- Abstract
Aims: The aim of this IVUS substudy was to assess the efficacy of the Y-shaped Medtronic bifurcation-dedicated stent (BDS) for the treatment of de novo coronary bifurcated lesions. Methods and Results: In the BRANCH trial, post-procedure IVUS was performed in 45 patients. IVUS was available in both branches in 19 lesions and only the main branch (MB) in 26 lesions. IVUS analysis included four distinct locations: proximal MB, bifurcation site, distal MB, and side branch (SB). Lumen symmetry was calculated as minimum/maximum lumen diameters. The quantity of isolated stent struts across the SB ostium was used to assess inadequate strut apposition to the carina resulting in partial jailing of the SB orifice. A minimum stent area (MSA) <4 mm2 was found in 0% of proximal and distal MB, and in 15.4% of SB. In SB, MSA was located mainly at mid or distal segments (84.6%), rather than at the SB ostium. Eccentric stent expansion and edge dissection were seen primarily at proximal MB. Isolated struts were seen in only 20.9% of SB ostia with a minimum length of 0.7+/-0.4 mm. Conclusion(s): Implantation of BDS resulted in adequate stent dimensions and strut apposition at the carina and SB ostium. - Abstract
We analyze the data of the TAMA300 detector to search for gravitational waves from inspiraling compact star binaries with masses of the component stars in the range 1M[sun]–3M[sun]. In this analysis, 2705 hours of data, taken during the years 2000–2004, are used for the event search. We combine the results of different observation runs, and obtain a single upper limit on the rate of the coalescence of compact binaries in our Galaxy of 20 per year at a 90% confidence level. In this upper limit, the effects of various systematic errors such as the uncertainty of the background estimation and the calibration of the detector's sensitivity are included.

- Abstract
Background: The Resolute stent is a newly developed system with a bio-histocompatible polymer that allows programmed drug delivery up to 180 days. The aim of this intravascular ultrasound (IVUS) analysis was to evaluate the short- (4 months) and mid-term (9 months) efficacy using the Resolute stent. Methods and Results: Data were derived from the RESOLUTE trial, a prospective, multicenter, non-randomized, single-arm study to treat de novo native coronary artery lesions. This trial included 2 cohorts with different follow-up periods, and all enrollment patients in this trial received IVUS study. Follow-up IVUS was available in 24 patients (4-month group) and 88 patients (9-month group). Neointimal obstruction (%) was defined as neointimal volume divided by stent volume. Cross-sectional narrowing (CSN, %) was defined as neointimal area divided by stent area. No significant differences in vessel, lumen and stent volume at post-procedure were observed within stented segments between the 4- and 9-month follow-up groups. Although neointimal volume and % neointimal obstruction showed no significant difference between the 2 groups (% neointimal obstruction: 2.2+/-2.5 vs 3.7+/-4.0%, P=0.09), maximum CSN was significantly larger in the 9-month group. There were 7 cases of late incomplete stent apposition. Conclusion(s): These IVUS results showed minimum growth of neointimal proliferation by the Resolute stent throughout the stented segment up to 9 months follow up.

24. Impact of bifurcation angioplasty on side branch ostium: Insights from BRANCH trial.

- Abstract
Background: Restenosis of bifurcation lesions remains high even in the DES era due to postprocedural stent under-expansion inadequate coverage, and/or multiple layers of overlapping stents at the side branch (SB) ostium and carina. The aim of this IVUS study was to investigate stent expansion and coverage at the SB ostium and carina in bifurcation lesions treated with the Medtronic Y-shaped bifurcation-dedicated stent (BDS; Medtronic Cardiovascular, SantaRosa, CA) and with the conventional side branch angioplasty (SBA). Method(s): Post procedure IVUS data of BDS (n=45) from the first-m-man, prospective, multicenter. non-randomized single-arm BRANCH trial was compared with data of SBA (n=19) without provisional stent placement in the SB. IVUS analysis included 4 distinct locations: proximal mam branch, bifurcation site, distal mam branch, and SB. In addition to the standard 3D IVUS parameters, lumen symmetry at the border between the bifurcation site and each branch was calculated as minimum/maximum lumen diameters. Floating struts were defined as stent struts across the SB ostium. Result(s): Lumen volume in SB was larger and a minimum lumen area <4 mm2 in SB was less frequent in BDS compared with SBA. The SB ostium was larger and more symmetric in BDS than in SBA. Floating struts at the SB ostium were detectedless frequently in BDS (20.9 vs. 78.9%, p<0.0001), and the axial segment length with floating struts was significantly shorter in BDS compared with SBA (0.7 +/- 0.4 vs. 1.5 +/- 0.8 mm, p=0.019), suggesting better struts coverage at the carina in BDS than in SBA. (Table Presented) Conclusion(s): Implantation of the bifurcation-dedicated stent appears to achieve better immediate results yielding adequate stent expansion and coverage at the side branch ostium and carina in comparison to conventional side branch angioplasty.

