subjects were asked to maintain vertical stance while very slow sinusoidal oscillations were being applied in the lateral and antero-posterior planes to the platform on which they were standing. The amplitude and frequency of their movements were kept below the semicircular canal perception threshold. Data were collected with the ELITE automatic motion analyzer and the two postural components (orientation and segmental stabilization) were analyzed at head and trunk levels while the subjects were performing the task with their eyes open and closed. The results show that 1) the parkinsonian groups’ performances were affected in terms of both the postural orientation and stabilization components in comparison with the control group, 2) the use of vision improved the parkinsonian patients’ postural performances, and 3) both parkinsonian patients and control subjects achieved better postural performances when antero-posterior perturbations rather than lateral perturbations were applied to the foot support. These results suggest that Parkinson’s disease is associated with proprioceptive impairment, which may be an important factor contributing to these patients’ postural deficits. On the basis of these results, the visual dependence observed in parkinsonian patients is re-defined as an adaptive strategy partly compensating for the impaired proprioception. High and low resolution solid state NMR methods have been applied to characterise a few samples of ancient wood. In an ancient larch wood sample, by applying 1H low resolution NMR methods as a function of the temperature, the average pore size and its distribution have been determined. In addition, high resolution NMR techniques have allowed addressing of the question of the proximity of water pools to cellulose and lignin. In particular, a model can be hypothesized in which water pools are surrounded by thin layers of amorphous cellulose and/or lignin while the crystalline domains of cellulose surround the layers of amorphous cellulose. Preliminary results obtained using a fully non invasive and portable NMR unilateral relaxometer, the Eureka-Mouse10 (EM10), are reported. Introduction – objectifs Le test QuantiFERON ® , utilisé pour le diagnostic de la tuberculose latente, est basé sur le dosage d’IFN-γ produit par les lymphocytes T en réponse à une stimulation par des protéines spécifiques du complexe M. tuberculosis . Le seuil de positivité recommandé par le fabricant est fixé à 0,35 UI/mL. Nous avons introduit au laboratoire une zone d’incertitude comprise entre 0,35 et 0,60 UI/mL pour l’interprétation des résultats. La pertinence de ce seuil a été vérifiée dans des études cliniques (1 , 2) . L’objectif de l’étude est de valider statistiquement la valeur du seuil de positivité sur un échantillon représentatif de la population explorée au laboratoire. Matériels et méthodes De novembre 2012 à janvier 2013, 1 066 tests QuantiFERON ® ont été réalisés. Les résultats > 10 UI/mL ont été éliminés afin de traiter uniquement des valeurs définies soit 1 000 résultats. Après transformation logarithmique, la répartition des résultats suit une loi normale et peut être représentée sous forme d’histogramme avec un intervalle des classes de 0,05 log (UI). La population « QuantiFERON négatif » déterminée graphiquement comprend 906 patients. Sur cette population, la moyenne (m) et l’écart-type (σ) des valeurs de résultats d’IFN-γ ont été calculés, ce qui permet de définir des seuils aux risques 1 % et 5 %, respectivement égaux à m +1,96 σ et m +2,58 σ. Résultats La moyenne du dosage d’IFN-γ dans cette population négative est de 0,09 UI/mL. Les seuils aux risques 1 % et 5 % sont respectivement de 0,78 et 0,60 UI/mL. Conclusion La limite supérieure de la zone d’incertitude à 0,60 UI/mL est justifiée au risque 5 % et valide parfaitement la zone d’incertitude [0,35 – 0,60 UI/ mL] qui avait été définie au laboratoire sur la base d’études cliniques. La dermatomyosite (DM) est une maladie polymorphe dans son expression phénotypique, immunologique et dans sa réponse thérapeutique. Nous rapportons le cas d'une patiente