Abstract

Background: Historical reports on surgical renal denervation consistently describe renal plexus as a triangle or fan-like structure converging at the kidney gate. Following that anatomy, we developed a distal mode of radiofrequency renal denervation (RDN) mainly in segmental branches of the renal artery and confirmed its superior efficacy over the conventional main trunk procedure in a 6-months double-blind randomized controlled trial (NCT02667912). To assess the long-term effects of distal RDN we extended the follow-up of our study to three years. Methods: BP, serum creatinine, eGFR were measured one and three years after randomization; major adverse renal events were assessed over the entire study period. The blinding was maintained over the entire three-year study period. Findings: Of 55 randomized patients, 47 (23/24, distal/main trunk RDN, respectively) were assessed at one year and 39 (21/18) at three years post-procedure. Twenty-four-hour ambulatory systolic BP remained powerfully lowered after distal RDN both at one- and three-years assessments(mean change from baseline: -18.0, 95% CI -27.6 to -8.5; p0·05. After main trunk RDN, the decrease of 24h systolic BP was quite moderate at one year: -12·1, 95% CI -19·2 to -5·0; p0.05. eGFR was almost unchanged at one year: -1·3, 95% CI -6·6 to 4·0; p>0·05, but significantly decreased at three years: -5·0, 95% CI -9·6 to -0·3; p

Abstract

Background: Despite advances in surgery and pharmacotherapy, there remains significant residual ischemic risk after coronary artery bypass grafting surgery. Methods: In REDUCE-IT (Reduction of Cardiovascular Events With Icosapent Ethyl–Intervention Trial), a multicenter, placebo-controlled, double-blind trial, statin-treated patients with controlled low-density lipoprotein cholesterol and mild to moderate hypertriglyceridemia were randomized to 4 g daily of icosapent ethyl or placebo. They experienced a 25% reduction in risk of a primary efficacy end point (composite of cardiovascular death, myocardial infarction, stroke, coronary revascularization, or hospitalization for unstable angina) and a 26% reduction in risk of a key secondary efficacy end point (composite of cardiovascular death, myocardial infarction, or stroke) when compared with placebo. The current analysis reports on the subgroup of patients from the trial with a history of coronary artery bypass grafting. Results: Of the 8179 patients randomized in REDUCE-IT, a total of 1837 (22.5%) had a history of coronary artery bypass grafting, with 897 patients randomized to icosapent ethyl and 940 to placebo. Baseline characteristics were similar between treatment groups. Randomization to icosapent ethyl was associated with a significant reduction in the primary end point (hazard ratio [HR], 0.76 [95% CI, 0.63–0.92]; P =0.004), in the key secondary end point (HR, 0.69 [95% CI, 0.56–0.87]; P =0.001), and in total (first plus subsequent or recurrent) ischemic events (rate ratio, 0.64 [95% CI, 0.50–0.81]; P =0.0002) compared with placebo. This yielded an absolute risk reduction of 6.2% (95% CI, 2.3%–10.2%) in first events, with a number needed to treat of 16 (95% CI, 10–44) during a median follow-up time of 4.8 years. Safety findings were similar to the overall study: beyond an increased rate of atrial fibrillation/flutter requiring hospitalization for at least 24 hours (5.0% vs 3.1%; P =0.03) and a nonsignificant increase in bleeding, occurrences of adverse events were comparable between groups. Conclusions: In REDUCE-IT patients with a history of coronary artery bypass grafting, treatment with icosapent ethyl was associated with significant reductions in first and recurrent ischemic events. Abstract

