Abstract

Background: Utility values of responders and nonresponders are essential inputs in cost-effectiveness studies of radiation therapy for painful bone metastases but, to our knowledge, they have not been reported separately. Objective: We sought to determine the utility values of responders and nonresponders using data from a prospective observational study on bone metastases. Methods: The original prospective observational study was conducted at 26 centers in Japan. Of 232 enrolled patients, 181 whose pain scores at baseline were ≥2 were analyzed. Health-related quality of life (QOL) was measured using the EuroQol 5-dimensions 5-levels (EQ-5D-5L) instrument at baseline and 2- and 6-month follow-up assessments. At follow-up assessments, patients were categorized as responders or nonresponders. Pain response was assessed using the International Consensus Pain Response Endpoints. Results: Of the 181 patients analyzed, 133 (73%) and 84 (46%) were evaluable at the 2- and 6-month follow-up assessment, respectively. The EQ-5D-5L index score (utility) increased from baseline to the 2- and 6-month follow-up assessments; regarding opioid analgesic use, no clear trend was observed during the same period. The mean utility was significantly higher in responders than in nonresponders at both follow-up times. The mean daily oral morphine equivalent dose was significantly lower in responders than in nonresponders at both follow-up times. Conclusion: We determined utility values for responders and nonresponders. Pain response was associated with better QOL and less opioid use. Abstract

Purpose of this study is to evaluate patient characteristics, treatments and outcomes in bone metastasis radiotherapy practice. Patients for whom radiotherapy for bone metastasis was planned at 26 institutions in Japan between December 2020 and March 2021 were consecutively registered in this prospective, observational study. Study measures included patient characteristics, pain relief, skeletal-related events (SREs), overall survival and incidence of radiation-related adverse events. Pain was evaluated using a numerical rating scale (NRS) from 0 to 10. Irradiated dose was analyzed by the biologically effective dose (BED) assuming α/β = 10. Overall, 232 patients were registered; 224 patients and 302 lesions were fully analyzed. Eastern Cooperative Oncology Group Performance Status was 0/1/2/3/4 in 23%/38%/22%/13%/4%; 59% of patients had spinal metastases and 84% had painful lesions (NRS ≥ 2). BED was <20 Gy (in 27%), 20–30 Gy (24%), 30–40 Gy (36%) and ≥ 40 Gy (13%); 9% of patients were treated by stereotactic body radiotherapy. Grade 3 adverse events occurred in 4% and no grade 4–5 toxicity was reported. Pain relief was achieved in 52% at 2 months. BED is not related to pain relief. The cumulative incidence of SREs was 6.5% (95% confidence interval (CI) 3.1–9.9) at 6 months; no factors were significantly associated with SREs. With spinal lesions, 18% of patients were not ambulatory at baseline and 50% of evaluable patients in this group could walk at 2 months. The 6-month overall survival rate was 70.2% (95% CI 64.2–76.9%). In conclusion, we report real-world details of radiotherapy in bone metastasis. [ABSTRACT FROM AUTHOR]

Abstract

Superconducting nanowires can be synthesised in high-throughput chemical routes and hold great promise as a low-dissipative material for superconducting devices. The applicability of superconducting nanowires, however, has been limited due to the lack of an adequate method to fit the nanowires into given electronic circuits. One of the biggest obstacles is to connect metal terminals to superconducting nanowires in order to establish electric contacts. One attractive method to surmount this difficulty is to synthesise superconducting nanowires directly upon metal terminals such that the nanowires electrically contact the metal surface. Here we demonstrate that this can be achieved by electrodeposition in molten salts. The 39-K superconductor magnesium diboride (MgB2) is electrodeposited to metal surfaces in the form of highly crystalline nanowires. The MgB2 nanowires achieve extensive electric contacts with the metal surfaces. The MgB2 nanowires carry high densities of supercurrents that are comparable to bulk materials. This approach to the electrodeposition of MgB2 nanowires provides a solution to one of the major challenges preventing practical applications of superconducting nanowires., Comment: 3 figures

24. Proton Therapy for Primary Renal Cell Carcinoma: The First Nationwide Retrospective Study in Japan

Abstract

Background/aim: This multi-institutional study aimed to investigate the efficacy and feasibility of proton beam therapy (PBT) for renal cell carcinoma (RCC) in Japan. Patients and methods: The survival, local control, and toxicities in 22 RCC patients treated between 2001 and 2016 at 6 Japanese PBT institutes were analyzed. Results: The 22 patients comprised 20 men and had a median age of 67 (range=42-88) years. The total irradiation dose was 60-79.6 Gy (relative biological effectiveness). Over a median follow-up of 37 months, the 3-year overall and disease-specific survival rates were 95% and 100%, respectively, and no recurrence occurred. No patient experienced grade 3 or higher adverse events. The serum blood urea nitrogen (p=0.25) and creatinine levels (p=0.95) were not significantly affected, although the mean estimated glomerular filtration rate was reduced by 7.1±11.2 ml/min/1.73 m2 Conclusion: Despite the small number of patients, high-dose PBT can control RCC while maintaining their renal function with high probability, and could be and alternative curative therapy especially for inoperable patients.

25. Proton beam therapy is a safe and effective treatment in elderly patients with esophageal squamous cell carcinoma

Abstract

Background: There are many elderly patients with esophageal squamous cell carcinoma (ESCC). However, there are currently few articles regarding the clinical outcome following proton beam therapy in these patients. The purpose of this study was to evaluate the clinical results of proton beam therapy in elderly patients with ESCC. Methods: Between January 2009 and December 2013, patients aged ≥75 years who underwent proton beam therapy were examined using multi-institutional data from Japan. Results: There were 38 inoperable patients (70.4%) and 16 operable patients (29.6%). More than 40% of patients had stage III/IV ESCC. The five-year overall and cancer-specific survival rates were 56.2% and 71.7%, respectively. Performance status was the only factor that significantly influenced overall survival during the multivariate analysis. The five-year local control rate was 61.8%, and local recurrence occurred within 13 months in 82.4% of patients,. There was no grade 3 or higher toxicity, excluding three patients with grade 3 esophageal ulcers. Conclusions: In conclusion, proton beam therapy may become an alternative treatment with lower toxicity in elderly patients with ESCC, compared to surgery or conventional X-ray radiotherapy. This includes inoperable patients.

Nb3(Al, Ge) conductors are prepared with a diffusion reaction process at a temperature around 1400 degree Celsius, from multifimamentary Nb/Al-25 at percent Ge precursors. The findings indicate that small matrix ratio of 0.8 and piece-length of 30 m is achieved in the precursor preparation without intermediate annealing.

Reporting a new fabrication approach to fabricate Nb/Al-Ge multifilamentary wire with fine Al-Ge microstructure cores, using a reel-to-reel intermediate rapid quenching (IRQ) technique is discussed. It is concluded that a reel-to-reel intermediate rapid quenching process dramatically converts the Al-Ge core structure into a very fine microstructure.

Investigating the cross-sections of the typical samples prepared is analyzed by optical microscopy on various rapid heating conditions is discussed.