- Abstract
We search for coincident gravitational wave signals from inspiralling neutron star binaries using LIGO and TAMA300 data taken during early 2003. Using a simple trigger exchange method, we perform an intercollaboration coincidence search during times when TAMA300 and only one of the LIGO sites were operational. We find no evidence of any gravitational wave signals. We place an observational upper limit on the rate of binary neutron star coalescence with component masses between 1 and 3M of 49 per year per Milky Way equivalent galaxy at a 90% confidence level. The methods developed during this search will find application in future network inspiral analyses. © 2006 The American Physical Society.

- Abstract
We report on the first joint search for gravitational waves by the TAMA and LIGO collaborations. We looked for millisecond-duration unmodeled gravitational-wave bursts in 473 hr of coincident data collected during early 2003. No candidate signals were found. We set an upper limit of 0.12 events per day on the rate of detectable gravitational-wave bursts, at 90% confidence level. From software simulations, we estimate that our detector network was sensitive to bursts with root-sum-square strain amplitude above approximately 1-3×10-19Hz-1/2 in the frequency band 700-2000 Hz. We describe the details of this collaborative search, with particular emphasis on its advantages and disadvantages compared to searches by LIGO and TAMA separately using the same data. Benefits include a lower background and longer observation time, at some cost in sensitivity and bandwidth. We also demonstrate techniques for performing coincidence searches with a heterogeneous network of detectors with different noise spectra and orientations. These techniques include using coordinated software signal injections to estimate the network sensitivity, and tuning the analysis to maximize the sensitivity and the livetime, subject to constraints on the background. © 2005 The American Physical Society.

KL-6 is a glycoprotein found predominantly on type II pneumocytes and alveolar macrophages, and often shows increased serum levels in patients with interstitial pneumonia. We report a case of mycobacterium avium complex (MAC) infection whose disease activity was correlated with KL-6 levels in serum. During treatment of rheumatoid arthritis (RA)-associated interstitial lung disease (ILD) with prednisolone, chest image findings improved in association with decreased KL-6 levels. Following tapering of prednisolone, chest image findings deteriorated again as levels of KL-6 increased, suggesting recurrence of RA-ILD. Bronchoscopic examination revealed active MAC infection. Treatment of MAC infection not only improved chest image findings but also decreased KL-6 levels in serum, suggesting that KL-6 was increased by active MAC infection by itself, not by recurrence of RA-ILD. To the best of our knowledge, this is the first documentation of KL-6 elevation in serum in association with active MAC infection.

An 81-year-old Japanese male with primary Sjögren syndrome (pSS) developed a low-grade fever and productive cough which were refractory to antibiotic therapy. Based on the high level of eosinophils observed in his bronchial alveolar lavage, he was diagnosed with chronic eosinophilic pneumonia (CEP) and successfully treated by oral prednisolone. Interstitial lung diseases associated with pSS (pSS-ILDs) usually present as nonspecific interstitial pneumonia or usual interstitial pneumonia; therefore, the present case is extremely unique in that the patient's condition was complicated with CEP. A diagnosis of advanced gallbladder cancer was made in the patient's clinical course, suggesting the advisability of a whole-body workup in cases of pSS, especially in elderly patients.

- Abstract
We search for coincident gravitational wave signals from inspiralling neutron star binaries using LIGO and TAMA300 data taken during early 2003. Using a simple trigger exchange method, we perform an intercollaboration coincidence search during times when TAMA300 and only one of the LIGO sites were operational. We find no evidence of any gravitational wave signals. We place an observational upper limit on the rate of binary neutron star coalescence with component masses between 1 and 3M[sun] of 49 per year per Milky Way equivalent galaxy at a 90% confidence level. The methods developed during this search will find application in future network inspiral analyses.