présentant une dermatomyosite avec anticorps anti-MDA5 (DM MDA5) associant une atteinte respiratoire sévère, des signes cutanés typiques et une atteinte musculaire. La prise en charge initiale par corticothérapie à forte dose, cyclophosphamide et immunoglobulines polyvalentes a entraîné une rémission partielle de la maladie et l'étude de la signature interféron a permis l'instauration d'un traitement d'entretien par anti-JAK1/2, apportant un contrôle des symptômes et la négativation de cette signature interféron. L'évolution a malheureusement été marquée par la survenue à 12 mois du suivi d'un carcinome pulmonaire à petites cellules, association paranéoplasique dont les données actuelles de la littérature sont considérées comme plutôt rassurantes pour ce sérotype de DM. Ce cas présente ainsi l'intérêt de décrire l'une des premières utilisations efficaces du ruxolitinib dans le traitement d'une dermatomyosite à anticorps anti-MDA5 avec interféronopathie en complément d'un traitement immunosuppresseur standard, et rapporte également la survenue d'une néoplasie non attendue dans ce phénotype. Dermatomyositis is an idiopathic inflammatory myopathy with various clinical and serological profiles, including poor prognosis forms for which aggressive immunosuppressive treatment is warranted. We report the case of a 60-year-old woman referred to our hospital for an anti-melanoma differentiation-associated 5 gene antibody-positive dermatomyositis (MDA5 DM) with rapidly progressive interstitial pneumonia, typical cutaneous features and muscular impairment. Treatment with high-dose methylprednisolone, cyclophosphamide and gamma globulin was performed, but the patient remained corticodependant. Blood detection of positive interferon signature justified the administration of an anti-JAK1/2, leading to the clinical remission and the regression of the interferon signature. After 12 months of follow up, a small cell carcinoma was discovered, raising the question of a paraneoplastic syndrome, for which the most recent datas are quite reassuring for this kind of MDA5 DM. The presentation of this case is of twofold interest: describing one of the first report of successful treatment of intereronopathy MDA5 DM with ruxolitinib and highlighting an association with a cancer, which is not expected for this phenotype of dermatomyositis. We report on the characterization of dioxin adsorption, notably 2,3-DCDD and 1,2,3,4-TeCDD, from isooctane onto zeolite FAU 13X Na + and FAU 13X La 3+ , with 80% Na + - La 3+ exchange level. Adsorption isotherms showed extreme affinities of 2,3-DCDD towards FAU 13X Na + and 1,2,3,4-TeCDD towards FAU 13X La 3+ in the experimentally accessible concentration range, which is limited by dioxin solubility in isooctane. Thermogravimetric studies were employed to understand the effect of water content level of zeolites onto the affinity and capacity of adsorption, but also to determine the temperature range of eventual dioxin regeneration. Moreover, microcalorimetric investigations indicated that high dioxin adsorption affinities of 2,3-DCDD towards FAU 13X Na + and 1,2,3,4-TeCDD towards FAU 13X La 3+ , respectively, were combined with quite different levels of adsorption enthalpies. Cross polarization magic angle spinning (CP-MAS) NMR experiments recorded with dynamic nuclear polarization (DNP) revealed that 1,2,3,4-TeCDD was adsorbed simultaneously with isooctane in the micropores of the zeolites, without chemical alteration. Drug hypersensitivity reactions ( DHRs) are a matter of great concern, both for outpatient and in hospital care. The evaluation of these patients is complex, because in vivo tests have a suboptimal sensitivity and can be time-consuming, expensive and potentially risky, especially drug provocation tests. There are several currently available in vitro methods that can be classified into two main groups: those that help to characterize the active phase of the reaction and those that help to identify the culprit drug. The utility of these in vitro methods depends on the mechanisms involved, meaning that they cannot be