3. Monitoring trace levels of ctDNA using integration of variant reads

Abstract

In patients with early-stage cancer, ctDNA detection rates can be low due to the presence of few or no copies of any individual mutation in each sample. Sensitivity may be increased by collecting larger plasma volumes, but this is not feasible in practice. Although cancers typically have thousands of mutations in their genome, previous analyses measured only individual or up to 32 tumour-specific mutations in plasma. Here, we demonstrate that sensitivity can be greatly enhanced for any given input DNA mass by analysing a large number of mutations via sequencing. We sequenced in plasma 10^2-10^4 mutated loci per patient, using custom capture panels, whole exome (WES) or whole genome sequencing (WGS). We developed a method for INtegration of VAriant Reads (INVAR) that aggregates reads carrying tumour mutations across multiple mutant loci, and assigns confidence to error-suppressed reads based on mutation context, fragment length and tumour representation. This workflow combines a number of concepts in a novel approach in order to quantify ctDNA with maximal sensitivity. We applied INVAR to plasma sequencing data from 45 patients with stage II-IV melanoma and 26 healthy individuals. ctDNA was detected to 1 mutant molecule per million, and tumour volumes of ~1cm^3. We show that this algorithm is applicable across targeted and untargeted sequencing methods. In patients with stage II-III melanoma who relapsed after resection, ctDNA was detected within 6 months post-surgery and prior to relapse in 50% of cases, compared to 16% in a similar cohort analysed with digital PCR. In addition, INVAR may enhance detection of ctDNA in samples with limited input or sequencing coverage by aggregating signal across a large number of mutations. Using low-depth WGS (0.6x), ctDNA was detected to 1 mutant per 10,000 molecules. Given that 60 genome copies of cfDNA may be obtained from 1 drop of blood (50-75μL), we suggest that INVAR may enable cancer monitoring from limited samples volumes. As tumour sequencing becomes more widespread allowing identification of a large number of mutations per patient, this method has potential to quantify ctDNA with enhanced sensitivity, and to enable routine cancer monitoring using low-depth sequencing, potentially from low-volume blood samples that might be self-collected.

Abstract

Medical students providing support to clinical teams during Covid-19 may have been an opportunity for service and learning. We aimed to understand why the reported educational impact has been mixed to inform future placements. We conducted a cross-sectional survey of medical students at UK medical schools during the first Covid-19 'lockdown' period in the UK (March–July 2020). Analysis was informed by the conceptual framework of service and learning. 1245 medical students from 37 UK medical schools responded. 57% of respondents provided clinical support across a variety of roles and reported benefits including increased preparedness for foundation year one compared to those who did not (p < 0.0001). However, not every individual's experience was equal. For some, roles complemented the curriculum and provided opportunities for clinical skill development, reflection, and meaningful contribution to the health service. For others, the relevance of their role to their education was limited; these roles typically focused on service provision, with few opportunities to develop. The conceptual framework of service and learning can help explain why student experiences have been heterogeneous. We highlight how this conceptual framework can be used to inform clinical placements in the future, in particular the risks, benefits, and structures. Practice points There was a benefit for most students who provided clinical support compared to those who did not during Covid-19. Most students found clinical support roles more beneficial than clinical placements and most final years wanted their final year clinical placements replaced by a formal role within a clinical team. Not every student's experience of clinical support was equal. The conceptual framework of service and learning can help explain this heterogeneity. The most beneficial roles for students complemented the curriculum and provided opportunities for clinical skill development, reflective practice, and meaningful contribution to the health service. There is an added benefit of combining service and learning if done correctly, and we can use this to inform the structure of clinical placements going forwards. Abstract

Background: The coronavirus disease 2019 pandemic has led to global disruption of healthcare. Many students volunteered to provide clinical support. Volunteering to work in a clinical capacity was a unique medical education opportunity; however, it is unknown whether this was a positive learning experience or which volunteering roles were of most benefit to students. Methods: The COVIDReady2 study is a national cross-sectional study of all medical students at medical schools in the United Kingdom. The primary outcome is to explore the experiences of medical students who volunteered during the pandemic in comparison to those who did not. We will compare responses to determine the educational benefit and issues they faced. In addition to quantitative analysis, thematic analysis will be used to identify themes in qualitative responses. Discussion: There is a growing body of evidence to suggest that service roles have potential to enhance medical education; yet, there is a shortage of studies able to offer practical advice for how these roles may be incorporated in future medical education. We anticipate that this study will help to identify volunteer structures that have been beneficial for students, so that similar infrastructures can be used in the future, and help inform medical education in a non-pandemic setting. Abstract

Background: Recent advances in the study and clinical applications of circulating tumor DNA (ctDNA) are limited by practical considerations of sample collection. Whole genome sequencing (WGS) is increasingly used for analysis of ctDNA, identifying copy-number alterations and fragmentation patterns. We hypothesized that low-depth/shallow WGS (sWGS) data may be generated from minute amounts of cell-free DNA, and that fragment-size selection may remove contaminating genomic DNA from small blood volumes. Dried blood spots have practical advantages for sample collection, may facilitate serial sampling, and could support novel study designs in humans and animal models. Methods: We developed a protocol for the isolation and analysis of cell-free DNA from dried blood spots using filter paper cards and bead-based size selection. DNA extracted and size-selected from dried spots was analyzed using sWGS and PCR. Results: Analyzing a 50L dried blood spot from frozen whole blood of a patient with melanoma, we identified ctDNA based on the presence of tumor-specific somatic copy-number alterations, and found a fragment size profile similar to that observed in plasma DNA. We found alterations in different chromosomes in blood-spots from two patients with high-grade serous ovarian carcinoma. Extending this approach to serial dried blood spots from mouse xenograft models, we detect tumor-derived cell-free DNA and identified ctDNA from the originally grafted ascites. Conclusion: Our data suggest that ctDNA can be detected and monitored in dried blood spots from archived and fresh blood samples, enabling new approaches for sample collection and novel study/trial designs for both patients and in vivo models., The University of Cambridge and Cancer Research UK (grant numbers A20240 and A29580).
