These typically involve posterior lateral mass or pedicle screw fixation with significant inherent risks and morbidities. More recently, cervical cages placed bilaterally between the facet joints (posterior cervical cages) have been used as a less disruptive alternative for posterior fixation. The purpose of this study was to compare the stability achieved by both posterior cages and ACDF at a single motion segment and determine the stability achieved with posterior cervical cages used as an adjunct to single- and multilevel ACDF.Methods: Seven cadaveric cervical spine (C2–T1) specimens were tested in the following sequence: intact, C5–C6 bilateral posterior cages, C6–C7 plated ACDF with and without posterior cages, and C3–C5 plated ACDF with and without posterior cages. Study Design: A biomechanical study of human cadaveric lumbar spine segments undergoing disc-space distraction for insertion of anterior lumbar interbody implants. Objective: To measure the distraction force and its relaxation during a period of up to 3 hours after disc-space distraction as a function of the distraction magnitude and disc level. Summary Of Background Data: Interbody implants depend on compressive preload produced by disc-space distraction (annular pretension) for initial stabilization of the implant-bone interface. However, the amount of preload produced by disc-space distraction due to insertion of the implant and its subsequent relaxation have not been quantified. Methods: Twenty-two fresh human lumbar motion segments (age: 51 ± 14.8 years) were used. An anterior lumbar discectomy was performed. The distraction test battery consisted of a tension stiffness test performed before and after each relaxation test, 2 distraction magnitudes of 2 and 4 mm, and a recovery period before each distraction input. The distraction forces and lordosis angles were measured. RESULTS.: Peak distraction force was significantly larger for the 4-mm distraction (431.8 ± 116.4 N) than for the 2-mm distraction (204.9 ± 55.5 N) (P < 0.01). The distraction force significantly decreased over time (P < 0.01), approximating steady-state values of 146.1 ± 47.3 N at 2-mm distraction and 289.8 ± 92.8 N at 4-mm distraction, respectively. The distraction force reduced in magnitude by more than 20% of peak value in the first 15 minutes and reduced by approximately 30% of the peak value at the end of the testing period. The spine segment relaxed by the same amount of force, regardless of the disc level (P > 0.05). Conclusion: The "tightness of fit" that the surgeon notes immediately after interbody device insertion in the disc space degrades in the very early postoperative period, which could compromise the stability of the bone-implant interface. Study design Biomechanical cadaveric study  Objective To compare biomechanical properties of a single stand-alone interbody fusion and a single-level pedicle screw construct above a previous lumbar pedicle fusion. Summary of background data Adjacent segment disease (ASD) is spondylosis of adjacent vertebral segments after previous spinal fusion. Despite the consensus that ASD is clinically significant, the surgical treatment of ASD is controversial. Methods Lateral lumbar interbody fusion (LLIF) and posterior spinal fusion (PSF) with pedicle screws were analyzed within a validated cadaveric lumbar fusion model. L3-4 vertebral segment motion was analyzed within the following simulations: without implants (intact), L3-4 LLIF-only, L3-4 LLIF with previous L4-S1 PSF, L3-4 PSF with previous L4-S1 PSF, and L4-S1 PSF alone. L3-4 motion values were measured during flexion/extension with and without axial load, side bending, and axial rotation. Results L3-4 motion in the intact model was found to be 4.7 ± 1.2 degrees. L3-4 LLIF-only decreased motion to 1.9 ± 1.1 degrees. L3-4 LLIF with previous L4-S1 fusion demonstrated less motion in all planes with and without loading (p < 0.05) compared to an intact spine. However, L3-4 motion with flexion/extension and lateral bending was noted to be greater compared to the L3-S1 construct (p < 0.5). The L3-S1 PSF construct decreased motion to less than 1° in all planes of motion with or without loading (p < 0.05). The L3-4 PSF with previous L4-S1 PSF constructs decreased the flexion/extension motion by 92.4% compared to the intact spine, whereas the L3-4 LLIF with previous L4-S1 PSF constructs decreased motion by 61.2%. Conclusions Stand-alone LLIF above a previous posterolateral fusion significantly decreases motion at the adjacent segment, demonstrating its utility in treating ASD without necessitating revision. Chronic Forward Head Posture is associated with headaches, neck pain, and disability, though few studies have investigated the effects it has on the suboccipital triangle. The objective of this study was to quantitatively assess whether the biomechanical changes in the suboccipital triangle help explain the clinical manifestations of Forward Head Posture. Specifically, this study aimed to identify whether the Greater Occipital Nerve or C2 nerve root may be compressed in Forward Head Posture. Three-dimensional, specimen-specific computer models were rendered from thirteen cadaveric cervical spine specimens. The spines transitioned from neutral to Forward head posture while motion data was collected. This data was synced with the computer models to make precise measurements. In Forward Head Posture, occiput-C1, C1-C2, and occiput-C2 segments extended by 10.7 ± 4.6 deg, 4.6 ± 4.3 deg, and 15.3 ± 2.3 deg, respectively. The Rectus Capitis Posterior Major and Minor and Obliquus Capitis Superior muscles shortened by 20.0 ± 4.6%, 15.0 ± 7.6%, and 6.6 ± 3.3%, respectively. The Obliquus Capitis muscle inferior length did not change. The suboccipital triangle area decreased by 18.7 ± 6.4%, but the protective gaps surrounding the C2 nerve root and the Greater Occipital Nerve did not reveal clinically significant impingement. The C2 nerve root gap decreased by 1.0 ± 1.3 mm and the Greater Occipital Nerve gap by 0.2 ± 0.18 mm. These results demonstrate that the C2 nerve root and the Greater Occipital Nerve are protected by the bony landscape of the cervical spine. However, there is likely persistent contraction of the rectus muscles in Forward Head Posture, which suggests a secondary tension-type etiology of the associated headache., (Copyright © 2019. Background: Cervical fusion is associated with adjacent segment degeneration. Cervical disc arthroplasty is considered an alternative to reduce risk of adjacent segment disease. Kinematics after arthroplasty should closely replicate healthy in vivo kinematics to reduce adjacent segment stresses. The purpose of this study was to assess the kinematics of a polycrystalline diamond cervical disc prosthesis., Methods: Nine cadaveric C3-T1 spines were tested intact and after one (C5-C6) and two level (C5-C7) arthroplasty (Triadyme-C, Dymicron Inc., Orem, UT, USA). Kinematics were evaluated in flexion-extension, lateral bending, and axial rotation., Findings: Prosthesis placement at C5-C6 and C6-C7 was 0.5 mm anterior and 0.6 mm posterior to midline respectively. C5-C6 flexion-extension motion was 12.8° intact and 10.5° after arthroplasty. C6-C7 flexion-extension motion was 10.0 and 11.4° after arthroplasty. C5-C6 lateral bending reduced from 8.5 to 3.7° after arthroplasty and at C6-C7 from 7.5 to 5.1°. C5-C6 axial rotation decreased from 10.4 to 6.2° after arthroplasty and at C6-C7 from 7.8 to 5.3°. Segmental lordosis increased by 4.2°, and middle disc height by 1.4 mm after arthroplasty. Change in center of rotation from intact to arthroplasty averaged 0.9 mm posteriorly and 0.1 mm caudally at C5-C6, and 1.4 mm posteriorly and 0.3 mm cranially at C6-C7., Interpretation: The cervical disc arthroplasty evaluated restored flexion-extension motion to intact levels and moderately increased segmental stiffness. Disc height increased by up to 1.5 mm and segmental lordosis by 4.2°. Introduction: Anterior cervical discectomy and fusion has been associated with the development of adjacent segment degeneration (ASD), with clinical incidence of approximately 3% per year. Cervical total disc arthroplasty (TDA) has been proposed as an alternative to prevent ASD., Hypotheses: TDA in optimal placement using an elastic-core cervical disc (RHINE, K2M Inc., Leesburg, Virginia) will replicate natural kinematics and will improve with optimal vs anterior placement., Methods: Seven C3-T1 cervical cadaver spines were tested intact first, then after one-level TDA at C5-C6 anterior placement, after TDA at C5-C6 optimal placement, after two-level TDA at C5-C6 and C6-C7 optimal placement, and finally after two-level TDA at C5-C6 lateral placement and C6-C7 optimal placement. The specimens were subjected to: Flexion-Extension moments (+1.5 Nm) with compressive preloads of 0 N and 150 N, lateral bending (LB) and axial rotation (AR) (+1.5 Nm) without preload., Results: C5-C6 TDA in optimal placement resulted in a non-significant increase in flexion-extension ROM compared to intact under 0 N and 150 N preload ( P  > 0.05). Both LB and AR ROM decreased with arthroplasty ( P  < 0.01). O ptimal placement of C6-C7 TDA resulted in an increase in flexion-extension ROM with preload compared to intact ( P  < 0.05) while LB and AR ROM decreased with arthroplasty ( P  < 0.01)., Conclusion: This six degree of freedom elastic-core disc arthroplasty effectively restored flexion-extension motion to intact levels. In LB the TDA maintained 42% ROM at C5-C6 and 60% at C6-C7. In AR 57% of the ROM was maintained at C5-C6 and 70% at C6-C7. These findings are supported by literature which shows cervical TDA results in restoration of approximately 50% ROM in LB and AR, which is a multifactorial phenomenon encompassing TDA design parameters and anatomical constraints. Background: A novel expandable lumbar interbody fusion cage has been developed which allows for a broad endplate footprint similar to an anterior lumbar interbody fusion; however, it is deployed from a minimally invasive transforaminal unilateral approach. The perceived benefit is a stable circumferential fusion from a single approach that maintains the anterior tension band of the anterior longitudinal ligament. The purpose of this biomechanics laboratory study was to evaluate the biomechanical stability of an expandable lumbar interbody cage inserted using a transforaminal approach and deployed in situ compared to a traditional lumbar interbody cage inserted using an anterior approach (control device)., Methods: Twelve cadaveric spine specimens (L1-5) were tested intact and after implantation of both the control and experimental devices in 2 (L2-3 and L3-4) segments of each specimen; the assignments of the control and experimental devices to these segments were alternated. Effect of supplemental pedicle screw-rod stabilization was also assessed. Moments were applied to the specimens in flexionextension (FE), lateral bending (LB), and axial rotation (AR). The effect of physiologic preload on construct stability was evaluated in FE. Segmental motions were measured using an optoelectronic motion measurement system., Results: The deployable expendable transforaminal lumbar interbody fusion (TLIF) cage and control devices significantly reduced FE motion with and without compressive preload when compared to the intact condition ( P < .05). Segmental motions in LB and AR were also significantly reduced with both devices ( P < .05). Under no preload, the deployable expendable TLIF cage construct resulted in significantly smaller FE motion compared to the control cage construct ( P < .01). Under all other testing modes (FE under 400N preload, LB, and AR), the postoperative motions of the 2 constructs did not differ statistically ( P > .05). Adding bilateral pedicle screws resulted in further reduction of range of motion for all loading modes compared to intact condition, with no statistical difference between the 2 constructs ( P > .05)., Conclusions: The ability of the deployable expendable interbody cage in reducing segmental motions was equivalent to the control cage when used as a standalone construct and also when supplemented with bilateral pedicle screw-rod instrumentation. Background: The need for posterior longitudinal ligament (PLL) resection during cervical total disc arthroplasty (TDA) has been debated. The purpose of this laboratory study was to investigate the effect of PLL resection on cervical kinematics after TDA., Methods: Eight cadaveric cervical spine specimens were tested in flexion-extension (FE), lateral bending (LB), and axial rotation (AR) to moments of ±1.5 Nm. After testing the intact condition, anterior C5-C6 cervical discectomy was performed followed by PLL resection and implantation of a compressible, 6-degrees-of-freedom disc prosthesis (M6-C, Spinal Kinetics Inc, Sunnyvale, California). Next, a second prosthesis was implanted at C6-C7 with PLL intact. Finally, the C6-C7 PLL was resected while the disc prosthesis remained in place. Segmental range of motion (ROM) and stiffness in the high flexibility zone around the neutral posture were analyzed using repeated measures ANOVA., Results: At C5-C6, following TDA and PLL resection, FE, LB, and AR ROMs decreased significantly. Anterior and posterior disc height, segmental lordosis, and flexion stiffness increased significantly. At C6-C7, TDA with the PLL intact resulted in a significant increase in anterior disc height and segmental lordosis with no change in posterior disc height. FE, LB, and AR ROMs all decreased significantly, while flexion stiffness increased significantly compared to intact. PLL resection at C6-C7 did not result in a notable change compared to TDA with PLL intact. At the same level, flexion stiffness decreased following PLL resection compared to TDA with a value closer to intact. Two-level TDA (C5-C7) with PLL resection did not result in a loss of segmental stability., Conclusion: PLL resection did not significantly affect motion segment kinematics following cervical TDA using a prosthesis with inherent stiffness. Background  An injury to the scapholunate interosseous ligament (SLIL) leads to instability in the scapholunate joint. Temporary fixation is used to protect the ligament during reconstruction or healing of the repair. Rigid screw fixation-by blocking relative physiological motion between the scaphoid and lunate-can lead to screw loosening, pullout, and fracture. Purpose  This study aims to evaluate changes in scaphoid and lunate kinematics following SLIL injury and the effectiveness of an articulating screw at restoring preinjury motion. Materials and Methods  The kinematics of the scaphoid and lunate were measured in 10 cadaver wrists through three motions driven by a motion simulator. The specimens were tested intact, immediately following SLIL injury, after subsequent cycling, and after fixation with a screw. Results  Significant changes in scaphoid and lunate motion occurred following SLIL injury. Postinjury cycling increased motion changes in flexion-extension and radial-ulnar deviation. The motion was not significantly different from the intact scapholunate joint after placement of the articulating screw. Conclusion  In agreement with other studies, sectioning of the SLIL led to significant kinematic changes of the scaphoid and lunate in all motions tested. Compared with intact scapholunate joint, no significant difference in kinematics was found after placement of the screw indicating a correction of some of the changes produced by SLIL transection. These findings suggest that the articulating screw may be effective for protecting a SLIL repair while allowing the physiological rotation to occur between the scaphoid and lunate. Clinical Relevance  A less rigid construct, such as the articulating screw, may allow earlier wrist rehabilitation with less screw pullout or failure.