BACKGROUND: It is expected that a steady increase in the incidence of diabetes and resistant hypertension (RHTN), along with an increase in life expectancy, will lead to a noticeable increase in the proportion of patients with heart failure with preserved ejection fraction (HFpEF). At the same time, data on the frequency of HFpEF in a selective group of patients with RHTN in combination with diabetes are still lacking, and the pathophysiological and molecular mechanisms of its formation have not been yet studied sufficiently.AIM: To assess the features of the development HFpEF in diabetic and non-diabetic patients with RHTN, as well as to determine the factors associated with HFpEF.MATERIALS AND METHODS: In the study were included 36 patients with RHTN and type 2 diabetes mellitus (DM) (mean age 61.4 ± 6.4 years, 14 men) and 33 patients with RHTN without diabetes, matched by sex, age and level of systolic blood pressure (BP). All patients underwent baseline office and 24-hour BP measurement, echocardiography with assess diastolic function, lab tests (basal glycemia, HbA1c, creatinine, aldosterone, TNF-alpha, hsCRP, brain naturetic peptide, metalloproteinases of types 2, 9 (MMP-2, MMP-9) and tissue inhibitor of MMP type 1 (TIMP-1)). HFpEF was diagnosed according to the 2019 AHA/ESC guidelines.RESULTS: The frequency of HFpEF was significantly higher in patients with RHTN with DM than those without DM (89% and 70%, respectively, p=0.045). This difference was due to a higher frequency of such major functional criterion of HFpEF as E/e’≥15 (p=0.042), as well as a tendency towards a higher frequency of an increase in left atrial volumes (p=0.081) and an increase in BNP (p=0.110). Despite the comparable frequency of diastolic dysfunction in patients with and without diabetes (100% and 97%, respectively), disturbance of the transmitral blood flow in patients with DM were more pronounced than in those without diabetes. Deterioration of transmitral blood flow and pseudo-normalization of diastolic function in diabetic patients with RHTN have relationship not only with signs of carbohydrate metabolism disturbance, but also with level of pulse blood pressure, TNF-alfa, TIMP-1 and TIMP-1 / MMP-2 ratio, which, along with the incidence of atherosclerosis, were higher in patients with DM than in those without diabetes.CONCLUSIONS: Thus, HFpEF occurs in the majority of diabetic patients with RHTN. The frequency of HFpEF in patients with DN is significantly higher than in patients without it, which is associated with more pronounced impairments of diastolic function. The progressive development of diastolic dysfunction in patients with diabetes mellitus is associated not only with metabolic disorders, but also with increased activity of chronic subclinical inflammation, profibrotic state and high severity of vascular changes.

Abstract

Aim To study the functional condition of sympathoadrenal system as evaluated by beta-adrenoreactivity of erythrocyte membranes (beta-ARM) during two years following renal denervation (RD) in patients with resistant arterial hypertension (RAH) and to determine the relationship of this index with long-term antihypertensive and cardioprotective effectivity of this invasive treatment.Material and methods The study included 48 patients (mean age, 57.2±8.7 years, 18 men) with RAH on a stable antihypertensive therapy. Averaged daily systolic and diastolic blood pressure (SBP and DBP) and levels of beta-ARM were determined at baseline and in 7 days and 2 years following RD. Measurement of beta-ARM was based on beta-adrenoblocker inhibition of erythrocyte hemolysis induced by exposure to hypo-osmotic environment. The beta-adrenoblocker binds to erythrocyte membrane beta-adrenoceptors to prevent the erythrocyte destruction. Increased values of beta-ARM reflect a decrease in the number of functionally active erythrocyte membrane beta-adrenoceptors associated with long-term sympathetic hyperactivity.Results For two years of follow-up, values of average daily BP decreased from 160.4±16.0 / 88.1±14.6 to 145.3±19.3 / 79.4±13.6 mm Hg. At 7 days, the number of beta-ARM had decreased in the group of RD responders (р=0.028) who at two years had decreased their BP by 10 mm Hg or more, while in the group of non-responders, the number of beta-ARM remained unchanged. At one week, beta-ARM values correlated with changes in SBP and DBP (r= –0.54; рConclusion The decrease in beta-ARM content at 7 days after RD shows the procedure efficacy and allows an expectation of clinically significant decreases in BP and LVMM in the long-term after the surgical treatment. At two years after the intervention, the content of beta-ARM increased, and the BP decrease was apparently due to some other mechanisms.

The endothelin pathway has been implicated in the pathogenesis of hypertension, but it is currently not targeted therapeutically, thereby leaving this relevant pathophysiological pathway unopposed with currently available drugs. The aim of the study was to assess the blood pressure lowering efficacy of the dual endothelin antagonist aprocitentan in patients with resistant hypertension. METHODS: PRECISION was a multicentre, blinded, randomised, parallel-group, phase 3 study, which was done in hospitals or research centres in Europe, North America, Asia, and Australia. Patients were eligible for randomisation if their sitting systolic blood pressure was 140 mm Hg or higher despite taking standardised background therapy consisting of three antihypertensive drugs, including a diuretic. The study consisted of three sequential parts: part 1 was the 4-week double-blind, randomised, and placebo-controlled part, in which patients received aprocitentan 12·5 mg, aprocitentan 25 mg, or placebo in a 1:1:1 ratio; part 2 was a 32-week single (patient)-blind part, in which all patients received aprocitentan 25 mg; and part 3 was a 12-week double-blind, randomised, and placebo-controlled withdrawal part, in which patients were re-randomised to aprocitentan 25 mg or placebo in a 1:1 ratio. The primary and key secondary endpoints were changes in unattended office systolic blood pressure from baseline to week 4 and from withdrawal baseline to week 40, respectively. Secondary endpoints included 24-h ambulatory blood pressure changes. The study is registered on ClinicalTrials.gov, NCT03541174. FINDINGS: The PRECISION study was done from June 18, 2018, to April 25, 2022. 1965 individuals were screened and 730 were randomly assigned. Of these 730 patients, 704 (96%) completed part 1 of the study; of these, 613 (87%) completed part 2 and, of these, 577 (94%) completed part 3 of the study. The least square mean (SE) change in office systolic blood pressure at 4 weeks was -15·3 (SE 0·9) mm Hg for aprocitentan 12·5 mg, -15·2 (0·9) mm Hg for aprocitentan 25 mg, and -11·5 (0·9) mm Hg for placebo, for a difference versus placebo of -3·8 (1·3) mm Hg (97·5% CI -6·8 to -0·8, p=0·0042) and -3·7 (1·3) mm Hg (-6·7 to -0·8; p=0·0046), respectively. Abstract

