Abstract Background While advances in endovascular thrombectomy (EVT) have led to high reperfusion rates, most patients treated with EVT do not avoid disability. Post‐reperfusion hemorrhagic transformation (HT) is a potential target for improving outcomes. This study examined pretreatment blood–brain barrier (BBB) disruption in tissue that would subsequently become part of the final infarct to evaluate its role in post‐EVT HT. Methods This post hoc analysis of the FRAME study, which enrolled patients with anterior large vessel occlusion who received EVT within 6 hours of onset, included patients if they had successful pretreatment MRI perfusion weighted imaging (PWI) and underwent successful EVT. BBB disruption was measured as the percent signal change due to gadolinium leakage on the PWI source images prior to thrombectomy. Mean permeability derangement (MPD) was defined as the average of all voxels in the stroke core that are two standard deviations above normal. The primary outcome was hemorrhagic transformation with parenchymal hematoma (PH). Results In total, 164 patients were included; mean age was 71 and 48% were female. PH occurred in 57 patients. 11. Efficacy and safety of cemiplimab in cutaneous squamous cell carcinoma on chronic wounds: A French retrospective study

Background: Chronic wounds increase the risk of the development of cutaneous squamous cell carcinoma (SCC), often resulting in poor prognosis in part because of delayed diagnosis. Cemiplimab, an anti-PD-1 agent, is recommended as first-line treatment for metastatic or locally advanced SCC not eligible for curative surgery, chemotherapy or radiation. However, its efficacy in SCC originating from chronic wounds remains uncertain. Objective: This retrospective study assessed the efficacy and safety of cemiplimab in patients with SCC originating from chronic wounds. Methods: We included patients receiving cemiplimab for SCC on chronic wounds between August 2018 and January 2021. The primary endpoint was progression-free survival (PFS) determined by Kaplan-Meier analysis. Secondary endpoints included response rate and safety. Results: We included 17 patients, predominantly female (59 %), with median age 58 years (interquartile range 49–77). SCC typically originated from leg ulcers (47 %) and presented at locally advanced (41 %) or metastatic stage (41 %). With cemiplimab, often received as third-line therapy (41 %), the median PFS was 6.1 months (95 % confidence interval [CI], 2.97–7.70), with the best response rate 47 % and complete response rate 12 %. We found immune-related adverse events in 24 % of cases. Short PFS was associated with distant metastasis at treatment initiation. Conclusion: Despite response rates comparable to other SCC types, median PFS was low with cemiplimab treatment for SCC originating from chronic wounds, likely due to the aggressive nature and/or high frequency of altered performance status. Anti-PD-1 therapy remains a primary treatment option for inoperable cases. Further prospective studies are warranted to confirm these findings and optimize treatment strategies.

Introduction Systemic sclerosis (SSc) is a rare autoimmune disease currently classified into two subgroups based on skin extension. The aim of this study was to determine in a large cohort whether the determination of autoantibody (AAb) profile among a full antinuclear AAbs panel including nine specificities had a higher impact than skin phenotype on stratifying the risk of organ involvement and mortality in SSc.Methods Data for patients with SSc followed in seven French university hospitals were retrospectively analysed in terms of skin phenotype, AAbs (anti-topoisomerase I (ATA), anticentromere (ACA), anti-RNA polymerase III (anti-RNAPIII), anti-U1RNP, anti-U3RNP, anti-Pm/Scl, anti-Ku, anti-Th/To, anti-NOR90), organ involvement and mortality. Multivariate analyses were performed to identify independent factors associated with organ involvement and mortality.Results We included 1605 patients with SSc (367 with diffuse cutaneous SSc). On multivariate analysis, ATAs were associated with interstitial lung disease and mortality (OR=3.27 (95% CI 2.42 to 4.42); HR=1.9 (95% CI 1.01 to 3.58)), anti-RNAPIII with scleroderma renal crisis and mortality (OR=7.05 (95% CI 2.98 to 16.72); HR=2.35 (95% CI 1.12 to 4.93)), anti-U1RNP with arthritis (OR=3.79 (95% CI 2.16 to 6.67)), anti-Pm/Scl and anti-Ku with myositis (OR=7.09 (95% CI 3.87 to 12.98) and 7.99 (95% CI 2.41 to 26.46)). The skin phenotype was not associated with survival or organ involvement on multivariate analysis without stepwise selection.Conclusion This study unravels, by contrast with skin phenotype, a strong association between AAbs specificities, organ involvement and outcome in SSc and suggests that patients’ classification based on only skin extension is not sufficient for defining prognosis and phenotype.

