Abstract

Background: Malaria remains an important public health problem, particularly in sub-Saharan Africa. In Rwanda, where malaria ranks among the leading causes of mortality and morbidity, disease transmission is influenced by climatic factors. However, there is a paucity of studies investigating the link between climate change and malaria dynamics, which hinders the development of effective national malaria response strategies. Addressing this critical gap, this study analyses how climatic factors influence malaria transmission across Rwanda, thereby informing tailored interventions and enhancing disease management frameworks. Methods: The study analysed the potential impact of temperature and cumulative rainfall on malaria incidence in Rwanda from 2012 to 2021 using meteorological data from the Rwanda Meteorological Agency and malaria case records from the Rwanda Health Management and Information System. The analysis was performed in two stages. First, district-specific generalized linear models with a quasi-Poisson distribution were applied, which were enhanced by distributed lag non-linear models to explore non-linear and lagged effects. Second, random effects multivariate meta-analysis was employed to pool the estimates and to refine them through best linear unbiased predictions. Results: A 1-month lag with specific temperature and rainfall thresholds influenced malaria incidence across Rwanda. Average temperature of 18.5 °C was associated with higher malaria risk, while temperature above 23.9 °C reduced the risk. Rainfall demonstrated a dual effect on malaria risk: conditions of low (below 73 mm per month) and high (above 223 mm per month) precipitation correlated with lower risk, while moderate rainfall (87 to 223 mm per month) correlated with higher risk. Seasonal patterns showed increased malaria risk during the major rainy season, while the short dry season presented lower risk. Conclusion:

Abstract

Background: In Northwestern Switzerland, recent legislation tackles the needs of community-dwelling older adults by creating Information and Advice Centers (IACs). IACs are a new service in the community that aims to assess the needs and provide information on age-related issues to community-dwelling older adults and their families. Previous studies reported difficulties in reaching community-dwelling older adults for community-based programs. We aimed to: 1) systematically identify implementation strategies to promote the IAC among community care providers, older adults and informal caregivers; 2) monitor the delivery of these strategies by the IAC management; and 3) describe the impact of those strategies on reach of community-dwelling older adults. This study was conducted as part of the TRANS-SENIOR project. Methods: As part of the INSPIRE feasibility assessment, we conducted a pre-test post-test study between March and September 2022. The sample included 8,840 older adults aged 65 + visiting/calling or being referred to the IAC for the first time. Implementation strategies were selected using implementation mapping and organized in bundles for each group of community care providers and older adults/caregivers. Our evaluation included: estimation of fidelity to the delivery of implementation strategies and bundles by the IAC management and their coverage; referral source of older adults to the IAC; and impact of the strategies on reach of the IAC on the 65 + population living in the care region. Adaptations to the strategies were documented using the FRAME-IS. Descriptive statistics were calculated and reported. Results: Seven implementation strategies were selected and organized in bundles for each community care provider and older adults and their caregivers. The lowest fidelity score was found in implementation strategies selected for nursing homes whereas the highest score corresponded to strategies targeting older adults and caregivers. "Informational visits" was the strategy with the lowest coverage (2.5% for nursing homes and 10.5% for hospitals and specialized clinics). The main referral sources were self-referrals and referrals by caregivers, followed by nursing homes. The IAC reach among the 65 + population was 5.4%. Conclusion: We demonstrated the use of implementation mapping to select implementation strategies to reach community-dwelling older adults. The reach was low suggesting that higher fidelity to the delivery of the strategies, and reflection on the causal pathway of the implementation strategies might be needed. [ABSTRACT FROM AUTHOR]

Abstract

This report presents summarised information on the status of light pollution in the 38 EEA member states, based on published literature and remote sensing data of artificial light emissions at night. It aims to assess the current knowledge on light pollution and its adverse impacts on human health, biodiversity and ecosystems as part of the Zero Pollution Monitoring Assessment. Furthermore, an analysis of the legislative basis illustrates the political progress made to mitigate light pollution, especially within the EU countries. A spatial and temporal comparison of artificial light emission levels gives insight into the current status of light pollution. This report aims to review the current knowledge on light pollution in Europe, as part of the Zero Pollution Action Plan. The key impacts of artificial-light-at-night (ALAN) on human health, biodiversity and ecosystems are summarised. Disrupted sleeping patterns, development of cancer, depressive disorders and weight gain received most scientific attention. However, evidence of an association between socio-economic status, urbanization, and light emissions suggests that the health effects of ALAN need further validation to infer causality and these results should be analysed with caution. Adverse effects on wildlife were linked to the alteration of the innate circadian clock controlling the behaviour of animals via natural light stimuli, which may result in altered behavioural patterns, reproduction, communication, or even physiological changes. Ecosystems such as natural night skies or dark environments serving as refuges for light-sensitive species are negatively impacted. An analysis of the legislative basis revealed no common EU policy currently regulating light pollution, but progressive national legislations have been adopted in France and Croatia, as well as in regions of Italy and Spain. Other EU countries, such as Austria, Liechtenstein or Ireland, have only non-binding guidelines. Satellite imagery was utilised to model historical trends in Europe and compare light pollution levels spatially and temporally. Overall, the terrestrial surface of Europe has experienced a net increase in artificial light exposure, especially in the Eastern regions. Changes in light emissions were modeled for the two thresholds 2 nW/cm2/sr, above which at least a small ecological impact is expected, and 0.5 nW/cm2/sr, the lowest values measured by VIIRS. While the area exposed to light emissions of less than 2 nW/cm2/sr has decreased by only 1 %, the regions with less than 0.5 nW/cm2/sr have shrunk by 5.2 %.
