Background: Caesarean section is one of the most frequently performed surgeries in obstetrics. Optimal pain relief of the mother is possible with multimodal methods. Wound infiltration is one of the simplest and safe methods. The aim of the study was to compare the analgesic efficacy of wound infiltration of bupivacaine, with tramadol and bupivacaine mixture for postoperative pain. Materials and Methods: After obtaining Institutional Ethical Committee approval, 60 pregnant women of age group 18–35 years, undergoing elective caesarean section under spinal anaesthesia were included in the study. The patients were randomized into two groups of 30 each. Patients belonging to group B received bupivacaine 0.25% 0.7 mg/kg (diluted to 20 mL with normal saline), while those belonging to group T received tramadol 2 mg/kg mixed in bupivacaine 0.25% 0.7 mg/kg (diluted to 20 mL with normal saline). The time for first request of analgesia, numerical rating scale score, and cumulative consumption of diclofenac and tramadol in 24 h were compared. Results: The time for first request of analgesia in group T was 386.17 ± 233.84 min compared to group B which was 192.50 ± 134.77 (P < 0.0002). The total diclofenac consumption in the first 24 h was significantly reduced in group T when compared to group B (P < 0.0068). Conclusions: Subcutaneous wound infiltration with tramadol and bupivacaine prolongs the pain-free period and analgesic consumption after caesarean section, while it also enhances the patient satisfaction.

High Energy Astrophysical Phenomena (astro-ph.HE) ,FOS: Physical sciences ,Astrophysics - High Energy Astrophysical Phenomena - Abstract
HESS J1809$-$193 is one of the unidentified very-high-energy gamma-ray sources in the H.E.S.S. Galactic Plane Survey (HGPS). It is located in a rich environment, with an energetic pulsar and associated X-ray pulsar wind nebula, several supernova remnants, and molecular clouds in the vicinity. Furthermore, HESS J1809$-$193 was recently detected at energies above 56 TeV with HAWC, which makes it a PeVatron candidate, that is, a source capable of accelerating cosmic rays up to PeV energies. We present a new analysis of the TeV gamma-ray emission of HESS J1809$-$193 with H.E.S.S., based on improved analysis techniques. We find that the emission is best described by two components with distinct morphologies and energy spectra. We complement this study with an analysis of Fermi-LAT data in the same region. Finally, taking into account further multi-wavelength data, we interpret our results both in a hadronic and leptonic framework., Comment: 6 pages, 5 figures. Objective: Fentanyl has evinced its property of being a potent opioid and benumbing stress response to laryngoscopy and intubation (L&I). However respiratory depression and chest wall rigidity limits its utility at high doses. Nalbuphine, reported to have a ceiling effect for respiratory depression, can serve as an alternative. We studied and compared the efficacy of fentanyl and nalbuphine in preventing the rise in heart rate and blood pressure after L&I. Materials and Methods: A randomized double blind study was conducted in 60 subjects divided in two groups (Group F n=30 and Group N n=30) was done. Group F received 2µg kg-1 fentanyl intravenously while group N received nalbuphine 0.2mg kg-1 intravenously. Hemodynamic variables including heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) and mean arterial pressure (MAP) were measured at baseline and at intervals of five minutes of administering test drug, each minute up to five minutes after intubation, 10 minutes and 30 minutes after intubation. Complications, if any, were recorded after surgery. Change in HR, SBP, DBP and MAP between the two groups was compared using independent t-test. Results: The two drugs were equally effective in preventing HR rise at all-time points except after 30 minutes where fentanyl performed better. The change in SBP, DBP and MAP from baseline in both the groups was comparable. Conclusions: Nalbuphine can serve as an alternative to fentanyl in obtunding stress response when given five minutes before intubation. Keywords: Nalbuphine, Fentanyl, Intubation.