RISC-V is an open instruction set architecture recently developed for embedded real-time systems. To achieve a lasting security on these systems and design efficient countermeasures, a better understanding of vulnerabilities to novel and potential future attacks is mandatory. This paper demonstrates that RISC-V is sensible to Jump-Oriented Programming, a class of complex code-reuse attacks, able to bypass existing protections. We provide a first analysis of RISC-V systems' attack surface exploitable by such attacks, and show how they can be chained together in order to build a full-fledged attack. We use a conservative hypothesis on exploited registers and instruction patterns, in an approach we called reserved registers. Large language models (LLMs) have been shown to be able to perform new tasks based on a few demonstrations or natural language instructions. While these capabilities have led to widespread adoption, most LLMs are developed by resource-rich organizations and are frequently kept from the public. As a step towards democratizing this powerful technology, we present BLOOM, a 176B-parameter open-access language model designed and built thanks to a collaboration of hundreds of researchers. BLOOM is a decoder-only Transformer language model that was trained on the ROOTS corpus, a dataset comprising hundreds of sources in 46 natural and 13 programming languages (59 in total). We find that BLOOM achieves competitive performance on a wide variety of benchmarks, with stronger results after undergoing multitask prompted finetuning. To facilitate future research and applications using LLMs, we publicly release our models and code under the Responsible AI License.

Progress in machine learning (ML) comes with a cost to the environment, given that training ML models requires significant computational resources, energy and materials. In the present article, we aim to quantify the carbon footprint of BLOOM, a 176-billion parameter language model, across its life cycle. We estimate that BLOOM's final training emitted approximately 24.7 tonnes of~\carboneq~if we consider only the dynamic power consumption, and 50.5 tonnes if we account for all processes ranging from equipment manufacturing to energy-based operational consumption. We also study the energy requirements and carbon emissions of its deployment for inference via an API endpoint receiving user queries in real-time. We conclude with a discussion regarding the difficulty of precisely estimating the carbon footprint of ML models and future research directions that can contribute towards improving carbon emissions reporting.

SETD2 is known to be the unique histone methyltransferase responsible for the trimethylation of the lysine 36 of histone H3 thus generating H3K36me3. This epigenetic mark is critical for transcriptional activation and elongation, DNA repair, mRNA splicing, and DNA methylation. Recurrent SETD2-inactivating mutations and altered H3K36me3 levels are found in cancer at high frequency and numerous studies indicate that SETD2 acts as a tumor suppressor. Recently, SETD2 was further shown to methylate non-histone proteins particularly the cytoskeletal proteins tubulin and actin with subsequent impacts on cytoskeleton structure, mitosis and cell migration.Herein, we provide a review of the role of SETD2 in different cancers with special emphasis on the structural basis of the functions of this key lysine methyltransferase. Moreover, beyond the role of this enzyme in epigenetics and H3K36me3-dependent processes, we highlight the putative role of ''non-epigenetic/H3K36me3'' functions of SETD2 in cancer, particularly those involving the cytoskeleton.

In 1996, Rothstein and Laumon simultaneously constructed a Fourier-Mukai transform for D-modules over a locally noetherian base of characteristic 0. This functor induces an equivalence of categories between quasi-coherent sheaves of D-modules over an abelian variety A and quasicoherent sheaves of O-modules over its universal vectorial extension A. In this article, we define a Fourier-Mukai transform for D-modules on an abelian formal scheme A/S = Spf (V), where V is a discrete valuation ring, and we discuss the extension of the classical results of Fourier-Mukai transform to this arithmetic case.