Purpose: In this article, we summarize our work on understanding the influence of cervical sagittal malalignment on the mechanics of the cervical spine., Methods: Biomechanical studies were performed using an ex vivo laboratory model to study the kinematic and kinetic response of human cervical spine specimens in the setting of cervical sagittal imbalance. The model allowed controlled variations of C2-C7 Sagittal Vertical Alignment (C2-C7 SVA) and T1-Slope so that clinically relevant sagittally malaligned profiles could be prescribed, while maintaining horizontal gaze, and their biomechanical consequences studied., Results: Our results demonstrated that increasing C2-C7 SVA caused flexion of lower cervical (C2-C7) segments and hyperextension of suboccipital (C0-C1-C2) segments to maintain horizontal gaze. An increase in C2-C7 SVA increased the lower cervical neural foraminal areas. Conversely, increasing T1-slope predominantly influenced subaxial cervical lordosis and, as a result, decreased cervical neural foraminal areas. Therefore, we believe patients with increased upper thoracic kyphosis and radicular symptoms may respond with increased forward head posture (FHP) as a compensatory mechanism to increase their lower cervical neural foraminal area and alleviate nerve root compression as well as reduce the burden on posterior muscles and soft and bony structures of the cervical spine. Increasing FHP (i.e., increased C2-C7 SVA) was associated with shortening of the cervical flexors and occipital extensors and lengthening of the cervical extensors and occipital flexors, which corresponds to C2-C7 flexion and C0-C2 extension. The greatest shortening occurred in the suboccipital muscles, suggesting considerable load bearing of these muscles during chronic FHP. Regardless, there was no evidence of nerve compression within the suboccipital triangle. Finally, cervical sagittal imbalance may play a role in exacerbating adjacent segment pathomechanics after multilevel cervical fusion and should be considered during surgical planning., Conclusions: The results of our biomechanical studies have improved our understanding of the impact of cervical sagittal malalignment on pathomechanics of the cervical spine. We believe this improved understanding will assist in clinical decision-making.

Objective: To test the following hypotheses: (i) anterior cervical discetomy and fusion (ACDF) using stand-alone interbody spacers will significantly reduce the range of motion from intact spine; and (ii) the use of a static or a rotational-dynamic plate will significantly augment the stability of stand-alone interbody spacers, with similar beneficial effect when compared to each other., Methods: Eleven human cadaveric subaxial cervical spines (age: 48.2 ± 5.4 years) were tested under the following sequence: (i) intact spine; (ii) ACDF at C 4 -C 5 using a stand-alone interbody spacer; (iii) ACDF at C 5 -C 6 and insertion of an interbody spacer (two-level construct); and (iv) randomized placement of either a two-level locking static plate or a rotational-dynamic plate., Results: Insertion of stand-alone cage at C 4 -C 5 and C 5 -C 6 caused a significant decrease in the range of motion compared to intact spine (P < 0.05). Placement of both the locking and the rotational dynamic plate further reduced the range of motion at C 4 -C 5 and C 5 -C 6 compared to stand-alone cage (P < 0.01). No significant differences in range of motion restriction at either C 4 -C 5 or C 5 -C 6 were found when the two plating systems were compared (P > 0.05)., Conclusions: Cervical stand-alone interbody spacers caused significant restriction in the range of motion. Literature describes only qualitative muscle length changes associated with FHP., Objective: The purpose of this study was to quantify how muscle-tendon unit lengths are altered when human cadaveric specimens are placed in alignments representing different severities of FHP., Design: This biomechanical study used 13 fresh-frozen cadaveric cervical spine specimens (Occiput-T1, 54±15 y)., Methods: Specimens' postural changes simulating increasing FHP severity while maintaining horizontal gaze were assessed. Specimen-specific anatomic models derived from computed tomography-based anatomic data were combined with postural data and specimen-specific anatomy of muscle attachment points to estimate the muscle length changes associated with FHP., Results: Forward head posture was associated with flexion of the mid-lower cervical spine and extension of the upper cervical (sub-occipital) spine. Muscles that insert on the cervical spine and function as flexors (termed "cervical flexors") as well as muscles that insert on the cranium and function as extensors ("occipital extensors") shortened in FHP when compared to neutral posture. In contrast, muscles that insert on the cervical spine and function as extensors ("cervical extensors") as well as muscles that insert on the cranium and function as flexors ("occipital flexors") lengthened. The greatest shortening was seen in the major and minor rectus capitis posterior muscles. These muscles cross the Occiput-C2 segments, which exhibited extension to maintain horizontal gaze. The greatest lengthening was seen in posterior muscles crossing the C4-C6 segments, which exhibited the most flexion., Limitations: This cadaver study did not incorporate the biomechanical influence of active musculature., Conclusions: This study offers a novel way to quantify postural alignment and muscle length changes associated with FHP. Devising patient-specific kinematic assessment techniques are critical for both patient diagnosis and treatment evaluation of complex biomechanical joints within the body. New non-invasive kinematic assessment techniques, such as bi-planar fluoroscopic registration, provide improved insight on joint biomechanics compared to traditional techniques, but at the expense of higher radiation exposure to the patient. The purpose of this study was to minimize the x-ray sample size required for evaluating spine kinematics, ultimately reducing radiation exposure, while maintaining a high degree of accuracy by improving upon existing 3D kinematic interpolation techniques. Existing interpolation methods were improved to account for non-uniformly sampled control points and applied to new motion descriptors, thus creating a new approach to 3D kinematic interpolation utilizing dual-quaternions. Interpolation reconstruction methods were applied to decimated gold standard ex vivo spinal kinematic data originally acquired at 30Hz. The effects of interpolation method and variables (motion descriptor, sample spacing, sampling correction factors) on accuracy were compared. Dual-quaternion interpolation methods and equal interval angular sampling showed superior reconstruction results. Accuracy also improved when using temporal correction factors. Less than 1% normalized root-mean-squared error and less than 2% normalized maximum error were achieved from 0.36% of the original data set. The new approach also demonstrated its scalability for larger movements. However, accuracy may vary when interpolating more complex motion patterns. Overall, multiple interpolation methods and factors were evaluated in reconstructing 3D spine kinematics. High accuracy at low sample sizes and advantageous scalability to motions with larger total displacement illustrate its viability for bi-planar fluoroscopy., (Published by Elsevier Ltd.)