Background There are very few data on the initial features of hemodynamics and its changes after renal denervation (RDN) in patients with resistant hypertension (RHTN) and type 2 diabetes mellitus (DM), depending on the presence of chronic kidney disease (CKD), and the mechanisms of this effect require further study. Purpose To assess the features of renal hemodynamics and its change after RDN in patients with RHTN with type 2 DM, depending on the presence or absence of CKD. Methods In the study were included 38 patients with RHTN with type 2 DM (mean age 60.6±8.1 years, 14 men, 24-hour systolic / diastolic blood pressure (24-hr SBP/DBP) 160.9±16.5 / 82.2±11.3 mm Hg, mean HbA1c 7.87±1.62%), of which CKD occurred in 18 pts. (eGFR – 47.1 ml/min/1.73 m2), and 20 pts. without CKD (eGFR – 80.3 ml/min/1.73m2). All patients underwent baseline office and 24-hr BP measurement with calculation DBP/SBP ratio (D/S ratio), Doppler ultrasonography, serum creatinine, estimated glomerular filtration rate (eGFR) (CKD-EPI formula), urinary albumin excretion (UAE). The six-month follow-up period was completed by 14 patients in the group with CKD and 18 patients without CKD. Results At baseline, patients with CKD compared with those without CKD had higher pulse BP (82.2±10.0 versus 75.6±15.9, p=0.061), renal resistive indices (RRI) (0.78±0.06 versus 0.69±0.05, p=0.001 in the main renal arteries (RA); 0.73±0.07 versus 0, 63±0.06, p=0.001 in segmental RA) and significantly lower level of D/S ratio (0.49±0.06 versus 0.53±0.06, p=0.040). At the same time, the frequency of increased RRI (>0.70) in the CKD group was almost 14 times higher than in those without CKD (73% and 5.3%, respectively, p0.05) in patients with CKD, the frequency of which was almost 4 times higher than that in persons without CKD (43% and 11%, respectively, p=0.049). Moreover, a decrease in RRI correlated with an increase in the D / S ratio of both the main RA (R = 0.50; p=0.005) and in segmental RA (R = 0.40; p=0.028). There were no significant changes in UAE and serum creatinine in both groups. Conclusions Thus, CKD in patients with RHTN with type 2 DM is characterized by a significant increase in renal vascular resistance, while RDN in this patients, in contrast to patients without CKD, can be an effective therapeutic option not only for BP reduction, but also for improvement of renal hemodynamics. Abstract

Background.The renin-angiotensin-aldosterone system (RAAS) plays a key role in target organ damage in arterial hypertension (HTN), initiating the development of left ventricular hypertrophy (LVH), as well as the heart and vascular wall fibrosis and remodeling. In addition, one of the mechanisms of the cardiovascular disease progression is the angiotensin II-induced inflammation.Objective.To study the changes in renin, aldosterone and high-sensitive C‑reactive protein (CRP) levels two years after sympathetic renal denervation (RDN), to compare these changes with antihypertensive efficacy of the intervention and LVH regression.Design and methods.We included 77 patients with drug-resistant hypertension in the absence of contraindications to renal denervation. All patients underwent renal radiofrequency ablation. The active renin, aldosterone and a high-sensitive CRP concentrations assessment, 24‑hour blood pressure (BP) measurement and echocardiography were performed before, at 6 months, one and two years after the intervention.Results.There was a gradual decrease in CRP levels (the difference was significant after 6 months), aldosterone (significant two years after surgical treatment), and active renin (the difference was the most pronounced after one year). At all follow-up assessments, plasma renin activity correlated with left ventricular mass. At the same time, there were no significant differences between responders and non-responders.Conclusions.RDN leads to a RAAS activity attenuation, manifested by the decrease in both renin and aldosterone and CRP, probably due to angiotensin II proinflammatory effects reduction. Given these effects are long-term, correlate with LVH degree and unrelated to the BP lowering, a direct cardioprotective effect of renal denervation should be considered.