In patients with moderate to severe atopic dermatitis (AD) showing an inadequate response to dupilumab 300mg/2weeks, few real-life studies reported the response to alternative regimen maintaining dupilumab.To assess and analyze the response to an increased dose of dupilumab or its combination with cyclosporin A (CsA), methotrexate (MTX), or itraconazole (ITRA), all adult AD patients from 7 French University Hospitals were retrospectively included if they achieved an inadequate response to dupilumab 300mg/2weeks and were subsequently treated with an increased dose of dupilumab (300mg every 7 or 10 days), or a combination of dupilumab 300mg/2weeks with CsA, MTX or ITRA. The response after 3 months, along with epidemiological, clinical, and therapeutic baseline characteristics, were collected.Overall, 68.75% of the 48 included patients achieved an improved response, including 45.8% of complete response (CR). No strategy proved significantly better. Patients showing an initial no response never achieved a further CR versus 52.4% of patients with an initial partial response (p = 0.025). Digestive intolerance and tachycardia led to MTX and ITRA discontinuation in 3 patients.Increasing the dose of dupilumab or combining it with CsA, MTX, or ITRA could be alternative and safe options, to be evaluated in further medico-economic studies.

Summary: Background: Sézary syndrome is an extremely rare and fatal cutaneous T-cell lymphoma (CTCL). Mogamulizumab, an anti-CCR4 monoclonal antibody, has recently been associated with increased progression-free survival in a randomized clinical trial in CTCL. We aimed to evaluate OS and prognostic factors in Sézary syndrome, including treatment with mogamulizumab, in a real-life setting. Methods: Data from patients with Sézary (ISCL/EORTC stage IV) and pre-Sézary (stage IIIB) syndrome diagnosed from 2000 to 2020 were obtained from 24 centers in Europe. Age, disease stage, plasma lactate dehydrogenases levels, blood eosinophilia at diagnosis, large-cell transformation and treatment received were analyzed in a multivariable Cox proportional hazard ratio model. This study has been registered in ClinicalTrials (SURPASSe01 study: NCT05206045). Findings: Three hundred and thirty-nine patients were included (58% men, median age at diagnosis of 70 years, Q1-Q3, 61–79): 33 pre-Sézary (9.7% of 339), 296 Sézary syndrome (87.3%), of whom 10 (2.9%) had large-cell transformation. One hundred and ten patients received mogamulizumab. Median follow-up was 58 months (95% confidence interval [CI], 53–68). OS was 46.5% (95% CI, 40.6%–53.3%) at 5 years. The intra-articular reconstruction of the cranial cruciate ligament (CrCL) by an organic graft or a synthetic implant allows the restoration of physiological stifle stability. This treatment is still marginal in routine practice. A Rottweiler presented an acute complete CrCL rupture treated using an ultra-high-molecular-weight polyethylene (UHMWPE) implant. The latter was positioned under arthroscopic guidance and fixed with interference screws through femoral and tibial bone tunnels. The dog was weight-bearing just after surgery and resumed normal standing posture and gait after one month, with mild signs of pain upon stifle manipulation. At three months postoperatively, minimal muscle atrophy and minimal craniocaudal translation were noted on the operated hindlimb, with no effects on the clinical outcome. The stifle was painless. At six months postoperatively, standing posture and gait were normal, muscle atrophy had decreased, the stifle was painless, and the craniocaudal translation was stable. On radiographs, congruent articular surfaces were observed without worsening of osteoarthrosis over the follow-up, as well as stable moderate joint effusion. Replacement of a ruptured CrCL with a UHMWPE ligament yielded good functional clinical outcome at six months postoperatively. This technique could be considered an alternative for the treatment of CrCL rupture in large dogs, but it needs confirmation from a prospective study with more dogs.