Study Design: Analysis of prospectively collected radiographic data., Objective: To investigate the influence of preoperative index-level range of motion (ROM) and disc height on postoperative ROM after cervical total disc arthroplasty (TDA) using compressible disc prostheses., Summary of Background Data: Clinical studies demonstrate benefits of motion preservation over fusion; however, questions remain unanswered as to which preoperative factors have the ability to identify patients who are most likely to have good postoperative motion, which is the primary rationale for TDA., Methods: We analyzed prospectively collected data from a single-arm, multicenter study with 2-year follow up of 30 patients with 48 implanted levels. All received compressible cervical disc prostheses of 6 mm-height (M6C, Spinal Kinetics, Sunnyvale, CA). The influence of index-level preoperative disc height and ROM (each with two levels: below-median and above-median) on postoperative ROM was analyzed using 2 x 2 ANOVA. We further analyzed the radiographic outcomes of a subset of discs with preoperative height less than 3 mm, the so-called "collapsed" discs., Results: Shorter (3.0 ± 0.4 mm) discs were significantly less mobile preoperatively than taller (4.4 ± 0.5 mm) discs (6.7° vs. 10.5°, P = 0.01). The postoperative ROM did not differ between the shorter and taller discs (5.6° vs. 5.0°, P = 0.63). Tall discs that were less mobile preoperatively had significantly smaller postoperative ROM than short discs with above-median preoperative mobility (P < 0.05). The "collapsed discs" (n = 8) were less mobile preoperatively compared with all discs combined (5.1° vs. 8.6°, P < 0.01). These discs were distracted to more than two times the preoperative height, from 2.6 to 5.7 mm, and had significantly greater postoperative ROM than all discs combined (7.6° vs. 5.3°, P < 0.05)., Conclusion: We observed a significant interaction between preoperative index-level disc height and ROM in influencing postoperative ROM. Although limited by small sample size, the results suggest discs with preoperative height less than 3 mm may be amenable to disc arthroplasty using compressible disc prostheses., Level of Evidence: 2.

Background: A lateral approach with open reduction and internal fixation with a plate is a very effective technique for the majority of distal fibular fractures. However, this open approach for ankle fixation may be complicated by wound dehiscence and infection, especially in high-risk patients. An alternative to plating is an intramedullary implant, which allows maintenance of length, alignment, and rotation and which allows for decreased soft tissue dissection. While there has been clinical data suggesting favorable short-term outcomes with these implants, there is no current biomechanical literature investigating this technology in this particular fracture pattern. This study sought to biomechanically compare an emerging technology with an established method of fixation for distal fibular fractures that traditionally require an extensive exposure., Methods: Ten matched cadaveric pairs from the proximal tibia to the foot were prepared to simulate an Arbeitsgemeinschaft für Osteosynthesefragen/Orthopaedic Trauma Association (AO/OTA) 44C2 ankle fracture and randomized to fixation with a distal fibular locking plate or intramedullary fibular rod. A constant 700-N axial load was applied, and all specimens underwent testing for external rotation stiffness, external rotation cyclic loading, and torque to failure. The syndesmotic diastasis, stiffness, torque to failure, angle at failure, and mode of failure were obtained from each specimen., Results: There was no significant difference in syndesmotic diastasis during cyclic loading or at maximal external rotation between the rod and plate groups. Post-cycle external rotation stiffness across the syndesmosis was significantly higher for the locking plate than the fibular rod. There was no significant difference between the rod and plate in torque at failure or external rotation angle. The majority of specimens had failure at the syndesmotic screw., Conclusions: In the present cadaveric study of an AO/OTA 44C2 ankle fracture, a modern fibular rod demonstrated less external rotation stiffness while maintaining the syndesmotic diastasis to within acceptable tolerances and having similar failure characteristics.

MRI allows non-invasive assessment of intervertebral disc degeneration with the added clinical benefit of using non-ionizing radiation. What has remained unclear is the relationship between assessed disc degeneration and lumbar spine kinematics. Kinematic outcomes of 54 multi-segment (L1-Sacrum) lumbar spine specimens were calculated to discover if such an underlying relationship exists with degeneration assessed using the Pfirrmann grading system. Further analyses were also conducted to determine if kinematic outcomes were affected by motion segment level, gender or applied compressive preload. Range of motion, hysteresis, high flexibility zone size and rotational stiffness in flexion-extension, lateral bending and axial rotation were the kinematic outcomes. Caudal intervertebral discs in our study sample were more degenerative than cranial discs. L5-S1 discs had the largest flexion-extension range of motion (p < 0.005) and L1-L2 discs the lowest flexion high flexibility zone size (p < 0.013). No other strict cranial-caudal differences in kinematic outcomes were found. Low flexibility zone rotational stiffness increased with disc degeneration grade in extension, lateral bending and axial rotation (p < 0.001). Trends towards higher hysteresis and lower range of motion with increased degeneration were observed in flexion-extension and lateral bending. Applied compressive preload increased flexion-extension hysteresis and augmented the effect of degeneration on hysteresis (p < 0.0005). Female specimens had about one degree larger range of motion in all rotational modes, and higher flexion extension hysteresis (p = 0.016). These results suggest that gender differences exist in lumbar spine kinematics. Additionally high disc loads, applied compressive preload or applied moment, are needed to kinematically distinguish discs with different levels of degeneration. © 2016 Orthopaedic Research Society. Purpose: Patients with cervical spondylosis commonly present with neck pain, radiculopathy or myelopathy. As degenerative changes progress, multiple factors including disc height loss, thoracic kyphosis, and facetogenic changes can increase the risk of neural structure compression. This study investigated the impact of cervical deformity including forward head posture (FHP) and upper thoracic kyphosis, on the anatomy of the cervical neural foramen., Methods: Postural changes of 13 human cervical spine specimens (Occiput-T1, age 50.6 years; range 21-67) were assessed in response to prescribed cervical sagittal malalignments using a previously reported experimental model. Two characteristics of cervical sagittal deformities, C2-C7 sagittal vertical alignment (SVA) and sagittal angle of the T1 vertebra (T1 tilt), were varied to create various cervical malalignments. The postural changes were documented by measuring vertebral positions and orientations. The vertebral motion data were combined with specimen-specific CT-based anatomical models, which allowed assessments of foraminal areas of subaxial cervical segments as a function of increasing C2-C7 SVA and changing T1 tilt., Results: Increasing C2-C7 SVA from neutral posture resulted in increased neural foraminal area in the lower cervical spine (largest increase at C4-C5: 13.8 ± 15.7 %, P < 0.01). Increasing SVA from a hyperkyphotic posture (greater T1 tilt) also increased the neural foraminal area in the lower cervical segments (C5-C6 demonstrated the largest increase: 13.4 ± 9.6 %, P < 0.01). The area of the cervical neural foramen decreased with increasing T1 tilt, with greater reduction occurring in the lower cervical spine, specifically at C5-C6 (-8.6 ± 7.0 %, P < 0.01) and C6-C7 (-9.6 ± 5.6 %, P < 0.01)., Conclusion: An increase in thoracic kyphosis (T1 tilt) decreased cervical neural foraminal areas. In contrast, an increase in cervical SVA increased the lower cervical neural foraminal areas. Patients with increased upper thoracic kyphosis may respond with increased cervical SVA as a compensatory mechanism to increase their lower cervical neural foraminal area.

Study Design: A biomechanical study using human spine specimens., Objective: The aim of this study was to assess whether the presence of cervical sagittal imbalance is an independent risk factor for increasing the mechanical burden on discs adjacent to cervical multilevel fusions., Summary of Background Data: The horizontal offset distance between the C2 plumbline and C7 vertebral body (C2-C7 Sagittal Vertical Axis (SVA)) or the angle made with vertical by a line connecting the C2 and C7 vertebral bodies (C2-C7 tilt angle) are used as radiographic measures to assess cervical sagittal balance. There is level III clinical evidence that sagittal imbalance caused by kyphotic fusions or global spinal sagittal malalignment may increase the risk of adjacent segment pathology., Methods: Thirteen human cadaveric cervical spines (Occiput-T1; age: 50.6 years; range: 21-67) were tested first in the native intact state and then after instrumentation across C4-C6 to simulate in situ two-level fusion. Specimens were tested using a previously validated experimental model that allowed measurement of spinal response to prescribed imbalance. The effects of fusion on segmental angular alignments and intradiscal pressures in the C3-C4 and C6-C7 discs, above and below the fusion, were evaluated at different magnitudes of C2-C7 tilt angle (or C2-C7 SVA)., Results: When compared with the pre-fusion state, in situ fusion across C4-C6 segments required increased flexion angulation and resulted in increased intradiscal pressure at the C6-C7 disc below the fusion in order to accommodate the same increase in C2-C7 tilt angle or C2-C7 SVA (P < 0.05). The adjacent segment mechanical burden due to fusion became greater with increasing C2-C7 tilt angle or SVA., Conclusion: Cervical sagittal imbalance arising from regional and/or global spinal sagittal malalignment may play a role in exacerbating adjacent segment pathomechanics after multilevel fusion and should be considered during surgical planning., Level of Evidence: N/A.