Abstract Background Chronic asthma is often associated with neutrophilic infiltration in the airways. Neutrophils contain elastase, a potent secretagogue in the airways, nonetheless the role for neutrophil elastase as well as neutrophilic inflammation in allergen-induced airway responses is not well defined. In this study, we have investigated the impact of neutrophil elastase inhibition on the development of allergic airway inflammation and airway hyperresponsiveness (AHR) in previously sensitized and challenged mice. Methods BALB/c mice were sensitized and challenged (primary) with ovalbumin (OVA). Six weeks later, a single OVA aerosol (secondary challenge) was delivered and airway inflammation and airway responses were monitored 6 and 48 hrs later. An inhibitor of neutrophil elastase was administered prior to secondary challenge. Results Mice developed a two-phase airway inflammatory response after secondary allergen challenge, one neutrophilic at 6 hr and the other eosinophilic, at 48 hr. PAR-2 expression in the lung tissues was enhanced following secondary challenge, and that PAR-2 intracellular expression on peribronchial lymph node (PBLN) T cells was also increased following allergen challenge of sensitized mice. Inhibition of neutrophil elastase significantly attenuated AHR, goblet cell metaplasia, and inflammatory cell accumulation in the airways following secondary OVA challenge. Levels of IL-4, IL-5 and IL-13, and eotaxin in BAL fluid 6 hr after secondary allergen challenge were significantly suppressed by the treatment. At 48 hr, treatment with the neutrophil elastase inhibitor significantly reduced the levels of IL-13 and TGF-β1 in the BAL fluid. In parallel, in vitro IL-13 production was significantly inhibited in spleen cells from sensitized mice. Conclusion These data indicate that neutrophil elastase plays an important role in the development of allergic airway inflammation and hyperresponsiveness, and would suggest that the neutrophil elastase inhibitor reduced AHR to inhaled methacholine indicating the potential for its use as a modulator of the immune/inflammatory response in both the neutrophil- and eosinophil-dominant phases of the response to secondary allergen challenge.

Abstract Background Pulmonary emphysema is characterized by alveolar destruction and persistent inflammation of the airways. Although IL-17A contributes to many chronic inflammatory diseases, it’s role in the inflammatory response of elastase-induced emphysema remains unclear. Methods In a model of elastase-induced pulmonary emphysema we examined the response of IL-17A-deficient mice, monitoring airway inflammation, static compliance, lung histology and levels of neutrophil-related chemokine and pro-inflammatory cytokines in bronchoalveolar lavage (BAL) fluid. Results Wild-type mice developed emphysematous changes in the lung tissue on day 21 after elastase treatment, whereas emphysematous changes were decreased in IL-17A-deficient mice compared to wild-type mice. Neutrophilia in BAL fluid, seen in elastase-treated wild-type mice, was reduced in elastase-treated IL-17A-deficient mice on day 4, associated with decreased levels of KC, MIP-2 and IL-1 beta. Elastase-treated wild-type mice showed increased IL-17A levels as well as increased numbers of IL-17A+ CD4 T cells in the lung in the initial period following elastase treatment. Conclusions These data identify the important contribution of IL-17A in the development of elastase-induced pulmonary inflammation and emphysema. Targeting IL-17A in emphysema may be a potential therapeutic strategy for delaying disease progression.

Background: The prognostic impact of coronary microvascular dysfunction after percutaneous coronary intervention (PCI) remains unclear in patients with stable coronary artery disease. This study sought to investigate the prognostic value of microvascular function measured immediately after PCI in patients with stable coronary artery disease. Methods: We enrolled 572 patients with stable coronary artery disease who underwent PCI and elective measurement of the index of microcirculatory resistance (IMR) immediately after PCI from 8 centers in 4 countries. Impaired microvascular function was defined as IMR≥25 (high IMR). Major adverse cardiac events, including death, myocardial infarction (MI) and target vessel revascularization, were evaluated. Results: During a median follow-up duration of 4.0 years, the cumulative major adverse cardiac events rate was significantly higher in the high IMR group (n=66/148) compared with the low IMR group (n=128/424; hazard ratio [HR], 1.56; 95% CI, 1.16−2.105; P =0.001), primarily due to a higher rate of periprocedural MI (HR, 1.59; 95% CI, 1.11−2.28; P =0.004) but also due to higher rates of mortality (HR, 1.59; 95% CI, 0.76−3.35; P =0.22), spontaneous MI (HR, 2.10; 95% CI, 0.67−6.63; P =0.20) and target vessel revascularization (HR, 1.40; 95% CI, 0.77−2.54; P =0.27). Cumulative risk for death, spontaneous MI, and target vessel revascularization was higher in the high IMR group (HR, 1.55; 95% CI, 0.99−2.43; P =0.056), as was death and spontaneous MI alone (HR, 1.79; 95% CI, 0.96−3.36; P =0.065). On multivariable analysis, high IMR post-PCI was an independent predictor of major adverse cardiac events. Conclusions: IMR measured immediately after PCI predicts adverse events in patients with stable coronary artery disease.
