Within the trial "Jena-PARADISE" a primary care practice team-supported exposure training for patients with panic disorder with or without agoraphobia was developed and evaluated. The present paper gives an overview of general practitioners' subjective views on the practicability, feasibility and effectiveness of this new intervention for both patients and GP teams. METHODS: Questions were operationalized based on Bellg's intervention fidelity framework. Fourteen GPs of the intervention group were sampled purposefully and interviewed in a semi-structured way. Generated data were analyzed following Mayring's content analysis approach. RESULTS: The treatment program was positively assessed among the GPs and seen as a useful therapeutic option for inadequately treated patients. The therapy elements 'psycho-education' and 'interoceptive exposure exercises' were described as feasible, while situational exercises and relapse prevention got a less positive rating. The active participation of the nurse in the treatment program was seen as supportive. CONCLUSION: From the GP perspective, the treatment program for patients with panic disorder and/or agoraphobia seems to be a viable therapeutic option in primary care.

The evidence-based guidelines of the German Society of General Practice and Family Medicine (DEGAM) are developed according to an established ten-stage plan. The twelfth guideline addresses the health care problem of dementia. The main focus is the sensitive handling of patients and their relatives, e.g., although the goal is an early diagnosis, patients should also be protected against overdiagnosis. The treatment of Alzheimer's disease consists of three components: optimal care and supervision, the use of nondrug therapy, and a critically weighed administration of medications. The successful implementation of the guideline should be achieved by a multimodal strategy. This includes not only the direct integration of general practitioners (GPs) in developing the guideline, but also a comprehensive publication offensive, a wide use in training sessions including new media, the testing of complex implementation approaches in trials, and the use of the guideline in the context of GP contracts.

Introduction: This survey aimed to investigate German ambulatory physicians' opinions about mandatory continuing medical education (CME) and CME resources shortly before the introduction of mandatory CME in 2004. Methods: A structured national telephone survey of general practitioners and specialists was conducted. Main outcome measures were opinions about mandatory CME, preferred CME media, and financial aspects of CME. Data analysis applied Pearson's correlation coefficient for explorative analysis of correlations, analysis of variance for group comparison, and 2-test for investigation of distribution of 2 or more categorical variables. Results: Of the 511 participants, 73% felt that CME supported them in keeping up to date with new medical knowledge, yet only half believed in its ability to minimize medical errors or increase quality of patient care. Traditional ways of learning were preferred. Significant differences in CME behavior were found between primary-care physicians and specialists, the latter using more communicative media. Also, a small group of very skeptical physicians who do not agree with the need for CME in general was identified. Average expenditure for CME was estimated between 500 and 1000 Euros per year. A majority of physicians called for other parties, eg, health insurances or government, to share the financial responsibility for CME. Discussion: Mandatory CME was accepted as a necessity already before the new legislation took effect, with prevailing skepticism at the same time. Future research is needed on how to motivate physicians and especially certain subgroups of physicians to use CME methods shown to be effective in changing clinical behavior.

Menschen mit einer Demenz-Erkrankung werden auf Grund der demografischen Entwicklung in den nächsten Jahren immer häufiger in der Praxis anzutreffen sein. Die neue evidenzbasierte Leitlinie zur Demenz der Deutschen Gesellschaft für Allgemeinmedizin und Familienmedizin (DEGAM) wurde von Hausärzten für Hausärzte entwickelt.

Objective: E-learning has the potential to provide effective education for general practice, but there are significant difficulties that must be overcome. Design: We initiated a two-round Delphi study, aiming to identify expectations and barriers to e-learning in primary healthcare education. Methods: We distributed questionnaires to 60 primary care experts who are also experts in the field of e-learning. Their responses were independently analysed by two of the authors (J.G., H.C.V.) and were clustered to form 32 themes. These were fed back to the participants in a second postal questionnaire with the objective of reaching agreement or disagreement, with a cut-off of 80%. Results: The response rate was 67% (n=40) in the first and 60% (n=36) in the second round. The extent of agreement reached ranged from 8% (e-learning is displacing practical teaching and learning") to 97% ("e-learning needs convincing didactical concepts"). Agreement was high with the themes "e-learning gets a new focus by mixed learning concepts" and "users will have a higher level of media competence 5 years from now" (94% each). There was a positive attitude to e-learning, but there was concern about the lack of orientation towards users' needs and the poor development of innovative didactical concepts. In implementing e-learning in primary care, education should be independent of financial influence from the healthcare industry in order to eliminate conflicts of interest. Conclusion: The experts' responses show that e-learning in primary healthcare education can contribute substantially to undergraduate, graduate, and continuing medical education, and should therefore be evaluated in systematic studies.