Study Design: Cadaveric study to accurately measure lumbar neuroforaminal area and height throughout the flexion-extension range of motion (ROM)., Objective: Create a new computed tomography (CT)-based specimen-specific model technique to provide insight on the effects of kinematics on lumbar neuroforamen morphology during flexion-extension ROM., Summary of Background Data: Nerve root compression is a key factor in symptomatic progression of degenerative disc disease because these changes directly affect neuroforaminal area. Traditional techniques to evaluate the neuroforamen suffer from poor accuracy, have inherent limitations, and fail to provide data throughout the ROM., Methods: Six cadaveric specimens (L1-sacrum) were instrumented with radiopaque spheres and CT scanned. 3-Dimensional reconstructions were made of each vertebra and the sphere locations determined. During kinematic testing, the spheres were located in relation to optoelectronic targets attached to each vertebra. The result was a 3-dimensional representation of the specimen's CT reconstruction moving in response to experimental data. Bony contours of the L2-L3 and L4-L5 neuroforamen were digitized producing continuous neuroforaminal area and height data throughout the ROM., Results: Neuroforaminal area and height linearly increased in flexion and decreased in extension. There was significant correlation between flexion-extension motion and percent change in area (L2-L3: 3.1%/deg, R = 0.94, L4-L5: 2.5%/deg, R = 0.90) and neuroforaminal height (L2-L3: 2.1%/deg, R = 0.95, L4-L5: 1.6%/deg, R = 0.93). Regression analysis showed that the ratio between neuroforaminal height and area is at least 1:1.5 such that a 100% increase in height is associated with an area increase of more than 150%., Conclusion: This is the first study to measure lumbar neuroforaminal area and height throughout flexion-extension ROM. The CT-based specimen-specific model technique can accurately evaluate the effect of kinematics on morphological features of the spine. The demonstrated increase in neuroforaminal dimension in flexion is consistent with treatment modalities used in clinical therapies to relieve radicular symptoms., Level of Evidence: N/A.

Study Design: A biomechanical study using human spine specimens., Objective: To study postural compensations in lordosis angles that are necessary to maintain horizontal gaze in the presence of forward head posture and increasing T1 sagittal tilt., Summary of Background Data: Forward head posture relative to the shoulders, assessed radiographically using the horizontal offset distance between the C2 and C7 vertebral bodies (C2-C7 [sagittal vertical alignment] SVA), is a measure of global cervical imbalance. This may result from kyphotic alignment of cervical segments, muscle imbalance, as well as malalignment of thoracolumbar spine., Methods: Ten cadaveric cervical spines (occiput-T1) were tested. The T1 vertebra was anchored to a tilting and translating base. The occiput was free to move vertically but its angular orientation was constrained to ensure horizontal gaze regardless of sagittal imbalance. A 5-kg mass was attached to the occiput to mimic head weight. Forward head posture magnitude and T1 tilt were varied and motions of individual vertebrae were measured to calculate C2-C7 SVA and lordosis across C0-C2 and C2-C7., Results: Increasing C2-C7 SVA caused flexion of lower cervical (C2-C7) segments and hyperextension of suboccipital (C0-C1-C2) segments to maintain horizontal gaze. Increasing kyphotic T1 tilt primarily increased lordosis across the C2-C7 segments. Regression models were developed to predict the compensatory C0-C2 and C2-C7 angulation needed to maintain horizontal gaze given values of C2-C7 SVA and T1 tilt., Conclusion: This study established predictive relationships between radiographical measures of forward head posture, T1 tilt, and postural compensations in the cervical lordosis angles needed to maintain horizontal gaze. The laboratory model predicted that normalization of C2-C7 SVA will reduce suboccipital (C0-C2) hyperextension, whereas T1 tilt reduction will reduce the hyperextension in the C2-C7 segments. The predictive relationships may help in planning corrective strategy in patients experiencing neck pain, which may be attributed to sagittal malalignment., Level of Evidence: N/A.

Much controversy exists regarding the mechanism of burst fracture (BF) induction and the proper techniques to treat such fractures. As such, there is a great need for validated preclinical models in which to study these injuries. In this study, an electronic search of the PubMed database (MEDLINE) was performed and the results were filtered to obtain only studies with a biomechanical focus. Forty five biomechanical studies were obtained, from which four distinct methods of injury induction were identified. Twenty one (46.7%) involved clinically relevant techniques for treating BF, involving anterior, posterior, circumferential, or a combination of approaches. Fifteen (71.4%) of the treatment studies used unstable spine specimens that do not fit the classical characteristics of BF. Introduction: We hypothesized that an Integrated Lumbar Interbody Fusion Device (PILLAR SA, Orthofix, Lewisville, TX) will function biomechanically similar to a traditional anterior interbody spacer (PILLAR AL, Orthofix, Lewisville, TX) plus posterior instrumentation (FIREBIRD, Orthofix, Lewisville, TX). Purpose of this study was to determine if an Integrated Interbody Fusion Device (PILLAR SA) can stabilize single motion segments as well as an anterior interbody spacer (PILLAR AL) + pedicle screw construct (FIREBIRD)., Methods: Eight cadaveric lumbar spines (age: 43.9±4.3 years) were used. Each specimen's range of motion was tested in flexion-extension (FE), lateral bending (LB), and axial rotation (AR) under intact condition, after L4-L5 PILLAR SA with intervertebral screws and after L4-L5 360° fusion (PILLAR AL + Pedicle Screws and rods (FIREBIRD). Each specimen was tested in flexion (8Nm) and extension (6Nm) without preload (0 N) and under 400N of preload, in lateral bending (±6 Nm) and axial rotation (±5 Nm) without preload., Results: Integrated fusion using the PILLAR SA device demonstrated statistically significant reductions in range of motion of the L4-L5 motion segment as compared to the intact condition for each test direction. PILLAR SA reduced ROM from 8.9±1.9 to 2.9±1.1° in FE with 400N follower preload (67.4%), 8.0±1.7 to 2.5±1.1° in LB, and 2.2±1.2 to 0.7±0.3° in AR. A comparison between the PILLAR SA integrated fusion device versus 360° fusion construct with spacer and bilateral pedicle screws was statistically significant in FE and LB. The 360° fusion yielded motion of 1.0±0.5° in FE, 1.0±0.8° in LB (p0.05)., Conclusions: The PILLAR SA resulted in motions of less than 3° in all modes of motion and was not as motion restricting as the traditional 360° using bilateral pedicle screws. The residual segmental motions compare very favorably with published biomechanical studies of other interbody integrated fusion devices.

Study Design: Human cadaveric biomechanical study., Objective: To determine the fixation strength of laterally directed, cortical pedicle screws under physiological loads., Summary of Background Data: Lateral trajectory cortical pedicle screws have been described as a means of obtaining improved fixation while minimizing soft-tissue dissection during lumbar instrumentation. Biomechanical data have demonstrated equivalent strength in a quasi-static model; however, no biomechanical information is available comparing the fixation of cortical with traditional pedicle screws under cyclic physiological loads., Methods: Seventeen vertebral levels (T11-L5) underwent quantitative computed tomography. On 1 side, a laterally directed, cortical pedicle screw was inserted with a traditional, medially directed pedicle screw placed on the contralateral side. With the specimen constrained in a testing apparatus, each screw underwent cyclic craniocaudal toggling under incrementally increasing physiological loads until 2 mm of head displacement occurred. Next, uniaxial pullout of each toggled screw was performed. The number of craniocaudal toggle cycles and load (N) required to achieve pedicle screw movement as well as axial pullout resistance (N) were compared between the 2 techniques., Results: The mean trabecular bone mineral density of the specimens was 202 K2HPO4 mg/cm. Cortical pedicle screws demonstrated significantly improved resistance to toggle testing, requiring 184 cycles to reach 2 mm of displacement compared with 102 cycles for the traditional pedicle screws (P=0.002). The force necessary to displace the screws was also significantly greater for the cortical versus the traditional screws (398 N vs. 300 N, P=0.004). There was no statistical difference in axial pullout strength between the previously toggled cortical and traditional pedicle screws (1722 N vs. 1741 N, P=0.837)., Conclusion: Laterally directed cortical pedicle screws have superior resistance to craniocaudal toggling compared with traditional pedicle screws., Level of Evidence: N/A.

Study Design: We quantified the segmental biomechanics of a cervical total disc replacement (TDR) before and after progressive posterior decompression. We hypothesized that posterior decompressive procedures would not significantly increase range of motion (ROM) at the index TDR level., Objective: To quantify the kinematics of a cervical total disc replacement (TDR) before and after posterior cervical decompression., Summary of Background Data: A reported yet unaddressed issue is the potential for the development of same-segment disease after implantation of a cervical TDR and the implications of same-segment posterior decompression on TDR mechanics., Methods: Eight human cadaveric cervical spines C3-C7 were tested in flexion-extension, lateral bending, and axial rotation while intact, after C5-C6 TDR, C5-C6 unilateral foraminotomy, C5-C6 bilateral foraminotomies, and after C5 laminectomy in combination with the bilateral foraminotomies. Moment versus angular motion curves were obtained for each testing step, and the load-displacement data were analyzed to determine the range of angular motion for each step., Results: Unilateral foraminotomy did not result in a statistically significant increase in flexion-extension ROM, and did not increase the ROM to a degree greater than normal. Although bilateral foraminotomies did increase flexion-extension ROM, motion remained within a physiological range. A full laminectomy added to the bilateral foraminotomies significantly increased ROM and was also associated with distortion of the load-displacement curves., Conclusion: With respect to segmental biomechanics as demonstrated, we think that for same-segment disease, a unilateral foraminotomy can be performed safely. However, the impact of in vivo conditions was not accounted for in this model, and it is possible that cyclical loading and other physiological stresses on such a construct may affect the behavior and lifespan of the implant in a way that cannot be predicted by a biomechanical study. Bilateral foraminotomies would require close observation and additional clinical follow-up, whereas complete laminectomy combined with bilateral foraminotomies should be avoided after TDR given the significant changes in kinematics. In addition, future disc replacement designs may need to account for changes after posterior decompression for same-segment disease., Level of Evidence: N/A.

