Abstract

The prospective 1-year EPIC study enrolled 341 patients with myelodysplastic syndromes (MDS); although baseline iron burden was >2500. ng/mL, ~50% were chelation-naive. Overall median serum ferritin decreased significantly at 1 year (p=0.002). Decreases occurred irrespective of whether patients were chelation-naive or previously chelated; changes were dependent on dose adjustments and ongoing iron intake. Sustained reductions in labile plasma iron were observed. Discontinuation rate (48.7%) and adverse event profile were consistent with previously reported deferasirox data in MDS. Alanine aminotransferase levels decreased significantly; change correlated significantly with reduction in serum ferritin (p<0.0001). This large dataset prospectively confirms the efficacy and well characterizes the safety profile of deferasirox in MDS. © 2010 Elsevier Ltd.

Abstract

Background: People with bipolar disorder (BD) experience parenting challenges associated with mood fluctuations. This talk describes the development and evaluation of a novel web- based self- management approach (Integrated Bipolar Parenting Intervention; IBPI) to support parents with BD developed with individuals with lived experience of bipolar disorder. Methods: IBPI content and structure was developed in collaboration with service users coordinated by a service user grant holder. Parents with BD with children aged 3- 10 years randomised to IBPI plus treatment as usual (TAU) or waitlist control (WL). IBPI offered 16 weeks access to interactive self- management information concerning BD and parenting issues. The trial was designed to access feasibility and acceptability as well as provide initial indications of potential benefits of IBPI. Feasibility was through recruitment, retention and web usage. Clinical outcomes were assessed at baseline, 16, 24, 36 and 48 weeks. A nested qualitative study was conducted focusing on participant experiences of the IBPI intervention. Results: The intervention and trial design were both feasible and acceptable. 97 participants were recruited with 90% retention to final follow- up. 77% of IBPI participants accessed the website. Child behaviour, parenting sense of competence and parenting stress improved significantly in IBPI compared to WL to end of intervention, sustained to 48 weeks. Qualitative feedback indicated participants’ valued this approach and made suggestions for further improvements. Conclusions: Developing an online self- management support for parents with BD is feasible, with promising improvements in parenting and child behaviour outcomes. A definitive clinical and cost- effectiveness trial is required.

17. A giant thymolipoma

Abstract

Thymolipomas are rare benign tumours, constituting one of the differential diagnoses of an anterior mediastinal mass. These tumours may run an indolent, asymptomatic course, often achieving massive dimensions before presentation. Once symptomatic, respiratory symptoms predominate. Diagnosis should be confirmed radiologically during the pre-operative work-up, with pathognomonic features demonstrated on both computed tomography (CT) scan and magnetic resonance imaging (MRI). Open surgical resection remains the treatment of choice for lesions of a significant size, and sternotomy is well tolerated in the paediatric population.

Abstract

Objectives: It is widely assumed that, at matched transfusional iron-loading rates, responses to chelation therapy are similar, irrespective of the underlying condition. However, data are limited for rare transfusiondependent anaemias, and it remains to be elucidated if response differs, depending on whether the anaemia has a primary haemolytic or production mechanism. Methods: The efficacy and safety of deferasirox (Exjade) in rare transfusion-dependent anaemias were evaluated over 1 yr, with change in serum ferritin as the primary efficacy endpoint. Initial deferasirox doses were 10–30 mg⁄ kg ⁄ d, depending on transfusion requirements; 34 patients had production anaemias, and 23 had haemolytic anaemias. Results: Patients with production anaemias or haemolytic anaemias had comparable transfusional iron-loading rates (0.31 vs. 0.30 mL red blood cells ⁄ kg ⁄ d), mean deferasirox dosing (19.3 vs. 19.0 mg⁄ kg ⁄ d) and baseline median serum ferritin (2926 vs. 2682 ng ⁄ mL). Baseline labile plasma iron (LPI) levels correlated significantly with the transfusional iron-loading rates and with serum ferritin levels in both cohorts. Reductions in median serum ferritin levels were initially faster in the production than the haemolytic anaemias, but at 1 yr, similar significant reductions of 940 and 617 ng ⁄mL were attained, respectively ()26.0% overall). Abstract

The prospective 1-year Evaluation of Patients' Iron Chelation with Exjade (EPIC) study enrolled a large cohort of 116 patients with aplastic anemia; the present analyses evaluated the efficacy and safety of deferasirox in this patient population. After 1 year, median serum ferritin decreased significantly from 3254 ng/mL at baseline to 1854 ng/mL (P < .001). Decreases occurred in chelation-naive (3229-1520 ng/mL; P < .001, last-observation-carried-forward analysis), and previously chelated (3263-2585 ng/mL; P = .21, last-observation-carried-forward analysis) patients and were reflective of dose adjustments and ongoing iron intake. Baseline labile plasma iron levels were within normal range despite high serum ferritin levels. The most common drug-related adverse events were nausea (22%) and diarrhea (16%). Serum creatinine increases more than 33% above baseline and the upper limit of normal occurred in 29 patients (25%), but there were no progressive increases; concomitant use of cyclosporine had a significant impact on serum creatinine levels. The decrease in mean alanine aminotransferase levels at 1 year correlated significantly with reduction in serum ferritin (r = 0.40, P < .001). Mean absolute neutrophil and platelet counts remained stable during treatment, and there were no drug-related cytopenias. This prospective dataset confirms the efficacy and well characterizes the tolerability profile of deferasirox in a large population of patients with aplastic anemia. Abstract

