Resume La culpabilite fait partie integrante de l’experience humaine et tout parent est confronte a ce sentiment, particulierement lors du processus diagnostique de leur enfant. Dans cette recherche qualitative, nous avons tout d’abord explore dix forums internet francophones traitant de la question des diagnostics pedopsychiatriques en nous centrant sur l’hyperactivite, l’autisme et le fonctionnement haut potentiel. Cela nous a permis de reperer les questionnements des parents et de creer un canevas d’entretien semi-directif. Onze familles ont participe a l’etude. L’analyse phenomenologique interpretative de ces entretiens montre l’utilite et le soulagement eprouve par les parents a l’annonce du diagnostic, signe d’un sentiment de culpabilite parfois diffus et sous-jacent. La culpabilite a un impact varie suivant les situations, mais elle est toujours presente et constitue un enjeu majeur pour la clinique de l’enfant et de l’adolescent. Quelle que soit la position du clinicien vis-a-vis des questions diagnostiques, entendre et elaborer ces sentiments de culpabilite semble essentiel.

Resume Le bilan psychologique est une demarche tres repandue en psychologie de l’enfant et de l’adolescent, mais peu de travaux scientifiques se sont penches sur le vecu des principaux interesses durant ce processus. La presente etude a ete menee aupres de 105 parents qui se sont adresses pour leur enfant a un service de consultation psychologique, et a investigue par des questionnaires ad-hoc des dimensions qui peuvent etre engagees a differents moments du bilan : les attentes et la perception du bilan, la motivation au changement, l’alliance de travail et la satisfaction relative a la prise en charge. Les resultats mettent en evidence l’importance des representations initiales des parents et de la qualite du premier contact avec les psychologues. Cette etude approfondit notre comprehension d’elements processuels peu etudies et pourtant determinants pour l’engagement des parents dans le bilan psychologique et l’eventuelle intervention qui y donnerait suite.

Psychological examination aims at objectifying the key symptoms of hyperactivity, namely the disorders of attention and of the exe-cutive functions (briefly, the activation-inhibition control). The -records of 237 patients, aged 5 to 17 and attending our day clinics between 2004 and 2016, are analyzed retrospectively. 40 cases present an attention-deficit/hyperactivity disorder (ADHD), combined presentation, after DSM-5 criteria. These children and adolescents show not only a typical impulsivity on the computerized test of -attention, but also some deficit in learning to write, a precocious manifestation of their neurodevelopmental disorders. This comorbidity correctly classifies 82.4 % of the hyperactivity and control cases, a quite strong effect in the context of the hard to reach -diagnosis of the ADHD syndrom.L’examen psychologique vise à objectiver les symptômes clés de l’hyperactivité, à savoir les troubles de l’attention et des fonctions exécutives (en bref, le contrôle activation-inhibition). Les dossiers de 237 patients de nos consultations, âgés de 5 à 17 ans et examinés entre 2004 et 2016, sont analysés rétrospectivement. 40 cas présentent un trouble déficitaire de l’attention avec hyperactivité (TDAH), présentation mixte, selon les critères du DSM-5. Ces enfants et adolescents montrent non seulement une impulsivité caractéristique aux épreuves informatisées de l’attention, mais aussi un déficit dans l’acquisition de l’écriture, une manifestation précoce de leurs troubles neurodéveloppementaux. Cette association classe correctement 82,4 % des hyperactifs et des contrôles, un effet fort, vu le diagnostic difficile du syndrome TDAH.

Background/Aim: Negative postoperative changes in children are frequent and have been described for decades. However, there is currently no theoretical framework, nor any consensual operational criteria for identifying them. This study aims at characterizing the many dimensions involved in postoperative behavioral disturbances in early childhood, using a qualitative analysis applied for the first time to these symptoms. Method: Fifty‐seven parents of preschool children (1‐5 years old; 38 boys), who underwent general anesthesia, were interviewed 10 days after surgery. Semi‐structured interviews investigated behavioral disturbances classically described in preschool children. Qualitative analysis of the transcripts allied both deductive and inductive reasoning, and inductive coding was carried out using constant comparison method with dedicated qualitative software. Results: Parents reported both positive and negative postoperative changes. Negative changes were classified in four main categories: (a) Externalizing and (b) Internalizing problems behaviors, (c) Feeding sleeping disruption and (d) Somatic problems, each comprising different sub‐categories. Importantly within these categories, the symptoms distribution changed in 5 years old children, compared to younger children. Finally, our method allowed defining whether these (negative or positive) changes were significant or not, that is, the importance of postoperative behavioral changes. Conclusion: The results of this study highlight the heterogeneity of postoperative disturbances in preschool children. These results are of primary importance for the definition and measurement of postoperative behavioral disturbances.