Highly extended gamma-ray emission around the Geminga pulsar was discovered by Milagro and verified by HAWC. Despite many observations with Imaging Atmospheric Cherenkov Telescopes (IACTs), detection of gamma-ray emission on angular scales exceeding the IACT field-of-view has proven challenging. Recent developments in analysis techniques have enabled the detection of significant emission around Geminga in archival data with H.E.S.S.. In 2019, further data on the Geminga region were obtained with an adapted observation strategy. Following the announcement of the detection of significant TeV emission around Geminga in archival data, in this contribution we present the detection in an independent dataset. New analysis results will be presented, and emphasis given to the technical challenges involved in observations of highly extended gamma-ray emission with IACTs., PoS: Proceedings of Science, 395, ISSN:1824-8039, Proceedings of 37th International Cosmic Ray Conference

Nowadays the implementation of artificial neural networks in high-energy physics has obtained excellent results on improving signal detection. In this work we propose to use neural networks (NNs) for event discrimination in HAWC. This observatory is a water Cherenkov gamma-ray detector that in recent years has implemented algorithms to identify horizontal muon tracks. However, these algorithms are not very efficient. In this work we describe the implementation of three NNs: two based on image classification and one based on object detection. Using these algorithms we obtain an increase in the number of identified tracks. The results of this study could be used in the future to improve the performance of the Earth-skimming technique for the indirect measurement of neutrinos with HAWC., Comment: 8 pages, 6 figures, Proceedings of the 37th International Cosmic Ray Conference (ICRC 2021)

The realtime follow-up of neutrino events is a promising approach to search for astrophysical neutrino sources. It has so far provided compelling evidence for a neutrino point source: the flaring gamma-ray blazar TXS 0506+056 observed in coincidence with the high-energy neutrino IceCube-170922A detected by IceCube. The detection of very-high-energy gamma rays (VHE, $\mathrm{E} > 100\,\mathrm{GeV}$) from this source helped establish the coincidence and constrained the modeling of the blazar emission at the time of the IceCube event. The four major imaging atmospheric Cherenkov telescope arrays (IACTs) - FACT, H.E.S.S., MAGIC, and VERITAS - operate an active follow-up program of target-of-opportunity observations of neutrino alerts sent by IceCube. This program has two main components. One are the observations of known gamma-ray sources around which a cluster of candidate neutrino events has been identified by IceCube (Gamma-ray Follow-Up, GFU). Second one is the follow-up of single high-energy neutrino candidate events of potential astrophysical origin such as IceCube-170922A. GFU has been recently upgraded by IceCube in collaboration with the IACT groups. We present here recent results from the IACT follow-up programs of IceCube neutrino alerts and a description of the upgraded IceCube GFU system., Presented at the 37th International Cosmic Ray Conference (ICRC 2021). Primordial Black Holes are expected to be formed in the early Universe by the gravitational collapse of overdense regions, among other mechanisms. They are also expected to loose their mass over time by the Hawking radiation process. As the rates of this radiation increase with temperature, the PBH evaporation should result in a violent explosion. The current upper limits on explosion rates are on the order of 10$^{4}$ − 10$^{5}$pc$^{-3}$yr$^{-1}$. In this contribution we'll present the results of a search for TeV γ-ray burst within timescale of few seconds, using nearly 5000 hours of H.E.S.S. data. The search algorithm and statistical estimation strategy will be presented as well as cosmological implications of this measurement.

Extreme high synchrotron peaked blazars (EHBLs) are amongst the most powerful accelerators found in nature. Usually the synchrotron peak frequency of an EHBL is above $10^{17}\,$Hz, i.e., lies in the range of medium to hard X-rays making them ideal sources to study particle acceleration and radiative processes. EHBL objects are commonly observed at energies beyond several TeV, making them also powerful probes of gamma-ray absorption in the intergalactic medium. During the last decade, several attempts have been made to increase the number of EHBL detected at TeV energies and probe their spectral characteristics. Here we report new detections of EHBLs in the TeV energy regime, each at a redshift of less than 0.2, by the High Energy Stereoscopic System (H.E.S.S.). Also, we report on X-ray observations of these EHBLs candidates with Swift XRT. In conjunction with the very high energy observations, this allows us to probe the radiation mechanisms and the underlying particle acceleration processes., Proceedings of the 37th International Cosmic Ray Conference (ICRC 2021)