used for the evaluation of all types of DHRs. Moreover, their effectiveness has not been defined by a consensus agreement between experts in the field. Thus, the European Network on Drug Allergy and Drug Allergy Interest Group of the European Academy of Allergy and Clinical Immunology has organized a task force to provide data and recommendations regarding the available in vitro methods for DHR diagnosis. We have found that although there are many in vitro tests, few of them can be given a recommendation of grade B or above mainly because there is a lack of well-controlled studies, most information comes from small studies with few subjects and results are not always confirmed in later studies. Therefore, it is necessary to validate the currently available in vitro tests in a large series of well-characterized patients with DHR and to develop new tests for diagnosis. [ABSTRACT FROM AUTHOR]

Two binary aqueous mixtures which contain the small amphiphilic molecules TMAO (trimethylamine-N-oxide) and TBA (tert-butyl alcohol) have been investigated by molecular dynamics simulations and NMR chemical shift and self-diffusion measurements. TMAO is an osmolyte, while TBA is a monohydrate alcohol. Both possess bulky hydrophobic groups and polar heads, namely, NO in TMAO and OH in TBA. The hydrophilic/hydrophobic content of these isosteric molecules strongly modulates the structure and dynamics of the hydration shell, which is thought to be responsible for the effects observed on proteins and phospholipids. Simulation results, especially on hydrogen-bond networking, spatial correlations, and self-diffusivity, are consistent with NMR data and agree well with previous numerical studies on similar solutions. The methods employed allow the elucidation of the microscopic features of the solutions. For TBA solutions, the hydration shell is found to have a low density and a large spatial spread, and thus, above the molar fraction of 0.03, reduction of hydrophobic hydration drives self-aggregation of the solute. This effect does not take place in TMAO solutions, where the hydration shell is more compact and stable, maintaining its structure over a wider range of solute concentrations. [ABSTRACT FROM AUTHOR]

La grippe est une infection sévère suivant une allogreffe de cellules souches hématopoïétiques (allo-CSH). L'efficacité de la vaccination antigrippale (VAG) mesurée par la séroconversion est inférieure à celle en population générale. Cette étude pilote évalue l'apport de tests d'immunité fonctionnelle (TIF) appréciant la réponse cellulaire à la VAG chez des receveurs d'allo-CSH. Inclusion prospective durant la saison grippale 2019–2020 de volontaires sains (personnel hospitalier) et de receveurs d'allo-CSH non revaccinés depuis l'allogreffe testés immédiatement avant et un mois après la VAG tétravalente. L'immunité cellulaire est évaluée par 3 types de TIF sur PBMC stimulés in vitro par le VAG, comprenant : – la mesure en cytométrie de flux de la prolifération lymphocytaire ; – une quantification de l'interféron gamma (IFNg) dans le surnageant ; – le dénombrement des cellules T sécrétrices d'IFNg en technique ELISPOT. Le test d'inhibition de l'hémagglutination sera effectué en parallèle. Monitoring des épisodes infectieux d'allure grippale par écouvillonnage nasopharyngé pour PCR multiplex de virus pneumotropes. En cas d'identification d'un virus influenza, les sous-typage et séquençage des gènes grippaux Ha et Na seront effectués. Huit receveurs d'allo-CSH (âge médian 41,5 ans [intervalle interquartile (IIQ) : 35,5–56,5], H/F 5/3) et 10 volontaires sains (âge médian 44,5 ans [IIQ : 38–53], H/F 3/10) ont été inclus. Les patients avaient subi une allo-CSH géno-identique (n = 5) ou phéno-identique (n = 3) de cellules souches périphériques (n = 8) en raison d'une leucémie aiguë myéloïde (n = 8). Au moment de la VAG, 6 receveurs d'allo-CSH présentaient une maladie du greffon contre l'hôte active et 5 receveurs d'allo-CSH recevaient ≥ 1 immunosuppresseur(s). L'intervalle médian greffe-VAG était de 12 mois [IIQ : 12–14]. Avant vaccination, les volontaires