1. Minimizing complications during retropubic radical prostatectomy - is ureteral stenting necessary?

Abstract

Abstract Objectives and aims To avoid damage to the ureters during bladder neck preparation in radical prostatectomy for prostate cancer, it may be helpful to insert ureteral stents temporarily or to intravenously administer indigo carmine dye for enhanced visualisation of ureteric orifices. We evaluated our bladder neck preserving technique at radical prostatectomy with regard to ureteric injuries. Patients and methods We analysed 369 consecutive radical prostatectomies operated in our clinic in a bladder neck preserving technique. The following parameters were assessed in this retrospective study: number of prophylactic ureteric stent insertions, application of indigo carmine dye, observed injuries of the ureters by the surgeon, postoperative increase of serum creatinine and postoperative status of kidney ultrasound. Results In 7/369 prostatectomies (1.90%) a ureteric stent insertion was performed, indigo carmine was not applied to any patient at all, yet no intraoperative injury of a ureter was observed by a surgeon. No revision was necessary due to a ureteral injury within the observation period of one year after surgery. In 17 patients with preoperative normal creatinine value a pathological value was observed on the first postoperative day (mean 1.4 mg/dl). In these patients no consecutive postrenal acute renal failure was observed, no hydronephrosis was monitored by ultrasound and no further intervention was necessary. Conclusions Bladder neck preserving operation technique does not implicate the need of prophylactic ureteric stent insertions and has no higher incidence of ureteric injuries.

Abstract

Three independent searches for an electric dipole moment (EDM) of the positive and negative muons have been performed, using spin precession data from the muon g-2 storage ring at Brookhaven National Laboratory. Details on the experimental apparatus and the three analyses are presented. Since the individual results on the positive and negative muon, as well as the combined result, d=-0.1(0.9)E-19 e-cm, are all consistent with zero, we set a new muon EDM limit, |d| < 1.9E-19 e-cm (95% C.L.). This represents a factor of 5 improvement over the previous best limit on the muon EDM., Comment: 19 pages, 15 figures, 7 tables

Abstract

The Cryogenic Underground Observatory for Rare Events (CUORE) is the first cryogenic experiment searching for neutrinoless double-beta (0vßß) decay that has been able to reach the one-ton scale. The detector, located at the Laboratori Nazionali del Gran Sasso in Italy, consists of an array of 988 TeO2 crystals arranged in a compact cylindrical structure of 19 towers. Following the completion of the detector construction in August 2016, CUORE began its first physics data run in 2017 at a base temperature of about 10 mK. Following multiple optimization campaigns in 2018, CUORE is currently in stable operating mode. In 2019, CUORE released its 2nd result of the search for 0vßß with a TeO2 exposure of 372.5 kg·yr and a median exclusion sensitivity to a 130Te 0vßß decay half-life of 1.7 · 1025 yr. We find no evidence for 0vßß decay and set a 90% C.I. (credibility interval) Bayesian lower limit of 3.2 · 1025 yr on the 130Te 0vßß decay half-life. In this work, we present the current status of CUORE's search for 0vßß, as well as review the detector performance. Finally, we give an update of the CUORE background model and the measurement of the 130Te two neutrino double-beta (2vßß) decay half-life.

Abstract

The purpose of this study was to assess the diversity of physiological stress responses elicited by tethering versus loose holding in the Swedish domestic goat (Capra hircus). Eight goats were randomly divided into two groups and the experiment was performed in a crossover design. Six goats had implanted transmitters that registered heart rate and arterial blood pressure telemetrically and blood samples were taken on separate days. Heart rate increased around feeding both when the goats were held loose in pairs, and when they were tied up. When the goats were tethered, the heart rates and blood pressures were higher, and the concentrations of β-endorphin and oxytocin lower, compared to when held loose in pairs. In conclusion, housing and the company of another animal affect arterial blood pressure, heart rate, and the concentrations of β-endorphin and oxytocin in goats, but the cortisol and vasopressin concentrations did not differ between the treatments. The higher arterial blood pressure and heart rate, and lower concentrations of β-endorphin and oxytocin in tethered goats indicate that being tied up may be more stressful for the goats compared to being held loose. The use of single measures, eg cortisol concentrations, was not sufficient to give an accurate picture of the animals' response to the management system. This study demonstrates the importance of using a wide variety of physiological measures when evaluating stress in animal welfare research. [ABSTRACT FROM AUTHOR]

Abstract

The muon anomalous magnetic moment measurement, when compared with theory, can be used to test many extensions to the standard model. Abstract

In the muon (g - 2) experiment at Brookhaven National Laboratory, the spin precession frequency omega(a) is obtained from a standard chi(2) minimization fit applied to the time distribution of decay electrons. The unusually high accuracy (similar to 0.5 ppm) of the experiment puts stringent requirements on the quality of the fit and the level of understanding of the statistical properties of the fitted parameters. We discuss the properties of the fits and their implications on the derived value for omega(a), including estimates of the effect of an imperfect fit function, methods of including additional external information to reduce the error, the effects of splitting the data into many smaller subsets of data, applying different weighting methods to the data using energy information, and various tests of data suitability. Abstract

