Gastrointestinale Blutungen sind vor allem bei alteren Menschen ein haufiges und im Einzelfall bedrohliches Ereignis. Oft liegt eine medikamenteninduzierte Ursache vor. Medikamente, die zu einer gastrointestinalen Blutung fuhren konnen, sind vor allem nichtsteroidale Antirheumatika wie Diclofenac oder Ibuprofen, Thrombozytenaggregationshemmer wie Acetylsalicylsaure, Clopidogrel oder Prasugrel und Antikoagulanzien wie die Vitamin-K-Antagonisten, Heparin oder die direkten oralen Antikoagulanzien. Eine Kombinationsbehandlung, beispielsweise eine duale Plattchenhemmung oder Thrombozytenaggregationshemmung plus Antikoagulation, geht mit einem hoheren Blutungsrisiko einher als eine Monotherapie. Es gibt Moglichkeiten der Primar- und Sekundarpravention wie die Helicobacter-pylori-Eradikation oder die Begleitmedikation mit einem Protonenpumpeninhibitor.

Gastrointestinal bleeding is a common and sometimes life-threatening event in older people. There is often a drug-induced cause. Drugs that can lead to gastrointestinal bleeding include non-steroidal anti-inflammatory drugs (NSAIDs) like diclofenac and ibuprofen, platelet inhibitors such as acetylsalicylic acid (ASS), clopidogrel and prasugrel, as well as anticoagulants like vitamin-K antagonists, heparin or direct oral anticoagulants (DOAKs). Combination antiplatelet therapy or combined medication with platelet inhibitor and anticoagulants increase the risk of gastrointestinal bleeding compared to monotherapy. Primary and secondary prevention options include Helicobacter pylori eradication and co-medication with a proton pump inhibitor (PPI).

Die vorliegende Arbeit gibt einen aktuellen Uberblick uber die Helicobacter-pylori-Infektion, ihre Diagnose und Therapie.

Lymphome des Gastrointestinaltrakts stellen eine heterogene Gruppe dar. Die einzelnen Subtypen zeichnen sich durch histologische, immunhistochemische und molekulargenetische Charakteristiken und eine unterschiedliche Prognose aus. Im Vergleich zu den gastralen Lymphomen sind intestinale Lymphome sehr viel seltener. Nach Diagnosestellung ist eine Ausbreitungsdiagnostik (Staging) erforderlich, da histologischer Typ und Lymphomstadium die Therapie determinieren. Diese umfasst Chemotherapie, Immunchemotherapie, Hochdosisprotokolle mit Stammzelltransplantation, Strahlentherapie, Operation und deren Kombination.

Der Nachweis eines diffusen, kleinzelligen, lymphoiden Infiltrats in der Lamina propria mucosae, das einen ausgepragten Epitheliotropismus zeigt und unter Ausbildung typischer lymphoepithelialer Lasionen (LEL) die Magenschleimhaut destruiert, legt die Diagnose eines extranodalen Marginalzonenlymphoms des mukosaassoziierten Gewebes (MALT-Lymphom) nahe. Durch den Nachweis einer monoklonalen B-Zell-Population mittels Immunhistochemie bzw. In-situ-Hybridisierung oder Polymerasekettenreaktion werden reaktive Lasionen im Magen ausgeschlossen. Zur Differenzialdiagnose indolenter B-Zell-Lymphome im Magen lassen sich mit dem Einsatz eines Panels von 5 immunhistochemischen Markern die chronische lymphatische B-Zell-Leukamie (Positivitat fur CD5 und CD23), das Mantelzelllymphom (Positivitat fur CD5 und CyclinD1) und das follikulare Lymphom (Positivitat fur CD10 und BCL2) zuverlassig diagnostizieren. Der molekulargenetische Nachweis der rekurrenten Translokation t(11;18)(q21;q21) bzw. des API2/MALT1-Translokationsprodukts liefert weitere Informationen zum klinischen Verlauf und zur Prognose des MALT-Lymphoms des Magens.