Study Design: In vitro biomechanical study., Objective: To test the hypotheses: (1) an anchored spacer device would decrease motion similarly to a plate-spacer construct, and (2) the anchored spacer would achieve a similar reduction in motion when placed adjacent to a previously fused segment., Summary of Background Data: An anchored spacer device has been shown to perform similar to the plate-spacer construct in previous biomechanical evaluation. The prevalence of adjacent segment disease after fusion is well established in the literature.There is currently no evidence supporting the use of an anchored interbody spacer device adjacent to a previous fusion., Methods: Eight human cervical spines (age: 45.1 ± 13.1 yr) were tested in moment control (±1.5 Nm) in flexion-extension, lateral bending, and axial rotation without preload. Flexion-extension was then retested under 150-N preload. Spines were tested intact and after anterior cervical discectomy and fusion (ACDF) at C4-C5 and C6-C7 with either a plate-spacer or anchored spacer construct (randomized). The specimens were tested finally with an ACDF at the floating C5-C6 segment using the anchored spacer device adjacent to the previous fusions., Results: Both the plate-spacer and anchored spacer significantly reduced motion from the intact spine in flexion-extension, lateral bending, and axial rotation (P < 0.005). There was no statistically significant difference between the 2 fusion constructs in their abilities to reduce motions (P = 1.0). ACDF using the anchored spacer at the floating C5-C6 level (in between the plate-spacer and anchored spacer constructs) resulted in significant motion reductions in all modes of testing (P < 0.05). These motion reductions did not significantly differ from those of a single-level anchored-spacer construct or a single-level plated ACDF., Conclusion: The anchored spacer provided significant motion reductions, similar to a plated ACDF, when used as a single-level fusion construct or placed adjacent to a previously plated segment., Level of Evidence: N/A.

Minimally invasive (MI) lumbar decompression became a common approach to treat lumbar stenosis. This approach may potentially mitigate postoperative increases in segmental motion. The goal of this study was to evaluate modifications to segmental motion in the lumbar spine following a MI unilateral approach as compared to traditional facet-sparing and non-facet sparing decompressions. Six human lumbar cadaveric specimens were used. Each specimen was tested in flexion-extension 0 N and 400 N of follower preload), axial rotation, and lateral bending. Each testing condition was evaluated following three separate interventions at L4-L5: 1) Minimally invasive decompression, 2) Facet-sparing, bilateral decompression, and 3) Bilateral decompression with a wide facetectomy. Range of motion following each testing condition was compared to intact specimens. Both MI and traditional decompression procedures create significant increases in ROM in all modes of loading. However, when compared to the MI approach, traditional decompression produces significantly larger increase in ROM in flexion-extension (p<0.005) and axial rotation (p<0.05). It additionally creates increased ROM with lateral bending on the approach side (p<0.05). Lateral bending on the non-approach side is not significantly changed. Lastly, wide medial facet removal (40% to 50%) causes significant hypermobility, especially in axial rotation. While both MI and traditional lumbar decompressions may increase post-operative ROM in all conditions, a MI approach causes significantly smaller increase in ROM. With an MI approach, increased movement with lateral bending is only toward the approach side. Further, non-facet sparing decompression is further destabilizing in all loading modes.

Study Design: Biomechanical human cadaveric study., Objective: We hypothesized that increasing compressive preload will reduce the segmental instability after nucleotomy, posterior ligament resection, and decompressive surgery., Summary of Background Data: The human spine experiences significant compressive preloads in vivo due to spinal musculature and gravity. Although the effect of destabilization procedures on spinal motion has been studied, the effect of compressive preload on the motion response of destabilized, multisegment lumbar spines has not been reported., Methods: Eight human cadaveric spines (L1-sacrum, 51.4 ± 14.1 yr) were tested intact, after L4-L5 nucleotomy, after interspinous and supraspinous ligaments transection, and after midline decompression (bilateral laminotomy, partial medial facetectomy, and foraminotomy). Specimens were loaded in flexion (8 Nm) and extension (6 Nm) under 0-N, 200-N, and 400-N compressive follower preload. L4-L5 range of motion (ROM) and flexion stiffness in the high-flexibility zone were analyzed using repeated-measures analysis of variance and multiple comparisons with the Bonferroni correction., Results: With a fixed set of loading conditions, a progressive increase in segmental ROM along with expansion of the high-flexibility zone (decrease of flexion stiffness) was noted with serial destabilizations. Application of increasing compressive preload did not substantially change segmental ROM, but did significantly increase the segmental stiffness in the high-flexibility zone. In the most destabilized condition, 400-N preload did not return the segmental stiffness to intact levels., Conclusion: Anatomical alterations representing degenerative and iatrogenic instabilities are associated with significant increases in segmental ROM and decreased segmental stiffness. Although application of compressive preload, mimicking the effect of increased axial muscular activity, significantly increased the segmental stiffness, it was not restored to intact levels; thereby suggesting that core strengthening alone may not compensate for the loss of structural stability associated with midline surgical decompression. This suggests that there may be a role for surgical implants or interventions that specifically increase flexion stiffness and limit flexion ROM to counteract the iatrogenic instability resulting from surgical decompression., Level of Evidence: N/A.

Purpose: Development of a dynamic stabilization system often involves costly and time-consuming design iterations, testing and computational modeling. The aims of this study were (1) develop a simple parametric model of lumbar flexion instability and use this model to identify the appropriate stiffness of a flexion restricting stabilization system (FRSS), and (2) in a cadaveric experiment, validate the predictive value of the parametric model., Methods: Literature was surveyed for typical parameters of intact and destabilized spines: stiffness in the high flexibility zone (HFZ) and high stiffness zone, and size of the HFZ. These values were used to construct a bilinear parametric model of flexion kinematics of intact and destabilized lumbar spines. FRSS implantation was modeled by iteratively superimposing constant flexion stiffnesses onto the parametric model. Five cadaveric lumbar spines were tested intact; after L4-L5 destabilization (nucleotomy, midline decompression); and after FRSS implantation. Specimens were loaded in flexion/extension (8 Nm/6 Nm) with 400 N follower load to characterize kinematics for comparison with the parametric model., Results: To accomplish the goal of reducing ROM to intact levels and increasing stiffness to approximately 50 % greater than intact levels, flexion stiffness contributed by the FRSS was determined to be 0.5 Nm/deg using the parametric model. In biomechanical testing, the FRSS restored ROM of the destabilized segment from 146 ± 13 to 105 ± 21 % of intact, and stiffness in the HFZ from 41 ± 7 to 135 ± 38 % of intact., Conclusions: Testing demonstrated excellent predictive value of the parametric model, and that the FRSS attained the desired biomechanical performance developed with the model. A simple parametric model may allow efficient optimization of kinematic design parameters.

Introduction: Anterior cervical decompression and fusion is a well-established procedure for treatment of degenerative disc disease and cervical trauma including flexion-distraction injuries. Low-profile interbody devices incorporating fixation have been introduced to avoid potential issues associated with dissection and traditional instrumentation. While these devices have been assessed in traditional models, they have not been evaluated in the setting of traumatic spine injury. This study investigated the ability of these devices to stabilize the subaxial cervical spine in the presence of flexion-distraction injuries of increasing severity., Methods: Thirteen human cadaveric subaxial cervical spines (C3-C7) were tested at C5-C6 in flexion-extension, lateral bending and axial rotation in the load-control mode under ±1.5 Nm moments. Six spines were tested with locked screw configuration and seven with variable angle screw configuration. After testing the range of motion (ROM) with implanted device, progressive posterior destabilization was performed in 3 stages at C5-C6., Results: The anchored spacer device with locked screw configuration significantly reduced C5-C6 flexion-extension (FE) motion from 14.8 ± 4.2 to 3.9 ± 1.8°, lateral bending (LB) from 10.3 ± 2.0 to 1.6 ± 0.8, and axial rotation (AR) from 11.0 ± 2.4 to 2.5 ± 0.8 compared with intact under (p < 0.01). The anchored spacer device with variable angle screw configuration also significantly reduced C5-C6 FE motion from 10.7 ± 1.7 to 5.5 ± 2.5°, LB from 8.3 ± 1.4 to 2.7 ± 1.0, and AR from 8.8 ± 2.7 to 4.6 ± 1.3 compared with intact (p < 0.01). The ROM of the C5-C6 segment with locked screw configuration and grade-3 F-D injury was significantly reduced from intact, with residual motions of 5.1 ± 2.1 in FE, 2.0 ± 1.1 in LB, and 3.3 ± 1.4 in AR. Conversely, the ROM of the C5-C6 segment with variable-angle screw configuration and grade-3 F-D injury was not significantly reduced from intact, with residual motions of 8.7 ± 4.5 in FE, 5.0 ± 1.6 in LB, and 9.5 ± 4.6 in AR., Conclusions: The locked screw spacer showed significantly reduced motion compared with the intact spine even in the setting of progressive flexion-distraction injury. The variable angle screw spacer did not sufficiently stabilize flexion-distraction injuries. The resulting motion for both constructs was higher than that reported in previous studies using traditional plating. Locked screw spacers may be utilized with additional external immobilization while variable angle screw spacers should not be used in patients with flexion-distraction injuries.