The muon anomalous magnetic moment has been measured in a new experiment at Brookhaven. Polarized muons were stored in a superferric ring, and the angular frequency difference, omega(a), between the spin precession and orbital frequencies was determined by measuring the time distribution of high-energy decay positrons. The ratio R of omega(a) to the Larmor precession frequency of free protons, omega(p), in the storage-ring magnetic field was measured. We find R = 3.707 220(48) X 10(-3). With mu(mu)/mu(p) = 3.183 345 47(47) this gives a(mu+) = 1 165 925(15) X 10(-9) (+/-13 ppm), in good agreement with the previous CERN measurements for mu(+) and mu(-) and of approximately the same precision.

44. Different Strokes for Impaired Folks: A Medical-Legal Case Report on Psychosis and Grave Disability following Cerebrovascular Accident.

Abstract

At least 30% of cerebrovascular accident (CVA) survivors suffer from neuropsychiatric symptoms as a direct sequela of their stroke, with poor long-term outcomes due to underdiagnosis and undertreatment. A 2018 systematic review of 76 studies looking at psychotic symptoms following stroke found an increased prevalence in delusional disorders, schizophrenia-like diagnoses, and mood disorders with congruent psychotic features. Right hemispheric lesions accounted for approximately 80% of patients demonstrating subsequent psychotic symptoms in studies with identification of lesion location. Among the many issues arising in management of these patients in addition to behavioral stabilization is decision-making concern should the patient be deemed gravely disabled. In this report, we present a case of a 58-year-old male with psychosis and mood disturbances following right-hemispheric ischemic stroke, and the resulting ethical and legal discussions regarding conservatorship and decision-making capacity. Case report: this is the case of a 58-year-old male with psychiatric history of posttraumatic stress disorder, methamphetamine use disorder, cannabis use disorder and medical history pertinent for hypertension, hyperlipidemia, coronary artery disease, and dilated ischemic cardiomyopathy who was hospitalized for subacute onset of altered mental status following ischemic cerebrovascular accident. Magnetic resonance imaging (MRI) two weeks following initial vascular event confirmed right hemispheric ischemic stroke with subsequent hemorrhagic conversion. Additionally, in the two weeks after the initial vascular event, the patient had begun to exhibit significant behavioral changes mostly notable for an increase in psychotic and labile mood symptoms not previously present at patient's baseline mental status. These included auditory hallucinations, disorganized thinking and behavior, avolition, hypersexuality, emotional lability, irritability, and distractibility that persisted over the course of the following six weeks. He did not have a history of a bipolar disorder or a psychotic disorder, and had recently completed rehabilitation of polysubstance use at the West LA Veterans Administration Domiciliary. Urine drug screen was negative. Patient was managed by neurosurgery and neurology, with psychiatry consultation-liaison team providing recommendations for behavioral management. Psychiatry advised an involuntary hold for grave disability owing to patient's ongoing psychotic symptoms following medical stabilization. Of pertinent concern was the degree of to which patient's behavioral symptoms were interfering with his decision-making. Throughout the course of his hospitalization, patient needed repeated encouragement to participate in assessment, vitals, labs, and med administration, all of which he would refuse on a regular basis without appropriate reason. Bioethics review of patient's case affirmed that he lacked capacity to make major decisions regarding his healthcare and that his girlfriend, who held no legal status with relation to the patient, could serve as medical surrogate. Patient's refusal of medical intervention necessitated psychiatry consultation team to file a petition for prolonged involuntary hold as well as court-ordered medication administration. The patient's psychosis was initially managed with increased doses of his home aripiprazole (30mg), which was later tapered and cross-titrated with risperidone (5mg) for management of his psychotic symptoms and impulse control. Following stabilization of acute medical concerns, patient was subsequently transferred to psychiatric unit for persistent psychosis and for continued psychotropic medication management as well as assessment of legal and social status. The patient's condition ultimately improved to the point of being able to participate in a capacity declaration, designate a durable power of attorney, and forego the need for probate conservatorship. He continues to be seen by psychiatry for ongoing management of his mood, posttraumatic stress disorder, and mild cognitive impairment stemming from vascular insult. Management of psychiatric and behavioral symptoms following stroke continues to be an important component of the post-cerebrovascular vascular accident treatment plan. In particular, contingency plans need to be anticipated preventively in patients who may have higher risk of considerable decompensation following cerebrovascular stroke. In this particular case, the patient carried several premorbid psychiatric, vascular, and social risk factors that converged into a challenging management scenario post-CVA. Abstract

The article presents a study which evaluated the safety and efficacy of a magnetic resonance (MR)-safe and compatible passive guide wire in helping MR-guided cardiac interventions in a swine model. Catheter manipulations were monitored in this study. The study concludes that with the availability of the new MR-compatible and safe guide wire, certain percutaneous cardiac interventions will become feasible to perform under MR guidance in the future.