Background: In current guidelines H. pylori eradication is recommended as first-line therapy in patients with gastric MALT lymphoma irrespective of stage and H. pylori status. However, data on treatment and clinical course of patients with H. pylori negative MALT lymphoma are rare. Aim: To evaluate therapeutical results in patients with H. pylori negative gastric MALT lymphoma. Methods: 21 patients (13 male and 8 female; 63.9 years, range 43 – 80) with gastric MALT lymphoma were analysed retrospectively on the basis of medical reports in all cases and repeated outpatient visits at our center in 17 cases. H. pylori infection was excluded by negative histology, rapid urease test, or C13 urease breath test, and serology in all cases. Follow-up was 56.4 (5 – 142) months. Results: Ten of 21 patients were treated with H. pylori eradication, and four of them received no further therapy. The other six patients underwent surgery, chemotherapy, and radiation, after eradication therapy. Those eleven patients without H. pylori eradication received radiation (n = 3), chemotherapy (n = 1), PPIs (n = 2), no treatment (n = 4) as first-line and radiation (n = 2) as second-line therapy while initial therapy remained unknown in one case. 13 patients (61.9 %) reached complete remission of lymphoma, and seven patients (33.3 %) showed minimal histological residuals. Overall and disease-free survival was found in 95 % and 90 %, respectively. Conclusion: Patients with H. pylori negative gastric MALT lymphoma have a good prognosis. We favor initial H. pylori eradication therapy and a watch-and-wait strategy in case of minimal histological residuals of MALT lymphoma. Non-responders to eradication therapy can be successfully treated by radiation and chemotherapy.

Aliment Pharmacol Ther 2012; 35: 222–237 Summary Background Previous reviews of bowel preparation for colonoscopy have given contradictory answers. Aim To provide a definitive insight, using PRISMA-compliant methodology. Methods A comprehensive literature review identified randomised controlled trials comparing bowel preparation regimens. Data for quality of bowel preparation were pooled in multiple meta-analyses exploring a range of inclusion criteria. Results A total of 104 qualifying studies were identified, the majority of which involved comparisons of sodium phosphate (NaP) or polyethylene glycol (PEG). There was no significant difference demonstrated between NaP and PEG overall (OR = 0.82; 95% CI = 0.56–1.21; P = 0.36). Cumulative meta-analysis demonstrated that this conclusion has been qualitatively similar since the mid 1990s, with little quantitative change for the past 10 years. Amongst studies with previous day dosing in both study arms there was a significant advantage in favour of PEG (OR = 1.78; 95% CI = 1.13–2.81; P = 0.006). Studies focussing on results in the proximal colon also favoured PEG (OR = 2.36; 95% CI = 1.16–4.77; P = 0.012). PEG was also significantly more effective than non-NaP bowel preparation regimens (OR = 2.02; 95% CI = 1.08–3.78; P = 0.03). Other comparisons showed no significant difference between regimens. Conclusions Although there is no compelling evidence favouring either of the two most commonly used bowel preparation regimens, this may reflect shortcomings in study design. Where studies have ensured comparable dosage, or the clinically relevant outcome of proximal bowel clearance is considered, PEG-based regimens offer the most effective option.

Limited financial resources in public health care have led to the introduction of clinical pathways as a means to a better effectivity and efficacy. Colorectal cancer met the requirements for establishing such a pathway in a distinguished way: high patient volume, high costs, interdisciplinary multi-modal treatment concepts in a relevant frequency, and existing evidence based guidelines. This article gives an example of a clinical pathway for colorectal cancer as established in our hospital. The potential of such pathways to save costs as well as their implications on treatment results and patients' satisfaction will have to be critically analyzed in the future before their value can be definitely estimated.

Non-Hodgkin-Lymphome werden in nodale und extranodale Lymphome unterteilt. Letztere haben einen Anteil von etwa 40%. Innerhalb der Gruppe der extranodalen Lymphome stellt der Gastrointestinaltrakt die bei weitem haufigste Manifestation dar. Obwohl Unterschiede im klinischen Bild, im biologischen Verhalten und in der Prognose schon lange bekannt waren, wurden primar gastrointestinale Lymphome wie auch alle anderen extranodalen Lymphome uber Jahrzehnte hinweg entsprechend der fur nodale Lymphome entwickelten Klassifizierungssysteme (Rappaport, Kiel, Working Formulation) eingeteilt. Dies fuhrte zwangslaufig zu Schwierigkeiten bei der Beurteilung klinischer Daten und trug dazu bei, dass die Definition primarer gastrointestinaler Lymphome lange Zeit nicht einheitlich war. Dieser Beitrag beschreibt die heute gultigen Klassifikationen sowie die Diagnostik und Therapie der Lymphome des Gastrointestinaltrakts.

