1. Animal experimental study of a new aortic arch fenestrated stent graft

Abstract

Objective To evaluate the safety and efficacy of a novel aortic arch fenestrated membrane-covered stent graft implanted in situ in large animals after long-term maintenance. Methods Three healthy male Large White pigs (named PD01, PD05, PZ02 respectively), weighing 86-150 kg and aged 12-14 months, were selected. Under general anesthesia and intubation, thoracotomy was performed through the fourth intercostal space on the left side of the pigs to establish extracorporeal circulation. The aortic arch fenestrated covered stent was implanted by incising the aorta. Postoperative follow-up was conducted for 6 months, with safety and efficacy analyzed through routine blood tests, biochemical indices, ultrasonography, angiography, dissection, and pathology. Results In all three animals, postoperative levels of red blood cells, plasma hemoglobin, hematocrit, and platelets remained stable. The ALT of three animals elevated immediately after operation, the AST only elevated in PZ02. The albumin of three animals decreased and the levels of serum creatinine and blood urea nitrogen increased immediately after operation. All the indexes returned to normal along with time. Cardiac ultrasound results after the implantation of the aortic arch fenestrated covered stent showed no significant clinical signs of aortic valve stenosis in terms of transvalvular pressure gradient and blood flow velocity until the endpoint. Digital subtraction angiography revealed that the positioning of the fenestrated covered stents was satisfactory, with no instances of displacement or other adverse phenomena. No obvious thrombosis, perforation, or infection occurred in the experimental animals. Conclusion The novel aortic arch fenestrated covered stent system meets the basic requirements for artificial vascular substitutes. It can simplify surgical procedures, shorten operation time, and reduce the incidence of postoperative complications. Additionally, this stent system exhibits good tissue compatibility and safety.

Abstract

Compressed Sensing (CS) has been proved to be effective in Synthetic Aperture Radar (SAR) imaging. Previous CS-SAR imaging algorithms are very time consuming, especially for producing high-resolution images. In this study, we propose a new CS-SAR imaging method based on the well-known omega-K algorithm, which is precise and convenient to use in SAR imaging. First, we derive an inverse omega-K algorithm to directly obtain echoes without any convolution between the transmitted signal and scene. Then, we formulate the SAR imaging problem into a sparse regularization problem and solve it using an iterative thresholding algorithm. With our derived inverse omega-K algorithm, we can save significant amounts of computation time and computer memory usage. Simulation results show that the proposed method can effectively recover SAR images with much less data than that required by the Nyquist rate.

Abstract

Background: Patients with comorbid coronary artery disease and valvular heart disease usually undergo coronary artery bypass grafting alongside valve replacement or ring repair surgeries. Following these procedures, they typically receive a combination of anticoagulation and antiplatelet therapy, which notably heightens their bleeding risk. However, Current scoring systems provide limited predictive capability. Methods: A total of 500 adult patients treated with anticoagulation plus antiplatelet therapy after cardiac surgery were randomly divided into the training set and the validation set at a ratio of 7:3. Predictive factors were identified using univariate logistic regression, LASSO regression and multivariable analysis. Various models were developed, validated and evaluated by using methods including ROC curves, calibration curves, the Hosmer-Lemeshow test, net reclassification improvement (NRI), integrated discrimination improvement (IDI) index, decision curve analysis (DCA) and clinical impact curves (CIC). Results: Mod2 showed the best performance (AUC of validation set = 0.863) which consists of 8 independent predictive factors (gender, age > 65 years, diabetes, anemia, atrial fibrillation, cardiopulmonary bypass time, intraoperative bleeding and postoperative drainage), with a significantly higher AUC compared to Mod1 (only preoperative factors) and Mod3 (the HAS-BLED scoring model). NRI and IDI analyses further confirmed the superior predictive ability of Mod2 (NRI < 0.05, IDI < 0.05). Both DCA and CIC indicated that Mod2 exhibited good clinical applicability. Conclusion: This research established and validated a nomogram model incorporating eight predictive factors to evaluate the bleeding risk in patients who receive anticoagulation combined with antiplatelet therapy following cardiac surgery. This study investigates the role of Neuronal Pentraxin 2 (NPTX2) as a potential biomarker for POD in ATAAD patients.Methods: This secondary analysis involved ATAAD patients from a prospective observational study. Serum NPTX2 levels were measured preoperatively and immediately postoperatively using Enzyme-Linked Immunosorbent Assay (ELISA). Delirium was assessed using the Confusion Assessment Method (CAM) or CAM for the ICU (CAM-ICU). Statistical analyses included the Pearson Correlation Coefficient and multivariate logistic regression to evaluate the association between NPTX2 levels and POD.Results: Among the 62 patients included, 46.77% developed POD. Patients with POD had significantly lower preoperative and postoperative serum NPTX2 levels.