The objective of this study was to measure intradiscal pressure (IDP) changes in the lower cervical spine during a manual cervical distraction (MCD) procedure. Incisions were made anteriorly, and pressure transducers were inserted into each nucleus at lower cervical discs. Four skilled doctors of chiropractic (DCs) performed MCD procedure on nine specimens in prone position with contacts at C5 or at C6 vertebrae with the headpiece in different positions. IDP changes, traction forces, and manually applied posterior-to-anterior forces were analyzed using descriptive statistics. IDP decreases were observed during MCD procedure at all lower cervical levels C4-C5, C5-C6, and C6-C7. The mean IDP decreases were as high as 168.7 KPa. Mean traction forces were as high as 119.2 N. Posterior-to-anterior forces applied during manual traction were as high as 82.6 N. Intraclinician reliability for IDP decrease was high for all four DCs. While two DCs had high intraclinician reliability for applied traction force, the other two DCs demonstrated only moderate reliability. IDP decreases were greatest during moving flexion and traction. They were progressevely less pronouced with neutral traction, fixed flexion and traction, and generalized traction.

Study Design: A biomechanical cadaveric study of lumbar spine segments., Objective: To compare the immediate stability provided by parallel-shaped and anatomically shaped carbon fiber interbody fusion I/F cages in posterior lumbar interbody fusion (PLIF) and transforaminal lumbar interbody fusion (TLIF) constructs with posterior pedicle screw instrumentation., Summary of Background Data: Few biomechanical data are available on the anatomically shaped cages in PLIF and TLIF constructs., Methods: Twenty human lumbar segments were tested in flexion-extension (FE) (8 N m flexion, 6 N m extension), lateral bending (LB) (± 6 N m), and torsional loading (± 5 N m). Each segment was tested in the intact state and after insertion of interbody cages in one of 3 constructs: PLIF with 2 parallel-shaped or anatomically shaped cages and TLIF with 1 anatomically shaped cage. All cages received supplementary pedicle screw fixation. The range-of-motion (ROM) values after cage insertion and posterior fixation were compared with the intact specimen values using analysis of variance and multiple comparisons with Bonferroni correction., Results: All constructs significantly reduced segmental motion relative to intact (P < 0.001). The motion reductions in FE, LB, and axial rotation were 85 ± 15%, 83 ± 18%, and 67 ± 6.8% for the PLIF construct using parallel cages, 79 ± 5.5%, 87 ± 10%, and 66 ± 20% for PLIF using anatomically shaped cages, and 90 ± 6.8%, 87 ± 12%, and 77 ± 22% for TLIF with an anatomically shaped cage. In FE and LB, the reductions in the ROM caused between the 3 constructs were equivalent (P > 0.05). In axial rotation, the TLIF cage provided significantly greater limitation in the ROM compared with the parallel-shaped PLIF cage (P = 0.01)., Conclusions: The parallel-shaped and anatomically shaped I/F cages provided similar stability in a PLIF construct. The greater stability of the TLIF construct was likely due to a more anterior placement of the TLIF cage and preservation of the contralateral facet joint.

Objective: We hypothesized that L5-S1 kinematics will not be affected by the lordosis distribution between the prosthesis endplates., Materials and Methods: Twelve cadaveric lumbosacral spines (51.3 ± 9.8 years) were implanted with 6° or 11° prostheses (ProDisc-L) with four combinations of superior/inferior lordosis (6°/0°, 3°/3°, 11°/0°, 3°/8°). Specimens were tested intact and after prostheses implantation with different lordosis distributions. Center of rotation (COR) and range of motion (ROM) were quantified., Results: Six-degree lordosis prostheses (n = 7) showed no difference in flexion-extension ROM, regardless of design (6°/0° or 3°/3°) (p > 0.05). In lateral bending (LB), both designs reduced ROM (p < 0.05). In axial rotation, only the 3°/3° design reduced ROM (p < 0.05). Eleven-degree lordosis prostheses (n = 5) showed no difference in flexion-extension ROM for either design (p > 0.05). LB ROM decreased with distributed lordosis prostheses (3°/8°) (p < 0.05). Overall, L5-S1 range of motion was not markedly influenced by lordosis distribution among the two prosthesis endplates. The ProDisc-L prosthesis design where all lordosis is concentrated in the superior endplate yielded COR locations that were anterior and caudal to intact controls. The prosthesis with lordosis distributed between the two endplates yielded a COR that tended to be closer to intact., Conclusions: Further clinical and biomechanical studies are needed to assess the long-term impact of lordosis angle distribution on the fate of the facet joints.

This study tested the hypotheses that (1) cervical total disc replacement with a compressible, six-degree-of-freedom prosthesis would allow restoration of physiologic range and quality of motion, and (2) the kinematic response would not be adversely affected by variability in prosthesis position in the sagittal plane. Twelve human cadaveric cervical spines were tested. Prostheses were implanted at C5-C6. Range of motion (ROM) was measured in flexion-extension, lateral bending, and axial rotation under ± 1.5 Nm moments. Motion coupling between axial rotation and lateral bending was calculated. Stiffness in the high flexibility zone was evaluated in all three testing modes, while the center of rotation (COR) was calculated using digital video fluoroscopic images in flexion-extension. Implantation in the middle position increased ROM in flexion-extension from 13.5 ± 2.3 to 15.7 ± 3.0° (p < 0.05), decreased axial rotation from 9.9 ± 1.7 to 8.3 ± 1.6° (p < 0.05), and decreased lateral bending from 8.0 ± 2.1 to 4.5 ± 1.1° (p < 0.05). Coupled lateral bending decreased from 0.62 ± 0.16 to 0.39 ± 0.15° for each degree of axial rotation (p < 0.05). Flexion-extension stiffness of the reconstructed segment with the prosthesis in the middle position did not deviate significantly from intact controls, whereas the lateral bending and axial rotation stiffness values were significantly larger than intact. Implanting the prosthesis in the posterior position as compared to the middle position did not significantly affect the ROM, motion coupling, or stiffness of the reconstructed segment; however, the COR location better approximated intact controls with the prosthesis midline located within ± 1 mm of the disc-space midline. Overall, the kinematic response after reconstruction with the compressible, six-degree-of-freedom prosthesis within ± 1 mm of the disc-space midline approximated the intact response in flexion-extension. Clinical studies are needed to understand and interpret the effects of limited restoration of lateral bending and axial rotation motions and motion coupling on clinical outcome.

Background: The biconvex mobile core of the CHARITÉ lumbar disc prosthesis forms two joints (spherical bearings) with the metal end plates. We quantified the intra-prosthesis motion to test the hypothesis that the total prosthesis motion would not be equally distributed between the two bearings of implanted CHARITÉ discs, which might explain the unequal wear distribution reported in explanted cores., Methods: The hypothesis was tested by studying the flexion-extension motion responses of (1) twenty-six monosegmental CHARITÉ III discs implanted in nineteen human cadaveric lumbar spines, and (2) twenty-one CHARITÉ III discs (fifteen monosegmental, six bisegmental) implanted in eighteen patients in other published clinical studies. Intra-prosthesis motions were quantified with use of a radiographic image analysis technique., Results: Eighty-eight percent of the CHARITÉ discs implanted in cadaveric specimens exhibited larger motion at the superior bearing, with 54% demonstrating more than twice as much motion at the superior bearing as at the inferior bearing. The ratio of motion at the superior bearing to motion at the inferior bearing averaged 2.68 ± 1.84, which was significantly larger than 1.0 (p < 0.001). Ninety percent of prostheses implanted in patients showed larger motion at the superior bearing. The motion ratio averaged 2.39 ± 2.47 for monosegmental cases and 2.55 ± 2.66 for all cases; both ratios were significantly larger than 1.0 (p < 0.05)., Conclusions: We found preferentially larger motion at the superior bearing of the CHARITÉ discs implanted in human cadaveric lumbar spines and in patients, regardless of the implanted level.