Summary Aim : To assess whether the eradication of Helicobacter pylori leads to long-term relief of symptoms in functional dyspepsia. Methods : Eight hundred patients with functional dyspepsia were randomized to receive double-blind treatment with twice-daily 30 mg lansoprazole, 1000 mg amoxicillin and 500 mg clarithromycin for 7 days (L30AC), twice-daily 15 mg lansoprazole, 1000 mg amoxicillin and 500 mg clarithromycin for 7 days (L15AC), or once-daily 15 mg lansoprazole for 14 days (LP). Dyspepsia and reflux symptoms were monitored for 12 months. Results : In intention-to-treat analysis, the non-ulcer dyspepsia sum score showed a statistically significant benefit in terms of symptom relief in the L30AC group (P = 0.0068) compared with the LP group, but there was no significant difference between the L15AC and LP groups (P = 0.2). BACKGROUND AND OBJECTIVE In view of numerous sequelae of a Helicobacter pylori (H.p.) infection and sparse epidemiological data on the infectious status of children and juveniles, a cross-sectional study was undertaken in schools of the Aschaffenburg area. Data were recorded on the number of H.p. infections in pupils aged 14-20 years, the possible influence of various socio-economic factors, and a possible association of the infection with upper abdominal symptoms. SUBJECTS AND METHODS In 540 pupils of grade 2 (aged 7-9 years: primary school, n = 183) grade 7 (aged 11-15: primary and secondary schools, n = 177) and grade 12 (aged 16-20 years: vocational or grammar school: n = 180), the presence of H.p. was elucidated with the 13C-urea breath test. An anonymous questionnaire was used to ascertain socio-economic status and any symptoms. RESULTS In 51 of the 540 pupils (9.4%) the test for H.p. was positive. The infectious state increased with age, but there was no significant difference between the various age groups. Ethnically German pupils were less often infected (7.1%) than those of foreign origin (28.2%). The number of persons in a household correlated with the H.p. status. Upper abdominal symptoms were more frequently reported by infected children and juveniles and were an independent risk factor for H.p. infection. CONCLUSION This study highlights the importance of socio-economic factors and the possible association of upper abdominal pain with H.p infection.

The Role of Internistic Gastroenterology in Diagnostics, Staging, and Follow-Up of Gastrointestinal Lymphomas Primary gastrointestinal lymphomas are presenting predominantly with uncharacteristic clinical symptoms, impairing the diagnosis especially if the symptoms are located in the small intestine. Thus, dyspeptic or ulcer-like symptoms, intermittent diarrhea, abdominal colics or weight loss are claiming for endoscopic or radiologic investigation in order to establish the diagnosis. Besides elective endoscopy and specification of the histological entity of lymphoma by biopsy, emergency surgery verifies the majority of intestinal lymphomas complicating with perforation or bleeding. In such cases, the histopathological diagnosis of tumor type and stage is based on resection specimens. The choice of the individual mode of therapy depends on site, histological type and stage of the gastrointestinal lymphoma. Among various staging systems the Ann Arbor system with Musshoff’s modification is most commonly applied, whereas the Lugano system and the TNM classification for lymphoma staging are seldom used. After the end of therapy, an assessment of the outcome by restaging is needed. In the case of complete remission of lymphoma, the follow-up schedule suggests endoscopy of the respective site every 6 months throughout the first 2 years, and yearly thereafter. Because of late relapses of lymphoma elsewhere in the gastrointestinal tract or in other mucosa-associated locations, a lifelong surveillance of lymphoma patients is necessary.

The detection of a diffuse infiltrate of heterogeneous small B-cells in the lamina propria mucosae invading the epithelium and destroying the glandular tissue by discrete aggregates of three or more marginal zone B-cells above the basal membrane (so-called lymphoepithelial lesions) is suspicious of a mucosa-associated lymphoid tissue (MALT) lymphoma of the stomach. The demonstration of a monoclonal B-cell population by immunohistochemistry, in situ hybridization or PCR excludes a benign lymphatic lesion. In the differential diagnosis with other small B-cell lymphomas in the stomach, a panel of five different immunohistochemical markers is useful to diagnose a small lymphocytic lymphoma (CD5 and CD23 positive), a mantle cell lymphoma (CD5 and cyclin D1 positive) or a follicular lymphoma (BCL2 and CD10 positive). The presence of the translocation t(11;18)(q21;q21) or the API2/MALT1 rearrangement could give further information about the clinical course and the prognosis of a gastric MALT lymphoma.

