Contexte et objectif: Bien que le fardeau de l'agression rénale aiguë (ARA) en Afrique subsaharienne soit présumé élever, mais son devenir à 3 mois de suivi reste peu étudier. L’objectif de la présente étude était de décrire les aspects cliniques et évolutifs de l’ARA. Méthodes. Il s’agissait d’une étude de suivi longitudinal analytique avec recueil prospectif des données, entre les 1er janvier et 30 septembre 2019,incluant tous les patients admis au CHUD-Borgou pour une altération de la fonction rénale et ayant donné leur consentement. L’ARA était retenue et classée selon les critères de KDIGO. L’issue était favorable devant une normalisation des valeurs de la créatininémie avant un délai de trois mois. L’évolution vers une insuffisance chronique était considérée comme défavorable. Résultats: Au total, 578 patients sur les 7975 admissions avaient une ARA soit une fréquence de 7,2 %. Les stades 1, 2 et 3 étaient rencontrésrespectivement, dans 53,8 %, 30,9 % et 15,2 %. L’âge moyen était de 46,9 ± 19,9 ans [extrêmes 5 ans et 93 ans]. Sur les 221 patients ayant été suivis, l’évolution était favorable (33,9 %) et défavorable dans 49,3 % des cas avec 17 % de décès. Le jeune âge (p=0,0253) et un bon état d’hydratationextracellulaire (p=0,0023) étaient associés à une évolution favorable de la fonction rénale, tandis que la présence de l’hypertension artérielle était péjorative (p=0,0147). Conclusion: L’ARA est fréquente. Son évolution est émaillée par le decès ou le passage à la chronicité, surtout chez les patients hypertendus. Les stratégies efficaces seraient le dépistage précoce et l’application des mesures préventives de l’ARA à tous les niveaux de la pyramide sanitaire. English title: Clinical and evolutionary aspects of acute kidney injury at the Borgou Departmental University Hospital (Benin): an observational prospective longitudinal follow-up study for 3 months Contexte and objective: Although the burden of acute kidney injury (AKI) in sub-Saharan Africa is presumed to be high, its outcome at 3-monthfollow-up remains poorly studied. Progression to chronic failure was considered unfavorable. Results: A total of 578 patients out of 7975 admissions had AKI, a frequency of 7.2 %. Stages 1, 2 and 3 were encountered in 53.8 %, 30.9 % and 15.2 %, respectively. The mean age was 46.9 ± 19.9 years [extremes 5 years and 93 years]. Of the 221 patients who were followed up, the evolution was favorable in 33.9 % of cases and unfavorable in 49.3 %, with 17 % of deaths. Younger age (p=0.0253) and good extracellular hydration status (p=0.0023) were associated with a favorable evolution of renal function, while the presence of arterial hypertension was pejorative (p=0.0147). Conclusion: AKI is common. Its evolution is marked by death or the passage to chronicity, especially in hypertensive patients. Effective strategies would be early detection and application of preventive measures for AKI at all levels of the health pyramid.

Introduction les relations entre les reins et l'hépatite B sont complexes. L'objectif de cette étude était de déterminer la prévalence et les facteurs associés à l'atteinte rénale chez des personnes vivant avec le virus de l'hépatite B (PVVHB) à Cotonou. Méthodes il s'agissait d'une étude transversale à visée descriptive et analytique, menée dans le service d'hépato-gastroentérologie du CNHU-HKM de mai à août 2017. Elle avait inclus les patients porteurs d'Ag HBs reçus dans la période. Une insuffisance rénale était retenue si le débit de filtration glomérulaire estimé par la formule du MDRD était en dessous de 90 mL/min/1,73m2. Résultats 105 patients porteurs d'Ag HBs étaient inclus. Parmi eux, 65 (61,9%) étaient sous traitement anti-VHB (dont 62 sous ténofovir). Des 105 patients, 41 avaient une insuffisance rénale (39%). Cette insuffisance rénale était associée à 2 cas d'atteinte tubulaire et 4 cas d'atteinte glomérulaire. La fonction rénale s'était dégradée dans le temps chez 22 patients (21%) et s'était améliorée chez 6 patients (5,7%). En analyse univariée, les facteurs associés à la survenue d'une insuffisance rénale étaient l'âge supérieur à 50 ans (p = 0,0125), la présence d'une hypertension artérielle (p = 0,0037), une fonction rénale initialement altérée (p < 0,0003) et les co-médications (p = 0,0007). Le traitement anti-VHB n'était pas associé à la survenue d'une insuffisance rénale (p = 0,2887). Conclusion la prévalence de l'insuffisance rénale chez les PVVHB était élevée (39%). L'âge, l'hypertension artérielle, une insuffisance rénale préexistante et les co-médications étaient identifiés comme des facteurs associés au déclin de la fonction rénale chez les PVVHB.

Introduction: General physicians can play an important role in the early detection of diabetic nephropathy (DN). Purpose: To assess the levels of general physicians’ knowledge, attitude and practice in terms of early detection of DN in Cotonou. Method: It was a cross-sectional, analytical and descriptive study which was conducted from 1st March 2015 to 30th September 2015. Every general physician working in a health structure in Cotonou who consented to participate in the study was included. We did not included medical specialists and general physicians working in nephrology department. Data were collected through a survey form designed with a score to assess the various items such as: knowledge, attitude and practice. The significance threshold is set to below 0.05. Results: In total, 202 general physicians were included. The average age was 30.9 ± 6.9 years ranging from 24 to 68 years. A male predominance was observed with 2.2 sex ratio. The majority of respondent medical physicians had poor knowledge in 76.2% cases, bad attitudes (61%) and bad practices (64.9%) in terms of early detection of diabetic nephropathy. There was positive impact of continuing medical training focused on diabetic nephropathy on attitudes (p = 0.016) and practices (p = 0.001) of these physicians. Conclusion: Diabetic nephropathy requires particular attention. General physicians’ continuous training is a principal solution.

Introduction: Acute renal failure (ARF) post-operative is a specific form of acute deficiencies causing multiple declining factors. Objective: This survey aims to study the incidence and risk factors associated with acute renal failure (ARF) in post-operative surgical intensive care units to University Hospital of Borgou (UH-B): during 2015. Patients and Methods this study is a cross sectional, descriptive and analytical ones with the prospective data collection from March 1st to August 31th, 2015. The research has involved all patients admitted to the operating room for surgery and motherhood whatever reason and then transferred respectively to the intensive care areas at UH-B. The postoperative ARF has been investigated inner patients following inclusion criteria and classification according to RIFLE score. The socio-demographic, clinical and biological variables monitoring, even support and evolution are experimented. A questionnaire is designed for data collection. Data are analyzed by Epi-Info means with 5% of significance level. Results: 130 patients are registered. The mean age is 27.68 ± 12.87 years. The sex ratio is 0.66. The frequency of post-operative ARF reaches 12.31%. The associated risk factors are: hypertension (p = 0.0018), diabetes (p = 0.002), heart failure (p = 0.0104), severe sepsis (p = 0.006) hypovolemic shock (p = 0.002), ASA class ≥ 3 (p = 0.0014), preeclampsia-eclampsia (p = 0.012), the Altémier class classification ≥ 3 (p = 0.0164), a pathological urinary sediment like a proteinuria (p=0.006), haematuria (p= 0.001) and nitrituria (p=0.007). Consequently, three (03) subjects out of sixteen (16) have died (18.75%). Conclusion: The post-operative ARF is a reality in University Hospital Borgou , with a higher mortality rate. Thus, the prevention strategy is the best treatment through the screening and monitoring promotion towards risk factors.