Background The management of hypertension is a key challenge in modern health systems. This study aimed to investigate hypertension treatment strategies among physicians specialized in complementary and alternative medicine (CAM) in Germany by analysing prescribing patterns and comparing these to the current treatment guidelines issued by the German Hypertension Society. Methods In this prospective, multicentre observational study, which included 25 primary care physicians specialized in CAM treatment, prescriptions and diagnoses were analysed for each consecutive hypertensive patient using routine electronic data. Data analysis was performed using univariate statistical tests (Chi square test, Cochran-Armitage trend test). Multiple logistic regression was used to determine factors associated with antihypertensive medication. Results In the year 2005, 1320 patients with 3278 prescriptions were included (mean age = 64.2 years (SD = 14.5), 63.5% women). Most patients were treated with conventional antihypertensive monotherapies (n = 838, 63.5%). Beta-blockers were the most commonly prescribed monotherapy (30.7%), followed by ACE inhibitors (24.0%). Combination treatment usually consisted of two antihypertensive drugs administered either as separate agents or as a coformulation. The most common combination was a diuretic plus an ACE inhibitor (31.2% of dual therapies). Patient gender, age, and comorbidities significantly influenced which treatment was prescribed. 187 patients (14.2%) received one or more CAM remedies, most of which were administered in addition to classic monotherapies (n = 104). Men (OR = 0.66; 95% CI: 0.54-0.80) and patients with diabetes (OR = 0.55; 95% CI: 0.42-0.0.73), hypercholesterolaemia (OR = 0.59; 95% CI: 0.47-0.75), obesity (OR = 0.74; 95% CI: 0.57-0.97), stroke (OR = 0.54; 95% CI: 0.40-0.74), or prior myocardial infarction (OR = 0.37; 95% CI: 0.17-0.81) were less likely to receive CAM treatment. Conclusions The large majority of antihypertensive treatments prescribed by CAM physicians in the present study complied with the current German Hypertension Society treatment guidelines. Deviations from the guidelines were observed in one of every seven patients receiving some form of CAM treatment.

Background Several studies suggest that General Practitioners (GPs) prefer "traditional" media such as journals or quality circles when they are seeking out different options to meet their continuing medical education (CME) requirements. A survey was designed in order to gain a better understanding of German General Practitioners` preferences for different forms of educational media that will meet their CME needs. Methods Four hundred and forty nine (N=449) German physicians were contacted to take part in this study on the occasion of one of their quality circle meetings. The participating physicians received a standardized 26-item-questionnaire that surveyed their preferences for different forms of educational media. A factor analysis was performed in order to determine whether the observed variables can be explained largely or entirely in terms of the underlying patterns. Results Two hundred and sixty-four physicians with an average age of 51.1 years participated (28.5% female, 71.5% male). We found that GPs favor learning environments such as: journals, colleagues, and quality circles. New media like the internet was used less often for their learning activities, even though the usage of the internet in general was quite high. The most important requirements for media in medical education as perceived by the participants were its relevancy for daily practice and dependability.

Hintergrund und Ziel: Untersuchungen zeigen, dass Hausärzte bei der Wahl ihrer Fortbildungsmedien eher traditionelle Varianten wie Fachzeitschriften oder Qualitätszirkel bevorzugen. In der vorliegenden Arbeit wurde im Jahr 2007 untersucht, welche Fortbildungsmedien aktuell genutzt und nachgefragt wurden, wie deren Effizienz beurteilt wurde und welche Anforderungen Hausärzte an Fortbildungsmedien stellten. Durch den Vergleich mit Daten, die 2001 im selben Kollektiv erhoben worden waren, war die Überprüfung etwaiger Veränderungen bei der Präferenz für bestimmte Fortbildungsmedien möglich. Methodik: Mit einem standardisierten Fragebogen zu den Themenkomplexen "Anteil des nach dem Studium erworbenen Wissens", "Nutzung und Effizienz ärztlicher Fortbildungsmedien" und "Anforderungen an Fortbildungsmedien" wurden 57 hausärztlich niedergelassene Ärztinnen (23%) und Ärzte (77%) befragt. Alle Teilnehmer hatten den Fragebogen bereits 6 Jahre zuvor ausgefüllt; mit Hilfe des Rangkorrelationskoeffizienten ?c von Kendall wurden die Korrelationen ermittelt. Zusätzlich wurden das Vorhandensein von Internetzugang und PDA ("personalisierte digitale Assistenten") sowie die Internetnutzung erfasst. Ergebnisse: Zu beiden Befragungszeitpunkten gaben viele Ärzte an, für ihre Fortbildung vor allem Qualitätszirkel, wissenschaftliche Zeitschriften und Kollegen, weniger das Internet, Pharmareferenten oder wissenschaftliche Mitarbeiter von Universitäten zu nutzen. Die angegebene Nutzung korrelierte positiv mit der Einschätzung der Effizienz. Die Anforderungsattribute, die Ärzte an Fortbildungsmedien stellten, waren vor allem "zuverlässig" und "praxisrelevant"; "interaktiv" oder "bildhaft" wurden am wenigsten gefordert. Im Mittel bestand eine hohe Übereinstimmung der Einschätzung in beiden Jahren. Schlussfolgerung: Die vorliegende Befragung zeichnete ein Bild von der Fortbildungssituation und den persönlichen Präferenzen von 57 primärärztlich tätigen Medizinern im 6-Jahres-Vergleich. Dabei wurde deutlich, dass trotz einer relativ hohen Nutzungsrate neuer Medien, die klassischen Lernwege für die persönliche Fortbildung favorisiert wurden und die Präferenzen über 6 Jahre relativ konstant blieben.

