Large defect of the lip and buccal mucosa created due to wide resection of the cancer is difficult to reconstruct and always a challenge. Such defects are complex as there is loss of outer skin, muscle, soft tissue, and inner mucosal lining. We report the use of submental artery island flap (SAIF) in the reconstruction of a large defect of oral commissure and buccal mucosa in a 70-year-old female with the diagnosis of verrucous carcinoma. The final outcome was optimal in terms of cosmetic and functional results. We obtained good lip seal without any problem in speech and swallowing. SAIF is an excellent alternative option for reconstruction of large complex defects of lips and oral commissure. It has a reliable vascular supply and provides a thin and pliable tissue for reconstruction in such critical area of face. [ABSTRACT FROM AUTHOR]

Parathyroid adenoma usually presents with the signs and symptoms of hypercalcemia due to hyperparathyroidism. Brown's tumor is the late clinical consequence of untreated hyperparathyroidism and is rarely seen now-a-days due to improved screening biochemical tests. These tumors are characterized by high orthoclastic activity and bone resorption. These are usually located in the pelvis, ribs, clavicles, and extremities. This manuscript highlights a rare presentation of brown's tumor of upper alveolus in a 35-year-old female with a previously undiagnosed case of hyperparathyroidism due to a parathyroid adenoma. Blood analysis depicted high levels of serum calcium and parathyroid hormone. The patient was subjected to selective left lower parathyroidectomy and will receive follow-up to prevent further developments of the disease. Clinicians should be aware of such rare presentation of parathyroid adenomas so that unnecessary extensive surgery of brown's tumor is avoided. This case also highlights the need of detailed work up to arrive at an exact diagnosis and direct surgical intervention to the adenoma rather the secondary manifestation of the disease. Knowledge of the variations of the pyramidal lobe of the thyroid assumes importance as it has direct implications on the outcome of the primary surgery and on the postoperative radioiodine ablation. If not recognized and completely removed at the time of initial thyroid surgery, it may be site of residual/recurrent disease and also may interfere with the efficient radioiodine ablation postoperatively. We report a unique case of the double pyramidal lobe of thyroid gland in an operated woman with papillary carcinoma thyroid. Our literature search revealed only one documented case of the double pyramidal lobe.