Abstract

Patients with resistant arterial hypertension have a high prevalence of cardiac pathology. In particular, left ventricular hypertrophy and reduced coronary reserve play independent role in determining the risk of cardiovascular complications. Diagnostic approaches to their detection are currently represented by a wide range of highly informative studies using modern ultrasound equipment, magnetic resonance imaging, computed tomography scanner, and gamma camera. The active study of the involvement of the cascade of pathophysiological mechanisms in the development of cardiac changes in hypertension allows not only to identify the factors associated with the severity of the lesion, but also to find new application points for the cardioprotection and to reduce cardiovascular risk. Along with the pharmacological capabilities, the renal denervation method is currently under active study. Despite high variability of the results and their poor predictability, a number of studies show the efficiency of this method in correcting the structural and functional changes in the heart, which is of important prognostic value for resistant hypertension.

Abstract

In the 21st century, hypertension remains a major health threat to the adult population worldwide, despite the phenomenal progress in the development of antihypertensive pharmacotherapy. Currently, the rate of pharmacological control of hypertension barely exceeds 50%. Indeed, pharmacotherapy is fundamentally limited by pharmacological/pharmacokinetic tolerance (adaptation) to the drugs as well as by non-compliance of patients with treatment regimens due to depletion of their psychophysiological function of self-control. Renal denervation (RDN) is a new non-drug treatment of hypertension. Renal denervation consists of a mini-electrosurgery on the renal sympathetic system where a catheter ablation is used to create a permanent block of conduction through the renal nerves. This procedure, in accordance with the function of the renal adrenergic receptors, increases glomerular filtration, reduces tubular water reabsorption, and inhibits glomerular secretion of renin. After the over-disputed failure of the early version of RDN using the first-generation single-electrode catheter system in the SYMPLICITY HTN-3 study, therapy was virtually reinvented with a new three-dimensional multi-electrode design and an anatomically optimized procedure. The new device design ensures deployment of the electrodes in a pre-defined fully circumferential pattern. Moreover, this design provides a radial contact that presses the electrodes against the arterial wall thereby maximizing the efficiency of radiofrequency (RF) tissue heating. Another major improvement of RDN therapy is the anatomical optimization of the procedure by extending the treatment into the segmental branches of the renal artery where the renal nerves concentrate the most around renal vessels. This article presents an analysis of the current state-of-the-development and future perspectives of RDN therapy.

Abstract

Aim. To study the long-term effect of renal denervation (RDN) on left ventricular mass (LVM) and inflammatory markers in resistant hypertensive patients.Material and methods. Forty-one patients with resistant hypertension and 24-h blood pressure (BP) 158,7±15,8/87,3+14,6 mmHg, aged 56,6+10,2 years, were enrolled in the study and undergone RDN. Mean 24-h BP, left ventricular mass (transthoracic echocardiography), high sensitivity C-reactive protein (hsCRP), interleukin- 1β (IL­1β), IL-6, IL-10) and tumor necrosis factor alpha (TNF- α) were assessed at baseline and 2 years after the RDN.Results. A baseline prevalence of left ventricular hypertrophy (LVH) was 90,2%. Two years after RDN LVM and interventricular septum (IVS) decreased significantly (p0 g) was documented in 24 patients. The regression of LVM was accompanied by a significant decrease in levels of inflammatory markers — hsCRP by 38,3% (p=0,031), TNF-α by 60,7% (p=0,009), IL- 1β — by 71,1% (p=0,001), and IL-10 by 58,2% (p=0,001). In patients in the absence of LVM regression only TNF-α decreased significantly (-68,8%, p=0,001). There was no correlation between changes of LVM and the inflammatory markers at 24 months after RDN.Conclusion. The RDN in RH patients may have long-term cardioprotective effect in terms of significant regress of LVH, which may be partly attributed to the regress in systemic or myocardial inflammation.