Background Context: The anatomy of the atlantoaxial joint makes stabilization at this level challenging. Current techniques that use transarticular screw fixation (Magerl) or segmental screw fixation (Harms) give dramatically improved stability but risk damage to the vertebral artery. A novel integrated device was designed and developed to obtain intra-articular stabilization via primary interference fixation within the C1-C2 lateral mass articulation., Purpose: To assess the atlantoaxial stability achieved with a novel integrated device when compared with the intact, destabilized, and stabilized state using the Harms technique., Study Design: A biomechanical study of implants in human cadaveric cervical spines., Methods: Six human cadaveric specimens were used. Biomechanical testing was performed with moment control in flexion-extension, lateral bending, and axial rotation. Range of motion (ROM) was measured in the intact state, after both destabilization by creation of a Type II odontoid peg fracture and sequential stabilization using the integrated device and the Harms technique., Results: Mean flexion-extension ROM of the intact specimens at C1-C2 was 14.1°±2.9°. Destabilization increased the ROM to 31.6°±4.6°. Instrumentation with the Harms technique reduced flexion-extension motion to 4.0°±1.4° (p<.01). The integrated device reduced flexion-extension motion to 3.6°±1.8° (p<.01). In lateral bending, the respective mean angular motions were 1.8°±1.1°, 14.1°±5.8°, 1.4°±0.7°, and 0.4°±0.3° for the intact destabilized Harms technique and integrated device. For axial rotation, the respective mean values were 67.3°±13.8°, 74.2°±16.1°, 1.4°±0.7° and 0.9°±0.7°. Both the Harms technique and integrated device significantly reduced motion compared with the destabilized spine in flexion-extension, lateral bending, and axial rotation (p<.05). Direct comparison of the Harms technique and the integrated device revealed no significant difference (p>.10)., Conclusions: The integrated device resulted in interference fixation at the C1-C2 lateral mass joints with comparable stability to the Harms technique. Perceived advantages with the integrated device include avoidance of fixation below the C2 lateral mass where the vertebral artery is susceptible to injury, and access to the C1 screw entry point through the blade of the integrated device avoiding extended dissection superior to the C2 nerve root and its surrounding venous plexus., (Copyright © 2012 Elsevier Inc. Background and Purpose: EPFs sustained during VCFs degrade the disk's ability to develop IDP under load. This inability to develop pressure in combination with residual kyphotic deformity increases the risk for adjacent vertebral fractures. We tested the hypothesis that StaXx FX reduces kyphosis and endplate deformity following vertebral compression fracture, restoring disk mechanics., Materials and Methods: Eight thoracolumbar, 5-vertebrae segments were tested. A void was selectively created in the middle vertebra. The specimens were compressed until EPF and to a grade I-II VCF. PEEK wafer kyphoplasty was then performed. The specimens were then tested in flexion-extension (±6 Nm) under 400-N preload intact, after EPF, VCF, and kyphoplasty. Endplate deformity, kyphosis, and IDP adjacent to the fractured body were measured., Results: Vertebral body height at the point of maximal endplate deformity decreased after EPF and VCF and was partially corrected after StaXx FX, remaining less than intact (P = .047). Anterior vertebral height decreased after VCF (P = .002) and was partially restored with StaXx FX, remaining less than intact (P = .015). Vertebral kyphosis increased after VCF (P < .001) and reduced after StaXx FX, remaining greater than intact (P = .03). EPF reduced IDP in the affected disk in compression-flexion loading (P < .001), which was restored after StaXx FX (P = 1.0). IDP in the unaffected disk did not change during testing (P > .3)., Conclusions: StaXx FX reduced endplate deformity and kyphosis, and significantly increased anterior height following VCF. Although height and kyphosis were not fully corrected, the disk's ability to pressurize under load was restored.

Object: There is limited data on the pullout strength of spinal fixation devices in the thoracic spine among individuals with different bone quality. An in vitro biomechanical study on the thoracic spine was performed to compare the pullout strength and the mechanism of failure of 4 posterior fixation thoracic constructs in relation to bone mineral density (BMD)., Methods: A total of 80 vertebrae from 11 fresh-frozen thoracic spines (T2-12) were used. Based on the results from peripheral quantitative CT, specimens were divided into 2 groups (normal and osteopenic) according to their BMD. They were then randomly assigned to 1 of 4 different instrumentation systems (sublaminar wires, pedicle screws, lamina claw hooks, or pedicle screws with wires). The construct was completed with 2 titanium rods and 2 transverse connectors, creating a stable frame. The pullout force to failure perpendicular to the rods as well as the pattern of fixation failure was recorded., Results: Mean pullout force in the osteopenic Group A (36 vertebrae) was 473.2 ± 179.2 N and in the normal BMD Group B (44 vertebrae) was 1414.5 ± 554.8 N. In Group A, no significant difference in pullout strength was encountered among the different implants (p = 0.96). In Group B, the hook system failed because of dislocation with significantly less force than the other 3 constructs (931.9 ± 345.1 N vs an average of 1538.6 ± 532.7 N; p = 0.02). In the osteopenic group, larger screws demonstrated greater resistance to pullout (p = 0.011). The most common failure mechanism in both groups was through pedicle base fracture., Conclusions: Bone quality is an important factor that influences stability of posterior thoracic implants. Fixation strength in the osteopenic group was one-fourth of the value measured in vertebrae with good bone quality, irrespective of the instrumentation used. However, in normal bone quality vertebrae, the lamina hook claw system dislocated with significantly less force when compared with other spinal implants. Further studies are needed to investigate the impact of different transpedicular screw designs on the pullout strength in normal and osteopenic thoracic spines.

Study Design: Biomechanical study using human cadaveric cervical spines., Objective: To evaluate the construct stability of 3 different segmental occipitoatlantoaxial (C0-C1-C2) stabilization techniques., Summary of Background Data: Different C0-C1-C2 stabilization techniques are used for unstable conditions in the upper cervical spine, all with different degrees of risk to the vertebral artery. Techniques with similar stability but less risk to the vertebral artery may be advantageous., Methods: Six human cadaveric cervical spines (C0-C5) (age: 74 +/- 5.0 years) were used. After testing the intact spines, instability was created by transecting the transverse and alar ligaments. The spines were instrumented from the occiput to C2 using 3 different techniques which varied in their attachment to C2. All spines had 6 screws placed into the occiput along with lateral mass screws at C1. The 3 variations used in attachment to C2 were (1) C2 crossing laminar screws, (2) C2 pedicle screws, and (3) C1-C2 transarticular screws. The C1 lateral mass screws were removed before placement of the C1-C2 transarticular screws. Range of motion across C0-C2 was measured for each construct. The data were analyzed using repeated measures ANOVA. The following post hoc comparisons were made: (1) intact spine versus each of the 3 techniques, (2) laminar screw technique versus the pedicle screw technique, and (3) laminar screw technique versus the transarticular screw technique. The level of significance was alpha = 0.01 (after Bonferroni correction for 5 comparisons)., Results: All 3 stabilization techniques significantly decreased range of motion across C0-C2 compared to the intact spine (P < 0.01). There was no statistical difference among the 3 stabilization methods in flexion/extension and axial rotation. In lateral bending, the technique using C2 crossing laminar screws demonstrated a trend toward increased range of motion compared to the other 2 techniques. CT scans in both axial and sagittal views demonstrated greater proximity to the vertebral artery in the pedicle and transarticular screw techniques compared to the crossing laminar screw technique., Conclusion: Occipitoatlantoaxial stabilization techniques using C2 crossing laminar screws, C2 pedicles screws, and C1-C2 transarticular screws offer similar biomechanical stability. Using the C2 crossing laminar screw technique may offer an advantage over the other techniques due to the reduction of the risk to the vertebral artery during C2 screw placement.

Background: Total disc replacement (TDR) and total facet replacement (TFR) have been the focus of recent kinematics evaluations. Yet their concurrent function as a total joint replacement of the lumbar spine's 3-joint complex has not been comprehensively reported. This study evaluated the effect of a TFR specifically designed to replace the natural facets and supplement the function with the natural disc and with TDR. The ability to replace degenerated facets to complement a pre-existing or simultaneously implanted TDR may allow surgeons to completely address degenerative pathologies of the 3-joint complex of the lumbar spine. We hypothesized that TFR would reproduce the biomechanical function of the natural facets when implanted in conjunction with TDR., Methods: Lumbar spines (L1-5, 51.3 ± 14.2 years, N = 6) were tested sequentially as follows: (1) intact, (2) after TDR implantation, and (3) after TFR implantation in conjunction with TDR, all at L3-4. Specimens were tested in flexion-extension (+ 8 Nm to - 6 Nm), lateral bending (± 6 Nm), and axial rotation (± 5 Nm). A 400 N compressive follower preload was applied during flexion-extension tests. Three-dimensional segmental motion was recorded and analyzed using analysis of variance in Systat (Systat Software Inc., Chicago, Illinois) and multiple comparisons with Bonferroni correction., Results: The TDR implantation (TDR + natural facets) allowed similar lateral bending (P = .66), but it generally increased flexion-extension (P = .06) and axial rotation (P < .05) range of motion (ROM) at the implanted level compared to intact. The TFR + TDR (following replacement of the natural facets with TFR) decreased ROM to levels similar to intact in lateral bending (P = .70) and axial rotation (P = .23). The TFR + TDR flexion-extension ROM was reduced in comparison to intact and TDR + natural facets (P < .05)., Conclusions: The TFR with TDR was able to restore stability to the lumbar segment after bilateral facetectomy, while allowing near-normal motions in all planes.

Background: Facet arthroplasty is a motion restoring procedure. It is normally suggested as an alternative to rigid fixation after destabilizing decompression procedures in the posterior lumbar spine. While previous studies have reported successful results in reproducing normal spine kinematics after facet replacement at L4-5 and L3-4, there are no data on the viability of facet replacement at the lumbosacral joint. The anatomy of posterior elements and the resulting kinematics at L5-S1 are distinctly different from those at superior levels, making the task of facet replacement at the lumbosacral level challenging. This study evaluated the kinematics of facet replacement at L5-S1., Methods: Six human cadaveric lumbar spines (L1-S1, 46.7 ± 13.0 years) were tested in the following sequence: (1) intact (L1-S1), (2) complete laminectomy and bilateral facetectomy at L5-S1, and (3) implantation of TFAS-LS (Lumbosacral Total Facet Arthroplasty System, Archus Orthopedics, Redmond, Washington) at L5-S1 using pedicle screws. Specimens were tested in flexion (8Nm), extension (6Nm), lateral bending (LB, ± 6Nm), and axial rotation (AR, ± 5Nm). The level of significance was α = .017 after Bonferroni correction for three comparisons: (1) intact vs. destabilized, (2) destabilized vs. reconstructed, and (3) intact vs. reconstructed., Results: Laminectomy-facetectomy at L5-S1 increased the L5-S1 angular range of motion (ROM) in all directions. Flexion-extension (F-E) ROM increased from 15.3 ± 2.9 to 18.7 ± 3.5 degrees (P < .017), LB from 8.2 ± 1.8 to 9.3 ± 1.6 degrees (P < .017), and AR from 3.7 ± 2.0 to 5.9 ± 1.8 degrees (P < .017). The facet arthroplasty system decreased ROM compared to the laminectomy-facetectomy condition in all tested directions (P < .017). The facet arthroplasty system restored the L5-S1 ROM to its intact levels in LB and AR (P > .017). F-E ROM after the facet arthroplasty system implantation was smaller than the intact value (10.1 ± 2.2 vs. 15.3 ± 2.9 degrees, P < .017). The load-displacement curves after the facet arthroplasty system implantation at L5-S1 were sigmoidal, and quality of motion measures were similar to intact, demonstrating graded resistance to angular motion in F-E, LB and AR., Conclusions: The facet arthroplasty system was able to restore stability to the lumbosacral segment after complete laminectomy and bilateral facetectomy, while also allowing near-normal kinematics in all planes. While F-E ROM after the facet arthroplasty system implantation was smaller than the intact value, it was within the physiologic norms for L5-S1. These results are consistent with previous studies of facet arthroplasty at L3-L4 and L4-L5 and demonstrate that TFAS technology can be adapted to the lumbosacral joint with functionality comparable to its application in superior lumbar levels.