sains avaient une réponse TIF sensiblement supérieure aux patients. Après vaccination, on observait un profil dissocié avec une différence non significative de prolifération lymphocytaire en réponse au VAG (p = 0,31), mais une production d'IFNg et une numération de cellules T sécrétrices d'IFNg significativement moindres (p = 0,009 et p = 0,002, respectivement). À ce jour, un receveur d'allo-CSH a développé une grippe A (H3N2) prouvée à 63 jours post-VAG. Ces TIF menés sur un échantillon limité ont montré une réponse fonctionnelle d'amplitude très diminuée et l'absence de mémoire immunologique spécifique après VAG des receveurs d'allo-CSH versus des volontaires sains. La standardisation de ces tests serait utile à de futurs essais interventionnels évaluant différentes modalités de VAG dans des populations à risque. [ABSTRACT FROM AUTHOR]

Introduction L'arthrite juvénile idiopathique (AJI) systémique, qui présente de nombreux points communs avec la maladie de Still de l'adulte, est un rhumatisme inflammatoire rare, dont la physiopathologie reste mal connue. L'atteinte hépatique touche 62 % des cas sous la forme d'une hépatique aiguë fébrile avec un ictère dans 7 % des cas. Nous rapportons le cas d'une hépatite aiguë ictérique sévère répondant à la ciclosporine A (CSA). Observation Une patiente de 24 ans, suivie depuis l'âge de 3 ans pour une AJI systémique de forme polycyclique est hospitalisée pour un ictère fébrile sous canakinumab introduit depuis 1 an. La patiente a présenté à deux reprises un syndrome d'activation macrophagique avec hépatite aiguë résolutive sous CSA depuis le début de la maladie. Elle est actuellement sous injection de canakinumab mensuelle à la posologie de 150 mg/injections avec une bonne observance. Il est observé un érythème fugace saumoné. Le bilan biologique retrouve une thrombopénie modérée (à 121 giga/l), un syndrome inflammatoire biologique (CRP à 37 mg/l), et une cholestase ictérique (bilirubine totale 90 mmol/l) associée à une cytolyse à 10 fois la normale, sans insuffisance hépatocellulaire. Il n'existe pas de dilatation des voies biliaires intra- ou extrahépatiques, ni d'anomalie du parenchyme sur l'échographie abdominale. Dans le service, la patiente est apyrétique, sans signe de gravité. Les bilans virologique (VHA, VHE, VHB, VIH, parvovirus B19, HSV, CMV EBV ; vaccination pour hépatite B, immunisation pour hépatite A) et bactériologique (fièvre Q, Bartonella , Leptospires , hémoculture) sont négatifs. L'hyperferritinémie est marquée à 11 000 μg/L. Il existe une augmentation modérée des triglycérides à 2,2 g/L sans cytopénie associée. Un syndrome d'activation macrophagique (SAM) est suspecté sur ces critères biologiques. Le bilan à la recherche d'une cause primitive est complété mais les tests de dégranulation sont normaux, ainsi que l'expression de la perforine. Il a été suspecté une rechute d'AJI systémique avec atteinte hépatique ou un SAM avec atteinte hépatique. Le traitement par prednisone (1 mg/kg), et des bolus de corticoïdes méthylprednisolone (1 g/jour pendant 3 jours) ont été entrepris sans efficacité sur l'hépatite. Le canakinumab réadministré à double dose, était lui aussi sans efficacité. L'administration de CSA (100 mg/jour) a permis une rémission complète de la maladie en 8 semaines. Conclusion Il s'agit d'un cas d'atteinte hépatique corticorésistante de l'AJI systémique. De plus, l'absence de réponse aux anti-IL1 questionne le mécanisme physiopathologique de cette atteinte. La réponse à la CSA permet d'évoquer un mécanisme de type lymphocytaire T et NK. L'évolution sous traitement suggère qu'il s'agit d'un SAM plutôt qu'une simple atteinte hépatique d'une AJI systémique en poussée. Ces dernières années, certains travaux ont mis en évidence des mutations de la perforine ou d'autres gènes des lympho-histiocytoses comme facteurs favorisant les SAM des AJI systémique et de la maladie de Still. Une production excessive d'IL-18 et IFN-g semble être au cœur du SAM et pourraient représenter des marqueurs utiles en clinique pour un diagnostic précoce de cette complication. [ABSTRACT FROM AUTHOR]