Abstract

Abstract: The porous TiO2/glass composite Ecopore has potential applications in hard tissue replacement. We describe the modification of Ecopore with the growth factor bone morphogenetic protein-2 (BMP-2) to add osteoinductive properties. Ecopore covalently coated with BMP-2 caused a weak induction of alkaline phosphatase in murine embryonal fibroblasts. In a rabbit bone defect model, BMP-2-coated Ecopore had moderately higher bone apposition rates and ingrown bone quantities at 6 weeks after implantation. To overcome loss of function due to chemical surface coupling, we filled the pore system of Ecopore with heparinized collagen sponge and loaded this secondary matrix with BMP-2. Heparinization of collagen filling increased the BMP-2 loading capacity of the matrix approximately 1.28-fold. Within 96h, 17.0±0.1 and 10.1±0.2% of the used BMP-2 was released from non-modified and heparinized Ecopore/collagen, respectively, indicating that the heparin modification retarded BMP-2 release. Revealed by energy-dispersive X-ray spectroscopy analysis of implant cross-sectional areas, BMP-2-loaded Ecopore/collagen had significantly higher bony ingrowth quantities in rabbits, with the heparinized modification yielding the highest value (16.09±3.51%, p <0.005) compared with the non-heparinized matrix (10.72±4.07%, p <0.05) and the BMP-2-free controls (5.60±1.47%). This suggested a beneficial effect of the biomimetic modification of Ecopore with heparinized collagen for bone healing and integration. [Copyright &y& Elsevier]

49. A STROKE OF BAD LUCK: NEW-ONSET MANIA IN ELDERLY PATIENT FOLLOWING UNDETECTED STROKE, A CASE REPORT.

Abstract

The phenomenon of developing depression after a stroke is fairly common in the literature (29%) but developing mania after a stroke is rare reportedly to be 1-2% of stroke survivors (Starkstein). One of the earliest descriptions of mania after stroke was described in 1989 of the right hemisphere (Starkstein). In 2011, a systematic review looking at 32 studies identified 49 cases describing symptoms of mania post-stroke. At initial presentation, 92% of these patients presented with elevated mood, 71% with increased rate of speech, 69% with insomnia and 63% with agitation (Santos). Mania is more common after strokes of the right hemisphere than the left hemisphere (Santos). There are a few cases in the literature reporting mania after left side infarct but nothing since 2019 (Jampla, Semiz, Fenn). In our case, we describe a case of 65 year old female with mania after left sided stroke. We report a case of a 65 year-old female with a history of major depressive disorder, generalized anxiety disorder, NSTEMI, chronic diastolic heart failure, and non-ischemic cardiomyopathy who was hospitalized for altered mental status and bilateral leg swelling. This patient presented with a two week history of behavioral activation, significant personality changes, emotional lability, fluctuating between states of extreme euphoria and tearfulness, grandiosity, and impulsive intrusiveness, speaking to strangers and hospital staff in inappropriately affectionate terms. She demonstrated a decrease need for sleep, and was noted to be easily irritated and hypersexual with others. She did not have a history of bipolar disorder. Prior to this hospitalization, she had been recently discharged from the cardiac care unit (CCU) after a five-day admission for chest pain, dyspnea, pneumonia, and NSTEMI. Neurology was consulted while patient was hospitalized and recommended imaging of the head for work-up of physiological causes of AMS. Computed tomagraphy (CT) and magnetic resonance imaging (MRI) scans conducted during her hospitalization were remarkable for small, remote, left parietal and left inferior temporal infarcts; the patient nor the patient's family were aware of a previous history of cerebrovascular accident (CVA). Further workup by medical team was unremarkable for organic cause of altered mental status, and patient was subsequently transferred to the inpatient psychiatric facility. Over the course of her hospitalization, the patient was treated and stabilized on divalproex, olanzapine, and temazepam, with steady improvement in her manic symptoms. Patient was discharged after a 3 week-stay on the inpatient psychiatric unit and followed up with outpatient services, where improvement in her manic symptoms continued to be noted. Though her manic behavior improved, she developed depressive symptoms following her hospitalization, a feature often characteristic following manic episodes. She continues to be seen on an outpatient basis and was referred to the geriatric partial hospitalization program for further treatment. Please see below for the discussion in conclusions It is common practice to screen patients for depression after acute stroke due to the increased prevalence of the disease. This case illustrates the need for implementing mania screening in patients post stroke especially if there is involvement of the dominant hemisphere. This patient with no prior history of bipolar disorder presented with an acute presentation of mania followed by a course of depression after sustaining a stroke. Due to the rarity of the postroke mania, we suggest further studies to consider creating an accessible reporting tool for clinicians to record possible cases of mania after a stroke. This will be important diagnostically to appropriately treat patients in contrast to attributing the presentation to other diagnosis such as delirium or neurocognitive disorders.
