Abstract

The study attempts to evaluate the C-Peptide levels after autologous nonmyeloablative hematopoietic stem cell transplantation (HSCT) among newly diagnosed type 1 diabetes mellitus. The results indicate that C-Peptide levels increased significantly following HSCT and patients exhibited insulin independence with good glycemic control.

Abstract

The safety and metabolic effects of high-dose immunosuppression were determined, followed by autologous nonmyeloablative hematopoietic stem cell transplantation (AHST) in newly diagnosed type 1 diabetes mellitus (DM). With AHST, beta cell function was found to be increased in all but 1 patient and induced prolonged insulin independence in the majority of the patients.

Abstract

Mesenchymal stem cells (MSCs) are known to induce the conversion of activated T cells into regulatory T cells in vitro . The marker CD69 is a target of canonical nuclear factor kappa-B (NF- B) signalling and is transiently expressed upon activation; however, stable CD69 expression defines cells with immunoregulatory properties. Given its enormous therapeutic potential, we explored the molecular mecha- nisms underlying the induction of regulatory cells by MSCs. Peripheral blood CD3 T cells were activated and cultured in the presence or absence of MSCs. CD4 cell mRNA expression was then characterized by microarray analysis. The drug BAY11-7082 (BAY) and a siRNA against v-rel reticuloendotheliosis viral oncogene homolog B (RELB) were used to explore the differential roles of canoni cal and non-canonical NF- B signalling, respectively. Flow cytometry and real-time PCR were used for analyses. Genes with immunoregulatory functions, CD69 and non-canonical NF- B subunits (RELB and NFKB2) were all expressed at higher levels in lymphocytes co-cultured with MSCs. The frequency of CD69 cells among lymphocytes cultured alone progressively decreased after activation. In contrast, the frequency of CD69 cells increased significantly following activation in lymphocytes co-cultured with MSCs. Inhibition of canonical NF- B signalling by BAY immediately following activation blocked the induction of CD69; however, inhibition of canonical NF- B signalling on the third day further induced the expression of CD69. Furthermore, late expression of CD69 was inhibited by RELB siRNA. Thes e results indicate that the canonical NF- B pathway controls the early expression of CD69 after activation; however, in an immunoregulatory con- text, late and sustained CD69 expression is promoted by the non-canonical pathway and is inhibited by canonical NF- B signalling.

Nesta revisão são discutidas várias alternativas de regeneração do conjunto de células produtoras de insulina do pâncreas, usando células-tronco embrionárias do cordão umbilical e adultas, e o trabalho que está sendo realizado em nosso grupo de pesquisas utilizando imunossupressão em altas doses combinada com a infusão de células-tronco hematopoéticas autólogas em diabete do tipo 1 recém-diagnosticado. In this review, we discuss several alternatives for the regeneration of the pool of insulin-producing cells by the pancreas using embryonic, cord blood or adult stem cells and the work being carried out by our research group using high dose immunosuppression with autologous hematopoietic stem cells in newly diagnosed type 1 diabetes mellitus.

Abstract

Invasive infection by Fusarium sp. is associated with high mortality in patients with hematologic cancer. Yet to the authors' knowledge, little is known regarding predictors of adverse outcome. The authors conducted a retrospective review of the records of patients with hematologic carcinoma and invasive fusariosis who were treated at one institution in the U.S. and at 11 centers in Brazil. The records of 84 patients were evaluated. Neutropenia was present in 83% and 33 patients had undergone stem cell transplantation. Only 18 patients (21%) were alive 90 days after the diagnosis of fusariosis. Multivariate predictors of poor outcome were persistent neutropenia (hazard ratio [HR] of 5.43; 95% confidence interval [95% CI], 2.64–11.11) and use of corticosteroids (HR of 2.18; 95% CI, 1.98–3.96). The actuarial survival rate of patients without any of these factors was 67% compared with 30% for patients who recovered from neutropenia but were receiving corticosteroids and 4% for patients with persistent neutropenia only. None of the patients with both risk factors survived (P &lt; 0.0001). Measures to reduce the duration of neutropenia, as well as the judicious use of corticosteroids, may reduce the high mortality rate of fusariosis in patients with hematologic cancer.