The psychological examination aims at objectifying the key symptoms of hyperactivity, i.e. attention and executive function disorders (in short, activation-inhibition control). The files of 237 patients from our consultations, aged between 5 and 17 years old and examined between 2004 and 2016, are analyzed retrospectively. Of whom 40 cases show the typical ADHD syndrome, mixed presentation, according to DSM-5 criteria. These ADHD children and adolescents show not only a characteristic impulsivity on computerized attention tests, but also a deficit in the acquisition of writing, an early manifestation of their neurodevelopmental disorders. Cette etude normative de l’epreuve de Rorschach porte sur une population suisse de 102 enfants et adolescents non-consultants de 8 a 14 ans, repartis par tranches d’âge, sexe et niveau intellectuel. Elle presente un apercu des differentes methodes utilisees dans l’interpretation du Rorschach et des normes francophones existantes. Une reflexion est aussi proposee autour de la question de l’utilite de l’analyse des facteurs quantitatifs au Rorschach. Suite a une presentation detaillee de la methodologie de recherche, les resultats sont exposes et compares aux normes anterieures. Ils ne revelent pas de differences importantes avec ces dernieres, a l’exception de la hausse du F%, de la baisse du F+% et du nombre de Banalites. Les comparaisons Age, Sexe et QI revelent quelques differences entre les groupes.

We performed a cross-sectional study in 450 nonreferred preschool children aged 4 to 6 years to assess the association between hyperactivity/inattention with adiposity and lifestyle characteristics. Measurements included scores of hyperactivity/inattention, adiposity, objectively measured physical activity, television viewing, and eating habits. Higher scores of hyperactivity/inattention were associated with lower percentage body fat, higher levels of physical activity, and less time spent in sedentary activity (all P ≤ .01). However, higher scores of hyperactivity/inattention were also associated with more television viewing and less healthy eating habits (all P ≤ .04). Except for some selected eating habits ( P ≥ .07), those relationships remained significant after adjustment for age, sex, and sociodemographic confounders. To conclude, higher scores of hyperactivity/inattention are linked to different lifestyle characteristics that may in part contribute to a future development of overweight/obesity. Precise mechanisms explaining these associations and possible preventive approaches should be further investigated.

Les auteurs se referent a des concepts issus de l’ecole de psychanalyse britannique de la relation d’objet pour monter comment le processus de « presence therapeutique » peut ameliorer et enrichir les interventions en milieu scolaire. Les benefices engendres par une « presence therapeutique » sur les lieux scolaires et educatifs et les defis rencontres seront discutes. Des illustrations cliniques ainsi que des exemples issus de notre pratique avec les enfants, leur famille et les professionnels de l’enfance, que ce soit dans les ecoles primaires et secondaires, les garderies, les services specialises ou les lieux de formation serviront d’illustration des concepts theoriques presentes.

Il est alors possible, dans un second temps, d’aborder les problematiques personnelles des parents qui desservent leur ajustement aux besoins specifiques de l’enfant.

Parents of children with attention-deficit/hyperactivity disorder (ADHD) frequently report that their children have a poor sense of time. Several studies looking at the perception of time mentioned a form of temporal myopia. The present study investigates the sense of time in children with ADHD. Twenty-two French-speaking Swiss children with ADHD and 22 controls between the ages of 6 and 13 years were tested using a conventional time knowledge questionnaire and two Piagetian time conservation tasks. Parents were asked to complete the “It’s about time” questionnaire. Better performance was observed in matched control group children than in children with ADHD on a conventional time knowledge questionnaire. For children under 10 years of age, the two Piagetian time conservation tasks were able to differentiate children with and without ADHD. Parents of ADHD children reported more frequently that their children had time-related difficulties in daily activities. This study suggests that children with ADHD take longer to develop several time-related abilities.

13. Validation of the French Version of the Hierarchical Personality Inventory for Children (HiPIC)

Abstract. The study was designed to investigate the psychometric properties of the French version and the cross-language replicability of the Hierarchical Personality Inventory for Children (HiPIC). The HiPIC is an instrument to assess the five dimensions of the five-factor model for children. Subjects were 552 children aged between 8 and 12 years, rated by one or both parents. At the domain level, reliability ranged from .83 to .93 and at the facet level, reliability ranged from .69 to .89. Differences between genders were congruent with those found in the Dutch sample. Girls scored higher on Benevolence and Conscientiousness. Age was negatively correlated with Extraversion and Imagination. For girls, we also observed a decrease of Emotional Stability. A series of exploratory factor analyses confirmed the overall five-factor structure for girls and boys. Targeted factor analyses and congruence coefficients revealed high cross-language replicability at the domain and at the facet levels. The results showed that the French version of the HiPIC is a reliable and valid instrument for assessing personality with children and has a particularly high cross-language replicability.

De la dyschronie chez les enfants instables : hypothese d'une difficulte d'instauration d'un temps transitionnel. Du syndrome d'asynergie motrice et mentale de Wallon (1925) au deficit d'inhibition du comportement de Barkley (1997), l'agitation de l'enfant a souvent ete liee a une difficulte a penser, organiser et partager les representations inherentes a la temporalite. La dyschronie, telle que formulee par Gibello (1976), constitue une theorie heuristique pour penser ce defaut de symbolisation du temps. Pour mieux comprendre ces phenomenes, plusieurs epreuves projectives (Rorschach, TAT, dessins) ont ete proposees a un jeune garcon instable dyschronique. Les analyses mettent en evidence une discontinuite des capacites representatives. Si une difference entre un protocole Rorschach qui s'est revele restreint et un TAT abondant en constitue le premier signe, des indices specifiques montrent une difficulte d'instauration d'une aire de jeu imaginaire stable ou plus precisement un manque de continuite dans les phenomenes transitionnels (Winnicott, 1951). Peut-on des lors envisager un lien entre la difficulte de representation des transformations (Gibello, 2004) et ce deficit d'instauration, non plus seulement d'un espace, mais aussi d'un temps ou se representent les transitions ?