Im Rahmen eines Modellprojektes wurden Palliativpatienten im Klinikum Aschaffen¬burg in einer Palliativmedizinischen Tagesklinik (PTK) behandelt. In den ersten eineinhalb Behandlungsjahren wurden bei 82 Patienten insgesamt 526 teilstationare, multiprofessionelle Behandlungen durchgefuhrt. Das Lebensalter der Patienten lag im Median bei 68 Lebensjahren. 77 der 82 Pati-enten (94%) litten an weit fortge¬schrittenen metastasierten malignen Grunderkrankungen, zwei Patienten an einer dekompensierten Leberzirrhose, ein Patient an einem chronischen Immundefizienz-Syndrom und zwei Patienten hatten schwere Organinsuffizienzen von Herz und Lunge mit irreversiblen Folge¬schaden. Die Patienten waren uber einen Behandlungszeitraum von im Median 21 Tagen an die Tagesklinik angebunden (25%-Quantil 7, 75%-Quantil 56 Tage). Im Median wurden 3 Behandlungseinheiten realisiert (25%-Quantil 2, 75%-Quantil 7 Behand¬lungen). Die Behandlungsdauer lag im Median bei 199 Minuten (25%-Quantil 120, 75%-Quantil 290 Minuten). Fuhrende klinische Symptome waren Kachexie in Verbindung mit korperlicher Schwache, gefolgt von Schmerzen, Fatigue, gastrointestinalen Problemen, neuropsychiatrischen Symptomen wie Unruhe, Angst und Depression sowie Atemnot. Bei den Patienten lagen im Median 9 palliativ-medizinisch zu behandelnde Symptome vor (25%-Quantil 7, 75%-Quantil 10 Symptome). Bei 45 der 82 Patienten (55%) hatte die Erkrankung zu einer masgeblichen psychosozialen Belastung oder zu einer Uberlastung der betreuenden Angehorigen gefuhrt. Im Rahmen der tagesklinischen Palliativbehandlung lagen die teilstationaren Therapieschwerpunkte am haufigsten im Bereich der Patientenedukation, gefolgt von Schmerztherapie, speziellen arztlichen und pflegerischen Masnahmen, Physiotherapie und/oder Lymphdrainage, Infusions- und/oder Transfusionstherapie sowie operativen und invasiven Masnahmen. Bei 39 der 82 Patienten (48%) waren psychosozial stabilisierende Masnahmen wie Psychoonkologie, Kunst- und Musiktherapie sowie Seelsorge wesentliche Behandlungselemente. Daruber hinaus wurden haufig Beratungsleistungen auch im Hinblick auf Entscheidungsfindungsprozesse - die Gestaltung des Lebensendes betreffend - von Patienten und Angehorigen in Anspruch genommen. Bei 54 der 82 Patienten mit onkologischer Grunderkrankung (66%) erfolgte die teilstationare palliativmedizinische Behandlung im Sinne der Fruhintegration palliativmedizinischer Masnahmen. Diese Behandlung fand als ein besonders niedrigschwelliges palliativmedizinisches Therapieangebot eine leichtere Akzeptanz durch die Betroffenen. Es resultierte eine fruhzeitige Integration palliativen Denkens und Handelns in eine zum Beispiel noch andauernde tumorspezifische, oft vom Patienten noch als kurativ empfundene Therapie. An Hand von Kasuistiken konnte gezeigt werden, wie durch die teilstationare Nutzung palliativmedizinischer Behandlungsmoglichkeiten und der Infrastruktur eines Krankenhauses eine stationare Krankenhausbehandlung von Palliativpatienten verzogert, verkurzt oder vermieden werden konnte. Daruber hinaus wurden die ambulanten Versorgungsstrukturen gestutzt und Patienten in sozialer Isolation psychosozial stabilisiert. Dies wurde im Rahmen der Evaluation von den uberweisenden Arzten sowie von den Patienten und Angehorigen eindrucksvoll bestatigt. Insbesondere beurteilten die Haus- und Facharzte die Unterstutzung ihrer arztlichen Arbeit durch die Palliativmedizinische Tagesklinik mit der Bestnote und beschrieben sie als „sinnvollen, erganzenden Baustein zu AAPV, SAPV und stationarer palliativmedizinischer Versorgung“. Die Aussagen der Hausarzte, Patienten und Angehorigen belegen, dass die Palliativmedizinische Tagesklinik ein wichtiger, erganzender Baustein eines palliativmedizinischen Versorgungsnetzes und keine Konkurrenz zu bereits etablierten spezialisierten Behandlungsstrukturen ist. Sektorenubergreifend, in Erganzung und nicht in Konkurrenz zu AAPV und SAPV, schliest die Tagesklinik somit eine Versorgungslucke fur Palliativpatienten im ambulanten Bereich und verstarkt daruber hinaus bei Patienten und deren Angehorigen die Empfindung von Verlasslichkeit, Sicherheit und Vertrauen in „End of Life Care“.

The treatment of primary gastric lymphoma is controversial. The role of surgery has come to be questioned with increasing knowledge about the pathogenesis of gastric lymphoma and with new therapeutic approaches such as eradication of Helicobacter pylori. We review published clinical trials of primary gastric lymphoma, including preliminary results of our own prospective multicenter trial. The results of 7 trials of H. pylori eradication and 12 prospective therapeutic trials trial are discussed. On basis of these data it is concluded that surgery with intention of R0 resection is the treatment of choice in stages EI2 and EII1 of low-grade lymphoma. In high-grade lymphomas it is still unclear whether surgery or its primary combination with radio- or chemotherapy should be preferred. The eradication of H. pylori is a promising therapeutic approach for localized low-grade mucosa-associated lymphoid tissue lymphoma. A randomized trial is needed to clarify whether medical or surgical management of localized gastric lymphoma or a combination of two is the best treatment modality.