Aim To compare the antihypertensive effectivity of renal denervation in patients with diabetes mellitus (DM) and associated refractory arterial hypertension (rfAH) (treated with 5 or more classes of antihypertensive drugs, including a thiazide diuretic and a mineralocorticoid receptor antagonist) and uncontrolled resistant AH (ucAH) (treated with 3-4 drugs).Material and methods This interventional study with renal denervation included 18 DM patients with rfAH and 40 DM patients with ucAH; 16 and 36 of them, respectively, completed the study in 6 months. At baseline, patients were sex- and age-matched. Study methods included measurement of office blood pressure (BP; systolic/diastolic BP, SBP/DBP); outpatient BP monitoring; evaluation of kidney function (estimated glomerular filtration rate by the CKD-EPI formula); diurnal diuresis volume; diurnal urinary excretion of albumin, potassium and sodium; diurnal excretion of metanephrines and normetanephrines; and plasma levels of glucose and glycated hemoglobin, aldosterone, and active renin. Patients were instructed about maintaining compliance with their antihypertensive and hypoglycemic therapy throughout the study.Results At baseline, patients of both groups were comparable by BP and major clinical indexes, except for higher values of nocturnal SBP variability (pConclusion DM patients with rfAH may be the best candidates for the procedure of renal denervation.

Abstract

Aim. We evaluated the influence of renal cysts on the efficiency of reduction of arterial pressure in patients with treatment-resistant hypertension after renal denervation. Materials and Methods. A total of 33 patients (aged 57.8±8.75 years) with treatment-resistant hypertension were included. All patients underwent radiofrequency ablation (RFA) of the sympathetic nerves of renal arteries. The average level of arterial pressure was as high as 137–210/57–148 mm Hg according to 24-h monitoring. MRI studies were performed using MRI scanner “Titan Vantage” (by Toshiba Medical Ltd., 1.5 Т) in T1-W and T2-W spin-echo models. After this, the intravenous contrast enhancement was carried out (with 0.5 М solution, 0.2 ml/Kg), with subsequently repeated acquisition of Т1-weighted spin-echo images 3-5 min after contrast injection, employing exactly the same protocols as before enhancement. Results. Preoperative images demonstrated cysts in kidneys in half of patients (16 individuals). Positive effect (the arterial pressure decreased by 10 mm Hg) was observed in 70% of them. However, in 30% of population, the arterial pressure increased. Among the patients without cysts in the kidneys, negative effect was observed in 8% only, and positive effect was found in 77% of patients (arterial pressure decreased by over 20 mm Hg). Conclusion. There is a statistically significant trend towards a greater antihypertensive effect of renal denervation in patients without cystic changes in the renal parenchyma (cortex). Thus, MRI is of prognostic value for prediction of renal denervation efficacy.

Abstract

Background Diabetic patients with resistant hypertension (RHT) are characterized by accelerated annual decline in estimated glomerular filtration rate (eGFR) (from 10 to 14 mL/min/1,73m2). The distal renal denervation (RD) can preserve renal function within first year follow-up in these patients. However, whether this effect will persist at a later period is unknown, especially taking into account the potential risks of kidney damage due to deep introduction of the catheter and contrast into the segmental branches of the artery, as well as exposure to radiofrequency energy near to the kidney. In addition, RHT and diabetes mellitus (DM) contribute to impaired renal autoregulation which can lead to a decrease in GFR in response to a decrease in BP, but the long-term safety of a powerful BP reduction for renal function in these patients remains unexplored. The purpose of this study was to assess the renal safety and efficacy of a distal RD compared to conventional intervention in diabetic patients with RHT during 3 years of follow-up. Methods Twenty seven patients with true RHT and type 2 DM were included in single-arm prospective interventional study (number NCT02667912 on ClinicalTrial.gov) (mean age 61.6±7.3 years, mean office (systolic/diastolic) BP 171.8±21.9/87.7±17.7 mmHg, mean eGFR 72.1±19.9 mL/min/1.73m2, 7 patients with CKD (stage 3), 11 patients with albuminuria (>30 mg/day), 8 men), among them 13 – undergone distal RD and 14 – conventional RD. Offices BP, ABPM, renal Doppler ultrasound, renal function (eGFR (CKD-EPI) and 24-hour urinary albumin excretion (UAE)) were assessed at baseline and annually for 3 years follow up. There were no any difference in sex, old, eGFR, UAE, BP level, and frequency both of CKD and albuminuria between distal RD group and conventional RD group. The patients were asked to refrain from changing their medication regimen for the duration of the study. Nineteen patients completed 36 months follow-up (8 treated by distal RD and 11 – by conventional mode). Results The change in eGFR for distal RD during 3 years follow-up was not significant (−5.3±0.4 mL/min/1.73m2, P=0.63) and was comparable with that for conventional RD (−6.7±4.2, P=0.3), (P=0.7 for the between-group difference), despite of a more powerful decrease in 24h-systolic BP and 24h-puls BP in the distal RD group than in the conventional RD group (−25.3±15.3 vs. −4.4±22 mmHg, P=0.04, and −14.3±15.1 vs. 0.1±7.7 mmHg, respectively). The degree of annual decline in GFR for the distal RD was the same as for the conventional RD (p=0.9), and was −2.7±2.4 mL/min/1.73m2/year, and −2.7±4.0 mL/min/1.73m2/year, respectively. There were no significant changes in mean values of renal blood flow, albuminuria, and the numbers of patients with albuminuria and CKD in both groups after 3 years of follow-up. Conclusions Distal RD in diabetic patients with RHT has a good renal safety profile and can slow the progression of CKD within 3 years of follow-up.