Background Context: Lumbar fusion is traditionally used to restore stability after wide surgical decompression for spinal stenosis. The Total Facet Arthroplasty System (TFAS) is a motion-restoring implant suggested as an alternative to rigid fixation after complete facetectomy., Purpose: To investigate the effect of TFAS on the kinematics of the implanted and adjacent lumbar segments., Study Design: Biomechanical in vitro study., Methods: Nine human lumbar spines (L1 to sacrum) were tested in flexion-extension (+8 to -6Nm), lateral bending (+/-6Nm), and axial rotation (+/-5Nm). Flexion-extension was tested under 400 N follower preload. Specimens were tested intact, after complete L3 laminectomy with L3-L4 facetectomy, after L3-L4 pedicle screw fixation, and after L3-L4 TFAS implantation. Range of motion (ROM) was assessed in all tested directions. Neutral zone and stiffness in flexion and extension were calculated to assess quality of motion., Results: Complete laminectomy-facetectomy increased L3-L4 ROM compared with intact in flexion-extension (8.7+/-2.0 degrees to 12.2+/-3.2 degrees, p<.05) lateral bending (9.0+/-2.5 degrees to 12.6+/-3.2 degrees, p=.09), and axial rotation (3.8+/-2.7 degrees to 7.8+/-4.5 degrees p<.05). Pedicle screw fixation decreased ROM compared with intact, resulting in 1.7+/-0.5 degrees flexion-extension (p<.05), 3.3+/-1.4 degrees lateral bending (p<.05), and 1.8+/-0.6 degrees axial rotation (p=.09). TFAS restored intact ROM (p>.05) resulting in 7.9+/-2.1 degrees flexion-extension, 10.1+/-3.0 degrees lateral bending, and 4.7+/-1.6 degrees axial rotation. Fusion significantly increased the normalized ROM at all remaining lumbar segments, whereas TFAS implantation resulted in near-normal distribution of normalized ROM at the implanted and remaining lumbar segments. Flexion and extension stiffness in the high-flexibility zone decreased after facetectomy (p<.05) and increased after simulated fusion (p<.05). TFAS restored quality of motion parameters (load-displacement curves) to intact (p>.05). The quality of motion parameters for the whole lumbar spine mimicked L3-L4 segmental results., Conclusions: TFAS restored range and quality of motion at the operated segment to intact values and restored near-normal motion at the adjacent segments.

Study Design: An in vitro three-dimensional (3D) flexibility test of human C3-C7 cervical spine specimens., Objective: To test the hypothesis that anterior cervical fusion with a wedged graft and a locked plate can effectively stabilize the cervical spine after complete anterior and posterior segmental ligamentous release., Summary of Background Data: Distraction-flexion Stage 3 injuries of the lower cervical spine (bilateral facet dislocations) are usually reduced under awake cranial traction. When the magnetic resonance imaging reveals a traumatic disc prolapse, anterior cervical discectomy and fusion (ACDF) is usually recommended. Most authors advise combining ACDF with posterior instrumentation to address the insufficiency of the posterior elements. However, there is clinical evidence that ACDF with a locked plate alone suffices for the treatment of these injuries, especially in young patients. Still, there are no biomechanical studies on the effect of a locked plate on the complete anterior and posterior ligamentous-deficient young cervical spine under physiologic preload., Methods: Eight fresh frozen human lower cervical spines (C3-C7) from young donors (age, 44.5 years; range, 21-63 years) were used. A 3D flexibility test was conducted using a moment of 0.8 Nm without preload. Flexion-extension was additionally tested using a moment of 1.5 Nm under 0 and 150 N follower preload. Spines were tested first intact, then after complete C5-C6 discectomy with posterior longitudinal ligament resection and ACDF with a wedged bone graft and a rigid locked plate, and finally after complete release of the supraspinous, interspinous, and intertransverse ligaments; the facet capsules; and ligamentum flavum. RESULTS.: When tested under 0.8 Nm moment without preload, complete posterior and anterior ligamentous release did not significantly increase the ROM of the ACDF construct in flexion-extension (P > 0.025), lateral bending (P > 0.025), and axial rotation (P > 0.025). When tested under 1.5 Nm moment with or without a compressive preload, the complete posterior and anterior ligamentous release did not significantly affect the ROM of the ACDF construct (P > 0.01). The application of preload significantly reduced the motion at the C5-C6 ACDF construct with ligamentous disruption in comparison with the motion in the absence of a preload (P < 0.01)., Conclusion: Anterior cervical fusion with a wedged graft and a rigid constrained (locked) plate can effectively stabilize the nonosteoporotic cervical spine after complete posterior element injury when excessive ROM is prevented (for example, by the use of postoperative external immobilization). Even when the construct is subjected to higher moments, adequate stability can be achieved when physiologic preload is present. Osteoporosis and lack of sufficient preload due to poor neuromuscular control may affect long-term screw stability, and additional external immobilization may be needed until fusion matures.

Study Design: Literature research., Objective: To analyze the available evidence about a variety of factors that might affect outcome of lumbar artificial disc replacement., Summary of Background Data: Evaluating the scientific merit of new technology is important for a clinician considering incorporating these techniques. An evidence-based medicine approach can aid in this decision-making process., Methods: Eleven questions were asked about patient selection issues, surgical accuracy of placement, and evidence that motion preservation alters the natural history of degeneration. Studies where answers were found were ranked according to their level of evidence., Results: The majority of studies found were level IV, with only limited numbers of higher level studies. Only lower level studies with conflicting results assess the effect on outcomes of single versus multilevel surgery, L4-L5 versus L5-S1 implantations, patient's age, and history of previous surgery. One lower level study suggests that mild-to-moderate facet degeneration does not influence outcomes. The extent of preoperative facet degeneration that can be accepted remains unclear, as level IV studies report degradation of facet degeneration after implantation. Higher level studies support the importance of surgical precision on clinical outcome and lower level studies give mixed results on the same issue. A level III prognostic study suggests that higher range of motion of the implanted segment may be associated with better outcomes, whereas 2 level IV therapeutic studies provide conflicting results. The incidence of adjacent level degeneration in lower level studies ranges between 17% and 28.6%, and can require additional surgery in 2% to 3% of patients. Two level IV studies suggest that preservation of motion may have a prophylactic effect on adjacent discs., Conclusion: Existing evidence does not provide definite conclusions in the majority of the questions regarding indications and factors that may affect outcomes. Where feasible, conclusions are mainly drawn from lower level, least reliable evidence. Highest quality data are short-term whereas longer-term data are of lower quality and in many instances conflicting. More high level studies with long-term follow-up are necessary to shed light to important clinical issues.

Study Design: Biomechanical study using human cadaver spines., Objective: To assess the stabilizing effect of a supplemental anterior tension band (ATB, Synthes) plate on L5-S1 anterior lumbar interbody fusion (ALIF) using a femoral ring allograft (FRA) under physiologic compressive preloads, and to compare the results with the stability achieved using FRA with supplemental transpedicular instrumentation., Summary of Background Data: Posterior instrumentation can improve the stability of ALIF cages. Anterior plates have been proposed as an alternative to avoid the additional posterior approach., Methods: Eight human specimens (L3 to sacrum) were tested in the following sequence: (i) intact, (ii) after anterior insertion of an FRA at L5-S1, (iii) after instrumentation with the ATB plate, and (iv) after removal of the plate and adding transpedicular instrumentation at the same level. Specimens were tested in flexion-extension, lateral bending, and axial rotation. Flexion-extension was tested under 0 N, 400 N, and 800 N compressive follower preload to simulate physiologic compressive preloads on the lumbar spine., Results: Stand-alone FRAs significantly decreased the range of motion (ROM) in all tested directions (P < 0.05); however, the resultant ROM was large in flexion-extension ranging between 6.1 +/- 3.1 degrees and 5.1 +/- 2.2 degrees under 0 N to 800 N preloads. The ATB plate resulted in a significant additional decrease in flexion-extension ROM under 400 N and 800 N preloads (P < 0.05). The flexion-extension ROM with the ATB plate was 4.1 +/- 2.3 under 0 N preload and ranged from 3.1 +/- 1.8 to 2.4 +/- 1.3 under 400 N to 800 N preloads. The plate did not significantly decrease lateral bending or axial rotation ROM compared with stand-alone FRA (P > 0.05), but the resultant ROM was 2.7 +/-1.9 degrees and 0.9 +/- 0.6 degrees , respectively. Compared with the ATB plate, the transpedicular instrumentation resulted in significantly less ROM in flexion-extension and lateral bending (P < 0.05), but not in axial rotation (P > 0.05)., Conclusion: The ATB plate can significantly increase the stability of the anterior FRA at L5-S1 level. Although supplemental transpedicular instrumentation results in a more stable biomechanical environment, the resultant ROM with the addition of a plate is small, especially under physiologic preload, suggesting that the plate can sufficiently resist motion. Therefore, clinical assessment of the ATB plate as an alternative to transpedicular instrumentation to enhance ALIF cage stability is considered reasonable.