While the youngest known supernova remnants (SNRs), such as Cassiopeia A (Cas A), have been proven to be able to accelerate cosmic rays (CRs) only up to ∼ 10$^{14}$ eV at their present evolutionary stages, recent studies have shown that particle energies larger than a few PeV (10$^{15}$ eV) could be reached during the early stages of a core-collapse Supernova (cc-SN), when the high-velocity forward shock expands into the dense circumstellar medium (CSM) shaped by the stellar progenitor wind. Such environments, in particular the type IIn SNe whose progenitors may exhibit mass loss rates as high as 10$^{-2}$ M$_{\bigodot}$ yr$^{-1}$, could thus lead to γ-ray emission from $\pi ^{0}$ decay in hadronic interactions, potentially detectable with current Cherenkov telescopes at very-high energies. Such a detection would provide direct evidence for efficient acceleration of CR protons/nuclei in supernovae, and hence new insights on the long-standing issue of the origin of Galactic Cosmic Rays. In that context, the High Energy Stereoscopic System (H.E.S.S.) has been carrying out a Target of Opportunity program since 2016 to search for such an early very-high-energy γ-ray emission towards nearby core-collapse supernovae and supernova candidates (up to ∼ 10 Mpc), within a few weeks after discovery. After giving an overview of this H.E.S.S. Target of Opportunity program, we present the results obtained from the July 2019 observations towards the transient AT2019krl, originally classified as a type IIn supernova, which occurred in the galaxy M74 at ∼ 9.8 Mpc. Although its nature still remains unclear, the derived H.E.S.S. constraints on this transient are placed in the general context of the expected VHE γ-ray emission from core-collapse supernovae.

General Medicine - Abstract
In this research work, the problem is to reduce the surface roughness of the Jacquard body which is a casting part to optimize the effects of different process parameters of surface roughness & to reduce surface roughness for milling operation by using the Taguchi method. Milling is better than other machines in the surface finish and accuracy. Greater quality product and higher productivity is the main vision of every entrepreneur. To achieve this, the process parameters of each process need to be optimized. In a milling machine, some of the important parameters like feed rate, depth of cut, speed, etc, play an important role in achieving the required quality product. To optimize these parameters, the most suitable and widely used method is the Taguchi method. The most significant objective of this research is to obtain effective quality parameters. Experimentation is done on the Jacquard body by varying the milling machine parameters. The experimental results indicate that the minimum surface roughness is obtained for the process parameters feed 0.7 mm per revolution, speed 225 rpm and 1.5 mm depth of cut. This study recommended that more surface finish be obtained by increasing speed, low feed rate, and depth of cut.

Gamma-ray bursts (GRBs) are among the most luminous sources in the universe. The nature of their emission at TeV energies is one of the most relevant open issues related to these events. The temporal and spectral features inferred from the early and late emissions usually known as prompt and afterglow, respectively, can be interpreted within the context of the fireball model. The synchrotron self-Compton process is expected during the afterglow phase. We explain how the theoretical SSC light curves can be compared with hypothetical upper limit located at z=0.3. We show the allowed parameter space of the microphysical parameters and density of the circumburst medium. The most restrictive results are obtained when the SSC process lies in the fast cooling regime, Comment: Presented at the 37th International Cosmic Ray Conference(ICRC2021),Berlin, Germany-online

The majority of the active galactic nuclei (AGN) detected at very-high-energies above 100 GeV belong to the class of blazars with a small angle between the jet-axis and the line-of-sight. Only about 10 percent of the gamma-ray AGN are objects with a larger viewing angle resulting in a smaller Doppler boosting of the emission. Originally, it was believed that gamma-ray emission can only be observed from blazars and those are variable in its brightness. Instead, the last years have shown that non-blazar active galaxies also show a fascinating variability behaviour which provide important new insights into the physical processes responsible for the gamma-ray production and especially for flaring events. Here, we report on the observation of gamma-ray variability of the active galaxy PKS 0625−354 detected with the H.E.S.S. telescopes in November 2018. The classification of PKS 0625−354 is a still matter of debate. The H.E.S.S. measurements were performed as part of a flux observing program and showed in the first night of the observation a detection of the object with > 5σ. A denser observation campaign followed for the next nine nights resulting in a decrease of the gamma-ray flux. Those observations were accompanied with Swift in the X-ray and UV/optical band allowing for the reconstruction of a multi-band broad-band spectral energy distribution. We will discuss the implications of the gamma-ray variability of the object.
