Despite significant progress regarding clinical detection/imaging evaluation modalities and genetic/molecular characterization of pathogenesis, advanced renal cell carcinoma (RCC) remains an incurable disease and overall RCC mortality has been steadily rising for decades. Concomitantly, clinical definitions have been greatly nuanced and refined. RCCs are currently viewed as a heterogeneous series of cancers, with the same anatomical origin, but fundamentally different metabolisms and clinical behaviors. Thus, RCC pathological diagnosis/subtyping guidelines have become increasingly intricate and cumbersome, routinely requiring ancillary studies, mainly immunohistochemistry. Meanwhile, RCC-associated-antigen targeted systemic therapy has been greatly diversified and emerging, novel clinical applications for RCC immunotherapy have already reported significant survival benefits, at least in the adjuvant setting. Even so, systemically disseminated RCCs still associate very poor clinical outcomes, with currently available therapeutic modalities only being able to prolong survival. In lack of a definitive cure for advanced RCCs, integration of the amounting scientific knowledge regarding RCC pathogenesis into RCC clinical management has been paramount for improving patient outcomes. The current review aims to offer an integrative perspective regarding contemporary RCC clinical definitions, proper RCC clinical work-up at initial diagnosis (semiology and multimodal imaging), RCC pathological evaluation, differential diagnosis/subtyping protocols, and novel clinical tools for RCC screening, risk stratification and therapeutic response prediction.

Image compression is a vital component for domains in which the computational resources are usually scarce such as automotive or telemedicine fields. Also, when discussing real-time systems, the large amount of data that must flow through the system can represent a bottleneck. Therefore, the storage of images, alongside the compression, transmission, and decompression procedures, becomes vital. In recent years, many compression techniques that only preserve the quality of the region of interest of an image have been developed, the other parts being either discarded or compressed with major quality loss. This paper proposes a study of relevant papers from the last decade which are focused on the selection of a region of interest of an image and on the compression techniques that can be applied to that area. To better highlight the novelty of the hybrid methods, classical state-of-the-art approaches are also analyzed. The current work will provide an overview of classical and hybrid compression methods alongside a categorization based on compression ratio and other quality factors such as mean-square error and peak signal-to-noise ratio, structural similarity index measure, and so on. This overview can help researchers to develop a better idea of what compression algorithms are used in certain domains and to find out if the presented performance parameters are of interest for the intended purpose. The development of communication networks has an increasingly important role in the development and modernization of the fields of activity of agriculture. This trend has effects between different actors in the production chain and creates a tighter connection between the stages of land cultivation or the maintenance of natural pastures, ecological grazing, milk, and meat production, respectively the preparation of the final products and their distribution, including marketing techniques. In the paper, an analysis will be made of the specialized publications in the field of Agribusiness in Romania and the effectiveness of the various communication channels used in the present will be discussed with the aim of contributing to mitigating the current problems of natural grazing activities and identifying the main pillars of development and progress. This research paper utilizes an academic research methodology aiming to highlight the importance of communication networks in promoting natural grazing activities. The results of the study demonstrate that the establishment of networks can significantly contribute to the modernization of the Romanian agricultural sector, aligning it with the principles of sustainable competitiveness and resilience. In conclusion, this study provides evidence that the establishment of such networks is essential for the development of a sustainable and resilient agricultural sector in Romania. The infraorbital complex belongs to the maxillary bone and it is an important landmark in dentistry, especially for loco-regional anaesthesia. For this study, bilateral measurements of the components of the infraorbital complex on twenty dry dentated human skulls were performed. Results: the average length of the the infraorbital groove (IOG) was 12.55 mm and the average length of the infraorbital canal (IOC) was 12.92 mm. Regarding the infraorbital foramen (IOF), its shape was oval in 65% of the cases and the average distance between the infraorbital foramen and the inferior orbital margin (IOF-IOM distance) was 7.87 mm. IOF was located on the vertical line drawn between premolars in 50% of the cases and on the vertical line drawn through the axis of the maxillary second premolar in 45% of the cases. The morphometric data obtained can be useful in dentoalveolar surgery.

The automotive industry is developing continuously, trying to improve, among others, the safety of drivers, passengers, and pedestrians. Using modern technology, the dangers caused by weather hazards like rain, snow, fog, or glare were identified and reduced. This paper presents an anti-glare solution using existing technologies that can be found already in a high-end car like the driver’s eyes tracking systems, light intensity sensors, or head-up displays. In addition to the existing elements, a sun tracking sensor is required to detect the point where the sun light has the maximum intensity on the windshield surface. Knowing the driver’s position and the point on the windshield where the sunlight has a high intensity, a dark spot can be created on the windshield in order to reduce the discomfort created by glare. Depending on the intensity of the light and taking into consideration the traffic safety laws, the spot’s transparency can vary between certain limits. Therefore, the dangers caused by glare will be diminished and the risks of not observing pedestrians, other traffic participants, traffic signs, or sudden curves will be considerably lower. Another advantage of using a digital sunshade based on smart glass or in-glass transparent displays, instead of a regular sunshade, is that the whole windshield can be protected against glare not just the top of it. The results were verified and highlighted using computer simulations done via a MATLAB environment.

Background/Objectives : Radical cystectomy (RC) represents one of the most complex and morbid surgical procedures in the field of Urology. Extraperitoneal open RC has emerged as an alternative to the traditional transperitoneal approach for the treatment of muscle-invasive bladder cancer. Frailty is one of the most important risk factors for perioperative morbidity and mortality, and this category of patients can benefit the most from the extraperitoneal approach. The purpose of this study was to evaluate the feasibility and the safety of extraperitoneal open RC in our experience; Methods : We retrospectively collected the data of 75 frail patients who underwent an extraperitoneal open RC, performed by a single experienced surgeon. We assessed their frailty status using the simplified frailty index (sFI). We recorded data regarding general characteristics, intraoperative, pathological, and postoperative complications, and mortality (within 90 days); Results: We analyzed 61 males and 14 females with an sFI equal to or higher than 3. The median age was 77 years. Fifty-one patients had an ASA score of 3 or more. Sixty procedures were with radical intention, while fifteen were palliative. Cutaneous ureterostomy was performed in 70 cases and extraperitonized ileal conduit in five cases. The median operative time was 150 min. The median blood loss was 400 mL. The median time to flatus was 2 days. The median postoperative stay was 7 days. Thirteen patients had Clavien-Dindo III or IV complications. Two patients died in first 90 days postoperatively; Conclusions : The extraperitoneal open RC in frail patients was demonstrated to be a feasible and safe alternative approach in definitive treatment or a palliative setting in our experience.