Background Following a clinical evaluation of deferasirox (Exjade) it was concluded that, in addition to baseline body iron burden, ongoing transfusional iron intake should be considered when selecting doses. The 1-year EPIC study, the largest ever investigation conducted for an iron chelator, is the first to evaluate whether fixed starting doses of deferasirox, based on transfusional iron intake, with dose titration guided by serum ferritin trends and safety markers, provides clinically acceptable chelation in patients (agedor=2 years) with transfusional hemosiderosis from various types of anemia.The recommended initial dose was 20 mg/kg/day for patients receiving 2-4 packed red blood cell units/month and 10 or 30 mg/kg/day was recommended for patients receiving less or more frequent transfusions, respectively. Dose adjustments were based on 3-month serum ferritin trends and continuous assessment of safety markers. The primary efficacy end-point was change in serum ferritin after 52 weeks compared with baseline.The 1744 patients enrolled had the following conditions; thalassemia (n=1115), myelodysplastic syndromes (n=341), aplastic anemia (n=116), sickle cell disease (n=80), rare anemias (n=43) and other transfused anemias (n=49). Overall, there was a significant reduction in serum ferritin from baseline (-264 ng/mL; P0.0001), reflecting dosage adjustments and ongoing iron intake. The most common (5%) adverse events were gastrointestinal disturbances (28%) and skin rash (10%). Conclusions Analysis of this large, prospectively collected data set confirms the response to chelation therapy across various anemias, supporting initial deferasirox doses based on transfusional iron intake, with subsequent dose titration guided by trends in serum ferritin and safety markers (clinicaltrials.gov identifier: NCT00171821).

23. Actinic exposure film dosimetry: indicators of occupational exposure risk for office workers and school teachers

Abstract

ICNIRP guidelines limit spectrally weighted (actinic) ultraviolet (UV) received by the unprotected skin or eye within an 8 hr period to 30 Jm-2. A technique for the calibration of miniaturised polyphenylene oxide (PPO) and polysulphone film dosimeters to the spectrally weighted actinic UV was employed in measuring occupational exposures received by school teachers and office workers for both short term (office workers) and long term (school teachers) applications. Short term polysulphone measurements (measured over several hours) found limited actinic exposures were received by office workers, while long term (weekly) exposure measurements using PPO film found that the recommended guidelines were routinely exceeded by school teachers as a consequence of yard and sport supervision duties. These findings raise important questions of balance for both occupational groups. Do school teachers need to modify their daily playground duties to limit the effects of potentially harmful exposure to solar UV and have office workers an accumulated solar safety budget as a result of spending long periods of time indoors and under the ICNIRP daily exposure threshold? The question of minimum recommended daily exposure and the need to optimise time in the sun due largely to the need to maintain personal vitamin D level is one that can be measured if the ICNIRP guidelines are taken as a maximum limiting value. This research presents preliminary actinic exposure measurements for both occupational groups and compares the upper actinic exposure limit with the corresponding vitamin D effective exposure.

Abstract

Regulation of virulence gene expression in enteropathogenic Escherichia coli (EPEC) and enterohemorrhagic E. coli (EHEC) is incompletely understood. In EPEC, the plasmid-encoded regulator Per is required for maximal expression of proteins encoded on the locus of enterocyte effacement (LEE), and a LEE-encoded regulator (Ler) is part of the Per-mediated regulatory cascade upregulating the LEE2, LEE3, and LEE4 promoters. We now report that Ler is essential for the expression of multiple LEE-located genes in both EPEC and EHEC, including those encoding the type III secretion pathway, the secreted Esp proteins, Tir, and intimin. Ler is therefore central to the process of attaching and effacing (AE) lesion formation. Ler also regulates the expression of LEE-located genes not required for AE-lesion formation, including rorf2, orf10, rorf10, orf19, and espF, indicating that Ler regulates additional virulence properties. In addition, Ler regulates the expression of proteins encoded outside the LEE that are not essential for AE lesion formation, including TagA in EHEC and EspC in EPEC. delta ler mutants of both EPEC and EHEC show altered adherence to epithelial cells and express novel fimbriae. Ler is therefore a global regulator of virulence gene expression in EPEC and EHEC.