Abstract

Background Short-term treatment for people with type 2 diabetes using a low dose of the selective endothelin A receptor antagonist atrasentan reduces albuminuria without causing significant sodium retention. We report the long-term effects of treatment with atrasentan on major renal outcomes.Methods We did this double-blind, randomised, placebo-controlled trial at 689 sites in 41 countries. We enrolled adults aged 18-85 years with type 2 diabetes, estimated glomerular filtration rate (eGFR) 25-75 mL/min per 1.73 m(2) of body surface area, and a urine albumin-to-creatinine ratio (UACR) of 300-5000 mg/g who had received maximum labelled or tolerated renin-angiotensin system inhibition for at least 4 weeks. Participants were given atrasentan 0.75 mg orally daily during an enrichment period before random group assignment. Those with a UACR decrease of at least 30% with no substantial fluid retention during the enrichment period (responders) were included in the double-blind treatment period. Responders were randomly assigned to receive either atrasentan 0.75 mg orally daily or placebo. All patients and investigators were masked to treatment assignment. The primary endpoint was a composite of doubling of serum creatinine (sustained for >= 30 days) or end-stage kidney disease (eGFR = 90 days, chronic dialysis for >= 90 days, kidney transplantation, or death from kidney failure) in the intention-to-treat population of all responders. Safety was assessed in all patients who received at least one dose of their assigned study treatment. The study is registered with ClinicalTrials. gov, number NCT01858532.Findings Between May 17, 2013, and July 13, 2017, 11 087 patients were screened; 5117 entered the enrichment period, and 4711 completed the enrichment period. Of these, 2648 patients were responders and were randomly assigned to the atrasentan group (n=1325) or placebo group (n=1323). Median follow-up was 2.2 years (IQR 1.4-2.9). 79 (6.0%) of 1325 patients in the atrasentan group and 105 (7.9%) of 1323 in the placebo group had a primary composite renal endpoint event (hazard ratio [HR] 0.65 [95% CI 0.49-0.88]; p=0.0047). Fluid retention and anaemia adverse events, which have been previously attributed to endothelin receptor antagonists, were more frequent in the atrasentan group than in the placebo group. Hospital admission for heart failure occurred in 47 (3.5%) of 1325 patients in the atrasentan group and 34 (2.6%) of 1323 patients in the placebo group (HR 1.33 [95% CI 0.85-2.07]; p=0.208). 58 (4.4%) patients in the atrasentan group and 52 (3.9%) in the placebo group died (HR 1.09 [95% CI 0.75-1.59]; p=0.65).Interpretation Atrasentan reduced the risk of renal events in patients with diabetes and chronic kidney disease who were selected to optimise efficacy and safety. These data support a potential role for selective endothelin receptor antagonists in protecting renal function in patients with type 2 diabetes at high risk of developing end-stage kidney disease. Abstract

to define and develop a procedure that can select patients with resistant hypertension (RH) for a renal sympathetic denervation (RSD) procedure, by being orienting to the specific initial values of β-adrenoreactivity (β-AR) and systolic blood pressure (SBP).The analysis included 23 RH patients receiving the maximally tolerable doses of 4 antihypertensive drugs. The investigations involving BP control and a Russian spectrophotometric procedure for determining β-AR in terms of the change in the osmoresistance of erythrocyte membranes (EM) were performed at baseline and 4 and 24 weeks. RSD was carried out using endovascular radiofrequency ablation of the renal arteries. The therapy was not changed during the observation.If SBP was170 mm Hg and β-AR of EM40 conditional units (CUs) at baseline, ΔSBP was 17.68±3.24 mm Hg and the efficiency of RSD was 100%. When SBP was170 mm Hg and β-AR of EM40 CUs at baseline, ΔSBP was 0.97±4.21 mm Hg (p0.05) and the efficiency of RSD was low.The overall estimate of baseline SBP and β-AR of EM in patients with RH could determine the expediency of the RSD procedure in order to lower BP. The proposed procedure can optimize the selection of patients and enhance the efficiency of RSD in the treatment of RH.Цель исследования. Разработать способ, позволяющий проводить отбор пациентов с резистентной артериальной гипертонией (РАГ) для процедуры симпатической денервации почек (СДП), ориентируясь на конкретные исходные показатели β-адренореактивности (β-АР) и систолического артериального давления (САД). Материалы и методы. В анализ включили 23 пациентов с РАГ, получавших максимально переносимые дозы 4 антигипертензивных препаратов. Исследования, включая контроль АД и отечественный спектрофотометрический метод определения β-АР по изменению осморезистентности эритроцитарных мембран (ЭМ), проведены исходно, через 4 и 24 нед. СДП осуществляли путем эндоваскулярной радиочастотной абляции почечных артерий (ПА). Терапия в процессе наблюдения не менялась. Результаты. В случае, если исходно САД170 мм рт.ст. и β-АР ЭМ40 усл. ед., то ΔСАД составило –17,68±3,24 мм рт.ст., а эффективность СДП 100%, при исходном САД170 мм рт.ст. и β-АР ЭМ40 усл. ед. ΔСАД составило 0,97±4,21 (p0,05) и эффективность СДП была низкой. Заключение. Суммарная оценка исходных САД и β-АР ЭМ у больных РАГ позволила определить целесообразность проведения процедуры СДП с целью снижения АД. Предлагаемый способ дает возможность оптимизировать подбор пациентов и повысить эффективность применения СДП при лечении РАГ.