Resume L'objectif de cette etude est d'examiner les frequences de consommation d'alcool et de cannabis ainsi que les prevalences d'abus et de dependance associees chez des adolescents tout-venant et delinquants. Quatre-vingt-deux adolescents de sexe masculin (36 delinquants et 42 tout-venant ; âge moyen = 16,10 ± 1,02) ont ete interroges lors d'un entretien structure a l'aide de la version francaise du MINI ( Mini International Neuropsychiatric Interview ). Les resultats indiquent que la majorite des adolescents sans distinction consomment des boissons alcoolisees (91,5 %), avec une prevalence d'abus et de dependance a l'alcool atteignant respectivement 37,3 et 13,3 % des consommateurs. En revanche, la proportion de consommateurs de cannabis est significativement plus elevee dans le groupe delinquant (94,4 vs 65,2 % chez les tout-venant). Globalement, la prevalence d'abus ou de dependance au cannabis chez les adolescents consommateurs est elevee (64,1 %). Toutefois, les adolescents consommateurs du groupe delinquant presentent de maniere statistiquement significative plus de diagnostics d'abus ou de dependance au cannabis que leurs homologues tout-venant (79,4 vs 46,7 %). Ces resultats montrent la gravite potentielle des conduites de consommation d'alcool et de cannabis adoptees par les adolescents. Cette etude souligne la necessite d'evaluer la gravite des conduites de consommation a l'adolescence et de promouvoir des actions preventives et therapeutiques.

Resume L’article presente le modele de la formulation de cas pour l’evaluation psychologique de l’enfant et de l’adolescent. La formulation de cas s’est developpee en reponse au diagnostic psychiatrique qui peut etre percu comme une approche reductionniste du fonctionnement psychique. En Europe, de nombreux psychologues cliniciens ont adopte ce modele, qui a initialement ete developpe par les psychotherapeutes d’orientation cognitivo-comportementale, et l’ont adapte a leur pratique respective. Ce modele, aujourd’hui bien ancre dans la pratique clinique quotidienne, consiste a elaborer differents regards theoriques integrant les resultats de la recherche scientifique pour favoriser la comprehension d’un cas clinique. Ce modele s’inscrit parallelement dans une approche centree sur les preuves et dans une pratique reflexive de la clinique. L’objectif de la formulation de cas est d’elaborer des hypotheses et de proposer des traitements adequats. La famille est toutefois integree dans le processus de discussion et de prise de decision, ce qui permet de renforcer l’alliance therapeutique. Cet article presente les trois courants theoriques les plus utilises dans la formulation de cas : les approches cognitivo-comportementale, psychodynamique et systemique. L’utilisation d’une seule et meme vignette permettra de montrer la pertinence du modele et la complementarite des differentes approches cliniques.

Aim: The psychoanalytic theories of Bion, Anzieu, Berger, and Gibello postulate that the development of thinking depends upon the formation of a psychic space. This thinking space has its origin in the body and in our interpersonal relations. This study aims to validate this psychodynamic hypothesis. Method: A group of 8- to 14-year-old children participated in this research. The presence of a thinking space was operationalized by the Barrier and Penetration scores on the Rorschach’s Fisher and Cleveland scales, and intellectual efficiency was measured using a short version of the WISC-IV. Results: Results show that extreme scores on the Barrier and Penetration variables predict a lower intellectual level than average scores on the same variables. Conclusion: The development of thinking and personality are undoubtedly linked and the Barrier and Penetration variables are useful measures when evaluating the development of a space for thought.

Current explanatory models for binge eating in binge eating disorder (BED) mostly rely on models for bulimia nervosa (BN), although research indicates different antecedents for binge eating in BED. This study investigates antecedents and maintaining factors in terms of positive mood, negative mood and tension in a sample of 22 women with BED using ecological momentary assessment over a 1-week. Values for negative mood were higher and those for positive mood lower during binge days compared with non-binge days. During binge days, negative mood and tension both strongly and significantly increased and positive mood strongly and significantly decreased at the first binge episode, followed by a slight though significant, and longer lasting decrease (negative mood, tension) or increase (positive mood) during a 4-h observation period following binge eating. Binge eating in BED seems to be triggered by an immediate breakdown of emotion regulation. There are no indications of an accumulation of negative mood triggering binge eating followed by immediate reinforcing mechanisms in terms of substantial and stable improvement of mood as observed in BN. These differences implicate a further specification of etiological models and could serve as a basis for developing new treatment approaches for BED.
