Zur Therapie maligner Tumoren werden Zytostatika, wie das Tubulin-bindende Paclitaxel (PTX), genutzt. Deren Anwendung wird jedoch durch Eigenschaften wie einer geringen Wasserlöslichkeit und der nicht tumorspezifischen Wirkungsweise eingeschränkt. Drug-Targeting-Systeme, beispielsweise synthetische Polymere, könnten die Administration vereinfachen und die Wirkstoffkonzentration im Zielgewebe erhöhen. Ein chemisch variables Polymer ist dendritisches Polyglycerolsulfat (dPGS), welches mit Wirkstoffen (PTX) und Fluorochromen (Indocarbocyanin [ICC]) konjugiert werden kann. dPGS wurde in vitro als gut verträglich evaluiert und erfüllt Anforderungen an Drug-Targeting-Systeme. In der vorliegenden Arbeit sollte deshalb untersucht werden, ob dPGS als potentielles Drug-Targeting-System genutzt werden kann. Dazu wurde die Therapie von humanen Lungentumoren in der Maus mit dPGS ausgewertet und die Tumoren hinsichtlich der Proliferation und Apoptose immunhistochemisch untersucht. Anhand eines Screenings von 14 kovalenten dPGS-Zytostatika- Konjugaten wurde ein Konjugat (dPGS-PTX) ausgewählt und hinsichtlich der Konzentrations-Wirkungs-Beziehung in vitro untersucht (MTT-Test). Die Analyse der zellulären Aufnahme und Elimination von dPGS-PTX erfolgte durch Fluorochrom-Markierungen (dPGS-PTX-ICC). Ergänzend wurden Zellzyklusanalysen und immunhistochemische Darstellungen von Tubulin durchgeführt. Unsulfatiertes dendritisches Polyglycerol (dPG) wurde als Kontrollpolymer genutzt. Die Behandlung der Tumoren mit D = 30 mg/kg Körpergewicht dPGS verminderte den Median der relativen Tumorvolumina und wies keine unerwünschten Arzneimittelwirkungen auf. Die Proliferation in den Tumorgeweben wurde signifikant reduziert und die mediane Apoptose erhöht. Durch eine Alcianblaufärbung wurde dPGS histologisch in den tumorumgebenden Bindegeweben nachgewiesen. Das dPGS-PTX-Konjugat wies eine teilweise signifikant verminderte Zytotoxizität bei Tumorzellen und nicht-malignen Zellen auf. Während die Fluoreszenzzunahme von dPGS-PTX-ICC in den Tumorzellen linear erfolgte, führte die dPG-PTX-ICC-Inkubation erwartungsgemäß zu vergleichbar geringen Fluoreszenzen wie die Negativkontrollen. Dennoch zeigte dPG-PTX eine unerwartete zytotoxische Wirkung, die histologisch durch Tubulindarstellungen bestätigte wurde. Die Beeinflussung der Tumorvolumina, der Nachweis von dPGS im tumorumgebenden Bindegewebe und die gute Verträglichkeit des dPGS deuten eine erfolgreiche Anwendung an. Möglicherweise sind die Sulfatgruppen für die Wirkung essentiell und führen zu einer Bindung von Wachstumsfaktoren. Die trotz fehlender Aufnahme des dPG-PTX-ICC-Konjugates nachgewiesene Wirkung des dPG-PTX ließ auf eine Freisetzung des PTX von dPG und dPGS schließen. Während dPGS pharmakologische Voraussetzungen für die Weiterentwicklung als Drug- Targeting-System erfüllte, stellte dPGS-PTX keine geeignete Verbindung zum Drug-Targeting dar., Cytostatic drugs such as tubulin-binding drug paclitaxel (PTX) are used for chemotherapy. Their applicability is limited due to their low water-solubility and non-specific modes of action. It has been proposed that drug-targeting systems such as polymers may overcome these difficulties and may increase drug concentrations within the targeted tissues. A suitable synthetic polymer is dendritic polyglycerol sulfate (dPGS), as it can be functionalised with cytostatic drugs (e.g. PTX) and fluorochromes (indocarbocyanine dye [ICC]). dPGS has been proven to be nontoxic in vitro and meets general requirements of drug-targeting systems. The present work examined whether dPGS represents a suitable drug-targeting system. Using a mouse xenograft model, human lung tumours were treated with dPGS and tumour tissues were analysed immunohistochemically regarding proliferation and apoptosis. Based on a screening assay of 14 conjugates, dPGS-PTX was chosen as lead candidate. Its in vitro dose-response curve was determined in different cell lines and its cellular uptake and clearance kinetics were determined using dPGS-PTX-ICC. Additionally, cell cycle analysis and tubulin staining were performed. Non- sulfated dendritic polyglycerol (dPG) was used as control throughout the experiments. Treatment of lung tumours with dPGS (30 mg/kg body weight) reduced the median of relative tumour volumes whilst not inducing toxic side effects. Analysis of tissues revealed that proliferation was significantly reduced and median apoptosis was increased. Using Alcian blue staining, dPGS was visualized in the tissues surrounding the tumours. The dPGS-PTX-conjugate exhibited a reduced cytotoxicity compared to PTX in both tumour and non- malignant cells. Intracellular fluorescence of tumour cells treated with dPGS- PTX-ICC increased linearly over time, whilst cells incubated with dPG-PTX-ICC exhibited fluorescence values comparable to those of untreated cells. The unexpected cytotoxic effect of dPG-PTX was confirmed via histological staining. Due to its ability to influence lung tumour volumes, its accumulation in tissues surrounding tumours, and its lack of toxic side effects dPGS represents a promising drug-targeting candidate. It may be the presence of sulfate groups, which may be responsible for the observed in vivo tumour response. As possible mode of action, binding of growth factors is proposed, yet this warrants further investigation. Despite a minimal uptake of dPG-PTX-ICC in malignant cells, cytotoxicity was observed for dPG-PTX. This can be attributed to the release of PTX from dPG-PTX-conjugate. While dPGS meets requirements for further development as drug-targeting system, its covalent conjugate with PTX, dPGS-PTX does not represent an applicable conjugate.