Abstract

to investigate time course of changes in the adrenoreactivity (AR) of erythrocyte membranes (EM) after radiofrequency ablation of the synaptic nerves of the renal arteries (RA) in patients with resistant hypertension (RH) and to assess whether this indicator can be used for the early evaluation of the efficiency of an invasive intervention into the RA.24 patients with RH, who received full-dose antihypertensive therapy with at least three drugs, including a diuretic, were examined. Renal sympathetic denervation (RSD) was carried out by endovascular radiofrequency ablation (RFA) of the RA. 24-hour blood pressure (BP) monitoring and determination of the β-adrenoreactivity (β-AR) of EM were performed, by taking into account the change in erythrocyte osmoresistence at baseline and 1 and 24 weeks after RFA. The therapy was not changed during the observation.The patients included in the study were divided into 2 groups. One week following RSD, 15 patients of Group 1 were noted to have a decrease in the β-AR of EM by 10 conditional units or more; average daily systolic BP (SBP) and diastolic BP (DBP) reduced by 8.3 and 2.8 mm Hg, respectively. In 9 patients of Group 2, the β-AR of EM was unchanged in this observation period or increased compared with baseline. In this group, the decrease in average daily SBP and DBP was noted to be less pronounced than that in Group 1 (by 1.4 and 1.5 mm Hg, respectively). At 24 weeks after RSD, Group 1 was seen to have an effective daily decrease in average daily SBP and DBP by 25.6 and 14.3 mm Hg, respectively (p=0.01 and 0.05). The average value of the β-AR of EM significantly declined compared with baseline. In Group 2, no statistically significant changes were reported; SPB and DBP lowered by 7.0 and 3.0 mm Hg, respectively. There was a significant decrease in the β-AR of EM compared with that at week 1.The decline in the β-AR of EM within the first week after RFA is suggestive of the decreased activity of the sympathoadrenal system and may be used as an early efficiency index of RSD after the procedure.Цель исследования. Изучить динамику адренореактивности мембран эритроцитов после радиочастотной деструкции симпатических нервов почечной артерии у пациентов с резистентной артериальной гипертонией (РАГ) и оценить возможность использования этого показателя для ранней оценки эффективности инвазивного вмешательства на почечных артериях. Материалы и методы. Обследовали 24 пациентов с РАГ, получавших полнодозовую антигипертензивную терапию не менее чем тремя препаратами, включая диуретик. Симпатическую денервацию почек (СДП) проводили путем эндоваскулярной радиочастотной абляции (РЧА) почечных артерий (ПА). Суточное мониторирование артериального давления (АД) и определение β-адренореактивности мембран (β-АРМЭ) по изменению осморезистентности эритроцитов выполняли исходно и спустя 1 и 24 нед после РЧА. Терапию в процессе наблюдения не меняли. Результаты. Включенных в исследование пациентов разделили на 2 группы. У 15 пациентов 1-й группы спустя 1 нед после СДП отмечено снижение β-АРМЭ эритроцитов на 10 усл. ед. и более, снижение среднесуточных систолического АД (САД) и диастолического АД (ДАД) составило –8,3 и –2,8 мм рт.ст. У 9 пациентов 2-й группы изменение β-АРМЭ на этом сроке наблюдения отсутствовало, либо показатель увеличивался относительно исходного. Отмечено менее выраженное, чем в 1-й группе, снижение среднесуточных САД и ДАД (1,4 и –1,5 мм рт.ст. соответственно). Спустя 24 нед после СДП у пациентов 1-й группы отмечено эффективное снижение среднесуточных САД и ДАД на –25,6/–14,3 мм рт.ст. соответственно (р=0,01 и 0,05). Средняя β-АРМЭ в этой группе пациентов достоверно снизилась по сравнению с исходной. У пациентов 2-й группы не зафиксировано статистически значимых изменений АД, снижение САД и ДАД составило –7 и –3 мм рт.ст. соответственно. Отмечено значительное снижение β-АРМЭ по сравнению с таковой на сроке 1 нед. Заключение. Снижение β-АРМЭ в течение 1-й недели после РЧА свидетельствует о снижении активности симпатико-адреналовой системы организма и может быть использовано как ранний показатель эффективности СДП после процедуры.