Eine Kausaltherapie der Alzheimer- oder vaskulären Demenz existiert bisher nicht. Ziele sollten deshalb eine Verzögerung der Progression und die Verbesserung der Versorgungssituation von Patienten und Angehörigen sein. Dazu eignet sich ein multimodales Behandlungskonzept auf 3 Ebenen.

Dieser Artikel soll den in der Primärversorgung tätigen Ärzten wissenschaftlich begründete Hinweise für das Erkennen einer Demenz und Empfehlungen für die Versorgung von Menschen mit Demenz geben. Zu den therapeutischen Optionen wird es einen Folgebeitrag geben. Es ist wichtig, adäquate Instrumentarien zu entwickeln und zu evaluieren, die Wissen, Einstellungen und Handlungskompetenz bei Hausärzten im Umgang mit dem Krankheitsbild "Demenz" nachhaltig fördern. Um diese Ziele zu erreichen, werden im folgenden Beitrag die Kernaussagen einer hausärztlichen Leitlinie zur Demenz dargestellt. Um den Zusammenhang zur Leitlinie transparent zu machen, werden die Empfehlungen am Rand mit den Empfehlungsstärken der Leitlinie dargestellt: A basiert auf wissenschaftlichen Studien hoher Qualität, B auf sonstigen Studien und C auf Konsensusaussagen oder Expertenurteilen.

Ein effizienter Wissenstransfer aus der Forschung in die Praxis ist eine Grundbedingung für ein modernes Gesundheitssystem. Evidenzbasierte Leitlinien versuchen Wissen zu speziellen Versorgungsproblemen zu bündeln und zu vermitteln, finden aber oft nicht die gewünschte Verbreitung. Etliche Barrieren für eine erfolgreiche Implementierung konnten identifiziert werden; ebenso einige Lösungsstrategien. Beispielsweise versucht das DEGAM-Konzept die Akzeptanz von Leitlinien bei Hausärzten durch Praxistests zu erhöhen. Zukünftig sind organisationales Lernen und individuelle Lernpräferenzen stärker zu berücksichtigen. Eine Integration von neuem Wissen in den medizinischen Alltag kann gelingen, sofern sich die (Informations-)Technologie den Bedürfnissen der Versorgungsrealität anpasst. Technologische oder Versorgungsmaßnahmen sind im Sinne einer evidenzbasierten Gesundheitsversorgung kritisch zu evaluieren und hinsichtlich ihrer Praxistauglichkeit zu überprüfen.

Das Bundesministerium für Bildung und Forschung wird angesichts der großen gesellschaftlichen Bedeutung der Demenz ein Deutsches Zentrum für Neurodegenerative Erkrankungen (DZNE) unter dem Dach der Helmholtz Gemeinschaft fördern. Das vor allem naturwissenschaftlich ausgerichtete Zentrum in Bonn soll sechs Partnerstandorte haben, von denen eines an der Universität Witten/Herdecke angesiedelt sein wird. Dieses nennt sich "Institut für Forschung und Transfer in der Pflege und Behandlung von Menschen mit Demenz" und soll in drei inhaltliche Schwerpunkte arbeiten: Versorgungsstrukturen, Versorgungsmaßnahmen, Wissenszirkulation und Wissenstransfer. Die geplanten Schwerpunkte werden in dem folgenden Artikel kurz umrissen.