Background: As isolated ruptures of the caudal cruciate ligament (CdCL) are rare in dogs, there is no consensus on the indications and the gold-standard surgical technique for treatment. Case Description: A 2-year-old Shepherd dog with an isolated rupture of the CdCL was treated with a new surgical technique for synthetic reconstruction. Three bone tunnels were drilled in the femur and the tibia under arthroscopic guidance to make sure the anatomical insertions of the physiological ligament were respected. An Ultra-High Molecular Weight Polyethylene (UHMWPE) implant was fixed with interference screws to reconstruct the CdCL. A synovial inflammation remained present on radiographs for six months after the surgery, together with a mild lameness. However, the dog fully recovered clinically and recovered a normal level of activity after six months. LOAD results at six months and one year postoperatively were excellent. Conclusion: The use of an UHMWPE implant fixed with interference screws to reconstruct the CdCL allowed a return to full function of the knee without complications, despite a persistent synovial inflammation and mild lameness for a 6-month period after the surgery. The success of this isolated surgical technique could lead to improvements in the surgical management of CdCL rupture, if these initial results are confirmed by a prospective study with a larger number of patients. Among vertebrates, allomothering (non-maternal care) is classified as cooperative breeding (help from sexually mature non-breeders, usually close relatives) or communal breeding (shared care between multiple breeders who are not necessarily related). Humans have been described with both labels, most frequently as cooperative breeders. However, few studies have quantified the relative contributions of allomothers according to whether they are (a) sexually mature and reproductively active and (b) related or unrelated. We constructed close-proximity networks of Agta and BaYaka hunter–gatherers. We used portable remote-sensing devices to quantify the proportion of time children under the age of 4 spent in close proximity to different categories of potential allomother. Both related and unrelated, and reproductively active and inactive, campmates had substantial involvement in children's close-proximity networks. Unrelated campmates, siblings and subadults were the most involved in both populations, whereas the involvement of fathers and grandmothers was the most variable between the two populations. Finally, the involvement of sexually mature, reproductively inactive adults was low. Where possible, we compared our findings with studies of other hunter–gatherer societies, and observed numerous consistent trends. Based on our results we discuss why hunter–gatherer allomothering cannot be fully characterised as cooperative or communal breeding.

Background: Rupture of the common calcaneal tendon is the second most frequent tendon rupture in dogs and may lead to severe lameness and pain. Surgical repair consists of re-apposition of the damaged tendon ends using sutures, but this type of repair is not always possible especially if the tendon has retracted. Tendon augmentation with an ultra-high molecular weight polyethylene (UHMWPE) implant is a recent solution to support the sutures and allow the repair of the canine calcaneal tendon. However, its biomechanical fixation strength remains untested for this pathology. Aim: To evaluate the biomechanical fixation strength of an UHMWPE implant for the repair of the canine calcaneal tendon. Methods: Ex-vivo biomechanical study was carried out on eight cadaveric hindlimbs from four adult dogs. Hindlimbs were tested under two independent modalities: proximal tendinous fixation (PTF) and distal calcaneus fixation (DCF), using a testing machine. PTF was achieved by eight simple interrupted polypropylene sutures performed through the UHMWPE implant. The latter was sandwiched inside the gastrocnemius tendon, which had previously been incised over about 5 cm longitudinally, and through the tendon of the superficial digital flexor. DCF was performed using an interference screw, which locked the UHMWPE implant into a calcaneus tunnel drilled perpendicularly. Results: Yield, failure load and linear stiffness (mean ± SD) for the DCF modality were 920 ± 139 N, 1007 ± 146 N and 92 ± 15.21, respectively, which were greater than for the PTF modality (663 ± 92 N, 685 ± 84 N and 25.71 ± 5.74, respectively, P [Open Vet J 2023; 13(5.000): 645-653]

We present twenty-five C programs, as a benchmark for C program verification using formal methods. This benchmark can be used for system demonstration, for comparison of verification effort between systems, and as a friendly competition. For this last purpose, we give a scoring formula that allows a verification system to score up to a hundred points.