Abstract

Aim. To test the hypothesis that anatomical optimisation of radiofrequency renal denervation (RDN) by redistribution of treatment to segmental branches of the renal artery causes a durable increase in the efficacy of the intervention.Methods. A double-blind randomised control study in parallel groups with a 3-year follow-up was performed in patients with apparent resistant hypertension (office systolic BP ≥160 or diastolic BP ≥100 mmHg despite stable treatment with ≥3 drugs, including a diuretic). Exclusion criteria were secondary hypertension, mean 24-h systolic BP mL/min/m2 and severe comorbidity increasing procedural risks (investigator’s assessment). Eligible patients were randomised 1:1 into distal RDN or RDN in the main trunk of the renal artery. Treatment assignment was done in the operation room immediately before the procedure. For the duration of the study, the assigned treatment was unknown to patients, physicians and other outcome assessors. Long-term efficacy of the procedure was measured by changes in BP (office and ambulatory), whereas safety was assessed by changes in the 24-h protein excretion, serum creatinine and eGFR 3 years post-procedure.Results. Of the 55 randomised patients, 39 (71%) completed a 3-year assessment (21 after distal RDN and 18 after RDN in the main trunk of the renal artery). Compared to baseline, the mean 24-h BP remained significantly lowered in the distal RDN group by −16.9 (95% CI −27.3;−6.5)/−8.5 (95% CI −14.2;−2.9) mmHg, p = 0.002/0.004 (systolic/diastolic, respectively). The lowering of the mean 24-h BP was almost twofold less and non-significant in the group of RDN in the main trunk of the renal artery: −8.7 (95% CI −19.6;2.2)/−5.8 (95% CI −11.8;0.1).Conclusion. Anatomical optimisation of endovascular renal denervation produces a powerful and sustained increase in the efficacy of the intervention.Received 19 May 2020. Revised 10 July 2020. Abstract

Objective. To evaluate the effi cacy and safety of transcatheter renal denervation (TRD) in drug-resistant hypertension in a single-institution study (NCT01499810). Design and methods . Subjects with essential hypertension aged 18–80 years with blood pressure (BP) > 160/100 mmHg despite three or more antihypertensive drugs who signed written informed consent were included in the study. Exclusion criteria were: mean 24-h systolic BP < 135 mmHg (ambulatory blood pressure monitoring), estimated glomerular fi ltration rate (eGFR) < 30 ml/min/m 2 , severe concomitant diseases. TRD was performed as 4–8 point radiofrequency (RF) ablations in each renal artery using standard EP catheter 5F. Effi cacy endpoints: changes in offi ce and ambulatory BP at 6 and 12 months after TRD. Safety endpoints: adverse events, changes in renal blood fl ow (ultrasound dopplerography) and renal function (proteinuria, serum creatinine, eGFR) at 1 st week, 6 and 12 months after TRD. Results. Interim analysis was done to monitor safety of TRD. The intervention was performed in 45 patients (aged 52,9 ± 12,2 years, 22 male), 29 patients were examined at 6 months and 16 patients — at 12 months after TRD. Intraoperative angiography showed no damage of renal arteries due to RF ablation. There were 5 minor periprocedural complications: small subcapsular kidney hematoma (80 ml by magnet-resonance tomography), contrast nephropathy, 2 access site pseudoaneurysms, allergy to dye. No additional events, signifi cant changes in renal blood fl ow or renal function were found at 6 and 12 months of follow-up. Offi ce BP fell by -34,8/-17,2 mmHg (p < 0,00001/0,00001) at 6 months and -41,0/-24,1 mmHg (p < 0,0001/0,0001) at 12 months after TRD (systolic/diastolic, respectively). Mean 24-h BP decreased by -11,1/-7,1 (p < 0,001/0,001) and 20,8/-13,1 mmHg (p < 0,0001/0,0001), respectively.
